Background: Ulcerative colitis (UC), belongs to inflammatory bowel diseases (IBD), frequently induces liver inflammation and injury. Previous studies have proved that bone marrow-derived mesenchymal stem cells (BMSCs) can suppress inflammation and improve intestinal mucosal injury in colitis, however, the effects of BMSCs on colitis-induced liver injury and the underlying molecular mechanisms remain unclear. Here, we investigated the effects and mechanisms of BMSCs in acute ulcerative colitis Balb/C mice, which were induced by 4 % dextran sodium sulphate (DSS). Methods: In this study, BMSCs derived from Balb/C mice were administrated by single intravenous injection with a dose of 5*10^7 cells/kg. And then, the effects and underlying molecular mechanisms were investigated. Firstly, degree of liver injury in colitis mice was evaluated by hepatic ALT, AST, ALP and TBIL levels, which were measured by specific determination kits, the levels of TNF-α, IL-6, IFNγ and LPS were examined by ELISA. Secondly, as the indicator of intestinal-liver barrier disorder, tight junction proteins were analyzed by western blot. Thirdly, the pathological changes in colon and liver were detected by H&E staining. At last, homing of BMSCs to lesion tissues was investigated by Immunofluorescence. Results: The results indicated that histopathological changes in model mice had been greatly alleviated, BMSCs infusion remarkably decreased the serum ALT, AST, ALP and TBIL level, and meanwhile reduced pro-inflammatory cytokines in liver tissues. Furthermore, homing of BMSCs was observed in colon and liver, and the disorder of intestinal-liver barrier was declined significantly. Conclusions: In conclusion, BMSCs alleviate liver injury induced by ulcerative colitis via repairing the intestinal-liver barrier and activating hepatocyte growth factor, it has potential application prospects in the treatment of liver injury induced by ulcerative colitis.

Feature selection (FS), which aims to identify the most informative subset of input features, is an important approach to dimensionality reduction. In this article, a novel FS framework is proposed for both unsupervised and semisupervised scenarios. To make efficient use of data distribution to evaluate features, the framework combines data structure learning (as referred to as data distribution modeling) and FS in a unified formulation such that the data structure learning improves the results of FS and vice versa. Moreover, two types of data structures, namely the soft and hard data structures, are learned and used in the proposed FS framework. The soft data structure refers to the pairwise weights among data samples, and the hard data structure refers to the estimated labels obtained from clustering or semisupervised classification. Both of these data structures are naturally formulated as regularization terms in the proposed framework. In the optimization process, the soft and hard data structures are learned from data represented by the selected features, and then, the most informative features are reselected by referring to the data structures. In this way, the framework uses the interactions between data structure learning and FS to select the most discriminative and informative features. Following the proposed framework, a new semisupervised FS (SSFS) method is derived and studied in depth. Experiments on real-world data sets demonstrate the effectiveness of the proposed method.

In order to satisfy timeliness requirements arising from environmental sensing applications, age of information (AoI) was proposed to characterise the freshness of the received updates. In this paper, we consider a wireless powered relay aided communication network (WPRCN), in which a relay wirelessly powered by a hybrid-access point (H-AP) receives environmental monitoring information update from a sensor and forwards it to the H-AP. The long-term average AoI is studied since the decision correctness depends on the update timely uploaded by the relay. The wireless powered relay adopts either a decode-and-forward (DF) or an amplify and forward (AF) protocols, respectively, subject to the energy causality. We also consider the decoding cost of DF protocol owing to the relay’s limited energy storage. Since the expression of the average AoI is non-elementary, we propose a Taylor approximation based algorithm to obtain its integral. We optimise the transmit power/the equivalent average power consumption of the relay for the sake of minimising the average AoI in the whole WPRCN with different forwarding protocols. Our simulation results demonstrate the accuracy of the theoretical analysis, while the average AoI of the WPRCN is optimised by our power allocation scheme at the relay. The proposed algorithm is verified to achieve great approximation effect.

A key energy consumption in steel metallurgy comes from an iron ore sintering process. Enhancing carbon utilization in this process is important for green manufacturing and energy saving and its prerequisite is a time-series prediction of carbon efficiency. The existing carbon efficiency models usually have a complex structure, leading to a time-consuming training process. In addition, a complete retraining process will be encountered if the models are inaccurate or data change. Analyzing the complex characteristics of the sintering process, we develop an original prediction framework, that is, a weighted kernel-based fuzzy C-means (WKFCM)-based broad learning model (BLM), to achieve fast and effective carbon efficiency modeling. First, sintering parameters affecting carbon efficiency are determined, following the sintering process mechanism. Next, WKFCM clustering is first presented for the identification of multiple operating conditions to better reflect the system dynamics of this process. Then, the BLM is built under each operating condition. Finally, a nearest neighbor criterion is used to determine which BLM is invoked for the time-series prediction of carbon efficiency. Experimental results using actual run data exhibit that, compared with other prediction models, the developed model can more accurately and efficiently achieve the time-series prediction of carbon efficiency. Furthermore, the developed model can also be used for the efficient and effective modeling of other industrial processes due to its flexible structure.

To systematically evaluate the clinicopathological and prognostic value of extra-hepatic bile duct resection (EHBDR) in the surgical management of patients with gallbladder carcinoma (GBC), especially in non-jaundiced patients.PubMed, EMBASE and the Cochrane Library were searched up to March 1EHBDR did not correlate with a better overall survival (OS) (P = 0.17) or disease-free survival (P = 0.27). No survival benefit was also observed in patients with T2N1 (P = 0.4), T3N0 (P = 0.14) disease and node-positive patients (P = 0.75), rather, EHBDR was even harmful for patients with T2N0 (P = 0.01) and node-negative disease (P = 0.02). Significantly higher incidences of recurrent disease (P = 0.0007), postoperative complications (P 0.00001) and positive margins (P = 0.02) were detected in the bile duct-resected group. The duration of postoperative hospital stay between the two groups was comparable (P = 0.58). Selection bias was also detected in our analysis that a significantly higher proportion of advanced lesions with T3-4 or III-IV disease was observed in the bile duct-resected group (P 0.00001). EHBDR only contributed to a greater lymph yield (P = 0.01).EHBDR has no survival advantage for patients with GBC, especially for those with non-jaundiced disease. Considering the unfairness of comparing OS between jaundiced patients receiving EHBDR with non-jaundiced patients without EHBDR, we could only conclude that routine EHBDR in non-jaundiced patients is not recommended and future well-designed studies with more specific subgroup analyses are required for further validation.

Tumor budding is a significant prognostic indicator for poor survival of several solid tumors. However, due to the lack of a standard scoring system, its clinical application for biliary tract cancer (BTC) is limited.To identify the prognostic significance of tumor budding in BTC.Tumor budding was associated with poor histologic differentiation, lymphovascular invasion, perineural invasion, lymph node metastasis, positive surgical margin, etc. Tumor budding was a predictor of poor OS in univariate (HR: 4.36; 95% CI 3.15 to 6.02; P 0.001) and multivariate (HR: 2.95; 95% CI 2.28 to 3.80; P 0.001) analysis. Similarly, it was also a predictor of poor DFS in univariate (HR: 3.26; 95% CI 2.12 to 4.99; P 0.001) and multivariate (HR: 3.21; 95% CI 1.90 to 5.40; P 0.001) analysis. In addition, tumor budding was also associated with advanced T-stage, poor histologic differentiation, lymph node metastasis, positive resection margin, lymphatic invasion, vascular invasion, and perineural invasion.Results of our study have shown that tumor budding is a strong predictor of poor survival for BTC. The clinical utility of tumor budding as a prognostic marker for BTC should be considered after developing a standard international consensus based on the current evidence.

Summary: Previous studies have explored the role of laparoscopic surgery (LS) in the surgical management of gallbladder carcinoma (GBC) and obtained satisfactory outcomes versus conventional open surgery. However, most of them either included a small number of patients or mainly focused on the early-staged lesions. Therefore, their results were less statistical powerful and a more comprehensive evaluation on the role of LS in GBC is warranted. A thorough database searching was performed in PubMed, EMBASE and Cochrane Library for comparative studies between the laparoscopic and open approach in the surgical management of GBC and 18 comparative studies were finally identified. RevMan 5.3 and Stata 13.0 software were used for statistical analyses. Pooled results revealed that patients in the laparoscopic group recovered faster with less intraoperative hemorrhage and less postoperative morbidity. Comparable operative time, overall recurrence rate, R0 resection rate, lymph node yield, intraoperative gallbladder violation rate and postoperative survival outcomes were also acquired. Regarding the debating issue of port-site recurrence, a significantly higher incidence of port-site recurrence was observed in laparoscopic group. However, having excluded studies on incidental gallbladder carcinoma, the subsequent pooled result showed no significant difference. Considering the inherent inconsistency of the surgical indication between laparoscopic and open surgeries and the deficiency of advanced lesions, we drew a conclusion that laparoscopic surgery seems to be only safe and feasible for early- or middle-staged lesions. Upcoming random controlled trials or comparative studies with equivalent surgical indication focused on advanced lesions are warranted for further evaluation.

Background and ObjectivesWomen have higher lifetime risk of stroke than men, and metabolic factors seem more strongly associated with stroke for women than men. However, few studies in either men or women have evaluated metabolomic profiles and incident stroke.MethodsWe applied liquid chromatography–tandem mass spectrometry to measure 519 plasma metabolites in a discovery set of women in the Nurses' Health Study (NHS; 454 incident ischemic stroke cases, 454 controls) with validation in 2 independent, prospective cohorts: Prevención con Dieta Mediterránea (PREDIMED; 118 stroke cases, 791 controls) and Nurses' Health Study 2 (NHS2; 49 ischemic stroke cases, 49 controls). We applied logistic regression models with stroke as the outcome to adjust for multiple risk factors; the false discovery rate was controlled through the q value method.ResultsTwenty-three metabolites were significantly associated with incident stroke in NHS after adjustment for traditional risk factors (q < 0.05). Of these, 14 metabolites were available in PREDIMED and 3 were significantly associated with incident stroke: methionine sulfoxide, N6-acetyllysine, and sucrose (q < 0.05). In NHS2, one of the 23 metabolites (glucuronate) was significantly associated with incident stroke (q < 0.05). For all 4 metabolites, higher levels were associated with increased risk. These 4 metabolites were used to create a stroke metabolite score (SMS) in the NHS and tested in PREDIMED. Per unit of standard deviation of SMS, the odds ratio for incident stroke was 4.12 (95% confidence interval [CI] 2.26–7.51) in PREDIMED, after adjustment for risk factors. In PREDIMED, the area under the receiver operating characteristic curve (AUC) for the model including SMS and traditional risk factors was 0.70 (95% CI 0.75–0.79) vs the AUC for the model including the traditional risk factors only of 0.65 (95% CI 0.70–0.75), corresponding to a 5% improvement in risk prediction with SMS (p < 0.005).DiscussionMetabolites associated with stroke included 2 amino acids, a carboxylic acid, and sucrose. A composite SMS including these metabolites was associated with ischemic stroke and showed improvement in risk prediction beyond traditional risk factors.Classification of EvidenceThis study provides Class II evidence that a SMS accurately predicts incident ischemic stroke risk.

PURPOSE Early detection is significant to reduce lung cancer-related death. Computer-aided detection system (CADs) can help radiologists to make an early diagnosis. In this paper, we propose a novel 3D gray density coding feature (3D GDC) and fuse it with extracted geometric features. The fusion feature and random forest are used for benign-malignant pulmonary nodule classification on Chest CT. METHODS First, a dictionary model is created to acquire codebook. It is used to obtain feature descriptors and includes 3D block database (BD) and distance matrix clustering centers. 3D BD is balanced and randomly selecting from benign and malignant pulmonary nodules of training data. Clustering centers is got by clustering the distance matrix, which is the distance between every two blocks in 3D BD. Then, feature descriptor is obtained by coding the pulmonary nodule with codebook, and 3D GDC feature is the result of histogram statistics on feature descriptor. Second, geometric features are extracted for fusion feature. Finally, random forest is performed for benign-malignant pulmonary nodule classification with fusion feature of the 3D gray density coding feature and the geometric features. RESULTS We verify the effectiveness of our method on the public LIDC-IDRI dataset and the private ZSHD dataset. For LIDC-IDRI dataset, compared with other state-of-the-art methods, we achieve more satisfactory results with 93.17 ± 1.94% for accuracy and 97.53 ± 1.62% for AUC. As for private ZSHD dataset, it contains a total of 238 lung nodules from 203 patients. The accuracy and AUC achieved by our method are 90.0% and 93.15%. CONCLUSIONS The results show that our method can provide doctors with more accurate results of benign-malignant pulmonary nodule classification for auxiliary diagnosis, and our method is more interpretable than 3D CNN methods, which can provide doctors with more auxiliary information.

Background Radiation exposure is inherently involved in endoscopic retrograde cholangiopancreatography (ERCP), which could cause radiation-induced injury to endoscopists with long-term exposure. Nonradiation ERCP has been applied to pregnant patients. Conceivably, the same techniques could be used to benefit endoscopists. This study was designed to evaluate the effectiveness and safety of nonradiation-to-endoscopist (NRE) ERCP, compared with standard ERCP. Methods A retrospective, single-center study was conducted from August 2010 to December 2015. Patients aged 18–90 years and with choledocholithiasis ( Results A total of 329 patients met inclusion criteria. After PSM, 73 patients were included in the NRE group and 146 in the standard group. The ERCP overall success rate for NRE and standard groups was equivalent (94.5% vs. 93.2%, P = 0.70). There was no difference in cannulation success rates between the two groups (95.6% vs. 97.8%, P = 0.39). A total of 88.3% of patients in the NRE group and 93.9% of patients in the standard group had stones cleared at initial ERCP (P = 0.57). No difference in overall stone clearance rate between the two groups (95.0% vs. 93.9%, P = 0.77) was found after second ERCP. The complication rate (1.4% vs. 1.4%, P = 1.00) and hospital duration (8.3 ± 5.1 vs. 10.2 ± 8.8 days, P = 0.07) were not different between the two groups. Conclusion Although technically demanding, NRE-ERCP is both safe and feasible in selected patients compared with standard ERCP.

Introduction Symptomatic juvenile idiopathic arthritis (sJIA) is an autoinflammatory disease, and monocytes/macrophages play an important role. However, which macrophage subtype plays a major role in different stages of sJIA is still unclear. This study aimed to explore macrophage subtypes in different stages of sJIA. Methods Twenty-two children with sJIA who were followed up at Shanghai Children’s Hospital from January 2018 to December 2020 were enrolled in this study. sJIA children were divided into an activity group (n = 12) and an inactivity group (n = 10). In the activity group, subjects with newly diagnosed sJIA and untreated were included; in the inactivity group, subjects with inactive sJIA meeting the 2011 ACR criteria for sJIA were recruited. Ten children with orthostatic proteinuria served as controls. Peripheral blood was collected. Flow cytometry was performed to detect macrophage subtypes: M1 (CD14+CD86+CD80+), M2a (CD14+CD206+CD301+), M2b (CD14+CD206+CD86+) and M2c (CD14+CD206+CD163+), and the contents of cytokines were also examined, including interleukins (IL) (IL-1β, IL-2, IL-4, IL-5, IL-6, IL-8, IL-10 and IL-17), interferon-α, interferon-γ, and tumor necrosis-α. Digital polymerase chain reaction (dPCR) has found widespread applications in molecular diagnosis of various diseases owing to its sensitive single-molecule detection capability. However, the existing dPCR platforms rely on the auxiliary procedure to disperse DNA samples, which needs complicated operation, expensive apparatus, and consumables. Besides, the complex and costly dPCR readers also impede the applications of dPCR for point-of-care testing (POCT). Herein, we developed a portable digital loop-mediated isothermal amplification (dLAMP) platform, integrating a microscale hydrogel (microgel) array chip for sample partition, a miniaturized heater for DNA amplification, and a hand-held reader for digital readout. In the platform, the chip with thousands of isolated microgels holds the capability of self-absorption and partition of DNA samples, thus avoiding auxiliary equipment and professional personnel operations. Using the integrated dLAMP platform, λDNA templates have been quantified with a good linear detection range of 2-1000 copies/μL and a detection limit of 1 copy/μL. As a demonstration, the epidermal growth factor receptor L858R gene mutation, a crucial factor for the susceptibility of the tyrosine kinase inhibitor in non-small-cell lung cancer treatment, has been accurately identified by the dLAMP platform with a spiked plasma sample. This work shows that the developed dLAMP platform provides a low-cost, facile, and user-friendly solution for the absolute quantification of DNA, showing great potential for the POCT of nucleic acids.

Improving carbon efficiency is an effective way to save energy and reduce harmful emission for a sintering process. Optimizing carbon efficiency is an effective way to achieve that goal, and its precondition is to assess the performance of the carbon efficiency. However, there is seldom research about how to assess the carbon efficiency whether it needs to be optimized. To address this, this paper introduces a performance assessment method for evaluating the performance of the carbon efficiency. First, the sintering process and the key characteristics are analyzed, and the carbon efficiency indexes are defined. Second, the structure of the assessment method is presented. The method consists of a prediction model based on three NNs, and an assessment method based on the fuzzy synthetic evaluation method. Two-level combination strategy is proposed to improve prediction performance and assessment accuracy, with the using of bootstrap aggregating, linear combination, and majority voting. Finally, verification based on process data shows that the proposed method can assess the performance of the carbon efficiency with high accuracy. More specially, the prediction errors of the combination model for the CCR are basically in the range of [-2.738 kg/t, 3.442 kg/t], and for the CO/CO 2 they are basically in the range of [-8.16 × 10−3, 4.828 × 10−3]. The combination models have better prediction performance than single NNs. Moreover, the assessment accuracy of the proposed method is 87%, which has higher accuracy than other models. This model lays the groundwork of improving the carbon efficiency for sintering process.

Obesity is one of major health burdens of modern society as it contributes to the growing prevalence of its related comorbidities, such as diabetes, cardiovascular diseases, and some cancers. A series of innate immune cells, especially macrophages, and adipocytes have been implicated in the pathogenesis of obesity. Mitochondrial dysfunction, which is induced by obesity, are critical mediators in initiating inflammation in macrophages and adipocytes, and subsequent systemic insulin resistance. In this review, we discuss new findings on how obesity impairs mitochondrial function in macrophages and adipocytes and how this dysfunction contributes to obesity and its comorbidities. We also summarize drugs that treat metabolic diseases by targeting mitochondrial dysfunction.

Background: This study aimed to evaluate the role of plasma microRNA panel (miR-122, miR-192, miR-21, miR-223, miR-26a, miR-27a and miR-801) for prediction and surveillance of early tumor recurrence in hepatocellular carcinoma (HCC) patients who had undergone liver transplantation (LT). Methods: The expression of plasma microRNA panel was assayed in 193 HCC patients (training cohort, n =151; validation cohort, n = 42). Sensitivity and specificity for detecting post-transplant HCC recurrence, and the relationship of microRNA panel expression with clinical characteristics were analyzed accordingly. The prognostic value of microRNA panel was compared with that of AFP (alpha-fetoprotein) and DCP (Des-gamma-carboxyprothrombin). Cox regression analyses were used to evaluate independent prognostic factors. Results: In the training cohort, the rate of positive plasma microRNA panel status at 7-14 days after LT (late phase; 44.2%) decreased than that before (76.2%, P < 0.001) and 1-6 days after LT (early phase; 78.5%, P < 0.001). At late phase, positive microRNA panel status correlated with higher early tumor recurrence rate (one year after LT) than negative status (45.9% vs 10.7%; P < 0.001). Patients with persistent positive microRNA panel status both before and after LT had the highest early tumor recurrence rate in this cohort (54.9%, P < 0.001). The results were consistent in the validation cohort. Cox regression analysis found that positive plasma microRNA panel status at late phase was the only independent risk factor for early recurrence (HR: 4.903, 95% CI = 2.195 - 10.951; P < 0.001). Dynamic monitoring demonstrated plasma microRNA panel status changed from negative to positive earlier than AFP and DCP upon recurrence, and the median time between positivity of plasma microRNA and imaging evidence of recurrence was 2.4 (0.5-10.0) months. Conclusions: Plasma microRNA panel could be a noninvasive biomarker for prediction and surveillance of early tumor recurrence in HCC patients who have undergone LT.

BACKGROUND Organophosphorus poisoning (OP) is one of the common critical conditions in emergency departments in China, which is usually caused by suicide by taking oral drugs. Patients with severe OP have disturbance of consciousness, respiratory failure, toxic shock, gastrointestinal dysfunction, and so on. As far as we know, the perforation of the duodenum caused by OP has not been reported yet. CASE SUMMARY A 33-year-old male patient suffered from acute severe OP, associated with abdominal pain. Multiple computed tomography scans of the upper abdomen showed no evidence of intestinal perforation. However, retrograde digital subtraction angiography, performed via an abdominal drainage tube, revealed duodenal perforation. After conservative treatment, the symptoms eased and the patient was discharged from hospital. CONCLUSION Clinicians should pay close attention to gastrointestinal dysfunction and abdominal signs in patients with severe OP. If clinical manifestation and vital signs cannot be explained by common complications, stress duodenal ulcer or perforation should be highly suspected.

Flexible strain sensors with high performance are actively and widely investigated for wearable electronic devices. However, the conventional sensors often suffer from a lack of detection of complex multidimensional strain, which severely limits their wide applications. To overcome this critical challenge, we propose a pattern design by screen printing to construct an asymmetrical cross-conductive network in the piezoresistive strain sensor, which can enhance the response to external stimuli in different directions. The unique network endows the prepared sensors with the excellent ability of instantaneous detection and accurate identification of multidimensional strains. Moreover, the sensor also demonstrates high sensitivity, fast response, an ultra-wide sensing range, and excellent stability and durability. Benefiting from the outstanding comprehensive performance of the prepared sensor, a full range of human actions (wink, smile, swallowing, and joint bending) and subtle bio-signals (pulse and breathing) are easily and accurately monitored. A wireless wearable device assembled by the sensor shows great potential applications in practical real-time physiological monitoring and intelligent mobile diagnosis for humans. This work provides an innovative and effective strategy for manufacturing flexible and multifunctional strain sensors to fully satisfy versatile applications of new-generation wearable electronic devices.

Semantic segmentation is crucial to the autonomous driving, as an accurate recognition and location of the surrounding scenes can be provided for the street scenes understanding task. Many existing segmentation networks usually fuse high‐level and low‐level features to boost segmentation performance. However, the simple fusion may impose a limited performance improvement because of the gap between high‐level and low‐level features. To alleviate this limitation, we respectively propose spatial aggregation and channel fusion to bridge the gap. Our implementation, inspired by the attention mechanism, consists of two steps: (1) Spatial aggregation relies on the proposed pyramid spatial context aggregation module to capture spatial similarities to enhance the spatial representation of high‐level features, which is more effective for the latter fusion. (2) Channel fusion relies on the proposed attention‐based channel fusion module to weight channel maps on different levels to enhance the fusion. In addition, the complete network with U‐shape structure is constructed. A series of ablation experiments are conducted to demonstrate the effectiveness of our designs, and the network achieves mIoU score of 81.4% on Cityscapes test dataset and 84.6% on PASCALVOC 2012 test dataset.

On-chip filters play an invaluable role in most detectors and instruments, blocking unwanted, harmonic, or mirrored signals. In this article, two coplanar waveguide bandpass filters working at 350 GHz are designed based on quarter-wavelength stepped impedance resonators, which can be easily integrated in on-chip systems. A second-order filter with a classical Chebyshev response and a fourth-order filter with quasi-elliptical characteristic is developed together to define the roll-off performance. Both filters are patterned based on superconducting niobium film by the optical lithography and lift-off process, which could reduce the significant metallic loss in high terahertz band. All the chips integrated with transitions are evaluated through a quasi-optical system, which is built based on the sensitive superconductor–insulator–superconductor tunnel junction detector at cryogenic temperature. The calibrated transmission responses, which are matched with simulations, have indicated that such superconducting coplanar waveguide filters can be enabled in future terahertz astronomical spectrometer.

The theory-practice gap (TPG) is well known in the environmental realm, referring to disconnects between knowledge generated through scientific research and the needs, expectations, and practices of knowledge users for environmental decision-making and practice. While the presence of the TPG is well established, we have yet to fully implement mechanisms for overcoming its challenges. Thus, our goal is to characterize the TPG and identify practical recommendations for minimizing it. Here, a diverse group of experts in the environmental realm (spanning landscape planning, conservation science, environmental sociology, resource management, political science, and anthropology, among others) present our perspectives on the TPG. More specifically, we share an organized framework for understanding the TPG and suggest recommendations that can help make progress in one or more dimension(s). Conceptual topics discussed are the implications of the gap and its persistence. Organizational/institutional topics include the implications of the overabundance, inaccessibility, and uncertainty of scientific information, and a need for mainstreaming boundary spanning activities. Lastly, cultural topics include differences in culture and epistemologies across knowledge generators and users, shifting cultures through co-production, and changes in educational curricula. Recommendations for minimizing the TPG include conceptually recasting what is considered ‘success,’ institutional reform, enhanced information delivery, leveraging knowledge brokers and boundary organizations, leveraging ‘champions’ in policy, using co-production and/or integrative research, confronting the contemporary ‘fake news’ phenomenon, and rethinking researcher and practitioner training and development. By sharing our framework and recommendations, we provide insight, as well as a starting point for those looking to narrow the TPG and improve knowledge generator-user relationships.

During the past years, multi-view clustering algorithms have demonstrated satisfactory clustering results by fusing the multiple views of the dataset. Nowadays, the researches of dimensionality reduction and learning discriminative features from multi-view data have soared in the literatures. As for clustering, generating the suitable subspace of the high dimensional multi-view data is crucial to boost the clustering performance. In addition, the relationship between the original data and the clusters still remains uncovered. In this article, we design a new multi-view low rank sparse representation method based on three-way clustering to tackle these challenges, which derive the common consensus low dimensional representation from the multi-view data and further proceed to get the relationship between the data items and clusters. Specifically, we accomplish this goal by taking advantage of the low-rank and the sparse factor on the data representation matrix. The $$L_{2,1}$$ norm is imposed on error matrix to reduce the impact of noise contained in the data. Finally, a new objective function is constructed to preserve the consistency between the views by using the low-rank sparse representation technique. The weighted low-rank matrix is utilized to build the consensus low rank matrix. Then, the whole objective function is optimized by using the Augmented Lagrange’s Multiplier algorithm. Further, to find the uncertain relationship between the data items and the clusters, we pursue the neighborhood based three-way clustering technique to reflect the data items into core and fringe regions. Experiments conducted on the real-world datasets show the superior performance of the proposed method compared with the state-of-the-art algorithms.

Doping heteroatoms and coupling cocatalysts are an effective way toward enhancing photocatalytic activity of semiconductor-based photocatalysts. Herein, Ni(II) modified P-doped g-C3N4 (NiPCN) was successfully prepared by an ion impregnation and phosphidation method. The Ni(II) as the electron cocatalyst and P-doping lead to 10 times of H2 evolution activity (1213 μmol h−1 g−1) compared with pristine g-C3N4 photocatalyst (117 μmol h−1 g−1). Detailed experimental results together with theoretical study revealed that the P-doping and Ni(II) as an electron cocatalyst can speed up electron transfer, leading to an enhancement of photocatalytic activity. The ultrafast transient absorption spectroscopy results disclosed that the synergistic effect of Ni(II) as an electron cocatalyst and P doping helps to increase the carrier's lifetime, resulting in the efficient electron transfer and long-lived charge separation. It is believed that this work opens up a possible way for optimizing the separation of photoexcited carriers for photocatalytic H2-evolution. BACKGROUND Approximately 90% of new cases of noncardiac gastric cancer (GC) are related to Helicobacter pylori (H. pylori), and cytotoxin-associated gene A (CagA) is one of the main pathogenic factors. Recent studies have shown that the pharmacological effects of cryptotanshinone (CTS) can be used to treat a variety of tumors. However, the effects of CTS on H. pylori, especially CagA+ strain-induced gastric mucosal lesions, on the development of GC is unknown. AIM To assess the role of CTS in CagA-induced proliferation and metastasis of GC cells, and determine if CagA+ H. pylori strains causes pathological changes in the gastric mucosa of mice. METHODS The effects of CTS on the proliferation of GC cells were assessed using the Cell Counting Kit-8 (CCK-8) assay, and the abnormal growth, migration and invasion caused by CagA were detected by CCK-8 and transwell assays. After transfection with pSR-HA-CagA and treatment with CTS, proliferation and metastasis were evaluated by CCK-8 and transwell assays, respectively, and the expression of Src homology 2 (SH2) domain–containing phosphatase 2 (SHP2) and phosphorylated SHP2 (p-SHP2) was detected using western blotting in AGS cells. The enzyme-linked immunosorbent assay was used to determine the immunoglobulin G (IgG) level against CagA in patient serum. Mice were divided into four groups and administered H. pylori strains (CagA+ or CagA-) and CTS (or PBS) intragastrically, and establishment of the chronic infection model was verified using polymerase chain reaction and sequencing of isolated strains. Hematoxylin and eosin staining was used to assess mucosal erosion in the stomach and toxicity to the liver and kidney. RESULTS CTS inhibited the growth of GC cells in dose- and time-dependent manners. Overexpression of CagA promoted the growth, migration, and invasion of GC cells. Importantly, we demonstrated that CTS significantly inhibited the CagA-induced abnormal proliferation, migration, and invasion of GC cells. Moreover, the expression of p-SHP2 protein in tumor tissue was related to the expression of IgG against CagA in the serum of GC patients. Additionally, CTS suppressed the protein expression levels of both SHP2 and p-SHP2 in GC cells. CTS suppressed CagA+ H. pylori strain-induced mucosal erosion in the stomach of mice but had no obvious effects on the CagA- H. pylori strain group. CONCLUSION CTS inhibited CagA-induced proliferation and the epithelial-mesenchymal transition of GC cells in vitro, and CagA+ H. pylori strains caused mucosal erosions of the stomach in vivo by decreasing the protein expression of SHP2.

In practical applications, electromagnetic measurements often contain damaged data due to malfunctioning receivers, which can severely influence the inversion performance. Thus, in this letter, a new receiver approximation machine learning (RAML) method is proposed to repair the data from the damaged receivers and the repaired data are the input to the dual-module nonlinear mapping module–image enhancing module machine learning scheme for the 2-D inverse scattering problem. The RAML method has been tested for the inversion of both measured transverse electric and transverse magnetic datasets provided by Institute Fresnel. Excellent inversion performance has been obtained when 10 out of 241 (4.15%) receivers are damaged. Furthermore, in an extreme situation, the RAML method can also have good inversion performance even though half of the receivers are damaged.

Objectives Tuberculosis (TB), an infectious disease caused by Mycobacterium tuberculosis (MTB), has similar clinical, radiological, and histopathological characteristics to sarcoidosis (SA). Accurately distinguishing SA from TB remains a clinical challenge. Methods A total of 44 TB patients and 47 SA patients who were clinically diagnosed using chest radiography, pathological examination, routine smear microscopy, and microbial culture were enrolled in this study. The MTB genome was captured and sequenced directly from tissue specimens obtained upon operation or biopsy, and the feasibility of next-generation sequencing (NGS) for the MTB genome in the differential diagnosis of TB from SA was evaluated. Results Using a depth >10× and coverage >15% of the sequencing data, TB patients were identified via the NGS approach directly using operation or biopsy specimens without clinical pretreatment. The sensitivity, specificity, and concordance of the NGS method were 81.8% (36/44), 95.7% (45/47), and 89.0% (81/91), respectively (kappa = 0.78, 95% confidence interval 0.65–0.91; P Conclusions This study established an improved NGS strategy for rapidly distinguishing patients with TB from those with SA and has potential clinical benefits.

Objective. To explore the potential mechanism of Huanglian Jiedu Decoction (HJD) treatment and prevention of metastatic Cutaneous Melanoma (CM) occurrence and metastasis based on network pharmacological methods and immune infiltration analysis. Methods. The GEO database was used to obtain metastatic CM disease targets, the TCMSP database and the HERB database were used to obtain HJD action targets, core genes were screened by protein interaction network, and the potential mechanism of HJD in the treatment of metastatic CM was explored by enrichment analysis, prognostic analysis and immune infiltration analysis. Results. HJD treatment of metastatic CM involved 60 targets, enrichment analysis showed that HJD treatment of metastatic CM involved Chemokine signaling pathway, NF-kappa B signaling pathway, and Fluid shear stress and atherosclerosis, etc. Prognostic analysis revealed that HJD had a certain ability to improve the prognosis of metastatic CM patients. Immune infiltration analysis showed that HJD could inhibit the immune cell infiltration of metastatic CM patients by acting on related targets. Conclusions. Our study identified the potential mechanism of HJD in the treatment of metastatic CM through network pharmacology, and revealed the mechanism of HJD in the prevention of Skin Cutaneous Melanoma metastasis through immune infiltration analysis and prognostic analysis.

Biological inspiration evaluation has been widely acknowledged as one of the most important phases in biologically inspired design (BID) as it substantially determines the direction of the following-up design activities. However, it is inherently an interdisciplinary assessment, which includes both the engineering domain and the biological systems. Due to the lack of knowledge at the early stage of product design, the risk assessments mainly depend on experts' subjective judgments, which values are vague, imprecise, and even inconsistent. How to objectively evaluate the biological inspiration under such uncertain and interdisciplinary scenarios remains an open issue. To bridge such gaps, this study proposes a fuzzy rough number extended multi-criteria group decision-making (MCGDM) to evaluate the biological inspiration for BID. A fuzzy rough number is introduced to represent the individual decision maker's risk assessment and aggregate respective evaluation values within the decision-making group. A fuzzy rough number extended decision-making trial and evaluation laboratory is presented to determine the criteria weights and a fuzzy rough number extended multi-attribute ideal real comparative analysis is proposed to rank the candidate biological inspirations. Experimental results and comparative analysis validate the superiority of the proposed MCGDM in handling the subjectivity and uncertainty in biological inspiration evaluation. Nevertheless, the prognostic value of PDW in gallbladder carcinoma (GBC) remains unknown. We aimed to explore the correlation between PDW and prognosis in patients with GBC.Methods: A total of 303 patients with GBC who underwent curative surgery between January 2005 and February 2017 were enrolled. The relationship between PDW and clinicopathological features was analyzed. Receiver operating characteristic (ROC) curve was used to identify the optimal cutoff value of PDW. The overall survival (OS) rate was estimated by Kaplan–Meier method. Meanwhile, univariable and multivariable Cox regression model were used to evaluate the risk factors for OS.Results: There was significant correlation between elevated PDW and AJCC stage. In addition, survival analysis revealed that the patients with PDW> 14.95 have a worse prognosis than patients with PDW 14.95 (P < 0.001). The multivariable Cox regression model analysis demonstrated that PDW was an independent prognostic factor in GBC patients (hazard ratio=1.976, 95% confidence interval:1.474– 2.650, P< 0.001).Conclusion: Elevated PDW can predict poor prognosis in GBC patients, and further studies are needed to verify the reliability and clarify the exact molecular mechanistic of PDW in GBC.Keywords: gallbladder carcinoma, platelet distribution width, prognosis

Objective: Combining ultrasound parameters with patient’s clinical symptoms to explore the feasibility of improving the diagnostic coincidence rate of hyperthyroidism.Methods: A total of 179 untreated patients with diffuse echogenic changes of the thyroid on ultrasound and abnormal laboratory examinations were enrolled this study. There were 119 cases of hyperthyroidism, 29 cases of subclinical hyperthyroidism, 26 cases of subclinical hypothyroidism, and 5 cases of Hashimoto’s thyroiditis with clinically confirmed diagnosis. The thyroid volume, blood supply grade, peak velocity of the superior thyroid artery (STA) by ultrasound, heart rate (HR), and clinical symptoms was used to determine the optimal indicators for the diagnosis of hyperthyroidism, and to compare these indicators (study group) with the previous ultrasound criteria reported in the literature (control group).Results: According to logistic regression analysis, from all the factors studied, hyperthyroidism symptoms had the most significant correlation with the diagnosis of hyperthyroidism (P = 0.001), followed by STA peak velocity (P = 0.005), HR (P = 0.009), echogenicity(P = 0.015)and blood flow Grade III (P = 0.041) in order of importance, the other factors include thyroid volume, blood flow Grade I and II were not correlated with the diagnosis of hyperthyroidism (P > 0.05). The diagnostic coincidence rate of hyperthyroidism by increased STA peak velocity was 85.7% (102/119), while the hyperthyroidism symptoms significantly increased the diagnostic coincidence rate to 95.8% (114/119). Comparing the study group to the control group, the area under the receiver operating curve (ROC) was 0.993 vs 0.899 (Z = 3.868, P = 0.001). The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value were 89.92% vs. 68.91%, 96.67% vs. 96.67%, 92.18% vs. 78.21%, 98.17% vs. 97.62%, and 82.80% vs. 61.05%, respectively.Conclusions: The optimal indicators for the diagnosis of hyperthyroidism is the presence of hyperthyroidism symptoms in untreated patients with diffuse echogenic changes of the thyroid, followed by increased STA peak velocity. This suggests that ultrasound doctors should carefully considerate clinical information and instead of just limited to ultrasound itself. By combining ultrasound parameters with the clinical manifestations of patients, the diagnostic coincidence rate can be truly improved.

Lung cancer is one of the most prevalent and lethal cancers worldwide (1). The traditional treatments include surgery, chemotherapy, radiotherapy and interventional therapy. The survival of lung cancer patients has dramatically been prolonged in recent years with the availability of targeted therapies, antiangiogenic agents and immune checkpoint inhibitors (ICIs). Meanwhile, technologies for the molecular detection of lung cancer have also advanced rapidly: the detection of single driver genes has evolved to cover combined multi-gene analysis, and whole exome sequencing (WES) has increasingly been applied in the clinical setting. In addition, life support technologies, including ventilators, artificial liver, and artificial kidney as well as extracorporeal membrane oxygenation (ECMO), have further matured, providing powerful forms of life support for patients with various acute and critical diseases. However, most clinical studies have only enrolled patients with Eastern Cooperative Oncology Group (ECOG) performance status (PS) scores from 0 to 1, with few patients having PS scores of 2; patients with a PS score of 3 or 4 have been typically excluded. Therefore, due to the lack of high-quality evidence, supportive care is recommended for patients with a PS score of 3 to 4 in the current guidelines. In the real-world, however, approximately 25% of lung cancer patients present with PS score of 3 or 4 (2) or attain scores between 3 and 4 during the course of treatment. Certain patients with high PS scores can benefit from individualized anti-tumor treatment plus appropriate life-support techniques. In 2017, the Lung Cancer Research Team at the First Affiliated Hospital of Guangzhou Medical College & Institute of Respiratory Diseases for the first time pioneered the concept of “advanced severe lung cancer” (3) and argued that standardized therapy for chronic obstructive pulmonary disease (COPD) plus anti-tumor therapy can improve both quality of life and prognosis in patients with lung cancer combined with COPD. The authors also found that the early detection of lung cancer driver genes and timely targeted therapy can be successful in treating patients with advanced severe lung adenocarcinoma with a PS score of 4 (4). In 2019, the concept of advanced severe lung cancer was further developed in a featured article (5) that indicated “Advanced severe lung cancer: does not refer to end-stage lung cancer but rather to stage IIIB, IIIC and IV lung cancers with a PS score of 2–4, which can result from a variety of factors related to the disease itself or anti-tumor drugs and which are highly likely to benefit from the currently available systemic anti-tumor therapies”. In recent years, with the advances in lung cancer diagnosis and treatment techniques and life support technologies, more clinical studies have enrolled patients with a PS score of 2, and some real-world studies have enrolled patients with PS scores of 3–4. Even for patients with early-stage lung cancer, studies have shown that patients with poor PS scores and co-morbidities have a reduced chance of undergoing surgery and an increased mortality rate (6); nevertheless, survival benefit may still be obtained through surgical modifications combined with individualized and multidisciplinary treatment (7). Therefore, the concept of severe lung cancer should not be limited to advanced lung cancer, but applied to all lung cancer patients. In particular, due to the increase in treatment options as well as substantially prolonged survival, the majority of patients may have a PS score between 2 and 4 for a certain period of time due to a variety of reasons. How to provide timely and reasonable treatment for these lung cancer patients has become a critically important real-world research topic. Therefore, we invited lung cancer experts at home and abroad to consider this issue, and this group has reached the following consensus.

Background Interference screw is commonly used for graft fixation in anterior cruciate ligament (ACL) reconstruction. However, previous studies had reported that the insertion of interference screws significantly caused graft laceration. The purposes of this study were to (1) quantitatively evaluate the graft laceration from one single insertion of PEEK interference screws; and (2) determine whether different types of sutures reduced the graft laceration after one single insertion of interference screws in ACL reconstruction. Methods The in-vitro ACL reconstruction model was created using porcine tibias and bovine extensor digitorum tendons of bovine hind limbs. The ends of grafts were sutured using three different sutures, including the bioabsorbable, Ethibond and ultra-high molecular weight polyethylene (UHMWPE) sutures. Poly-ether-ether-ketone (PEEK) interference screws were used for tibial fixation. This study was divided into five groups (n = 10 in each group): the non-fixed group, the non-sutured group, the absorbable suture group, the Ethibond suture group and the UHMWPE suture group. Biomechanical tests were performed using the mode of pull-to-failure loading tests at 10 mm/min. Tensile stiffness (newtons per millimeter), energy absorbed to failure (in joules) and ultimate load (newtons) were recorded for analysis. Results All prepared tendons and bone specimens showed similar characteristics (length, weight, and pre-tension of the tendons, tibial bone mineral density) among all groups (P > 0.05). The biomechanical tests demonstrated that PEEK interference screws significantly caused the graft laceration (P < 0.05). However, all sutures (the bioabsorbable, Ethibond and UHMWPE sutures) did not reduce the graft laceration in ACL reconstruction (P > 0.05). Conclusions Our biomechanical study suggested that the ultimate failure load of grafts was reduced of approximately 25 % after one single insertion of a PEEK interference screw in ACL reconstruction. Suturing the ends of the grafts using different sutures (absorbable, Ethibond and UHMWPE sutures) did not decrease the graft laceration caused by interference screws.

Hispanic American women have an increased risk for the development of cardiovascular disease (CVD). This study examined CVD risk in a sample of immigrant Hispanic women using Framingham point scores and the Atherosclerotic Cardiovascular Disease (ASCVD) Risk Estimator for 10-year CVD risk and prevalence of metabolic syndrome. A cross-sectional study using baseline data from a diabetes self-management intervention study in a sample of 118 Hispanic American women was conducted. Data were c ollected with interviewer-administered surveys, and HbA1C and lipid profiles were obtained through capillary finger stick blood at clinics and churches in rural counties in central North Carolina. A majority (58%) of the women had type 2 diabetes and 61% had metabolic syndrome. Using the Framingham point scores for 10-year CVD risk, 2.5% were determined to have intermediate risk, compared to 7.6% at intermediate risk and 2.5% at high risk using the ASCVD 10-year risk estimator. High rates of CVD risk factors were found among this sample of Hispanic women. Early recognition of risk, mitigation of modifiable risk factors, and effective self-care programs are needed for Hispanic women. Hispanic American women may benefit from community-based CVD educational programs that have been culturally and linguistically tailored.

Angiomyolipoma (AML) is apparent a part of mesenchymal tumor arising from perivascular epithelioid cell origin. AML is formed of blood vessels, spindle and epithelioid cells, and mature adipocytes. Adrenal AML is very rare. Indeed, only 21 cases of adrenal AML have been reported in the English-language literature. In these cases, patients were usually asymptomatic, and AML was often examined passingly during a health checkup or a common abdominal ultrasound (US) or a computed tomography (CT) examination for a other unrelated disease. This is a case report of adrenal AML in a 40-year-old male who presented with no clinical manifestations. Some 3 months ago, a first abdominal routine CT scan revealed a left adrenal mass. Three months later, a routine US showed a hypoechoic area of 21 mm × 15 mm in the left adrenal gland. A retroperitoneoscopic resection of the left adrenal gland tumor was then performed. A postoperative histopathology report confirmed a benign AML composed of blood vessels, smooth muscle, and mature adipose tissue. The patient was discharged without event. Due to the paucity of the literature on this subject, more clinical and imaging information about this disease is necessary for diagnosis, differential diagnosis, and treatment. In this paper, we present a complete case report, including the clinical presentation, imaging, treatment, and conduct a review of the relevant literature.

The use of laparoscopic liver resection for curative surgery of intrahepatic cholangiocarcinoma (ICC) is not well established. Herein, we perform a meta-analysis to compare the differences between laparoscopic liver resection (LLR) and open liver resection (OLR) for ICC.Multiple electronic databases were searched and 8 relevant studies containing 552 patients treated by LLR and 2320 treated by OLR were identified. The fixed effects and a random-effects model were used to perform a meta-analysis.Compared with OLR, LLR for ICC was associated with less blood transfusion (7.14% versus 17.11%; OR: 0.32; 95% CI 0.15 to 0.71; P 0.05), higher R0 resection (85.63% versus 74.69%; OR: 1.48; 95% CI 1.13 to 1.95; P 0.05), shorter length of stay (LOS) (SMD: -0.40; 95% CI -0.80 to 0.00; P = 0.05), less overall morbidities (20% versus 32.69%; OR: 0.50; 95% CI 0.33 to 0.78; P 0.05), and less death due to tumor recurrence (22.39% versus 35.48%; OR: 0.50; 95% CI 0.29 to 0.86; P0.05); but LLR was associated with smaller ICC, fewer major hepatectomies, less lymph node (LN) dissection rate, and inferior 5-year overall survival (OS) (P 0.05). Duration of operation, blood loss, average LN retrieved, LN metastasis, major morbidities, mortality, tumor recurrence, 3-year OS and disease free survival (DFS), and 5-year DFS were comparable (P0.05).LLR for ICC is in the initial phase of exploration. More evidence is necessary to validate LLR for ICC.

Background Angiopoietin-like proteins (ANGPTLs) are closely related to insulin resistance and lipid metabolism, and may be a key in metabolic syndrome. Non-alcoholic fatty liver disease (NAFLD) (newly named metabolic-associated fatty liver disease (MAFLD)) is based on metabolic dysfunction. There may be some correlation between ANGPTLs and MAFLD, but the specific correlation is unclear. This study aims to explore the predictive role of ANGPTLs in MAFLD and its progression. Methods Seven databases (PubMed, EMBASE, Cochrane Library, CNKI, WANFANG, CBM and Clinicaltrials.gov) were searched with free terms and MeSH terms. The random-effects model was used to pool the data, and Standardized Mean Difference (SMD) and 95% confidence intervals (CI) were taken as the overall outcome. No language restrictions existed in the article selection. RevMan 5.3, Stata 16 and MetaXL software were applied to analyse the data and the GRADE system was utilized to assess the certainty of evidence. Results After reviewing 823 related articles, 13 studies (854 cases and 610 controls) met the inclusion criteria, and contributed to this meta-analysis. The results showed that circulating ANGPTL8 level was significantly elevated in the MAFLD group than in the healthy control group (SMD = 0.97 pg/mL, 95%CI: 0.77, 1.18). Conversely, there was no significant difference in the ANGPTL4 (SMD = 0.11 ng/mL, 95%CI: − 0.32, 0.54) and ANGPTL3 (SMD = − 0.95 ng/mL, 95%CI: − 4.38, 2.48) between the two groups. Subgroup analysis showed that: 1) the MAFLD group had significantly higher ANGPTL8 levels than the healthy control group in Asian and other races; 2) the ANGPTL8 levels in Body Mass Index (BMI) > 25 kg/m2 patients with MAFLD were higher than those in the healthy control group; 3) the higher ANGPTL8 levels were observed in moderate to severe MAFLD group than the healthy control group. Meta-regression demonstrated that BMI might effectively explain the high heterogeneity. No significant publication bias existed (P > 0.05). The certainty of evidence was assessed as very low by the GRADE system. Conclusions The ANGPTLs may be related to MAFLD. The increased ANGPTL8 level may be positively correlated with different situations of MAFLD, which may act as a potential indicator to monitor the development trends.

Relying on the propagation of modulated radio-frequency (RF) signals, we can achieve simultaneous wireless information and power transfer (SWIPT) to support low-power communication devices. In this paper, we proposed a unary coding based SWIPT encoder by considering a practical M-QAM. Markov chains are exploited for characterising coherent binary information source and for modelling the generation process of modulated symbols. Therefore, both mutual information and the average energy harvesting performance at the SWIPT receiver are analysed in semi-closed-form. With the aid of the genetic algorithm, the sub-optimal codeword distribution of the coded information source is obtained by maximising the average energy harvesting performance, while satisfying the requirement of the mutual information. Simulation results demonstrate the advantage of the SWIPT encoder. Moreover, a higher-level unary code and a lower-order M-QAM results in higher WPT performance, when the maximum transmit power of the modulated symbol is fixed.

OBJECTIVES: To explore the scope of dental and oral health care (DOHC) provided by the pharmacy profession and associated outcomes, and attitudes of pharmacy staff and other key stakeholders about pharmacy involvement in this context. METHODS: A scoping review of full-text articles describing outcomes related to pharmacy staff/student involvement in, knowledge of or attitudes towards DOHC in any setting, and stakeholder perspectives was conducted. Articles written in English were searched for using PubMed, EMBASE, EBSCO, International Pharmaceutical Abstracts, Education Resources Information Centre, Web of Science, Google Scholar, and ProQuest for Dissertations and Theses. No date restrictions were used. Key outcomes were mapped: role and practice, knowledge and attitudes, and training and education. RESULTS: Seventy studies met the inclusion criteria: 49 were conducted in developed countries; 60 were quantitative in design; and 38 involved community pharmacy settings only. Pharmacists and pharmacy support staff commonly managed DOHC inquiries (n = 13), including the provision of advice and products with or without a further referral. Integrated pharmacist-led services in dental settings (n = 4) showed improved prescribing and quality use of medicines but low community pharmacy referrals were identified in studies involving mystery shoppers with potential oral cancer (n = 7). DOHC promotion programmes delivered by pharmacy staff and collaborations with dentists were limited. There was interest from the pharmacy profession for a role in DOHC; however, knowledge gaps were reported and needs for further training identified. Consumer participant DOHC outcomes were not reported. CONCLUSIONS: Community pharmacists and pharmacy staff were interested in an expanded role in DOHC; however, this scoping review identified lack of knowledge and sub-optimal practice as potential barriers. Research on development, implementation and evaluation of DOHC-related services and practice by pharmacy staff was scarce. Further evidence of consumer-related oral health outcomes and their perceptions of the role of pharmacy is needed.

Chronic internal carotid artery occlusion (CICAO) significantly increases the risk of recurrent stroke. Given unfavorable outcomes, revascularization procedures are not generally recommended for CICAO. In the last several years, loads of studies reported successful surgical revascularization for CICAO with promising success rate and favorable short-term outcomes. Meanwhile, due to the lack of high-quality evidence, the safety and efficacy of revascularization procedures remain debatable. This systematic review aims to scrutinize current evidence for the applicability of revascularization for CICAO. We also investigated potential predictors of postoperative prognosis. We searched clinical studies on surgical treatment of CICAO on the Medline, Cochrane library, and Embase databases, published from Jan 1990 to Jan 2021. Surgical operation was restricted to bypass surgery, endarterectomy, endovascular therapy, and hybrid surgery. Controlled clinical studies were included for clinical outcomes. Large-sample single-arm studies were supplemented to assess complications and success rate. Co-primary endpoints were technical success rate and neurological function; secondary endpoints were recurrent stroke/cerebrovascular events, complications, and deaths within follow-up. This systematic review has been registered in PROSPERO (CRD42020181250). One RCT and 5 cohort studies with a total of 465 patients were included in this review. Seven single-arm studies were supplemented for assessing success rate and complications. Bypass surgery presented the highest graft patency of 96% and a low incidence of complications, but no benefits on neurological function, recurrent stroke, or deaths. Endovascular therapy (carotid stenting) was characterized by a relatively lower technical success rate, significant neurological function recovery, and nonsignificant reduction of cerebrovascular events and deaths. Hybrid surgery was considered as a potential treatment for CICAO because of a high technical success rate and significant neurological improvement. Endarterectomy is only suitable for short-segment occlusion. Prospective clinical trials should focus on carotid stenting and hybrid surgery for their significant capacity of improving neurologic function and potential capacity of reducing deaths and cerebrovascular events.

Clustering large and high-dimensional document data has got a great interest. However, current clustering algorithms lack efficient representation learning. Implementing deep learning techniques in document clustering can strengthen the learning processes. In this work, we simultaneously disentangle the problem of learned representation by preserving important information from the initial data while pushing the original samples and their augmentations together in one hand. Furthermore, we handle the cluster locality preservation issue by pushing neighboring data points together. To that end, we first introduce Contractive Autoencoders. Then we propose a deep embedding clustering framework based on contractive autoencoder (DECCA) to learn document representations. Furthermore, to grasp relevant document or word features, we append the Frobenius norm as penalty term to the conventional autoencoder framework, which helps the autoencoder to perform better. In this way, the contractive autoencoders apprehend the local manifold structure of the input data and compete with the representations learned by existing methods. Finally, we confirm the supremacy of our proposed algorithm over the state-of-the-art results on six real-world images and text datasets.

Background Anomalous origin of one pulmonary artery from the ascending aorta (AOPA) is a rare and potentially deadly anomaly. Little research, aside from case reports on APOA, has been published, especially for patients with late referrals. Methods This study was a retrospective review of 57 patients with AOPA who underwent reimplantation of the pulmonary artery (PA) from 2009 to 2019. Two different reimplantation methods were used to correct the anomaly, including direct anastomosis in 36 patients and angioplasty with autologous tissue in 21 patients. Results The median age at repair was 2.8 months (range, 8 days to 3.6 years). In-hospital death occurred in 2 patients (3.5%). Five patients (9.1%) with a median age of 9.3 months (range, 5.2 months to 3.6 years) experienced a pulmonary hypertensive crisis. Patients older than 4.9 months were more likely to have a pulmonary hypertensive crisis (P = .001). The 2-year freedom from postoperative PA stenosis rate was 75.3% in patients who underwent direct anastomosis and 46.8% in patients who underwent autologous tissue angioplasty (χ2 = 4.878; P = .027). Angioplasty with autologous tissue (hazard ratio, 5.03; 95% confidence interval, 1.61 to 15.71; P = .005) and an innately smaller diameter of the aberrant PA (hazard ratio, 0.65; 95% confidence interval, 0.45 to 0.92; P = .015) were 2 independent risk factors for postoperative PA stenosis. Conclusions Surgical reimplantation of the PA in patients with AOPA has resulted in favorable early and midterm outcomes. Pulmonary hypertensive crisis occurs more commonly in patients who receive a diagnosis after the age of 4.9 months. Reimplantation with autologous tissue augmentation and an intrinsically smaller diameter in affected PAs are 2 independent risk factors for postoperative PA stenosis.

During the drilling process, the data quality influences the reliability of condition monitoring results. However, the actual drilling data may encounter various kinds of abnormal data, and different kinds of them need to be handled differently. The abnormal data caused by external factors such as sensor failure, storage errors, etc., should be corrected, while the abnormal data caused by drilling accidents should be protected. Therefore, not only the anomaly detection is needed, but the causes of anomalies should be further discriminated. This paper proposes a method for discrimination and correction of abnormal data in the drilling process. First, the local outlier factor anomaly detection algorithm is developed to detect all kinds of the abnormal data. Then, the dynamic time warping and fuzzy c-means are combined for the discrimination of causes of anomalies. Finally, the discriminated abnormal data caused by external factors are corrected with the k nearest neighbor interpolation. Simulation results involving actual data illustrate that the causes of anomalies can be discriminated effectively, and the monitoring results of monitoring models based on neural network improve after using the proposed method, which verify the necessity of anomaly discrimination.

Background: Respiratory tract infection (RTIs) is one of common diseases among the children. In recent years, the incidence of mycoplasma pneumoniae (M. pneumoniae) infection rate has been increasing, which cause respiratory tract infection. This study sought to analyze the epidemiological characteristics of M. pneumoniae hospitalized children with RTIs to provide a theoretical basis for clinical diagnosis and treatments in Chengdu, China. Methods: The data of 22,882 cases of children who had been hospitalized for RTIs were collected. M. pneumoniae immunoglobulin M (IgM) antibody was detected using the indirect immunofluorescence method and passive agglutination method. The demographic features of patients, clinical diagnoses and laboratory data were also analyzed. Results: A total of 4,213 children tested positive for M. pneumoniae. The total positive rate was 18.41% (18.30% for males and 22.72% for females). Female children had statistically significant higher positive rates than male children (chi2=198.078, P 0.05). Numerous works implemented multi-view clustering algorithms in document clustering. A challenging problem in document clustering is the similarity metric. Existing multi-view document clustering methods broadly utilized two measurements: the Cosine similarity (CS) and the Euclidean distance (ED). The first did not consider the magnitude difference (MD) between the two vectors. The second can’t register the divergence of two vectors that offer a similar ED. In this paper, we originally created five models of similarity metric. This methodology foils the downside of the CS and ED similarity metrics by figuring the divergence between documents with the same ED while thinking about their sizes. Furthermore, we proposed our multi-view document clustering plan which dependent on the proposed similarity metric. Firstly, CS, ED, triangle’s area similarity and sector’s area similarity metric, and our five similarity metrics have been applied to every view of a dataset to generate a corresponding similarity matrix. Afterward, we ran clustering algorithms on these similarity matrices to evaluate the performance of single view. Later, we aggregated these similarity matrices to obtain a unified similarity matrix and apply spectral clustering algorithm on it to generate the final clusters. The experimental results show that the proposed similarity functions can gauge the similitude between documents more accurately than the existing metrics, and the proposed clustering scheme surpasses considerably up-to-date algorithms.
