Background The aim of this multicenter, randomized phase II study was to analyze the feasibility and safety of alternate-day S-1, an oral fluoropyrimidine, for adjuvant chemotherapy in patients with completely resected pathological stage I (tumor diameter > 2 cm) non-small cell lung cancer (NSCLC). Methods Patients were randomly assigned to receive adjuvant chemotherapy for 1 year comprising either alternate-day oral administration of S-1 (80 mg/m2/day) for 4 days a week (Group A) or a 2-week oral administration of S-1 (80 mg/m2/day) followed by 1 week of rest (Group B). The primary endpoint was feasibility, which was defined as the proportion of patients who completed the allocated intervention for 6 months with a relative dose intensity (RDI) of 70% or more. Results Ninety-three patients were enrolled of whom 90 patients received S-1 treatment. Median follow-up was 66.9 months. The treatment completion rate based on an RDI of 70% or more for 6 months was 84.4% (95%CI; 70.5–93.5%) in group A and 64.4% (95%CI; 48.8–78.1%) in group B. There were no grade 4 adverse events in either group. Moderate or severe adverse events (grade 2 or grade 3) were significantly more frequent in group B (67%) compared with group A (29%, P = 0.001). The 5-year relapse-free survival rate was 87.0 and 80.9% for group A and B, respectively (P = 0.451). The 5-year overall survival rate for all patients (n = 93) was 100 and 89.4% for group A and B, respectively (P = 0.136). Conclusion Alternate-day oral administration of S-1 for 1 year as adjuvant chemotherapy was demonstrated to be feasible with low toxicity in completely resected stage I (tumor diameter > 2 cm) NSCLC. Background Ureteroarterial fistula is a rare life-threatening complication of indwelling ureteral stents. The mechanism has not yet been fully evaluated using intravascular imaging. Case presentation An-84-year-old female was referred to our unit because of large volume pulsatile bleeding from the left ureter during routine stent exchange in the urology department. The hematuria was initially managed by rapidly exchanging for a new stent; however, the patient went into hypovolemic shock due to acute blood loss. The patient underwent implantation of the bilateral ureteral stents due to urinary retention caused by retroperitoneal fibrosis 2 years ago. To prevent ureteral infection, occlusion of the stents and stone formation, the stents were exchanged every 6 months. Computed tomography revealed contact between the left ureter and the common iliac artery. Therefore, ureteroarterial fistula was suspected and endovascular therapy was performed. Although angiography did not show definite blood flow into the ureter, a soft guidewire was advanced from the subintima of the external iliac artery to the left ureter. The diagnosis of ureteroarterial fistula was confirmed. Intravascular ultrasound identified the stent in the ureter and its connection to the subintima of the external iliac artery. The ureter did not contact directly to the inner lumen of the iliac arteries according to the ultrasound findings; therefore, we considered that the risk of stent-graft infection might not be high. After coil embolization of the ipsilateral internal iliac artery, a covered stent was implanted in the external iliac artery to seal the subintimal entry. The patient had no further episodes of any gross hematuria on dual anti-platelet therapy, when the ureteral stent was exchanged three time during 1 year after the endovascular therapy. Conclusions We demonstrated a case of ureteroarterial fistula, in which intravascular ultrasound allowed to visualize the communication between the ureter and the subintimal lumen in the external iliac artery. Background: Patients with squamous cell carcinoma (SqCC) of the lung sometimes have a comorbid pulmonary disease such as pulmonary emphysema or an interstitial lung disease (ILD), both of which negatively affect patient outcome. The aim of this study was to determine the outcome of patients in a multicenter database who underwent surgery for cT1aN0M0 peripheral SqCC lung cancer. Methods: The medical records of a total of 228 eligible patients from seven institutions were reviewed to evaluate the impact of concomitant impaired pulmonary function and other clinicopathological factors on overall survival (OS) and relapse-free survival (RFS). Results: Six patients with positive or unclear tumor margins were excluded. Of the 222 remaining study patients, 42 (18.9%) and 97 (43.7%) patients were found to have coexisting restrictive or obstructive ventilatory impairment, respectively. Over a median follow-up period of 30.6 months, the 5-year OS and RFS were 69.0% and 62.6%, respectively. By multivariate analysis, ILDs identified on high-resolution computed tomography (HRCT), pulmonary function test results indicating a restrictive ventilatory impairment, and wedge resection were found to be independent risk factors for poor OS. An increased level of serum squamous cell carcinoma antigen (SCC-Ag) (>1.5 ng/mL) and the same risk factors for poor OS were independent risk factors for recurrence. Among patients who underwent anatomical lung resection (lobectomy and segmentectomy, n=173), a restrictive ventilatory impairment was an independent risk factor for poor OS, and increased serum SCC-Ag level, ILDs on HRCT, and restrictive ventilatory impairment were independent risk factors for poor RFS by multivariate analysis. Factors such as visceral pleural invasion, and lymphatic or vascular invasion were not significantly associated with outcome. Conclusions: A restrictive ventilatory impairment negatively affects the outcome of patients with cT1aN0M0 peripheral SqCC lung cancer.

背景．縦隔リンパ節転移を発見契機とした甲状腺癌の報告は少ない．症例．77歳男性．検診で右上葉に腫瘤が認められた．PETで肺腫瘤にSUVmax 8.2の集積を認めた．高位縦隔リンパ節2個にも異常集積を認めた．胸骨正中切開下に，右上葉切除+縦隔リンパ節郭清を施行した．混合型小細胞癌（小細胞癌+腺癌）と診断された．縦隔リンパ節は甲状腺乳頭癌の転移であった．術後補助化学療法後に甲状腺の手術を施行し，8 mmの甲状腺乳頭癌が認められた．結語．肺癌手術時に縦隔リンパ節転移で発見された不顕性甲状腺癌の1例を報告した．肺癌で気管周囲にリンパ節転移を認める時，不顕性甲状腺癌の転移の可能性がある．

DNA修復機能阻害は放射線感受性を増強させるため，DNA修復に関与する因子の阻害剤は放射線増感剤となり得る．我々の開発したテロメラーゼ依存的腫瘍融解アデノウイルス製剤OBP-301（テロメライシン）は，アデノウイルスE1B55kDaタンパクを介して細胞のDNA修復に重要な役割を果たすMRN複合体（Mre11，Rad50，NBS1）を分解する機能を有する．このMRN複合体の分解によりATM（ataxia-telangiectasia mutated）の活性化が抑制され結果的にDNA修復機構が阻害される．我々はOBP-301と放射線との併用が強力な相乗効果を生み出すことをマウスの皮下腫瘍モデルおよび食道癌同所性モデルにおいて証明した．これらの結果はOBP-301が将来有望な放射線増感剤となり得ることだけでなく，E1B55kDaタンパクを産生する腫瘍融解アデノウイルス製剤と放射線との併用が悪性腫瘍に対する有力な治療戦略となり得ることを示す．

急性膿胸は早期に適切な抗生剤の投与と胸腔ドレナージを行えば保存的に治癒が見込まれるが，線維素が析出し胸膜肥厚により肺膨張不全を伴ってくると，慢性膿胸に進行する．われわれは急性膿胸に対して比較的早期に胸腔鏡下手術を行い治癒しえた5例を検討した．年齢中央値61±12.9歳，男性3例，女性2例であった．術前病悩期間は8.4±3.6日で，原因は肺炎4例，胸膜炎1例であった．手術は全例胸腔鏡下に肺剥皮術・ドレナージを行った．平均手術時間は131.8±14.6分，平均出血量54±42.9gであった．5例中2例で抜管直後に肺水腫を来たしたが，非侵襲的陽圧換気（NIPPV）により，速やかに改善した．ドレーン留置期間は平均5.2±2.2日で，術後在院日数は平均17.2±5.9日であった．全例胸腔鏡下手術は低侵襲であり，急性膿胸が慢性化するのを妨げるのに有用な治療法と考えられた．

背景．稀に原発巣が明らかでなく，肺門縦隔リンパ節にのみ癌病巣を認める原発不明リンパ節癌を経験することがある．症例．65歳，男性．約1か月前からの左季肋部痛を主訴に前医を受診した．精査にてCEA高値およびCTにて左肺門異常陰影を指摘され，当院へ紹介となった．CTでは左肺門部腫瘤を認めた．PETでは腫瘤部への異常集積と左第10肋骨へ軽度の集積を認め，同部は疼痛部と一致していた．組織診断が困難であり，骨転移が完全に否定できないこと，CEAが異常高値であることから，方針の決定に難渋した．肋骨に関しては明らかな溶骨性，造骨性変化を認めず，外傷性の可能性が高いと考え経過観察とした．左肺門部腫瘤は原発不明肺癌のリンパ節転移または原発性リンパ節癌の可能性が考えられた．遠隔転移は明らかでなく，生検診断が困難であったことから手術の方針とした．手術は左上葉切除，ND2a郭清を行った．病理診断では，リンパ節内の大細胞癌と診断された．左上葉内に病変はなく，郭清した他のリンパ節に転移は認めなかった．術後補助化学療法を行い，経過観察中である．結語．単一領域のリンパ節癌は完全切除ができた場合の予後は良好であるとされ，積極的な治療が有効と考えられた1例を経験した．

第2肺癌に対する切除術では根治性と肺機能温存の両立という問題がある．症例は57歳の男性，右S3の末梢型扁平上皮癌（長径21mm，pT1N0M0 stage I A）で右上葉切除＋リンパ節郭清術施行後6ヵ月の胸部CTで気管支内腔に突出する腫瘍が指摘された．気管支鏡検査で左上下幹分岐部に発生した中心型扁平上皮癌で壁外に及んでいると診断された．第2肺癌と考えられ治療法に苦慮したが，腫瘍の縮小を期待し化学療法（carboplatin＋weekly paclitaxel）2コース施行した．15mmの腫瘍は4mmに縮小し，S6区域切除を伴う左上下幹分岐部管状切除で完全切除でき二連銃式気管支形成術で再建した．最終病理はpT2N1M0 stage II Bであったが，現在5年間再発なく通院中である．肺葉切除後の第2肺癌が中心型肺癌であった場合，腫瘍の占拠部位から気管支形成術によって断端陰性となるよう工夫し，さらには化学療法を組み合わせることで根治性を保ちながら肺機能温存が可能な術式を選択すべきである．

症例は56歳，女性．2009年5月検診での胸部異常陰影にて近医を受診，CTにて前縦隔腫瘍を認めたため当科紹介となった．1年前の検診では異常は認めなかった．CTでは前縦隔に10×9cmの軽度造影効果を伴い内部不均一で脂肪成分を含む腫瘤を認めた．MRIでも同様に脂肪成分を含み内部不均一であった．肺動脈や心嚢との境界がやや不明瞭であったが炎症所見と考えた．1年前の検診では異常を認めておらず成長速度が速いこと，周囲との境界不明瞭な画像所見を呈していたことより悪性疾患である可能性も考えられたが，成熟奇形腫を疑い手術を施行した．胸骨縦切開にて腫瘍を摘出し迅速病理検査に提出したところ粘液性腫瘍とのことであった．摘出標本では11×10×9cmの腫瘤を認めた．術後病理検査では混合型脂肪肉腫との診断であった．脂肪成分を含む縦隔腫瘍を認めた場合には，脂肪肉腫も念頭において加療を行うことが重要であると考えられた．

背景．原発不明縦隔リンパ節転移は，まれである．症例．症例は8年前に早期胃癌切除の既往がある69歳の男性で，CEAの上昇を指摘された．FDG-PETが施行され，右上縦隔と右鎖骨上のリンパ節に異常集積が認められた．胸部CTにて，右上縦隔と右鎖骨上のリンパ節腫大を認め，肺内には腫瘤を認めなかった．リンパ節転移が示唆されたが，原発病巣は明らかではなかった．系統的右縦隔リンパ節郭清と腫大した鎖骨上リンパ節の摘出が施行された．multi-stationの縦隔リンパ節と鎖骨上リンパ節に及ぶ大細胞癌と病理診断された．17か月後，PETにて右鎖骨上リンパ節に異常集積が認められた．右頚部リンパ節郭清が施行され，1個のリンパ節に大細胞癌が認められた．術後補助化学療法として2年間UFT®が投与された．2回目の手術後4年5か月経過し，PET上での異常集積を認めておらず，CEAも上昇していない．結語．multi-stationの縦隔リンパ節，鎖骨上リンパ節に及ぶ原発不明癌の長期生存例を報告した．原発不明縦隔リンパ節癌はsingle-stationだけではなくmulti-stationであっても，完全切除を目指した手術が有効である可能性が示唆された．

症例は69歳，女性．喘息発作にて近医受診し，胸部Xpにて右上肺野の異常陰影を指摘された．胸部CTで右S1の結節影と胸水および中葉無気肺を認めた．肺機能検査では一秒量700mlと低肺機能を示していた．精査目的で当院紹介となった．初診時，胸腔穿刺を行った．胸部CTでは，膨張がえられた中葉内および下葉に新たに結節影が確認できた．肺機能検査を再検したところ，一秒量1380mlと著明に改善が得られ，手術可能と判断した．胸腔鏡補助右中葉切除，上葉部分切除，下葉部分切除を施行した．胸腔内を検索すると，胸水はなく巨大な胸腺嚢胞を認め，これを摘出した．術後の病理所見では，肺内の3箇所の病変はいずれも肺胞上皮癌，野口分類B型であった．嚢胞は一層の上皮に覆われた組織で壁内に脂肪織と胸腺組織を伴っていた．術後，合併症なく経過し，無再発生存中である．巨大な胸腺嚢胞によって多発肺癌が潜在していた1例を経験したので報告する．

肺摘除術後の気管支瘻は致命的になりうる合併症であり，早急かつ適切な処置が必要とされる．今回我々は右肺摘除後の気管支瘻に対し，大網を楔入充填することで瘻孔閉鎖に成功した2例を経験したので報告する．症例1は78歳，男性，右主気管支から中間幹に広く浸潤するpleomorphic carcinomaでsleeve pneumonectomyを施行した．術後16日目に吻合部縫合不全のため瘻孔大網楔入充填術を行った．症例2は59歳，男性，扁平上皮癌で体外循環下に左房合併切除を伴う右肺摘除術を施行した．外来化学療法中に膿胸を併発し，気管支断端瘻となったため，術後8ヵ月で同様の手術を行った．気管支瘻の断端や瘻孔周囲は，すでに血流障害に陥っているため，一期的縫合閉鎖では気管支瘻が再発しやすい．抗炎症作用に優れ局所の血流を改善させる大網を瘻孔に直接楔入充填するこの方法は，手技が容易で大変有用であった．

背景．肺癌の術後補助化学療法中に膿胸を合併したという報告は少ない．症例．74歳男性がCT上の異常影で当院に紹介された．右上葉切除とND2aリンパ節郭清術を施行した．病理診断はpT4N0M0 p0d0E0pm1 stage IIIBの腺癌であった．carboplatinとweekly paclitaxelによる術後補助化学療法を施行した．4コース目の14日目に高熱にて入院した．胸部X線写真とCTにて多量の右胸水を認めた．胸腔ドレナージを施行し，胸水培養にてメチシリン耐性黄色ブドウ球菌（MRSA）による膿胸と診断された．持続洗浄を施行し，vancomycinを投与した．入院7日目のCTにて，背側及び横隔膜上に胸水を認めた．入院10日目に，小開胸併用胸腔鏡下膿胸腔掻爬術を施行し，3本のdouble-lumen tubeを留置した．持続洗浄を施行し，vancomycinをlinezolidに変更した．ドレーンは胸水の培養が陰性になった後，順に抜去した．術後47日で退院となった．初回術後22か月の現在，膿胸も癌も再発を認めていない．結語．肺癌術後補助化学療法中に免疫能低下から膿胸を合併する可能性がある．保存的治療により改善しない場合，時期を失せず胸腔鏡下膿胸腔掻爬術を施行すべきであると考えられた．

背景．重症筋無力症（MG）に合併した肺癌の報告は少ない．症例．76歳の女性で，主訴は眼瞼下垂と下肢の脱力．エドロフォニウムの注射により筋力の改善を認めた．Sero-negative MGと診断された．胸部X線写真にて，右上肺野に異常影が認められた．胸部CTにて，右S1に3 cmのmixed ground glass opacity（GGO），右S3に1 cmのGGO，左S10に5 mmのGGOの3病変が認められた．肋間開胸を伴わない胸骨正中切開にて，拡大胸腺摘出術と根治的右上葉切除，リンパ節郭清術を同時に施行した．病理学的検索により，S1とS3の病変はそれぞれ混合型腺癌と肺胞上皮癌と診断された．術後経過は良好で，術後10日目にMGの薬物治療のために神経内科に転科した．4年2か月後の現在，患者は無再発生存しており，左S10のGGOに変化を認めず，pyridostigmine bromideの内服のみで筋力はほぼ正常に維持されている．結論．胸腺腫を合併しないMGと原発性多発肺癌の同時手術症例を報告した．MGに肺癌を合併する可能性がまれながら存在することを念頭に置くことが必要である．

肺癌術後の荒蕪肺にアスペルギルス症を，膿胸腔にはMethicillin-resistant Staphylococcus aureus（MRSA）と緑膿菌を感染した有瘻性膿胸に対し開窓術後に広範な胸壁合併切除を伴う胸郭形成と胸膜残肺全摘，大網充填術を行い，さらに術後の胸郭動揺を防止する目的で遊離の大腿筋膜弁（大腿筋膜張筋＋腸径靭帯）を胸壁に固定し治癒できた症例を経験したので報告する．有瘻性膿胸の治療において重要なことは膿胸腔の浄化と遺残腔をできるだけ残さないことである．さらに荒蕪肺は感染の温床になるため心肺機能が許せば切除すべきと考える．そして栄養管理を含めた全身状態の改善を図りつつ，根治術の時期を逃さないことである．広範な胸壁切除は整容性の問題はあるが，視野が良好で手術時間や出血量が抑えられ術後の死腔を完全に消失することができる．また術後の胸郭動揺を防止するために大腿筋膜弁による固定術を追加するという方法は今回検索した範囲では報告がなく新しい知見と思われた．

背景．肺原発リンパ上皮腫様癌（lymphoepithelioma-like carcinoma: LELC）はWHO分類上，大細胞癌の特殊型に分類される稀な腫瘍である．症例．66歳，男性．検診にて胸部異常陰影を指摘された．胸部CTで右上葉に約40 mm大の腫瘤と気管前リンパ節の腫大を認めた．PET-CTでは腫瘍及び気管前リンパ節で著明にFDGの異常集積を認めた．気管支鏡検査を行い，経気管支肺生検で非小細胞癌と診断した．cN2症例であったが孤発性であったため，初回手術療法を選択した．右上葉切除，ND2aを施行した．術後病理診断はLELCで，病理学的病期は，pT2N2M0 stage IIIAとなった．Epstein-Barrウイルス（EBV）の有無をEBER1のin situ hybridizationにより検索したが，陰性であった．術後補助化学療法として，carboplatin/paclitaxel併用療法を4コース行い，術後1年6ヶ月経過し無再発生存中である．結論．リンパ上皮腫様癌は本邦での報告例は少なく，稀な疾患であると考えられたので，若干の文献的考察を加えて報告した．

背景．肺硬化性血管腫は，術中迅速病理検査にて腺癌と診断されたという報告が散見される腫瘍である．症例．25歳の女性が，検診にて肺腫瘍を指摘された．胸部X線写真では，左下肺野に腫瘤影が認められた．胸部CTでは，左S8に40 mmの境界明瞭な腫瘤が認められた．PETではFDGの異常集積（SUV 5.0）が認められた．気管支鏡は咳にて挿入できなかったため，胸腔鏡下手術を施行することとした．腫瘍の針生検を施行し術中迅速病理診断に提出し，カルチノイドと診断されたため，左下葉切除＋リンパ節郭清術を施行した．しかしながら，切除標本の術後検索では，硬化性血管腫の組織像に矛盾しない組織所見を呈していた．硬化性血管腫は，組織学的には乳頭状部分，充実性部分，硬化性部分，血管腫様部分が混在すると報告されている．乳頭状の部分は腺癌と診断される可能性があり，充実性部分はカルチノイドと診断される可能性がある．術中の針生検の標本に充実性部分しか含まれていなかったため，今回の腫瘍はカルチノイドと診断された．結論．硬化性血管腫が疑われる場合，術中針生検では複数の標本を採取すべきと考えられた．

肺癌診療においてFDG-PET検査の有用性は知られているが，偽陽性，偽陰性症例も少なからず認められる．我々は，2005年7月～2007年6月までの2年間で良悪性診断または病期診断の目的でPET検査を行い，最終的に当施設で手術を施行した283症例を対象として，その有用性と問題点について検討した．内訳は男：女＝156：127，平均年齢は66.3歳であった．良悪性診断の感度72.7％，特異度60.5％で，腫瘍径20mm以下のものでは偽陰性が多かった．組織型では肺胞上皮癌の成分を多く含む高分化腺癌や粘液の多い病変は多くの場合PET陰性であった．その一方で細胞密度の高い扁平上皮癌などは高感度に検出された．リンパ節転移診断は感度68.0％，特異度93.2％で特異度は高いものの偽陰性が多く，過小評価をする可能性があった．PET検査の有する長所，短所を十分理解し，臨床的に活用していく必要がある．特に腺癌では原発病巣が偽陰性を呈することが多く，CT診断と併せて手術適応を決定すべきである．

背景．肺全摘後の肺癌に対する対側肺切除についての報告は少なく，そのほとんどは1回の再切除の報告である．症例．45歳男性で，胸部異常影にて入院した．CTにて，右主気管支への壁浸潤を伴うS6原発の腫瘍を認めた．右肺全摘とND2aのリンパ節郭清を施行した．病理学的には，扁平上皮癌で，pT3N1M0 Stage IIIAであった．5年後，CTにて左下葉に腫瘤影を認めた．呼吸機能は，2回目の手術に十分な程度保持されており，左肺下葉の部分切除を施行した．16 mmの扁平上皮癌と診断され，Martiniらの診断基準により異時性多発癌とした．さらに3年8ヶ月後のCTにて，左上葉のbullaに接して腫瘤影が出現し，FDG-PETで弱い集積を認めた．混合性呼吸機能障害を認めるも，PSは良好であり，3回目の手術を希望された．左肺上葉部分切除を施行し，病理にて15 mmの腺癌と診断された．術後経過は良好であった．術後PSの低下を認めるも，日常生活に支障なく，術後1年5ヶ月の現在再発を認めていない．結語．右肺全摘後に2回の左肺部分切除を施行した，異時性多発肺癌の症例を経験した．積極的な手術が，初回手術より10年という長期生存につながっていると考えられた．

症例は32歳女性，咳，胸痛を主訴に近医受診し，胸部X線検査上，縦隔の異常影を指摘された．CTでは前縦隔に内部均一な腫瘤を認めた．胸腺腫との術前診断にて，胸骨正中切開，拡大胸腺摘出術を施行した．肺への浸潤，肺転移巣を認めたため，肺部分切除を追加した．病理組織学的には，線維性隔壁に境される結節性病変を呈し，大型のH細胞，R-S細胞を認めた．免疫染色ではCD15（＋），CD30（＋）であり，Classical Hodgkin's lymphoma，Nodular sclerosis type，CS II A，PS II A（L＋）と診断された．ホジキンリンパ腫はリンパ節に原発することが多く，胸腺原発例の報告は散見されるが比較的まれである．本症例は術後放射線治療を行い，術後10ヵ月無再発生存中である．適切な治療を施せば予後良好な疾患であり，今後の厳重な経過観察が必要である．

背景．薄壁空洞病変を呈する原発性肺癌はまれである．症例．51歳女性が，右下肺野の薄壁空洞を呈する胸部異常影にて入院した．CTにて，右S10に65×30 mmの薄壁空洞病変が認められた．空洞壁は不整で，厚みは2～6 mmであった．PETでは，空洞壁の一部にFDGの異常集積を認めた．SUVは最高で3.2であった．胸腔鏡下に手術を施行し，まず底区域切除を施行した．術中迅速病理診断にて，粘液産生型細気管支肺胞上皮癌と診断されたので，S6を追加切除し，リンパ節郭清も施行した．最終病理診断はほとんどが粘液産生型肺胞上皮癌である混合型腺癌と診断された．病理病期はT2N0M0 stage IBであった．結論．画像所見が薄壁空洞を呈する場合，胸腔鏡下手術を含めた精査を考慮すべきである．FDG-PETが薄壁空洞病変の鑑別診断に有用である可能性があるが，症例の集積が必要である．

This study assessed the techniques of the free jejunal graft for the reconstruction of hypopharynx or cervical esophagus and discussed the main aspects related to those procedures. By using free jejunal grafts, we reconstructed 54 hypopharyngeal and cervical esophageal cancers. In this study, 23 out of 54 patients had a malignant tumor located in the hypopharynx and 31 in the cervical esophagus (27 primary cases and four secondary cases). Despite the multi-step and time-consuming procedure, we did not incur any trans-operative complication. Furthermore, we undertook the larynx preserving cervical esophagectomy and free jejunal graft reconstruction in six patients with cervical esophageal cancer, and those patients acquired a good quality of life. For the reconstruction of hypopharynx or cervical esophagus, the free jejunal graft is a very useful technique and improves the patient’s quality of life.

症例は30歳,男性.心窩部痛を主訴に前医受診,急性膵炎として加療された.膵炎の改善後,通院にて経過観察していたところ,膵体尾部に腫瘤を認めるようになったため,当院紹介となった.造影CTおよび造影MRI検査にて,造影早期相でhypovascularで,後期相で徐々に淡く染まってくる腫瘤を認めた.また, MRCPでは主膵管の途絶と末梢膵管の拡張を認めた.若年者ではあるものの膵管癌を第一に疑い,膵体尾部切除術を行った.術後病理組織診断では膵ラ島細胞癌であった.免疫組織化学染色ではChromogranin Aのみ陽性であった.膵内分泌腫瘍は通常は圧排性に発育し, vascularityが高いことが特徴である.本症例は画像上,非典型的なパターンを示し,術前診断に難渋した.

われわれは術後胆管狭窄により敗血症をきたした症例にexpandablemetallicstent(EMS)を留置し有効であったと考えられたので報告する.症例は70歳男性.胆管内発育型胆管細胞癌にて Hr2(M, L),右肝管空腸吻合施行.外来通院中, 38°C台の波状熱を繰り返し,胆管炎として,入院,抗生剤の点滴で緩解することが数回あった.術後1年目,発熱,腹水,黄疸を認め,再度入院.腹部超音波, CT検査にて肝内胆管拡張を認めた.肝生検ではspotnecrosisを認め,胆汁うっ滞性肝炎と診断した.肝炎治療を続けていたが,急激な血圧低下,尿量減少, DIC徴候を呈し,肝不全状態に陥った. GI療法,抗DIC療法などで改善をみたが,肝内胆管拡張,ビリルビン高値は持続するため,右肝管空腸吻合狭窄の診断でPTBD施行.黄疸は著明に改善され, 2度のバルーン拡張術の後, EMSを挿入した.狭窄は解除され,現在肝炎治療を継続している.術後胆管狭窄は放置すれば,胆汁性肝硬変や敗血症などの重篤な合併症をおこすことがあり,適切な治療が必要である.

Currently available methods for detection of tumors in vivo such as computed tomography and magnetic resonance imaging are not specific for tumors. Here we describe a new approach for visualizing tumors whose fluorescence can be detected using telomerase-specific replication-competent adenovirus expressing green fluorescent protein (GFP) (OBP-401). OBP-401 contains the replication cassette, in which the human telomerase reverse transcriptase (hTERT) promoter drives expression of E1 genes, and the GFP gene for monitoring viral replication. When OBP-401 was intratumorally injected into HT29 tumors orthotopically implanted into the rectum in BALB/c nu/nu mice, para-aortic lymph node metastasis could be visualized at laparotomy under a three-chip color cooled charged-coupled device camera. Our results indicate that OBP-401 causes viral spread into the regional lymphatic area and selectively replicates in neoplastic lesions, resulting in GFP expression in metastatic lymph nodes. This technology is adaptable to detect lymph node metastasis in vivo as a preclinical model of surgical navigation.

Background During perioperative management of patients with gastrointestinal cancer complicated by diabetes mellitus, adequate alimentation is required, but we often face difficulties associated with hyperglycemia and other accompanying complications. Recently, we investigated the effects of a novel palatinose based enteral formula (MHN-01) in suppressing post-prandial hyperglycemia and improving lipid metabolism in experimental animals and perioperative management of patients with esophageal cancer complicated by diabetes mellitus. Materials and methods We gave normal rats and rats with type 2 diabetes mellitus a single oral dose of fluid diet, and analyzed comparatively the time course of blood glucose level in each group until 3 h after the dose. In both the normal rat group and the type 2 diabetes group, peak blood glucose level after the MHN-01 dose was significantly lower than after a dose of ordinary fluid diet and was comparable to the peak level after a dose of a fluid diet rich in MUFA (monounsaturated fatty acid). We allowed normal mice free access to fluid diet for 43 days, and measured their body fat levels. Fat accumulation was significantly lower in mice given MHN-01 than in mice given ordinary fluid diet. We also analyzed the respiratory quotient and resting energy expenditure of normal Sprague-Dawley rats fed by MHN-01 or an ordinary fluid diet. The respiratory quotient of the MHN-01 group was significantly lower than the ordinary fluid group, although the resting energy expenditure of both groups was almost the same level. The effect of MHN-01 was estimated to be based on improvement of lipid metabolism. Results Between 2003 and 2005, among 164 patients who underwent radical thoracic esophagectomy and/or reconstruction for esophageal carcinoma at Okayama University Hospital, nine patients (5.5%) were diagnosed with diabetes mellitus in pre-operative screening and were treated with MHN-01. Clinical courses of two cases with severe status of diabetes mellitus were presented as successful case reports of MHN-01. Conclusion MHN-01 was very useful in perioperative management of patients complicated by diabetes mellitus, unable to ingest food p.o. such as esophageal cancer or other diseases.
