Summary : Periodontal disease and increased missing teeth were associated with incident vertebral fractures. In contrast, professional dental cleaning and frequent tooth brushing, was associated with a lower risk of vertebral fracture. Better oral hygiene care attenuated the risk associated with dental diseases. Purpose: To investigate the association between oral health and the risk of vertebral fractures. Methods: We included 2,532,253 individuals aged ≥40 years who underwent the Korean National Health Insurance Service health examinations in 2008 and followed up until December 31, 2017. We performed multivariable Cox proportional hazard regression analyses to evaluate the association between dental diseases and oral hygiene care and the risk of vertebral fractures. Results: Over the 9.3-year median follow-up, 1.46% (n = 36,857) experienced vertebral fractures. Individuals with dental diseases had a higher risk of vertebral fracture than those without (hazard ratio [HR] 1.04, 95% confidence interval [CI]: 1.02–1.07 for periodontal diseases; 1.02, 1.00–1.05 for dental caries; 1.12, 1.05–1.20 for ≥15 missing teeth). Good oral hygiene care was associated with a lower vertebral fracture risk (HR 0.89, 95% CI: 0.86–0.91 for ≥1 time/year [vs. <1 time/year] of professional dental cleaning; 0.90, 0.87–0.93 for ≥2 times/day [vs. 0–1 time/day] of toothbrushing). The combined dental diseases was significantly associated with an increased vertebral fracture risk, whereas combined oral hygiene care was associated with further risk reduction. Better oral hygiene care reduced vertebral fracture risk associated with dental diseases (all P <0.001). Conclusion: Periodontal disease, dental caries, and an increased number of missing teeth were independently associated with higher risks for vertebral fractures. Introduction: The occurrence of dental emergencies, now termed as dental disease nonbattle injuries (D-DNBIs), has long been an impacting factor on militaries' operational effectiveness. Owing to D-DNBIs contributing to low morale, the removal of personnel from duty, causing logistical hardships, and requiring deployable dental teams to operate in theater, there remains a significant benefit in the reduction in the occurrence of D-DNBIs. No study to date has reviewed D-DNBI rates specific to a modern military, and insight into whether militaries are seeing improvements in their dental preparedness remains to be gained.Materials and Methods: A scoping review was conducted in accordance with the guidelines set out by Joanna Briggs Institute. Databases searched included SCOPUS, PubMed, OVID, and DOSS. Six hundred and one articles were initially screened, and six articles were included in the final review.Results: A D-DNBI rate of 172 per 1,000 members per year was reported across the coalition, with the U.S., UK, and French militaries reporting on their dental experience. Both the French data and one UK study reported significantly higher D-DNBIs than their U.S. counterparts, who described 124 D-DNBIs per 1,000 members per year. A significant proportion of these D-DNBIs were reported to be "preventable," which raises questions on the efficacy of garrison healthcare services. Dental disease nonbattle injury rates appear to be largely consistent with what was first reported decades ago, and there remains ongoing and significant value in expeditionary dental services providing supportive healthcare for any deployed military personnel.Conclusion: Rates of D-DNBIs appear to be stable with what was historically reported when an international coalition is considered, with national variation. There remains a significant number of D- DNBIs which require dental treatment within the operational theater, and further efficiencies can be gained from predeployment treatment of "preventable" D-DNBIs. Objective: Older Americans are at risk of many oral diseases and conditions, but limited access to dental care may lead to the use of hospital emergency departments (EDs) or inpatient admissions. This study examined the use of hospitals for non-traumatic dental conditions (NTDCs) among persons aged ≥65 years in the Florida.Methods: We analysed hospital use for NTDCs in Florida in 2018 from health department data sets. We calculated frequencies and rates of ED visits and hospital inpatient admissions, overall and by county, age, sex, race/ethnicity and principal payer. We also examined the source of hospital admission, primary diagnoses and charges.Results: In 2018, there were 4894 ED visits and 747 inpatient admissions for NTDCs among persons aged ≥65 years in Florida. The rate for ED visits was 112.30 per 100 000 population and for hospital inpatient admissions was 17.14; rates varied widely among counties. Medicare was the most common primary payer. Referrals from EDs accounted for 76.2% of inpatient admissions for NTDCs by older adults. Total charges were $21.4 million for ED visits and $40.2 million for inpatient admissions. The most common principal diagnosis for ED visits for NTDCs was "periapical abscess without sinus tract" (K04.7; 26.2%) and for inpatient admissions was "inflammatory conditions of the jaw" (M27.2; 19.1%). Of the 747 inpatient admissions, two people died.Conclusions: Hospital use for NTDCs in Florida among persons aged ≥65 years is a clinical and financial challenge. Background and Aims: Advances of genetic science in genomic techniques have led introducing new diagnostic systems to study the diseases or treatment efficacies. In this science, which named "Personalized Medicine", human genetic structure is used for evaluation of the diagnosis, treatment, and prevention of disease. Regarding the limited number of studies regarding this issue in Oral & Maxillofacial diseases, this study was aimed to evaluate the benefits and challenges of this new area (Personalized Medicine) in dental treatments. Materials and Methods: In this study, complete evaluation of abilities and challenges in order to apply the "Personalized Dentistry" in Dental treatments, and also its effect on oral health indexes were done. All the existing related articles were searched with the relevant key words in the scientific data bases of PubMed, Google Scholar, Medline, Scopus, SID and Magiran. Based on the inclusion and exclusion criteria, 5 articles were selected, assessed and summarized. Results: In the evaluated articles, the benefits of "Personalized Dentistry" in dental treatments as targeted and specialized treatment could result in lower time, energy and cost. In addition, its disadvantage as unknown biologic effective factors and also, the cross effects of some of the above items on each other and the treatment procedures were mentioned. Conclusions: Regarding the specialized sciences and diagnostic tools for oral & maxillofacial disease, the benefits of "Personalized Dentistry" application can be increased for special treatments of patients. Objectives: To study the determinants of emergency department (ED) utilization for non-traumatic dental conditions (NTDCs) by adults in Massachusetts.Methods: We analyzed patient-level factors associated with ED utilization for NTDCs in Massachusetts during 2013, using the Massachusetts All Payer Claims Dataset. The primary independent variables of interest were use of preventive dental service in the year preceding the ED visit and dental insurance coverage. Key covariates included age, gender, income, day of the ED visit, payer type, and residing in a geographically designated dental health professional shortage area (DHPSA). A multilevel logistic regression model was used to estimate the odds of NTDC ED visits as compared to two other categories of ED visits.Results: 1.1 percent of all ED visits in Massachusetts were for NTDCs in 2013. Preventive dental service use in the preceding year decreased the odds (OR = 0.72) of a NTDC ED visit, whereas having dental insurance coverage increased the odds (OR = 1.1) of a NTDC ED visit. Other patient-level characteristics that significantly increased odds of a NTDC ED visit included being between 26 and 35 years of age (OR = 1.2), male (OR = 1.3), uninsured (OR = 1.7) or enrolled in Medicaid (OR = 1.2), and visiting on a weekend (OR = 1.3).Conclusions: Increased access to preventive dental services may lower likelihood of ED use for NTDCs. Introduction: Patients diagnosed with head and neck tumors, are treated by surgery, radiation therapy (RT), chemotherapy (CT) or a combination thereof. The goal is to describe the management protocol for patients subjected to radiation therapy (RT) and to study their long-term oral health status.Materials and Methods: A retrospective study was performed in a sample of 50 patients. We analyzed the oral health status and all the variables included applying the adapted protocol of pre-RT.Results: Prior to RT, 84% of patients required scaling and 32% conservative treatment. Around 74% of patients required at least one exodontia. The mean of exodontias for patients presenting Grade 3 of oral hygiene was 6.50 per person (p<0.0001). The pre-RT study of possible candida infection showed that 78% of patients were negative for this infection. The mean resting saliva production was 39.10 (SD: 23.30; range: 13-145), with a stimulated value of 64.78 (SD: 33.92; range: 16-200).Conclusions: All patients should be protocoled to ensure that they present optimal oral conditions prior to initiating RT treatment. Background. No evidence-based guidelines exist for preventive dental care before radiation therapy (RT) in patients with head and neck cancer (HNC). An ongoing multicenter, prospective cohort study, Clinical Registry of Dental Outcomes in Head and Neck Cancer Patients (OraRad), is addressing this knowledge gap. The authors evaluated the level of dental disease before RT in the OraRad cohort, factors associated with dental disease, and dental treatment recommendations made before RT. Methods. As part of OraRad, the authors assessed caries, periodontal disease, dental recommendations, and dental interventions performed before RT. Results. Baseline measures were reported for 356 participants (77% men) with mean (standard deviation) age of 59.9 (11.0) years. Measures included mean number of teeth (22.9), participants with at least 1 tooth with caries (37.2%), and participants with at least 1 tooth with probing depth 5 millimeters or greater (47.4%). Factors associated with less extensive dental disease before RT included having at least a high school diploma, having dental insurance, history of routine dental care, and a smaller tumor size (T1 or T2). Based on the dental examination before RT, 163 (49.5%) participants had dental treatment recommended before RT, with extractions recommended most frequently. Conclusion. Many patients with HNC require dental treatment before RT; more than one-third require extractions. Practical Implications. Most patients have some level of dental disease at the start of RT, indicating the importance of dental evaluation before RT. Introduction: Proper cooperation between patient and dentist is of great help to make a good oral impression. However, patients are frequently confused when information is given through traditional verbal description. The present study compared the effectiveness of the multimedia information delivery ways with the traditional verbal manner on patients' understanding level in oral impression taking.Methods: The recruited 191 participating patients were randomly assigned to the control group (the verbal group) and two intervention groups (the video group and the picture group) according to the information delivery manner. After intervention, the patients' understanding degree was measured by questionnaire and performance evaluation of behavior feedback on the provided information quantitatively. Also, patients' self-assessment of satisfaction was interviewed by telephone. All data was analyzed by SPSS 14.0 software, and p≤0.05 was set as significant difference in advance.Results: One-Way ANOVA and Chi-square showed there were no statistically significant differences in the mean age, gender composition, and educational level among the three groups (P>0.05). In both questionnaire assessment and performance evaluation, One-Way ANOVA followed by LSD indicate that the video group gained a higher score than the verbal group or the picture group (P<0.05). The questionnaire score in the picture group is significantly higher than in the control group (P<0.05), but no significant difference was shown between these two groups in patients' performance evaluation (P>0.05). Higher percentage of satisfaction was reported by patients in the two multimedia groups than that in the control group.Discussion: Oral impression taking is a consecutive process that requires action cooperation between dentists and patients simultaneously. This particularity makes it more suitable for multimedia delivery. The delivery of tailored information using multimedia in this study was favored by most patients and could improve the degree of patient understanding of the oral impression taking procedures. The objective of this systematic review is to identify current computer-assisted technologies used for managing patients with a need to re-establish craniofacial appearance, subjective discomfort and stomatognathic function, and the extent of their clinical documentation. Electronic search strategies were used for locating clinical studies in MEDLINE through PubMed and in the Cochrane library, and in the grey literature through searches on Google Scholar. The searches for commercial digital products for use in oral rehabilitation resulted in identifying 225 products per November 2016, used for patient diagnostics, communication and therapy purposes, and for other computer-assisted applications in context with oral rehabilitation. About one-third of these products were described in about 350 papers reporting from clinical human studies. The great majority of digital products for use in oral rehabilitation has no clinical documentation at all, while the products from a distinct minority of manufacturers have frequently appeared in more or less scientific reports. Moore's law apply also to digital dentistry, which predicts that the capacity of microprocessors will continue to become faster and with lower cost per performance unit, and innovative software programs will harness these improvements in performance. The net effect is the noticeable short product life cycle of digital products developed for use in oral rehabilitation and often lack of supportive clinical documentation. Nonetheless, clinicians must request clinically meaningful information about new digital products to assess net benefits for the patients or the dental professionals and not accept only technological verbiage as a basis for product purchases. Objectives: To describe emergency department (ED) utilization for oral conditions (OCs) focusing on total volume, costs, repeat utilization, small area predictors, and implications for dental and medical care coordination.Methods: Administrative and demographic data for New Jersey were used to identify users of the ED for OCs and a group of "high users," defined as individuals with four or more ED visits for OCs during 2008-2010. Cost-to-charge ratios were used to estimate costs, and linear regression models applied to zip codes were used to determine strongest predictors of small area variation in ED use for OCs.Results: ED visits for OC generated $25.5 million in costs during 2008-2010. High users represented 4.2 percent of all users and 21.3 percent of ED visits for OCs. Almost all high users had a diagnosis of "dental disorder not otherwise specified," and tobacco use disorder was the most common comorbid medical condition. Young adults (ages 19-34), non-Hispanic blacks, and the medically uninsured were over-represented among high users. High users routinely spread their use across multiple hospitals (e.g., 40 percent of them visited three or more hospitals). Local dentist supply and ED use for other conditions were significant predictors of zip code-level ED use for OCs.Conclusions: Coordination of medical and dental treatment might improve health and reduce costs if targeted to high user populations. Health-care delivery reforms, such as accountable care organizations, could provide vehicles for achieving this coordination. Aims: To evaluate the perception of parents about the impact of oral problems in their children, and of demographic, socioeconomic and psychosocial factors on the oral health-related quality of life (OHRQoL) of Brazilian adolescents and young adults with intellectual disability (ID), in 2014.Methods: The sample consisted of 119 individuals. Data collection was performed in stages: calibration and pilot study; Application of questionnaires; Exam of caries, periodontal and need for dental prostheses, in accordance with the World Health Organization criteria. The data were subjected to both bivariate and multivariate analyses to test the association of independent variables with the dependent variable (QoL).Results: Worse QoL was associated with the presence of periodontal pocket (p = 0.03; OR = 2.50; 95% CI 1.06 to 5.90), treatment need (p = 0.01; OR = 5.00; 95% CI 1.37 to 18.31), and need for dental prostheses (p = 0.03; OR = 2.89; 95% CI 1.13 to 7.37).Conclusion: It was concluded that clinical determinants had the greatest influence on OHRQoL of individuals with ID. The increasing prevalence and severity of oral disease in South Africa and the resultant effect on quality of life remain of major concern. However, to aid in the relief thereof a variety of over-the-counter (OTC) locally acting mouth and throat preparations are available. The use of these preparations broadly include the symptomatic relief of painful, inflamed and irritated mouths and throats due to various causes, use in dental procedures and the treatment of fungal infections. Objectives: The inability to access regular dental care may lead to care seeking at hospital emergency departments (EDs). However, EDs generally are not equipped or staffed to provide definitive dental services. This study examined trends and patterns of hospital ED use for dental-related reasons in Florida, a large, diverse state with serious barriers to accessing dental care.Methods: Data for this study were drawn from ambulatory ED discharge records compiled by Florida's Agency for Health Care Administration for 2005-2014. Visits for dental-related reasons in Florida were defined by the patient's reported reason for seeking care or the ED physician's primary diagnosis using ICD-9-CM codes. We calculated frequencies, age-specific and age-adjusted rates per 100,000 population, and secular trends in dental-related ED visits and their associated charges.Results: The number of dental-related visits to Florida EDs increased each year, from 104,642 in 2005 to 163,900 in 2014; the age-adjusted rate increased by 43.6 percent. Total charges for dental-related ED visits in Florida increased more than threefold during this time period, from $47.7 million in 2005 to $193.4 million in 2014 (adjusted for inflation). The primary payers for dental-related ED visits in 2014 were Medicaid (38 percent), self-pay (38 percent), commercial insurance (11 percent), Medicare (8 percent), and other (5 percent).Conclusions: Dental-related visits to hospital EDs in Florida have increased substantially during the past decade, as have their associated charges. Objectives: Prior studies document increased numbers of nontraumatic dental condition (NTDC) visits to U.S. emergency departments (EDs). However, the influence of travel distance on ED use for NTDCs, particularly for Medicaid enrollees has hitherto received little attention. The authors examined the effect of travel distance on Wisconsin Medicaid enrollees' NTDC visits to EDs after adjustment for covariates.Methods: NTDC-related visits claims data for Wisconsin Medicaid (2001-2009) was analyzed. For each enrollee, travel distance to the nearest of 130 EDs in Wisconsin was determined. The number of NTDC visits per person-year was aggregated by ZIP+4 of residence. Negative binomial regression adjusting for the expected number of visits based on race, sex, age of the residents and calendar year was used to evaluate the effect of travel distance, urbanicity, and dentist-population ratio on rate of visits.Results: Enrollees residing in rural counties, entire dental health professional shortage areas, areas with dentist population ratios >20,000: 1 and non-Hispanic Whites travelled the furthest, compared to nearest mean ED distance of 2.9 miles. Enrollees residing 3 miles away or further had significantly lower rates of NTDC visits to EDs.Conclusions: This study demonstrates that distance is a barrier to making NTDC-related visits to EDs. Background: Looking for and treating the portal of entry (POE) of infective endocarditis (IE) is important, but published research on this topic is nonexistent.Objectives: The goal of this study was to systematically search for the POEs of present and potentially new episodes of IEs.Methods: Patients were systematically seen by a stomatologist, an ear, nose, and throat specialist, and a urologist; women were systematically seen by a gynecologist; patients were seen by a dermatologist when there were cutaneous and/or mucous lesions. Colonoscopy and gastroscopy were performed if the microorganism came from the gastrointestinal tract in patients ≥50 years of age and in those with familial histories of colonic polyposis. Treatment of the POE was systematically considered.Results: The POEs of the present IE episodes were identified in 74% of the 318 included patients. The most frequent POE was cutaneous (40% of identified POEs). It was mainly (62% of cutaneous POEs) associated with health care and with intravenous drug use. The second most frequent POE was oral or dental (29%). A dental infectious focus was more often involved (59% of oral or dental POEs) than a dental procedure (12%). POEs were gastrointestinal in 23% of patients. Colonic polyps were found in one-half of the patients and colorectal adenocarcinomas in 14%. Performance was good regarding the search for an oral or dental or a colonic potential POE, which were found in 53% and 40% of patients, respectively.Conclusions: Our search for the POEs of present IEs was often successful, as was searching for an oral or dental or a gastrointestinal POE of a new IE episode. We advise the systematic performance of stomatologic examinations in patients with IE and performance of colonoscopy in patients ≥50 years of age or at high risk for colorectal cancer. Objectives: This study's aim was to present data on the sociodemographic conditions, knowledge of dental diseases, dental care, and dietary habits among children aged 3, 5, 10, and 15 years in Da Nang, Vietnam.Methods: A cross-sectional epidemiological questionnaire study was conducted in a population of 840 children randomly selected by their year and month of birth (January to July), including 210 individuals in each age group. A self-reported questionnaire was completed by the parents of 3- and 5-year-olds, and a modified questionnaire was given to 10- and 15-year-olds to complete by themselves.Results: Mass media constituted the main source of oral healthcare information. Parents assisted with tooth brushing in 86 percent of 3-year-olds and 71 percent of 5-year-olds. Fluoride toothpaste was used by 44-78 percent of children, with no clear age-related trend. Within the past year, 60 percent of 3- and 5-year-olds, 20 percent of 10-year-olds, and 49 percent of 15-year-olds reported they had not visited a dental professional. Sweets were consumed between principal meals by 70-80 percent of children. Milk with sugar was regularly consumed by 71 percent of 3-year-olds and 91 percent of 5-year-olds.Conclusions: Children showed frequent sugar consumption and insufficient frequency of brushing their teeth with fluoride toothpaste. Food-based dietary guidelines should play a significant role in nutrition and oral health. It is especially important that oral health prevention programs reach preschool children before they establish unhealthy dietary habits. Objectives: Emergency department (ED) visits for nontraumatic dental conditions (NTDCs) may be a sign of unmet need for dental care. The objective of this study was to determine the accuracy of the International Classification of Diseases codes (ICD-10-CA) for ED visits for NTDC.Methods: ED visits in 2008-2099 at one hospital in Toronto were identified if the discharge diagnosis in the administrative database system was an ICD-10-CA code for a NTDC (K00-K14). A random sample of 100 visits was selected, and the medical records for these visits were reviewed by a dentist. The description of the clinical signs and symptoms were evaluated, and a diagnosis was assigned. This diagnosis was compared with the diagnosis assigned by the physician and the code assigned to the visit.Results: The 100 ED visits reviewed were associated with 16 different ICD-10-CA codes for NTDC. Only 2 percent of these visits were clearly caused by trauma. The code K0887 (toothache) was the most frequent diagnostic code (31 percent). We found 43.3 percent disagreement on the discharge diagnosis reported by the physician, and 58.0 percent disagreement on the code in the administrative database assigned by the abstractor, compared with what it was suggested by the dentist reviewing the chart.Conclusion: There are substantial discrepancies between the ICD-10-CA diagnosis assigned in administrative databases and the diagnosis assigned by a dentist reviewing the chart retrospectively. Objectives: Poor oral health can have a negative impact on overall health. This is especially concerning for individuals with sickle cell disease (SCD), an inherited blood disorder that affects hemoglobin and can lead to an increased risk of infection and hyperalgesia. Because the majority of individuals with SCD have Medicaid insurance and no dental coverage, we provided free basic dental care to individuals with SCD to determine whether it decreased overall healthcare utilization.Methods: Through a contract with a private dental office, we provided free basic dental care (eg, cleanings, fillings, x-rays) to individuals with SCD. We reviewed medical records for the 12 months before and after their initial dental visit to determine whether there were any changes in acute care visits (defined as a visit to the emergency department, sickle cell infusion center, or visits to both in the same day), hospitalizations, and total days hospitalized. We conducted a negative binomial regression to determine any differences in the pre-post periods.Results: In our multivariable analysis, there was a statistically significant decrease in hospital admissions. In addition, there was a significant decrease in total days hospitalized if dental work was completed, but an increase in days hospitalized in men.Conclusions: Providing dental care to individuals with SCD who did not have dental insurance did not greatly alter acute care visits. Background:Dental disease is a recognised cause of sinusitis. We perceived an increased incidence of sinusitis secondary to dental disease in recent years. This study reviews the incidence of odontogenic sinusitis, its clinical features and treatment.Methods:Medical records of patients with odontogenic sinusitis were identified using the senior author's clinical database and Hospital Information Support System data (January 2004 to December 2009).Results:Twenty-six patients were identified, nine females and 17 males (age range, 17–73 years). Rhinorrhoea and cacosmia were the commonest symptoms (81 and 73 per cent, respectively), with presence of pus the commonest examination finding (73 per cent). Causative dental pathology included periapical infection (73 per cent), oroantral fistula (23 per cent) and a retained root (4 per cent). In all 26 cases, treatment resulted in complete resolution of symptoms; 21 (81 per cent) required sinus surgery. The number of patients with odontogenic sinusitis undergoing surgery has steadily increased, from no cases in 2004 to 10 in 2009 (accounting for 8 per cent of all patients requiring sinus surgery). Reduced access to dental care may be responsible.Conclusion:The incidence of odontogenic sinusitis appears to be increasing. The Interdisciplinary Orofacial Examination Protocol for Children and Adolescents (Protocolo de exploración interdisciplinaria orofacial para niños y adolescents, Barcelona, 2008) is very useful in providing a fast, initial, expedient detection of possible morphological and functional disorders, and to guide the patient toward the appropriate professionals. With this tool it is possible to detect the risk factors which can negatively affect morphological and functional harmony and guide patients toward the necessary treatment as early as possible. This Protocol, developed by 4 orthodontists, 1 ENT and 3 speech language therapists, also contributes to the unification of concepts and nomenclature used by distinct specialists, thus making professional understanding easier and more dynamic. Background: Though great strides have been taken globally in the fight against oral diseases, problems persist especially among poor, disadvantaged and socially marginalised communities like tribes. Konda Reddies are one of the most primitive tribes of Bhadrachalam who because of their isolation, inaccessibility to dental resources and lack of dental knowledge follow traditional methods of oral hygiene practices, which are found to be inadequate to maintain oral health. Aim and Objectives: To assess the periodontal status of Konda reddy tribes residing in Bhadrachalam of Khammam district, India. Materials and Methods: Based on availability and accessibility the tribes were gathered using convenient sampling method. The total study population consisted of 500 subjects. Using a preformed Performa the oral hygiene practices and periodontal status was recorded using CPI index. Results: The total study population was 500, comprising of 225 males and 275 females in the age group ranging from 20- >70 years. The mean number of sextants with healthy condition were 0.04±0.19; calculus 4.69±1.21; pockets 4-5 mm 0.91±0.91 and pockets >6mm 0.02±0.15. Conclusion: Poor oral hygiene and periodontal status was seen among the tribes. Under these circumstances, the implementation of a basic oral health care programme for these tribes is a high priority. [ABSTRACT FROM AUTHOR]
