Tension pneumothorax is a life-threatening condition that can develop when either the visceral pleura is disrupted, or with injury to the tracheobronchial tree. Rapid, accurate diagnosis and appropriate management are required to prevent significant atelectasis, hypoxia, circulatory arrest, and ultimate patient demise. Needle decompression is the current standard of care for the management of tension pneumothorax. Healthcare providers struggle to assess the success of decompression due to a lack of any immediate objective feedback. The gaseous composition of tension pneumothorax is similar to that of end respiratory gas. This includes an increased partial pressure of carbon dioxide in comparison to atmospheric air, which makes colorimetric capnography an ideal confirmatory test. This colorimetric capnography device may help the healthcare providers to make an objective and accurate assessment of the success of the needle decompression, in particular in prehospital environments.

Introduction Plasma thrombin generation kinetics as measured by the calibrated automated thrombogram (CAT) assay is a predictor of symptomatic venous thromboembolism after trauma. We hypothesized that data from a new prototype assay for measurement of thrombin generation kinetics in fresh whole blood (near patient testing of thrombin generation), will correlate with the standard CAT assay in the same patients, making it a potential tool in the future care of trauma patients. Methods Patients were enrolled from June 2018 to February 2020. Within 12 hours of injury, blood samples were collected simultaneously for both assays. Tension pneumothorax is a common cause of mortality in trauma. Tension pneumothorax is the confinement of respired gases within the pleural cavity at increasing pressure resulting in hemodynamic collapse. Decompression is crucial in management. Emergency needle thoracostomy is a life-saving maneuver that allows atmospheric pressure equilibration and partial restoration of cardiac filling. Needle decompressions are usually performed under noisy, tense, and stressful circumstances, and objective assessment of success is difficult in the field. A device which is simple that objectively informs operators of successful decompression would be clinically useful. In previous work, we have demonstrated end-expiratory gas and gaseous composition of tension pneumothorax are similar due to increased carbon dioxide partial pressure relative to atmospheric gas composition. Therefore, a simple solution to objective needle decompression may be colorimetric capnography.We report a case of 58-year-old male treated by EMS following a motorcycle accident with left-sided chest pain, hypoxia, hypotension, and clinical findings of tension pneumothorax. Needle decompression with colorimetric capnography using the device indicated decompression of his tension pneumothorax, with appropriate temporizing success.

Key Points Question What factors contribute to cost conversations about anticoagulation treatment between patients with atrial fibrillation and their clinicians, and what outcomes are associated with these conversations? Findings In this cohort study of 830 audiovisual recordings of encounters and participant surveys from a randomized trial comparing atrial fibrillation care with and without a shared decision-making (SDM) tool, cost conversations were associated with the use of an SDM tool, with middle-income patients, and with consultations conducted by female primary care staff clinicians. Cost conversations were associated with patients’ decision-making processes but not final treatment choice. Meaning These findings suggest that SDM tools may inform efforts to promote cost conversations in practice, an important consideration when increasing costs of care are being passed on to patients., This cohort study, a secondary analysis of a randomized clinical trial, assesses factors contributing to cost conversations about care plans between clinicians and patients with atrial fibrillation., Importance How patients with atrial fibrillation (AF) and their clinicians consider cost in forming care plans remains unknown. Objective To identify factors that inform conversations regarding costs of anticoagulants for treatment of AF between patients and clinicians and outcomes associated with these conversations. Design, Setting, and Participants This cohort study of recorded encounters and participant surveys at 5 US medical centers (including academic, community, and safety-net centers) from the SDM4AFib randomized trial compared standard AF care with and without use of a shared decision-making (SDM) tool. Included patients were considering anticoagulation treatment and were recruited by their clinicians between January 30, 2017, and June 27, 2019. Data were analyzed between August and November 2019. Main Outcomes and Measures The incidence of and factors associated with cost conversations, and the association of cost conversations with patients’ consideration of treatment cost burden and their choice of anticoagulation. Results A total of 830 encounters (out of 922 enrolled participants) were recorded. Patients’ mean (SD) age was 71.0 (10.4) years; 511 patients (61.6%) were men, 704 (86.0%) were White, 303 (40.9%) earned between $40 000 and $99 999 in annual income, and 657 (79.2%) were receiving anticoagulants. Clinicians’ mean (SD) age was 44.8 (13.2) years; 75 clinicians (53.2%) were men, and 111 (76%) practiced as physicians, with approximately half (69 [48.9%]) specializing in either internal medicine or cardiology. Cost conversations occurred in 639 encounters (77.0%) and were more likely in the SDM arm (378 [90%] vs 261 [64%]; OR, 9.69; 95% CI, 5.77-16.29). In multivariable analysis, cost conversations were more likely to occur with female clinicians (66 [47%]; OR, 2.85; 95% CI, 1.21-6.71); consultants vs in-training clinicians (113 [75%]; OR, 4.0; 95% CI, 1.4-11.1); clinicians practicing family medicine (24 [16%]; OR, 12.12; 95% CI, 2.75-53.38]), internal medicine (35 [23%]; OR, 3.82; 95% CI, 1.25-11.70), or other clinicians (21 [14%]; OR, 4.90; 95% CI, 1.32-18.16) when compared with cardiologists; and for patients with an annual household income between $40 000 and $99 999 (249 [82.2%]; OR, 1.86; 95% CI, 1.05-3.29) compared with income below $40 000 or above $99 999. More patients who had cost conversations reported cost as a factor in their decision (244 [89.1%] vs 327 [69.0%]; OR 3.66; 95% CI, 2.43-5.50), but cost conversations were not associated with the choice of anticoagulation agent. Conclusions and Relevance Cost conversations were common, particularly for middle-income patients and with female and consultant-level primary care clinicians, as well as in encounters using an SDM tool; they were associated with patients’ consideration of treatment cost burden but not final treatment choice. With increasing costs of care passed on to patients, these findings can inform efforts to promote cost conversations in practice. Trial Registration ClinicalTrials.gov Identifier: NCT02905032

BACKGROUND Reliable biomarkers predictive of venous thromboembolism (VTE) after acute trauma are uncertain. The objective of the study was to identify risk factors for symptomatic VTE after trauma, including individual plasma coagulome characteristics as reflected by thrombin generation. METHODS In a prospective, case-cohort study, trauma patients were enrolled over the 4.5-year period, 2011 to 2015. Blood was collected by venipuncture into 3.2% trisodium citrate at 0, 6, 12, 24, and 72 hours after injury and at hospital discharge. Platelet poor plasma was stored at -80 °C until analysis. Thrombin generation, as determined by the calibrated automated thrombogram (CAT) using 5 pM tissue factor (TF)/4 μM phospholipid (PS), was reported as peak height (nM thrombin) and time to peak height (ttPeak [minutes]). Data are presented as median [IQR] or hazard ratio with 95% CI. RESULTS Among 453 trauma patients (injury severity score = 13.0 [6.0, 22.0], hospital length of stay = 4.0 [2.0, 10.0] days, age = 49 [28, 64] years, 71% male, 96% with blunt mechanism, mortality 3.2%), 83 developed symptomatic VTE within 92 days after injury (35 [42%] after hospital discharge). In a weighted, multivariate Cox model that included clinical and CAT characteristics available within 24 hours of admission, increased patient age (1.35 [1.19,1.52] per 10 years, p < 0.0001), body mass index ≥30 kg/m (4.45 [2.13,9.31], p < 0.0001), any surgery requiring general anesthesia (2.53 [1.53,4.19], p = 0.0003) and first available ttPeak (1.67 [1.29, 2.15], p < 0.00001) were independent predictors of incident symptomatic VTE within 92 days after trauma (C-statistic = 0.799). CONCLUSION The individual's plasma coagulome (as reflected by thrombin generation) is an independent predictor of VTE after trauma. Clinical characteristics and ttPeak can be used to stratify acute trauma patients into high and low risk for VTE. LEVEL OF EVIDENCE Prognostic, level III.

BackgroundNew‐onset diabetes mellitus after a pancreaticoduodenectomy (PD) remains poorly defined. The aim of this study was to define the incidence and predictive factors of immediate post‐resection diabetes mellitus (iPRDM).MethodsRetrospective review of patients undergoing PD from January 2004 through to July 2010. Immediate post‐resection diabetes mellitus was defined as diabetes requiring pharmacological treatment within 30 days post‐operatively. Logistic regression was conducted to identify factors predictive of iPRDM.ResultsOf 778 patients undergoing PD, 214 were excluded owing to pre‐operative diabetes (n= 192), declined research authorization (n= 14) or death prior to hospital discharge (n= 8); the remaining 564 patients comprised the study population. iPRDM occurred in 22 patients (4%) who were more likely to be male, have pre‐operative glucose intolerance, or an increased creatinine, body mass index (BMI), pre‐operative glucose, operative time, tumour size or specimen length compared with patients without iPRDM (P < 0.05). On multivariate analysis, pre‐operative impaired glucose intolerance (P < 0.001), pre‐operative glucose ≥ 126 (P < 0.001) and specimen length (P= 0.002) were independent predictors of iPRDM. A predictive model using these three factors demonstrated a c‐index of 0.842.DiscussionNew‐onset, post‐resection diabetes occurs in 4% of patients undergoing PD. Factors predictive of iPRDM include pre‐operative glucose intolerance, elevated pre‐operative glucose and increased specimen length. These data are important for patient education and predicting outcomes after PD.

Increased demands on time to market and new technology introductions create challenges for package reliability testing to be as efficient and comprehensive as possible within a limited time frame. Further, new package technologies and their interactions pose additional reliability risk with the need to be carefully evaluated. This paper will analyze this trend and identify solutions in the form of better and more efficient upfront methodologies such as potential problem analysis, delta analysis, detailed statistical analysis, and daisy chain/ test to failure methodologies. Future trends will also be evaluated.

Background Damage-control laparotomy (DCL) is a lifesaving operation used in critically ill patients; however, interval primary fascial closure remains a challenge. We hypothesized that flaccid paralysis of the lateral abdominal wall musculature induced by botulinum toxin A (BTX) would improve rates of primary fascial closure, decrease duration of hospital stay, and enhance pain control. Methods Consenting adults who had undergone a DCL at two institutions were prospectively randomized to receive ultrasound-guided injections of their external oblique, internal oblique, and transversus abdominus muscles with either BTX (150 mL, 2 U/mL) or placebo (150-mL 0.9% NaCl). Patients were excluded if they had a body mass index of greater than 50, remained unstable or coagulopathic, were home O2 dependent, or had an existing neuromuscular disorder. Outcomes were assessed in a double-blinded manner. Univariate and Kaplan-Meier estimates of cumulative probability of abdominal closure were performed. Results We randomized 46 patients (24 BTX, 22 placebo). There were no significant differences in demographics, comorbidities, and physiologic status. Injections were performed on average 1.8 ± 2.8 days (range, 0-14 days) after DCL. The 10-day cumulative probability of primary fascial closure was similar between groups: 96% for BTX (95% confidence interval [CI], 72-99%) and 93% for placebo (95% CI, 61-99%) (HR, 1.0; 95% CI, 0.5-1.8). No difference between BTX and placebo groups was observed for hospital length of stay (37 days vs. 26 days, p = 0.30) or intensive care unit length of stay (17 days vs. 11 days, p = 0.27). There was no difference in median morphine equivalents following DCL. The overall complication rate was similar (63% vs. 68%, p = 0.69), with two deaths in the placebo group and none in the BTX group. No BTX or injection procedure complications were observed. Conclusion The use of BTX after DCL was safe but did not seem to affect primary fascial closure, hospital length of stay, or pain modulation after DCL. Given higher-than-expected rates of primary fascial closure, Type II error may have occurred. Level of evidence Therapeutic study, level III.

With the increasing complexity of the power amplifier (PA) module architecture, the probability of a thermally induced stress related failure mechanism increases. To help evaluate the increase in module complexity, a more sophisticated in situ monitored thermal cycle reliability test is available. The module is monitored in real time using a resistance daisy chain methodology designed to provide coverage using resistance feedback throughout the entire hierarchy of the module and carrier board interface. Monitored temperature cycling allows for real time failure feedback and enhanced failure signature information. Further, the testing technique has proven to be a valuable method for capturing the early stages of a module mechanical failure at the temperature extremes. Moreover, statistical evaluation of the failure data (Weibull analysis) is improved and better accuracy of the failures in time (FIT) rate can be determined.

The increase in sophistication of reliability testing over the past several years is in part driven by the increase in complexity of package architecture it is meant to evaluate. Movement from a pass/ fail centric suite of testing toward a trend of increased design for reliability and potential problem analysis driven testing criteria is evident. A facet of this is the increased emphasis on test to failure approaches for certain package related tests such as drop shock testing. Additionally, there is an increased importance for extracting more meaningful data from standard fixed duration tests such as temperature cycling. In-situ monitored reliability testing for both drop shock and temperature cycling is an effective method for evaluating package technologies from a test to failure and immediate failure feedback perspective.

The two sides of trauma-induced coagulopathy, the hypocoagulable and the hypercoagulable states, are poorly understood. To identify potential mechanisms for venous thromboembolism and bleeding after acute trauma, we estimated changes in circulating procoagulant microparticles (MPs) and thrombin activity during hospitalization for trauma.Whole blood was collected by venipuncture into 3.2% trisodium citrate at 0, 6, 12, 24, and 72 hours after injury and discharge. Platelet-poor plasma was harvested and stored at -80°C until analysis. Thrombin generation was determined using the calibrated automated thrombogram (CAT), reported as lag time (minutes), peak height (nM thrombin), and time to reach peak height (ttPeak, minutes). The concentration of total procoagulant MPs (number/μL) was measured by flow cytometry. Data are presented as median (interquartile range [IQR]).Among 443 trauma patients (1,734 samples; Injury Severity Score [ISS], 13.0 [IQR, 6.0-22.0]; hospital length of stay, 4.0 days [IQR, 2.0-10.0]; age, 48 years [IQR, 28-65]; 70.7% male; 95% with blunt mechanism; mortality, 3.2%), no discernable patterns in thrombin generation or MP concentration were observed over time. The peak height and MPs were significantly different from healthy volunteers and were 337 nM (IQR, 285-395) and 400/μL plasma (IQR, 211-772), respectively. Extreme (defined as highest or lowest 5%) values reflecting a possible "hypercoagulable state" (lag time ≤ 1.98, peak height ≥ 486.2, ttPeak ≤ 3.61, and total procoagulant MP ≥ 2,278) were reached within 12 hours after acute trauma, while extreme values representing a possible "hypocoagulable state" (lag time ≥ 18.6, peak height ≤ 17.8, and ttPeak ≥ 29.45) were not reached until 1 day to 3 days.Although there was no predictable pattern of coagulopathy observed in each patient after trauma, those who reached extreme values did so relatively early after injury. These findings should be taken into account when designing risk model tools involving coagulation laboratory parameters.Epidemiologic study, level III.

for relevant studies (Search date Sept 2013). Case control studies with gastrectomy for benign diseases were included. Gastrectomy for malignant conditions was excluded. Standardized forms were used to extract data. Data was extracted by two reviewers independently. Odds ratio was calculated using comprehensive Meta-analysis software. Newcastle-Ottawa Scale (NOS) was used to assess the quality of study. Heterogeneity was also assessed. Random effects model was used for pooling the data. Results: Eleven case-control studies were included in this study (3951 cases/9609 control). Studies were reported from USA (6), Europe (4) and Taiwan (1). Seven studies included hospital based controls and 4 included population based controls. The sample sized ranged from 142 to 720 (cases) and 180 to 2098 (control). The age of the patients ranged from 20 79 yrs. The risk of pancreatic cancer was not increased in patients with prior history of gastrectomy (OR 1.39; 95% CI, 0.95 -2.03, p= 0.09). Subgroup analysis based on the geographical location revealed the risk was not increased in the USA (OR 1.65; 95% CI, 0.96-2.82, p=0.068) or Europe (OR 1.13; 95% CI 0.61-2.08, p=0.07). Subgroup analysis for Asia was not performed as there was only one study. Subgroup analysis based on the design of the study (population based control or hospital based controls) showed no significant difference. As per NewcastleOttawa Scale, nine studies were of high quality (7 or more NOS points) and two were of low quality (6 NOS points). Subgroup analysis based on quality did not show any significant difference. Moderate heterogeneity was present (I square =56%) and thus random effects model was used for analysis. Conclusion: A prior history of gastrectomy for benign gastric disorders is not associated with an increased risk for pancreatic cancer.

Background: Diabetes is a known, but poorly understood sequela of pancreatic resection in a subset of patients. Our aim was to define the incidence and risk factors of postresection diabetes (PRD) after distal pancreatectomy (DP). Methods: A retrospective review of all consecutive patients undergoing DP at our institution from January 2004 through July 2010. Data was obtained from medical records, pathology reports, and postoperative imaging. Postoperative CT scans were evaluated to determine extent of resection, defined by site of transection as limited (adrenal or left), standard (superior mesenteric artery or left to adrenal), extended (neck), subtotal (into head). Comparisons between patients with and without PRD were evaluated using chi-square, Fisher's exact, and Wilcoxon rank sum tests. Univariate and multivariable associations with PRD were evaluated using logistic regression models and summarized with odds ratios and 95% confidence intervals. Results: Of 583 patients undergoing DP, 90 patients with pre-existing type I (n=24) or type II (n=66) diabetes were excluded. The remaining 493 patients represent the study population with a mean age of 57 years and BMI (kg/m2) of 27.4. Operative indications included benign lesions in 253 patients (51%) and malignancy in 240 (49%). Extent of pancreatic resection was limited in 141 patients (37%), standard in 87 (23%), extended in 128 (33%), and subtotal in 30 (8%). Postoperatively, 179 patients (36%) required no postoperative treatment, 269 (55%) required perioperative hyperglycemic control, and 45 (9%) developed diabetes including 34 requiring insulin, and 11 oral hypoglycemic medication.
