Introduction: Fiberglass posts (FPs) are presented as a practical and economical alternative capable of rehabilitating endodontically treated teeth with 50% structural losses greater than 50%. Objective: To review the literature on the clinical protocol for the use of FPs, as well as explain the different techniques that can be used. Materials and methods: A literature review of studies published between 2000 and 2021 was carried out through a search on the Scielo (Scientific Electronic Library) and Google Scholar databases. The following descriptors were used for the research: Non-Vital Tooth, Dental Pins, and Technique for Intraradicular Retainer (Post and Core Technique). After careful screening, we selected 24 papers and 10 books, considered relevant for this review. Results: During planning, it is necessary to determine specific factors for clinical success, such as the amount of remaining tooth structure, the tooth position and the occlusal forces received, and restorative and esthetic needs required by the case and the patient. Given each case's particularities, the clinician must select among the most ideal techniques: the direct FP technique/conventional technique; the Anatomical fiberglass post technique, and the direct technique with FP associated with accessory posts. Conclusion: The FP technique has simplified clinical steps and its correct performance is successful in the treatment. Faced with situations in which the FP does not present adequate adaptation, the clinician must not use the conventional technique, using techniques for personalizing the FP. [ABSTRACT FROM AUTHOR]

Introduction: Every day, dentistry demonstrates great advances in techniques and materials applied to preserve the tooth structure. In this sense, fiberglass posts (FGPs) attract attention as an alternative to intraradicular posts for the rehabilitation of endodontically treated teeth with structural losses greater than 50%. Objective: Review the literature on aesthetic fiberglass posts, emphasizing their general aspects, properties, and biomechanical considerations. Material and Methods: A literature review of studies published in the last 21 years (2000-2021) through a search on the databases: PubMED / Medline, Scielo (Scientific Electronic Library), and Google Academic. The following descriptors were used: Tooth, Nonvital, Dental Pins, and Post and Core Technique. After careful filtering, 30 articles were selected for inclusion in the study, in addition to 10 books considered relevant to this review. Results: The FGPs demonstrate excellent aesthetic properties, easiness of execution and low cost, biocompatibility with dental and periradicular tissues, in addition to advantageous biomechanical characteristics, which result in the transmission of less stress to the tooth structure, reducing the probability of fractures. Conclusion: The numerous advantages and excellent biomechanical behavior of these pins explain their prominence concerning other intraradicular retainers, which, when properly indicated, are the first option for the rehabilitation of endodontically treated teeth with extensive coronary loss. Introdução: Pinos pré-fabricados de fibra de vidro possuem boas propriedades mecânicas e estéticas. Porém, estes podem não se adaptar bem em canais amplos ou excessivamente cônicos, prejudicando sua retenção ao canal radicular. Uma alternativa para esta desvantagem é a utilização da técnica de pino anatômico. Objetivo: O objetivo do relato de caso foi apresentar a técnica de confecção de um pino fibra de vidro anatômico em dente anterior, com reduzido remanescente coronário e ampla embocadura do canal radicular. Relato de Caso: Paciente masculino, 49 anos, procurou atendimento odontológico com queixa da aparência estética de seus dentes anteriores superiores. Após remoção de uma coroa metalocerâmica e um pino-núcleo metálico fundido do elemento 12, foram confeccionados um pino de fibra de vidro anatômico, núcleo de preenchimento e coroa provisória. Foram realizadas as facetas em resina composta nos elementos 13, 11, 21, 22 e 23. Uma coroa de cerâmica reforçada por dissilicato de lítio (IPS e.max CAD, Ivoclar Vivadent) foi cimentada no elemento 12. Conclusões: Através da técnica pino de fibra de vidro anatômico, é possível reabilitar dentes anteriores tratados endodonticamente sem a utilização de pinos metálicos, apresentando bons resultados estéticos. Entretanto, uma condição oclusal dentro dos princípios de uma oclusão mutuamente protegida deve ser almejada para garantir longevidade da restauração.

Abstract Introduction Self-adhesive resin cements do not require prior preparation of the tooth surface, therefore dentin pretreatments may influence long-term bond strength. Objective To evaluate the influence of 100% ethanol (ET) and 2% chlorhexidine (CL) treatment of intraradicular dentin on the long-term bond strength (BS) of a self-adhesive resin cement (SRC). Material and method 80 bovine roots were restored with fiber posts and SRC (U200 3M/ESPE) and distributed into 4 groups according to dentin treatment: Group 1 – without treatment; Group 2 – 2% CL for 1 minute; Group 3 – 100% ET for 1 minute; Group 4 – 2% CL, followed by 100% ET. The samples were cross-sectioned to obtain two sections (0.7 mm) thick for each root third: coronal, middle and apical. The immediate push-out test was carried out after 48 hours, and the long-term push-out test, after 180 days. Result The three-way ANOVA test for randomized blocks showed no difference between the BS values at 48 hours and 80 days, irrespective of the treatment and the third (p>0.05). The interaction of the treatment/third pairing was significant (p = 0.041) since the treatment with CL promoted lower BS in the coronal third, while treatment with ET promoted better BS in the apical third. Conclusion Treatment with CL and ET, separately or combined, promoted no differences between the BS values of the SRC to root dentin over time.

Resumo Introdução O tratamento endodôntico tem como objetivo erradicar microrganismos bacterianos e fúngicos do canal radicular, ou pelo menos possibilitar sua redução para níveis compatíveis com a saúde do tecido perirradicular, uma vez que, após o tratamento endodôntico, colônias de microrganismos residuais podem estar presentes nos túbulos dentinários e no cemento. Objetivo Avaliar o potencial antimicrobiano de diferentes tipos de retentores intrarradicularess em dentes bovinos. Material e método 50 dentes unirradiculares após tratamento endodôntico concluído foram desobturados, bem como tiveram seus condutos preparados para a cimentação dos retentores intrarradiculares. Posteriormente, os dentes foram contaminados por Enterococcus faecalis por meio da montagem de um dispositivo, o qual permitiu a infiltração bacteriana. Os dados resultantes da contagem das Unidades Formadoras de Colônia foram analisados utilizando o teste de ANOVA one-way, seguido pela análise de comparações múltiplas de Tukey. Resultado Decorrido o período de 14 dias, o grupo que apresentou a maior infiltração bacteriana foi o grupo-controle, o qual não continha nenhum tipo de retentor intrarradicular, seguido do G1-FV (grupo de continha retentor radicular de fibra de vidro) e G2-FC (grupo de que continha retentores radiculares de fibra de carbono). O menor grau de recuperação bacteriana foi encontrado nos grupos G3-Ni-Cr e G4-Cu-Al, os quais continham retentores intrarradiculares metálicos formados por ligas de níquel-cromo e cobre-alumínio, respectivamente. Conclusão Os retentores aqui avaliados apresentam diferentes efeitos antimicrobianos. Assim, a utilização de um pino com melhores resultados antimicrobianos favorece maiores chances de sucesso nos tratamentos reabilitadores.

- Abstract
Aesthetics is one of the concerns of the human being, and smile is one of its fundamental constituents. The use of composite resin restorations associated with intraradicular retainers is indicated in cases of major anterior and posterior tooth loss. The present report aims to present a rehabilitation treatment using pin prefabricated fiberglass associate the direct restoration in composite resin. A 37-year-old male patient was referred for aesthetic rehabilitation of a lower left first molar (36) after completion of endodontic treatment. For this purpose, the two conduits (mesial and distal) were planned and performed, two fiberglass pins obeying clinical protocol and coronary reconstruction of the mesial, occlusal, distal and vestibular faces in composite resin. The work emphasizes from the aesthetic importance of dentistry, passing through the support and importance of the pins of glass fiber and reconstruction of the tooth with composite resin, providing an integrated treatment and with viability for the patient who cannot opt for a prosthetic treatment. Dentes tratados endodonticamente necessitam ser restaurados, para reestabelecer estética e função. Para isso, podemos reconstruí-los de várias formas, com o uso de núcleos e pinos. Uma vez determinado a necessidade dos retentores intrarradiculares, o próximo passo é selecionar o melhor material. Com isso o objetivo desse estudo é apresentar uma revisão de literatura que consiga orientar o cirurgião dentista para a melhor escolha do retentor intrarradicular. Foram analisados artigos indexados que avaliavam a biomecânica, a seleção do pino e a adesão intrarradicular. Como resultado observou-se que não existe um protocolo a seguir que sirva para todos os casos, devido a grande diversidade clinica. Com isso concluiu-se que o cirurgião dentista deve ter conhecimento sobre as propriedades físicas e estéticas dos materiais para fazer a melhor escolha para cada caso.

Objective: To investigate the fracture strength and mode of endodontically treated teeth with structurally weakened roots reconstructed with woven polyethylene fiber posts and biological posts. Methods: After removing the crowns, 60 endodontically treated maxillary canines were distributed into 4 groups: 1) conventional root canal preparation and reconstruction with polyethylene fiber posts; 2) conventional root canal preparation and reconstruction with biological posts; 3) moderately flared root canals and reconstruction with biological posts; 4) widely flared root canals and reconstruction with biological posts. The posts were cemented with resin cement Enforce (Dentsply Ind. e Com., Petrópolis, Rio de Janeiro, Brasil) and the core was constructed with Ti-Core (EssentialDental Systems, S. Hackensack, NJ, USA) resin composite. The specimens were tested under compression in a universal testing machine. Results: The following fracture strength values were obtained: Group 1 – 45.46kgf; Group 2 – 53.30kgf; Group 3 – 58.67kgf; Group 4 – 47.91kgf, with statistically significant differences between Groups 1 and 3 (p

This article describes a case report on anterior prosthetic rehabilitation with metal-free crown and fiberglass core made using CAD-CAM system. The technique of intraradicular fiberglass retainers, made by the CAD-CAM system, allows rehabilitation in cases where there is no remaining coronal structure, ensuring high retention and excellent adaptation. The clinical case presented shows that the use of the CAD-CAM system for the fabrication of fiberglass cores allows for clinically satisfactory results, promoting aesthetics, fracture resistance, and ease of clinical application. Objective The objective of this study by the finite element method was to analyze the stress distribution on a single-rooted mandibular premolar tooth restored in two different ways. Methods Laboratory simulation considered the following situations: 1) premolar restored with gold cast post and ceramic crown; 2) premolar restored with glass fiber post, composite resin core and ceramic crown. For this we used a two-dimensional model of a premolar with load application of 300N in the axial and oblique directions within the ANSYS program for Windows. Results Under the axial load, a higher concentration of stresses was observed around the cast post (166 MPa). In the oblique load simulation, there was a concentration of stresses on the buccal root wall, internally and externally, in the tooth restored with cast post (222 MPa). In both load directions, the glass fiber post dissipated the stress received with greater uniformity (55 to 111 MPa). Conclusion These data suggested that from the biomechanical point of view, the use of a glass fiber post and composite resin core could be more advantageous than the cast post technique for a mandibular single-rooted premolar tooth. Objetivo Analisar, por meio do método dos elementos finitos, a distribuição de tensões em elemento dental pré-molar inferior unirradicular restaurado de duas maneiras distintas. Métodos A simulação laboratorial considerou as seguintes situações: 1) pré-molar restaurado com núcleo metálico fundido em liga áurea e coroa em cerâmica; 2) pré-molar restaurado com pino de fibra de vidro, núcleo de preenchimento em resina composta e coroa em cerâmica. Para isso foi utilizado um modelo bidimensional de um pré-molar com aplicação de carga de 300N nas direções axial e oblíqua com uso de programa ANSYS para Windows. Resultados Diante da carga axial, foi observada uma maior concentração de tensões na região do núcleo fundido (166 Mpa). Na simulação de carga oblíqua, observou-se uma concentração de tensões na parede radicular vestibular, interna e externamente, no dente restaurado com núcleo fundido (222 Mpa). Nas duas situações em estudo, o pino de fibra de vidro e núcleo de preenchimento dissipou, com maior uniformidade, a tensão recebida (55 a 111 Mpa). Conclusão Esses dados sugerem que o uso de pino de fibra pré-fabricado e núcleo de preenchimento em resina composta pode ser mais vantajoso que a indicação de núcleo fundido, sob o ponto-de-vista biomecânico, em dente pré-molar inferior unirradicular.

26. Efeito do Módulo Elástico da Resina Composta em Pinos de Fibra de Vidro Customizados na Tensão de Contração do Cimento em Incisivos Centrais.

Este estudo avaliou o efeito do módulo elástico da resina composta utilizada para a personalização de pinos de fibra de vidro (PFV) na tensão de contração do cimento resinoso intrarradicular em incisivos centrais superiores. Modelos tridimensionais foram confeccionados para análise por elementos finitos (FEA), sendo os grupos avaliados: pino de fibra de vidro (PFV) de menor diâmetro sem customização com resina composta (SC1); PFV de maior diâmetro sem customização (SC2); PFV customizado com resina de baixo módulo elástico - 13 GPa (BM); e PFV customizado com resina de alto módulo elástico - 21 GPa (AM). A contração de polimerização foi simulada por analogia térmica em um utilizando o critério de Tensão Máxima Principal. Os sólidos foram considerados homogêneos, lineares e isotrópicos, exceto o PFV que foi considerado ortotrópico e os contatos foram considerados colados. Os picos de tensão na camada de cimento foram mais altos nos grupos não customizados (15 MPa). A resina de menor módulo obteve picos mais baixos (3 MPa), em comparação com a de alto módulo (7 MPa). Para a dentina, os grupos não customizados tiveram maiores valores de tensão, sendo SC1 (27 MPa), seguido de SC2 (22 MPa), não havendo diferença para a resina utilizada (19 MPa). Para o PFV, os grupos SC1 e SC2 tiveram picos semelhantes (7 MPa), assim como para os grupos AM e BM (4 MPa). A customização de pinos de fibra diminui a contração da linha de cimento, independente do módulo elástico da resina composta. [ABSTRACT FROM AUTHOR]

27. Fracture resistance of metal-ceramic crown copings cemented to two types of intra-radicular posts

Abstract Introduction Endodontically treated teeth are more susceptible to root fracture than vital teeth. In order to reduce the risk of fracture, the use of intra-radicular posts and crowns is indicated. However, their own fracture resistance remains unclear. Objective To analyze the behavior of metal-ceramic crown copings cemented to two types of intra-radicular posts under tensile stress. Material and method Sixteen metal-ceramic crown copings cemented with zinc phosphate cement to cast metal posts and cores (group 1, n = 8) or with self-adhesive resin cement to glass-fiber posts rebased with composite resin (group 2, n = 8) were subjected to tensile testing after endodontic treatment and standardized preparation. Failure occurred when the crown coping and/or post-core assembly fractured and/or detached. Result In group 1, after the application of a mean tensile load of 46.83 N, 7 crown copings and metal cores separated as a whole, while in 1 specimen the coping detached from the metal core. In group 2, a mean tensile load of 127.68 N resulted in glass-fiber post fracture, and in 1 case the entire crown-post-core assembly was detached. Tensile strength differed significantly between the two groups (p = 0.0085). Conclusion Our findings suggest that metal-ceramic crown copings cemented with self-adhesive resin cement show strong adhesion to composite resin cores associated with glass-fiber posts, thus providing a safe alternative to the use of cast metal posts and cores.
