Abstract The reliable operation of power systems while integrating renewable energy systems depends on Optimal Power Flow (OPF). Power systems meet the operational demands by efficiently managing the OPF. Identifying the optimal solution for the OPF problem is essential to ensure voltage stability, and minimize power loss and fuel cost when the power system is integrated with renewable energy resources. The traditional procedure to find the optimal solution utilizes nature-inspired metaheuristic optimization algorithms which exhibit performance drop in terms of high convergence rate and local optimal solution while handling uncertainties and nonlinearities in Hybrid Renewable Energy Systems (HRES). Thus, a novel hybrid model is presented in this research work using Deep Reinforcement Learning (DRL) with Quantum Inspired Genetic Algorithm (DRL-QIGA). The DRL in the proposed model effectively combines the proximal policy network to optimize power generation in real-time. The ability to learn and adapt to the changes in a real-time environment makes DRL to be suitable for the proposed model. Meanwhile, the QIGA enhances the global search process through the quantum computing principle, and this improves the exploitation and exploration features while searching for optimal solutions for the OPF problem. The proposed model experimental evaluation utilizes a modified IEEE 30-bus system to validate the performance. Comparative analysis demonstrates the proposed model’s better performance in terms of reduced fuel cost of $620.45, minimized power loss of 1.85 MW, and voltage deviation of 0.065 compared with traditional optimization algorithms.

Abstract Cucumo- and tospoviruses are the most destructive viruses infecting hot pepper (chilli). A diagnostic survey was conducted to assess the prevalence of cucumo and tospoviruses in chilli growing tracts of Tamil Nadu. Infected plants showing mosaic with chlorotic and necrotic rings, veinal necrosis, mosaic mottling, leaf filiformity and malformation were collected. Molecular indexing carried out through reverse transcription polymerase chain reaction (RT-PCR) with coat protein gene specific primer of Cucumber mosaic virus (CMV) and tospovirus degenerate primer corresponding to the L segment (RdRp). Ostensibly, amplifications were observed for both CMV and tospoviruses as sole as well for mixed infections. The sequence analysis indicated that the Capsicum chlorosis virus (CaCV) and Groundnut bud necrosis virus (GBNV) to be involved with CMV in causing combined infections. The co-infection of CMV with CaCV was detected in 10.41% of the symptomatic plant samples and combined infection of CMV with GBNV was recorded in around 6.25% of the symptomatic plants surveyed. The amino acid substitution of Ser129 over conserved Pro129 in coat protein of CMV implies that CMV strain involved in mixed infection as chlorosis inducing strain. Further, the electron microscopy of symptomatic plant samples explicated the presence of isometric particles of CMV and quasi spherical particles of tospoviruses. This is the first molecular evidence for the natural co-existence of chlorosis inducing CMV strain with CaCV and GBNV on hot pepper in India.

Heart failure (HF) is a major health problem in India with a postadmission mortality of 20%–30%. Medication adherence ranges from 25% to 50%, and the tolerance of guideline-based medication is low for Indian patients. We took out guidelines on HF in 2015, and this update covers the changes which have occurred in HF management in the past 2 years. As a number of new drugs such as angiotensin receptor-neprilysin inhibitor and ivabradine have been approved for use in India, devices such as implantable cardioverter-defibrillators and cardiac resynchronization therapy are becoming more accessible and more left ventricular assist devices and transplants are being done in India, there is need for clear guidelines for the use of each which are practical for India.

Prevention of coronary artery disease requires control of risk factors. It is not enough to take out guidelines, it is necessary to implement the guidelines. There are a number of studies which have shown that quite often guidelines are not followed. In this article, we have abstracted the essence of the guidelines into simple to follow algorithms so that they are easy to remember and also teach. For Cardiovascular risk reduction, general measures include stopping smoking, perform 150 minutes of moderate activity per week and take a low fat and low salt diet. For hypertension: All classes of drugs are suitable but generally (NICE 2011) Angiotensin receptor blockers or Angiotensin converting enzyme inhibitors for young (55 years, many with isolated systolic hypertension) Calcium channel blockers, diuretics if needed , are prefered. Combinations are used if monotherapy does not work. For dyslipidemia: Statin benefit groups are defined and manifest coronary artery disease and Diabetics above 40 years should get high dose statins. Lipid levels are only tested to look for compliance or further intensification of regime. Diabetes: Metformin is the drug of choice, Sulfonylureas can be used in combination. Glyptins are also safe and can be used. Other drugs should be used with the help of the endocrinologist.

BACKGROUND: Among patients with stable coronary disease and moderate or severe ischemia, whether clinical outcomes are better in those who receive an invasive intervention plus medical therapy than in those who receive medical therapy alone is uncertain. METHODS: We randomly assigned 5179 patients with moderate or severe ischemia to an initial invasive strategy (angiography and revascularization when feasible) and medical therapy or to an initial conservative strategy of medical therapy alone and angiography if medical therapy failed. The primary outcome was a composite of death from cardiovascular causes, myocardial infarction, or hospitalization for unstable angina, heart failure, or resuscitated cardiac arrest. A key secondary outcome was death from cardiovascular causes or myocardial infarction. RESULTS: Over a median of 3.2 years, 318 primary outcome events occurred in the invasive-strategy group and 352 occurred in the conservative-strategy group. At 6 months, the cumulative event rate was 5.3% in the invasive-strategy group and 3.4% in the conservative-strategy group (difference, 1.9 percentage points; 95% confidence interval [CI], 0.8 to 3.0); at 5 years, the cumulative event rate was 16.4% and 18.2%, respectively (difference, -1.8 percentage points; 95% CI, -4.7 to 1.0). Results were similar with respect to the key secondary outcome. The incidence of the primary outcome was sensitive to the definition of myocardial infarction; a secondary analysis yielded more procedural myocardial infarctions of uncertain clinical importance. There were 145 deaths in the invasive-strategy group and 144 deaths in the conservative-strategy group (hazard ratio, 1.05; 95% CI, 0.83 to 1.32). CONCLUSIONS: Among patients with stable coronary disease and moderate or severe ischemia, we did not find evidence that an initial invasive strategy, as compared with an initial conservative strategy, reduced the risk of ischemic cardiovascular events or death from any cause over a median of 3.2 years. The trial findings were sensitive to the definition of myocardial infarction that was used. Today with the help of advancement in the biotechnological processes, researchers are concentrating more on economical and versatile ways to recover value-added products from natural resources such as waste waters. Pseudomonas aeruginosa is known to produce a broad range of secondary metabolites during its active growth phase. In this study, P aeruginosa was densely isolated from the waste dumping zone of the highly polluted Cooum River (CR), Tamil Nadu, India. The strain was identified through biochemical analysis and molecular characterization by 16S rRNA sequencing and named as JAAKPA. Enzyme profiling of the strain revealed that it had potential lipase and protease activities. Antibacterial and antifungal activities of the strain against ten clinical pathogens were tested. A significant antibacterial activity against Gram-positive strains such as methicillin-resistant Staphylococcus aureus (MRSA), Bacillus subtilis, and moderate antifungal activity against Candida albicans, were observed. The culture supernatant of JAAKPA was subjected to thin-layer chromatography and column chromatography to extract and purify the antimicrobial compound (Phenazine). The purified compound was further subjected to gas chromatography–mass spectrometry (GC-MS), fourier transform infrared spectroscopy (FTIR), and nuclear magnetic resonance (NMR) studies. The results revealed phenazine to be the major secondary metabolite responsible for the antimicrobial activities.

Metastatic castration-resistant prostate cancer (mCRPC) is a natural sequela in advanced prostate cancer following resistance to standard treatment regimes, where patients develop with rising PSA, bone pains, and high disease volume. Further palliative treatment is the need of the hour for ensuring disease control and quality of life. In recent times, many novel methods have been evolved for these patients. Endo-radioligand therapy with Lutetium 177 prostate-specific membrane antigen 617 (Lu-177 PSMA) based on the Theranostic concept has emerged as a promising tool among these. We present here the current status of Lu177-PSMA for mCRPC patient and future directions.

The flushing of by-products from the interelectrode gap (EG) of a few microns is the major concern in the electrical discharge machining (EDM). The inadequate removal of debris from the gap causes short-circuits and reduces machining efficiency. Researchers have explored different techniques to enhance debris flushing from the gap. In electrical discharge milling (ED milling), the rotation of the tool along with linear travel is the major driving force for the debris flushing. In this paper, the application of an improvised tool to enhance rotation induced debris flushing is proposed. The flow-fields of dielectric and debris were modeled using computational fluid dynamics (CFD). The simulations results were utilized to optimize the tool geometries. A comparative study of the cylindrical and improvised tool in terms of material removal rate (MRR), tool wear rate (TWR) and surface finish was calculated. Relative to conventional cylindrical tools, the improvised tool enhanced the MRR by 150 %.

The advent of more conservative methods of treatment for renal pathologies has necessitated a precise knowledge of renal pelvicalyceal system. Knowledge on the patterns of pelvicalyceal system and its relation to the renal segmental arteries is of importance in procedures in kidney. The aim is to analyze the morphometry of calyceal system in kidney donors using 64- Slice Computed Tomography Angiography. The study was conducted in specialized scan center located in Chennai after getting appropriate permission from medical director of the center. The study group was drawn from kidney donors who approached the scan center who had no diseases related to kidney. In this study, a total of 99 kidney donors were included. This was a prospective cross-sectional study conducted from June 2014 to September 2017. The statistical analysis and plotting of graphs were carried out using Sigma stat 13.0 (Systat Software, USA). Various patterns of the pelvicalyceal system along with morphometric dimensions of the infundibulum, which leads from the major calyces to the renal pelvis were studied. The human sacrum is a complex structure and provides accommodation for spinal nerves which stabilises the pelvis. Morphometric measurements of sacrum determine the sex of the individual. The measurement of sacral index using the sacrum bone is considered as one of the best methods for determination of sex. The aim of this study is to find out the importance of sacral index in sexual dimorphism and to compare the findings with various age groups. 100 fully ossified sacral bones were collected from department of Anatomy. Average sacral length and breadth were taken by using sliding Vernier caliper and sacral index was calculated. The obtained values were tabulated against their respective parameters and then interpreted for the study. Avergae sacral index in females (109) is significantly greater than males (94). As per present study sacral index can reliably identify 58% of male sacrum and 42% of female sacrum. It is therefore a quite reliable and significant criteria for sex determination of sacrum. Thus, regional and racial differences in sacrum along with sacral index can be used for anatomical, medico-legal and anthropological purposes.

Migraine is characterized by headache due to imbalance between excitation and inhibition of neurons disabling normal day to day activities. The excitations of neurons are done by excitatory neurotransmitter glutamate which plays the key role in creating any pathology related to neurons. This study was done to identify GluR1 a subunit of AMPA glutamate receptor in the cells of trigeminal ganglion after inducing migraine and compare it with control rats. The GluR1 subunits were localized in the cytoplasm of neurons, and these subunits were up-regulated following a migraine. The GluR1 was also localized in satellite glial cells and nerve fibers, indicating these subunits expressed in neurons and migrate during nociceptive sensitization. This GluR1 expression in the cells of trigeminal ganglion may be crucial in nociceptive sensitization leading to migraine and other painful conditions like trigeminal neuralgia.

Candida albicans is a mucosal commensal organism capable of causing superficial (oral and vaginal thrush) infections in immune normal hosts, but is a major pathogen causing systemic and mucosal infections in immunocompromised individuals. Azoles have been very effective anti-fungal agents and the mainstay in treating opportunistic mold and yeast infections. Azole resistant strains have emerged compromising the utility of this class of drugs. It has been shown that azole resistance can be reversed by the co-administration of a histone deacetylase (HDAC) inhibitor, suggesting that resistance is mediated by epigenetic mechanisms possibly involving Hos2, a fungal deacetylase. We report here the cloning and functional characterization of HOS2 (HighOsmolarity Sensitive), a gene coding for fungal histone deacetylase from C. albicans. Inhibition studies showed that Hos2 is susceptible to pan inhibitors such as trichostatin A (TSA) and suberoylanilide hydroxamic acid (SAHA), but is not inhibited by class I inhibitors such as MS-275. This in vitro enzymatic assay, which is amenable to high throughput could be used for screening potent fungal Hos2 inhibitors that could be a potential anti-fungal adjuvant. Purified Hos2 protein consistently deacetylated tubulins, rather than histones from TSA-treated cells. Hos2 has been reported to be a putative NAD+ dependent histone deacetylase, a feature of sirtuins. We assayed for sirtuin activation with resveratrol and purified Hos2 protein and did not find any sirtuin activity.

Cucumo- and tospoviruses are the most destructive viruses infecting hot pepper (chilli).

Objectives This study sought to establish worldwide and regional diagnostic reference levels (DRLs) and achievable administered activities (AAAs) for single-photon emission computed tomography (SPECT) myocardial perfusion imaging (MPI). Background Reference levels serve as radiation dose benchmarks to compare individual laboratories against aggregated data, helping to identify sites in greatest need of dose reduction interventions. DRLs for SPECT MPI have previously been derived from national or regional registries. To date there have been no multiregional reports of DRLs for SPECT MPI from a single standardized dataset. Methods Data were submitted voluntarily to the INCAPS (International Atomic Energy Agency Nuclear Cardiology Protocols Study), a cross-sectional, multinational registry of MPI protocols. A total of 7,103 studies were included. DRLs and AAAs were calculated by protocol for each world region and for aggregated worldwide data. Results The aggregated worldwide DRLs for rest-stress or stress-rest studies employing technetium Tc 99m–labeled radiopharmaceuticals were 11.2 mCi (first dose) and 32.0 mCi (second dose) for 1-day protocols, and 23.0 mCi (first dose) and 24.0 mCi (second dose) for multiday protocols. Corresponding AAAs were 10.1 mCi (first dose) and 28.0 mCi (second dose) for 1-day protocols, and 17.8 mCi (first dose) and 18.7 mCi (second dose) for multiday protocols. For stress-only technetium Tc 99m studies, the worldwide DRL and AAA were 18.0 mCi and 12.5 mCi, respectively. Stress-first imaging was used in 26% to 92% of regional studies except in North America where it was used in just 7% of cases. Significant differences in DRLs and AAAs were observed between regions. Conclusions This study reports reference levels for SPECT MPI for each major world region from one of the largest international registries of clinical MPI studies. Regional DRLs may be useful in establishing or revising guidelines or simply comparing individual laboratory protocols to regional trends. Organizations should continue to focus on establishing standardized reporting methods to improve the validity and comparability of regional DRLs.

Objectives This study sought to describe worldwide variations in the use of myocardial perfusion imaging hardware, software, and imaging protocols and their impact on radiation effective dose (ED). Background Concerns about long-term effects of ionizing radiation have prompted efforts to identify strategies for dose optimization in myocardial perfusion scintigraphy. Studies have increasingly shown opportunities for dose reduction using newer technologies and optimized protocols. Methods Data were submitted voluntarily to the INCAPS (International Atomic Energy Agency Nuclear Cardiology Protocols Study) registry, a multinational, cross-sectional study comprising 7,911 imaging studies from 308 labs in 65 countries. The study compared regional use of camera technologies, advanced post-processing software, and protocol characteristics and analyzed the influence of each factor on ED. Results Cadmium-zinc-telluride and positron emission tomography (PET) cameras were used in 10% (regional range 0% to 26%) and 6% (regional range 0% to 17%) of studies worldwide. Attenuation correction was used in 26% of cases (range 10% to 57%), and advanced post-processing software was used in 38% of cases (range 26% to 64%). Stress-first single-photon emission computed tomography (SPECT) imaging comprised nearly 20% of cases from all world regions, except North America, where it was used in just 7% of cases. Factors associated with lower ED and odds ratio for achieving radiation dose ≤9 mSv included use of cadmium-zinc-telluride, PET, advanced post-processing software, and stress- or rest-only imaging. Overall, 39% of all studies (97% PET and 35% SPECT) were ≤9 mSv, while just 6% of all studies (32% PET and 4% SPECT) achieved a dose ≤3 mSv. Conclusions Newer-technology cameras, advanced software, and stress-only protocols were associated with reduced ED, but worldwide adoption of these practices was generally low and varied significantly between regions. The implementation of dose-optimizing technologies and protocols offers an opportunity to reduce patient radiation exposure across all world regions.

Many pharmaceuticals are present in reclaimed wastewater and effluent-dominated water bodies used to irrigate edible crops. Previous research has shown that plants irrigated with reclaimed wastewater can accumulate pharmaceuticals. However, plant-driven processes that contribute to differences in accumulation among compounds are not well understood. Here, we tested the effects of exposure to mixtures on spinach accumulation and metabolism of four psychoactive pharmaceuticals found in reclaimed wastewater: carbamazepine, fluoxetine, amitriptyline, and lamotrigine. Coexposure of plants to carbamazepine and fluoxetine or amitriptyline decreased accumulation of the toxic carbamazepine metabolite 10,11-epoxycarbamazepine. Furthermore, we tested a simple transpiration-based accumulation model and found that transpiration is a strong predictor for accumulation of the studied compounds. Amitriptyline accumulated to a larger extent than predicted from transpiration alone, and we suggest the possibility that a transporter protein may be involved in its uptake. Our findings highlight the need to consider plant physiology and mixture effects in studying accumulation of polar and ionizable organic contaminants and their metabolites.

Many ionizable organic contaminants (IOCs) are present in treated wastewater used to irrigate edible crops and accumulate in plants under field conditions. Phytoavailability of IOCs with pKa values between 4 and 9 may be affected by the pH of the rhizosphere (the water and soil within 2–3 mm of the root surface). Plants can alter rhizosphere pH by 2–3 units in either direction in response to nutrient availability. The effects of plant modulation of rhizosphere pH on IOC accumulation have not been previously reported. Here we provide direct evidence that plant-driven changes in rhizosphere pH impact accumulation of an IOC in plant leaves. Using a modified hydroponic system, we found that rhizosphere pH was higher by 1.5–2.4 units when plants received only nitrate rather than a combination of nitrate and ammonium. Plant-driven changes to rhizosphere pH altered accumulation of lamotrigine (pKa = 5.7) but not carbamazepine, a non-ionizable contaminant. Lamotrigine accumulation in leaves correlated strongly wi...

Jaundice persisting beyond 14 days of life is a commonly encountered problem in the neonatal follow-up clinic. Although by far the most common underlying etiology is breast milk jaundice, clinician should not miss out an important surgical condition the extrahepatic biliary atresia (an incidence of about 1 in 10000 live births) whose outcome without a surgical intervention before 8 weeks is dismal. Eminently treatable entities such as hypothyroidism and urinary tract infection also can present as prolonged jaundice. This article will synthesize the available literature on this topic and present an algorithm for evaluation of neonates presenting with prolonged jaundice useful to pediatricians in outpatient practice.

Perturbation in the microbial population/colony index has harmful consequences on human health. Both biological and social factors influence the composition of the gut microbiota and also promote gastric diseases. Changes in the gut microbiota manifest in disease progression owing to epigenetic modification in the host, which in turn influences differentiation and function of immune cells adversely. Uncontrolled use of antibiotics, chemotherapeutic drugs, and any change in the diet pattern usually contribute to the changes in the colony index of sensitive strains known to release microbial content in the tissue micromilieu. Ligands released from dying microbes induce Toll-like receptor (TLR) mimicry, skew hypoxia, and cause sterile inflammation, which further contributes to the severity of inflammatory, autoimmune, and tumorous diseases. The major aim and scope of this review is both to discuss various modalities/interventions across the globe and to utilize microbiota-based therapeutic approaches for mitigating the disease burden.

Soybean (Glycine max L.), the most popular oil seed crop, grown in the experimental fields of Tamil Nadu Agricultural University and adjoining areas exhibited symptoms of necrosis and stunting. The symptoms were similar to bud blight of soybean caused by the Tobacco streak virus (TSV) and the disease is most prevalent throughout the country. To study the nature of infection, sap inoculation of the soybean strain induced local as well systemic infection on cowpea plants cv. C 152 and resulted in the production of circular necrotic lesions and death of plants. The samples were also serologically positive in DAC-ELISA and it has also yielded a protein band of approximately 29 kDa corresponding to coat protein of TSV in Western blot assay. For the characterization of virus, RT-PCR was carried out with a newly designed coat protein gene specific primer, which resulted in amplification of the expected 929 bp size. Sequence analysis of the CP gene had nucleotide similarity of 80.6 to 99.3% with known isolates of TSV. The multiple sequence alignment revealed that CP gene showed single unique variation and some of deletion and addition mutation was found in nucleotide and amino acid sequences against the isolates of other soybean Brazil and USA isolates and produced single unique variation at position 344 where adenine was substituted with guanine. There was no deletion and addition between nucleotide sequences in the group of Indian isolates, further confirms the placement of the soybean isolate of TSV in a single subgroup. Key words: Soybean, Tobacco streak virus, coat protein gene, diversity analysis.

Objectives The aim of this study was to investigate gender-based differences in nuclear cardiology practice globally, with a particular focus on laboratory volume, radiation dose, protocols, and best practices. Background It is unclear whether gender-based differences exist in radiation exposure for nuclear cardiology procedures. Methods In a large, multicenter, observational, cross-sectional study encompassing 7,911 patients in 65 countries, radiation effective dose was estimated for each examination. Patient-level best practices relating to radiation exposure were compared between genders. Analysis of covariance was used to determine any difference in radiation exposure according to gender, region, and the interaction between gender and region. Linear, logistic, and hierarchical regression models were developed to evaluate gender-based differences in radiation exposure and laboratory adherence to best practices. The study also included the United Nations Gender Inequality Index and Human Development Index as covariates in multivariable models. Results The proportion of myocardial perfusion imaging studies performed in women varied among countries; however, there was no significant correlation with the Gender Inequality Index. Globally, mean effective dose for nuclear cardiology procedures was only slightly lower in women (9.6 ± 4.5 mSv) than in men (10.3 ± 4.5 mSv; p Conclusions Despite significant worldwide variation in best practice use and radiation doses from nuclear cardiology procedures, only small differences were observed between genders worldwide. Regional variations noted in myocardial perfusion imaging use and radiation dose offer potential opportunities to address gender-related differences in delivery of nuclear cardiology care.

Aims and Objectives: This study was designed to compare streptokinase (STK) versus recombinant tissue-type plasminogen activator (rt-PA) for thrombolysis in patients with prosthetic heart valve thrombosis (PHVT). Background: Optimal management of PHVT remains controversial. Thrombolytic therapy is a reasonable alternative to surgery. Numerous thrombolytic protocols with STK, urokinase, and rt-PA have been used over the past 40 years. So far there is a lack of consensus on the ideal thrombolytic agent for PHVT. There are no major studies comparing efficacy of STK versus rt-PA. Methods and Results: Sixty patients who underwent thrombolysis for PHVT over 1 year were studied. Forty-eight patients were thrombolysed with STK, whereas 12 patients received rt-PA. Overall 68.33% (41/60) patients showed complete response (CR), whereas 85% (51/60) showed some improvement in hemodynamics, with no significant difference in efficacy according to age, sex, duration from surgery, New York Heart Association Class III–IV, presence of atrial fibrillation, type of valve, or history of stroke. Overall 10% patients developed major complications with mortality of 5%. CR to thrombolytic therapy was numerically better with STK (72.9% vs. 50%; P = 0.12) and mortality was significantly higher in rt-PA (16.6% vs. 2%, P = 0.038). Mean duration of successful thrombolysis was significantly longer with STK as compared to rt-PA (22.96 ± 19.2 vs. 3 h). Conclusion: STK use as compared to rt-PA was associated with numerically more successful thrombolysis and significant lower mortality, hence may be optimal for elective thrombolysis.

Rhinosporidiosis is a chronic granulomatous disease caused by Rhinosporidium seeberi. Rhinosporidium seeberi causes granulomatous inflammation of mucocutaneous sites, presenting most frequently as polypoidal lesions in the nose. Sites like the conjunctiva, trachea, nasopharnyx, skin, and genitourinary tract are less frequently involved. Primary cutaneous lesion is extremely rare and it is often mistaken for soft-tissue tumour. We present a case of 55 years male patient from Thanjavur, Tamilnadu, with primary cutaneous rhinosporidiosis without mucosal involvement.

Background Anterolateral thigh (ALT) flap is a common flap used for many microsurgical reconstructive procedures. Majority of these flaps are based on the musculocutaneous perforators, which, after arising from source artery, course through the bulk of the vastus lateralis muscle. The vascular anatomy of the ALT flap has been often decried as its limiting factor. Methods In this prospective study conducted over 3 years, 207 cases of ALT flap were evaluated. During the harvest of ALT flap, musculocutaneous perforators were laid open through the vastus lateralis, and its variations and pattern were studied for clinical anatomy as well as impact of anatomy on the viability of the flap postoperatively. Results The perforators were recognized to be grouped into three broadly distinguishable anatomical patterns: linear (I), branching (Y), or tortuous (S). While the majority of perforators (51%) were linear “I” perforators, serpentine “S” and branching “Y” perforators were 28 and 21%, respectively. The clinical outcomes were also related to the simplicity of these perforators course as linear “I” perforators had 100% flap viability. Conclusion During the harvest of 207 ALT flaps through 3 years, it could be deciphered that the musculocutaneous perforators traverse through the bulk of vastus lateralis muscle in only three types of patterns (IYS pattern). This identification of pattern could help in prevention of injury while delineating the perforators. Identification and typing of anatomical variation of the musculocutaneous perforator within the vastus muscle could lead to a predictable and safer harvest of the ALT flap.

Background: The soft tissue defects around knee joint give a challenging experience to reconstruct. There are various ways to reconstruct soft tissue defects around knee joint. In this study, a reconstructive algorithm is formed for soft tissue defect around knee joint using pedicled flaps. Methods: Reconstructive procedures were done for 15 cases of soft tissue defects around knee joints following different etiologies during the tenure of January 2017 to December 2018 in the Department of Burns, Plastic and Reconstructive Surgery, Kilpauk Medical College, Chennai. Results: Soft tissue defects around knee joint need flap to have a durable cover, to protect the underlying structures – bones, vessels, tendon, and to enable ease of free movement of knee joint without producing contracture. There are various etiologies for soft tissue defects around knee joint-post traumatic injuries, post total knee replacement, post electric burn injuries, post burn scar contracture release of knee joint. 15 cases of soft tissue defects around knee joint were reconstructed using various pedicled flaps-medial thigh rotation flap, gastrocnemius muscle and myocutaneous flap, reverse anterolateral thigh flap, propeller flap. In this retrospective study, reconstructive algorithm for soft tissue defects around knee joint is provided. Conclusions: The reconstruction of soft tissue defects around knee joint is difficult one. In this study, a simple reconstructive algorithm is provided based on the experience in Department of Burns, Plastic, and Reconstructive Surgery, Kilpauk Medical College, Chennai.

Wound healing activity of the bark extracts of Acacia leucophloea Willd., Fabaceae, was investigated by excision and incision wound healing models in Wistar male rats. Ethanolic extract based ointment of A. leucophloea bark (2 and 5% (w/w)) was formulated and evaluated for its wound healing in Wistar male rats. In comparision with a standard wound healing ointment betadine. A. leucophloea ethanolic extract ointment exhibited marked wound healing activity and significantly enhanced the wound contraction and the period of epithelialization as assessed by wound contraction rate, tensile strength, increasing of DNA, collagen and protein synthesis and histopathological examination. The formulated ointment might well find use as skin repair agent without hazard to human health based on these results.

Avulsion of skin from the hand or fingers is an injury that has a dramatic presentation. The entire musculo-skeletal unit of the finger is intact, and the patient can often move the parts of his naked hand quite normally. The challenge for the reconstructive surgeon lies in resurfacing the hand or finger with a good quality pliable sensate skin cover while preserving the movements and function of the hand. Traditionally, skin grafting has been the standard method of reconstruction in such injuries. However, skin grafting does have many disadvantages, too. This article deals with the features of such injuries, management protocols and other reconstructive options available in the armamentarium of the hand surgeon.

Humic substances, by altering the surface properties and/ or competing for available reaction sites, can either suppress or promote sorption of organic compounds to mineral surfaces. Limited literature evidence points to the reduction in sorption of the antibiotic tetracycline to clay minerals in the presence of humic substances. We investigated the surface interaction of Elliott soil humic acid (ESHA) with hydrous Al oxide (HAO) and the effect of this association on tetracycline sorption. Strong interaction between ESHA and HAO led to ESHA-promoted dissolution of HAO and surface charge reversal. The ESHA-HAO sorption-desorption isotherms were successfully described using a modified Langmuir model that accounted for the heterogeneity of HAO surface and ESHA Ligand exchange was proposed as the major interaction mechanism, and the edge Al atoms on HAO surface were considered as the sorption sites for ESHA macromolecules. ESHA was coated onto HAO to achieve two different organic content (f oc ) levels of 0.81 and 1.52%. Sorption results were compared for the binary ESHA-tetracycline and HAO-tetracycline systems, and the ternary ESHA-HAO-tecracycline system. The coating of ESHA on HAO significantly suppressed tetracycline sorption levels, attributable to altered HAO surface charge characteristics and/or direct competition between ESHA and tetracycline for potential sorption sites. Higher f oc level, besides increasing the extent of sorption suppression, also resulted in greater ionic strength dependence and increased nonlinearity of sorption behavior. It, therefore, appears that the presence of humic substances, in both dissolved and mineral-bound forms, is likely to increase the environmental mobility of tetracycline compounds.

The effect of solution chemistry and sorbate-to-sorbent ratio on the interaction of the antibiotic tetracycline with Elliott soil humic acid (ESHA) was investigated using equilibrium dialysis and FITEQL modeling. Tetracycline speciation strongly influenced its sorption to ESHA. Sorption was strongly pH-dependent with a maximum around pH 4.3, and competition with H+ and electrolyte cation (Na+) was evident. The pH-dependent trend was consistent with complexation between the cationic/zwitterionic tetracycline species and deprotonated sites in ESHA (mainly carboxylic functional groups). Modification of ESHA by Ca2+ addition increased tetracycline sorption suggesting that ternary complex formation (ESHA-metal-tetracycline) may be important at higher concentrations of multivalent metal cations. The macroscopic data (pH-envelope and sorption isotherms) were successfully modeled using a discrete logK function with the FITEQL 4.0 chemical equilibrium program indicating that ESHA-tetracycline interaction could be reasonably represented as complex formation of a monoacid with discrete sites in humic acid. Sorption-desorption hysteresis was observed; both sorption and desorption isotherms were well described by the Freundlich equation.
