Tannins are polyphenols that can be extracted from different parts of the plant and have different known commercial applications. The extraction of tannins generates a significant amount of low-density wastes. An alternative that can be applied to ensure energy reuse of this low-density waste is a densification process, such as briquetting. Thus, this research work aimed to assess the energy potential of the residue from the extraction of tannins of different forest species, aiming at its energy reuse in the form of briquettes. Stem barks of 6 forest species (Acacia mangium, AM; Anacardium occidentale, AO; Anadenanthera colubrina, AC; Azadirachta indica, AI; Mimosa caesalpiniaefolia, MC; Mimosa tenuiflora, MT) were used after tannin extraction. Bulk density, energy (higher, lower, and net heating value) and thermal (TG/DTG) properties, and chemical constitution (extractives, lignin, and holocellulose) of the materials were determined. In addition, briquette compaction ratio, apparent density, volumetric expansion, and water adsorption over the days were evaluated. O presente estudo de caso intitulado “Só tem brincadeira em dia de sol” tem como proposta solucionar um problema que envolve uma comunidade que sofre com a questão de alagamentos em dias de chuva. Tem por objetivo a proposição de resoluções coerentes e efetivas capazes de gerar melhorias para a comunidade além de favorecer a aprendizagem significativa dos alunos envolvidos, por meio de uma construção ativa. Espera-se que os alunos sejam capazes de criar hipóteses para a solução deste caso estruturado viabilizando novas possibilidades para aquele contexto. A sequência didática foi pensada para ser desenvolvida por meio do ensino remoto com alunos do ensino fundamental II e médio.

Sub-optimal sensitivity and specificity in current allograft monitoring methodologies underscore the need for more accurate and reflexive immunosurveillance to uncover the flux in alloimmunity between allograft health and the onset and progression of rejection. QSant—a urine based multi-analyte diagnostic test—was developed to profile renal transplant health and prognosticate injury, risk of evolution, and resolution of acute rejection. Q-Score—the composite score, across measurements of DNA, protein and metabolic biomarkers in the QSant assay—enables this risk prognostication. The domain of immune quiescence—below a Q-Score threshold of 32—is well established, based on published AUC of 98% for QSant. However, the trajectory of rejection is variable, given that causality is multi-factorial. Injury and subtypes of rejection are captured by the progression of Q-Score. This publication explores the clinical utility of QSant across the alloimmunity gradient of 32–100 for the early diagnosis of allograft injury and rejection.

Despite all quality management and integration literature prescriptions to implement strategies for a better organizational performance, healthcare organizations support a model that is inefficient, expensive, and unsustainable over time. This work aims to examine the interpersonal relationships in three large hospitals located in Southern Brazil and its relation with organizational efficiency. Through a qualitative and explanatory research, semi-structured interviews were applied to 32 professionals, in addition to a document analysis. The data analysis shows that integration occurs at the formal leadership level only in one of the organizations and does not involve the medical and operational professionals. Quality management seems not to be fully incorporated into care routines, and are related to efficiency losses. This scenario is probably related to the lack of integration among the professionals and the consolidation of trust, leadership, and communication. Resumo Apesar das prescrições da literatura sobre gestão da qualidade e integração para implementar estratégias que melhorem o desempenho organizacional, as organizações de saúde sustentam um modelo que é ineficiente, caro e insustentável ao longo do tempo. Este estudo objetiva examinar os relacionamentos interpessoais em três hospitais de grande porte localizados no sul do Brasil e sua relação com a eficiência organizacional. Através de uma pesquisa qualitativa e explicativa, entrevistas semiestruturadas foram aplicadas em 32 profissionais em adição à análise documental. Os dados analisados sugerem que a integração ocorre ao nível das lideranças formais em apenas um dos hospitais e não envolve os profissionais médicos e os operacionais. A gestão da qualidade não parece estar completamente incorporada às rotinas assistenciais e estão relacionadas com perdas de eficiência. Este cenário está provavelmente relacionado com a falta de integração entre os profissionais e da consolidação da confiança, da liderança e da comunicação.

Background: The best technique for incisional hernioplasty has not been established yet. One of the difficulties to compare these techniques is heterogeneity in the profile of the patients evaluated. Aim: To analyze the results of three techniques for incisional hernioplasty after open bariatric surgery. Method: Patients who underwent incisional hernioplasty were divided into three groups: onlay technique, simple suture and retromuscular technique. Results and quality of life after repair using Carolina’s Comfort Scale were evaluated through analysis of medical records, telephone contact and elective appointments. Results: 363 surgical reports were analyzed and 263 were included: onlay technique (n=89), simple suture (n=100), retromuscular technique (n=74). The epidemiological profile of patients was similar between groups. The onlay technique showed higher seroma rates (28.89%) and used a surgical drain more frequently (55.56%). The simple suture technique required longer hospital stay (2.86 days). The quality of life score was worse for the retromuscular technique (8.43) in relation to the onlay technique (4.7) and the simple suture (2.34), especially because of complaints of chronic pain. There was no difference in short-term recurrence. Conclusion: The retromuscular technique showed a worse quality of life than the other techniques in a homogeneous group of patients. Introdução: O transtorno do espectro autista é classificado como um distúrbio agressivo do desenvolvimento. Este compromete todo o desenvolvimento neurológico, e se caracteriza pelo desequilíbrio comportamental exposto pela inaptidãona inclusão social. Objetivo: Analisar as percepções e experiências emocionais de mães com filhos diagnosticados comtranstornos do espectro autista. Metodologia: Estudo de caráter qualitativo, que teve como público alvo mães de filhoscom diagnóstico de transtorno do espectro autista, participantes das APAEs. Foi realizada uma entrevista semi-estruturadagravada por gravador digital, e posteriormente transcrita. Resultados: Notoriamente as mães possuem dificuldades paracompreender e se adaptar a realidade do filho diagnosticado de autismo. Criam expectativas e perspectivas quanto aofuturo dos filhos almejando que estes desenvolvam prioritariamente a independência. Considerações Finais: Obtivemos apossibilidade de compreender melhor as expectativas e as dificuldades das mães das crianças com diagnóstico de TEA. Foiencontrado idade média para o diagnóstico de espectro autista do filho entre 3 anos e 4 meses. A descoberta do diagnóstico do TEA causou importantes sentimentos prejudiciais e de não aceitação à essas mães. Surgem sentimentos negativos,afetando assim a saúde mental dessas mães e familiares.Palavras-chave: Autismo; Psicodinâmica; Percepção. ABSTRACTIntroduction: Autism spectrum disorder is classified as an aggressive developmental disorder. This compromises the entireneurological development and is characterized by the behavioral imbalance exposed by the inaptitude in social inclusion.Objective: To analyze the perceptions and emotional experiences of mothers with children diagnosed with autism spectrum disorders. Methodology: This was a qualitative study that had the target audience of mothers of children diagnosedwith autism spectrum disorder, who participated in APAEs. A semi-structured interview was recorded by digital recorder,and later transcribed. Results: Notoriously, mothers have difficulties in understanding and adapting the reality of a childdiagnosed with autism. They create expectations and perspectives on the future of children, aiming for them to developindependence first. Final Considerations: We obtained the possibility to better understand the expectations and difficultiesof the mothers of children diagnosed with ASD. We found a mean age for the diagnosis of autistic spectrum of the childbetween 3 years and 4 months. The discovery of the diagnosis of ASD has caused important feelings of detriment and non-acceptance to these mothers. Negative feelings arise, thus affecting the mental health of these mothers and their families.Keywords: Autism; Psychodynamics; Perception.

The present work aims to observe the spatial distribution of metals associated with carbon forms (fraction < 2 mm) in surface sediments of two macrotidal estuaries, Sao Marcos Bay and Anil River Estuary, which are located within the transition region between the Amazonian and the semi-arid northeast regions. Grain size, metal content (Al, Fe, Mn, Cu, Pb, Cr, Zn, and Ni), organic matter, and calcium carbonate content were determined. Grain size analyses showed the predominance of the sand-sized fraction < 2 mm due to the local hydrodynamic conditions. Anil River Estuary sediments exhibited high organic matter content due to both the mangrove outwelling and domestic sewage discharge. They also presented high calcium carbonate content as a result of abundant remnants of gastropod shells. Organic matter acted as the primary geochemical carrier for most metals in both estuaries, while calcium carbonate acted as the secondary carrier. Enrichment factors indicated Mn sediment contamination in Sao Marcos Bay and Fe, Pb, and Zn contamination in the Anil River Estuary. These results also suggest that Sao Marcos Bay is influenced by harbor activities, mostly ore shipment, whereas Anil River Estuary sediments are enriched in these metals as a result of domestic and hospital effluents reaching the urbanized drainage basin.

Muito se tem discutido acerca do papel da educação inclusiva em nossas escolas, e a formação docente sobre essa perspectiva: Mendes (2006), Mantoan (2003; 2006; 2007), Demo (2017), Freire (1996; 2013), entre outros. Assim sendo, este trabalho de conclusão do curso de Especialização em Ensino Interdisciplinar em Infância e Direitos Humanos da Universidade Federal de Goiás/Regional de Catalão, que ao percurso de dois anos de muitas leituras voltadas à Inclusão, reforçou a nossa ideia de direito a educação, como sinônimo de igualdade e respeito à dignidade humana. Partindo desses princípios, propomos um novo olhar sobre a Educação Inclusiva, a partir da formação inicial dos pedagogos, de como são estruturadas as matrizes curriculares do curso de Pedagogia no que tange o tema inclusão. Neste caso, escolhemos o curso ofertado pela Universidade Estadual de Goiás na unidade de Formosa, no anseio de realizarmos a análise de como essa formação inicial tem contribuído para preparação desses docentes para a atuação na área inclusiva. Com efeito, o objetivo deste estudo foi analisar, por meio de uma pesquisa qualitativa de cunho documental, com a coleta de dados relacionados ao tema inclusão e usando como instrumentos: o Projeto Político desse curso de Pedagogia, os planos de ensino e as ementas de cada disciplina ofertadas pelo currículo atual, vigente desde 2015, para compreendermos se houve mudança especificamente no que concerne a inclusão escolar. Por fim, os resultados ressaltaram a complexidade do processo de implementação dessa temática no universo acadêmico e de como esse tema é abordado na formação inicial de Pedagogia do campus da Universidade Estadual de Goiás (UEG) de Formosa-GO.

A partir das análises, se conclui que há expectativas na realização da construção de um regime de bem-estar renovado, mesmo em tempos de crise democrática, visando que o Estado deve criar possibilidades político-jurídicas estruturais favoráveis para a promoção da inclusão social.

A relevância do tema se manifesta tendo em vista a questão da dignidade humana perspectivada no comprometimento de espécimes da fauna marinha em diferentes territórios geográficos, ladeada pela análise do direito comparado. A metodologia utilizada é a qualitativa, envolvendo a análise bibliográfica disponível que esteja relacionada ao tema pesquisado.

Apesar de essas políticas apresentarem enfoques específicos e distintos, investiga-se de que maneiras a política de defesa da concorrência pode influenciar na tutela dos direitos do consumidor, principalmente, por meio da garantia e proteção do direito de escolha, nas relações consumeristas. Para atingimento desses objetivos será realizada pesquisa doutrinária, utilizando do método hipotético-dedutivo, com intuito de demonstrar que a legislação antitruste tem como um de seus principais fundamentos a proteção do consumidor. Além do que, nessa sociedade pós-moderna, deve ficar evidente que o consumidor carece dessa proteção.

17. Emission factor and balance of mercury in fish farms in an artificial reservoir in NE Brazil

This paper estimated the mercury (Hg), emission factor, and mass balance from caged fish farming in the Castanhão Reservoir, NE Brazil, based on monitoring of a typical farm of Nile tilapia (Oreochromis niloticus). The total Hg input to the farm reached 1.45 gHg ha(-1) year(-1), from which 0.21 gHg ha(-1) year(-1) was exported out as fish biomass, ultimately resulting in an emission factor of 1.24 gHg ha(-1) year(-1) for the reservoir or approximately 8.27 mgHg ton fish(-1) year(-1) produced. Most of the input came from aquafeeds with concentrations varying from 1.4 to 31.1 ng g(-1), depending on the type of aquafeed. The Hg concentrations in fish were very low and varied from 1.0 to 2.9 ng g(-1). These values are two orders of magnitude lower than the legal limit for human consumption. The estimated total annual discharge of Hg from farming into the reservoir is 174 g for 18,000 tons of fish produced and may reach 387 g when the reservoir reaches its total capacity (40,000 tons), which is expected to occur in 2020. The mass balance, considering the deposition and accumulation rates, showed that approximately 40% of the total Hg input accumulate in farm sediments (0.72 g ha(-1) year(-1)), which is approximately 60% of the deposition rate estimated through the sediment traps and suggests that 0.54 gHg ha(-1) year(-1) could eventually be transported out of the farm to the reservoir. Notwithstanding these facts, the total annual input of Hg from fish farming to the Castanhão Reservoir is less than 1.0% of the total input from anthropogenic sources.

A experiência profissional dos gestores é fator influente na qualidade do atendimento na educação infantil. Esse estudo teve como objetivos realizar uma breve revisão bibliográfica da gestão da educação infantil no Brasil e analisar a práxis de formação continuada desenvolvida junto a gestores dos Centros Municipais de Educação Infantil (CMEIs). Participaram do estudo 180 gestores, todos do sexo feminino, em duas fases, no período de um ano letivo. Utilizou-se da metodologia da pesquisa-ação colaborativa e foram realizadas reuniões em grupos no Centro de Formação Continuada da Secretaria Municipal de Educação, divididos por núcleos regionais, com a apresentação e a discussão coletiva dos projetos realizados nos CMEIs. Os resultados indicaram que a gestão democrática na educação infantil demanda a compreensão da criança pequena como cidadão de direitos, bem como a própria concepção de gestão democrática. A educação infantil tem peculiaridades importantes, como organização do ambiente e planejamento das práticas educativas, incluindo o cuidado de rotinas. A revisão bibliográfica apontou para escassez na produção nacional específica de gestão educacional na educação infantil e múltiplas compreensões do significado de gestão democrática. Conclui-se que tem ocorrido avanços no campo da gestão democrática e que a participação da comunidade contribui para a transparência das decisões tomadas, mas ainda necessita ampliar o conceito de autonomia, bem como a participação das crianças. A superficialidade do conhecimento teórico sobre gestão democrática demanda formação continuada dos gestores e o permanente abandono do improviso no exercício da função.

Celiac disease, characterized by autoimmune reactions to dietary gluten, affects up to 3 million in the US and approximately 0.5%–1% globally. A strict, lifelong gluten-free diet is the only treatment. An economic, simple, accurate, rapid and portable gluten testing device would enable gluten-sensitive individuals to safeguard their food safety. We developed a novel solution, Nima™, a gluten sensor that integrates food processing, gluten detection, result interpretation and data transmission in a portable device, detecting gluten proteins at or below the accepted 20 ppm threshold. We developed specific monoclonal antibodies, an optimized lateral flow immunoassay strip, and one-step aqueous extraction. Compared with reference R5, Nima TM antibodies (13F6 and 14G11) had 35- and 6.6-fold higher gliadin affinities, respectively. We demonstrated device performance using a comprehensive list of foods, assessing detection sensitivity, reproducibility, and cross-reactivity. Nima™ presented a 99.0% true positive rate, with a 95% confidence interval of 97.8%–100%.

Este estudo teve como objetivo analisar a composição química e o poder calorífico da madeira de Copaifera arenicola visando aplicações energéticas. Para isso foram retiradas amostras das posições da base, 25%, 50%, DAP, 75% e 100% da altura total do fuste de árvore. Foram realizados ensaios para a determinação da composição química (teores de extrativos totais, lignina e holocelulose) e do poder calorifico superior (PCS) e poder calorifico inferior (PCI). Foram observados valores médios de 4.427 kcal kg-1 para o PCS e 4.123 kcal kg-1 para o PCI, teores de extrativos de 20,2% e 31,5% de lignina; para o teor de holocelulose obteve-se 48,4% e para o teor de cinzas o valor médio foi de 1,93%. As variáveis analisadas da madeira foram consideradas elevadas ao se comparar com os de outras espécies relatadas na literatura, podendo considerar a madeira de C. arenicola com potencial para uso energético.

Sabe-se que o direito fundamental de propriedade não é, assim como todos os demais direitos fundamentais, um direito absoluto, podendo ser relativizado, como, por exemplo, pela função social e pela função econômica da propriedade. O objetivo deste artigo é analisar o conteúdo e as restrições ao direito de propriedade no tocante à edificação, levando em consideração os custos com enfoque dado pela teoria interna e pela teoria externa, haja vista que, a depender da teoria a ser adotada pelo ordenamento jurídico, o suporte fático do direito de propriedade será diferente, com diferentes implicações práticas e jurídicas. Entendimento dos tribunais superiores que o uso da propriedade imobiliária para edificação, constitucionalmente garantido, deve ser restringido, a depender do caso, quando colocado em confronto com normas protetoras do meio ambiente urbano e consequentemente da função social da propriedade. Para o atingimento de tal finalidade utilizou-se como metodologia a pesquisa bibliográfica doutrinária e jurisprudencial e o método hipotético-dedutivo. Evidenciou-se essencialmente que as decisões dos tribunais superiores estão a aplicar a teoria externa.

Os conceitos que fundamentam bo trabalho são advindos da teoria da enunciação e da abordagem dinâmica da sintaxe, apresentando como objeto específico de estudo os lugares sintáticos de sujeito e de objeto. No âmbito dessa abordagem, os aspectos estruturais da língua são tratados na relação com os aspectos enunciativos. O desenvolvimento da hipótese principal do trabalho indicou que a constituição e a ocupação dos lugares sintáticos sujeito e objeto são definidas no contato entre as regularidades linguísticas e o acontecimento da enunciação, na relação com um campo de memória.

Origina-se a partir de então, como reflexo imediato e conveniente, o aumento indiscriminado do controle social através de seus instrumentos e táticas criminalizantes. The profile and levels of bioactive amines in green and roasted coffee were investigated. Coffea arabica L., variety ‘catuai vermelho’, was harvested in Sao Sebastiao do Paraiso, state of Minas Gerais, Brazil. The green coffee was roasted at two degrees—American and French (300 °C for 6 and 12 min, respectively). The samples were analysed for bioactive amines. Roasted samples were also analysed for moisture content, water activity and L ∗ a ∗ b ∗ colour characteristics. Total amine levels in green coffee ranged from 3.03 to 4.44 mg/100 g. The predominant amines in green coffee were serotonin and putrescine, followed by spermidine and spermine. The degree of roasting did not affect moisture content or water activity of the coffee bean. Hue angle and saturation were higher for American than for French coffee. The profile and levels of bioactive amines in roasted coffee differed significantly from green coffee. The prevailing amine in roasted coffee was serotonin, followed by spermidine. Putrescine and spermine were not detected in roasted coffee. The presence of agmatine was detected in French roasted coffee. American roasted coffee had lower amine levels than French, which indicated that, the stronger the degree of roasting, the higher were the total levels of amines.

Neste artigo é analisada a efetividade dos programas de investimento em infraestrutura de transporte do Estado de Goiás que, devido à localização geográfica, tende a impactar diretamente no desenvolvimento socioeconômico do país. Em especial, são destacados os planejamentos em investimentos na infraestrutura rodoviária, aérea e ferroviária durante o período de 2006 a 2014. Os dados foram coletados junto ao Portal da Transparência do Estado de Goiás, à Secretaria de Estado de Gestão e Planejamento de Goiás, ao Portal Brasil – Programa de Investimento em Logística, e ao Instituto Mauro Borges. Entre seus mais importantes resultados, o artigo mostra a existência de falhas na capacidade de execução orçamentária, que vêm comprometendo o aumento de investimentos em infraestrutura logística, com reflexos inequívocos sobre o desenvolvimento nacional.Código JEL | O18; P25; R42.

This study was to compare the behavior of five native Brazilian species: angelim (Andira anthelmia), a canela (Anila firmula), cedro (Cedrela fissilis), ipe amarelo (Tabebuia alba) e a muiracatiara (Astronium lecointein) through testing machining. It was made ten samples for each kind of dimensions following the U.S. ASTM D 1666-87. Still determined the apparent density of the wood (g.cm-3) based on the NBR 7190 in order to establish possible relations For machining tests, all species behave well with relation to the sanding and drilling for peg no significant differences between the grades obtained. However, the ipe submitted the worst result for the planing test and cedro due to its smaller apparent density obtained the worst result front of drilling tests. With respect to turning test, species of ipe and canela were rated as the best among the tested species. Overall, it was concluded that the species showed potentials of uses in furniture, frames and general finishing occasions.

Para isso, introduziu-se os conceitos de formacao nominal e formacao verbal, desenvolvidos no Grupo de Estudos da Enunciacao da UFMG, a partir de uma semântica da enunciacao. Esses conceitos permitiram constituir dispositivos de abordagem do poema, que sao produtivos para o aprimoramento de uma analise enunciativa, em que a significacao e concebida de forma dinâmica. Alem disso, discute-se o papel do lexico na constituicao das sentencas da lingua portuguesa em contexto de elaboracao literaria.

Este estudo foi conduzido com o objetivo de avaliar a acao do BIOTECH® (acido citrico 10-2 M + complexo enzimatico) na prevencao de entupimentos em gotejadores em sistema de fertirrigacao. O experimento foi instalado em blocos casualizados num fatorial 8 x 4 com seis repeticoes sendo oito tratamentos, com solucoes que continham ou nao BIOTECH® misturados com alguns fertilizantes utilizados em fertirrigacao e quatro periodos de avaliacao da vazao dos tubogotejadores. As solucoes fluiram sob pressao em tubulacao de polietileno dotada de tubogotejadores com a vazao proxima a 1,6 L h-1, espacados de 0,30 m, sendo cada tratamento composto por seis gotejadores. O fluxo foi cronometrado por 45 minutos, repetidos diariamente por um periodo de 34 semanas, subdividido em quatro periodos de avaliacao. Durante as simulacoes de irrigacao as precipitacoes quimicas ocorreram gradualmente, modificando os coeficientes de variacao de vazao e as uniformidades de distribuicao dos tubogotejadores sendo possivel concluir que o BIOTECH® minimizou a reducao da vazao causada por entupimento, mas nao impediu a obstrucao total dos tubogotejadores pelas solucoes indutoras de precipitacao.

A method based on the polymerase chain reaction (PCR) for highly sensitive detection and identification of human malaria parasites was applied to blood and mosquito samples obtained from a village in Guinea Bissau. The prevalence of parasites in the human population was shown to be greatly underestimated by microscopical examination. In particular, a high incidence of Plasmodium malariae and P. ovale parasites was revealed only by the PCR assay. Preliminary evidence was obtained to show that the distribution of P. malariae infections within the village was non-random. This was supported by analysis of the parasite species infecting the mosquito vector. The implication of these results for the design and interpretation of epidemiological surveys is discussed.

Background: HIV and other sexually transmitted infections are a growing problem in the military personnel of Africa, and information about this problem in Guinea-Bissau is lacking. The aims of this study were to determine the prevalence and trends of the HIV epidemics in the military forces of Guinea Bissau and to explore possible risk factors for HIV infection. Methodology: Repeated cross-sectional surveys of HIV-1 and HIV-2 were conducted between 1992 and 2005, and knowledge, sexual behaviour and risk factors for HIV-1 and HIV-2 in military personnel in Guinea-Bissau were assessed. Results: The seroprevalence of HIV-1, HIV-2 and HIV-1+HIV-2 dual reactivity was 1.1%, 8.4% and 0.1% in 1992-95, and in 2005 7.7%, 5.1% and 1.9%, respectively. Both the increase of HIV-1 and the decline of HIV-2 between 1992-95 and 2005 were significant when adjusted for age (p < 0.001 for both changes). Only a minority did not know how HIV transmits, but sexual risk taking was high. Several significant risk factors were found in univariate analyses for HIV-1 and HIV-2, but the only risk factor that remained significant after multivariate regression analysis was previous contact with a prostitute among HIV-1-positive subjects (single and dually reactive) (p < 0.01). Conclusion: The increasing trend of HIV-1 and the high risky sexual behavior illustrate the need for improvement in HIV/AIDS prevention efforts among military personnel in Guinea Bissau.

The recommended dose of 10 mg quinine/kg bodyweight 3 times a day for 7 days for treatment of malaria is so high that many patients experience cinchonism. We have earlier obtained good results with 7 days' treatment with 20 mg Quinimax/kg bodyweight divided into 2 daily doses. In order to identify the lowest effective dose, children with symptomatic malaria were treated with quinine twice a day for 7 days. They were assigned to 1 of 3 groups treated daily with 10 mg/kg, 15 mg/kg, or 20 mg/kg bodyweight, respectively; 42, 46, and 34 children, respectively, received treatment and completed 5 weeks of follow-up. The cumulative percentages of all children with parasitaemia during follow-up on day 28 or before were 33%, 13% and 12%, respectively. Treatment with 10 mg quinine salt/kg daily for 7 days gave a significantly higher rate of recrudescence than did treatment with 15 or 20 mg/kg daily. Thus at least 15 mg of quinine salt/kg bodyweight daily should be recommended for treatment of symptomatic Plasmodium falciparum malaria in Guinea-Bissau.

O artigo trata da relação entre sintaxe e semântica. Especificamente, a teoria da enunciação foi adotada como fundamento da análise dos dados. A perspectiva teórica permitiu a introdução de alguns conceitos da teoria do discurso, como memória discursiva e efeito de sentido. O conceito de domínio de referência, no âmbito da semântica, também foi fundamental na análise. O foco de interesse nas sentenças analisadas é o complemento verbal. O artigo defende a seguinte proposta: o complemento verbal é um lugar sintático projetado pelo verbo, mas a ocupação desse lugar é comandada pela enunciação. Sendo assim, a ocupação do lugar sintático é diretamente influenciada pelas discursividades que afetam a constituição da sentença. Dessa maneira, a sintaxe se situaria na confluência entre o plano da estruturação linguística e o plano da enunciação.

Exploratory research, in a quantitative approach, carried out in Pediactrs Hospitals with the following objectives: identify in literature the publications about Peripheric Intermittent Intravenous Device - PIID, with their respective approach; describe the technique of utilization of PIID in the Pediatrics Institutions of Goiânia and analyze the intervening factors to the use of PIID and its implications in nurse performances. The data when collected by questionnaires previously tested. In the first time we went through the existent publications about PIID; in second time that identified the Hospital Institutions which used the PIID and they did it. We came to the conclusion that there are few specific studies about PIID and those which approach the theme do it in a very general way, leaving out many questions about the subject. From the total of seventy existent Pediatrics Institutions, only two use the PIID and even those not systematically. We observed that the medical and nurse staff use the technique without any protocol, and each professional prescribes and performs the procedure in his own adequate way. The most common solution used to maintain the catheter permeable to the heparin; however, it doesn't exist consensus in relation to the proper concentration or even to the storage of the solution. Thus, we suggest research to be in the others contexts, with the objective of simplifying the technique, publishing it in order to make it safer for those who want to use it. Key Word: Intravenous device, Peripheric Intermittent Intravenous Device, PIID RESUMO: Pesquisa exploratória, com abordagem quantiqualitativa, realizada em hospitais pediátricos, com os seguintes objetivos: identificar na literatura as publicações sobre Dispositivo Intravenoso Periférico Intermitente (DIPI) com seus respectivos enfoques; descrever a técnica de utilização do DIPI nas instituições pediátricas de Goiânia e analisar os fatores intervenientes quanto ao uso do DIPI e suas implicações nas ações de enfermagem. Os dados foram coletados através de questionários previamente testados. Inicialmente estudou-se as publicações encontradas sobre o DIPI e posteriormente foi investigado o modo como o DIPI vinha sendo utilizado nas instituições hospitalares. Concluiu-se que existem poucos estudos específicos sobre o DIPI e geralmente tratados de modo generalizado, deixando de abordar muitas questões a respeito do tema. Do total de dezessete instituições pesquisadas, apenas duas utilizavam o DIPI e de forma assistemática. As equipes de Enfermagem e Médica utilizavam a técnica sem nenhum protocolo, onde cada profissional prescreve e executa o procedimento da maneira que considera mais adequada. Constatou-se que a solução mais utilizada para manter a permeabilidade do cateter foi a heparina, porém, não existe consenso em relação à concentração indicada, nem mesmo quanto ao armazenamento da solução. Neste sentido, sugere-se que pesquisas sejam feitas em outros contextos, com o objetivo de simplificar a técnica, divulgá-la e torná-la sem riscos para os usuários. With the increasing resistance to commonly used antimalarial drugs, different untested 'local' treatment regimens for malaria will arise. We compared commonly used treatment regimens for children in Guinea-Bissau. Symptomatic children with Plasmodium falciparum mono-infection were allocated at random to one of 4 treatments: 15 mg/kg quinine twice a day for 3 d (group 1); 10 mg/kg quinine twice a day for 3 d followed by a total dose of 25 mg chloroquine base given over 3 d (group 2); a total dose of 50 mg/kg chloroquine base given in 2 daily doses for 3 d (group 3), or sulfadoxine-pyrimethamine (group 4). On day 28 more children from group 1 (33%; relative risk [RR] = 2.9, 95% confidence interval [CI] 1.5-5.7) and group 2 (26%; RR = 2.1, CI 1.0-4.3) had had parasitaemia than in group 4 (12%), whereas no significant difference was found between group 3 (17%; RR = 1.3, CI 0.6-2.2) and group 4. No severe adverse reaction was observed in any of the groups. Chloroquine is still effective in Guinea-Bissau at an increased dose of 50 mg/kg, which appears safe when given orally in 2 daily doses for 3 d. Sulfadoxine-pyrimethamine could serve as an efficient, cheap and easy to administer second-line drug, leaving quinine to be used for third-line treatment. Quinine should not be used in short courses, nor does the combination of quinine and chloroquine have any advantage.

Because the seroprevalence of HIV-2 has been shown to be high in older age groups, we conducted a survey of all persons aged 50 years or over in two districts in Bissau, investigating the presence of HIV antibodies and possible risk factors for HIV infection with a particular emphasis on age, the impact of the war of independence (1963-74), traditional marital and extramarital sex patterns, blood contact and contact with monkeys. In 670 participants, the HIV-2 prevalence was 14.3%; 16.1% in women and 12.3% in men. The HIV-1 prevalence was only 0.5% (3/670). The HIV-2 prevalence peaked for men in the 60-69 years age group, and for women in the 50-59 years age group, declining markedly in the following age group for both men and women (OR = 0.09 (0.01-0.51), OR = 0.37 (0.15-0.82), respectively). This pattern could be due to differential mortality for HIV-2 infected individuals or to a cohort effect for a generation who were sexually active at the time of the war of independence in the 1960s and early 1970s in Bissau. Supporting the link with the colonial army, women who had had sex with a white man had a higher seroprevalence (OR = 3.63 (1.12-11.24)). The ethnic group indigenous to Bissau city had a much lower prevalence, but demographic and cultural risk factors such as marital status, religion, education and having lived outside Bissau were not associated with HIV-2. In the multivariate analyses for women, variables related to extramarital sex or prostitution (having sex with a white man, having lived in Senegal, not living with husband, and not marrying first sexual partner) were associated with higher risk. For men, previous spouses who had died or had divorced were associated with higher prevalence. Having married the first sexual partner was protective against HIV-2 infection for both men (OR = 0.29 (0.09-0.76)) and women (OR = 0.19 (0.04-1.00)). Hospitalizations, possibly due to transfusions, tended to be associated with higher risk, but only for women (OR = 1.83 (0.97-3.48)). The focus of the HIV-2 epidemic in Guinea-Bissau is likely to be endogenous, and the war of independence and the colonial army with its associated prostitution may have played an important part in propagating the virus.

Clinical symptoms and immunological changes associated with HIV-2 infection were studied in a cohort of police officers in Guinea-Bissau. HIV-related symptoms were classified according to the WHO clinical staging system. The inclusion period was from January 1990 to January 1997, and among 2637 subjects included (90.7%M, 9.3%F), the prevalence of HIV-1, HIV-2 and dual reactivity to both HIV-1 and HIV-2 was 0.9%, 9.7% and 0.5%, respectively. Weight loss10%, diarrhoea or fever1 month, generalized lymphadenopathy and generalized pruritic dermatitis were significantly associated with HIV-2 infection as well as suppression of CD4 cells as compared with HIV-negative controls. Females had significantly higher CD4 cell counts than males, both among HIV-negative and HIV-2-positive asymptomatic individuals. The mortality rates/100 person-years (p.y.) were 0.4 in HIV-negative and 2.6 in HIV-2-positive subjects, giving an age-adjusted mortality rate ratio of 6.6 (95% CI, 4.0-10.9; p0.001). The mortality rate among HIV-2-infected individuals varied considerably in different stages of the WHO clinical staging system; 1.7 and 8.0/100 p.y. in stage 1 and 3, respectively.

The impact of HIV-1 on child health and mortality in Africa has been demonstrated in many studies. Overall reported rates of mother-to-child transmission of HIV-1 range from 13% to 48%. Studies on vertical transmission of HIV-2 are limited but available data suggest that it is rare and occurs in < 5%. Birthweight is an important determinant of child survival in Africa and studies have shown significantly decreased birthweight in children born to HIV-1-seropositive mothers compared with seronegative controls whereas in industrialized countries only marginal differences have been observed. In a study from Rwanda which also took into consideration HIV-status of the newborn only HIV-infected children had lower birthweight and uninfected children born to HIV-infected mothers had similar birthweights as children born to HIV-negative mothers. Only one study has previously described the effect of maternal HIV-2 infection on the birthweight of the offspring but in a limited number of infants. (excerpt)

Community studies with 1-3 years of follow-up have reported four to five times higher mortality in HIV-2-infected than in uninfected adults. In a cohort study of HIV-1, an increasing difference in mortality rates of HIV-1-infected and uninfected individuals is expected over time, because of rising mortality with advancing HIV-1 infection. We therefore investigated long-term survival of HIV-2-infected adults.Adults enrolled in 1987 in a community study of HIV-2 infection in Guinea-Bissau were followed up with serological surveys in 1989 and 1992. Survival was assessed in 1995, 9 years after enrollment.The annual incidence of HIV-2 was 0.7% for adults and tended to be higher for older individuals than for participants aged 15-44 years (relative risk 3.21 [95% CI 0.91-11.37]). With control for age, HIV-2-infected adults had twice as high mortality as uninfected individuals (mortality ratio 2.32 [1.18-4.57]); the mortality ratio was highest in the first year of the study (4.50 [1.31-15.43]). The difference between infected and uninfected individuals was stronger for adults under 45 years of age (mortality ratio 4.72 [1.86-11.97]) than for older people (1.35 [0.51-3.56]). HIV-2-infected individuals living with an infected spouse had significantly higher mortality than HIV-2-infected individuals living with an uninfected spouse (p = 0.027).HIV-2-associated mortality is not increasing with length of follow-up. Mortality in HIV-2-infected adults is only twice as high as that in uninfected individuals. In the majority of adults, HIV-2 has no effect on survival.While HIV-2 infection can lead to AIDS, it takes longer than HIV-1 to induce immunosuppression and AIDS, it is less transmissible, and it is associated with lower mortality than HIV-1 infection. 1329 people from among 100 houses in Guinea-Bissau participated in a 1987 study of HIV seroprevalence in their community. 8.9% of the family members older than age 14 years were infected with HIV-2, as well as 0.6% of the 677 children, but no one was infected with HIV-1. All subjects enrolled in the 1987 study over age 14 were followed up with serological surveys in 1989 and 1992, with their survival assessed in 1995. HIV-2 associated mortality did not increase over time and mortality in HIV-2-infected adults was only twice as high as that among uninfected individuals. In the majority of adults, HIV-2 has no effect upon survival. The difference in mortality between the infected and the uninfected was greater for adults under age 45 years than for older people. Furthermore, HIV-2-infected individuals living with an infected spouse had significantly higher mortality than those living with an uninfected spouse.

We studied the association between HIV-2 infection and bacterial pneumonia, sepsis or pyomyositis, as well as the influence of HIV-2 infection on the clinical outcome in patients with these bacterial infections. A total of 201 consecutive hospitalized patients were included at the Simao Mendes National Hospital in Bissau, Guinea-Bissau. Age- and sex-matched controls were selected from an ongoing census in a semi-urban area of Bissau. Among 201 cases with such bacterial infection the prevalence of HIV-1 was 5.4%, HIV-2, 27.9%, and both HIV-1 and HIV-2 reactivity 6.4%. Among controls, the corresponding prevalence rates were significantly lower, 1.5%, 9.0% and 1.0%, respectively. A total of 140, 31 and 30 cases of pneumonia, sepsis and pyomyositis were included, and the differences in prevalence of HIV-2 compared with the controls also remained significant for each diagnosis separately. Lymphocyte subsets were determined in 93 consecutive patients, and the CD4 cell counts and CD4/CD8 lymphocyte ratios were markedly suppressed in the HIV-2-seropositive group. Due to excess mortality in the seropositive groups with sepsis (75.0%) and pyomyositis (25.0%), the mortality during hospitalization was significantly higher among HIV-2 infected compared to HIV-negative patients. Among cases of pneumonia the mortality was low in the HIV-2-seropositive (2.9%) as well as in the HIV-seronegative (3.4%) group.The association between HIV-2 infection and bacterial pneumonia, sepsis, and pyomyositis was examined in 201 consecutive patients hospitalized at Simao Mendes National Hospital in Bissau, Guinea-Bissau with such bacterial diseases and 201 age- and sex-matched controls drawn from a census in a semi-urban area of Bissau. Among cases, HIV-1 prevalence was 5.4%, HIV-2 prevalence was 27.9%, and combined HIV-1 and HIV-2 prevalence was 6.4%. Among controls, these prevalences were 1.5%, 9.0%, and 1.0%, respectively. The prevalence of HIV-2 infection was 25.0% among cases with pneumonia (n = 140), 38.7% among those with sepsis (n = 31), and 30.0% among those with pyomyositis (n = 30). Among the 93 cases for whom T lymphocytes were determined, the absolute number and percentage of CD4 cells and the CD4/CD8 cell ratios were markedly suppressed in the HIV-2-positive group, especially in those with sepsis. Of the 194 patients available for follow-up, 160 were classified as cured or improved, 10 did not improve, and 24 died. Mortality from sepsis and pyomyositis was significantly greater among HIV-2-infected cases than HIV-negative patients. The median percentage of CD4 cells was significantly lower among HIV-2-positive patients who died (9.0%) than survivors (16.5%). These findings confirm the existence of a significant association between HIV-2 and severe bacterial infections as well as a significantly higher mortality during hospitalization from sepsis and pyomyositis in HIV-2-positive patients compared to HIV-negative patients.

O presente trabalho trata do instituto da responsabilidade aplicado aos empreendedores individuais; enfoca a origem histórica desse instituto, a contar da Revolução Francesa, bem como o entendimento consolidado nas codificações napoleônicas e nas Escolas de Direito desenvolvidas na França: a Escola da Exegese. Em paralelo, analisa as especulações teóricas ocorridas na Escola Histórica de Direito alemã; registra, também, a evolução histórica que os países de tradição romano-germânica obtiveram no arco de dois séculos; além disso, constata e analisa a difusão do instituto da responsabilidade limitada aos empreendedores individuais nos demais países da Europa: França, Portugal, Itália, entre outros, resultando no final, pela adoção, por parte da Comunidade Europeia, da XII Diretiva, que estabelece as sociedades unipessoais de responsabilidade limitada a todos os integrantes da Comunidade; avalia a legislação nacional, bem como sua influência franco-portuguesa e as políticas legislativas produzidas; por fim registra a contribuição que a Ciência Ontopsicológica pode oferecer aos operadores de direito na resolução do problema e sintetiza o trabalho pela necessidade de adequação da legislação nacional aos novos tempos.
