BACKGROUND The treatment of elderly patients with dizziness/vertigo/balance disorders (VDB) can be challenging for their general practitioner. Patient-centered care with a focus on self-management support could be a possible approach. Primary objective of this study was to investigate the correlations between quality of primary care and health-related quality of life (HRQOL) from the patient's perspective. METHOD Data had been collected in the area of Munich and in the area of Dresden between 2017 and 2019. Questionnaires of n = 157 elderly patients from primary care practices in Munich and Dresden were evaluated. Multiple linear regression was used to analyse quality of care, physical activity, depression, and their correlations with HRQOL. RESULTS In this cohort of patients aged 65 to 94 no significant correlation between quality of care and HRQOL could be detected. Depression correlated negatively with HRQOL, whereas physical activity showed a positive correlation with HRQOL. CONCLUSIONS Physical activity can be beneficial for elderly dizzy patients' HRQOL. Additionally, a potential depression should be taken into account when treating patients with dizziness to improve their HRQOL. The results can be well transferred to elderly patients with VDB. Analysing more patients however, could increase the significance of the results. ZUSAMMENFASSUNG Hintergrund: Die Behandlung von alteren Patienten mit Schwindelbeschwerden kann eine Herausforderung fur den Hausarzt darstellen. Ein moglicher Ansatz ist eine patientenzentrierte Versorgung mit Fokus auf der Unterstutzung des Selbstmanagements. Die Untersuchung des Zusammenhangs zwischen der Versorgungsqualitat durch den Hausarzt und der gesundheitsbezogenen Lebensqualitat (HRQOL) aus der Patientenperspektive war primares Ziel dieser Analyse. Methode: Die Datenerhebung fand zwischen 2017 und 2019 im Raum Munchen und im Raum Dresden statt. Insgesamt wurden Fragebogen von n = 157 Patienten mit Schwindel aus der hausarztlichen Versorgung analysiert. Die Versorgungsqualitat, korperliche Aktivitat, Depressivitat und deren Zusammenhang mit der HRQOL wurden mithilfe multipler linearer Regressionsmodelle im Quer- und Langsschnitt (nach 6 Monaten) betrachtet. Ergebnisse: In der beobachteten Kohorte von Patienten zwischen 65-94 Jahren ergaben sich keine signifikanten Zusammenhange zwischen der Versorgungsqualitat und der HRQOL. Es zeigte sich ein negativer Zusammenhang zwischen Depressivitat und HRQOL und ein positiver Zusammenhang zwischen korperlicher Aktivitat und HRQOL. Schlussfolgerungen: Fur die HRQOL alterer Patienten mit Schwindel kann korperliche Aktivitat von Vorteil sein. Auch sollte zur Verbesserung der HRQOL eine mogliche Depressivitat bei der Behandlung von Patienten mit Schwindel berucksichtigt werden. Die Ergebnisse lassen sich sehr gut auf altere hausarztliche Patienten mit Schwindel ubertragen. Die Untersuchung einer groseren Kohorte konnte die Aussagekraft der Ergebnisse noch erhohen. BACKGROUND Shared decision making (SDM) is a promising approach, to bridge major barriers concerning vaccination by patient education and personal interaction of health care provider (HCP) and patient. SDM affects patient adherence, enhances patient knowledge, decreases decisional conflict and improves trust in the physician in most areas of health care. The shared decision making process (SDM process) is characterised by three key components: patient activation, bi-directional exchange of information and bi-directional deliberation of options. OBJECTIVES To assess the impact of SDM processes on influenza vaccination rates in outpatient care patients. METHODS A systematic literature search in MEDLINE, CENTRAL, EMBASE, PsycINFO and ERIC was conducted (2020-02-05). Randomized controlled trials (RCTs) and cluster RCTs, that aimed to improve influenza vaccination rates in adult patients in outpatient care were included. We examined effects of SDM processes on influenza vaccination rates by meta-analysis, and considered the extent of SDM processes in the analysed interventions and possible effect modifiers in subgroup analyses. RESULTS We included 21 studies, with interventions including face-to-face sessions, telephone outreach, home visits, Health Care Practitioner (HCP) trainings and supporting educational material. In 12 studies, interventions included all elements of a SDM process. A meta-analysis of 15 studies showed a positive effect on vaccination rates (OR of 1.96 (95% CI: 1.31 to 2.95)). Findings further suggest that interventions are effective across different patients groups and could increase effectiveness when the interaction is facilitated by multidisciplinary teams of HCP in comparison to interventions delivered by individual HCP. DISCUSSION This systematic review and meta-analysis provide evidence that SDM processes can be an effective strategy to increase influenza vaccination rates. Further research with more detailed descriptions of SDM implementation modalities is necessary to better understand which components of SDM are most effective. Zusammenfassung Ziel der Studie Der Anteil an erwachsenen Bluthochdruckpatienten wurde 2010 weltweit auf 31,1% geschatzt. Ziel dieser Studie war es die Effekte von Lebensstilanderungen bei Patienten mit arterieller Hypertonie (AH) in der Primarversorgung zu untersuchen. Methodik Systematische Literaturrecherche in den Online-Datenbanken PubMed, Embase, Cochrane und Opengrey. Es wurden nur randomisierte kontrollierte Studien der Jahre 2005-2017 in deutscher oder englischer Sprache berucksichtigt. Ergebnisse Von 458 identifizierten Texten, wurden 11 Studien ausgewertet. Die untersuchten Patientengruppen waren sehr heterogen und unterzogen sich verschiedenen Interventionstypen. Besonders haufig waren Schulungen zum Krankheitsbild, regelmasige Selbstmessungen des Blutdrucks oder Patientenerinnerungen zur Einhaltung eines gesunden Lebensstils angewandt worden. Schlussfolgerung Es bedarf weiterer Studien, welche gezielt die hausarztliche Versorgung betrachten. Viele Lebensstilinterventionen scheinen aber sehr gute Effekte bei Patienten mit bereits bestehender AH zu zeigen (Sekundarprophylaxe), so dass diese Masnahmen die Basis einer antihypertensiven Therapie bei allen Patienten mit AH bilden sollten. Zudem ist es wichtig die Beratung zu einem gesunden Lebensstil wahrend der medikamentosen Therapie aufrecht zu erhalten.

Das digitale Impfmanagement ist in manchen Landern, z. B. England, bereits seit einigen Jahren erfolgreich implementiert. Deutschland hinkt da hinterher. Uber die Vorteile der elektronischen Dokumentation von Schutzimpfungen sowie die Umsetzung in der Praxis informiert Sie der nachfolgende Beitrag.

Fur ausreichenden Impfschutz von Jung und Alt zu sorgen, ist eine zentrale Aufgabe des Hausarztes. Doch in der Praxis gibt es Schwachstellen, z. B. wenn Jugendliche gesund sind und nicht in die Praxis kommen. Dann werden Impftermine leicht mal vergessen.

In ihren Empfehlungen geht die Standige Impfkommission (STIKO) im Jahr 2018 auf viele praxisrelevante Ablaufe ein, so auch auf das richtige Impfmanagement. Dieses orientiert sich an den etablierten Standards des Qualitatsmanagements nach DIN EN ISO 9000 ff. und kann durch die Implementierung einer Impfsoftware sinnvoll unterstutzt werden. Im vorliegenden Beitrag sollen die Wichtigkeit und Durchfuhrbarkeit des standardisierten Impfmanagements in der taglichen Praxis niedergelassener Arztinnen und Arzte veranschaulicht werden. Dazu erfolgte eine selektive Literaturrecherche, in die verschiedene offentliche Empfehlungen, klinische Studien, Beobachtungsstudien und systematische Reviews einbezogen wurden. Es werden im Wesentlichen vier Bereiche unterschieden, in denen das Impfmanagement ansetzt: Patient, Impfstoff, Personal und Lagerung. Mehrere Arbeiten zeigen, dass in Deutschland eine Steigerung der Impfquoten moglich ist, wenn das Praxispersonal bzw. die Patientinnen und Patienten durch eine Software an fehlende Impfungen erinnert werden. Zudem kann eine entsprechende Software auch bei der Dokumentation des Lagerbestands, regelmasigen Nachbestellungen oder Abrechnungsmodalitaten eine grose Unterstutzung darstellen. Obwohl viele niedergelassene Arztinnen und Arzte einen qualitativ hochwertigen Impfprozess anstreben, wird die Bedeutung des Impfmanagements in Deutschland eher unterschatzt und eine Impfsoftware relativ selten eingesetzt. Um den taglichen Arbeitsablauf in der Arztpraxis zu verbessern und langfristig eine Anhebung der Impfquoten in Deutschland zu erreichen, sollte das Impfmanagement gefordert werden. Besonders durch den Einsatz einer passenden Impfsoftware konnten Verbesserungen erreicht werden.

Background The number of adults with congenital heart disease (ACHD) is steadily rising due to increased survival rate and improved medical resources. Accordingly, more than 330,000 ACHD are currently living in Germany. Almost all of them require lifelong specialized medical follow-up for their chronic heart disease, often accompanied by residua, sequelae, or comorbidities. Primary care physicians (PCPs) are a crucial factor in directing patients to ACHD specialists or specialized institutions, but despite all efforts, the number of ACHD under specialized care is low, the lost-to-follow-up rate is high, and the morbidity and mortality is substantial. The present cross-sectional study was designed to systematically characterize the health care of ACHD from a patient perspective, including (I) use of medical care by ACHD, (II) medical counselling needs, and (III) perceived satisfaction with health care. Methods The German-wide analysis was based on a 25-item questionnaire designed to address different aspects of medical status and health issues of ACHD from their own perspective, performed between May of 2017 and July of 2020. Results A total of 4,008 ACHD (52% female; mean age 41.9±17.2 years) completed the questionnaire. The majority of ACHD (3,524, 87.9%) reported, that they consulted their PCP for non-cardiac health problems, and 49.7% (n=1,991) consulted their PCP also for medical problems associated with the underlying CHD. Almost all ACHD reported a need for medical advice concerning exercise capacity and daily life activities, occupational skills, pregnancy, rehabilitation, genetic counselling, insurance, and retirement. A total of 1,840 (45.9%) patients were not aware of the existence of certified ACHD specialists or centers. Moreover, 2,552 (67.6%) of those surveyed were uninformed about patient organizations for ACHD. Conclusions The present study demonstrates that ACHD are largely uninformed about the ACHD care structures available nationwide, although the patients have a great need for specialized follow-up, advice, and care.

Zusammenfassung Hintergrund Die aktuelle SARS-CoV-2-Pandemie erfordert hohe Durchimpfungsraten von chronisch Kranken gegen Influenza, um das Gesundheitssystem nicht zusatzlich zu belasten. Trotz klarer Evidenz fur Sicherheit und Wirksamkeit der Influenza-Impfung sind die Impfquoten in den vergangenen Jahren international auf insuffizientem Niveau verblieben. Der Hausarzt hat eine zentrale Bedeutung fur die Versorgung dieser Population. Ziel dieser systematischen Ubersichtsarbeit war daher die Evaluation verschiedener Masnahmen in der Allgemeinmedizin zur Steigerung der saisonalen Influenza-Impfquoten bei chronisch Kranken unter Berucksichtigung verschiedener internationaler Gesundheitssysteme. Methoden Eine systematische Literaturrecherche wurde in MEDLINE, CENTRAL, EMBASE und ERIC sowie manuell in Studienregistern und Literaturlisten durchgefuhrt. Dabei wurden ausschlieslich randomisierte kontrollierte Studien berucksichtigt. Die Methodik wurde im Vorfeld in einem Studienprotokoll festgelegt (PROSPERO CRD42018114163). Ergebnisse Insgesamt wurden 14 Studien im Rahmen eines nationalen Gesundheitsdienstes (Grosbritannien), einer staatlichen (Australien) und sozialen (Schweiz) Krankenversicherung und eines privaten Gesundheitssystems (USA) in unsere Ubersichtsarbeit eingeschlossen. Analoge Patientenerinnerungen und automatisierte Arzterinnerungen sowie Veranderungen der beruflichen Rollen fuhrten ausschlieslich im privaten Gesundheitssystem zu einem deutlichen Anstieg der Influenzaimpfquoten. Im nationalen Gesundheitsdienst konnte keine der analysierten Interventionen einen signifikanten Anstieg der Impfquoten erreichen, wobei im nationalen Gesundheitsdienst Grosbritanniens verhaltnismasig gute Basisimpfraten gegen Influenza bereits vor Durchfuhrung der Studien interventionsunabhangig vorlagen. Fortbildungsveranstaltungen fur Praxisteams und Erinnerungs-SMS zeigten in den sozialen und staatlichen Krankenversicherungssystemen der Schweiz und in Australien gute Resultate. Schlussfolgerungen In Deutschland konnten vor allem Fortbildungsveranstaltungen fur medizinische Teams und sowie zentral organisierte Einladungs- und Monitoringsysteme zur Verbesserung der Impfquoten bei chronisch Kranken geeignet sein. Eine staatliche Kostenubernahme der Impfkosten scheint in verschiedenen Gesundheitssystemen fur eine gute Basisimpfquote gegen Influenza bei Indikationspatienten zu sorgen.

Insufficient vaccination rates against pneumococcal disease are a major problem in primary health care, especially in adult patients. Shared decision-making (SDM) may address major barriers to vaccination. The objective of this review was to assess the impact of SDM on pneumococcal vaccination rates in adult patients. We conducted a systematic literature search in MEDLINE, EMBASE, CENTRAL, PsycINFO, and ERIC. RCTs and cluster RCTs were included, if they aimed to enhance pneumococcal vaccination rates in adult patients and comprised a personal interaction between health care provider (HCP) and patient. Three further aspects of the SDM process (patient activation, bi-directional exchange of information and bi-directional deliberation) were assessed. A meta-analysis was conducted for the effects of interventions on vaccination rates. We identified eight studies meeting the inclusion criteria. The pooled effect size was OR (95% CI): 2.26 (1.60–3.18) comparing intervention and control groups. Our findings demonstrate the efficacy of interventions that enable a SDM process to enhance pneumococcal vaccination rates; although, the quality of evidence was low. In exploratory subgroup analyses, we concluded that an impersonal patient activation and an exchange of information facilitated by nurses are sufficient to increase vaccination rates against pneumococcal disease in adult patients. However, the deliberation of options between physicians and patients seemed to be more effective than deliberation of options between nurses and patients.

Background A shortage of general practitioners (GPs) is common to many European countries. To counteract this, it is essential to understand the factors that encourage or discourage medical students from choosing to become a GP Objective To evaluate medical students’ attitudes towards general practice and to identify factors that discourage them from considering a career as a GP. Methods In this multinational cross-sectional online survey, 29 284 students from nine German, four Austrian and two Slovenian universities were invited to answer a questionnaire consisting of 146 closed and 13 open-ended items. Results Of the 4486 students that responded (response rate: 15.3%), 3.6% wanted to become a GP, 48.1% were undecided and 34.6% did not want to be a GP. Significant predictors for interest in becoming a GP were higher age [odds ratio (OR) = 1.06; 95% confidence interval (CI) = 1.02–1.10], positive evaluation of the content of a GP’s work (OR = 4.44; 95% CI = 3.26–6.06), organizational aspects (OR = 1.42; 95% CI = 1.13–1.78), practical experience of general practice (OR = 1.66; 95% CI = 1.08–2.56) and the country of the survey [Slovenian versus German students (Reference): OR = 2.19; 95% CI = 1.10–4.38; Austrian versus German students (Reference): OR = 0.50; 95% CI = 0.32–0.79]. Conclusion Strategies to convince undecided students to opt for a career as a GP should include a positive representation of a GP’s work and early and repeated experience of working in a general practice during medical school.

Background: Due to the increase in survival rates for congenital heart disease (CHD) in the last decades, over 90% of patients today reach adulthood. Currently, there are more than 300,000 adults with CHD (ACHD) living in Germany. They have an increased need for specialized medical care, since almost all ACHD have chronic heart disease and suffer from specific chronic symptoms, risks, and sequelae. Primary care physicians (PCPs) play a crucial role in referring patients to ACHD specialists or specialized institutions. This cross-sectional study is intended to clarify the real-world care of ACHD from the PCP&rsquo, s perspective. Methods: This analysis, initiated by the German Heart Centre Munich, was based on a 27-item questionnaire on actual ACHD health care practice in Germany from the PCP&rsquo, s perspective. Results: In total, 767 questionnaires were considered valid for inclusion. The majority of the PCPs were general practitioners (95.9%), and 84.1% had cared for ACHD during the past year. A majority (69.2%) of the PCPs had cared for patients with simple CHD, while 50.6% and 33.4% had cared for patients with moderate and severe CHD, respectively, in all age groups. PCPs treated almost all typical residual symptoms and sequelae, and advised patients regarding difficult questions, including exercise capacity, pregnancy, genetics, and insurance matters. However, 33.8% of the PCPs did not even know about the existence of certified ACHD specialists or centers. Only 23.9% involved an ACHD-specialized physician in their treatment. In cases of severe cardiac issues, 70.8% of the PCPs referred patients to ACHD-certified centers. Although 52.5% of the PCPs were not sufficiently informed about existing structures, 64.2% rated the current care situation as either &ldquo, very good&rdquo, or &ldquo, good&rdquo, Only 26.3% (n = 190) of the responding physicians were aware of patient organizations for ACHD. Conclusions: The present study showed that the majority of PCPs are not informed about the ACHD care structures available in Germany. The need for specialized ACHD follow-up care is largely underestimated, with an urgent need for optimization to reduce morbidity and mortality. For the future, solutions must be developed to integrate PCPs more intensively into the ACHD care network.

In den meisten Untersuchungen mit Medizinstudierenden wurden hohe Zustimmungsraten zur generellen Impfnotwendigkeit festgestellt. Aufgrund indikationsspezifischer Impfempfehlungen und unterschiedlich wahrgenommener Risiken einzelner impfpraventabler Infektionskrankheiten ist anzunehmen, dass die Impfnotwendigkeit einzeln erfragt werden sollte. Die Arbeit uberpruft, ob Medizinstudierende die Notwendigkeit von Impfungen fur verschiedene Impfindikationen unterschiedlich beurteilen und ob diese Beurteilungen mit Studienstandort (Dresden vs. Munchen) oder Studienjahr assoziiert sind. Fragen zur indikationsbezogenen Impfnotwendigkeit wurden in einer multizentrischen Querschnittstudie („health survey“) zum Gesundheitsverhalten von Medizinstudierenden (n = 1081) untersucht. Subgruppenunterschiede wurden mittels bivariater und multivariater Verfahren gepruft. Die Haufigkeit der Bewertung, dass Impfungen absolut notwendig sind, variierte je nach Impfindikation: sie lag zwischen 68 % fur Hepatitis A und 91 % fur Tetanus. Dabei wies das Studienjahr gegenuber dem Studienstandort fur fast alle Impfindikationen signifikant hohere Effektwerte auf: signifikant mehr Studierende des 5. Studienjahres als des 3. oder 1. Studienjahres bewerteten Impfungen als „absolut notwendig“. Die Impfnotwendigkeit sollte impfindikationsspezifisch untersucht werden. Fur fast alle untersuchten Impfindikationen war die Zahl der Studierenden, die die einzelnen Impfungen als absolut notwendig bewerteten mit zunehmenden Studienjahren in allen Studienorten hoher. Hinsichtlich steigender Inzidenzen impfpraventabler Infektionserkrankungen besteht weiterer Aufklarungsbedarf insbesondere in den jungeren Studienjahren.

Zusammenfassung Hintergrund Eine kritische Auseinandersetzung und Sensibilisierung der klinisch tatigen Arzte und Hausarzte mit dem Thema Indikationsimpfungen bei chronisch Kranken ist dringend notwendig, betrachtet man die niedrigen oder auch nicht bekannten Impfraten der betroffenen Patienten. Kliniken spielen im Impfprozess eine oft untergeordnete Rolle, wahrend ihnen eine wichtige Rolle in der Versorgung chronisch kranker Patienten zufallt. Methoden In der vorliegenden Arbeit wurde die breite Anwendbarkeit von Impfempfehlungen in Entlassungsbriefen, als Mittel zur Steigerung der Aufmerksamkeit und Miteinbeziehung des stationaren Sektors in den Impfprozess, untersucht. Zudem erfolgte eine Untersuchung der Einstellungen zu Impfempfehlungen, Zustandigkeiten und Impfverhalten von Klinik- und Hausarzten. Ergebnisse 88,5% der Hausarzte und 81,3% der Klinikarzte halten die Implementierung von Impfempfehlungen in den Arztbrief fur sinnvoll. Nach Kenntnisnahme der im Arztbrief ausgesprochenen Impfempfehlung sind 86,3% der Hausarzte gewillt, die empfohlenen Impfungen umzusetzen. Die Hauptverantwortlichkeit im Impfprozess wird durch die Arzte beider Sektoren bei den Hausarzten gesehen, jedoch mochten viele klinische Arzte die Aufmerksamkeit auf dieses Thema in Ihren Abteilungen erhohen. Schlussfolgerung Durch eine standardisierte Aufnahme von Impfempfehlungen in den Entlassungsbrief konnen Klinikarzte sinnvoll in den Impfprozess, als Teil der praventiven Therapie bei chronischen Erkrankungen, mit einbezogen werden. Zugleich werden die Hausarzte regelmasig uber die aktuell notwendigen Impfungen eines jeden Patienten unterrichtet.

Zusammenfassung Hintergrund Die Palliativmedizin ist ein essentieller Bestandteil der medizinischen Versorgung einer Gesellschaft. Unter idealen Bedingungen erfolgt diese von der Primärversorgung (Hausärzte und Pflegedienste) mit palliativmedizinischer Basisqualifikation. Hierbei ist es oft schwierig zu identifizieren, welche Patienten von einem spezialisierten Palliativ-Care Team profitieren würden. Zur Beurteilung der Komplexitätssituation eines palliativen Patienten stellen wir das spanische Diagnostik-Tool IDC-Pal (Instrumento Diagnóstico de la Complejidad en Cuidados Paliativos) vor, welches versucht zu spezifizieren wann, warum und wohin ein/-e palliative/-r Patient/-in überwiesen werden sollte. Die Ziele dieser Studie waren daher zum einen die deutsche Übersetzung und kulturelle Anpassung des bisher in Spanien verwandten Diagnostik-Tools IDC-Pal, sowie die Überprüfung der Machbarkeit und der Augenschein Validität dieses Diagnosetools. Methoden In der ersten Phase der Studie erfolgten die Vorwärts-Rückwärts-Übersetzung mit sprachlicher und kultureller Anpassung von IDC-Pal, sowie die Validierung des Inhalts durch ein Revisionskomitee. In der zweiten Phase wurde die vorläufige Version des Diagnostiktools von 38 Hausärzten mithilfe einer 10-stufigen Likert-Skala (1= „auf keinen Fall einverstanden“ und 10= „völlig einverstanden“) qualitativ bewertet. Entsprechend wurde abschließend eine endgültige Version des Diagnostiktools IDC-Pal entwickelt. Ergebnisse Die Übersetzung und die Anpassung konnten ohne Probleme verwirklicht werden. Die Machbarkeit und die Augenschein Validität des Diagnostiktools IDC-Pal wurden als überdurchschnittlich gut eingestuft. So betrug der Mittelwert der globalen Antworten in der Likert-Skala 7,79 mit einer Standardabweichung (SD) von 0,36. Die Teilnehmer bestätigten ebenfalls die Augenschein Validität mit einem Durchschnitt von 7,82 und einer SD von 0,26 und eine Machbarkeit des Werkzeugs mit einem Durchschnitt von 7,79 und einer SD von 0,39. Schlussfolgerungen Eine konzeptionelle, kulturelle und sprachlich gleichwertige Version des Diagnostiktools IDC-Pal konnte entwickelt werden. Die befragten deutschen Hausärzte waren sich einig, dass sie mit dem IDC-Pal Tool die Komplexitätssituation von palliativen Patienten einschätzen können. Diese Ergebnisse zeigen, dass die Ärzte in Bayern und eventuell in Deutschland von der vollständigen Validierung von IDC-Pal profitieren könnten.

Aim: To develop and pilot a General Practice (GPr) OSCE assessing medical students dealing with patient encounters, which are typical for GPr and to compare different measurement instruments (global ratings, content-specific checklists).Methods: A blueprint based on Entrusted Professional Activities was used to develop prototypical OSCE stations. Four stations were tested with voluntary medical students. Students were videotaped and assessed with self-developed content-specific checklists, a global rating for communication skills, and mini-CEX. Results were compared according to students' phases of studies.Results: All three measurements were able to discriminate between clinical and pre-clinical students. Clearest results were achieved by using mini-CEX. Content-specific checklists were not able to differentiate between those groups for the more difficult stations. Inter-station reliability for the global ratings was sufficient for high-stakes exams. Students enjoyed the OSCE-setting simulating GPr consultation hours. They would prefer feedback from GPs after the OSCE and from simulated patients after each encounter.Discussion and conclusion: Although the OSCE was short, results indicate advantages for using a global rating instead of checklists. Further research should include validating these results with a larger group of students and to find the threshold during the phases of education for switching from checklists to global ratings.

Background The safety and efficacy of influenza vaccination for the chronically ill are clearly supported by the evidence, yet vaccination rates in this vulnerable popu- lation remain low. This leads to many avoidable hospitalizations and deaths in Germany every year. The goal of this systematic review is to identify measures in primary care medicine that can be used to increase influenza vaccination rates among the chronically ill. Methods This review was carried out as recommended in the PRISMA statement. A systematic literature search was performed. Only randomized, controlled trials were included in the analysis. Details can be found in the study protocol (PROSPERO, CRD42018114163). Results 15 trials were included in the analysis. Training sessions for medical practice teams focusing on a particular disease raised the vaccination rates by as much as 22%. A financial incentive had the greatest effect (relative risk [RR]: 2.79; 95% confidence interval: [1.18; 6.62]). Reminders via text message yielded a maximum 3.8% absolute increase in vaccination rates. Complex interventions were not found to be of any greater benefit than simple ones. Conclusion A variety of approaches can be effective. Focusing training sessions for medical practice teams on certain diseases may be of greater benefit than vacci- nation-centered training sessions. Reminder systems for doctors should be more reliably implemented. Simple strategies are perhaps the most suitable ones in the heterogeneous population of chronically ill persons. The limitations of this system- atic review include the heterogeneity of the studies that we examined and the small number of studies in each category.

Introduction Mobility limitations have a multitude of different negative consequences on elderly patients including decreasing opportunities for social participation, increasing the risk for morbidity and mortality. However, current healthcare has several shortcomings regarding mobility sustainment of older adults, namely a narrow focus on the underlying pathology, fragmentation of care across services and health professions and deficiencies in personalising care based on patients' needs and experiences. A tailored healthcare strategy targeted at mobility of older adults is still missing. Objective The objective is to develop multiprofessional care pathways targeted at mobility sustainment and social participation in patients with vertigo/dizziness/balance disorders (VDB) and osteoarthritis (OA). Methods Data regarding quality of life, mobility limitation, pain, stiffness and physical function is collected in a longitudinal observational study between 2017 and 2019. General practitioners (GPs) recruit their patients with VDB or OA. Patients who visited their GP in the last quarter will be identified in the practice software based on VDB and OA-related International Classification of Diseases 10th Revision. Study material will be sent from the practice to patients by mail. Six months and 12 months after baseline, all patients will receive a mail directly from the study team containing the follow-up questionnaire. GPs fill out questionnaires regarding patient diagnostics, therapy and referrals. Ethics and dissemination The study was approved by the ethical committee of the Ludwig-Maximilians-Universität München and of the Technische Universität Dresden. Results will be published in scientific, peer-reviewed journals and at national and international conferences. Results will be disseminated via newsletters, the project website and a regional conference for representatives of local and national authorities.

In 2016, the number of refugees worldwide reached 65.6 million. So far, only limited data are available on the health status of refugees and asylum seekers (RAs). Especially, notifiable infectious diseases (NIDs) carry the risk of outbreaks in communal accommodations hosting RAs. We conducted a monocentric retrolective cross-sectional study including 15,137 RAs treated in a special health care unit for RAs located in the major reception center in Munich from November 2014 to October 2016. Altogether 811 RAs with NIDs according to sections 6 and 7 of the German Infection Protection Act or with other infections relevant in the setting of a communal accommodation (RIDs) could be identified. The gender and age distribution was generally comparable to that of refugees in Germany. However, patients from East Africa and Nigeria were significantly overrepresented. NIDs/RIDs were dominated by cases of tuberculosis, hepatitis B, and vaccine-preventable and parasitic diseases. Significant risk factors included country of origin (COI) and age for hepatitis B, age for hepatitis C, gender and age for HIV, and COI, gender and age for tuberculosis and ectoparasitosis. Calculated prevalences of hepatitis B, hepatitis C, and HIV were mostly below those of the COI. Incidences of tuberculosis were mostly strongly elevated. COI, gender, and age have an impact on the occurrence of NIDs/RIDs. Early vaccinations and improved hygiene could be effective in preventing NIDs/RIDs in communal accommodations. Screening, prompt therapy, and infection protection measures are necessary to prevent the transmission of diseases.

Background The World Health Organization (WHO) set the year 2020 as a target date for the eradication of measles in Europe, yet Germany is still far away from this goal. In this article, we provide an overview of current vaccination gaps and barriers to vaccination among children and adults in Germany, as well as potential strategies for overcoming them. Methods This review is based on pertinent publications identified by a selective literature search in PubMed (Medline). Results Measles vaccinations are not carried out in the appropriate timely fashion in Germany. Moreover, current vaccination rates among both children and adults are too low to achieve the goal of measles eradication. For example, among children born in 2014, the recommended vaccination rate of more than 95% was only reached when these children were 24 months old. Primary care physicians bear the responsibility for this situation, as they have the greatest influence on the decision to vaccinate. The main causes of vaccination gaps are safety worries and complacen- cy on the patients' part, and partial skepticism regarding vaccination on the part of the caregivers. We identified promising strategies for overcoming these problems: an instructive talk to provide evidence-based information to patients in an atmos- phere of mutual trust, reminder systems, multifactorial interventions, and facilitated access to vaccination, or, as a last resort, the reintroduction of compulsory vacci- nation. Conclusion Primary care physicians play a key role in vaccination. The focus of further strategies should lie above all in improved patient education and in targeted reminders for patients who neglect to vaccinate themselves and/or their children.

Background Although 17.5% of German travelers were business travelers in the years 2011–2013, little is known about their pathologies. Recent publications indicate that infections are the primary health issue in general travelers. Our aim was to investigate whether business travelers from Germany to China also primarily suffer from infections. Methods From 2011 to 2013, 587 calls for service of German business travelers to China were collected by a medical assistance provider. 482 of these calls were evaluated regarding demographics, reported diseases and conditions and the type of service provided by the medical assistance company. Results The most common reasons for calls for service were "factors influencing health status and contact with health service" (18.8%), "injury and poisoning" (16.0%) as well as "symptoms, signs, and ill-defined conditions" (13.7%). Most patients asked for "medical advice" (37.8%), referral to "outpatient care" (25.1%) or "inpatient care" (16.6%). "Evacuation and/or repatriation" was required mainly due to "injury and poisoning" (n = 12), "diseases of the circulatory system" (n = 5) or "mental disorders" (n = 3). Conclusion German business travelers to China are seeking primarily administrative support from a medical assistance provider and are mostly affected by non-infectious diseases. Pre-travel preparation of such travelers need to place more emphasis on non-communicable health risks and prevention.

Background In September 2011, the German Standing Committee on Vaccinations (STIKO) changed their recommendation regarding the mumps–measles–rubella–varicella vaccination (MMRV). We compared the immunization rates against MMRV in Germany before and after the STIKO intervention. Methods We recorded the immunization status of children born between 09/2008 and 08/2012 in 35 selected doctor’s surgeries in Germany. Results After the STIKO intervention, the ratio of the combined MMRV vaccine as the first dose immunization was reduced to approximately 25% of the initial value. A slight increase in the number of children not sufficiently vaccinated against varicella (1.2%) was observed, but the immunization rates against measles, mumps, rubella and varicella did not significantly decrease. Conclusions The STIKO intervention led to a significant change in physicians’ vaccination procedures. The separate administration MMR + V vaccination may be a helpful option to improve the immunization rates in general.
