Objective To define the accuracy of robotic assistance in deep brain stimulation surgery and to compare the results in the literature. Methods The data of 10 patients with dyskinesia treated by deep brain electrical stimulation assisted by mechanical arm (Huake precision) were reviewed retrospectively. Postoperative CT review confirmed the comparison between electrode position coordinates, approach angle and preoperative designed target coordinates and approach angle data. The accuracy was analyzed between the intended location of the target and actual location of leads. Results There is no statistically significance between the actual and intended entry angle. Right side X,Y and left side Y coordinates of actual distal contacts were significantly different between the actual and intended entry angle (P < 0.05-0.01). Others had no statistically significance. The euclidian 3D distances between the intended and actual location of the contact were (1.81 ±0.81) mm. Mean radial error was (1.41 ±0.82) mm. Conclusion Robot-assisted technology for stereotactic surgery is safe and accurate. [ABSTRACT FROM AUTHOR]

ObjectiveTo compare subthalamic nucleus (STN) deep brain stimulation (DBS) with globus pallidus interna(GPi) DBS for tremor suppression in Parkinsons disease (PD). MethodsBetween January 2016 and December 2017, 56 PD patients received bilateral DBS in Xuanwu Hospital. Participants were divided into two groups based on different targets. STN group included 20 male and 10 female, mean age was (59.3 ± 10.7) years old. GPi group included 19 male and 7 female, mean age was (61.3 ± 7.2) years old. Patients were assessed before operation and in the following up three months using the Unified PD Rating Scale. ResultsDBS ON versus OFF were compared at med on and med off state and were found a significant difference before and after operation except GPi group med on state. However,the difference between the STN and GPi was not significant. ConclusionsDBS is effective in reducing tremor in PD patients regardless of stimulation target. However, the degree of tremor suppression in STN DBS versus GPi DBS is equivalent. [ABSTRACT FROM AUTHOR]

Objective To analyze the surgical complications of deep brain stimulation ( DBS) for movement disorders ( MD) to explore its reason and prevention and treatment measures .Methods 615 patients with MD were received 1008 DBS electrodes.Of 615 patients, 222 received unilateral DBS surgery and 393 received bilateral DBS surgery .Of 615 patients, 566 were medically refractory Parkinson disease, 18 essential tremor, 11 dystonia, 7 cervical distonia, 9 tourette, 2 Meige syndrome and 2 other hyperkinetic movement disorders .The surgical procedure or hardware failure related complications were analyzed retrospectively .Results 615 patients were followed up for 1-3 years.Five severe intraoperative adverse events included intracerebral hemorrhage in 4 (0.7%) and acute edema in 1 ( 0.2%) .Hardware-related complications requiring surgical revision included wound infections in 10 patients (1.6%),Subcutaneous effusion around IPG in 10(1.6%),lead malposition and/or migration in 9(1.5%),component fracture in 10(1.6%), hardware discomfort in 1 patient ( 0 .2%) .Conclusions Many operative skills in DBS are associated with postoperative complications .Improving the experience of surgeon can significantly reduce the incidence of surgical complications. Objective: To clarify the clinical application of pedunculopontine nucleus(PPN) stimulation in Parkinson disease. Methods： Three consecutive patients with Parkinson disease complicated by severe gait freezing，postural instability and frequently falls (all persisting even while the patient was on medication). Two patients received bilateral stimulation without costimulation of other brain targets and one received unilateral stimulation with contralateral subthalamic nucleus(STN) stimulation. Results: The primary outcome improved significantly with stimulation.Unified Parkinson Disease Rating Scale part Ⅲ(27~30 items) improvement rate are 63.6%，66.7%(bilateral stimulation) and 57.1%(unilateral stimulation) respectively.There were no permanent operative complications. Conclusion: Stimulation of PPN may be beneficial for the subgroup of patients with Parkinson disease who experience severe gait freezing and postural instability with frequently falls，which persist even while on medication.
