Objective: To analyze the prognostic factors for clinical outcomes and cartilage regeneration after the implantation of allogeneic human umbilical cord blood mesenchymal stem cell (hUCB-MSC) for treating large-sized cartilage defects with osteoarthritis., Design: This study is a case-series with multiple subgroup analyses that divides the included patients into multiple subgroups based on various factors. Overall, 47 patients who underwent hUCB-MSC implantation were included. The patient-reported outcomes, magnetic resonance imaging (MRI), and second-look arthroscopy were used to assess the outcomes., Results: Combined realignment surgery significantly correlated with clinical outcomes, particularly pain. No other factors significantly influenced the clinical outcomes in short-term period. Subgroups with large defect sizes or meniscal insufficiency showed significantly poor MRI and arthroscopy outcomes (MRI, P = 0.001, P = 0.001; arthroscopy, P = 0.032, P = 0.042). The logistic regression showed that patients with a 1 cm 2 larger defect size were 1.91 times less likely to achieve favorable MRI outcomes ( P = 0.017; odds ratio [OR], 1.91). Cut-off value to predict the poor outcome was >5.7 cm 2 (area under the curve, 0.756). A cartilage defect size >5.7 cm 2 was the major poor prognostic factor for cartilage regeneration on MRI ( P = 0.010; OR, 17.46). If the postoperative alignment shifted by 1° opposite to the cartilage defect, it was 1.4 times more likely to achieve favorable MRI outcomes ( P = 0.028; OR, 1.4)., Conclusion: Combining realignment surgery showed a better prognosis for pain improvement. Cartilage defect size, meniscal function, and postoperative alignment are significant prognostic factors for cartilage regeneration. In this prospective, multi-reader, multi-vendor study, we evaluated the performance of a commercially available deep neural network (DNN)-based MR image reconstruction in enabling accelerated 2D fast spin-echo (FSE) knee imaging. Forty-five subjects were prospectively enrolled and randomly divided into three 3T MRIs. Conventional 2D FSE and accelerated 2D FSE sequences were acquired for each subject, and the accelerated FSE images were reconstructed and enhanced with DNN-based reconstruction software (FSE-DNN). Quantitative assessments and diagnostic performances were independently evaluated by three musculoskeletal radiologists. For statistical analyses, paired t-tests, and Pearson's correlation were used for image quality comparison and inter-reader agreements. Accelerated FSE-DNN reduced scan times by 41.0% on average. FSE-DNN showed better SNR and CNR (p < 0.001). Overall image quality of FSE-DNN was comparable (p > 0.05), and diagnostic performances of FSE-DNN showed comparable lesion detection. Two of cartilage lesions were under-graded or over-graded (n = 2) while there was no significant difference in other image sets (n = 43). Overall inter-reader agreement between FSE-conventional and FSE-DNN showed good agreement (R 2  = 0.76; p < 0.001). In conclusion, DNN-based reconstruction can be applied to accelerated knee imaging in multi-vendor MRI scanners, with reduced scan time and comparable image quality. The magnetic navigation system (MNS) with gradient and uniform saddle coils is an effective system for manipulating various medical magnetic robots because of its compact structure and the uniformity of its magnetic field and field gradient. Since each coil of the MNS was geometrically optimized to generate strong uniform magnetic field or field gradient, it is considered that no special optimization is required for the MNS. However, its electrical characteristics can be still optimized to utilize the maximum power of a power supply unit with improved operating time and a stronger time-varying magnetic field. Furthermore, the conventional arrangement of the coils limits the maximum three-dimensional (3D) rotating magnetic field. In this paper, we propose an electrical optimization method based on a novel arrangement of the MNS. We introduce the objective functions, constraints, and design variables of the MNS considering electrical characteristics such as resistance, current density, and inductance. Then, we design an MNS using an optimization algorithm and compare it with the conventional MNS; the proposed MNS generates a magnetic field or field gradient 22% stronger on average than that of the conventional MNS with a sevenfold longer operating time limit, and the maximum three-dimensional rotating magnetic field is improved by 42%. We also demonstrate that the unclogging performance of the helical robot improves by 54% with the constructed MNS.

Obtaining multiple series of magnetic resonance (MR) images with different contrasts is useful for accurate diagnosis of human spinal conditions. However, this can be time consuming and a burden on both the patient and the hospital. We propose a Bloch equation-based autoencoder regularization generative adversarial network (BlochGAN) to generate a fat saturation T2-weighted (T2 FS) image from T1-weighted (T1-w) and T2-weighted (T2-w) images of human spine. To achieve this, our approach was to utilize the relationship between the contrasts using Bloch equation since it is a fundamental principle of MR physics and serves as a physical basis of each contrasts. BlochGAN properly generated the target-contrast images using the autoencoder regularization based on the Bloch equation to identify the physical basis of the contrasts. BlochGAN consists of four sub-networks: an encoder, a decoder, a generator, and a discriminator. The encoder extracts features from the multi-contrast input images, and the generator creates target T2 FS images using the features extracted from the encoder. The discriminator assists network learning by providing adversarial loss, and the decoder reconstructs the input multi-contrast images and regularizes the learning process by providing reconstruction loss. The discriminator and the decoder are only used in the training process. Purpose: To show the effectiveness of magnetic resonance imaging (MRI) for the detection of tram track lesions in the ankle compared with ankle arthroscopy., Methods: We retrospectively assessed all patients who underwent arthroscopic ankle surgery between January 2013 and July 2015. Patients with anterior impingement spurs were included, but those with an osteochondral lesion or arthritis were excluded. Anterior ankle bony spurs on preoperative weight-bearing radiographs were scored using an impingement classification system. The 3.0-tesla MRIs were reviewed for tram track lesions (defined as focal high signal intensity along the talar dome cartilage surface on coronal views) and compared with arthroscopic findings. The cartilage defect grade at arthroscopy was stratified according to the International Cartilage Repair Society (ICRS) grading system., Results: Overall, 175 ankles in 170 patients were evaluated. Tram track lesions were identified on MRI in 14 ankles (8.0%) and at arthroscopy in 16 ankles (9.1%). The overall sensitivity of MRI for the detection of tram track lesions was 87.5% and the specificity was 100%. On plain weight-bearing radiographs, of the 16 patients with confirmed tram track lesions on arthroscopy, 4 patients had grade 1, 2 had grade 2, and 10 had grade 3 impingement spurs. Under the ICRS grading system, 4 patients had grade II, 4 had grade III, and 8 had grade IV cartilage defects at arthroscopy. On MRI, 2 patients had grade II (50% of arthroscopy), 4 had grade III (100% of arthroscopy), and 8 had grade IV defects (100% of arthroscopy). The impingement spur grade showed no significant correlation with the arthroscopic ICRS grade of the tram track lesion (P = .609)., Conclusion: Tram track lesions can be confidently detected on MRI with high sensitivity and specificity. Objective: To investigate the interobserver and test-retest reproducibility of T1ρ and T2 measurements of lumbar intervertebral discs using 3T magnetic resonance imaging (MRI)., Materials and Methods: This study included a total of 51 volunteers (female, 26; male, 25; mean age, 54 ± 16.3 years) who underwent lumbar spine MRI with a 3.0 T scanner. Amongst these subjects, 40 underwent repeat T1ρ and T2 measurement acquisitions with identical image protocol. Two observers independently performed the region of interest measurements in the nuclei pulposi of the discs from L1-2 through L5-S1 levels. Statistical analysis was performed using intraclass correlation coefficient (ICC) with a two-way random model of absolute agreement. Comparison of the ICC values was done after acquisition of ICC values using Z test. Statistical significance was defined as p value < 0.05., Results: The ICCs of interobserver reproducibility were 0.951 and 0.672 for T1ρ and T2 mapping, respectively. The ICCs of test-retest reproducibility (40 subjects) for T1ρ and T2 measurements were 0.922 and 0.617 for observer A and 0.914 and 0.628 for observer B, respectively. In the comparison of the aforementioned ICCs, ICCs of interobserver and test-retest reproducibility for T1ρ mapping were significantly higher than T2 mapping ( p < 0.001)., Conclusion: The interobserver and test-retest reproducibility of T1ρ mapping were significantly higher than those of T2 mapping for the quantitative assessment of nuclei pulposi of lumbar intervertebral discs.

Background: Although many three-dimensional (3D) magnetic resonance imaging (MRI) sequences have been used in the clinical field, there are no studies on fat-suppressed (FS) 3D sequences for the diagnosis of knee abnormalities., Purpose: To evaluate the usefulness of FS 3D volume isotropic turbo spin echo acquisition (VISTA) imaging for diagnosis of internal structures of knee., Material and Methods: In 137 patients with 138 knee MRI examinations who had undergone both FS 3D VISTA MRI and two-dimensional (2D) MRI of the knee at 3.0 T, arthroscopic surgery was subsequently performed. Multiplanar reformations (MPR) of FS 3D VISTA images and their MPR sequences were compared with 2D MR images of axial T1-weighted (T1W) images, axial FS T2-weighted (T2W) images, coronal FS T2W images, and sagittal T2W images. Two musculoskeletal radiologists reviewed the images independently., Results: Arthroscopy revealed 33 anterior cruciate ligament tears, three posterior cruciate ligament tears, 42 lateral meniscus tears, and 68 medial meniscus tears. Five medial collateral ligament tears were clinically confirmed. Image acquisition time was shorter for FS 3D VISTA imaging than 2D imaging. There were no significant differences in diagnostic values between FS 3D VISTA and 2D imaging (P > 0.05). There were excellent inter-observer agreements for both FS 3D VISTA and 2D imaging (κ > 0.84)., Conclusion: FS 3D VISTA imaging could replace 2D imaging because of equal diagnostic ability and shorter scan time., (© The Foundation Acta Radiologica 2015.)

Objective: To assess whether multi-echo Dixon magnetic resonance (MR) imaging with simultaneous T2* estimation and correction yields more accurate fat-signal fraction (FF) measurement of the lumbar paravertebral muscles, in comparison with non-T2*-corrected two-echo Dixon or T2*-corrected three-echo Dixon, using the FF measurements from single-voxel MR spectroscopy as the reference standard., Materials and Methods: Sixty patients with low back pain underwent MR imaging with a 1.5T scanner. FF mapping images automatically obtained using T2*-corrected Dixon technique with two (non-T2*-corrected), three, and six echoes, were compared with images from single-voxel MR spectroscopy at the paravertebral muscles on levels L4 through L5. FFs were measured directly by two radiologists, who independently drew the region of interest on the mapping images from the three sequences., Results: A total of 117 spectroscopic measurements were performed either bilaterally (57 of 60 subjects) or unilaterally (3 of 60 subjects). The mean spectroscopic FF was 14.3 ± 11.7% (range, 1.9-63.7%). Interobserver agreement was excellent between the two radiologists. Lin's concordance correlation between the spectroscopic findings and all the imaging-based FFs were statistically significant (p < 0.001). FFs obtained from the T2*-corrected six-echo Dixon sequences showed a significantly better concordance with the spectroscopic data, with its concordance correlation coefficient being 0.99 and 0.98 (p < 0.001), as compared with two- or three-echo methods., Conclusion: T2*-corrected six-echo Dixon sequence would be a better option than two- or three-echo methods for noninvasive quantification of lumbar muscle fat quantification.

Rationale and Objectives: To assess the usefulness of T2*-corrected fat fraction (FF) map from volume-interpolated breathhold examination (VIBE) magnetic resonance (MR) sequence in patients with the rotator cuff pathology., Materials and Methods: The phantom study was performed to validate the FF maps. Eighty-nine shoulder MR arthrographies were analyzed: (1) divided into three groups namely tendinopathy/normal tendons, partial-thickness tears, and full-thickness tears, and (2) occupation ratio (OR) was measured for muscular atrophy. Uncorrected and T2*-corrected FF maps were reconstructed from the VIBE images. The Pearson correlation test was used to correlate the FFs with ORs. The FF and the OR were compared between groups using the Student t test., Results: T2*-corrected FF maps could provide a higher correlation than uncorrected FF maps. There were significantly negative correlations between the ORs and the FFs (P < .01). In the normal and the partial-thickness tear group, the OR did not show a significant difference, although the FF maps showed a significant difference (P < .01)., Conclusions: This quantitative assessment of the T2*-corrected FF in the rotator cuff muscles was found to be reliable and correlated well with the ORs. Background: The use of double-bundle and selective-bundle anterior cruciate ligament (ACL) reconstruction is increasing. Few studies have been conducted to determine the diagnostic efficacy of additional oblique views of knee magnetic resonance imaging (MRI) for grading ACL graft injury for double-bundle and selective-bundle ACL reconstructions., Purpose: To evaluate the diagnostic value of combining oblique coronal and sagittal imaging with orthogonal views for diagnosis of ACL graft failure after double-bundle or selective-bundle ACL reconstruction., Material and Methods: This retrospective study included 64 patients who underwent double-bundle or selective-bundle ACL reconstruction surgery and received oblique coronal and oblique sagittal MRI of the knee. Subjective scoring of the images was performed by two radiologists who assessed the possibility of ACL graft failure based on full length view, full width view, and margin sharpness according to a 4-point scale. Diagnostic performance for ACL graft failure based on orthogonal views alone, ACL views alone, and orthogonal views with additional ACL views was evaluated by calculating the sensitivity, specificity, and accuracy., Results: The full length scores and total scores on ACL views were significantly higher than those of orthogonal views. The specificities and accuracies of diagnoses were highest for a combination of orthogonal, ACL sagittal, and ACL coronal views., Conclusion: After double-bundle or selective-bundle ACL reconstruction, oblique sagittal and coronal images of the ACL provide better anatomic evaluation of the ACL than orthogonal views and specificity and accuracy of diagnoses were improved when oblique views were combined., (© The Foundation Acta Radiologica 2014 Reprints and permissions: sagepub.co.uk/journalsPermissions.nav.)

Objective: To assess the feasibility of T2*-corrected fat-signal fraction (FF) map by using the three-echo volume interpolated breath-hold gradient echo (VIBE) Dixon sequence to differentiate between malignant marrow-replacing lesions and benign red marrow deposition of vertebrae., Materials and Methods: We assessed 32 lesions from 32 patients who underwent magnetic resonance imaging after being referred for assessment of a known or possible vertebral marrow abnormality. The lesions were divided into 21 malignant marrow-replacing lesions and 11 benign red marrow depositions. Three sequences for the parameter measurements were obtained by using a 1.5-T MR imaging scanner as follows: three-echo VIBE Dixon sequence for FF; conventional T1-weighted imaging for the lesion-disc ratio (LDR); pre- and post-gadolinium enhanced fat-suppressed T1-weighted images for the contrast-enhancement ratio (CER). A region of interest was drawn for each lesion for parameter measurements. The areas under the curve (AUC) of the parameters and their sensitivities and specificities at the most ideal cutoff values from receiver operating characteristic curve analysis were obtained. AUC, sensitivity, and specificity were respectively compared between FF and CER., Results: The AUCs of FF, LDR, and CER were 0.96, 0.80, and 0.72, respectively. In the comparison of diagnostic performance between the FF and CER, the FF showed a significantly larger AUC as compared to the CER (p = 0.030), although the difference of sensitivity (p = 0.157) and specificity (p = 0.157) were not significant., Conclusion: Fat-signal fraction measurement using T2*-corrected three-echo VIBE Dixon sequence is feasible and has a more accurate diagnostic performance, than the CER, in distinguishing benign red marrow deposition from malignant bone marrow-replacing lesions.

Background and Purpose: Fluid-sensitive MR imaging in postoperative evaluation is important, however, metallic artifacts is inevitable. The purpose is to investigate the feasibility of fat-saturated slice encoding for metal artifact correction (SEMAC)-corrected T2-weighted magnetic resonance (MR) at 3T in patients with spinal prostheses., Methods: Following institutional review board approval, 27 SEMAC-encoded spinal MRs between September 2012 and October 2013 in patients with spinal metallic prostheses were analyzed. The MR images were scanned on a 3T MR system including SEMAC-corrected and uncorrected fast spin echo (FSE) T2-weighted MR images with fat-saturation. Two musculoskeletal radiologists compared the image sets and qualitatively analyzed the images using a five-point scale in terms of artifact reduction around the prosthesis, visualization of the prosthesis and pedicle, and intervertebral neural foramina. Quantitative assessments were performed by calculating the ratio of signal intensity from the fixated vertebra and that from upper level vertebra. For statistical analyses, paired t-test was used., Results: Fat-saturated SEMAC-corrected T2-weighted MR images enabled significantly improved metallic artifact reduction (P<0.05). Quantitative evaluation of the signal intensity ratio of screw-fixated vertebra and upper level vertebra showed a significantly lower ratio on fat-saturated SEMAC images (P<0.05), however, the high signal intensity of signal pile-up could be not completely corrected., Conclusion: SEMAC correction in fat-suppressed T2-weighted MR images can overcome the signal loss of metallic artifacts and provide improved delineation of the pedicle screw and peri-prosthetic region. Signal pile-up, however, could not be corrected completely, therefore readers should be cautious in the evaluation of marrow around the prosthesis., (Copyright © 2014 Elsevier Inc. Background: Ultrashort echo time (UTE) image to directly visualize short T2 tissues requires postprocessing for the suppression of the surrounding long T2 tissues in a clinical magnetic resonance imaging (MRI) scanner. Weighted subtraction of UTE images with an optimal weighting factor could provide high positive contrast with adequate suppression., Purpose: To demonstrate in-vivo UTE MRI with weighted subtractions of dual echo UTE imaging using a 3 T clinical MRI and to determine the optimal weighting factors through the analyses of signal-to-noise ratios (SNRs) and contrast-to-noise ratios (CNRs)., Material and Methods: Forty-seven consecutive knee MR including dual echo 3D UTE sequence (TE of 70 µs and 3.82 ms) were reviewed. Seven patients with normal findings in the knee MR were included in the current study. For variably weighted subtraction images, the initial UTE image was rescaled relative to that of the second echo image with weighting factors from 0.1 to 4.0. Thirty-five pairs of the ROI measurements of the UTE image and second long echo images were repeated after 2 weeks. For quantitative assessment, SNR and CNR were measured from the ROIs on the patellar tendon, cartilage, cortical bone, meniscus, and infrapatellar fat pad. Intra-observer agreement was assessed by using both Cohen's Kappa and Bland-Altman approach., Results: The short T2 tissue could be visualized with adequate suppression on the subtraction images. Considering the CNRs and SNRs, the optimal ranges of the weighting factors could be suggested: 0.3 for the tendon, 0.4 for the cortical bone, and 1.0 for the meniscus. The 35 paired measurements showed excellent agreement with statistical significance (P < 0.05)., Conclusion: The 3D UTE MRI provides imaging of short T2 tissues which cannot be visualized by conventional MRI. Using weighted subtractions with optimal weighting values, each tissue can be optimally depicted by overcoming the reduced T2 contrast.

Purpose: The purpose of the study was to assess the usefulness of slice encoding for metal artifact correction (SEMAC) in 3.0-T magnetic resonance (MR) in minimizing metallic artifacts in patients with spinal prostheses., Materials and Methods: Institutional review board approval and informed consent were obtained for this study. Twenty-seven spine MR scans were performed with metal artifact reduction SEMAC between May 2011 and July 2012 in patients with metallic devices. The MR scans were performed on a 3-T MR system (Achieva; Philips Healthcare, Best, the Netherlands) including SEMAC-corrected T2-weighted axial/sagittal images and two-dimensional fast spin echo (FSE) axial/sagittal images. The SEMAC-corrected images were compared to conventional T2-weighted FSE images. Two musculoskeletal radiologists qualitatively analyzed the images in terms of visualization of the pedicle, vertebral body, dural sac, intervertebral disc, intervertebral neural foramina, screws and metallic artifacts. The paired images were rated using a 5-point scale. P values less than .05 were considered to indicate statistically significant differences., Results: The SEMAC-corrected MR images significantly reduced the metal-related artifacts. The T2-weighted images with SEMAC sequences enabled significantly improved periprosthetic visualizations of the pedicle, vertebral body, dural sac and neural foramina, with the exception of the intervertebral disc (P<.05). In addition, there was significant improvement in prosthesis visualization (P<.05)., Conclusion: MR images with SEMAC can reduce metal-related artifacts, providing improved delineation of the prosthesis and periprosthetic region. Purpose: The purpose of the study was to validate the diagnostic performance of high-resolution isovolumetric magnetic resonance arthrography (MRA) for intrinsic ligament and triangular fibrocartilage complex (TFCC) tears of the wrist as compared to conventional MR imaging (MRI)., Materials and Methods: Forty-eight patients with traumatic TFCC tears at arthroscopy were enrolled. All patients had underwent proton-density- and T2-weighted MRI before arthrography and three-dimensional T1 high-resolution isovolumetric examination (3D-THRIVE) MRA on a 3-T MR. We assessed the presence of scapholunate interosseous ligament (SLIL)/lunotriquetral interosseous ligament (LTIL) or TFCC tears using the arthroscopy as a gold standard., Results: Arthroscopy revealed 37 TFCC central tears, 15 TFCC peripheral tears, 20 SLIL tears and 13 LTIL tears. Sensitivities of MRI and MRA were 70.3% and 94.6% for detection of TFCC central tears, 60.0% and 93.3% for detection of TFCC peripheral tears, 65.0% and 85.0% for SLIL tears, and 61.5% and 84.6% for LTIL tears. The specificity of the MRI was 100% for the detection of ligaments and TFCC tears. The specificities of the MRA for detection of TFCC central tears, TFCC peripheral tears, SLIL tears and LTIL tears were 100%, 97%, 96.4% and 100%, respectively., Conclusion: Isovolumetric 3D-THRIVE wrist MRA provided better results for depiction of intrinsic ligament and TFCC tears than wrist MRI., (Copyright © 2013 Elsevier Inc. Purpose: To describe the appearance of the infrapatellar plica (IPP) on magnetic resonance arthrography (MRA) taken in 70° knee flexion, corresponding to the arthroscopic posture., Materials and Methods: Twenty-two patients (23 knee joints) who underwent MRA with 70° knee flexion were enrolled. All patients underwent MRA with 70° knee flexion to simulate operative arthroscopy. The images included fat-suppressed T1-weighted spin echo axial, sagittal, and coronal images. The visualization and morphology of the IPP were retrospectively assessed by two musculoskeletal radiologists., Results: The IPP was demonstrated in 78.3% (n=18/23) and was best visualized on the sagittal section through the intercondylar notch. The IPP manifested as a linear hypointense structure with variable thicknesses. The intercondylar component was delineated clearly, arising from the anterior intercondylar notch in parallel with the ACL and curving gently downward to attach to the infrapatellar fat pad. On the other hand, the Hoffa's fat pad component was not depicted clearly. The morphology of the IPP was either a separate type (60.9%) or a split type (17.4%)., Conclusion: The IPPs can be visualized with a high rate of detection and various morphologic appearances must be appreciated under the review of a flexed knee MRA., (Copyright © 2012 Elsevier Ireland Ltd. Purpose: To evaluate meniscofibular ligament (MFibL) at the posterolateral corner of the knee joint on the magnetic resonance arthrography (MRA) with 70° knee flexion., Materials and Methods: The MRA of the knee joint was performed at 70° knee flexion. Eighteen patients (19 knee joints) underwent scanning of sagittal, coronal, and axial fat-suppressed T1 weighted images (T1FS), and coronal fat-suppressed T2 weighted images. Sagittal three-dimensional (3D) gradient echo (GRE) images were also obtained. Retrospective review of 19 knee MRA studies was independently performed by two musculoskeletal radiologists. The statistical significance was proved by chi-square test., Results: The MFibL ligament was optimally demonstrated on the far lateral sagittal 3D GRE and T1FS MRA images. The MFibL appeared as a curvilinear or straight hypointense band of variable thickness, extended from the posterolateral meniscus to upper anteromedial aspect of the fibular head. The MFibL was demonstrated with scale 2 (more than a half length of the ligament) by both reviewers in 73.68% (n=14/19) of the knee 3D GRE images and 89.47% (n=17/19) of the knee T1FS images. The visualization on T1FS and that on GRE were not statistically different from each other (p>0.05). The interobserver agreements were significantly good on both 3D GRE and T1FS images in detecting the ligament (kappa values, 0.642 and 0.683, respectively)., Conclusion: The MFibL is well visualized on the far lateral sagittal MRA at 70° knee flexion, which could potentially be useful in recognizing structures in the posterolateral corner of the knee, including the MFibL.

Scoliosis is defined as a lateral spinal curvature with a Cobb angle of 10° or more. This abnormal curvature may be the result of an underlying congenital or developmental osseous or neurologic abnormality, but in most cases the cause is unknown. Imaging modalities such as radiography, computed tomography (CT), and magnetic resonance (MR) imaging play pivotal roles in the diagnosis, monitoring, and management of scoliosis, with radiography having the primary role and with MR imaging or CT indicated when the presence of an underlying osseous or neurologic cause is suspected. In interpreting the imaging features of scoliosis, it is essential to identify the significance of vertebrae in or near the curved segment (apex, end vertebra, neutral vertebra, stable vertebra), the curve type (primary or secondary, structural or nonstructural), the degree of angulation (measured with the Cobb method), the degree of vertebral rotation (measured with the Nash-Moe method), and the longitudinal extent of spinal involvement (according to the Lenke system). The treatment of idiopathic scoliosis is governed by the severity of the initial curvature and the probability of progression. When planning treatment or follow-up imaging, the biomechanics of curve progression must be considered: In idiopathic scoliosis, progression is most likely during periods of rapid growth, and the optimal follow-up interval in skeletally immature patients may be as short as 4 months. Objective: The objective of this study was to investigate the usefulness of virtual MR arthroscopy as an adjunct to conventional MR arthrography in detecting various anteroinferior glenoid labral lesions., Materials and Methods: The study group was composed of 28 patients (22 men and six women; mean age, 25 years) who underwent direct MR arthrography of the shoulder as well as arthroscopy due to recurrent shoulder dislocations. MR arthrography examinations were reviewed retrospectively with radiologists blinded to the arthroscopic diagnoses. Virtual MR arthroscopy was produced from fat-suppressed 3D gradient-recalled echo MR arthrography using volume-rendering software. The sensitivity, specificity, and accuracy for diagnosis were calculated. The diagnostic performances were validated by receiver operating characteristic curve analysis., Results: The structure of the glenoid labrum was well visualized in a 3D perspective, and characterization of the injured anterior glenoid labrum was aided by adjunct use of virtual MR arthroscopy. The accuracy for detecting classic Bankart, anterior labroligamentous periosteal sleeve avulsion, and Perthes lesions improved to 85.7%, 85.7%, and 92.9%, respectively, with adjunct virtual MR arthroscopy with MR arthrography versus 78.6%, 82.1%, and 89.3%, respectively, with MR arthrography alone. A statistically significant improvement of the diagnostic capability of MR arthrography with adjunct virtual MR arthroscopy was proven for the detection of a classic Bankart lesion (p < 0.05)., Conclusion: Having a 3D perspective to validate chronic recurrent glenoid labral injuries of the shoulder facilitated understanding of the anatomic relationships and helped diagnosis. Adjunct virtual MR arthroscopy is potentially useful for detecting and characterizing chronic recurrent glenoid labral lesions to improve diagnosis.

At magnetic resonance (MR) imaging and multidetector computed tomography (CT), artifacts arising from metallic orthopedic hardware are an obstacle to obtaining optimal images. Although various techniques for reducing such artifacts have been developed and corroborated by previous researchers, a new era of more powerful MR imaging and multidetector CT modalities has renewed the importance of a systematic consideration of methods for artifact reduction. Knowledge of the factors that contribute to artifacts, of related theories, and of artifact reduction techniques has become mandatory for radiologists. Factors that affect artifacts on MR images include the composition of the metallic hardware, the orientation of the hardware in relation to the direction of the main magnetic field, the strength of the magnetic field, the pulse sequence type, and other MR imaging parameters (mainly voxel size, which is determined by the field of view, image matrix, section thickness, and echo train length). At multidetector CT, the factors that affect artifacts include the composition of the hardware, orientation of the hardware, acquisition parameters (peak voltage, tube charge, collimation, and acquired section thickness), and reconstruction parameters (reconstructed section thickness, reconstruction algorithm used, and whether an extended CT scale was used). A comparison of images obtained with different hardware and different acquisition and reconstruction parameters facilitates an understanding of methods for reducing or overcoming artifacts related to metallic implants., ((c) RSNA, 2007.)

Background: The purpose of this study was to determine the sensitivity and specificity of MRI in chronic syndesmosis injury by comparing the arthroscopic findings with MRI findings and to suggest therapeutic guidelines of syndesmotic fixation., Methods: Between January, 2003, and January, 2004, 20 patients were diagnosed with chronic syndesmosis injury according to syndesmosis widening (>2 mm) on arthroscopic examination. The average age of the patients was 32 years. The minimum followup was 22 (22 to 30) months. MRI was obtained in all patients. For evaluating the therapeutic role of syndesmotic fixation, we separated the patients into two groups (with and without transfixation) and compared preoperative and postoperative American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scores and patient satisfaction., Results: The sensitivity, specificity, and accuracy of MRI were 90.0%, 94.8%, and 93.4%, respectively. In group I with syndesmotic fixation, the AOFAS ankle-hindfoot scale improved from 52 to 87, and in group II without syndesmotic fixation the score improved from 63 to 90 at last followup, which showed no statistically significant difference (p = 0.6453). Also, patient satisfaction showed no difference between the two groups., Conclusions: MRI showed high sensitivity, specificity, and accuracy under the criteria we proposed, and we recommend it as the main diagnostic tool for diagnosing a chronic syndesmosis injury. These findings might suggest that pain is mainly caused by hypertrophy and impingement of the soft tissue in the distal tibiofibular joint. Arthroscopic debridement alone can be recommended if the distal tibiofibular chronic syndesmosis injury is not combined with medial ankle instability and lateral displacement of the talus.

Objective: The purpose of this study was to compare the diagnostic accuracy of both ultrasonography (US) and magnetic resonance imaging (MRI) for the assessment of Morton neuroma., Materials and Methods: Our study group was comprised of 20 neuromas from 17 patients, and the neuromas were confirmed on surgery following evaluation with US, MRI, or both US and MRI. The diagnostic criterion for Morton neuroma, as examined by US, was the presence of a round or ovoid, well-defined, hypoechoic mass. The diagnostic criterion, based on MR imaging, was a well defined mass with intermediate to low signal intensity on both the T1- and T2-weighted images. The retrospective comparison between the sonographic and MR images was done by two experienced radiologists working in consensus with the surgical and pathologic correlations., Results: The detection rate of Morton neuroma was 79% for 14 neuromas from 11 patients who had undergone US followed by an operation. The detection rate was 76% for 17 neuromas from 15 patients who had undergone MRI and a subsequent operation. The mean size of the examined neuromas was 4.9 mm on the US images and it was 5.1 mm on the MRI studies. Ten neuromas (71%) were 5 mm or less as measured by US, and three neuromas were not detected, whereas on the MRI analysis, 10 neuromas (59%) were 5 mm or less and four neuromas were not visualized. Among the patients examined during postoperative follow-up, symptoms were completely relieved in 85% and the symptoms were partially relieved in 15%., Conclusion: US and MR imaging are comparable modalities with high detection rate for the evaluation of Morton neuroma.

The diagnosis of nerve entrapment and compressive neuropathy has been traditionally based on the clinical and electrodiagnostic examinations. As a result of improvements in the magnetic resonance (MR) imaging modality, it plays not only a fundamental role in the detection of space-occupying lesions, but also a compensatory role in clinically and electrodiagnostically inconclusive cases. Although ultrasound has undergone further development in the past decades and shows high resolution capabilities, it has inherent limitations due to its operator dependency. We review the course of normal peripheral nerves, as well as various clinical demonstrations and pathological features of compressed and entrapped nerves in the upper extremities on MR imaging, according to the nerves involved. The common sites of nerve entrapment of the upper extremity are as follows: the brachial plexus of the thoracic outlet; axillary nerve of the quadrilateral space; radial nerve of the radial tunnel; ulnar nerve of the cubital tunnel and Guyon's canal; median nerve of the pronator syndrome, anterior interosseous nerve syndrome, and carpal tunnel syndrome. Although MR imaging can depict the peripheral nerves in the extremities effectively, radiologists should be familiar with nerve pathways, common sites of nerve compression, and common space-occupying lesions resulting in nerve compression in MR imaging.

Purpose: To retrospectively determine the accuracy of coronal contrast material-enhanced fat-suppressed three-dimensional (3D) fast spoiled gradient-recalled acquisition in the steady state (SPGR) magnetic resonance (MR) imaging, as compared with that of routine transverse MR imaging, in the assessment of distal tibiofibular syndesmosis injury, with arthroscopy as the reference standard., Materials and Methods: The review board of the College of Medicine in Yonsei University approved this study; informed consent was waived. The study group comprised 45 patients (26 men, 19 women; mean age, 32.1 years; range, 18-58 years) with a chronic ankle injury who had undergone MR imaging and arthroscopic surgery. Three independent readers retrospectively reviewed the two sets of MR images (one set of gadolinium-enhanced 3D fast SPGR images and one set of routine T1-, T2-, and intermediate-weighted images). Scores from 1 to 5 in increasing order of the probability of injury were assigned to both sets. Arthroscopy was the reference standard. Syndesmotic recess height was measured on contrast-enhanced images. The two sets of images were compared for diagnostic performance with receiver operating characteristic (ROC) analysis. Dissection and histologic examination of six cadaveric ankles was performed to assess the syndesmotic area and ascertain the enhancing structure at MR imaging., Results: At arthroscopy, syndesmotic injury was found in 24 ankles but not in 21 ankles. Areas under the ROC curve were significantly higher for the contrast-enhanced images (P<.05). The contrast-enhanced set showed higher accuracy, sensitivity, and specificity compared with the routine set for the assessment of syndesmosis injury. Mean syndesmotic recess height was significantly greater (P<.05) in patients with syndesmotic injury. Dissection and histologic examination revealed a highly vascular synovial fold in the syndesmotic area that is expected to enhance at MR imaging., Conclusion: In the assessment of chronic syndesmosis injury, coronal gadolinium-enhanced fat-suppressed 3D fast SPGR MR images were more sensitive, specific, and accurate than routine MR images., (Copyright (c) RSNA, 2007.)

Successful development of ultra-sensitive molecular imaging nanoprobes for the detection of targeted biological objects is a challenging task. Although magnetic nanoprobes have the potential to perform such a role, the results from probes that are currently available have been far from optimal. Here we used artificial engineering approaches to develop innovative magnetic nanoprobes, through a process that involved the systematic evaluation of the magnetic spin, size and type of spinel metal ferrites. These magnetism-engineered iron oxide (MEIO) nanoprobes, when conjugated with antibodies, showed enhanced magnetic resonance imaging (MRI) sensitivity for the detection of cancer markers compared with probes currently available. Also, we successfully visualized small tumors implanted in a mouse. Such high-performance, nanotechnology-based molecular probes could enhance the ability to visualize other biological events critical to diagnostics and therapeutics.

The common sites of nerve entrapment of the lower extremity are as follows: sciatic nerve around the piriformis muscle; tibial nerve at the popliteal fossa and tarsal tunnel, common peroneal nerve around the fibular neck, and digital nerve near the metatarsal head. Although MR imaging can depict the peripheral nerves in the extremities effectively, radiologists should be familiar with nerve pathways, common sites of nerve compression, and common space-occupying lesions resulting in nerve compression in MR imaging.

Objective: The purpose of this study was to identify the intermalleolar ligament morphologically and to correlate its shape with MR images., Materials and Methods: Seventy-seven ankles were used in this study. After the intermalleolar ligament had been located in the posterior ankle space, its medial and lateral attaching sites were identified, and its length, width, and thickness were measured. MRI was performed on 26 ankles before they were dissected (20 specimens) or serially sectioned (six specimens). The serial sections were taken at a thickness of 2 mm in the sagittal and horizontal directions., Results: The intermalleolar ligament was observed in 81.8% of the specimens and was composed of more than two bundles of fibers in all cases. The medial arising sites of the ligament were diverse (e.g., from the medial malleolus to the floor of the fibrous tunnel of the flexor hallucis longus). The ligament narrowed laterally and attached with the posterior talofibular ligament to the medial fossa of the lateral malleolus. Their morphologic shapes were also diverse, depending on their medial arising sites, the number of the composing fiber bundles, and the degree of bundle compactness. The intermalleolar ligament appeared as a thick string or as more than two fine parallel stripes on coronal MR images and as a linear structure on axial images. On sagittal images, the ligament appeared as scattered dots in the medial part and as a thin flat or nodular structure in the lateral part., Conclusion: The intermalleolar ligament seemed to be an almost invariably present anatomic entity with diverse morphologic features on MR images.

Purpose: To introduce and evaluate the usefulness of the adduction internal rotation (ADIR) position in MR arthrography for discriminating the following subtypes of Bankart lesions: classic Bankart lesions, anterior labroligamentous periosteal sleeve avulsion (ALPSA) lesions, and Perthes lesions., Materials and Methods: The study group consisted of 25 patients who had been referred for MR arthrography of the shoulder and then underwent arthroscopy due to recurrent episodes of shoulder dislocation. MR arthrography was performed in three different positions: neutral, abduction external rotation (ABER), and ADIR. The authors evaluated the statistical significance of the method's discriminative diagnostic ability for the subtypes of Bankart lesions according to the position changes., Results: Twenty-three patients had the following single or complex labral lesions: classic Bankart (N = 8), ALPSA (N = 3), complex classic Bankart/ALPSA (N = 8), and complex ALPSA/Perthes (N = 4). Two patients had no visible anteroinferior labral lesions. The performance of ADIR positioning for differentiating ALPSA lesions was superior to the neutral or ABER positioning. The difference of the discriminative lesion detection ability was statistically significant (P <.01) only for detecting ALPSA lesions in the ADIR position., Conclusion: MR arthrography in the ADIR position provides high accuracy for the diagnosis of ALPSA lesions, and complements routine MR arthrography when used to diagnose labroligamentous lesions in patients with recurrent shoulder dislocations.

The unique properties of magnetic nanocrystals provide them with high potential as key probes and vectors in the next generation of biomedical applications. Although superparamagnetic iron oxide nanocrystals have been extensively studied as excellent magnetic resonance imaging (MRI) probes for various cell trafficking, gene expression, and cancer diagnosis, further development of in vivo MRI applications has been very limited. Here, we describe in vivo diagnosis of cancer, utilizing a well-defined magnetic nanocrystal probe system with multiple capabilities, such as small size, strong magnetism, high biocompatibility, and the possession of active functionality for desired receptors. Our magnetic nanocrystals are conjugated to a cancer-targeting antibody, Herceptin, and subsequent utilization of these conjugates as MRI probes has been successfully demonstrated for the monitoring of in vivo selective targeting events of human cancer cells implanted in live mice. Further conjugation of these nanocrystal probes with fluorescent dye-labeled antibodies enables both in vitro and ex vivo optical detection of cancer as well as in vivo MRI, which are potentially applicable for an advanced multimodal detection system. Our study finds that high performance in vivo MR diagnosis of cancer is achievable by utilizing improved and multifunctional material properties of iron oxide nanocrystal probes.

Since the use of magnetic nanocrystals as probes for biomedical system is attractive, it is important to develop optimal synthetic protocols for high-quality magnetic nanocrystals and to have the systematic understanding of their nanoscale properties. Here we present the development of a synthetically controlled magnetic nanocrystal model system that correlates the nanoscale tunabilities in terms of size, magnetism, and induced nuclear spin relaxation processes. This system further led to the development of high-performance nanocrystal-antibody probe systems for the diagnosis of breast cancer cells via magnetic resonance imaging.

Objective: We wanted to assess the role of the popliteal lymph nodes for differentiating rheumatoid arthritis (RA) from osteoarthritis (OA), and we also wanted to investigate the relationship between the popliteal lymph nodes and the inflamed synovial volume (ISV) by using contrast enhanced (CE), fat suppressed, three dimensional-fast spoiled gradient echo (3D-FSPGR) MR imaging., Materials and Methods: Contrast enhanced 3D-FSGPR MR imaging of 94 knees (21 with RA and 73 with OA) was analyzed. A lymph node was defined as being 'observed' if it could be seen in at least two planes of the three orthogonal reformatted planes. The number of observed lymph nodes, the mean of the smallest dimension of each lymph node and the existence of central fatty change were recorded. The OA group was graded according to the ISV calculated by a segmentation method: grade I was < 20 cm(3); grade II ranged from 20 cm(3) to 40 cm(3); and grade III was > 40 cm(3). Statistical analysis of the number and the mean size of the popliteal lymph nodes among the four groups (the RA group and the grade I-III OA groups) was performed., Results: The prevalence of the observed popliteal lymph nodes was significantly different between all the OA groups or between the grade III OA group and the RA group (p < 0.0001, 0.0001, respectively). The popliteal lymph node was observed in 32 out of 73 OA cases, whereas it was visible in all of the 21 RA cases. The number (mean +/- standard deviation) of lymph nodes in the grade I OA group, the grade II OA group, the grade III OA group and the RA group was 1.2 +/- 0.4, 1.2 +/- 0.4, 1.3 +/- 0.5, and 2.7 +/- 1.1, respectively. The mean size (mean +/- standard deviation) of the lymph nodes was 3.8 +/- 1.0 mm, 3.6 +/- 1.1 mm, 4.1 +/- 0.8 mm, and 5.4 +/- 1.3 mm, respectively. The incidence of central fatty changes was significantly lower in the RA group than in all the OA groups and the grade III OA group. When differentiating RA from OA, and when the differentiation was confined to the RA group and grade III OA group, respectively, the criteria of the number of lymph nodes, their size, their central fatty change and a combination of all these three criteria showed statistical significance (Az values for the former were 0.869, 0.847, 0.776, and 0.942; Az values for the latter were 0.855, 0.799, 0.712, and 0.916). The number and mean size of the lymph nodes correlated with the ISVs (r = 0.49, p < 0.001; 0.50, 0.001, respectively)., Conclusion: The number, size and central fatty changes in the popliteal lymph nodes observed on the MR images might serve as simple and useful markers in differentiating RA disease from OA disease. These markers would be particular helpful in cases of severe synovial enhancement where the ISVs of both RA and OA overlap. The number and mean size of the lymph nodes also correlated well with the ISV.

Obtaining multiple series of magnetic resonance (MR) images with different contrasts is useful for accurate diagnosis of human spinal conditions. However, this can be time consuming and a burden on both the patient and the hospital. We propose a Bloch equation-based autoencoder regularization generative adversarial network (BlochGAN) to generate a fat saturation T2-weighted (T2 FS) image from T1-weighted (T1-w) and T2-weighted (T2-w) images of human spine. To achieve this, our approach was to utilize the relationship between the contrasts using Bloch equation since it is a fundamental principle of MR physics and serves as a physical basis of each contrasts. BlochGAN properly generated the target-contrast images using the autoencoder regularization based on the Bloch equation to identify the physical basis of the contrasts. BlochGAN consists of four sub-networks: an encoder, a decoder, a generator, and a discriminator. The encoder extracts features from the multi-contrast input images, and the generator creates target T2 FS images using the features extracted from the encoder. The discriminator assists network learning by providing adversarial loss, and the decoder reconstructs the input multi-contrast images and regularizes the learning process by providing reconstruction loss. The discriminator and the decoder are only used in the training process. Our results demonstrate that BlochGAN achieved quantitatively and qualitatively superior performance compared to conventional medical image synthesis methods in generating spine T2 FS images from T1-w, and T2-w images.

Purpose: To compare the outcomes of treating large cartilage defects in knee osteoarthritis using human allogeneic umbilical cord blood‐derived mesenchymal stem cell (hUCB‐MSC) implantation or arthroscopic microdrilling as a supplementary cartilage regenerative procedure combined with high tibial osteotomy (HTO). Methods: This 1‐year prospective comparative study included 25 patients with large, near full‐thickness cartilage defects (International Cartilage Repair Society grade ≥ IIIB) in the medial femoral condyles and varus malalignment. Defects were treated with hUCB‐MSC implantation or arthroscopic microdrilling combined with HTO. The primary outcomes were pain visual analogue scale and International Knee Documentation Committee subjective scores at 12, 24 and 48 weeks. Secondary outcomes included arthroscopic, histological and magnetic resonance imaging assessments at 1 year. Results: Fifteen and 10 patients were treated via hUCB‐MSC implantation and microdrilling, respectively. Baseline demographics, limb alignment and clinical outcomes did not significantly differ between the groups. Cartilage defects and total restored areas were significantly larger in the hUCB‐MSC group (7.2 ± 1.9 vs. 5.2 ± 2.1 cm2, p = 0.023; 4.5 ± 1.4 vs. 3.0 ± 1.6 cm2, p = 0.035). The proportion of moderate‐to‐strong positive type II collagen staining was significantly higher in the hUCB‐MSC group compared to that in the microdrilled group (93.3% vs. 60%, respectively). Rigidity upon probing resembled that of normal cartilage tissue more in the hUCB‐MSC group (86.7% vs. 50.0%, p = 0.075). Histological findings revealed a higher proportion of hyaline cartilage in the group with implanted hUCB‐MSC (p = 0.041). Conclusion: hUCB‐MSC implantation showed comparable clinical outcomes to those of microdrilling as supplementary cartilage procedures combined with HTO in the short term, despite the significantly larger cartilage defect in the hUCB‐MSC group. The repaired cartilage after hUCB‐MSC implantation showed greater hyaline‐type cartilage with rigidity than that after microdrilling. With the increased availability of magnetic resonance imaging (MRI) and a progressive rise in the frequency of cardiac device implantation, there is an increased chance that patients with implanted cardiac devices require MRI examination during their lifetime. Though MRI is generally contraindicated in patients who have undergone pacemaker implantation with electronic circuits, the recent introduction of MR Conditional pacemaker allows physicians to take advantage of MRI to assess these patients during diagnosis and treatment. When MRI examinations of patients with pacemaker are requested, physicians must confirm whether the device is a conventional pacemaker or an MR Conditional pacemaker by reviewing chest radiographs or the electronic medical records (EMRs). The purpose of this study was to evaluate the utility of a deep convolutional neural network (DCNN) trained to detect pacemakers on chest radiographs and to determine the device's subclassification. The DCNN perfectly detected pacemakers on chest radiographs and the accuracy of the subclassification of pacemakers using the internal and external test datasets were 100.0% (n = 106/106) and 90.1% (n = 279/308). The DCNN can be applied to the radiologic workflow for double-checking purposes, thereby improving patient safety during MRI and preventing busy physicians from making errors. [ABSTRACT FROM AUTHOR]

Artificial intelligence (AI) is expected to bring greater efficiency in radiology by performing tasks that would otherwise require human intelligence, also at a much faster rate than human performance. In recent years, milestone deep learning models with unprecedented low error rates and high computational efficiency have shown remarkable performance for lesion detection, classification, and segmentation tasks. However, the growing field of AI has significant implications for radiology that are not limited to visual tasks. These are essential applications for optimizing imaging workflows and improving noninterpretive tasks. The substantial developments of advanced algorithms, the emergence of massive quantities of medical data, and the interest of researchers and clinicians reveal the potential for the growing applications of AI to augment the day-to-day efficiency of musculoskeletal radiologists. Abstract: Objective: To assess the diagnostic value of fat-suppressed (FS) three-dimensional (3D) volume isotropic turbo spin echo acquisition (VISTA) imaging in detecting radial and root tears of the meniscus, including the reader-defined reformatted axial (RDA) plane. Materials and methods: Twenty-three patients with arthroscopically confirmed radial or root tears of the meniscus underwent magnetic resonance imaging (MRI) with 2D and FS 3D VISTA sequences. MRIs were reviewed independently by two musculoskeletal radiologists blinded to the arthroscopic findings. Sensitivity, specificity, accuracy, and interobserver agreement were calculated for radial and root tears. Both radiologists reported confidence scale for the presence of meniscal tears in 2D axial imaging, 3D axial imaging, and RDA imaging, based on a five-point scale. Wilcoxon's signed rank test was used to compare confidence scale. Results: The sensitivity, specificity, and accuracy of FS 3D VISTA MR imaging versus 2D MR imaging were as follows: 96%, 96%, and 96% versus 91%, 91%, and 91%, respectively in reader 1, and 96%, 96%, and 96% versus 83%, 91%, and 87%, respectively, in reader 2. Interobserver agreement for detecting meniscal tears was excellent (κ =1) with FS 3D VISTA. The confidence scale was significantly higher for 3D axial images than 2D imaging (p =0.03) and significantly higher in RDA images than 3D axial image in detecting radial and root tears. Conclusions: FS 3D VISTA had a better diagnostic performance in evaluating radial and root tears of the meniscus. The reader-defined reformatted axial plane obtained from FS 3D VISTA MR imaging is useful in detecting radial and root tears of the meniscus. [Copyright &y& Elsevier]
