Introduction Fetal exposition to valproate can lead to a cluster of facial dysmorphism, congenital anomalies and neurodevelopmental retardation. Case Report In this report we describe 2 cases of fetal valproate syndrome. In the first case, the gravida had a valproate medication before and during pregnancy with additional folic acid. She delivered a male premature infant at 25+2 weeks of gestation due to preterm labor and rupture of the membranes. Physical examination showed even in the premature infant typical signs of fetal valproate syndrome with trigonocephaly, epicanthal folds, broad root of the nose, low-set ears, thin upper lip and anteverted nares. In the second case, the gravida was under antiepileptic therapy with valproate and lamotrigine before and during pregnancy without any prophylaxis with folic acid. Sonographic examination during pregnancy diagnosed a spina bifida, Chiari II malformation and clubfeet. A female newborn was delivered at 39+4 weeks of gestation. Besides the prenatally detected anomalies, facial dysmorphism including microcephaly, low-set ears, thin upper lip and shallow philtrum were seen after birth. Conclusion Valproate, a widely used anticonvulsant medication, is known for its teratogenic effects. The risk of congenital anomalies is even higher in combination with other antiepileptic drugs. Therefore, the avoidance of valproate or at least supplementation with a high dose prophylactic folic acid before and during pregnancy is highly recommended for women with epilepsy.

Bei monochorialen Zwillingsschwangerschaften ist die perinatale Mortalitat um das 3–5-fache gegenuber dichorialen Zwillingsschwangerschaften erhoht. Dies erklart sich unter anderem durch haufigere Fruhgeburtlichkeit etwa im Rahmen von Gestosen, jedoch auch zum Teil durch das fetofetale Transfusionssyndrom (FFTS). Durch unidirektionalen Blutaustausch uber plazentare Anastomosen kommt es im Verlauf zu Wachstumsdifferenzen bis uber 20% mit oder ohne ein Oligohydramnion des einen (Donor) und ein Polyhydramnion des anderen Zwillings (Akzeptor). Lewi et al. konnten in einer Studie mit 202 monochorialen Gemini-Schwangerschaften in 9% ein schweres FFTS nachweisen. Unbehandelt weist das FFTS Mortalitatsraten bis zu 90% auf. Neben der chronischen Verlaufsform wurde bisher nur wenig uber die akut perinatal verlaufende Form des FFTS berichtet. Sie zeigt sich erst postnatal durch Anamie und Hypovolamie beim einen und Polyglobulie beim anderen Zwilling. Die Anamie wurde hierbei meist beim zweiten nach komplikationsloser Geburt des ersten Geminus beobachtet. Aufgrund des akuten Auftretens sind bisher keine Fruherkennungsmasnahmen bekannt. Es wird der Fall einer 30-jahrigen Primigravida mit monochorialer diamnialer Geminigraviditat beschrieben. Im Schwangerschaftsverlauf konnten keine Hinweise auf ein chronisches FFTS festgestellt werden. Mit 37 + 1 SSW erfolgte nach Risikoaufklarung die Geburtseinleitung mit Minprostin-Gel® bei Schadellage der beiden Feten. Der erste Geminus zeigte neben einem suspekten Cardiotokogramm (CTG) in der Austreibungsperiode postnatal das Bild eines Volumenmangelschocks bei APGAR Werten von 2/7/8. Bei klinischem Verdacht auf ein akutes fetofetales Transfusionssyndrom und einem Hamoglobinwert von 13,7 g/dl erfolgte die Notfalltransfusion von 20 ml/kg Korpergewicht eines Erythrozytenkonzentrates beim ersten Geminus. Der Hamoglobinwert nach Transfusion betrug 15,2 g/dl bei einem Hamatokrit von 42%. Beim zweiten Geminus wurde ein APGAR von 10/10/10 sowie ein hochnormaler Hamoglobinwert von 19,6 g/dl festgestellt. Das Geburtsgewicht differierte lediglich um 10,4%. Das hier akut aufgetretene fetofetale Transfusionssyndrom mit Manifestation von Anamie und Volumenmangelschock beim erstgeborenen Geminus sollte bei der Differenzialdiagnose spontan entbundener monochorialer Gemini Berucksichtigung finden, da die rechtzeitige Erythrozyten-Transfusion entscheidend fur Morbiditat und Mortalitat ist.

Trisomy 22 is a common trisomy in spontaneous abortions. In contrast, live-born trisomy 22 is rarely seen due to severe organ malformations associated with this condition. Here, we report on a male infant with complete, non-mosaic trisomy 22 born at 35 + 5 weeks via caesarean section. Peripheral blood lymphocytes and fibroblasts showed an additional chromosome 22 in all metaphases analyzed (47,XY,+22). In addition, array CGH confirmed complete trisomy 22. The patient’s clinical features included dolichocephalus, hypertelorism, flattened nasal bridge, dysplastic ears with preauricular sinuses and tags, medial cleft palate, anal atresia, and coronary hypospadias with scrotum bipartitum. Essential treatment was implemented in close coordination with the parents. The child died 29 days after birth due to respiratory insufficiency and deterioration of renal function. Our patient’s history complements other reports illustrating that children with complete trisomy 22 may survive until birth and beyond.
