A retrospective analysis of 60 patients with hybrid ICD systems: Boston Scientific device paired with non-Boston leads. In 10 (17%) patients transient, out-of-range peaks of ventricular pace impedance trend were observed. Probable cause is header-lead interaction incompatibility. This matter is known mainly for pacemakers systems but not for ICDs. Investigation this issue is crucial because consequences in ICD systems are unpredictable and risk might be higher than in pacing systems. This article is protected by copyright. Objective: Patients with anorexia nervosa (AN) are at increased risk of ventricular arrhythmias, which are considered to be associated with QT interval prolongation. The aims of this study was to analyse the QT interval in patients with AN considering the potential impact of pharmacotherapy and to verify various QT correction formulas. Method: Fifty-six patients hospitalized with AN (average age: 22.8±5.6 years; F/M: 54/2, mean BMI=13.6±2.6 kg/m2) were enrolled to analysis: Group non-D (n=44; 78,6%) included patients, who did not use drugs that prolong the QT interval, group D (n=12; 21,4%) included patients, who were treated with such drugs. QT intervals were measured in a 12-lead ECG and corrected using the four formulas: Bazett, Fridericia, Framingham and Hodges. Results: Mean heart rate (HR) was similar in both groups (61±16.3 bpm in group D vs 63.1±18.7 bpm in non-D, p > 0.05). Pathological bradycardia (HR < 50 bpm) was present in 5 patients (41.7%) in group D and in 13 patients (29.5%) in group non-D. QTc interval corrected with Framingham formula was longer in Group-D (459±81ms) vs non D-group (413±33ms) p=0.04. QT interval corrected with Bazett and Hodges formulas was significantly dependent on HR (R= -0.29, p=0.03 and R= -0.42, p=0.001, respectively). Influence of HR on results of Fridericia and Framingham formulas was not significant (R = -0.22, p=0.1 and R = -0.11, p=0.4). Conclussions: Information about pharmacotherapy in AN patients is key for QTc assessment. Choice of correction formula has impact on the QTc. QTc obtained using Framingham and Fridericia formulas were the least dependent on heart rate.

Objectives According to the Organization for Economic Cooperation and Development (OECD) data, 13% of deaths recorded in the European Union in 2010 were related to coronary heart disease. The Polish Central Statistical Office data show that cardiovascular mortality in 2014 was at the level of 100.1/100 000 general population. The aim of the study was to assess the current burden of deaths due to acute myocardial infarction (AMI) with the assessment of temporal and spatial variability in the Silesian Voivodeship, Poland. Material and Methods Depersonalized data obtained from the Silesian Voivodeship Branch of the National Health Fund of Poland, based in Katowice, were used as the study material. The death rate due to acute or subsequent myocardial infarction in each of the subregions of the Silesian Voivodeship was standardized to the European Standard Population 2013. The analyses of the annual AMI death rate for 2009–2014 were performed and assigned to all the subregions of the Silesian Voivodeship, according to the patients’ domicile. Results In this study, 37.7% of the patients (N = 20 806) were females, and 30 142 healthcare services were granted to them, accounting for 36.64% of all services provided to all patients. The average patient’s age during the service provision was 66±12 years, with women being about 6.5 years older than men (70±12 years vs. 64±11 years, respectively). The standardized death rate (SDR) values in each of the 8 subregions of the Silesian Voivodeship were analyzed. In 2009–2014, a substantial decrease in the SDR was noted in 7 of them, except for the Sosnowiec subregion in which an increase in the average annual SDR value was observed. Moreover, its values were the highest in the whole Silesian Voivodeship. Conclusions The obtained results confirmed the spatial variability of mortality due to AMI in the study region. The worst situation was observed in the Sosnowiec subregion in which the number of specific deaths continuously increased, probably due to the limited availability of cardiological and invasive cardiology treatments or adverse health conditions. The aim of the study was: (1) to verify the hypothesis that left ventricular global longitudinal strain (LVGLS) may be of additive prognostic value in prediction CRT response and (2) to obtain such a LVGLS value that in the best optimal way enables to characterize potential CRT responders. Forty-nine HF patients (age 66.5 ± 10 years, LVEF 24.9 ± 6.4%, LBBB 71.4%, 57.1% ischemic aetiology of HF) underwent CRT implantation. Transthoracic echocardiography was performed prior to and 15 ± 7 months after CRT implantation. Speckle-tracking echocardiography was performed to assess longitudinal left ventricular function as LVGLS. The response to CRT was defined as a ≥ 15% reduction in the left ventricular end-systolic volume (∆LVESV). Thirty-six (73.5%) patients responded to CRT. There was no linear correlation between baseline LVGLS and ∆LVESV (r = 0.09;p = 0.56). The patients were divided according to the percentile of baseline LVGLS: above 80th percentile; between 80 and 40th percentile; below 40th percentile. Two peripheral groups (above 80th and below 40th percentile) formed “peripheral LVGLS” and the middle group was called “mid-range LVGLS”. The absolute LVGLS cutoff values were − 6.07% (40th percentile) and − 8.67% (80th percentile). For the group of 20 (40.8%) “mid-range LVGLS” patients mean ΔLVESV was 33.3 ± 16.9% while for “peripheral LVGLS” ΔLVESV was 16.2 ± 18.8% (p p = 0.015). Baseline LVGLS may have a potential prognostic value in prediction CRT response with relationship of inverted J-shaped pattern. “Mid-range LVGLS” values should help to select CRT responders, especially in non-ischemic HF etiology patients.

Scant data exist on long-term outcomes, including death, in patients with complica-tions related to transvenous lead extractions (TLE).We sought to characterize the population and examine the outcomes including risk factors for in-hospital complications and 12-month mortality and morbidity related to the complications in a large administrative database of patients undergoing TLE.From the database of patients hospitalized for cardiovascular diseases and included in the Silesian Cardiovascular Database (SILCARD) registry, we selected the admissions of those who underwent TLE according to the appropriate ICD-9 codes. The patients were divided into two groups based on whether they did or did not manifest any complications during their hospitalization for the TLE procedure. Between 2007 and 2019, we found a total of 835 patients who underwent TLE. TLE-related complications occurred in 56 patients (6.7%) of the Complications-Yes group while no complications were recorded in 779 (93.3%) patients of the Complications-No group. A significant difference in the rate of all-cause mortality (23.9% vs. 6.5%; P0.001) and major adverse cardiac events (MACE) (58.7% vs. 39.4%; P = 0.01) between the Complications-Yes and Complications-No groups were recorded. Multivariable analysis of the entire study population revealed that prior dialysis, chronic kidney disease, and ventricular tachycardia were independent factors of a higher risk of TLE-related in-hospital complications. Multivariable analysis of the patients discharged from the hospital after the TLE procedure showed that TLE-related complications, a history of heart failure, and older age independently affected 12-month mortality.The presence of TLE-related in-hospital complications increased 12-month mortality.

This paper presents studies on the impact of the erbium ions Er3+ on the properties of magneto - optical of the tellurite glasses Magneto-optical properties were investigated based on measurements of the Faraday effect and magnetic susceptibility as well as optical properties of the glasses, i.e. transmission in the visible range. The tests were carried out for clean glass matrices synthesized using various modifiers and glasses additionally doped with erbium ions Er3+. The aim of doped with ions of the paramagnetic element from the lanthanide group was to try to obtain higher Verdet constant values in the examined materials. The dominant type of magnetic ordering was determined on the basis of measurements of the permanent magnetic susceptibility using a vibration magnetometer. The obtained results indicate the paramagnetic nature of the magnetic order in the examined glasses. Glass doped with paramagnetic ions Er3+ resulted in the increase of their magnetic susceptibility, which suggests the possibility of observing better magneto-optical properties. The magneto-optical properties of tellurite glasses were determined based on measurements of the Faraday effect and calculation of the Verdet constant for different wavelengths of polarized light. The obtained results confirm the improvement of the magneto-optical properties of the examined glasses under the influence of doped with erbium ions. The obtained optical parameters are interesting from the point of view of the design of optoelectronic systems. The results of transmission tests showed high permeability of the examined tellurite glasses. The presented results may be important for the development of new materials based on tellurite glasses for applications in optoelectronic devices.

Introduction. The pharmacological treatment of ventricular arrhythmias (VA) has significant limitations. Ranolazine is a relatively new drug with documented antianginal and anti-ischaemic mechanisms and where preclinical data provides evidence of additional antiarrhythmic properties. The aim of this article was to evaluate the safety and efficacy of ranolazine in patients with recurrent antiarrhythmic therapy-refractory VA. Material and methods. This prospective evaluation included 30 patients (pts) (male/female: 26/4; mean age: 65 ± 10 years; coronary artery disease/dilated cardiomyopathy: 20/10; New York Heart Association class I/II/III/IV: 2/14/12/2, left ventricular ejection fraction: 27 ± 10%; implantable cardioverter-defibrillator (ICD): 15 pts, implantable cardioverter- -defibrillator with cardiac resynchronisation therapy (CRT-D): 14 pts with recurrent significant VA [ventricular fibrillation, sustained ventricular tachycardia (VT) and/or non-sustained VT, multiple ventricular premature complexes > 1,000/ /day, biventricular stimulation (BiV) i.e. pharmacotherapy, coronary revascularisation, and percutaneous ablation, had proved ineffective. The severity of the arrhythmia was assessed by 24-hour electrocardiographic (ECG) Holter monitoring and in ICD/CRT-D memory recording. The patients received, in addition to the standard pharmacotherapy (amiodarone: 18 pts, beta-blocker: 26 pts) ranolazine 375 mg twice daily for three months. Baseline data was compared to the data obtained after the three months of ranolazine treatment. Results. We observed a significant reduction of total ventricular extrasystoles determined by ECG Holter monitoring (median: 1,737 vs 1,260, p = 0.04). Similarly, significant VA in ICD/CRT-D memory recording was diminished (67.7 vs 35.5%, p = 0.03). The number of ICD interventions in terms of both antitachycardia pacing (9 pts vs 2 pts, p = 0.01), and shock delivery (8 pts vs 2 pts, p = 0.01), was lower after the three-month observation. The therapy was ineffective for nine (29%) patients — two were hospitalised during the three-month follow-up because of recurrent arrhythmia and in seven pts there was no noticeable reduction in the amount of VA. Adverse effects, in the form of gastrointestinal symptoms (diarrhoea: two, constipation: one), occurred in three (10%) patients. Conclusions. Authors observed no significant QT prolongation in any patient. There were no differences between the baseline and the post-ranolazine patient clinical characteristics. Ranolazine seems to be a safe and effective second- line therapy in the reduction of VA and ICD interventions in patients with recurrent antiarrhythmic therapy-refractory events.

The implantation of a subcutaneous cardioverter-defibrillator (S-ICD) may be used instead of a traditional transvenous system to prevent sudden cardiac death. Our aim was to compare the characteristics of S-ICD patients from the multi-center registry of S-ICD implantations in Poland with the published results of the European Snapshot Survey on S-ICD Implantation (ESSS-SICDI). We compared data of 137 Polish S-ICD patients with 68 patients from the ESSS-SICDI registry. The groups did not differ significantly in terms of sex, prevalence of ischemic cardiomyopathy, concomitant diseases, and the rate of primary prevention indication. Polish patients had more advanced heart failure (New York Heart Association (NYHA) class III: 11.7% vs. 2.9%, NYHA II: 48.9% vs. 29.4%, NYHA I: 39.4% vs. 67.7%, p &lt, 0.05 each). Young age (75.9% vs. 50%, p &lt, 0.05) and no vascular access (7.3% vs. 0%, p &lt, 0.05) were more often indications for S-ICD. The percentage of patients after transvenous system removal due to infections was significantly higher in the Polish group (11% vs. 1.5%, p &lt, 0.05). In the European population, S-ICD was more frequently chosen because of patients’ active lifestyle and patients’ preference (both 10.3% vs. 0%, p &lt, 0.05). Our analysis shows that in Poland, compared to other European countries, subcutaneous cardioverters-defibrillators are being implanted in patients at a more advanced stage of chronic heart failure. The most frequent reason for choosing a subcutaneous system instead of a transvenous ICD is the young age of a patient.

Background: Cardiac resynchronization therapy (CRT) improves outcome in patients with heart failure (HF) however approximately 30% of patients still remain non-responsive. We propose a novel index&mdash, Regional Strain Pattern Index (RSPI)&mdash, to prospectively evaluate response to CRT. Methods: Echocardiography was performed in 49 patients with HF (66.5 &plusmn, 10 years, LVEF 24.9 &plusmn, 6.4%, QRS width 173.1 &plusmn, 19.1 ms) two times: before CRT implantation and 15 &plusmn, 7 months after. At baseline, dyssynchrony was assessed including RSPI and strain pattern. RSPI was calculated from all three apical views across 12 segments as the sum of dyssynchronous components. From every apical view, presence of four components were assessed: (1) contraction of the early-activated wall, (2) prestretching of the late activated wall, (3) contraction of the early-activated wall in the first 70% of the systolic ejection phase, (4) peak contraction of the late-activated wall after aortic valve closure. Each component scored 1 point, thus the maximum was 12 points. Results: Responders reached higher mean RSPI values than non-responders (5.86 &plusmn, 2.9 vs. 4.08 &plusmn, 2.4, p = 0.044). In logistic regression analysis value of RSPI &ge, 7 points was a predictor of favorable CRT effect (OR: 12, 95% CI = 1.33&ndash, 108.17, p = 0.004). Conclusions: RSPI could be a valuable predictor of positive outcome in HF patients treated with CRT.

Background: Implantable cardioverter-defibrillator (ICD) therapy has been proven effective in the prevention of sudden cardiac death, but data on outcomes of ICD therapy in the young and otherwise healthy patients with long QT syndrome (LQTS) are limited. Aim: We sought to collect data on appropriate and inappropriate ICD discharges, risk factors, and ICD-related complications. Methods: All LQTS patients implanted with an ICD in 14 centres were investigated. Demographic, clinical, and ICD therapy data were collected. Results: The study included 67 patients (88% female). Median age at ICD implantation was 31 years (12–77 years). ICD indication was based on resuscitated cardiac arrest in 46 patients, syncope in 18 patients, and malignant family history in three patients. During a median follow-up of 48 months, 39 (58%) patients received one or more ICD therapies. Time to first appropriate discharge was up to 55 months. Inappropriate therapies were triggered by fast sinus rhythm, atrial fibrillation, and T-wave oversensing. No predictors of inappropriate shocks were identified. Risk factors for appropriate ICD therapy were: (1) recurrent syncope despite b-blocker treatment before ICD implantation, (2) pacemaker therapy before ICD implantation, (3) single-chamber ICD, and (4) noncompliance to b-blockers. In 38 (57%) patients, at least one complication occurred. Conclusions: ICD therapy is effective in nearly half the patient population; however, the rates of early and late complica­tions are high. Although the number of unnecessary ICD shocks and reimplantation procedures may be lowered by modern programming and increased longevity of newer ICD generators, other adverse events are less likely to be reduced.

Introduction The aim of our study was to determine the risk factors for electrical storm (ES) and to assess the impact of ES on the long-term prognosis in patients after myocardial infarction (MI) with an implantable cardioverter-defibrillator (ICD) for secondary prevention of sudden cardiac death (SCD). Material and methods We retrospectively analyzed 416 patients with coronary artery disease after MI who had an implanted ICD for secondary prevention of SCD. Fifty (12%) patients had one or more incidents of an electrical storm – the ES (+) group. We matched the reference group of 47 patients from 366 ES (–) patients. Results We analyzed 3,408 episodes of ventricular arrhythmias: 3,148 ventricular tachyarrhythmic episodes in the ES (+) group (including 187 episodes of ES) and 260 in the ES (–) group. Multivariate logistic regression showed that inferior wall MI (RR = 3.98, 95% CI: 1.52–10.41) and the absence of coronary revascularization (RR = 2.92, 95% CI: 1.18–7.21) were independent predictors of ES (p = 0.0014). During 6-year observation of 97 patients, there were 39 (40%) deaths: 25 (50%) subjects in the ES (+) group and 14 (30%) in the ES (–) group (p = 0.036). Independent predictors of death were: the occurrence of ES (HR = 1.93), older age (HR = 1.06), and lower left ventricular ejection fraction (HR = 0.95) (for all p < 0.001). Conclusions Electrical storm in patients after MI with ICD for secondary prevention is a relatively common phenomenon and has a negative prognostic significance. Myocardial infarction of the inferior wall and the absence of coronary revascularization are predisposing factors for the occurrence of an ES.

Background Cardiac resynchronization therapy (CRT) reduces morbidity and mortality in heart failure individuals. Despite many efforts 30–40% patients with CRT still do not respond to therapy. Purpose The purpose was to propose and evaluate a novel dyssynchrony scoring system – Regional Strain Pattern Index (RSPI) in prediction of response to CRT. Methods Forty-nine HF patients (age: 66.5±10 years, LV ejection fraction: 24.9±6.4%, QRS: 173.1±19.1 ms, NYHA class II/III: 34.7%/63.3%, ischemic aetiology: 57.1%) underwent CRT implantation. Transthoracic echocardiography was performed before and 15±7 months after CRT implantation. Baseline dyssynchrony was studied by standard indices, strain pattern analysis and novel, author's index- RSPI. RSPI constitutes a method of quantification of LBBB-related strain curve pattern analysis. RSPI was calculated using longitudinal strain by 2D speckle tracking as the sum of dyssynchronous components across 12 segments from all three apical views. In one view, four components were assessed: 1) the contraction of the early-activated wall, 2) the prestretching of the late activated wall, 3) the contraction of the early-activated wall in the first 70% of the systolic ejection phase and 4) the peak contraction of the late-activated wall after aortic valve closure. One point was attributed to each component, thus the maximum was 12 points. RSPI was prospectively evaluated in predicting response to CRT. The response to CRT was defined as ≥15% reduction in the left ventricular end-systolic volume after 15 month follow-up. Results Thirty-six (73.5%) patients were responders. There were no significant differences in the baseline demographics, clinical, echocardiographic parameters between responders and non-responders. The mean RSPI in the overall population was 5.39±2.9 and higher RSPI values were observed in responders (5.86±2.9) than in non-responders (4.08±2.4, p=0.044). The ROC curve indicated that RSPI significantly predicted CRT response (area under curve, AUC=0.691, p=0.014) with the cut-off value at 7 points. Therefore, study population was divided into two groups according to RSPI score: ≥7 points (19 patients, 38.8% of population) and ROC curve for RSPI Conclusion RSPI constitutes a novel, valuable predictor for CRT response. Atrial fibrillation (AF) significantly increases the risk of thromboembolic events, in particular the risk of stroke. Anticoagulation therapy has been shown to reduce this risk; therefore, the treatment should be lifelong. However, the risk in patients with nonvalvular AF is not equally distributed, and there is a population of patients at low risk. According to the current guidelines, the decision on the need of anticoagulation is primarily dependent on whether the patient is at low risk. The CHA2DS2-VASc is currently the most commonly recommended scheme for assessing thromboembolic risk in patients with nonvalvular AF. In a large group of nontreated patients with a CHA2DS2-VASc of 0 (1 in women), the annual risk of stroke was 0.49%; ischemic stroke, 0.43%; bleeding, 1.08%; intracranial bleeding, 0.15%; and death, 3.87%. In patients on warfarin, the frequency of ischemic stroke was similar. Patients with a CHA2DS2-VASc of 0 (1 in women) are low-risk patients who do not benefit from anticoagulation. The low-risk group is also defined as patients younger than 65 years of age without structural cardiovascular disease, regardless of sex. They represent from 6% to 10% of patients with nonvalvular AF. Thromboembolic risk in patients with a score of 1 (2 in women) is much more controversial, as reflected by several recently published cohort studies. In a Swedish study, the risk was found to be low, while in Danish and Taiwanese studies-as significantly higher. Another analysis has shown that the use of vitamin K antagonists is appropriate when the risk of stroke is higher than 1.7%/year. Owing to a lower risk of intracranial bleeding, anticoagulation with nonvitamin K antagonist oral anticoagulants may be considered already at an annual risk of stroke exceeding 0.9%. Patients at low risk do not require chronic anticoagulation.

Background: The prevalence of abdominal obesity and type 2 diabetes mellitus (T2DM) is a public health challenge. New solutions need to be developed to help patients implement lifestyle changes. Objective: The objective of the study was to evaluate a fully automated Web-based intervention designed to help users improve their dietary habits and increase their physical activity. Methods: The Accompagnement Nutritionnel de l’Obesite et du Diabete par E-coaching (ANODE) study was a 16-week, 1:1 parallel-arm, open-label randomized clinical trial. Patients with T2DM and abdominal obesity (n=120, aged 18-75 years) were recruited. Patients in the intervention arm (n=60) had access to a fully automated program (ANODE) to improve their lifestyle. Patients were asked to log on at least once per week. Human contact was limited to hotline support in cases of technical issues. The dietetic tool provided personalized menus and a shopping list for the day or the week. Stepwise physical activity was prescribed. The control arm (n=60) received general nutritional advice. The primary outcome was the change of the dietary score (International Diet Quality Index; DQI-I) between baseline and the end of the study. Secondary endpoints included changes in body weight, waist circumference, hemoglobin A1c (HbA1c) and measured maximum oxygen consumption (VO2 max). Results: The mean age of the participants was 57 years (standard deviation [SD] 9), mean body mass index was 33 kg/m² (SD 4), mean HbA1c was 7.2% (SD 1.1), and 66.7% (80/120) of participants were women. Using an intention-to-treat analysis, the DQI-I score (54.0, SD 5.7 in the ANODE arm; 52.8, SD 6.2 in the control arm; P=.28) increased significantly in the ANODE arm compared to the control arm (+4.55, SD 5.91 vs -1.68, SD 5.18; between arms P

A newly modified correlation equation between defect-related positron lifetime t₂ (ns) defined within two-state model and corresponding radius R (Å) of freevolume positron traps in the full non-linear form or simplified linear-approximated is proved to account for compositional trends in void volume evolution of chalcogenide semiconductor compounds like binary As-S(Se) glasses. Specific chemical environment of free-volume voids associated with neighbouring network-forming polyhedrons is shown to play a decisive role in this correlation, leading to systematically enhanced estimated void sizes in comparison with typical molecular substrates, such as polymers.

Despite very different aetiologies and clinical expressions, advancing knowledge in the physiopathology and treatment of immune and inflammatory diseases (IID) prompts us to consider them as a whole. These are chronic, often incapacitating and painful illnesses that progress and destroy organs. Management by discipline too often leads to erroneous diagnoses and sometimes inappropriate treatment. More integrated translational research would further understanding of the complex relationships between cytokines and organ damage, which vary with the conditions and patients, making it possible to develop new biomarkers and personalize treatment. The research in France has very many strengths but its organization is fragmented. Better coordinated research into IID, which could be based on creating a strategic valorization field (domaine de valorisation stratégique, DVS) and thematic multi-organization institute (Institut thématique multi-organismes ITMO), would advance patient management.

Resume Malgre des etiologies et expressions cliniques tres diverses, la progression des connaissances dans les domaines de laphysiopathologie et de la therapeutique des pathologies immuno-inflammatoires (P2I)amene a les considerer de facon globale.Ce sont des maladies chroniques, souvent invalidantes et douloureuses, evolutives et destructrices d’organes. Leur prise en charge par discipline conduit trop souvent a des errances diagnostiques avec parfois une prise en charge inadaptee. Une recherche translationnelle plus integree permettrait de mieux comprendre les relations complexes entre cytokines et atteintes d’organes, qui sont variables selon les pathologies et les patients, de developper de nouveaux biomarqueurs et de personnaliser la the rapeutique. La recherche en France presente de tres nombreux atouts mais son organisation est morcelee. Une recherche sur les P2I mieux coordonnee, qui pourrait reposer sur la creation d’un domaine de valorisation strategique (DVS) et d’un institut thematique multi-organisme (ITMO) dedie, permettrait de faire progresser la prise en charge des patients.

We assessed the value of left atrium speckle tracking imaging (STI) indices, and clinical and other echocardiographic parameters in persistent atrial fibrillation (AF) patients to predict the efficacy of electrical cardioversion (EC) and sinus rhythm (SR) maintenance at 6 months.Eighty persistent AF patients planned to receive EC, underwent echocardiography including STI. After 24h, patients with successful EC were examined to predict SR maintenance. After 6 months patients with no AF recurrence in electrocardiogram (ECG) underwent 7-day ECG to exclude silent AF. Every AF1 min was a recurrence.SR restored in 61 patients, 19 unsuccessful. Prior use of statins (68.8% vs. 42.1%, p=0.03) was the only factor, determined later by univariate analysis to be a significant EC success predictor (OR=1.09, CL ± 95% 1.001-1.019, p0.03). Both groups received similar antiarrhythmics and had similar echocardiographic parameters. After 6 months, SR was maintained in 19 patients (31.1%, Group I); AF recurrences were registered in 42 patients (68.8%, Group II). In Group I, only the mitral valve deceleration time (MVDT) 224.18 ± 88.13 vs. 181.6 ± 60.6 in Group II, p=0.04) and the dispersion of time to peak longitudinal strain (dTPLS) (86.0 ± 68.3 vs. 151.8 ± 89.6, p=0.03) differed significantly. The univariate analysis revealed dTPLS as a significant predictor of SR maintenance.High EC efficacy and frequent AF recurrences were observed. The dispersion of time to the maximal longitudinal strain (LS) of left atrial segments facilitated prediction of SR maintenance. The value of 7-day ECG monitoring for detection of arrhythmia after 6 months was limited.

WOS: 000418210700016, PubMed ID: 28336205, Background: Andersen-Tawil syndrome (ATS) is rare channelopathy caused by KCNJ2 mutation and probably KCNJ5. It is characterized by arrhythmias, neurological symptoms, and dysmorphic features. The present study retrospectively examined the characteristics of 11 unrelated families with ATS. Methods: This study consisted of 11 probands positive for KCNJ2 variants and 33 family members (mean age 30.0 +/- 17.3 years, female n=31). Additional genetic screening of 3 LQTS genes (KCNQ1, KCNH2, SCN5A) was performed in 9 families. Predictors of arrhythmias [premature ventricular beats>2000/24h, biventricular and polymorphic ventricular tachycardia (VT)], syncope, and/or cardiac arrest (CA) were evaluated. Results: In KCNJ2 mutation carriers vs non-carriers (n=25 vs n=19) significant differences were observed in U-wave manifestations in V2-V4, T-peak-T-end duration, QTUc duration (p2000/24h was higher (p=0.005). Conclusion: A higher risk of arrhythmia, syncope, and/or CA is associated with the presence of micrognathia, periodic paralysis, and prolonged T-peak-T-end time. Long-wave shift of the optical transmission spectrum in the region of fundamental optical absorption edge is registered for As2S3 chalcogenide glassy semiconductors after γ-irradiation. This effect is explained in the frameworks of the destruction-polymerization transformations concept by accepting the switching of the heteropolar As–S covalent bonds into homopolar As–As ones. It is assumed that (As4+; S1–) defect pairs are created under such switching. Formula to calculate content of the induced defects in chalcogenide glassy semiconductors is proposed. It is assumed that defects concentration depends on energy of broken covalent bond, bond-switching energy balance, correlation energy, optical band-gap and energy of excitation light. It is shown that theoretically calculated maximally possible content of radiation-induced defects in As2S3 is about 1.6% while concentration of native defects is negligible. The processes of atomic shrinkage in network-forming solids initiated by external influences are tested using technique of positron annihilation lifetime spectroscopy at the example of chalcogenide vitreous semiconductors of arsenic sulphide type and acrylic polymers for dental application. Two state positron trapping is shown to be responsible for atomic shrinkage in chalcogenide glasses, while mixed trapping and ortho-positronium decaying is character for volumetric densification and stress propagation in acrylic dental polymers. At the basis of the obtained results it is concluded that correct analysis of externally-induced shrinkage in polymer networks under consideration can be developed by using original positron lifetime data treatment algorithms to compensate defect-free bulk annihilation channel within two-state positron trapping model and account for an interbalance between simultaneously co-existing positron trapping and orth-positronium related decaying channels within mixed three-state positron annihilation model (© 2012 WILEY-VCH Verlag GmbH & Co. Background: Repolarization dynamicity (QT/RR) is supposed to be a prognostic marker in post-MI patients. However, data on the relationships between early and late phases of QT and RR intervals (QT peak/RR and T peak–T end/RR) are insufficient, and which ECG lead should be used for the analysis is unclear. We analyzed repolarization dynamicity in patients after anterior MI with and without VT/VF history using two leads of Holter recordings- modified V5 and V3. The daytime and nighttime periods were also analyzed. Methods: Cohort of 88 patients after anterior MI (>6 months) consisted of 43 patients without VT/VF (33 males; 59 ± 12 years; LVEF: 41 ± 7%; NoVT/VF), and 45 patients with VT/VF history- ICD implanted as secondary prevention (40 males; 64 ± 10 years; LVEF: 32 ± 8%; VT/VF). QT/RR, QT peak/RR and T peak–T end/RR were calculated from 24-hour ECG for the entire recording, daytime and nighttime periods, from V5 and V3 leads, respectively. Results: VT/VF patients had lower LVEF (P = 0.001). There were no differences in age and gender. VT/VF group had steeper QT/RR, QT peak/RR, and T peak–T end/RR in V5: 0.233 ± 0.04 versus 0.150 ± 0.05, P = 0.0001, 0.181 ± 0.04 versus 0.120 ± 0.04, P = 0.0001, 0.052 ± 0.02 versus 0.030 ± 0.02, P = 0.0001, and in V3: 0.201 ± 0.04 versus 0.149 ± 0.05, P = 0.0001, 0.159 ± 0.03 versus 0.118 ± 0.04, P = 0.0001, and 0.042 ± 0.02 versus 0.031 ± 0.02, P = 0.004; respectively. VT/VF patients had higher indices in V5 than in V3 lead (P = 0.001). QT/RR and QT peak/RR were steeper at daytime period in both leads. It was not found for T peak–T end/RR. Conclusions: Patients with VT/VF history are characterized by steeper relationships between repolarization duration and RR intervals. These findings are more evident in modified V5 lead.

UNLABELLED Reperfusion therapy, mainly primary percutaneous coronary intervention (PCI), has improved survival and lowered complication rate in patients with ST elevation myocardial infarction (STEMI). Nevertheless, some patients develop left ventricular remodeling (LVR) during long-term follow-up. AIMS To assess the incidence of ischemic mitral regurgitation (MR) in the acute phase of STEMI treated with primary PCI. To assess prognostic value of MR during acute STEMI for prediction of LVR during long-term follow-up. METHODS This is a prospective, single-center study in 83 patients with the first STEMI. Inclusion criteria were as follows: time from symptom onset to PCI less than 12 h and successful restoration of blood flow (thrombolysis in myocardial infarction 3) in the infarct-related coronary artery. Transthoracic echocardiography was performed at discharge and 6 months after the MI. RESULTS At hospital discharge, ischemic MR was found in 35 (42%) patients. At 6 months follow-up, LVR was present in 21 (25%) patients. Univariate analysis revealed that remodeling could be predicted by age, weight, treatment with abciximab, left ventricular ejection fraction (LVEF), leaflets coaptation, coaptation height, tenting area, presence of MR, degree of MR. The best multivariate logistic regression model for remodeling prediction at 6 months was combination of ischemic MR degree (odds ratio (OR)=14.5; 95% confidence interval (CI): 3.89-54.0, P

(As 2 S 3 ) 0.6 (GeS 2 ) 0.4 glass in non-irradiated and γ-irradiated states has been studied by using high-energy synchrotron X-ray diffraction, extended X-ray absorption fine structure spectroscopy, and positron annihilation lifetime spectroscopy. The experimental results are explained by the local changes around As and Ge atoms upon irradiation. These changes are suggested to involve chemical bonds distortion, formation of defective bonds with wrong coordination, rotation of structural units and appearance of additional free volume in the glass network.

It was shown that under-coordinated topological defects induced by high-energy γ-irradiation can be a reason for significant changes in positron annihilation lifetime spectra of multicomponent chalcogenide glassy semiconductors within ternary Ge-As(Sb)-S systems. In the case of negatively-charged sulphur and arsenic atoms, the excess of free volume is quite enough to produce additional input in the second defect-related channel of positron trapping, while under-coordinated germanium atoms are practically non-detectable with this technique because of low associated free volume. Despite radiation-induced densification, the average positron lifetime demonstrate both growing and decaying tendencies after γ-irradiation depending on glass composition. BACKGROUND: QT/RR relationship was found to be both rate-dependent and rate-independent, what suggests the influence of autonomic drive and other not-autonomic related factors on it. The steeper QT/RR slope in patients after acute myocardial infarction (MI) was described, but the relationship to ventricular arrhythmias is unknown. The purpose of this study was to calculate differences in QT/RR relationship in patients after remote anterior MI with left ventricular dysfunction and different types of ventricular arrhythmias. METHODS: The cohort of 95 patients (age: 63 +/- 11 years, LVEF: 35 +/- 9%) with previous anterior MI (mean 1.1 years) was divided into two well-matched groups-50 patients without episodes of ventricular tachycardia (VT) or ventricular fibrillation (VF) (NoVT/VF: 39 males, 64 +/- 12 years, LVEF 37 +/- 8%) and 45 patients with VT and/or VF (all with ICD implanted) (VT/VF: 35 males, 62 +/- 10 years, LVEF 34 +/- 10%). No true antiarrhythmics were used. QT/RR slope was calculated from 24-hour Holter ECG for the entire recording (E), daytime (D) and nighttime (N) periods. RESULTS: Groups did not differ in basic clinical data (age, LVEF, treatment). QT/RR slopes were steeper in VT/VF than in NoVT/VF group in all analyzed periods: E - 0.195 +/- 0.03 versus 0.15 +/- 0.03 (P < 0.001), N - 0.190 +/- 0.03 versus 0.138 +/- 0.03 (P < 0.001) and D - 0.200 +/- 0.04 versus 0.152 +/- 0.03 (P < 0.001). No significant day-to-night differences were found in both groups. CONCLUSIONS: Steeper QT/RR slope and complete lack of day-to-night differences in VT/VF patients show inappropriate QT adaptation to the heart rate changes. The prognostic significance of this parameter needs prospective studies.

Resume Introduction. – L'objectif de cette etude etait d'evaluer l'efficacite et la tolerance de la nouvelle forme a liberation modifiee de la clarithromycine (CLA-LM) (500 mg × 2) vs la telithromycine (TELI) (400 mg × 2), en traitement court des EABC (une prise quotidienne durant cinq jours). Methode. – Cette etude randomisee, en double insu incluait des patients atteints d'EABC sans syndrome obstructif severe connu (VEMS > 35 %), avec purulence franche des crachats et soit, aggravation de la dyspnee, soit augmentation du volume de l'expectoration (Anthonisen II), soit les deux (Anthonisen I). Resultats. – Trois cent soixante-deux patients ont ete analyses (62,6 ± 12,9 ans; hommes : 58,8 %) ; 53,8 % avaient une culture positive a l'inclusion; Haemophilus influenzae ( n = 57), Moraxella catarrhalis ( n = 42) et Streptococcus pneumoniae ( n = 41). En per protocole , le taux de succes clinique a j8 etait de 97 % (161/166) et 97 % (146/151), IC 97,5 % [–4,12 –4,71], le taux de guerison clinique a j30 de 78 (129/166) vs 77 % (116/151), p = 0,85 et le delai moyen sans recidive de 62 vs 61 jours, p = 0,08, dans les groupes CLA-LM et TELI respectivement. Quatorze patients du groupe CLA-LM (8,2 %) et 20 du groupe TELI (12,4 %) ont presente au moins un effet indesirable lie au traitement, le plus souvent de type digestif ( p = 0,21). Conclusion. – Administree pendant cinq jours, la clarithromycine forme a liberation modifiee (1000 mg en une prise/jour), est un traitement au moins aussi efficace que la telithromycine dans les EABC sans syndrome obstructif severe et bien tolere.

Mapping of Atrial Fibrillation. Introduction: While atrial fibrillation (AF) initiation in the pulmonary veins has been well-studied, simultaneous biatrial and three-dimensional noncontact mapping (NCM) has not been performed. We hypothesized that these two techniques would provide novel information on triggers, initiation, and evolution of spontaneous AF and permit study of different AF populations. Methods and Results: The origin of atrial premature beats (APBs), onset of spontaneous AF and its evolution were analyzed in 50 patients with AF in the presence or absence of structural heart disease (SHD) and in different AF presentations (group A: Persistent, group B: Paroxysmal). In 45 patients, spontaneous APBs in the right atrium (RA; n = 60) and left atrium (LA; n = 25) with similar regional distributions regardless of heart disease status were demonstrated. In total, 22 patients (44%) had >2 disparate regional origins. Biatrial regional foci were seen with equal frequency in patients with SHD (31%), without SHD (40%), in group A (32%), and in group B (36%). Biatrial mapping and NCM showed organized monomorphic atrial tachyarrhythmias arising in the RA (17), septum (17), or LA (21) and were classified as atrial flutter (RA = 34, LA = 8), macro-reentrant atrial tachycardia (RA = 1, LA = 3) or focal atrial tachycardia (RA = 2, LA = 7). Their regional distribution was more extensive in patients with SHD and persistent AF compared with patients without SHD or paroxysmal AF. Simultaneous biatrial tachycardias were observed only in group A patients and those with SHD. Conclusions: Simultaneous biatrial and NCM permits successful AF mapping in different AF populations and demonstrates a biatrial spectrum of spontaneous triggers and tachycardias. Organized monomorphic tachycardias with multiple unilateral or biatrial locations are commonly observed in human AF. Patients with heart disease or persistent AF have a more extensive distribution as well as simultaneous coexistence of multiple tachycardias during AF.

A meaningful interpretation of positron lifetime characteristics for glassy-like g-As2Se3 is developed taking into account calculations of Jensen et al. (J. Non-Cryst. Solids 170 (1994) 57) for positrons trapped by free-volume extended defects in orthorhombic As2Se3 and void volume distribution for 146-atoms layer-biased model of amorphous As2Se3 presented by Popescu (J. Non-Cryst. Solids 35–36 (1980) 549). The obtained results are compared for samples having different thermal pre-history. Two groups of experimental results with close lifetime characteristics are distinguished for each of the investigated samples. This feature is explained in terms of average positron lifetime by applying two-state positron trapping model for mathematical treatment of the obtained spectra. r 2003 Elsevier B.V. Mutations in sodium channel alpha-subunit gene (SCN5A) result in multiple arrhythmic syndromes, including long QT3 (LQT3), Brugada syndrome (BS), an inherited cardiac conduction defect, sudden unexpected nocturnal death syndrome (SUNDS) and sudden infant death syndrome (SIDS), constituting a spectrum of disease entities termed Na+ channelopathies. These diseases are allelic disorders, if not the same disease with variable penetrance and variable modifiers worldwide. Interestingly, death occurs during sleep in all of these disorders, suggesting a common mechanism. To date, mutational analyses have revealed about 103 distinct mutations in SCN5A, of which at least more than 30 mutations are associated with LQT3, whereas the rest of the mutations are affiliated with the remaining sodium channel disorders. The majority of these mutations are missense. However, other types such as deletions, insertions, frameshifts, nonsense and splice-donor errors have also been reported.

The modified model of native and radiation-induced microvoid-type positron traps in vitreous chalcogenide semiconductors is developed to explain compositional features of positron annihilation lifetime measurements in stoichiometric As2S3–GeS2 and non-stoichiometric As2S3–Ge2S3 chalcogenide glasses before and after γ-irradiation.

The influence of γ-irradiation on the positron annihilation lifetime spectra in chalcogenide vitreous semiconductors of As-Ge-S system has been analysed. The correlations between lifetime data, structural features and chemical compositions of glasses have been discussed. The observed lifetime components are connected with bulk positron annihilation and positron annihilation on various native and γ-induced open volume defects. It is concluded that after γ-irradiation of investigated materials the γ-induced microvoids based on S - 1 , As2, and Ge - 3 coordination defects play the major role in positron annihilation processes.

The defect formation process in vitreous chalcogenide semiconductors (VChS) of the ternary As–Ge–S system, induced by 60 Co γ-irradiation with 2.82 MGy absorbed dose, is studied using positron annihilation lifetime method. The measured results are explained in terms of a modified model of coordination topological defects (CTD) with associated open volume microvoids, described by radiation-structural transformations at the level of both short-, and medium-range ordering.

Atrial flutter or tachycardia may coexist with atrial fibrillation [AF] and can be treated with ablation techniques in attempt to reduce the total AF burden. The role of ablation of latent atrial tachyarrhythmias elicited at electrophysiologic study in conjunction with atrial pacing and antiarrhythmic drugs in patients with refractory AF has not been evaluated. We evaluated the efficacy of catheter ablation of electrically induced atrial flutter or atrial tachycardia in improving rhythm control in patients with refractory AF. Methods: Consecutive patients with refractory AF, and spontaneous atrial flutter (Group 1) or without spontaneous atrial flutter (Group 2) underwent programmed stimulation in a baseline drug-free state. All patients had electrically induced atrial flutter or tachycardia. Radiofrequency ablation of the arrhythmia substrate was performed in all patients. Primary endpoints evaluated for patient outcome in both groups included maintenance of rhythm control and freedom from recurrent atrial tachyarrhythmias. Results: Forty-three patients, with a mean age of 66±13 years were studied. Group 1 consisted of 22 patients while Group 2 had 21 patients. Ablation of the tricuspid valve-inferior venacaval isthmus was performed in 41 patients who had common atrial flutter induced at electrophysiologic study. Ablation of other atrial sites was performed in 8 patients with induced atypical flutter and 4 patients with induced atrial tachycardia. Ten of these patients had ablation of more than one arrhythmia. 17 patients (40%) had atrial pacing instituted and 28 patients remained on a class 1/3 antiarrhythmic drug. During a mean follow-up of 26±14 months, 33 patients (82.5%) remained in rhythm control. Actuarial analysis showed 96% of patients in rhythm control at 6 months, 94% at 12 months, and 90% at 24 months. Freedom from symptomatic AF recurrence was 64% at 6 months, 58% at 12 months, and 42% at 24 months. The outcome for both of these endpoints was similar for Group 1 and Group 2 (p = NS). The AF free interval increased significantly from 7±9 days to 172±121 days (p < 0.01) after ablation. This increase was again similar in both the groups. In the 14 patients were who did not receive atrial pacing and who remained on the same class 1/3 antiarrhythmic drug, the AF free interval increased from 18±17 days to 212±102 days (p < 0.01).

The obtained results are explained at the level of both short- and medium-range ordering in the framework of coordination topological defects model modified with free-volume microvoids formation.

The experimental results on time degradation of radiation-induced darkening effects in vitreous chalcogenide semiconductors of ternary stoichiometric (As 2 S 3 ) y (GeS 2 ) 1 (y 0.1, 0.2, 0.4, 0.6) and non-stoichiometric (As 2 S 3 ) x (Ge 2 S 3 ) 1 (x: 0.1, 0.2, 0.4, 0.8) systems with a variation of the average co-ordination number, Z, are discussed. It is established that adequate model for the quantitative description of these effects can be developed at the basis of bimolecular relaxation function (RF) proper to annihilation of specific structural defects in the form of oppositively charged atoms with various co-ordinations. The observed features of the investigated dynamic radiation-optical changes in these glasses are explained by compositional dependences of their atomic compactness, as well as covalent bond statistics.

Neuron-specific enolase (NSE) is a glycolytic enzyme localized within neuronal and neuroendocrine tissues. Serum NSE is widely used as a marker of neuroendocrine tumors. Moderate serum NSE elevation has been reported in some patients with benign lung diseases. We decided to investigate whether the elevation of serum NSE in non-neoplastic lung diseases is connected with hypoxemia and to what extent the recovery of sufficient ventilation with a respirator may influence NSE concentrations. Serum NSE was estimated by means of radioimmunoassay in 83 patients with various non-neoplastic lung diseases. Serum NSE exceeding 12.5 micrograms/L was significantly more frequent in patients with marked hypoxemia (PaO26.67 kPa; p = 0.03) than in others. The median NSE value in the group of patients without respiratory failure (Ro) was 7.2 micrograms/L (10%12.5 micrograms/L), in the group of patients with respiratory failure not requiring mechanical ventilation (Rf) it was 8.5 micrograms/L (24%12.5 micrograms/L), and in the group of patients with respiratory failure requiring mechanical ventilation (Rfv) 13.1 micrograms/L (60%12.5 micrograms/L). The differences between the Rfv group and the other two groups (Rf and Ro) were significant (P = 0.049 and p = 0.0004, respectively). During successful mechanical ventilation elevated serum NSE decreased to values below the cutoff in 8/10 patients. We conclude that serum NSE elevation is a frequent event in patients with terminal hypoxemia in the course of benign lung diseases. Normalization of serum NSE is observed in the majority of patients during the first week of mechanical ventilation.

This study was designed to assess the value of tumor marker evaluation in pericardial fluid for the recognition of malignant pericarditis. Thirty-six patients with signs and symptoms of large pericardial effusion entered the study. Pericardiocentesis with pericardial fluid drainage was performed in all of them. CEA and NSE levels were evaluated in the pericardial fluid and compared to pericardial fluid cytology. The median CEA value in malignant effusions was 80 ng/ml (range 0-305 ng/ml) and in non-malignant ones 1.26 ng/ml (range 0.2-18.4 ng/ml), p Pericardial fluid cytology was positive in 22 of 26 patients with malignant pericarditis (85%). CEA exceeding 5 ng/ml or positive cytology were seen in 96% of the patients with malignant pericarditis. Low temperature anomalies in PbO-Bi2O3-Ga2O3 glasses were observed. Nonlinear optical methods, including photoinduced second harmonic generation (SHG) and piezooptics, were applied. Nitrogen (λ = 337 nm) and ruby (λ = 694 nm) lasers were used as sources of the photoinduced changes. A temperature split of the photoinduced SHG signal was observed at wavelength λ = 1.06μm. Investigations of hyper-Raman light scattering were also performed. A strict correlation between the temperature dependence of the hyper Raman scattering and the nonlinear optical results was found. To investigate the contribution of spontaneous polarization and deformation we performed dielectric and dilatometry measurements. The photoinduced changes can be explained by the strong electron-phonon interaction in such systems.

Modified model for positron annihilation in vitreous chalcogenide semiconductors is developed to explain a number of previously obtained results on positron lifetime measurements in glassy As–Ge–S of stoichiometric As2S3–GeS2 and non-stoichiometric As2S3–Ge2S3 cut-sections.

Current risk stratification after acute myocardial infarction (MI) depends on left ventricular ejection fraction. Microvolt T-wave alternans (MTWA) is one of promising markers to predict cardiac events in patients after acute MI treated according to current guidelines.In this single center study, 112 consecutive patients with the first anterior ST-elevation MI undergoing PCI12 hours from symptom onset, were enrolled prospectively. Demographics, established risk factors, myocardial contrast echocardiography (MCE) perfusion, index event data and MTWA were assessed. Composite cardiac events (CCE) defined as: death, recurrent MI, sustained ventricular tachycardia (sVT) or readmission for acute heart failure (HF) were recorded during follow-up.MTWA test was negative in 76, positive in 18 and undetermined in 7 patients. MTWA negative patients had significantly higher LVEF at 30 days. At 4 years, 26 patients experienced CCE (10 died, 2 reinfarcted and 14 HF events). In multivariate Cox proportional hazard model maximum CKMB, non-negative MTWA and reduced LVEF made the best model to predict CCE. Four year CCE free survival was 77% and was significantly lower for non-negative MTWA (94% vs 50%, p0.003).Non-negative MTWA with infarct size index and reduced LVEF could predict cardiac events in patients with anterior STEMI treated with primary PCI. MTWA non-negative patients have significantly worse outcome.

Partindo do estudo da experiência de uma instituição pública de pesquisa, procuramos caracterizar o modelo de plataformas que emergiu do processo de re-construção local.Palavras-chaves plataformas tecnológicas; biomedicina; gestão; instituições públicas; equipamentos.Abstract Biomedical research is distinguished by the emergence of an industrial logic in the process of scientific knowledge production, justified by the creation of an environment conducive to the acceleration of industrial innovation. This process resulted in the adoption of research management strategies from the industrial world. National S&T policy encourages new forms of management in research institutions, aimed at increasing productivity and developing areas with greater economic potential. Among other devices, the hardware platforms listed systematically in the actions of research promotion. However, there is little systematic knowledge about the use of local and dynamic platforms. From the study of the experience of a public research institution, we characterize the platform model that emerged from the process of local re-construction.Keywords technological platforms; biomedicine; management; public institution; equipment
