Children who begin kindergarten with stronger skills learn faster than do those who enter with lower skills. Minority children tend to enter kindergarten already at a disadvantage, and the gap widens across time. However, little is known about cognitive development among American Indian young children. In this study, 110 American Indian infants from one Northern Plains reservation community were assessed four times between ages 6 months and 36 months, with the Mullen Scales of Early Learning. At 6 months of age, scores were near the national norms; a drop occurred between 6 months and 15 months. Scores then tended to level off below the norms through 36 months. In each domain, we observed a crucial decline over the 1st year of life and relatively little change in the 2nd and 3rd years of life, highlighting the importance of developing culturally syntonic interventions to facilitate cognitive development during the 1st year of life. Objective: To present data from a general infant psychiatry clinic, including range and frequency of presenting symptoms, relationship between symptoms and diagnoses, and comparison of two diagnostic systems, DSM-IV and Diagnostic Classification of Mental Health and Developmental Disorders of Infancy and Early Childhood (DC: 0-3). Method: A retrospective chart review was conducted for 177 children, ages birth to 58 months, who were seen between 1982 and 1997. Presenting symptoms were identified. Subjects were given diagnoses using both DSM-IV and DC: 0-3. Presenting symptom variables were reduced using subjective and empirical determinants. Stepwise regression analyses were used to determine whether presenting symptoms and demographic variables predicted diagnoses. Results: Descriptive statistics on the distribution of current presenting symptoms and assigned diagnosis were compared with previous studies. The study demonstrated interrater reliability for diagnoses using both diagnostic systems, evidence of diagnostic validity via the regression analyses, and good concordance where DSM-IV and DC: 0-3 overlap. DSM-IV disruptive behavior disorders may be better conceptualized in DC: 0-3 as regulatory disorders, leading to alternative conceptualization of the disorder and a different course of treatment. Conclusions: This study clarifies a developmental progression of presenting symptoms and diagnoses and suggests the need for continued diagnostic re-evaluation in early childhood. Fourteen children (ages 3-6) from a therapeutic day treatment preschool program for maltreated children were administered the Weschler Preschool and Primary Scales of Intelligence-Revised. The abused preschoolers scored significantly lower than the population mean on four of the five subscales. Only on Picture Completion did they score significantly higher. (Author/CR)

The coronavirus disease 2019 (COVID-19) pandemic continues to demonstrate far-reaching impact on individuals, families, and communities. Emerging research highlights the detrimental impact of the pandemic on perinatal mental health. Warm Connections is a behavioral health program embedded in Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) clinics. The authors describe changes made by Warm Connections to address the increased needs of this population during the pandemic. In collaboration with WIC staf, the program developed a new outreach model to provide timely, comprehensive support for caregivers in the early postpartum period. The Tennessee First Five Training Institute (TFFTI) is a workforce development project to build infant and early childhood mental health (IECMH) capacity across Tennessee's community mental health agencies. TFFTI focused on foundational trainings in IECMH concepts, training in child--parent psychotherapy, dyadic, trauma treatment for children birth to 6 years old, and reflective consultation. This article will discuss TFFTI's programmatic shift in the wake of Tennessee's March 3, 2021, tornado, COVID-19, and the murder of George Floyd. The authors highlight TFFTI's collaborations with national partnerships to adapt training and provide support through virtual formats. Purpose Of Review: This paper reviews literature on perinatal depression prevalence, consequences, and screening among low-income women and women of color. We introduce the Warm Connections program's innovative perinatal depression screening protocol and explore perinatal depression patterns among WIC participants.Recent Findings: Perinatal depression negatively impacts maternal and child outcomes. Research shows mixed findings of perinatal depression prevalence rates among low-income women and women of color. The Warm Connections program supports the ability of WIC staff to administer the EPDS to WIC participants. Perinatal depression rates appeared lower in the Warm Connections program than in studies using less specific perinatal depression screening instruments with similar samples. Future research should continue to explore perinatal depression patterns among low-income women and women of color. Partnering with community-based settings such as WIC provides innovative opportunities to provide screening, referral, and treatment for low income women and women of color. [ABSTRACT FROM AUTHOR]

Maternal and child mental health disparities exist in the United States among people of color and people with low socioeconomic status. Adverse maternal and child mental health has short- and long-term consequences for both mothers and children. The persistent and intergenerational nature of maternal and child mental health disparities reflect an important social justice issue to which social workers must respond. The infant mental health framework can guide social workers in understanding and addressing these disparities. This framework provides a useful lens through which practitioners can understand and support the context and development of caregiver-infant dyads. This manuscript reviews maternal and child mental health disparities, provides an overview of the infant mental health framework, and describes implications for the social work practice. With roots in the social work profession and emphasis on human development within social context, the infant mental health provides insight to social work practitioners, scholars, and educators. ABSTRACT M.C. Sarche, C.D. Croy, C. Big Crow, C. Mitchell, and P. Spicer () provided first-ever information relating the socioemotional development of American Indian toddlers to the immediate context of their mothers' lives. The current study sought to replicate and build on their earlier work by examining the impact of additional maternal risk factors, identified in previous research with non-American Indian populations, on the development of American Indian toddlers: maternal depression, negative social influences, and mother's feelings of isolation. At 27 months, American Indian mothers ( N = 110) completed the Parent Demographic Questionnaire, which measured maternal psychosocial characteristics (e.g., depressed affect, social support, drug and alcohol use, isolation) and demographics. Mothers also completed the Infant-Toddler Social Emotional Assessment (A.S. Carter & M.J. Briggs-Gowan, ) and the Parent-Child Dysfunctional Interaction subscale of the Parenting Stress Index (R.R. Abidin, , ). Some results replicated the original study, but others did not. Reports of a dysfunctional mother-child relationship related to externalizing and internalizing problems, replicating the earlier study. This study also found associations between a dysfunctional mother-child relationship and socioemotional competence as well as dysregulation. The previous finding of a relationship between American Indian identity and socioemotional competence was supported. A recent partnership between the Irving Harris Program in Child Development and Infant Mental Health and the Community Based Psychiatry Program at University of Colorado Hospital joined two different approaches to child mental health treatment: infant mental health and multisystemic therapy (MST). This article illustrates the compatibility of these two approaches by detailing the application of both models to the treatment of 4-year-old Noah and his substance-abusing mother, Kim. The authors highlight helpful aspects of each approach, such as using ecologically informed interventions (MST) and reflective practice (infant mental health) to show how openness to contrasting frameworks can ultimately benefit families. The specialized needs of pregnant and parenting women in the treatment of drug addiction must not be underestimated. The impact of substance abuse on developmental outcomes for young infants and children supports the notion that attention to the parent–child relationship is a critical aspect of addiction treatment for this population. As such, the standard of care appears to be shifting from separating mothers and young children while the mother completes addiction treatment to women residing with their children while in treatment and receiving concurrent addiction treatment and parenting education. While parenting education is important, it may not provide the needed relationship intervention to address the myriad of issues often present for female recovering addicts and their children. This article describes the evolution and workings of a program for integrating infant mental health practice into a long-term residential treatment community for pregnant and parenting women with addiction. The principles and structure of the modified therapeutic community are described, as are the ways in which infant mental health practice have been effectively integrated and incorporated into the addiction treatment philosophy. This article details the development of the Infant Mental Health Clinical and Training Program at the University of Colorado School of Medicine from 1976 to the present. The article discusses the clinical sites where the program takes place, which include the Neonatal Intensive Care Unit, outpatient child psychiatry clinic, and day treatment programs. Trainees from several mental health disciplines participate in the program, including predoctoral psychology externs, interns from the Clinical Psychology Internship, residents from the Child Psychiatry Training Program, and postdoctoral research fellows in developmental psychopathology. Recently, the Infant Mental Health Program has also been able to offer its own training positions. The philosophical models of clinical service and training are outlined. Previous research has documented that children of depressed mothers are at risk for a variety of emotional/behavioral problems and impairments in mother-child interaction. Depressed mothers have been characterized as withdrawn and unavailable. the present study examined behavior of preschool children of depressed and nondepressed mothers in response to their mothers' feigned sadness. The study assessed maternal depression and maternal emotional availability to determine how these related to preschoolers' expression of empathy. Sixty-two mothers and their 3½-year-olds participated in the study. Mother-child interaction was coded from four tasks: free play, eating a snack, problem solving, and sadness simulation. Children of depressed mothers were not less empathic than children of nondepressed mothers. However, the mother's mood on day of testing related to child response. Maternal emotional availability interacted with the credibility/intensity of her simulation of sadness to predict child empathy. [ABSTRACT FROM AUTHOR]

Although there has been a growth in the emergence of evidence-based practices for preschoolers, the need for effective treatments geared toward infants, very young children, and their caregivers, as well as providers who are skilled in these interventions, far exceeds the availability. Participants will learn about a variety of infant and early childhood interventions, including a parenting intervention that was successful in a low-income urban clinic, a community-based model of consultation and education, a video feedback exposure intervention for traumatized parents and infants, and a CBT intervention for preschoolers, with and without their caregivers. The emergence and rapid spread of COVID-19 led to unprecedented changes for families and systems of care. This study sought to understand the needs of families participating in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) before and during the pandemic and considers the impact remote service delivery has on access to an integrated behavioral health intervention to support the psychosocial needs of children and caregivers. Needs for referral varied significantly pre- and post-pandemic onset. Analyses revealed that significantly more referrals were made regarding social determinants of health after the onset of COVID-19 (13.8%) compared with prior to the COVID-19 pandemic (4.1%, p <.05).
