Micro-CT scanning has become an accepted standard for anatomical imaging in small animal disease and genome mutation models. Concurrently, perfusion imaging via tracking contrast dynamics after injection of an iodinated contrast agent is a well-established tool for clinical CT scanners. However, perfusion imaging is not yet commercially available on the micro-CT platform due to limitations in both radiation dose and temporal resolution. Recent hardware developments in micro-CT scanners enable continuous imaging of a given volume through the use of a slip-ring gantry. Now that dynamic CT imaging is feasible, data may be acquired to measure tissue perfusion using a micro-CT scanner (CT Imaging, Erlangen, Germany). However, rapid imaging using micro-CT scanners leads to high image noise in individual time frames. Using the standard filtered backprojection (FBP) image reconstruction, images are prohibitively noisy for calculation of voxel-by-voxel perfusion maps. In this study, we apply prior image constrained compressed sensing (PICCS) to reconstruct images with significantly lower noise variance. In perfusion phantom experiments performed on a micro-CT scanner, the PICCS reconstruction enabled a reduction to 1/16 of the noise variance of standard FBP reconstruction, without compromising the spatial or temporal resolution. This enables a significant increase in dose efficiency, and thus, significantly less exposure time is needed to acquire images amenable to perfusion processing. This reduction in required irradiation time enables voxel-by-voxel perfusion maps to be generated on micro-CT scanners. Sample perfusion maps using a deconvolution-based perfusion analysis are included to demonstrate the improvement in image quality using the PICCS algorithm.

Purpose: To evaluate the potential of MSCT and a novel AmicroCT system to assess the volume of malignant brain tumors in rats compared to histology., Materials and Methods: Fourteen rats underwent stereotactic implantation of GFP-marked F 98-glioma cells. On day 10 after implantation, animals received double-dose contrast-enhanced AmicroCT and MSCT imaging using Iomeprol. MSCT- and AmicroCT-derived tumor volumes were calculated and compared to histology (fluorescence staining) as the gold standard., Results: There was good correlation between the AmicroCT-derived tumor volume (69 A+ or - 23 mm(3)) and histology (81 A + or - 14 mm(3); p > 0.14). MSCT, however, showed significantly smaller tumor volumes (55 A + or - 25 mm(3)) compared to histology (p < 0.01) but was able to detect the tumors in all animals., Conclusion: AmicroCT allows in vivo imaging of the contrast-enhancing parts of experimental gliomas with high correlation to histology. Although MSCT is less suitable for assessing exact tumor volume, this method reliably detects tumors in rats. Due to the high spatial resolution, AmicroCT-systems could play an important role for fusion imaging, e. g. to assess experimental brain gliomas with multimodal AmicroCT/PET- or AmicroCT/MRI-fusion images., (A A Georg Thieme Verlag KG Stuttgart A New York.)

Background and Purpose: Metallic implants induce massive artifacts in CT images which deteriorate image quality and often superimpose structures of interest. The purpose of this study was to apply and evaluate a dedicated MAR method for neuroradiologic intracranial clips and detachable platinum coiling events. We here report the first clinical results for MAR in FDCT., Materials and Methods: FDCT volume scans of several patients treated with endovascular coiling or intracranial clipping were corrected by using a dedicated FDCT MAR correction algorithm combined with an edge-preserving attenuation-normalization method in the projection space. Corrected and uncorrected images were compared by 2 experienced radiologists and evaluated for several image-quality features., Results: After application of our algorithm, implant delineation and visibility were highly improved. CT values compared with values in metal artifact-unaffected areas showed good agreement (average correction of 1300 HU). Image noise was reduced overall by 27%. Intracranial hemorrhage in the direct surroundings of the implanted coil or clip material was displayed without worrisome metal artifacts, and our algorithm even allowed diagnosis in areas where extensive information losses were seen. The high spatial resolution provided by FDCT imaging was well preserved., Conclusions: Our MAR method provided metal artifact-reduced images in every studied case. It reduced image noise and corrected CT values to levels comparable with images measured without metallic implants. An overall improvement of brain tissue modeling and implant visibility was achieved. MAR in neuroradiologic FDCT imaging is a promising step forward for better image quality and diagnosis in the presence of metallic implants.

Currently, CT scanning is often performed using flat detectors which are mounted on C-arm units or dedicated gantries as in radiation therapy or micro CT. For perspective cone-beam backprojection of the Feldkamp type (FDK) the geometry of an approximately circular scan trajectory has to be available for reconstruction. If the system or the scan geometry is afflicted with geometrical instabilities, referred to as misalignment, a non-perfect approximate circular scan is the case. Reconstructing a misaligned scan without knowledge of the true trajectory results in severe artefacts in the CT images. Unlike current methods which use a pre-scan calibration of the geometry for defined scan protocols and calibration phantoms, we propose a real-time iterative restoration of reconstruction geometry by means of entropy minimization. Entropy minimization is performed combining a simplex algorithm for multi-parameter optimization and iterative graphics card (GPU)-based FDK-reconstructions. Images reconstructed with the misaligned geometry were used as an input for the entropy minimization algorithm. A simplex algorithm changes the geometrical parameters of the source and detector with respect to the reduction of entropy. In order to reduce the size of the high-dimensional space required for minimization, the trajectory was described by only eight fix points. A virtual trajectory is generated for each iteration using a least-mean-squares algorithm to calculate an approximately circular path including these points. Entropy was minimal for the ideal dataset, whereas strong misalignment resulted in a higher entropy value. For the datasets used in this study, the simplex algorithm required 64-200 iterations to achieve an entropy value equivalent to the ideal dataset, depending on the grade of misalignment using random initialization conditions. The use of the GPU reduced the time per iteration as compared to a quad core CPU-based backprojection by a factor of 10 resulting in a total of 15-20 ms per iteration, and thus providing an online geometry restoration after a total computation time of approximately 1-3 s, depending on the number of iterations. The proposed method provides accurate geometry restoration for approximately circular scans and eliminates the need for an elaborate off-line calibration for each scan. If a priori information about the trajectory is used to initialize the simplex algorithm, it is expected that the entropy minimization will converge significantly faster.

Objectives: We evaluated the influence of heart rate on image quality and diagnostic accuracy of dual-source computed tomography (DSCT) coronary angiography., Background: Multidetector computed tomography (MDCT) coronary angiography has demonstrated an inverse relationship between heart rate and image quality. Dual-source CT provides a higher temporal resolution., Methods: One hundred patients were studied by DSCT (DEFINITION, Siemens Medical Solutions, Forchheim, Germany). A contrast-enhanced volume dataset was acquired (two tubes, 120 kV, 400 mAs/rot, collimation 64 x 0.6 mm). Datasets were evaluated concerning the presence of significant coronary stenoses and validated against invasive coronary angiography., Results: In 44 patients with a heart rate > or =65 beats/min, 566 of 616 coronary segments were evaluable (92%), whereas in 56 patients with a heart rate <65 beats/min, 777 of 778 coronary segments were evaluable (100%, p < 0.001). On a per-patient basis, 93% of patients (> or =65 beats/min) and 100% of patients (<65 beats/min) were considered evaluable. By classifying unevaluable segments as positive for stenosis, per-patient sensitivity was 95% (19 of 20) for heart rates > or =65 beats/min and 100% (22 of 22) for heart rates <65 beats/min. Specificity was 87% (21 of 24) versus 76% (26 of 34), and overall diagnostic accuracy was 91% (40 of 44) versus 86% (48 of 56). None of these differences were statistically significant. Similarly, no difference in diagnostic accuracy was found in per-vessel and -segment analyses., Conclusions: In 100 patients studied without beta-blocker pre-medication, DSCT demonstrated slightly lower per-segment evaluability for high heart rates but no decrease in diagnostic accuracy for the detection of coronary artery stenoses.

If the object exceeds the field of measurement (FOM) of a given CT scanner, severe artifacts may result. In this work, we propose an adaptive detruncation (ADT) method to reconstruct images from medical CT projections which are truncated in the transaxial direction. The truncated projections are extrapolated by estimating the convex hull of the patient. The ADT method allows us not only to achieve artifact-free images in the FOM but also to extend the images beyond the FOM, and can therefore be very attractive, for example, in PET/CT scanners for attenuation correction.

This paper presents the adaptive axial interpolator (AAI), a novel spiral interpolation approach for multislice spiral computed tomography (CT) implemented in a clinical multislice CT scanner, the SOMATOM Volume Zoom (Siemens Medical Systems, Forchheim, Germany). The method works on parallel-beam data generated from the acquired fan-beam data by azimuthal rebinning. Spiral interpolation is performed by distance-dependent weighting; i.e., for each ray, its distance to the image plane is evaluated and serves as an argument to a freely selectable weighting function, resulting in a weight factor. A normalization step is applied to the weight factors to ensure that the sum of all corresponding weights (i.e., the weights applied to rays that contribute to the same ray in the interpolated sinogram) is 1. By selection of appropriate weighting functions and suitable adjustment of the tube current, it is possible to keep the slice sensitivity profiles (SSP) as well as the pixel noise constant for all pitch values in the relevant range. Also, a large range of slice-thickness can be reconstructed from a given collimation. The method is, thus, very versatile. Further advantages are that it uses the entire applied dose for imaging and allows for efficient implementation using a table lookup approach.

The new spiral multislice computed tomography (CT) scanners and the significant increase in rotation speed offer great potential for cardiac imaging with X-ray CT. We have therefore developed the dedicated cardiac reconstruction algorithms 180 degrees multislice cardio interpolation (MCI) and 180 degrees multislice cardio delta (MCD) and here offer further details and validation. The algorithm 180 degreesMCI is an electrocardiogram (ECG)-correlated filtering (or weighting) algorithm in both the cardiac phase and in the z-position. Effective scan times (absolute temporal resolution) of as low as t(eff) = 56 ms are possible, assuming M 4 simultaneously measured slices at a rotation time of t(rot) = 0.5 s and S < or = d < or = 3S for the table feed d per rotation, where S denotes the collimated slice thickness. The relative temporal resolution w (fraction of the heart cycle depicted in the image), which is the more important parameter in cardiac imaging, will then be as low as w = 12.5% of the heart cycle. The second approach, 180 degreesMCD, is an ECG-correlated partial scan reconstruction of 180 degrees + delta data with delta << phi (fan-angle). Its absolute temporal resolution lies in the order of 250 ms (for the central ray, i.e., for the center of rotation), and the relative temporal resolution w increases with increasing heart rate, e.g., from typically w = 25% at fH = 60 min(-1) to w = 50% at fH = 120 min(-1), assuming again t(rot) = 0.5 s. For validation purposes, we have done simulations of a virtual cardiac motion phantom, measurements of a dedicated cardiac calibration and motion phantom, and we have reconstructed patient data with simultaneously acquired ECG. Both algorithms significantly improve the image quality compared with the standard reconstruction algorithms 180 degrees multislice linear interpolation (MLI) and 180 degrees multislice filtered interpolation (MFI). However, 180 degreesMCI is clearly superior to 180 degreesMCD for all heart rates. This is best illustrated by multiplanar reformations (MPR) or other three-dimensional (3-D) displays of the volume. 180 degreesMCI, due to its higher temporal resolution, is best for spatial and temporal four-dimensional (4-D) tracking of the anatomy. A tunable scanner rotation time to avoid resonance behavior of the heart rate and the scanner's rotation and shorter rotation times would be of further benefit.

The recently introduced multislice data acquisition for computed tomography (CT) is based on multirow detector design, increased rotation speed, and advanced z-interpolation and z-filtering algorithms. We evaluated slice sensitivity profiles (SSPs) and noise of a clinical multislice spiral CT (MSCT) scanner with M = 4 simultaneously acquired slices and adaptive axial interpolator (AAI) reconstruction software. SSPs were measured with a small gold disk of 50 microm thickness and 2-mm diameter located at the center of rotation (COR) and 100 mm off center. The standard deviation of CT values within a 20-cm water phantom was used as a measure of image noise. With a detector slice collimation of S = 1.0 mm, we varied spiral pitch p from 0.25 to 2.0 in steps of 0.025. Nominal reconstructed slice thicknesses were 1.25, 1.5, and 2.0 mm. For all possible pitch values, we found the full-width at half maximum (FWHM) of the respective sensitivity profile at the COR equivalent to the selected nominal slice thickness. The profiles at 100 mm off center are broadened less than 7 % on the average compared with the FWHM at the COR. In addition, variation of the full-width at tenth maximum (FWTM) at the COR was below 10% for p < or = 1.75. Within this range, image noise varied less than 10% with respect to the mean noise level. The slight increase in measured slice-width above p = 1.75 for nominal slice-widths of 1.25 and 1.50 mm is accompanied by a decrease of noise according to the inverse square root relationship. The MSCT system that we scrutinized provides reconstructed slice-widths and image noise, which can be regarded as constant within a wide range of table speeds. With respect to this, MSCT is superior to single-slice spiral CT. These facts can be made use of when defining and optimizing clinical protocols: the spiral pitch can be selected almost freely, and scan protocols can follow the diagnostic requirements without technical restrictions. In summary, MSCT offers constant image quality while scan times are reduced drastically. Volume scans with three-dimensional (3-D) isotropic resolution are routinely feasible for complete anatomical regions.

Multislice spiral CT offers many new possibilities for clinical CT imaging. Drastically increased scan speeds and z-resolution, respectively, as well as applications such as cardiac CT that have become feasible for the first time in routine clinical use. The concept of multiple simultaneously acquired slices yields image quality equivalent or better than single-slice spiral CT. Especially, there is no dependence on spiral pitch, neither with regard to noise nor to slice sensitivity. The reconstructed slice width can be chosen freely and retrospectively, which offers additional flexibility when evaluating optimal protocols for various kinds of examinations. Three-dimensional isotropic resolution can be achieved routinely with examinations fast enough to scan in a single breath hold (Fig. 9). Without any drawbacks in image quality, MSCT in combination with online tube current modulation can reduce patient dose. In some body regions, dose is decreased to 50% compared to a scan with constant tube current. One of the most promising new applications is the dedicated ECG-gated cardiac interpolation 180 degrees MCI, which allows four-dimensional (4-D) imaging of the heart. The complete beating heart can be reconstructed in well-defined phases of the heart cycle, thereby adding high temporal resolution to isotropic 3-D spatial resolution (for more examples refer to http://www.imp.uni-erlangen.de/e/research/cardio/).

Subsecond spiral computed tomography (CT) offers great potential for improving heart imaging. The new multi-row detector technology adds significantly to this potential. We therefore developed and validated dedicated cardiac reconstruction algorithms for imaging the heart with subsecond multi-slice spiral CT utilizing electrocardiogram (ECG) information. The single-slice cardiac z-interpolation algorithms 180 degrees CI and 180 degrees CD [Med. Phys. 25, 2417-2431 (1998)] were generalized to allow imaging of the heart for M-slice scanners. Two classes of algorithms were investigated: 180 degrees MCD (multi-slice cardio delta), a partial scan reconstruction of 180 degrees + delta data with a < phi (fan angle) resulting in effective scan times of 250 ms (central ray) when a 0.5 s rotation mode is available, and 180 degrees MCI (multi-slice cardio interpolation), a piecewise weighted interpolation between successive spiral data segments belonging to the same heart phase, potentially providing a relative temporal resolution of 12.5% of the heart cycle when a four-slice scanner is used and the table increment is chosen to be greater than or equal to the collimated slice thickness. Data segments are selected by correlation with the simultaneously recorded ECG signal. Theoretical studies, computer simulations, as well as patient measurements were carried out for a multi-slice scanner providing M = 4 slices to evaluate these new approaches and determine the optimal scan protocol. Both algorithms, 180 degrees MCD and 180 degrees MCI, provide significant improvements in image quality, including extremely arythmic cases. Artifacts in the reconstructed images as well as in 3D displays such as multiplanar reformations were largely reduced as compared to the standard z-interpolation algorithm 180 degrees MLI (multi-slice linear interpolation). Image quality appears adequate for precise calcium scoring and CT angiography of the coronary arteries with conventional subsecond multislice spiral CT. It turned out that for heart rates fH > or = 70 min(-1) the partial scan approach 180 degrees MCD yields unsatisfactory results as compared to 180 degrees MCI. Our theoretical considerations show that a freely selectable scanner rotation time chosen as a function of the patient's heart rate, would further improve the relative temporal resolution and thus further reduce motion artifacts. In our case an additional 0.6 s mode besides the available 0.5 s mode would be very helpful. Moreover, if technically feasible, lower rotation times such as 0.3 s or even less would result in improved image quality. The use of multi-slice techniques for cardiac CT together with the new z-interpolation methods improves the quality of heart imaging significantly. The high temporal resolution of 180 degrees MCI is adequate for spatial and temporal tracking of anatomic structures of the heart (4D reconstruction).

Motivation: Usually, phantoms are used to quantify properties of computer algorithms working on spiral CT data. The high complexity of the human bronchial tree, however, complicates the construction of a physical phantom enormously; hence, a virtual phantom must be employed., Methods: Because the human bronchial tree is fractal, Lindenmayer systems were utilized to generate its structure. Applying the correct diameter and length ratios and branching angles, a virtual anthropomorphic phantom could be generated by a three-dimensional turtle graphic. The simulation of a spiral CT measurement on the virtual phantom delivered synthetic spiral CT data., Results: The computer-controlled growing process supplies exact morphologic and morphometric data. The spiral CT simulation saves the time-consuming and complicated generation of a physical phantom., Perspective: The virtual phantom can be completed by the introduction of blood vessels and lung parenchyma partitioned into lobes and segments. The field of application can thus be extended.

X-ray computed tomography (CT) has exhibited rapid technological advances in recent years which has made it a potential alternative to electron beam computed tomography (EBCT). In addition to the development of spiral CT, rotation times in the subsecond range and the development of multislice detectors were decisive steps. These technical developments will be sketched briefly; the necessary adaptation of image reconstruction and evaluation will be explained. Particular emphasis will be placed on the necessary steps of quality assurance and calibration in quantitative procedures, as for example coronary calcium measurements. The exposure for typical CT examinations of the heart are in the order of half to five times the natural exposure per year. With synchronous recording of the ECG, the complete heart can be imaged continuously with thin slices and high spatial resolution in less than 30 s. In spite of the very short validation phase up to now, we consider multislice spiral CT an alternative to EBCT.

Since modern imaging modalities deliver huge amounts of data the visualization techniques utilized must emphasize the structures of interest. To compare them, the different visualization techniques are regressed to a common ground whereby their strengths and weaknesses can be revealed. Perspective, lighting, color and motion support the real world look and feel. Virtual 3D techniques allow the observer to move through 3D images of anatomy almost in real time. In virtual cisternoscopy this concept is applied to 3D data of the head to reveal a closer look at the cerebral vessels. The more realistic presentation of the 3D data allows for higher efficiency.

Vascular investigations by CT have experienced a decisive advance and found a high acceptance since the introduction of fast volume scanning (spiral CT). We have investigated the underlying physical foundations and optimized the operational aspects of the method now introduced as CT angiography (CTA). Investigations are carried out with a table feed of 1-10 mm/s. Images are reconstructed at 1-2 mm separations by use of algorithms which optimize the layer profile. The parameters must be adapted to the region being investigated. The diagnosis is generally made with interactive cine runs; for this the original images, multiplanar reformations, 3D surface shaded displays (SSD), and maximum intensity projection (MIP) images are used. The 3D representations are discussed in the context of the principle and illustrative examples. Important applications for CTA are the evaluation of aortic aneurysms and dissections, pulmonary vessels, renal arteries, and vessel stents. CTA is characterized by short examination times, low invasiveness, and relatively low cost; in typical cases it is associated with an effective dose of 2-10 mSv. The advantages and disadvantages of the new method are discussed in terms of diagnostic value, image quality, patient dose, contrast medium techniques, and practical aspects in comparison to other angiographic methods.

Accurate determination of intracochlear electrode position in patients with cochlear implants could provide a basis for detecting migration of the implant and could aid in the selection of stimulation parameters for sound processor programming. New computer algorithms for submillimeter resolution and 3-D reconstruction from spiral computed tomographic (CT) scans now make it possible to accurately determine the position of implanted electrodes within the cochlear canal. The accuracy of these algorithms was tested using an electrode array placed in a phantom model. Measurements of electrode length and interelectrode distance from spiral CT scan reconstructions were in close agreement with those from stereo microscopy. Although apparent electrode width was increased on CT scans due to partial volume averaging, a correction factor was developed for measurements from conventional radiographs and an expanded CT absorption value scale added to detect the presence of platinum electrodes and wires. The length of the cochlear canal was calculated from preoperative spiral CT scans for one patient, and the length of insertion of the electrode array was calculated from her postoperative spiral CT scans. The cross-sectional position of electrodes in relation to the outer bony wall and modiolus was measured and plotted as a function of distance with the electrode width correction applied.

A technique to reduce metallic implant artifacts on computed tomography scans is presented. The implant boundaries are determined semiautomatically; the missing projection data are replaced by linear interpolation. The complete procedure requires 1-2 minutes per scan. Images with greatly improved quality were obtained in the presence of surgical clips and pelvic implants; success is limited in highly structured regions, such as the facial skull.
