Considering the role of autonomic nerve activity in blood pressure control, this study aimed to investigate the cardiac autonomic nerve responses after an aerobic exercise session in Anabolic Androgenic Steroids (AAS) users. Twenty men (AAS, n = 9; control group, n = 11) performed an aerobic exercise session (60 min, 70 to 80% of HRmax). Heart rate variability (HRV) was assessed before and during a 60-min post-exercise recovery period. RMSSD (root mean square successive difference of the RR intervals) and the LF/HF ratio (low frequency/high frequency spectra) were also evaluated. The Student's t-test for independent samples was used to compare differences between initial group characteristics. Repeated measures ANOVA was used to compare pre- and post-exercise HRV recovery (p < 0.05). AAS had a lower SDNN (standard deviation of the intervals) (40.8 ± 16.8 vs. 71.6 ± 24.7 ms; p = 0.04, d = 1.4) and a higher LF/HF (3.4 ± 2.1 vs. 1.8 ± 0.9%; p = 0.03, d = 0.9) before exercise. AAS and controls had similar RMSSD (14.0 ± 15.8 vs. 18.9 ± 12.1 ms; p = 0.20) and a LF/HF (2.8 ± 1.6 vs. 2.4 ± 1.2 ms; p = 0.41) immediately post-exercise. The between-groups comparison revealed a higher HF/LF at 30 min (4.3 ± 1.4 vs. 2.5 ± 1.3%; p = 0.008, d = 1.3) and 60 min (5.0 ± 2.2 vs. 2.3 ± 0.8%; p = 0.001, d = 1.6) for the AAS group in the recovery time. This study demonstrated impaired parasympathetic activity at rest and immediately after the exercise session as an adverse effect of AAS usage, but similar behavior regarding the restoration of sympathetic activity.

Purpose of review The purpose of this review is to examine the recent evidence regarding the effects of exogenous androgens on the brain. Understanding these effects is of high importance, as the consequences of androgens on the reproductive and endocrine system are well documented, while fewer studies have focused on the neural and cerebral consequences of androgen use. Recent findings Supraphysiological doses of androgens have been shown to contribute to neurodegeneration, decreased brain-derived neurotrophic factor, increased inflammation and decreased neuronal density in animal studies, which may correspond to changes in mood, cognition and aggression. Findings from human studies suggest that similar behavioural and cognitive deficits may occur as a result of prolonged use of androgens. Additional evidence suggests that androgen use, particularly in high doses, may contribute to brain ageing and cerebrovascular problems. Summary Findings from recent human and animal studies indicate that androgen use likely contributes to brain alterations, which may cause the frequently observed deficits in cognitive and emotional functioning. Although exogenous testosterone in appropriate doses for therapeutic purposes likely have some neurobiological benefits for certain populations, supraphysiological doses may cause multiple mental and physical health problems, indicating a need for additional large-scale studies in humans.

Purpose of review Given governmental policy responses to the COVID-19 pandemic include various foci on establishing preventive measures to increase the health and constitution of populations (i.e. through renewed efforts to tackle obesity), the use of androgens in a recreational manner requires attention because of existing medical evidence related to their role in immunosuppression. Recent findings Whilst a broad body of work exists that outlines the mechanisms that underpin COVID-19 and public health responses, as well as the use of androgens in both recreational and medicinal contexts, the recent nature of the disease has left an area requiring greater investigation and clarification. There is emerging literature that highlights the potential complications that existing androgens use may bring to those infected with COVID-19, as well as the start of empirical evidence detailing this. Summary In this article, we outline the latest viewpoints and literature related to how the use of androgens may contribute to less robust immunoresponses.

Background Nowadays bodybuilders use anabolic steroids frequently. Abuse of these substances can cause significant side effects; therefore, we aim to investigate the effect of anabolic steroids on DNA damage in bodybuilders’ blood lymphocytes.Methods and materials This case-control study was performed on 36 male bodybuilders in Gonabad. The case group included bodybuilders with a history of taking anabolic-androgenic steroids (n = 18), and the control group composed of bodybuilders who did not use anabolic-androgenic steroids (n = 18). Intravenous blood samples were obtained and then the lymphocytes, cells and electrophoresis of blood were extracted. Afterward, the coloured slides and DNA damage were measured using a fluorescent microscope and CometScore software. The DNA damage was compared using t-tests .Results Results showed that there was no significant difference between age, marital status, BMI, systolic and diastolic blood pressure in the case and control group. However, parameters related to the DNA damage including tail length, percent tail DNA, and tail moment were significantly higher in the case group.Conclusion The use of anabolic-androgenic steroids increases DNA damage in the bodybuilders’ blood lymphocytes.

Background Anabolic–androgenic steroids (AAS) contain testosterone-like androgens and are used as supplements to improve performance, therapeutic measures, appearance, and muscular development. Purpose This study aimed to estimate using anabolic–androgenic steroids (AAS) and good and bad practices about dietary supplements among resistance-trained individuals. It further seeked to determine the use of common drugs and supplements containing anabolic steroids among resistance-trained individuals (who work out at the sports centre) and assess users' knowledge about its side effects. Methods A cross-sectional survey was conducted at the sports centres of the western cities of Saudi Arabia. A self-administered questionnaire was used to collect data from 120 male resistance-trained individuals. Results The majority of the participants (80%) reported that they had not used any hormonal bodybuilding supplement last year, while 20% said they had used such hormonal supplements. Approximately half (52.5%) of participants reported that they always used dietary supplements. A total of 44.2% of participants possessed inadequate knowledge of these products. The main reason behind the use of hormones and supplements was to increase muscle mass. Conclusions A minority of resistance-trained individuals in the studied population frequently misused AAS. However, the results cannot be generalised to the whole of Saudi Arabia. AAS consumption can be reduced by enhancing the level of awareness and knowledge of potential adverse health outcomes.

Anabolic Androgenic Steroids (AAS) are commonly used for their anabolic effects and potential detrimental consequences are well documented. Most studies focus on the motivations of increased muscle development and report increased libido as a secondary motivation. This paper aims to explore users’ reports of libido as a motivator for starting and restarting AAS use. This mixed-methods study comprised questionnaires with 133 adult male AAS users (38% of selected increased libido as motivation for using), and 23 semi-structured interviews with a sub-sample. Thematic analysis identified four interlinked themes: increased libido as a motivation for starting/re-starting AAS; increased libido/sexual performance as a beneficial effect; mixed experiences with physicians regarding libido changes and hypogonadism and reduced sexual functioning and fertility after AAS cessation. Increased libido was identified as a benefit by 90% but motivations for use changed over time. Reasons for AAS use included mitigating the effects of aging particularly linked to the concept of virility. AAS as self-medication for low testosterone needs further investigation as does the idea of hegemonic masculinity as a reinforcing driver for AAS use. This study highlights the need to consider how AAS users’ views of medical support impact on self-medication choices.

Performance enhancing agents (PEAs) are substances used to improve any form of activity performed by humans. These are widely used by athletes and body builders. Being a frequent subject of controversy in the sporting world, the use of performance-enhancing drugs has shown a disturbing growth over the past fifty years. The use of drugs to enhance human performance in sports has occurred at least since the time of the original Olympic Games (from 776 to 393 BC). In 1860s, a group of swimmers in Amsterdam were charged for taking drugs to speed up their races. For the next 80 years, athletes who wanted to cheat focused mostly on stimulants to speed themselves up. A wide range of PEAs are used today in the sports field. They include Anabolic Androgenic Steroids (AASs), peptide hormones, growth factors, beta-2 agonists, diuretics, masking agents, stimulants, narcotics and even cannabinoids. Though extensive research is being done on the use of PEAs and doping in the sports industry, throughout the developed world, very few published research is available from the third world countries including Sri Lanka. This preliminary study revealed that a wide variety of PEAs are being inadvertently used without justifiable scientific reason and without a reasonable understanding of how they work and what their adverse effects are, by the gym users in Sri Lanka. It also showed that a significant proportion of them experience side effects directly attributable to the use of PEAs.

Sports nutrition supplements have previously been reported to contain undeclared doping substances. The use of such supplements can lead to general health risks and may give rise to unintentional doping violations in elite sports. To assess the prevalence of doping substances in a range of high-risk sports nutrition supplements available from Dutch web shops. A total of 66 sports nutrition supplements - identified as potentially high-risk products claiming to modulate hormone regulation, stimulate muscle mass gain, increase fat loss, and/or boost energy - were selected from 21 different brands and purchased from 17 web shops. All products were analyzed for doping substances by the UK life sciences testing company LGC, formerly known as the Laboratory of the Government Chemist, using an extended version of their ISO17025 accredited nutritional supplement screen. A total of 25 out of the 66 products (38%) contained undeclared doping substances, which included high levels of the stimulants oxilofrine, beta-methylphenethylamine (BMPEA) and N,beta-dimethylphenethylamine (NBDMPEA), the stimulant 4-methylhexan-2-amine (methylhexaneamine, 1,3-dimethylamylamine, DMAA), the anabolic steroids boldione (1,4-androstadiene-3,17-dione) and 5-androstene-3 beta,17 alpha-diol (17 alpha-AED), the beta-2 agonist higenamine and the beta-blocker bisoprolol. Based upon the recommended dose and the potential variability of analyte concentration, the ingestion of some products identified within this study could pose a significant risk of unintentional doping violations. In addition to inadvertent doping risks, the prescribed use of 3 products (4.5%) could likely impose general health risks.

For decades, the class of anabolic androgenic steroids has represented the most frequently detected doping agents in athletes’ urine samples. Roughly 50% of all adverse analytical findings per year can be attributed to anabolic androgenic steroids, of which about 2/3 are synthetic exogenous steroids, where a qualitative analytical approach is sufficient for routine doping controls. For the remaining 1/3 of findings, caused by endogenous steroid-derived analytical test results, a more sophisticated quantitative approach is required, as their sheer presence in urine cannot be directly linked to an illicit administration. Here, the determination of urinary concentrations and concentration ratios proved to be a suitable tool to identify abnormal steroid profiles. Due to the large inter-individual variability of both concentrations and ratios, population-based thresholds demonstrated to be of limited practicability, leading to the introduction of the steroidal module of the Athlete Biological Passport. The passport enabled the generation of athlete-specific individual reference ranges for steroid profile parameters. Besides an increase in sensitivity, several other aspects like sample substitution or numerous confounding factors affecting the steroid profile are addressed by the Athlete Biological Passport-based approach. This narrative review provides a comprehensive overview on current prospects, supporting professionals in sports drug testing and steroid physiology.

The anabolic androgenic steroids (AAS) are the most frequently consumed performance enhancing drugs (PED) in sports. In the anti-doping field, the detection of AAS is carried out by the analysis of the athlete’s urine using methodologies based on liquid/gas chromatography-mass spectrometry. Unfortunately, the detection of unknown compounds is not possible. BDS’s AR CALUX® bio detection technology was studied as an indirect method to detect the administration of a single dose of testosterone (T). Twelve T and placebo single dose administered men volunteers underwent a triple-blind crossover clinical trial. The UGT2B17 deletion was present among the volunteers and evenly distributed in heterozygous (ins/del), wild-type homozygous (ins/ins), and mutated homozygous (del/del) groups. A significant statistical difference in terms of bioluminescence was observed after the testosterone (T) administration for the three types of polymorphic groups. The ratio of means between the pre- and post-T administration periods, depending on the type of polymorphism, was in group ins/ins 3.31 (CI. 95%: 2.07–5.29), group ins/del 4.15 (CI 95%: 3.05–5.67), and group del/del 2.89 (CI 95%: 2.42–3.46). The results of the study are very promising, as they may offer us the possibility of designing a detection approach that, based on intra-individual monitoring of androgenic values, in the UGT2B17 deletion type.

RationaleAggression and irritability are notable psychiatric side effects of anabolic-androgenic steroid (AAS) use. However, no previous study has systematically reviewed and quantitatively synthesized effects reported by experimental studies on this topic.ObjectiveWe conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) investigating the effect of AAS administration on self-reported and observer-reported aggression.MethodsTwelve RCTs comprising a total of 562 healthy males were identified through systematic searches of MEDLINE, PsycInfo, ISI Web of Science, ProQuest, Google Scholar, and the Cochrane Library.ResultsAfter excluding one outlier, AAS administration was associated with an increase in self-reported aggression under a random-effects model, albeit small (Hedges’g= 0.171, 95% CI: 0.029–0.312,k= 11,p= .018), and when restricting the analysis to the effect of acute AAS administration on self-reported aggression under a fixed-effect model (g= 0.291, 95% CI: 0.014–0.524,p= .014). However, the above effects were neither replicated in the analysis of observer-reported aggression nor after restricting the analysis to the effects of the administration of higher (over 500 mg) and long-term (3 days to 14 weeks) doses.ConclusionsThe present meta-analysis provides evidence of an increase, although small, in self-reported aggression in healthy males following AAS administration in RCTs. Ecologically rational RCTs are warranted to better explore the effect of AAS administration on aggression in humans.

The effect of anabolic–androgenic steroid (AAS) abuse on the contact activation system (CAS) is not known in detail. We hypothesized that current AAS abuse reduces the kallikrein-generating capacity of CAS significantly and investigated the impact of AAS on the proteins and capacity of CAS in current and former AAS abusers and healthy age-matched controls. Men 18 to 50 years of age were included as current AAS abusers, former AAS abusers, or controls. Blood samples were collected after overnight fasting. Kallikrein generation (lag time, peak height, and endogenous kallikrein potential [EKP]), coagulation factor XII (FXII), prekallikrein, high-molecular-weight kininogen (HK), and Complement C1 esterase inhibitor (C1inh) were assessed. Groups were compared by analysis of variance or Kruskal–Wallis test and probabilities were corrected for multiple comparisons. Associations were evaluated by linear regression models. Common abuse of anabolic androgenic steroids (AAS) is no longer confined to high performance athletes, as it has spread among the general population. Epidemiological data about the abuse of these substances show that it is a common practice in young populations. Its use is based on the desire to increase muscle mass and strength, as well as improving physical performance. The ease of acquisition of this type of substances has developed a "sophisticated" knowledge of steroid pharmacology based on subjective and anecdotal analysis with no adverse event information, which translates into a public health crisis. Unfortunately, athletes seem to be more influenced by these experiences than by their physician's advice. The abuse of AAS by the athlete and non-athlete population and its adverse events ought to be evaluated in order to improve routine clinical practice on this regard.

Background A primary consequence of illicit drug markets and the absence of regulation is the variable quality or purity of the final product. Analysis of anabolic-androgenic steroid seizures shows that these products can contain adulterated products, product not included on the label, or product of unsatisfactory standard. While the potential negative effects of counterfeit anabolic-androgenic steroids (AAS) use is a recognised risk associated with use, no study has explored personal experiences associated with use. The aim of the present study was to use online discussion forums to investigate and explore the experiences associated with the purchase and consumption of counterfeit AAS among consumers. Methods An online search was conducted to identify online forums that discussed counterfeit or contaminated AAS; three were deemed suitable for the study. The primary source of data for this study was the ‘threads’ from these online forums, identified using search terms including ‘counterfeit’, ‘tampered’, and ‘fake’. Threads were thematically analysed for overall content, leading to the identification of themes. Results Data from 134 threads (2743 posts from 875 unique avatars) was included. Two main themes were identified from the analysis: (1) experiences with counterfeit product and (2) harms and benefits associated with counterfeit product. Conclusions The use of counterfeit or contaminated substances represents a public health concern. Those who report using performance and image enhancing drugs such as AAS for non-medical purposes report consuming these substances and experiencing harm as a result. Consumers take steps to limit coming into contact with counterfeit or contaminated product, though recognise that many of these have limitations. The implementation of accessible drug safety checking services may provide an opportunity to provide consumers with information to assist them with making healthier choices.

The purposes of this study are twofold. First, it investigates how the digital media discursively react against the use of anabolic-androgenic steroids (AAS). Second, it examines how Thailand is portrayed as a venue for steroid holiday among recreational bodybuilders from the Western countries. Adhering to the principle of theoretical triangulation, the study employs two frameworks including Reisigl and Wodak’s discursive strategies and Van Dijk’s ideological structures. The methodology undertaken is a corpus-assisted discourse analysis. The corpus contains 100 articles published on various websites between the period of 2014 and 2019 with the total number of 107,116 words. Main findings indicate that, regarding the first purpose, the media portray AAS as the ‘outgroup’ by employing various negative lexical items while attempting to dilute any claimed benefits. Concerning the second purpose, Thailand is represented as a venue in which AAS are easily available coupled with corrupt dealers and lax regulations.

Introduction: Nandrolone decanoate is a synthetic testosterone analogue considered one of the most widely used anabolic androgenic steroids (AAS) among adolescents and athletes. Chronic ingestion of AAS increases the incidence of cardiovascular abnormalities in athletes, but the mechanism that causes these changes remains unknown. Objectives: The purpose of this study is to verify the possible effects of the use of anabolic androgenic steroids (AAS) on the morphology and oxidative metabolism of the heart in exercised and sedentary rats. Methods: This is a comparative prospective level II study. Twenty-four Wistar rats were distributed in groups that performed voluntary (TG) and sedentary (SG) running exercises, and used AAS: the Anabolic Training Group (ATG), and the Anabolic Sedentary Group (ASG). During the three months of the running protocol, the animals received an intramuscular injection of 5 mg/kg b.p. of AAS. After the training period, the rats were euthanized and the hearts were removed for evaluation of lipid peroxidation and antioxidant capacity, and for morphometric analysis. Results: The anabolic groups, ASG (0.3072 ± 0.0531) and ATG (0.2732 ± 0.0413), presented higher lipid peroxidation when compared to the non-anabolic groups SG (0.1705 ± 0.0224) and TG (0.1785 ± 0.0340). Conclusion: There was no change in total antioxidant capacity or in the thickness of the interventricular septum and left ventricular wall. Thus, the use of anabolic androgenic steroids did not cause morphological changes in the myocardium. However it did alter the oxidative metabolism. It was also verified that aerobic exercise had no protective effect against lipid peroxidation in the myocardium caused by the use of AAS. Level of evidence II; Prospective comparative study. Objetivos: O estudo teve como objetivo verificar os possíveis efeitos do uso de esteroides anabólicos androgênicos (EAA) na morfologia e no metabolismo oxidativo do coração de ratos treinados e sedentários. Métodos: Trata-se de um estudo prospectivo comparativo nível II. Vinte e quatro ratos Wistar foram distribuídos em grupos que realizaram exercícios de corrida voluntária (GT) e sedentários (GS) e faziam uso dos EAA, Grupo Treinado com Anabolizante (GTA) e Grupo Sedentário com Anabolizante (GSA). Durante os três meses do protocolo de corrida, os animais receberam injeção intramuscular de 5 mg/kg p.c. de EAA. Após o período de treinamento, houve a eutanásia e remoção do coração dos ratos para avaliação da peroxidação lipídica e capacidade antioxidante, além da análise morfométrica. Resultados: Verificaram-se que os grupos anabolizantes, GSA (0,3072 ± 0,0531) e GTA (0,2732 ± 0,0413), apresentaram maior peroxidação lipídica quando comparados aos grupos não anabolizantes GS (0,1705 ± 0,0224) e GT (0,1785 ± 0,0340). Conclusões: Não houve alteração na capacidade antioxidante total, assim como não houve alteração na espessura do septo interventricular e da parede ventricular esquerda. Portanto, o uso de esteroides anabólicos androgênicos não provocou alterações morfológicas no miocárdio, contudo alterou o metabolismo oxidativo. Verificou-se também que a prática de exercício aeróbico não teve efeito protetor contra a peroxidação lipídica no miocárdio provocada pelo uso dos EAA. Métodos: Se trata de un estudio prospectivo comparativo nivel II. Fueron distribuidos 24 ratones Wistar en grupos que realizaron ejercicios de carrera voluntaria (GE) y sedentarios (GS), y hacían uso de los EAA, Grupo Entrenado con Anabolizante (GEA) y Grupo Sedentario con Anabolizante (GSA). Durante los tres meses del protocolo de carrera, los animales recibieron inyección intramuscular de 5 mg/kg p.c. de EAA. Después del período de entrenamiento, hubo la eutanasia y remoción del corazón de los ratones, para evaluación de la peroxidación lipídica y capacidad antioxidante, además del análisis morfométrico. Resultados: Se verificó que los grupos anabolizantes, GSA (0,3072 ± 0,0531) y GEA (0,2732 ± 0,0413), presentaron mayor peroxidación lipídica cuando comparados a los grupos no anabolizantes GS (0,1705 ± 0,0224) y GE (0,1785 ± 0,0340). Conclusiones: No hubo alteración en la capacidad antioxidante total, así como no hubo alteración en el espesor del septo interventricular y de la pared ventricular izquierda. Platelet-rich plasma (PRP) is widely utilized in the treatment of sports injuries. However, potential systemic effects after localized PRP injection are unclear at present. In this prospective randomized study, 24 Taiwanese male athletes with tendinopathy were randomized into a PRP group (n = 13) or a saline group (n = 11). The concentrations of serum and urine biomarkers were quantified by enzyme-linked immunosorbent assay assessment as well as gas chromatographic and mass spectrometric analysis, respectively. The results showed no significant differences in serum levels of growth hormone, insulin-like growth factor-1, insulin-like growth factor-binding protein 3, vascular endothelial growth factor, platelet-derived growth factor-BB, or serum substance P (SP) between the two groups before intervention, nor at 1, 2, or 7 days after intervention. However, a significant decrease in the serum SP level 1 and 7 days after PRP injection was observed. Regarding urinary concentrations of metabolites of anabolic androgenic steroids (AAS), no between-group differences before intervention, nor at 1, 2, or 7 days after intervention, were observed. Our study failed to observe significant surge of serum anabolic molecules and urinary excretion of anabolic AAS metabolites after PRP injection.

Anabolic androgenic drugs include testosterone and its synthetic steroidal derivatives (also known as anabolic androgenic steroids [AAS]), as well as nonsteroidal selective androgen receptor modulators, which exert tissue-specific androgenic effects by binding to the androgen receptor. As these agents increase muscle mass and strength, they have been widely abused by professional athletes to seek competitive advantage. Recently the use of these compounds has also increased among amateur athletes and also among men who are not athletes but want to enhance their body image. The use of AAS is associated with many adverse effects, some of which are serious and difficult to manage. In this article, we briefly review the types of AAS, the mechanism behind their ergogenic effects, and finally discuss their adverse effects in some detail.

Summary Novel substances of performance have appeared since the 1960s. These substances are synthesized from anabolic androgenic steroids and are presented as designer steroids. They are used by top athletes for doping purposes as they increase lean body mass, strength, aggressiveness and lead to a shorter recovery time between workouts, but also by amateurs to improve their body aesthetic. Indeed, the use of designer steroids enables to improve performance and increase muscle mass. However, an overconsumption can cause health issues such as testicular cancer, hypogonadism or neuro-psychic disorders, including domestic violence. The designer steroids are available on the internet as dietary supplements. Their easy access and the difficulty of their detection is a big challenge for toxicological and forensic laboratories. The use of these drugs is problematic for doping, clinical and forensic aspects. As it is the case with other new psychoactive substances, obtaining reference material can be a challenge for toxicology laboratories. This short review of the literature aims to highlight some issues related to the abuse of designer steroids.

Se propone gestionar la aprobación de una iniciativa de Ley basada en la evidencia para regular la prescripción y venta de esteroides anabólicos androgénicos (EAA) a través de una Reforma a la Ley General de Salud en los artículos 17 Bis fracción VII al artículo 181, proponiendo emitir normas técnicas obligatorias o directivas generales, aplicables a los establecimientos cubiertos o descubiertos, destinados para la práctica de ejercicios o deporte. Las evidencias que sustentan dicha investigación se obtuvieron a través de un Estudio transversal descriptivo, en el cual se aplicó un instrumento en 15 gimnasios para fisicoconstructivismo en la ciudad de Xalapa, Ver. Entre otros datos importantes, se encontró prevalencias de 25% en el uso de EAA, los principales motivos para consumirlos fueron el aspecto personal (52%) y el rendimiento físico (24%). En la gran mayoría de los casos quien prescribió estas sustancias fue el entrenador (63%) y en algunos casos algún compañero del mismo gimnasio. El 71% de quienes dijeron consumir estas sustancias los adquieren en farmacias y un 29% los consume de diseño. It is proposed to manage the approval of an initiative of Law based on evidence to regulate the prescription and sale of anabolic androgenic steroids (EAA) through a Reform to the General Law of Health in articles 17 Bis fraction VII to article 181, proposing issue mandatory technical norms or general directives, applicable to covered or discovered establishments, intended for the practice of exercises or sports. The evidence supporting this research was obtained through a descriptive cross-sectional study, in which an instrument was applied in 15 gyms for physicoconstructivism in the city of Xalapa, Ver. Among other important data, prevalences of 25% were found in the use of EAA, the main reasons for consuming them were the personal aspect (52%) and the physical performance (24%). In the vast majority of cases who prescribed these substances was the coach (63%) and in some cases a partner of the same gym. 71% of those who said they consume these substances acquire them in pharmacies and 29% consume them in design. This panorama represents a serious problem of public health due to the implications that the consumption of doping substances represents for the organism, since the damage to health can range from cancer of the liver and prostate, elevation of blood pressure, acne, psychological disturbances, thickening of the voice, arrhythmias of the heart, nervousness, loss of coordination, aggressive behavior, joint and ligament problems, hair loss, even disfigurement due to excessive bone growth.Keywords: Anabolic Steroids; Prevalence; Initiative of Law.

The use of Anabolic-Androgenic Steroids (AAS) has been associated with increased aggressiveness and violent behavior. We therefore investigated the proposed correlation between the use of AAS and criminality while controlling for important socio-economics covariates and for psychiatric comorbidity.The primary endpoints were prison sentences, and time to first prison sentence. A retrospective matched cohort study design consisting of 545 males, who tested positive for AAS in Danish gyms during the period January 3, 2006 to January 31, 2017. They were matched with 5450 randomly chosen male controls. Data were cross-referenced with national register information on education, employment status, substance abuse and psychiatric comorbidity. In addition, 638 males sanctioned because they rejected to participate in the doping control and 6380 controls were used as a replication cohort.Already at baseline, 20.6% of the AAS users had a previous prison sentence whereas the rate was 3.7% in the control cohort (p 0.0001). During the follow-up period the cumulative prevalence increased to 29.5% and 4.9%, respectively (unadjusted HR 9.15, 95% CI 6.33-13.20). The associations remained highly significant after controlling for socio-economic factors, drug abuse and psychiatric comorbidity. The results could be replicated in a similar cohort.Our study shows that AAS users have a 9-fold increased risk of being convicted of a crime compared to matched controls, randomly chosen from the general population. This association could not be explained by common socioeconomic factors or by psychiatric comorbidity.

Background Few studies have assessed the use of dietary supplements, anabolic androgenic steroids (AAS) and selective androgen receptor modulators (SARM) in male gym users. The comparison of physical appearance with others on social media and the exposure to fitness-related content on social media (i.e., image-centric social media use) may have a profound role in using these compounds due to its role in creating negative body images in male gym users. Objective Provide contemporary data on the use of dietary supplements, AAS and SARM among young male gym users, and test the hypothesis that social media is associated with the use of dietary supplements, AAS and SARM, as a result of a negative body image. Methods In this cross-sectional study, conducted in the Netherlands, male gym users (N = 2269; 24 ± 6 years) completed an online questionnaire including self-reported measures regarding resistance training participation, image-centric social media use, dietary supplement intake, and body image. The prevalence of AAS and SARM use was assessed with randomized response, a technique to ask sensitive questions indirectly. Results Of all participants, 83% used ergogenic dietary supplements (mainly protein and creatine), and an estimated 9 versus 2.7% had ever used AAS versus SARM. Image-centric social media use was positively associated with the use of dietary supplements (r = .26; p p r = .34; p Conclusions The use of dietary supplements in young male gym users is exorbitant, with the use of AAS and SARM being substantial. Image-centric social media use is positively associated with the use of dietary supplements and AAS.

Background First identified in 1935, testosterone is the principal hormone controlling the development of androgenic-masculinizing effects in the male body, along with its anabolic properties that increase lean muscle mass. Objectives We aim at studying the influence of anabolic androgenic steroids on the semen parameters and serum sex hormonal levels in Egyptian male bodybuilders. Patients and Methods The type of study was cross sectional study. Study setting was conducted in Dermatology, Venereology and Andrology Department, Ain Shams University Hospitals. The study included 20 male body builders using AAS as the study group and 20 body builders who exercise only and not using AAS. 20 subjects who exercise only without androgenic anabolic steroids use. Results In the present study, there were statistical significant differences between ASS group and control group as regard semen volume, sperm concentration, total sperm count, sperm agglutination and rounded cells. Bodybuilders in ASS group had significantly lower semen volume, sperm concentration, total sperm count; and higher liquefaction, sperm agglutination, and rounded cells. Conclusion AAS has a negative impact on semen quality and sex hormone levels among male bodybuilders. In the present study, AAS users exhibited significantly lower plasma testosterone levels and increase FSH and decrease LH hormone higher frequencies of abnormal semen parameters than healthy control participants. These results should provoke concern, particularly because the use of anabolic steroids by exercising men for muscle building is based on self-designed personal schemes or is suggested by a partner, by field magazines, or by trainers at the gyms. Further, large-scale, studies are still needed to confirm our findings.

Background: The use of anabolic androgenic steroids (AAS) is not uncommon among strength athletes. Several cross-sectional studies have linked AAS use to heart disease, but a causal role for AAS is not certain and it is unknown whether cardiac changes are reversible.Methods: Men of at least 18 years old intending to start an AAS cycle on short notice were included for comprehensive 3D echocardiographic examination before (T0), at the end of the cycle (T1), and 1 year after inclusion (T2) after a recovery period. Details of the AAS cycle performed and the use of other performance and image-enhancing drugs (PIEDs) as well as illicit drug use were recorded. Trend analysis and multivariable regression analysis were performed with mixed effects linear models.Results: Thirty-one subjects were included. Between start (T0) and end of the cycle (T1), after a median AAS cycle duration of 16 weeks, 3D left ventricular ejection fraction declined with 4.9% (CI −7.2 to −2.5, P < 0.001), E/A-ratio declined with−0.45 (CI −0.69 to −0.21, P < 0.001), and 3D left atrial volume increased with 9.2 ml (CI 2.9–15.4, P = 0.004). Left ventricular mass increased with 28.3 g (CI 14.2–42.4, P < 0.001) and was positively correlated with AAS average weekly dose. After a median recovery time of 8 months (T2), all parameters returned to baseline.Conclusion: AAS induce left ventricular hypertrophy and impaired systolic and diastolic function in amateur strength athletes. The structural cardiac changes are positively associated with AAS dose and complete recovery occurred after AAS were discontinued.

International audience; Background: Left ventricular (LV) remodeling, characterized by increased LV hypertrophy and depressed function, is observed in strength-trained athletes who use anabolic-androgenic steroids (AAS). Previous studies reported an increase in cardiac fibrosis in these athletes, which could promote intraventricular dyssynchrony. In this context, this study evaluated LV dyssynchrony in strength-trained athletes using AA; hypothesizing that the use of AAS leads to an increase in LV dyssynchrony with an increase in post-systolic shortening.Methods: Forty-four participants (aged 20-40 years) were divided into three age-matched groups: strength-trained athletes using AAS (users, n=14) and those who were not (non-users, n=15), and healthy sedentary men (controls, n=15),. After completing a survey, each participant was assessed with 2D-strain echocardiography. Left ventricular dyssynchrony was quantified using the standard deviation of the time to peak for longitudinal strains (SD), the longitudinal strain delay index (LSDI) and the segmental post-systolic index (PSI).Results: Users exhibited a greater LV mass index and higher systolic and diastolic functions than both controls and non-users. The decrease in LV strains in users was predominantly observed at the interventricular segments. The SD, LSDI and PSI, calculated on the basal inferoseptal, basal anteroseptal and basal inferolateral segments, were higher in users.Conclusion: The results strongly support that the specific LV remodeling observed in young AAS users was associated with an increase in LV dyssynchrony. The correlations with ejection fraction suggested that wasted energy, due to post-systolic shortenings, contributed in part to the decrease in LV function in strength-trained athletes using AAS.

Misuse of anabolic-androgenic steroids (AAS) to boost performance and appearance is rising in the United States (US) with approximately one million people experiencing dependence. Long-term AAS misuse can lead to cardiovascular pathology but rarely cardiogenic shock. We report the case of an acute ischemic stroke secondary to an intracardiac thrombus in a patient with biventricular failure and cardiogenic shock associated with AAS misuse. Discontinuation of AAS and institution of guideline-directed medical therapy might reverse AAS-related heart failure.

Background Emerging research has suggested that the COVID-19 pandemic has had some impact on substance use patterns. The aim of the study was to conduct a rapid survey to assess the impact of COVID-19 on performance and image enhancing drug (PIED) use and training, and any subsequent negative physical or mental health outcomes. Methods During 2020, a convenience sample of 60 PIED consumers (mean age = 26.69; 68.3% located outside Australia) completed a quantitative anonymous online survey exploring how the coronavirus pandemic impacted patterns of PIED use and associated exercise habits. The survey was administered via the Qualtrics platform and distributed online through PIED forums as well as through the investigators’ networks. Participants were asked about their PIED use and exercise habits prior to and during restricted movement ‘lockdowns’. Results During pre-COVID, the majority of the sample opted to ‘blast-cruise’ (an initial high dose, followed by a lower maintenance dose; 71.7%, n = 43). During lockdown, 45% (n = 27) reported a change in PIED use as a result of the restrictions. In light of health concerns during COVID-19, a majority of men (60%, n = 36) did not take any extra precautions relating to their PIED use. A subgroup of men ceased using PIEDs completely (16.7%, n = 10) with the majority (80%, n = 8) of that subgroup following post-cycle therapy (PCT) of some kind. Only a small proportion of the sample reported negative mental health issues as a result of PCT access issues. Conclusions This study contributes to the emerging literature of the impact of the COVID-19 pandemic on substance use, specifically PIED use among men. The results suggest that the pandemic did influence the choice of PIEDs that participants consumed, although there was little disruption to patterns of exercise, an important aspect of PIED use. Of the men who did cease use completely, the majority reported little issue with PCT access; those who reported difficulty accessing PCT compounds indicated experience some mental health concerns related to ceasing their PIED use. Clinicians and those who come into contact with this group should be alert for any negative physical or mental health concerns resulting from disrupted or ceased PIED use.

Androgens, also referred to as anabolic-androgenic steroids (AAS), are a cluster of hormones comprising natural androgens, such as testosterone, as well as synthetic steroidal derivatives. Performance and image enhancing drugs (PIEDs) are categorised based on their reason for use: AAS, the focus of this chapter and the most widely used PIED, promote growth of the skeletal muscle, development of male sexual characteristics in both males and females, reduce body fat and can improve overall well-being. Muscle dysmorphia is not only a gateway to commencing AAS use but also feeds dependence as individuals are petrified of losing their muscular physique. The use of AAS can cause many adverse effects ranging from the mild to life-threatening. AAS contribute to the development of cardiovascular disease by increasing low-density lipoprotein (LDL) and decreasing high-density lipoprotein (HDL), an effect seen soon after starting AAS.

The effects of androgen anabolic steroids (AAS) use on athletes’ cardiac autonomic activity in terms of baroreflex sensitivity (BRS), and heart rate variability (HRV) have not yet been adequately studied. Furthermore, there is no information to describe the possible relationship between the structural and functional cardiac remodeling and the cardiac autonomic nervous system changes caused by AAS abuse. Thus, we aimed to study the effects of long-term AAS abuse on cardiac autonomic efficacy and cardiac adaptations in strength-trained athletes. In total, 80 strength-trained athletes (weightlifters and bodybuilders) participated in the study. Notably, 40 of them using AAS according to their state formed group A, 40 nonuser strength-trained athletes comprised group B, and 40 healthy nonathletes (group C) were used as controls. All subjects underwent a head-up tilt test using the 30 min protocol to evaluate the baroreflex sensitivity and short HRV modulation. Furthermore, all athletes undertook standard echocardiography, a cardiac tissue Doppler imaging (TDI) study, and a maximal spiroergometric test on a treadmill to estimate their maximum oxygen consumption (VO2max). The tilt test results showed that group A presented a significantly lower BRS and baroreflex effectiveness index than group B by 13.8% and 10.7%, respectively (p &lt, 0.05). Regarding short-term HRV analysis, a significant increase was observed in sympathetic activity in AAS users. Moreover, athletes of group A showed increased left ventricular (LV) mass index (LVMI) by 8.9% (p &lt, 0.05), compared to group B. However, no difference was found in LV ejection fraction between the groups. TDI measurements indicated that AAS users had decreased septal and lateral peak E’ by 38.0% (p &lt, 0.05) and 32.1% (p &lt, 0.05), respectively, and increased E/E’ by 32.0% (p &lt, 0.05), compared to group B. This LV diastolic function alteration was correlated with the year of AAS abuse. A significant correlation was established between BRS depression and LV diastolic impairment in AAS users. Cardiopulmonary test results showed that AAS users had significantly higher time to exhaustion by 11.0 % (p &lt, 0.05) and VO2max by 15.1% (p &lt, 0.05), compared to controls. A significant correlation was found between VO2max and LVMI in AAS users. The results of the present study indicated that long-term AAS use in strength-trained athletes led to altered cardiovascular autonomic modulations, which were associated with indices of early LV diastolic dysfunction.

Case A 36-year-old male competitive powerlifter sustained asynchronous bilateral Achilles tendon ruptures after using 2 types of selective androgen receptor modulators (SARMs). Both tendon ruptures occurred near the myotendinous junction and were treated with open surgical repair and an initial period of immobilization followed by progressive weightbearing and rehabilitation; no postoperative complications were observed. Conclusion Previous studies have reported that anabolic androgenic steroids have a deleterious impact on tendon structure and function. This case suggests that SARM compounds may also predispose users to Achilles tendon rupture.

Introduction: Misuse of anabolic androgenic steroids (AAS) has become a widespread health problem with subsequent multisystem adverse effects. However, among their adverse effects, renal toxicity remains one of the least discussed.Aim of work: To study the histopathological effects of Nandrolone Decanoate (ND) abuse on renal cortex and examine the possible protective role of taurine. Materials and methods: Thirty-six adult male albino rats, aging 4-6 months and weighing 200 - 250 gms, were used in this study. Animals were divided equally into three groups; Group I (control group): subdivided into Subgroup IA: negative control and Subgroup IB: rats received intramuscular 0.5ml sesame oil twice weekly for six weeks and Subgroup IC: received 100 mg/kg/day Taurine by gavage, for six weeks. Group II (ND Group): received intramuscular 5 mg/kg ND, dissolved in sesame oil, twice weekly for six weeks. Group III (Taurine and ND Group): given both Taurine and ND in the same dose and duration as subgroup IC and group II respectively. At the end of the experiment, venous blood samples were collected from the tail vein for biochemical assays. Kidneys were dissected out and processed for light and electron microscopic examination. Results: Examination of group II revealed marked congestion, obliteration of Bowman’s space and lumina of both proximal and distal tubules. Ultrastructural distortion was evident in glomerular filtration barrier and tubular lining cells. Statistically significant biochemical changes were also observed. However, group III showed almost preserved glomerular and tubular architecture with few persistent changes. Biochemical results also revealed marked improvement. Conclusion: Marked renal cortical affection with renal functional deterioration were noted following ND. Taurine exerted marked reno-protection thus, it would be also of great help to protect AAS users against possible renal hazards in case their AAS use was inevitable.

Anabolic androgenic steroids (AASs) are often used by individuals engaged in physical recreational activity. AASs inhibit the hypothalamus-pituitary-gonad axis and can cause erectile dysfunction and reduced fertility. There is no data on the use of AASs in this category of people in the Russian Federation; therefore, a study exploring the rate and patterns of using steroids for non-medical purposes is topical. Aim - of this study was to investigate the rate and patterns of using AASs in males attending gyms in Saint Petersburg.We used individual anonymous postal survey of males attending gyms. We analyzed demographic and anthropometric data, information on the use of AASs, awareness of their side effects, used agents, patterns and duration of their use, and rehabilitation therapy.Out of 1,815 sent questionnaires, we received back 762 ones. The criteria were met by 550 questionnaires. The mean age was 29.3±7.4 years. The use of AASs was reported by 30.4% of respondents. The main AAS (74.3%) consumers were males aged 22 to 35 years. The most popular drug was Testosterone Propionate (51.5%); the drug was often combined with Oxandrolone (19.7%). In 70.6% of cases, drugs were administered by injection or injection combined with tablet intake. The injectable testosterone dose ranged from 500 to 2,000 mg/week and above. The most common dose was 1,000 mg/week (23.9%). AAS administration for more than 1 year was reported in 16.1% of males. Anastrozole (55%), hCG (51.3%), Clomiphene (41.3%), and Tamoxifen (30.5%) were used during the recovery period. The main source of information on AASs, doses, and dosage patterns was the Internet (48.7%). A negative attitude towards AASs was found in 17.3% of respondents. The desire to receive qualified information about AASs and their impact on health was reported by 54.8% of the surveyed respondents.Almost every fourth gym visitor has experience in using AASs. These are males of an optimal reproductive age. The common pattern of using AASs is an aggressive steroid course followed by a recovery period. The list of used drugs and their doses indicate a significant pharmacological intervention and a high risk to health.Обоснование. Анаболические андрогенные стероиды (ААС) нередко применяют лица, занимающиеся физической рекреационной активностью. ААС ингибируют ось гипоталамус-гипофиз-гонады и могут быть причиной развития эректильной дисфункции и снижения фертильности. В РФ отсутствуют данные о распространенности использования ААС среди этой категории людей, поэтому исследование, раскрывающее частоту и схему использования стероидов в немедицинских целях, является актуальным. Цель исследования — изучение распространенности и стереотипа применения ААС среди мужчин, посещающих тренажерные залы Санкт-Петербурга. Материал и методы. Индивидуальное заочное анонимное анкетирование мужчин, занимающихся в тренажерных залах. Изучали демографические, антропометрические данные, сведения о применении ААС, информированности о побочных эффектах их применения, используемых препаратах, схемах и длительности их применения, восстановительной терапии. Результаты. Из 1815 розданных анкет получено 762. Соответствовало критериям 550 анкет. Средний возраст составил 29,3±7,4 года. На применение ААС указали 30,4% респондентов. Основными потребителями ААС были мужчины (74,3%) в возрасте от 22 до 35 лет. Самым востребованным препаратом был тестостерона пропионат (51,5%), его чаще комбинировали с Оксандролоном (19,7%). В 70,6% случаев методом введения препаратов был инъекционный или комбинированный с приемом таблетированных препаратов. Диапазон доз инъекционного препарата тестостерона — от 500 до 2000 мг/нед и выше. Наиболее распространенная дозировка — 1000 мг в неделю (23,9%). Применяли ААС более 1 года 16,1% мужчин. В восстановительном периоде использовались Анастразол (55%), чХГ (51,3%), Кломифен (41,3%), Тамоксифен (30,5%). Основным источником информации об ААС, дозах и схемах применения был указан Интернет (48,7%). Негативное отношение к ААС сформировано у 17,3% респондентов. О желании получить квалифицированную информацию об ААС и их влиянии на здоровье высказались 54,8% принявших участие в анкетировании. Заключение. Почти каждый 4-й посетитель тренажерных залов имеет опыт применения ААС. Это мужчины оптимального репродуктивного возраста. Стереотипом использования ААС было агрессивное применение стероидов определенными курсами с последующим восстановительным периодом. Перечень используемых препаратов, их дозы указывают на значимое фармакологическое вмешательство и высокий риск для здоровья.

Abstract. Anabolic Androgenic Steroids (AAS) usage has been repeatedly linked with a body image disorder called Muscle Dysmorphia (MD). However, evidence for how this relationship manifests is currently inconclusive. This study focused on the extent to which MD may precipitate or perpetuate the use of AAS. Utilizing a cross-sectional design, the sample consisted of 57 users (current and lifetime) and 51 non-using (never used AAS) bodybuilders recruited from two bodybuilding dedicated gymnasiums located in Wales, UK. Significantly higher levels of MD were found in users compared to non-users across training experience duration (0–2, 3–5, and 6+ years). MD levels irrespective of AAS usage (users vs. non-users) were consistent between 0–2 and 3–5 years of training declining however between those of 6+ years training experience. No differences were reported in symptoms of MD between users of AAS with varying lengths of AAS use exposure. This provides preliminary evidence suggesting MD may influence both initiation and maintenance of AAS use; however, neither regular gym attendance nor prolonged AAS usage may exacerbate MD symptoms.

Aim To identify differences in brain morphology between dependent and non‐dependent male anabolic–androgenic steroid (AAS) users. Design This study used cross‐sectional data from a longitudinal study on male weightlifters. Participants Oslo University Hospital, Norway. Setting Eighty‐one AAS users were divided into two groups; AAS‐dependent (n = 43) and AAS‐non‐dependent (n = 38). Measurements Neuroanatomical volumes and cerebral cortical thickness were estimated based on magnetic resonance imaging (MRI) using FreeSurfer. Background and health information were obtained using a semi‐structured interview. AAS‐dependence was evaluated in a standardized clinical interview using a version of the Structured Clinical Interview for DSM‐IV, adapted to apply to AAS‐dependence. Findings Compared with non‐dependent users, dependent users had significantly thinner cortex in three clusters of the right hemisphere and in five clusters of the left hemisphere, including frontal, temporal, parietal and occipital regions. Stanozolol is an anabolic-androgenic steroid which is commonly abused by athletes for improved energy, appearance, and physical size. It has been previously shown to cause changes in behaviour and has various physical effects. Studies have previously been conducted on its neurotoxic effect on the central nervous system (CNS), which are typically psychological in nature. This study was performed to investigate the apoptotic effect of stanozolol on different parts of the rat hippocampus. Sixteen male Wistar rats were divided randomly into two groups (experimental and control). The experimental group received subcutaneous injections of stanozolol (5mg/kg/day) for consecutive 28 days, whereas the control group received saline using the same dosing schedule and administration route. After routine procedures, coronal sections of rat brain were stained with Toluidine blue and TUNEL for pre-apoptotic and apoptotic cell detection, respectively. In order to compare groups, the mean number of TUNEL-positive and pre-apoptotic neurons per unit area were calculated and analysed. Histopathological examination revealed that the mean number of pre-apoptotic and apoptotic neurons in the CA1, CA2, CA3 and DG areas of the hippocampus were significantly increased in the stanozolol treated group. In conclusion, stanozolol abuse may induce pre-apoptotic and apoptotic cell formation in different regions of the hippocampus.

Anabolic androgenic steroids (AAS) are synthetic compounds derived from testosterone, which are widely used in supraphysiological doses by people seeking an aesthetic effect. The objective of the present experiment was to evaluate the possible morphometric changes in the cardiac left ventricle caused by the administration of supraphysiological doses of the anabolic steroids testosterone cypionate and stanozolol in the hearts of young sedentary mice, to serve as a comparative parameter with young mice that were submitted to exercise. We have used 60 hearts of sedentary young Swiss mice, aged ∼ 90 days old (young-adult), with a body weight between 40 and 50 g. The animals were divided into three groups: the control group, the testosterone cypionate group, and the stanozolol group. For the analysis, 10 distinct sections of the apex, of the middle region, and of the base of the heart were selected, followed by an optical microscope measurement with a 2.5x magnification. The results obtained show an increase in both myocardial thickness and left ventricular cavity diameter in the two groups of male animals in relation to the control group; however, in females, an increase in the thickness of the left ventricular myocardium was observed only for the stanozolol group. These results suggest that the cardiac alterations observed in the present study may be directly related to some signs and symptoms already described in the literature, such as hypertension, arrhythmias, infarction, sudden death, and other cardiovascular diseases.

The use of anabolic androgenic steroids (AAS) has grown into a worldwide substance abuse problem over the last decades, with the doses taken by illegal users being 10 to 100 times higher than the therapeutic ones. In the present experiment, 60 mice were divided into 3 groups of 20 animals. Group 1 received testosterone cypionate (Deposteron [EMS, São Bernardo do Campo, SP, Brazil]); group 2 received stanozolol (Stanozolol Depot, Landerlan, Lambaré, Paraguay), and group 3 received saline solution), each one composed by 10 males and 10 females, treated once a week and put to swimming thrice a week for 2 months. After euthanasia, their chests were opened, the hearts removed and processed histologically for morphometric analyses. The specimens were cut into 6 different sections and each one was measured with the help of an optical microscope with a 40-fold magnification. For such analyses, the Axiovision Rel. 4.8.2 (Carl Zeiss Microscopy LLC, Peabody, MA, USA) and Axiovision 4 Module Interactive Measurement (Carl Zeiss Microscopy LLC) software were used. The results showed that there was an increase in the diameter of the left ventricles in the male mice treated with Deposteron while in the female animals treated with Winstrol, there was a decrease in the left ventricular diameter in relation to the other two groups. Thus, one can conclude that the use of supraphysiological doses of the given AAS significantly alters the ventricular diameter in both male and female animals, which can cause a considerable change in both heart rate and blood pressure, and potentially induce disorders that are very relevant to the organism.

Summary Objective Urinary steroids profiling was used to monitor the intake of endogenous anabolic androgenic steroids by athletes. However these profile can be influenced by different factors such as age, gender, ethnicity, bacterial and microorganism activities in urine. This paper discussed the difference of steroids profile between different populations and how reliable the cut-off values set by WADA to the steroids profile data obtained from present study. Equipment and methods The urine samples were taken from 126 healthy and non-dope athletes which represent different genders, ethnicities and athletic status. The extraction procedures consisted of the solid phase extraction, enzymatic hydrolysis, liquid-liquid extraction and derivatization with trimethylsilyl. All the samples were analysed using gas chromatography coupled with mass spectrometry detector. Results The comparison with other population (African, Caucasian and Hispanic) within the same sport genre showed a lower level of urinary steroids profile of Malaysian athlete population. Furthermore, the cut-off values set by WADA were too high compared to the data obtained from this study. Conclusion The lower level of steroids parameter of Malaysian athlete population could be hypothesised due to the polymorphism in the gene coding for UGT2B17, 5α reductase, 5β-reductase and/or hydroxysteroid dehydrogenases (HSD). The development of cut-off values by WADA needs to consider samples from various populations to ensure the validity of the values worldwide.

Background: One sub-population potentially affected by the COVID-19 pandemic are strength athletes who use anabolic-androgenic steroids (AAS). We examined links between disruption in AAS use and training due to the pandemic and mental health outcomes in this population, hypothesising: (a) the pandemic would be linked with reduced training and AAS use; and (b) athletes perceiving greater impact on their training and AAS use would report increases in detrimental mental health outcomes.Methods: Male strength athletes using AAS (N = 237) from 42 countries completed an online questionnaire in May 2020. A sub-sample (N = 90) from 20 countries participated again 4 months later. The questionnaire assessed pre-pandemic and current AAS use and training, alongside several mental health outcomes.Results: At Time 1, most participants perceived an impact of the pandemic on AAS use (91.1%) and/or training (57.8%). Dependent t-tests demonstrated significant reductions in training frequency (t = 7.78; p < 0.001) and AAS dose (t = 6.44; p < 0.001) compared to pre-pandemic. Linear regression showed the impact of the pandemic on training was a significant positive predictor of excessive body checking (B = 0.35) and mood swings (B = 0.26), and AAS dose was a significant positive predictor of anxiety (B = 0.67), insomnia (B = 0.52), mood swings (B = 0.37). At Time 2, fewer participants perceived an impact of the pandemic on AAS use (29.9%) and/or training (66.7%) than at Time 1. Training frequency (t = 3.02; p < 0.01) and AAS dose (t = 2.11; p < 0.05) were depressed in comparison to pre-pandemic. However, AAS dose had increased compared to Time 1 (t = 2.11; p < 0.05). Linear regression showed the impact of the pandemic on training/AAS use did not significantly predict any mental-health outcomes. However, AAS dose was a significant negative predictor of depressive thoughts (B = −0.83) and mood swings (B = −2.65).Conclusion: Our findings showed impact of the pandemic on the training and AAS use, reflected in reduced training frequency and AAS dose. However, whilst we detected some short-term consequential effects on mental health, these did not appear to be long-lasting.

Background: Anabolic androgenic steroids (AAS ) abuse is considered as a public issue as a result of their widespread use to enhance muscular building .Aim of the work: The aim of the study is to clarify the hazards effects that occurred in the skeletal muscles of adult male albino rats as a result of administration of Nandrolone Decanoate histologically as well as immunohistochemically and the possible role of the interaction between (AAS) and training exercise . Materials and Methods: Forty adult male albino rats were divided randomly into four groups; group I (control group ) , group II (steroids treated group ), group III (trained group ) and group IV (steroid treated-trained group) . Both groups II and IV were administered 5 mg/kg body weight, of Nandrolone Decanoate twice a week by intramuscular injection for 5weeks. Group III and Group IV were subjected to training protocol in the form of 4 sets of 10 jumps into water for 5 weeks . At the end of the experiments the rats were sacrificed and their quadriceps muscles were processed for histological and immunohistochemically procedures .Results: Group II (steroid treated group) had hypertrophy of muscle fibers and disrupted striations with wide spacing between them. In addition to areas of degeneration and congested blood vessels. Group III had hypertrophy of muscle fibers. Group IV exhibited normal histological appearance of the muscle fibers. Minute areas of focal degeneration and congested blood vessels were observed.Conclusion: Administration of (AAS) was presented with noticeable degenerative changes in the quadriceps muscles of the adult male albino rats . These hazards effects could be attenuated in the group of rats that were subjected to (AAS) in concomitant with training exercise.

Several performance-enhancing or ergogenic drugs have been linked to both significant adverse cardiovascular effects and increased cardiovascular risk. Even with increased scrutiny on the governance of performance-enhancing drugs (PEDs) in professional sport and heightened awareness of the associated cardiovascular risk, there are some who are prepared to risk their use to gain competitive advantage. Caffeine is the most commonly consumed drug in the world and its ergogenic properties have been reported for decades. Thus, the removal of caffeine from the World Anti-Doping Agency (WADA) list of banned substances, in 2004, has naturally led to an exponential rise in its use amongst athletes. The response to caffeine is complex and influenced by both genetic and environmental factors. Whilst the evidence may be equivocal, the ability of an athlete to train longer or at a greater power output cannot be overlooked. Furthermore, its impact on the myocardium remains unanswered. In contrast, anabolic androgenic steroids are recognised PEDs that improve athletic performance, increase muscle growth and suppress fatigue. Their use, however, comes at a cost, afflicting the individual with several side effects, including those that are detrimental to the cardiovascular system. This review addresses the effects of the two commonest PEDs, one legal, the other prohibited, and their respective effects on the heart, as well as the challenge in defining its long-term implications.

Understanding of the choices and motivations of people who use anabolic androgenic steroids (AAS) for muscular enhancement has increased greatly in the past thirty years, along with understanding of a wide range of health harms associated with this form of drug use in the community. During this period the predominant public health intervention for this population in the UK has consistently remained the provision of injecting equipment to prevent blood borne virus (BBV) transmission. The study explored the health professionals’ and other stakeholders’ perceptions on: whether the current UK public health response is sufficient to address the needs of people who use AAS, and if not, what other needs they might have. This included an exploration of whether there were gaps in harm reduction strategies or other behavioral outcomes and interventions that were needed. Interviews with 27 stakeholders who provide support to people who use AAS in a variety of roles established consensus on the need for a range of interventions to reduce harm and risk in those that choose to use AAS, to prevent initiation, to motivate and support cessation, and to prevent relapse. Study findings indicate that while providing sterile injecting equipment remains essential, it should be considered a bare minimum. The challenge is to develop and deliver a range of harm reduction interventions that look beyond BBV prevention to provide appropriate support to who choose to use AAS at all points in their cycles of use and ultimately for those choosing the temporary or permanent cessation of use.
