Introdução: O domínio da interpretação radiográfica contribui para o correto diagnóstico de alterações do complexo dento-alveolar, e seu aprimoramento durante o período da graduação é essencial. No momento em que se inicia a carreira profissional fora da faculdade, é esperado que o conhecimento teórico adquirido, associado à experiência clínica durante a graduação, melhore a performance diagnóstica do dentista. Objetivo: Esse estudo objetivou avaliar diagnósticos feitos a partir de exames radiográficos intrabucais, realizados por alunos de graduação de uma faculdade de odontologia do sul do Brasil, antes e após a sua introdução na prática clínica. Materiais e métodos: Para tal, diagnósticos de radiografias interproximais e periapicais, feitos durantes o Segundo ano (T0) e Quarto ano (T1) do curso de graduação, foram avaliados. Uma análise descritiva foi expressa e o teste de McNemar foi realizado para comparar as respostas em T0 e T1. Resultados: Quinze alunos completaram os dois questionários. De uma forma geral, a porcentagem de respostas corretas em T0 foi de 70.5%, e em T1 foi de 61.6%, com uma diferença significativa entre elas (p = 0.024). Discussão: Este estudo salienta a necessidade de estratégias educacionais que melhorem a competência diagnóstica durante as atividades clínicas da graduação, uma vez que exames radiográficos são essenciais em todas as áreas da odontologia. Conclusão: Neste estudo, o correto diagnóstico de alterações dentoalveolares a partir de radiografias intraorais apresentou um redução significativa após os primeiros anos de prática clínica., Mastery of interpretation of radiographic images is a contributing factor in correct diagnosis of conditions affecting the dentoalveolar apparatus. It is therefore essential that students’ perfect these skills while studying for their degrees. Once they embark upon their professional careers outside of the university setting, it is expected that the theoretical knowledge and clinical experience accumulated during the course of their studies will have led to improved diagnostic performance. Objective: The objective of this study was to evaluate diagnoses made using intraoral radiographs by undergraduate students at a School of Dentistry in the South of Brazil, before and after their introduction to clinical practice. Materials and methods: Diagnoses made during the second (T0) and fourth (T1) years of the undergraduate course in dentistry using interproximal and periapical radiographs were analyzed. Descriptive statistics were calculated and the McNemar test was used to compare answers at T0 and T1. Results: Fifteen students answered questionnaires at both T0 and T1. The overall percentage of correct answers was 70.5% at T0 and 61.6% at T1, with a significant difference (p = 0.024). Discussion: This study highlights the need for educational strategies that improve diagnostic competence during undergraduate clinical activities, since radiographic examinations are essential in all areas of dentistry. Conclusions: Therefore, it was concluded that the rate of correct diagnosis of dentoalveolar disorders by intraoral radiography decreased significantly after the initial years of clinical training.

Odontogenic Keratocyst (OKC) is a cyst that presents local aggression and a high recurrence; thus, a better understanding of its clinical characteristics and treatment options is required. The aim of this study is to report a case and describe the epidemiology and characteristics of OKCs diagnosed at a referral service for patients with oral lesions. Patients with histopathological diagnosis of OKC were included in the sample over 22 years. The study highlights the demographic and clinical-radiological characteristics, as well as the treatment options. In addition, a case of an extensive OKC in the mandible is reported. 3064 medical records were analyzed and 16 cases of OKC (0.52%) were found, with two recurrences (12.5%). The mean age of the patients was 33.19 years, being the third decade of life more affected (31.25%), with equal prevalence for both sexes. The mandible was more affected (68.75%), mainly its body. The differential diagnosis included the dentigerous cyst (11.54%) and the ameloblastoma (11.54%). The most frequent treatment was the curettage enucleation (50%). In the reported case, enucleation associated with cryotherapy was performed, followed by rehabilitation with xenogen graft and osseointegrated implant. OKC presents a wide spectrum of differential diagnoses due to its non-specific clinical-radiographic characteristics. The case reported shows that treatments such as enucleation may be efficient when accompanied by adjuvant techniques., O Queratocisto Odontogênico (QO) é um cisto que apresenta agressividade local e alta taxa de recorrência, necessitando de um profundo entendimento acerca das características clínicas e opções de tratamento. O objetivo deste estudo é apresentar um caso clínico e descrever a epidemiologia e as características de QOs diagnosticados em um serviço regional de referência a pacientes com lesões bucais. Pacientes com diagnóstico histopatológico de QO foram incluídos na amostra ao longo de 22 anos. O estudo destaca as características demográficas, clínico-radiológicas e tratamento, além de relatar um caso clínico de um extenso QO em mandíbula. Dos 3064 prontuários avaliados retrospectivamente, foram encontrados 16 casos de QO (0,52%), com duas recidivas (12,5%). A média de idade dos pacientes foi de 33,19 anos, com maior frequência na 3 a década de vida (31,25%) e com mesma prevalência para ambos os sexos. A mandíbula foi mais acometida (68,75%), principalmente o corpo. O diagnóstico diferencial incluiu o cisto dentígero (11,54%) e o ameloblastoma (11,54%). A escolha terapêutica mais frequente foi a enucleação seguida de curetagem (50%). No caso relatado, a enucleação associada à crioterapia foi o tratamento de escolha, seguido de reabilitação com enxerto xenógeno e implante osseointegrado. O QO apresenta um amplo espectro de diagnósticos diferenciais devido às suas características clínico-radiográficas inespecíficas. O caso clínico relatado reforça que tratamentos como a enucleação podem ser eficientes quando acompanhados de técnicas adjuvantes.

Keratocystic odontogenic is an intraosseous benign tumor whose prevalence corresponds to 11% of cystic lesions of the maxilla and mandible. Objective: perform an organized review of the literature of a keratocystic odontogenic tumor. Material and Methods: a search of clinical case reports was performed through PubMed database under the following criteria: type of study (case report), in English, only studies in humans, no publication date limit. Results: the relevant data of 85 selected articles were registered. The data collection established that the Keratocystic odontogenic has a predilection for male patients, between the third and fourth decades of age. Commonly occurs in the jaw, preferably in posterior region. The most frequent initial clinical aspect was swelling. Regarding imaging aspects, features as a single or multiple radiolucent image, uni or multilocular. Histopathologic examination is essential for diagnosis. Enucleation was the therapeutic approach established in most cases. In cases of multiple lesions, the Gorlin-Goltz syndrome should be investigated. The follow-up time, the rate of recurrence and malignant transformation were variable. Final considerations: histopathological examination it is essential to stablish a definitive diagnosis of keratocystic odontogenic. The most appropriate therapeutic approach is enucleation associated with an adjuvant treatment. Periodic follow-up is mandatory given the high potential of recurrence of this type of lesion., O ceratocisto odontogênico (CO) é um tumor benigno intra-ósseo de origem odontogênica cuja prevalência corresponde a 11% das lesões císticas da maxila e mandíbula. Objetivo: realizar uma revisão da literatura sobre o CO. Metodologia: Foi realizada uma busca de relatos de caso clínico na base de dados PubMed sob os critérios: tipo de estudo (relato de caso), na língua inglesa, realizados apenas em humanos, sem limites de data de publicação. Resultados: Selecionou-se 85 artigos dos quais foram registrados os dados relevantes sobre os casos clínicos. Constatou-se que o CO possui predileção por pacientes do sexo masculino, entre a terceira e quarta décadas de idade. Ocorre comumente em mandíbula, preferencialmente em região posterior. O aspecto clínico inicial mais frequente foi aumento de volume. Em relação aos aspectos imaginológicos, pode apresentar imagem radiolúcida única ou múltipla, uni ou multilocular. O exame histopatológico é fundamental para o diagnóstico. Teve enucleação como conduta terapêutica instituída na maioria dos casos. Em casos de múltiplas lesões, a investigação voltada para Síndrome de Gorlin-Goltz deve ser considerada. O tempo de acompanhamento, a taxa de recorrência e de transformação maligna foram variáveis. Considerações finais: É fundamental o exame histopatológico para o diagnóstico do CO. A conduta terapêutica mais indicada é a enucleação associada a outro tratamento adjuvante. O acompanhamento periódico é mandatório visto o alto potencial de recorrência deste tipo de lesão.

The aim of this study was to evaluate the correlation among chronological age, sex and stages of mineralization of third molars using orthopantomograms in a Brazilian population. For such purpose, 1,200 orthopantomograms of 9-20-year-old individuals (654 females and 546 males) were examined using as reference the Nicodemo’s table of mineralization chronology of permanent teeth among Brazilians. Estimated mean ages, standard deviations, standard errors and percent age distribution in each mineralization stage were established. Mann-Whitney and Kruskal-Wallis (p≤0.05) nonparametric tests were used to verify the influence of sex in the stages of mineralization of the third molars. The development of maxillary and mandibular and right and left third molars was similar in all stages. Females presented a more advanced stage of mineralization of third molars than males. Statistically significant difference in third molar development between males and females was found in stage 4 in teeth 18 (p=0.008), 28 (p=0.029), 38 (p=0.010) and 48 (p=0.046). The results suggest that the development of third molars is similar in both sexes, regardless of the quadrant of dental arch and the tooth mineralization stage table employed in this study was applicable for using third molars as parameters for age estimation in the studied population.

Cephalometric analysis is used by dentists to assist in the diagnosis, planning and follow-up of orthodontic, orthopedic and surgical treatments. However, cephalometry is not a precise tool, and there are significant errors in the measures obtained by the same or different examiners. Objectives: The purpose of this study was to evaluate the reproducibility of cephalometric measures obtained by three specialists in oral radiology, and to compare them with the results obtained by three radiology clinics. Materials and Methods: Examiners traced cephalograms from 39 conventional cephalograms from the sample used by Silveira e Silveira (2006). ANOVA revealed good reproducibility of 17 of the 32 factors in the analyses conducted by dental specialists. Results: Agreement between dental specialists was found for 53.1% of the factors, whereas agreement for the results obtained by radiology clinics was only 12.5%. Conclusion: Results showed that tracings by the 3 radiology specialists had greater agreement than those by the radiology clinics under study., Objetivo: A análise cefalométrica é utilizada por dentistas no auxílio ao diagnóstico, planejamento e acompanhamento dos tratamentos ortodôntico, ortopédico e cirúrgico. Entretanto, a cefalometria não é uma ferramenta precisa e há erros significativos nas medidas obtidas por examinadores diferentes ou pelo mesmo avaliador em momentos distintos. O objetivo deste estudo foi avaliar a reprodutibilidade das medidas cefalométricas de três especialistas em radiologia odontológica e compará-las com os resultados obtidos por três clínicas de radiologia. Materiais e métodos: Os examinadores traçaram os cefalogramas de 39 telerradiografias convencionais da amostra utilizada por Silveira e Silveira (2006). Resultados: O teste de ANOVA revelou uma boa reprodutibilidade para 17 dos 32 fatores nas análises realizadas pelos dentistas especialistas. Houve concordância entre os especialistas para 53,1% dos fatores, enquanto que para as clínicas de radiologia a concordância foi de somente 12,5%. Conclusão: Os resultados mostraram que os três especialistas em radiologia obtiveram maior concordância nas análises cefalométricas realizadas do que aquelas recebidas pelas clínicas de radiologia estudadas previamente.

Purpose: To identify and quantify the frequency of the projection of the oblique line over the alveolar crests of the mandibular molars in periapical radiographs. Methods: Five hundred periapical radiographs of the mandibular molar region were selected and evaluated for the presence or absence of the oblique line. When present, its interference on the image of the alveolar crest of the mandibular molars was analyzed. The data obtained was expressed as a percentage of the overlap of the oblique line to the alveolar crests of the mandibular molars. Results: The oblique line was observed in 363 out of the 500 periapical radiographs (72.6%). Out of the 363, 308 (84.8%) showed an overlap of the oblique line over the alveolar crests. However, 55 radiographs (15.1%) did not show such interference. Of the radiographs showing an overlap (308), the incidence of an overlap was 64.6% at a single site (199 radiographs), 33.4% at two sites (103 radiographs), and 1.9% (6 radiographs) at three sites. Conclusion: The overlap of the oblique line over the image of the alveolar crests in the mandibular molar region was observed in a significant portion of the total sample. Objetivo: Identificar e quantificar a frequência da projeção da linha oblíqua sobre as cristas alveolares dos molares inferiores em radiografias periapicais. Metodologia: Foram selecionadas 500 radiografias periapicais da região de molares inferiores e avaliada a presença ou a ausência da linha oblíqua. Quando presente, foi analisada a interferência desta estrutura sobre a imagem das cristas alveolares dos molares inferiores. Os dados obtidos, considerando a frequência da sobreposição da linha oblíqua sobre as cristas alveolares dos molares inferiores foram expressos em percentagens. Resultados: Nas 500 radiografias periapicais avaliadas, a linha oblíqua foi observada em 363 radiografias (72,6%). Nestas, em 308 (84,8%), houve sobreposição da linha oblíqua sobre as cristas alveolares. Entretanto, em 55 radiografias (15,1%) não foi encontrada interferência. Nas radiografias em que houve a sobreposição da linha oblíqua sobre as cristas alveolares (308), a incidência de sobreposição em apenas um local foi de 64,6% (199 radiografias); em dois locais foi de 33,4% (103 radiografias) e em três locais foi de 1,9% (6 radiografias). Conclusão: A sobreposição da linha oblíqua sobre a imagem das cristas alveolares na região de molares inferiores representou uma fração significativa do total da amostra.

OBJECTIVE: to compare assessment of panoramic radiographs using Mandibular Cortical Index (MCI) by a radiologist with experience and two dentists with no experience in this index using original radiograph images and inverted images.METHODS: 64 panoramic radiographs were analyzed by three observers: a radiologist with previous experience in MCI and two trained dentists with no previous experience. First, the original images were assessed by the radiologist and defined as the gold standard. Then, dentists received training in the MCI. Assessment results obtained from the three observers were analyzed.RESULTS: When compared results from the experienced radiologist to positive and inverted images, no statistical significant difference was found. When considering the two trained observers, we found that inverted images had a lower agreement between the gold standard original images.CONCLUSION: We concluded that, within the limitations of this study, inverted radiographs are not recommended for MCI assessment. OBJETIVO: Comparar a avaliação de radiografias panorâmicas usando o Índice Cortical Mandibular (ICM) por um radiologista com experiência e dois cirurgiões dentistas sem experiência neste índice usando imagens de radiografia originais e imagens invertidas.METODOLOGIA: 64 radiografias panorâmicas foram analisadas por três observadores: um radiologista com experiência prévia com o ICM e dois dentistas treinados sem experiência prévia. Inicialmente, as imagens originais foram avaliadas pelo radiologista e definidas como padrão-ouro. Então, os dentistas receberam treinamento para aplicar o ICM. Os resultados da avaliação obtidos dos três observadores foram analisados.RESULTADOS: Qquando comparados os resultados do radiologista experiente em imagens positivas e invertidas, não foi encontrada diferença estatística significante. Ao considerar os dois observadores treinados, descobrimos que as imagens invertidas apresentaram menor concordância com o padrão ouro.CONCLUSÃO: Concluímos que, dentro das limitações deste estudo, as radiografias invertidas não são recomendadas para a avaliação do ICM.

A análise cefalométrica é um dos exames utilizados para o diagnóstico e planejamento de um tratamento ortodôntico. Com o objetivo de avaliar o uso que os profissionais fazem da cefalometria, foram enviados questionários a todos os especialistas em Ortodontia e Ortopedia Facial do estado do Rio Grande do Sul. Os resultados mostram que a maioria dos profissionais (96,4%) solicita o exame ao início de todos os tratamentos ortodônticos, apenas a metade (50,4%) solicita-o no final. Ainda, que a maioria (88,3%) valoriza mais o perfil do paciente no estabelecimento do plano de tratamento e apenas 19,2% dos profissionais declararam conferir os cefalogramas recebidos das clínicas. A partir dos resultados conclui-se que a maioria dos especialistas em Ortodontia e Ortopedia Facial no Estado do Rio Grande do Sul solicita a análise cefalométrica computadorizada Padrão USP ao início de todos os tratamentos ortodônticos, considera o perfil do paciente determinante para a escolha da conduta a ser seguida e não questiona os cefalogramas enviados pelas clínicas de radiologia., Cephalometric analysis is one of the approaches employed to diagnose and plan orthodontic treatment. Aiming to assess the manner orthodentists utilize cephalometry, questionnaires were sent to all specialists on Orthodontics and Facial Orthopedics working in the state of Rio Grande do Sul, south Brazil. The results show that the exam is requested by the majority of the professionals (96.4%) as of the beginning of orthodontic treatments, and by only half (50.4%) at the end of treatment. Also, most orthodontists (88.3%) put great value to the patient profile in defining treatment strategies and only 19,2% reported checking the cephalograms they receive. The results afforded to conclude that most specialists on Orthodontics and Facial Orthopedics working in Rio Grande do Sul request computerized cephalometric analysis according to the USP standard at the beginning of all orthodontic treatments, and considers patient profile as a determining factor in the choice of the approach to the treatment. Moreover, cephalograms produced by radiography centers are not mistrusted.

Purpose The aim was to evaluate the reproducibility of a radiographic technique using a new customized film holder in the follow-up of screwed single-tooth implants. Materials and methods Ten implants placed in the mandible (n = 6) and maxilla (n = 4) of 8 patients were submitted to radiographic assessments. Radiographs were taken using a customized film holder adapted to implant abutment after 4 (T0) and 8 (T1) months of implant placement. Quantitative evaluations were performed through horizontal and vertical measurements to analyze radiographic standardization. Results No differences were detected after comparison of implant and abutment diameter measurements between radiographs after 4 and 8 months. No significant variations were detected in vertical measurements (implant and abutment length) between radiographs obtained in T0 and T1. Conclusions The use of customized film holder adapted to the implant abutment seems to contribute to the standardization of radiographs at different times of screwed single-tooth implants.

Forensic or scientific practices may require producing orthopantomography of dry skulls, that lack soft tissue, therefore modifications on the usual technique should be necessary. With the objective of standardizing the postmortem digital and analogue panoramic radiography this research produced a single a digital radiography and 13 analogue panoramic images, using different exposure configurations and four different objects to try to overcome the effects of the lack of soft tissue radiation absorption on the analog radiographs. A gel based ice pack, a 20 mm dental wax block, and a 0.5 mm metallic foil were placed in different points of the device. Also a previously exposed and developed film was placed inside the cassette between the unexposed film and the intensifying screen. The images were presented to 15 dentists of several specialties, who graded and ranked the images. In conclusion, the digital panoramic was easily obtained at a 60 kV and 2 mA settings and on analog equipments the use of two films inside the cassette achieved best results with 63 kV and 06 mA exposure settings.
