For many decades, pediatric bronchoscopy has been an integral part of the diagnosis and treatment of acute and chronic pulmonary diseases in children. Rapid technical advances have continuously influenced the performance of the procedure. Over the years, the application of pediatric bronchoscopy has considerably expanded to a broad range of indications. In this comprehensive and up-to-date guideline, the Special Interest Group of the Society for Pediatric Pneumology reviewed the most recent literature on pediatric bronchoscopy and reached a consensus on a safe technical performance of the procedure.

Childhood arterial ischemic stroke differs in essential aspects from adult stroke. It is rare, often relatively unknown among laypersons and physicians and the wide variety of age-specific differential diagnoses (stroke mimics) as well as less established care structures often lead to a considerable delay in the diagnosis of stroke. The possible treatment options in childhood are mostly off-label. Experiences in well-established acute treatment modalities in adult stroke, such as thrombolysis and mechanical thrombectomy are therefore limited in children and only based on case reports and case series. The etiological clarification is time-consuming due to the multitude of risk factors which must be considered. Identifying each child's individual risk profile is mandatory for acute treatment and secondary prevention strategies and has an influence on the individual outcome. In addition to the clinical neurological outcome the residual neurological effects of stroke on cognition and behavior are decisive for the integration of the child into its educational, later professional and social environment.

Background Appropriate size selection of pediatric rigid bronchoscopes is fundamental to avoidance of airway trauma and to a high success rate with the first intubation attempt. The aim of the present study was to compare the outer diameters of pediatric rigid bronchoscopes with the anatomical data on the pediatric airway. Methods Outer diameters (OD) of pediatric rigid ventilating bronchoscopes as given by the manufacturer were compared with published computed tomography internal diameters (ID) of the cricoid outlet, the trachea as well as the left and right proximal main stem bronchus, using six published recommendations for age-related size selection. The ratios between a specific OD and ID were calculated and given as percentage values (%) for lower and upper age ranges. Results Nominal sized 2.5, 3, 3.5, 3.7, 4, 4.5, 5 and 6 rigid bronchoscopes were included. In five recommendations the rigid bronchoscopes` OD was larger than the internal cricoid diameter in the upper age range, whereas in one recommendation the ratio (OD rigid bronchoscope to ID of cricoid) ranged between 77% and 90% for the upper age and between 95% and 109% for the lower age range. In only one of the six recommendations was the rigid bronchoscope OD larger than the tracheal ID. Ratios for the left and right proximal mainstem bronchus ranged from 70% to 146% for the upper and from 78% to 156% for the lower age range, with the highest ratios for the left proximal mainstem bronchus. Conclusion Based on this in vitro study most of the recommendations analyzed result in a balanced fit of rigid bronchoscopes within the cricoid. Since the left mainstem bronchus is considerably smaller than the cricoid, any insertion on this level will require careful endoscopic guidance to avoid damage to the left mainstem bronchus.

Pediatric Pulmonology publishes original research, case reports and review articles on topics related to a wide range of children's respiratory disorders. In this article (Part 1 of a series), we summarize the past year's publications in our major topic areas, as well as selected literature in these areas from other journals. In Part 1, we review selected articles on diagnostic testing/endoscopy, respiratory complications of neuromuscular disorders, and rare lung diseases.

ZusammenfassungDie vorliegende Leitlinie ist eine Neufassung und Aktualisierung der Leitlinie zur Diagnostik und Therapie von Patienten mit Asthma, welche die bisher für den deutschen Sprachraum gültige Version aus dem Jahr 2006 ablöst. Die Fülle an neuen Erkenntnissen zur Pathophysiologie und zu den Phänotypen von Asthma und das erweiterte Spektrum an diagnostischen und therapeutischen Möglichkeiten bei dieser Erkrankung machte eine Neufassung und Aktualisierung notwendig. Es werden sowohl für Kinder und Jugendliche als auch für Erwachsene mit Asthma die aktuellen, Evidenz-basierten diagnostischen und therapeutischen Empfehlungen dargelegt.

Zusammenfassung Akutes Atemversagen kann auch im Kindesalter mit nichtinvasiver Beatmung (NIV) behandelt werden. In den letzten Jahren wurden die technischen Voraussetzungen der NIV-Beatmungsgeräte für das Kindesalter verbessert, zudem steht heutzutage eine größere Auswahl an konfektionierten nasalen und Full-face-Masken für Kinder ab dem Säuglingsalter als Interface zur Verfügung. Vor allem Kinder mit hyperkapnischem Atemversagen profitieren von der NIV. Evidenz für Vorteile der NIV besteht für die Bronchiolitis, Asthma sowie für das „Acute-on-chronic“-Atemversagen, d. h. ein Atemversagen bedingt durch eine akute Infektion bei einer zugrunde liegenden neuromuskulären Erkrankung, Zerebralparese oder einer chronischen Lungenerkrankung wie der zystischen Fibrose. Nichtinvasive Beatmung ist bei prolongiertem Weaning von invasiver Beatmung und bei Fehlbildungen des oberen Atemwegs hilfreich. Alveoläre Erkrankungen mit vorwiegend hypoxämischem Atemversagen wie ambulant erworbene Pneumonien oder das „acute respiratory distress syndrome“ (ARDS) stellen keine Indikationen für die NIV dar. Aufgrund relevanter möglicher Nebenwirkungen und der Möglichkeit der schnellen Verschlechterung des Gasaustauschs bei NIV-Versagen sollte die NIV auf der pädiatrischen Intensivstation mit der Möglichkeit zu einem raschen Wechsel auf einen invasiven Beatmungszugang stattfinden.

Pediatric Pulmonology continues to publish research and clinical topics related to the entire range of children's respiratory disorders. As we have done annually in recent years, we here summarize the past year's publications in our major topic areas, as well as selected literature in these areas from other core journals relevant to our discipline. This review (Part 1) covers selected articles on sleep, diagnostic testing/endoscopy, respiratory complications of neuromuscular disorders, and rare lung diseases.

PURPOSE To determine whether bag-mask ventilation between induction of anaesthesia and tracheal intubation in children with post-tonsillectomy bleeding reduces the incidence of hypoxaemia and difficult direct laryngoscopy without increasing perioperative respiratory complications. METHODS Medical records, anaesthesia protocols and vital sign data were analysed from February 2005 to March 2017 for patients undergoing anaesthesia for surgical revision of bleeding tonsils. Type of rapid sequence induction and intubation (RSII; classical, ie, apnoeic, vs controlled, ie, with gentle bag-mask ventilation) was noted. Primary outcomes were incidence of moderate and severe hypoxaemia, grade of direct laryngoscopic views as well as occurrence of noted tracheal intubation difficulties. Haemodynamic alterations during RSII and perioperative adverse events such as noted gastric regurgitation, pulmonary aspiration and perioperative pulmonary morbidity were also recorded. RESULTS A classical RSII was performed for 22 surgical revisions in 22 children and a controlled RSII was used for 88 surgical revisions in 81 children. Patients undergoing controlled RSII had less incidence of severe hypoxaemia (1 vs 3; P = .025), better direct laryngoscopic views (P = .048) and less hypertension (5 vs 9; P

Atemwegsinfektionen (AWI) sind die haufigste Indikation fur eine Antibiotikatherapie bei Kindern und Jugendlichen, v. a. im ambulanten Bereich und im kinderarztlichen Notdienst der Kliniken. Die hier vorliegende Konsensusempfehlung verschiedener padiatrischer Fachgesellschaften und des Berufsverbandes der niedergelassenen Kinder‐ und Jugendarzte beschreibt in Kurze die klinischen Symptome und die Diagnostik sowie die Indikationen fur eine leitlinienkonforme antibiotische Therapie von AWI (exklusive Pneumonie). Sie soll Kinder- und Jugendmediziner bei einem restriktiven Antibiotikaeinsatz, bei der Auswahl des bestmoglichen Antibiotikums und im Hinblick auf eine moglichst kurze Therapiedauer unterstutzen. Des Weiteren werden AWI beschrieben, bei denen keine Antibiotikatherapie erfolgen sollte oder bei denen nur sehr selten eine Antibiotikatherapie erforderlich ist.

Zusammenfassung Hintergrund Begrenzte Zeit fur den Patienten und allgegenwartige administrative Verpflichtungen kennzeichnen das Arbeitsprofil von klinisch tatigen ArztInnen in der Erwachsenenmedizin. Ziel der Arbeit Pilotuntersuchung des arztlichen Arbeitsprofils in einer Universitatsklinik fur Kinder- und Jugendliche. Material und Methoden Befragung klinisch tatiger Kinder- und JugendarztInnen der Universitat Munchen an einem zufallig ausgewahlten Wochentag durch einen kurzen Fragebogen. Die Zeiterfassung fokussierte auf patientenbezogene Aufgaben einerseits und verwaltungsbezogenen Aufgaben andererseits. Die unterschiedliche klinische Erfahrung und die unterschiedlichen Arbeitsbereiche der Padiatrie (Station, allgemeine (Notfall-) Ambulanz, spezialisierte Hochschulambulanz, integriertes Sozialpadiatrisches Zentrum) wurden berucksichtigt. Ergebnisse Tagesarbeitszeiten von uber 8,5 Stunden wurden von 91,7% der Kinder- und JugendarztInnen angegeben. Der prozentuale Anteil patientenbezogener Zeit lag bei 31,2% (95%-KI = [25,2–37,1]). Die Zeit pro Patient im direkten Patientenkontakt variierte je nach Einsatzort von 14 Minuten pro Patient im stationaren Bereich bis zu 52 Minuten pro Patient im integrierten Sozialpadiatrischen Zentrum. Fur die korperliche Untersuchung (als der direkte klinische Zugang zum Kind) wurden 3 bis 10 Minuten pro Patient und Tag eingesetzt. Die unterschiedliche klinische Erfahrung der Kinder- und JugendarztInnen generierte keinen wesentlichen Unterschied fur die berichteten Zeitkontingente. Zusammenfassung Im realen Arbeitsalltag einer Universitatsklinik fur Kinder und Jugendliche werden ca. 30% der taglichen Arbeitszeit fur patientenbezogene Aufgaben eingesetzt. Daraus resultieren kurze Zeiteinheiten fur die korperliche Untersuchung und den direkten Arzt-Patientenkontakt.

Aspiration (Larynx, Trachea oder Bronchien) bzw. Ingestion von Fremdkorpern (Hypopharynx, Osophagus, Magen oder Dunndarm) gehoren zu den relativ haufigen Verdachtsdiagnosen in der padiatrischen Akutmedizin. Sie sind mit einer relevanten pra- und innerklinischen Morbiditat und Mortalitat verbunden. Betroffen sind meist Kinder im Alter zwischen 6 Monaten und 4 Jahren. Ziel der vorgestellten S2k-Leitlinie gemas der AWMF – Arbeitsgemeinschaft der Wissenschaftlichen Medizinischen Fachgesellschaften ist die strukturierte Beschreibung der wesentlichen organisatorischen, diagnostischen und therapeutischen Aspekte fur die sichere, interdisziplinare Versorgung von Kindern nach Fremdkorperaspiration und -ingestion. Hierbei sind folgende 3 Aspekte essenziell: daran denken (Anamnese, Reevaluation), im Zweifel endoskopieren und das Vorgehen interdisziplinar absprechen.

Der Schlaganfall im Kindesalter ist ein Notfall mit hoher Mortalitat und Morbiditat. Er hat – in Abhangigkeit von der zugrunde liegenden Atiologie – ein klinisch bedeutsames Rezidivrisiko. Die Atiologie ist vielfaltig. Vaskulopathien, Infektionen, kardiale Probleme und Koagulopathien spielen pathophysiologisch eine wesentliche Rolle. Die atiologische Abklarung muss umfassend sein – der kindliche Schlaganfall ist eine „Multiple-Risk-Erkrankung“. Klassische Leitsymptome sind die akute Hemiparese, die faziale Parese und die Sprachstorung: Sie bestehen bei ca. 75 % der betroffenen Kinder. Aufgrund der mannigfaltigen, teils unspezifischen Symptome, der breiten Differentialdiagnosen und des fehlenden Bewusstseins fur den kindlichen Schlaganfall wird die Diagnose mittels Bildgebung oft erst nach einer erheblichen Zeitverzogerung gestellt (im Mittel nach 24 h). Die in speziellen Fallen moglichen Therapien wie die Thrombolyse oder die mechanische Thrombektomie konnen dabei grundsatzlich nur innerhalb weniger Stunden nach dem Ereignis eingesetzt werden – sie sind spezialisierten Zentren vorbehalten. Als neurologisches Residualsymptom steht die Hemiparese im Vordergrund, je nach betroffenem Infarktareal treten cerebrale Krampfanfalle, Sprachstorungen etc. auf. Die Auswirkungen auf Kognition und Verhalten werden in der Versorgungspraxis oft ubersehen und unterschatzt.

Die Bronchoskopie bietet in der Hand des erfahrenen Anwenders und besonders des padiatrischen Pneumologen sowie in der Intensivmedizin und Anasthesie wertvolle diagnostische und therapeutische Moglichkeiten. Es gibt eine grose Zahl von Publikationen zum Indikationsspektrum, zu den Ergebnissen, zu Sicherheit bzw. Nebenwirkungen und zur Sedierung/Anasthesie bei diesem Eingriff, auch seitens der AG Kinderbronchoskopie der Gesellschaft fur Padiatrische Pneumologie (GPP). Wie bei vielen seit Langem etablierten medizinischen Verfahren zu Diagnostik und Therapie existieren jedoch keine kontrollierten Studien im Sinne der evidenzbasierten Medizin zu einzelnen Aspekten der Durchfuhrung, und solche Studien erscheinen aus verschiedenen Grunden (ethische Uberlegungen, Finanzierbarkeit, Akzeptanz durch Eltern) bei etablierten Verfahren auch in Zukunft kaum durchfuhrbar. Es ist das Ziel dieses Positionspapiers, unser heutiges Wissen zusammenzufassen und einen Konsens zu einer rationalen Indikationsstellung und sicheren technischen Durchfuhrung der Bronchoskopie bei Kindern zu formulieren, der als Basis fur die praktische Anwendung dienen soll.

Die Nutzung der In-situ-Simulation als Instrument fur einen Systemcheck neuer Raumlichkeiten bietet die Moglichkeit, potenzielle Fehlerquellen zu erkennen und zu beheben, noch bevor es zu Zwischenfallen mit echten Patienten kommen kann. Mithilfe der Simulationen der zu erwartenden Ablaufe in der Patientenversorgung sollte ein wichtiger Bestandteil des modernen Risikomanagements erfullt werden. In Zusammenarbeit mit dem Institut fur Notfallmedizin und Medizinmanagement (INM) konnte vor der Wiedereroffnung der erweiterten und renovierten Kinderintensivpflegestation (KIPS) der Kinderklinik und Kinderpoliklinik im Dr. von Haunerschen Kinderspital ein 3-tagiges In-situ-Simulationstraining durchgefuhrt werden. An jedem Trainingstag wurden die neuen Raumlichkeiten, die Ausrustung und die Arbeitsablaufe in verschiedenen Szenarien erprobt. Der Fokus wurde auf Systemverbesserungen der neuen Arbeitsumgebung, aber auch auf Aspekte der Teamarbeit gelegt. Durch das Training vor Wiedereroffnung der KIPS konnten potenzielle patientensicherheitsgefahrdende Aspekte aufgedeckt und entsprechende Masnahmen ergriffen werden. Weiterhin konnten die Teilnehmer durch das Simulationstraining in ihren technischen und nichttechnischen Fahigkeiten (Kommunikation, Teamarbeit, Entscheidungsfindung) sowie bei der Zusammenarbeit in interprofessionellen Teams geschult werden. Ihre Handlungssicherheit in den neuen Raumlichkeiten erhohe sich. Das In-situ-Simulationstraining als Instrument fur einen Systemcheck neuer Raumlichkeiten leistet einen wichtigen Beitrag zur Erhohung der Patientensicherheit.

SummaryBackground Tracheal follicular pattern or so-called tracheal cobblestoning is a poorly described entity in the literature and is depicted as a nodular or lumpy appearance of the tracheal wall mucosa suggesting tracheal irritation from factors like gastro-esophageal reflux (GERD) or pulmonary infection. The aim of the present study was to investigate the prevalence and characteristics of tracheal cobblestones in a large pediatric population. Methods A large database of rigid tracheoscopies recorded in children (0–6 years of age) undergoing general anesthesia with planned tracheal intubation for elective surgical procedures was retrospectively analyzed. Auch in der Notfallversorgung von Kindern wird die Ergebnisqualitat von Algorithmen in Tiermodellen, Simulationsstudien mit teilweise hochdiffiziler Technik oder klinischen Studien auf ihre Wirksamkeit und Nebenwirkungen hin uberpruft. Ziel ist es, durch Datenabgleich, Teambildung und Ablaufoptimierung das Outcome bei padiatrischen Notfallbehandlungen zu verbessern, aber auch zusatzlichen Schaden abzuwenden. Rein technische Neuerungen haben bereits Einzug in den klinischen Alltag gehalten. So konnen Geschulte mithilfe einer Bohrmaschine (EZ-IO®) in weniger als 1 min sicher einen i.o.-Zugang anlegen. Die Anwendung von Maskenbeatmung und Larynxmasken zur Behebung der Ateminsuffizienz hat klaren Vorrang gegenuber der Intubation, die ausschlieslich Erfahrenen vorbehalten bleiben sollte. Zusatzlich konnen im Sinne einer Eskalationsstrategie nichtinvasive Beatmungsstrategien zur Anwendung kommen. Das Kompression-Ventilation-Verhaltnis wurde auch bei Kindern zugunsten der Thoraxkompressionen und einer damit verbesserten Koronarperfusion auf 15:2 verschoben. Die medikamentose Reanimation mit Adrenalin sollte ausschlieslich in der Standarddosis zur Anwendung kommen. Eine Hypothermiebehandlung nach erfolgreicher Reanimation zur Neuroprotektion ist nur fur Neugeborene gesichert. Die nasale Medikamentenapplikation kann, dank schneller Resorption, fur die akute Krampfanfallstherapie, bei fehlendem i.v. Zugang auch fur eine Analgosedierung genutzt werden. Auch bei Kindern hat sich in der Behandlung des septischen Schocks die fruhzeitige und aggressive Volumengabe – „early goal directed therapy“ – als sinnvoll erwiesen. „Point-of-care“-Ultraschall hat im innerklinischen Notfallmanagement inzwischen einen festen, unstrittigen Stellenwert. Sowohl Empfehlungen von Fachgesellschaften als auch eine kritische Einordnung anhand der vorhandenen Literatur und eigener Erfahrungen werden bereitgestellt.

BACKGROUND: Tracheal bronchus (TB) is defined as an abnormal bronchus that originates directly from the lateral wall of the trachea above the carina and goes towards the upper lobe territory of the lung. We analyzed rigid endoscopies of the trachea in children to determine the incidence and characteristics of TB. METHODS: In total, 1021 rigid endoscopies of the trachea recorded from children aged 0 to 6 years were analyzed. Endoscopic examination was performed from supraglottic region to carina using a 0-degree Hopkins rod-lens telescope. Patients with a TB were identified and the site of origin of the TB and its level above the carina was noted. Data of the identified patients was reviewed for the presence of preoperative airway findings such as stridor, upper lobe pneumonia and wheezing or atelectasis, other congenital anomalies, and intraoperative complications. RESULTS: TB was detected in 11 (1.06%) of 1021 upper airway endoscopic examinations. All originated from the right lateral wall of the trachea. Six children had retained secretions in the TB, and 3 children had perioperative airway problems unrelated to the TB. One child showed right main stem bronchus narrowing as seen at the true carina, in the presence of a TB. All the children with TB exhibited at least 1 additional congenital anomaly at birth besides TB. CONCLUSIONS: TB is a relatively common congenital endoscopic lower airway anomaly in childhood, which is itself rarely symptomatic, but almost always coexists with other congenital anomalies.

Kommen Kinder in die Notaufnahme, dann meist wegen respiratorischer Notfalle. Diese sind meist infektioser Natur. Durch typische klinische Symptomenkonstellationen lassen sich die verschiedenen Erkrankungen rasch einordnen.

Trotz der Wichtigkeit ist standardisiertes Weaning in der Padiatrie ein bisher zu wenig beachteter Aspekt padiatrischer klinischer Intensivtherapie und Forschung. Erste masige Evidenz aus einzelnen Studien fur Verbesserung des Weaningerfolgs besteht fur die tagliche Evaluation der Weaningfahigkeit, fur die Verwendung von Sedierungsprotokollen, fur das tagliche Unterbrechen von Sedierung nach Abschluss der Akutphase, fur den Einsatz von nicht invasiver Beatmung bei zu erwartendem schwierigenWeaning sowie fur die Gabe von Steroiden zur Prophylaxe von Postextubationsstridor bei Risikopatienten. Aufgrund der in der padiatrischen Intensivmedizin sehr unterschiedlichen Patientengruppen ist jedoch auch in diesen Punkten eine Generalisierung der Daten erschwert.

Hintergrund Die im Kindesalter haufigen respiratorischen Notfalle haben ihre Ursache in der (klein)kindlichen Atemwegsanatomie, der Zahl relevanter Fehlbildungen sowie der Infektanfalligkeit dieser Patientengruppe.

Paediatric airway endoscopy is accepted as a diagnostic and therapeutic procedure, with an expanding number of indications and applications in children. The aim of this European Respiratory Society task force was to produce a statement on interventional bronchoscopy in children, describing the evidence available at present and current clinical practice, and identifying areas deserving further investigation. The multidisciplinary task force panel performed a systematic review of the literature, focusing on whole lung lavage, transbronchial and endobronchial biopsy, transbronchial needle aspiration with endobronchial ultrasound, foreign body extraction, balloon dilation and occlusion, laser-assisted procedures, usage of airway stents, microdebriders, cryotherapy, endoscopic intubation, application of drugs and other liquids, and caregiver perspectives. There is a scarcity of published evidence in this field, and in many cases the task force had to resort to the collective clinical experience of the committee to develop this statement. The highlighted gaps in knowledge underline the need for further research and serve as a call to paediatric bronchoscopists to work together in multicentre collaborations, for the benefit of children with airway disorders.

International audience; BACKGROUND: It has been shown that nontoxic concentrations of ethanol are absorbed after hand hygiene using ethanol-based hand rubs. This study investigated whether absorption of propan-1-ol and propan-2-ol from commercially available hand rubs results in measurable concentrations after use. METHODS: The pulmonary and dermal absorption of propanol during hand rubs was investigated. Rubs contained 70% (w/w) propan-1-ol, 63.14% (w/w) propan-2-ol, or 45% (w/w) propan-2-ol in combination with 30% (w/w) propan-1-ol. RESULTS: Peak median blood levels were 9.15 mg/L for propan-1-ol and 5.3 mg/L for propan-2-ol after hygienic hand rubs and 18.0 mg/L and 10.0 mg/L, respectively, after surgical hand rubs. Under actual surgical conditions, the highest median blood levels were 4.08 mg/L for propan-1-ol and 2.56 mg/L for propan-2-ol. The same procedure performed with prevention of pulmonary exposure through the use of a gas-tight mask resulted in peak median blood levels of 1.16 mg/L of propan-1-ol and 1.74 mg/L of propan-2-ol. CONCLUSION: Only minimal amounts of propanols are absorbed through the use of hand rubs. Based on our experimental data, the risk of chronic systemic toxic effects caused by hand rubs is likely negligible. However, our study did not evaluate the consequences of long-term daily and frequent use of hygienic hand rubs.

Verbruhungen und Verbrennungen nehmen in der Gesamtstatistik kindlicher Unfalle den zweiten Platz ein, dabei sind Verbruhungen mit 80% am haufigsten. Pro Jahr werden etwa 600 Verbrennungsunfalle bei Kindern stationar behandelt, davon 290 in Verbrennungszentren (44 Kinderspezialbetten in Deutschland; Leitstelle 040-2882–3998). Ziel der Erstversorgung sind die Sicherstellung der Vitalfunktionen sowie die Schmerzbekampfung. In den ersten Tagen steht bei groseren Verbrennungen das Management des massiven Flussigkeitsverlusts im Vordergrund. In der Folge treten Wundheilung, Hauttransplantation und Vermeidung bzw. Bekampfung sekundarer Infektionen in den Vordergrund. Durch die moderne Intensivmedizin und die Behandlung in padiatrischen Verbrennungszentren (Versorgung ist in Deutschland flachendeckend sichergestellt) sind Todesfalle auch bei schweren Verbrennungen selten geworden. Es bestehen erhebliche Unterschiede zur Vorgehensweise beim Erwachsenen. Wichtig ist die primare Pravention durch passive Masnahmen und Aufklarung der Eltern.

Background: Home mechanical ventilation (HMV) is increasingly used in children with chronic respiratory insufficiency, but data on incidence and type of adverse events are limited. Setting: Pediatric HMV program at a tertiary university hospital. Methods: The authors retrospectively analyzed the type and incidence of severe emergencies in a mixed pediatric HMV program. Summary Background: Pulmonary alveolar proteinosis (PAP) is a rare alveolar filling syndrome where the mainstay of treatment is therapeutic whole-lung lavage (WLL). WLL techniques used in adults have to be modified for children because of their small-diameter airways. Aim: To describe a technique for WLL adapted for small children. Methods: We describe a WLL technique that combines safe single-lung ventilation with the use of an age-appropriate endotracheal tube and selective occlusion of the other main bronchus with a balloon catheter through which the lavage is performed. Effectiveness measured by change in oxygen requirements and adverse effects was noted. Results: We performed 64 WLL procedures in four children (age 13 months to 7 years; body weight 4.7–14 kg). Two children had idiopathic and two had secondary PAP. At referral, all children had dyspnoea at rest and required continuous oxygen supplementation. Two patients showed significantly decreased oxygen demands and radiological improvement after WLL. Two patients showed no significant response. The only adverse effect observed was transient hypoxemia. Complications comprised fluid leak at the balloon (4), balloon rupture (1), and pneumothorax (1). Conclusions: This technique for WLL combining single-lung ventilation with an endotracheal tube and lung exclusion for lavage with a balloon catheter can be safely and effectively performed in small children with PAP.

Congenital choanal atresia is a rare malformation of the upper airways occurring sole or within the scope of syndromes. The controversy regarding standardized surgical approach and optimal technique preventing re-stenosis as well as the proceeding in case of a unilateral malformation persists. To address these issues, a retrospective analysis of patients presenting with choanal atresia in our department within the last 10 years was performed. 15 infants aged 3 days to 13 years (mean 42 months) who presented with unilateral (10 cases) and bilateral (5 cases) choanal atresia underwent surgical repair. 4 patients failed previous surgeries. All interventions were performed in transnasal endoscopic technique. 7 patients were intraoperatively stented for 6 weeks, 4 patients for 1 week. In 73% (11 out of 15 children) further congenital anomalies were identified. In 7 cases a re-stenosis requiring treatment occurred, predominantly in long-term stented patients. No intra- or postoperative hemorrhages or infections occurred. To provide an adequate work-up of affected patient including a screening for further congenital anomalies, a multidisciplinary approach is required. Endonasal endoscopic approach represents a safe technique for surgical repair of choanal atresia. The indication for surgical repair in case of a unilateral atresia is based on the severity of clinical symptoms. The use of stents, especially of long term, remains controversial and needs further evaluation.

Akute Atemnot gehort zu den haufigsten padiatrischen Notfallsituationen und stellt ein potenziell lebensbedrohliches Krankheitsbild dar. Infolgedessen sind Erkennung, Beurteilung und Behandlung von Atemnot bei Kindern eine sehr haufig geforderte Fahigkeit und wichtige praventive Masnahme. Aufgrund des geringen Durchmessers der kindlichen Atemwege, haufiger Atemwegsinfekte mit Schleimhautschwellung und dem hoheren Sauerstoffbedarf im Vergleich zu Erwachsenen sind Kinder in besonderer Weise fur das Auftreten von Atemnot pradisponiert. Es reicht deshalb oftmals die Beseitigung der respiratorischen Storung aus, um die akute Gefahrensituation zu uberwinden. Im anderen Fall kommt es infolge der respiratorischen Hypoxie im Kindesalter haufig zum Herz-Kreislauf-Stillstand, der rasches Handeln erfordert. In der diagnostischen Einordnung ist die Unterteilung von Krankheiten mit in- bzw. exspiratorischem Stridor oder Giemen zusammen mit dem Alter des Kindes richtungsweisend fur die Verdachtsdiagnose und damit fur das praklinische Prozedere. Dabei ist beim bewusstseinsklaren Kind moglichst wenig invasiv vorzugehen, da zusatzliche Aufregung zur Dekompensation fuhren kann. Die haufigsten Krankheitsbilder, das therapeutische Notfallmanagement und wichtige Differenzialdiagnosen werden anhand eines Algorithmus in diesem Beitrag diskutiert.

Die Synkope – ein kurz dauernder, spontan reversibler Anfall von Bewusstlosigkeit – stellt auch im Kindes- und Jugendalter ein haufiges Krankheitsbild dar. In der praklinischen diagnostischen Abklarung steht die Unterscheidung zwischen harmloseren, neural vermittelten Ohnmachtsanfallen sowie Synkopen aufgrund organischer Erkrankungen im Mittelpunkt. Mit Hilfe einer exakten Anamnese, korperlichen Untersuchung sowie einem Echokardiogramm ist es in vielen Fallen moglich, die Genese naher einzuordnen. Bei Verdacht auf eine organisch bedingte Synkope sind weitergehende Untersuchungen indiziert sowie eine adaquate Behandlung des Grundleidens einzuleiten. Unter den organisch bedingten Synkopen sind vor allem die kardialen Synkopen wegen ihrer hohen Mortalitat hervorzuheben. Die neural vermittelten Synkopen sind in der Regel harmlos und durch einen exzellenten Langzeitverlauf gekennzeichnet. Im Folgenden soll das Krankheitsbild der Synkope in Kindes- und Jugendalter dargestellt werden. Besonderes Augenmerk gilt hierbei der Entscheidungshilfe fur das praktische Vorgehen bezuglich Diagnostik und Therapie.

Auf der Grundlage der Empfehlungen des International Liaison Committee on Resuscitation (ILCOR) und neuer Studien ist u. E. zu empfehlen, die Reanimation eines Neugeborenen im Kreissaal unabhangig vom Gestationsalter – bis auf wenige Ausnahmen – mit Raumluft zu beginnen und nur bei klinischem Bedarf die inspiratorische Sauerstoffkonzentration zu erhohen. Ferner pladieren die Autoren vorerst fur eine Beibehaltung des intrapartalen Absaugens bei mekoniumhaltigem Fruchtwasser und die Anwendung therapeutischer Hypothermie bei reifen und nahezu reifen, asphyktischen Neugeborenen. Insbesondere Mekoniumaspiration, primares Atemnotsyndrom des Fruhgeborenen und ductusabhangige Herzfehler erfordern ein differenziertes, problemorientiertes Vorgehen in OP und Kreissaal, im Rettungsdienst und auf der Intensivstation. Im Folgenden wird auf den schwierigen Atemweg, das Management der Mekoniumaspiration, die Versorgung Neugeborener mit kritischen angeborenen Herzfehlern und die Erstversorgung von Fruhgeborenen

Summary The complex interplay between the characteristics of the respiratory system, its changes as a result of respiratory disease and the effects of mechanical ventilation makes it important to analyse the physiology of the respiratory system in any given clinical situation. Key variables to be considered are the elastic recoil, resistance and time constant of the respiratory system, as well as the characteristics of the respiratory pump. The effects of respiratory support on the intrathoracic organs and lung perfusion must be taken into account. Respiratory support modes can then be tailored to provide maximum benefit and a minimum of negative effects. When the relatively few basic principles that underlie the mechanical behaviour of the respiratory system in most circumstances are kept in mind, the challenge of optimal support seems solvable for most children with respiratory insufficiency.

Background Cough is a frequent symptom in children, but the differentiation of asthmatic cough from cough of other origins can be difficult. Chemokines recruit T lymphocytes to inflamed tissues, and the corresponding chemokine receptors are differentially expressed on T H 1 and T H 2 cells. Objective We sought to determine whether levels of T H 1/T H 2-related chemokines and their receptors differ in bronchoalveolar lavage fluid (BALF) from 12 children with allergic asthma, 15 nonatopic children with chronic cough, and 10 children without airway disease. Methods The T H 1-related (IFN-γ–inducible protein of 10 kd [IP-10], IFN-γ–inducible T cell α chemoattractant [ITAC], monokine induced by IFN-γ [Mig], and IFN-γ) and T H 2-related (thymus- and activation-regulated chemokine [TARC], macrophage-derived chemokine [MDC], IL-5, and IL-4) chemokines and cytokines were quantified in BALF by ELISA and a particle-based multiplex array. Percentages of pulmonary lymphocytes expressing CXCR3 + and CCR5 + (T H 1) and CCR4 + and CCR3 + (T H 2) chemokine receptors were determined in BALF by flow cytometry. Results Pulmonary CCR4 + CD4 + cells and levels of TARC and MDC were significantly increased in asthmatic children versus children with chronic cough or without airway disease. In asthmatic children CCR4 + CD4 + cells correlated positively with levels of TARC, MDC, and serum IgE levels and negatively with FEV 1 . In contrast, CXCR3 + CD8 + cells and levels of ITAC were significantly increased in children with non-atopic chronic cough compared with the other groups. In children with chronic cough, CXCR3 + CD8 + cells correlated with levels of ITAC and IFN-γ. Conclusion Pulmonary CCR4 + CD4 + and CXCR3 + CD8 + cells and their ligands TARC, MDC, and ITAC clearly differentiate asthmatic children from nonatopic children with chronic cough. The analysis of these markers could facilitate the diagnostic discrimination of asthma versus other reasons for chronic cough in children.

Background: Auto-antibodies against granulocyte-macrophage colony stimulating factor (GM-CSF) may be central to the pathogenesis of adult sporadic pulmonary alveolar proteinosis (PAP). The role of anti-GM-CSF auto-antibodies in paediatric forms of PAP is as yet unclear. Methods: Anti-GM-CSF auto-antibodies were determined with the help of an antigen capture assay using serum and/or bronchoalveolar lavage (BAL) fluid from 27 patients with PAP (nine adults, 15 children, three neonates) and from 185 children with different diseases as disease controls (various pulmonary conditions and patients with malignancies). Results: Anti-GM-CSF auto-antibodies were detected in the serum of five of seven adult PAP patients. They were not found in the serum of any of the children or neonates with PAP nor in any of the disease control patients. Raised anti-GM-CSF titres were found in BAL fluid from three of four adult patients with PAP. Anti-GM-CSF auto-antibodies were detected in BAL fluid of only one of the 15 children (age at diagnosis 11 years, age at BAL 24 years) and in none of the neonates with PAP, nor in any of the disease control patients. Conclusions: The presence of anti-GM-CSF auto-antibodies seems to define an autoimmune disease underlying most of the adult sporadic type of PAP, but age at diagnosis may cause an overlap with children in some rare instances. In most of the children and all of the neonates the anti-GM-CSF titres were not significantly increased, indicating that alternative explanations are needed for the pathogenesis of the disease in these patients.

We hypothesised that long-term tracheostomy in infants and children may perpetuate chronic airway inflammation and airway remodeling due to easier access to the lungs for microorganisms. Pulmonary surfactant represents an important part of the initial host defense, and in particular, the surfactant proteins (SP) A and D may directly interact with invading microorganisms and also modulate the activity of local immune cells. The goals of this study were to determine the presence and intensity of a peripheral airway inflammation and of potential deficiency states of surfactant proteins in nonsymptomatic children with tracheostomy. Bronchoalveolar lavage (BAL) cell pattern, bacteria and viruses recovered, and concentrations of SP-A, SP-B, SP-C, and SP-D were assessed in 46 children (4.3 years (1.6–6)) median (range) carrying a tracheostomy for 2.4 years (1.3–4.9), and were compared to 16 children with no lung disease. Children with tracheostomy had an increased total number of cells, increased neutrophils, and more frequently bacteria, but no viruses were recovered. SP-D concentration was reduced by 50% on average (P = 0.0002). SP-A, SP-B, and SP-C were not different between the two groups. SP-D was inversely correlated to neutrophils, and high numbers of bacteria were associated with lower SP-D concentrations. We suggest that bacteria and low SP-D support neutrophilic inflammation in the lower respiratory tract of nonsymptomatic with children with tracheostomy. We report on a patient with the hitherto undescribed combination of chronic granulomatous disease, pulmonary hemosiderosis, and celiac disease. The hemosiderosis resolved with a gluten-free diet and glucocorticosteroid pulse therapy, but the restrictive lung function pattern remained unchanged. Lung function improved markedly by immunosuppression with daily glucocorticosteroid and azathioprine treatment.
