INTRODUCTION The aim of this study was to compare individual accuracy and speed of laser application using the Navilas laser system in patients with diabetic and vascular macular oedema. METHODS The Navilas laser system was used by an experienced retina specialist and a relatively inexperienced ophthalmologist. Obtained results were retrospectively assessed with regard to precision and speed of laser application. RESULTS Precision of laser therapy was quite similar, 88 % and 84 % for experienced and inexperienced operators, respectively. The treatment procedure took on average 5.5 minutes and 5.34 minutes for experienced and inexperienced operators, respectively. CONCLUSIONS Precision and duration of therapy using the Navilas laser system do not seem to be correlated with the individual experience of the treating physician.

The short-term effect of the glucose-controlled insulin infusion system (GCIIS) Biostator on metabolic and hormonal responses was studied in 10 non-obese subjects with glucose intolerance and insulin low response to glucose. Glucose tolerance characterized by means of a 2 h glucose infusion test (12 mg/kg/min) primed by i.v. injection of 0.33 g glucose/kg body weight was completely normalized by GCIIS. Results provide further support that normalization of glucose tolerance by means of GCIIS is accompanied by peripheral hyperinsulinaemia if compared with 33 non-obese healthy controls. Glucose-induced endogenous insulin secretion (C-peptide) was significantly reduced during the GCIIS study possibly due to inhibition of insulin secretion by exogenous insulin and/or by lower blood glucose concentration after normalization of glucose tolerance. Acute normalization of glucose tolerance in these patients failed to alter pancreatic glucagon, NEFA and glycerol responses but normalized paradoxical growth hormone response to glucose.

In the search for metastases computed tomography and ultrasonography of the abdomen were performed in a 47-year-old man with a known malignant melanoma in the left lower leg. Numerous fluid-filled cavities, about 8 cm in diameter, were an incidental finding. He died two years after diagnosis of the melanoma, which had extensively metastasized. At autopsy a large multicystic tumour (21 x 15 x 8 cm) was found in the mesentery of the small intestine. The cysts were filled with chyle. Histologically they proved to be multicystic lymphangiomas. Mesenteric lymphangiomas are very rare, multicystic ones and manifestation in adults even more so.

A typical problem concerning in-patient detoxification is the high drop-out rate. This phenomenon represents a significant problem for both the patient and the therapeutic team, as it is a factor which reduces motivation. While several studies confirm a positive shift of early drop-out behavior with the aid of methadone-supported detoxification, the overall drop-out rate still saw no improvement. Unexpectedly the latter rate was found to be around 50 %. The aim of the present study was to determine the sociodemographic, dispositional and addiction-related factors, as well as subjective ratings of the present state, which have predictive value, i. e. which could discriminate drop-outs from those patients who bring their detoxification to a regular end. 176 patients from two different clinical detoxification wards of the Hannover Clinic of Psychiatry and Psychotherapy participated in the study. The overall drop-out rate was 49.4 %, the rate of female patients failing to conclude detoxification was significantly higher than that of males (68.4 % vs. 44.2 %). On average drop-out patients were 2.2 years younger and their former (i. e. uninterrupted) periods without drug consumption were shorter, while criminal circumstances (e. g. therapy as probation vs. voluntary participation) did not play a major role in influencing staying power. As expected, patients terminating their treatment against medical advice evaluated the therapeutic program more critically than the others. Interestingly drop outs rated their own mental and somatic condition very pessimistic. We concluded from these results two essentials being necessary to improve staying power of drug patients during clinical detoxification: systematic induction of (1) a positive self concept and (2) an optimistic view of chances to reach social adjustment and health.

Quantitative computed tomography (QCT) for measuring bone mineral content of lumbar vertebrae is increasingly used internationally. The effect of using conventional CT (single energy CT, SE-CT) and dual energy CT (DE-CT) on reproducibility has been examined. We defined a standard measurement protocol, which automatically evaluates a calibration phantom. This should ensure an in vivo reproducibility of 1 to 2%. Reference data, which has been obtained with this protocol from 113 normal subjects, using SE-CT and DE-CT, are presented.

The short-term effect of the glucose-controlled insulin infusion system (GCIIS) Biostator on metabolic and hormonal responses to a 2 h glucose infusion (0.33 g/kg body weight glucose i.v. followed by an infusion of 12 mg/kg/min) was studied in 8 insulin-dependent diabetic patients (IDDM). Normalization of glucose tolerance by means of GCIIS was associated with significant improvement of lipid metabolism in IDDM. Endogenous insulin secretion (C-peptide) was not altered significantly under the experimental conditions. The results demonstrate that immunoreactive glucagon response to intravenous glucose infusion was restored by treatment with GCIIS in IDDM irrespective of no prolonged duration of normoglycemia. The results provide further support that abnormal glucagon response in some IDDM is secondary to insulin deficiency.

Medical research is increasingly using data from care. However, this 'secondary use' of treatment data brings with it numerous legal questions. Social and health data are among the most intimate information about a person. This legal opinion provides for the first time a detailed overview of the specific legal framework for the secondary use of medical data in the individual federal states, which also takes into account the different sponsorship and legal form of the treating institution. The analysis provides researchers and other stakeholders in the health sector with guidance in the legally compliant handling of medical treatment data. The book also contains a supplementary report, which represents the legal framework for the secondary use of the data managed via the electronic health card in accordance with Section 291a Social Code Book V., Zunehmend greift medizinische Forschung auf Daten aus der Versorgung zurück. Diese ‚Sekundärnutzung‘ von Behandlungsdaten bringt jedoch zahlreiche Rechtsfragen mit sich. Sozial- und Gesundheitsdaten gehören zu den intimsten Informationen über einen Menschen. Mit dem vorliegenden Rechtsgutachten steht erstmals eine detaillierte Übersicht über die spezifischen rechtlichen Rahmenbedingungen für die Sekundärnutzung medizinischer Daten in den einzelnen Bundesländern zur Verfügung, die auch jeweils die unterschiedliche Trägerschaft und Rechtsform der behandelnden Institution berücksichtigt. An alternative approach that is particularly suitable for the radiation health risk assessment (HRA) of astronauts is presented. The quantity, Radiation Attributed Decrease of Survival (RADS), representing the cumulative decrease in the unknown survival curve at a certain attained age, due to the radiation exposure at an earlier age, forms the basis for this alternative approach. Results are provided for all solid cancer plus leukemia incidence RADS from estimated doses from theoretical radiation exposures accumulated during long-term missions to the Moon or Mars. For example, it is shown that a 1000-day Mars exploration mission with a hypothetical mission effective dose of 1.07 Sv at typical astronaut ages around 40 years old, will result in the probability of surviving free of all types of solid cancer and leukemia until retirement age (65 years) being reduced by 4.2% (95% CI 3.2; 5.3) for males and 5.8% (95% CI 4.8; 7.0) for females. RADS dose–responses are given, for the outcomes for incidence of all solid cancer, leukemia, lung and female breast cancer. Results showing how RADS varies with age at exposure, attained age and other factors are also presented. The advantages of this alternative approach, over currently applied methodologies for the long-term radiation protection of astronauts after mission exposures, are presented with example calculations applicable to European astronaut occupational HRA. Some tentative suggestions for new types of occupational risk limits for space missions are given while acknowledging that the setting of astronaut radiation-related risk limits will ultimately be decided by the Space Agencies. Antisynthetase syndrome (ASSD) is a rare clinical condition that is characterized by the occurrence of a classic clinical triad, encompassing myositis, arthritis, and interstitial lung disease (ILD), along with specific autoantibodies that are addressed to different aminoacyl tRNA synthetases (ARS). Until now, it has been unknown whether the presence of a different ARS might affect the clinical presentation, evolution, and outcome of ASSD. In this study, we retrospectively recorded the time of onset, characteristics, clustering of triad findings, and survival of 828 ASSD patients (593 anti-Jo1, 95 anti-PL7, 84 anti-PL12, 38 anti-EJ, and 18 anti-OJ), referring to AENEAS (American and European NEtwork of Antisynthetase Syndrome) collaborative group's cohort. Comparisons were performed first between all ARS cases and then, in the case of significance, while using anti-Jo1 positive patients as the reference group. The characteristics of triad findings were similar and the onset mainly began with a single triad finding in all groups despite some differences in overall prevalence. The "ex-novo" occurrence of triad findings was only reduced in the anti-PL12-positive cohort, however, it occurred in a clinically relevant percentage of patients (30%). Moreover, survival was not influenced by the underlying anti-aminoacyl tRNA synthetase antibodies' positivity, which confirmed that antisynthetase syndrome is a heterogeneous condition and that antibody specificity only partially influences the clinical presentation and evolution of this condition. ispartof: JOURNAL OF CLINICAL MEDICINE vol:8 issue:11 ispartof: location:Switzerland status: published

The tracking performance parameters of the ATLAS Transition Radiation Tracker (TRT) as part of the ATLAS inner detector are described in this paper for different data-taking conditions in proton-proton, proton-lead and lead-lead collisions at the Large Hadron Collider (LHC). The performance is studied using data collected during the first period of LHC operation (Run 1) and is compared with Monte Carlo simulations. The performance of the TRT, operating with two different gas mixtures (xenon-based and argon-based) and its dependence on the TRT occupancy is presented. These studies show that the tracking performance of the TRT is similar for the two gas mixtures and that a significant contribution to the particle momentum resolution is made by the TRT up to high particle densities., La lista completa de autores que integran el documento puede consultarse en el archivo, Instituto de Física La Plata

Purpose: To develop a simple model of therapeutic and stray absorbed dose for a variety of treatment machines and techniques without relying on proprietary machine-specific parameters. Methods: Dosimetry measurements conducted in this study and from the literature were used to develop an analytical model of absorbed dose from a variety of treatment machines and techniques in the 6 to 25 MV interval. A modified one-dimensional gamma-index analysis was performed to evaluate dosimetric accuracy of the model on an independent dataset consisting of measured dose profiles from seven treatment units spanning four manufacturers. Results: The average difference between the calculated and measured absorbed dose values was 9.9% for those datasets on which the model was trained. Additionally, these results indicate that the model can provide accurate calculations of both therapeutic and stray radiation dose from a wide variety of radiotherapy units and techniques. Conclusions: We have developed a simple analytical model of absorbed dose from external beam radiotherapy treatments in the 6 to 25 MV beam energy range. The model has been tested on measured data from multiple treatment machines and techniques, and is broadly applicable to contemporary external beam radiation therapy.

Background Purpose of this study is to explore the trade-offs between radio-induced toxicities and second malignant neoplasm (SMN) induction risk of different emerging radiotherapy techniques for Hodgkin’s lymphoma (HL) through a comprehensive dosimetric analysis on a representative clinical model. Methods Three different planning target volume (PTVi) scenarios of a female patient with supradiaphragmatic HL were used as models for the purpose of this study. Five treatment radiation techniques were simulated: an anterior-posterior parallel-opposed (AP-PA), a forward intensity modulated (FIMRT), an inverse intensity modulated (IMRT), a Tomotherapy (TOMO), a proton (PRO) technique. A radiation dose of 30 Gy or CGE was prescribed. Dose-volume histograms of PTVs and organs-at-risk (OARs) were calculated and related to available dose-volume constraints. SMN risk for breasts, thyroid, and lungs was estimated through the Organ Equivalent Dose model considering cell repopulation and inhomogeneous organ doses. Results With similar level of PTVi coverage, IMRT, TOMO and PRO plans generally reduced the OARs’ dose and accordingly the related radio-induced toxicities. However, only TOMO and PRO plans were compliant with all constraints in all scenarios. For the IMRT and TOMO plans an increased risk of development of breast, and lung SMN compared with AP-PA and FIMRT techniques was estimated. Only PRO plans seemed to reduce the risk of predicted SMN compared with AP-PA technique. Conclusions Our model–based study supports the use of advanced RT techniques to successfully spare OARs and to reduce the risk of radio-induced toxicities in HL patients. However, the estimated increase of SMNs’ risk inherent to TOMO and IMRT techniques should be carefully considered in the evaluation of a risk-adapted therapeutic strategy.
