Introduction In pediatric out-of-hospital cardiac arrest (OHCA) the effect of intraosseous (IO) or intravenous (IV) access on outcomes is unclear. Methods We analyzed prospectively collected data of non-traumatic OHCA in the Resuscitation Outcomes Consortium registry from 2011 to 2015. We included EMS-treated patients ≤17 years of age, classified patients based on vascular access routes, and calculated success rates of IO and IV attempts. After excluding patients with obvious non-cardiac etiologies and those with unsuccessful vascular access or multiple routes, we fit a logistic regression model to evaluate the association of IO vascular access (reference IV access) with the primary outcome of survival, using multiple imputation to address missing data. We analyzed a subgroup of patients at least 2 years of age. Results There were 1549 non-traumatic OHCA: 895 (57.8%) patients had an IO line attempted with 822 (91.8%) successful; 488 (31.5%) had an IV line attempted with 345 (70.7%) successful (difference 21%, 95% CI 17 to 26%). Of the 761 patients included in our logistic regression, 601 received IO (30 [5.2%] survived) and 160 received IV (40 [25%] survived) vascular access. Intraosseous access was associated with a decreased probability of survival (adjusted OR 0.46; 95% CI 0.21–0.98). Patients at least 2 years of age showed a similar association (adjusted OR 0.36; CI 0.15–0.86). Conclusions Intraosseous access was associated with decreased survival among pediatric non-traumatic OHCA. These results are exploratory and support the need for further study to evaluate the effect of intravascular access method on outcomes.

Background: Cardiopulmonary resuscitation (CPR) is occurring more frequently at community hospitals but most patients undergoing CPR do not survive to discharge. Tools to predict CPR survival can be improved by the identification of high-yield clinical indicators.Objective: To identify variables associated with survival to discharge following in-hospital cardiac arrest.Methods: Retrospective cohort study of 463,530 hospital admissions from the Nationwide Inpatient Sample (2012–2016). The analysis includes adults (age ≥50) who underwent in-hospital CPR at US community hospitals.Results: Overall survival to discharge was 29.8% (95% CI: 29.5–30.1%). Age was the strongest predictor of survival and had greater prognostic value than the Charlson comorbidity index. Obesity was associated with improved survival (35.9%, 95% CI: 35.1–36.7%), whereas underweight patients had decreased survival (24.0%, 95% CI: 22.2–25.7%). Acute indicators of poor survival included hyperkalemia, hypercalcemia, and sepsis. We generated an ABCD index based upon four high-yield variables (age, body habitus, comorbidity, day of hospital admission). An ABCD score of 2 or less was a sensitive but non-specific predictor of post-CPR survival (96.8% sensitivity, 95% CI: 96.6–97.0), and those with extreme scores differed 3.8-fold with respect to post-CPR survival probability (46.0% versus 12.1%).Conclusion: Age is the strongest predictor of post-CPR survival, but body habitus is also an important indicator that may currently be underutilized. Our results support improved post-CPR survival of obese patients, consistent with an ‘obesity paradox’. The ABCD score provides an efficient means of risk-stratifying patients and can be calculated in less than 1 minute.

Background: It is unknown if improvements in ischemic stroke (IS) outcomes reported after cerebral reperfusion therapies (CRT) in developed countries are also applicable to the “real world” scenario of low and middle-income countries. We aimed to measure the long-term outcomes of severe IS treated or not with CRT in Brazil. Methods: Patients from a stroke center of a state-run hospital were included. We compared the survival probability and functional status at 3 and 12 months in patients with severe IS treated or not with CRT. From 2010 to 2011, we performed intravenous reperfusion when patients arrived within 4.5 h time-window (IVT group) and after 2011, mechanical thrombectomy (MT) combined or not with intravenous alteplase (IAT group). Those who arrived >4.5 h in 2010-2011 and >6 h in 2012-2017 did not undergo CRT (NCRT group). Results: From 2010 to 2017, we registered 917 patients: 74% (677/917) in the NCRT group, 19% (178/917) in the IVT group and 7% (62/917) in the IAT group. Compared to the NCRT group, IVT patients had a 28% higher (HR: 0.72; 95% CI 0.53-0.96) 3-month adjusted probability of survival and risk of functional dependence was 19% lower (adjusted RR: 0.81; 95% CI 0.73-0.91). For those who underwent MT, the adjusted probability of survival was 59 % higher (HR: 0.41; 95% CI 0.21-0.77) and the risk of functional dependence was 21% lower (adjusted RR: 0.79; 95% CI 0.66-094). These outcomes remained significantly better throughout the first year. Conclusion: CRT led to better outcomes in patients with severe IS in Brazil.

La Diálisis Peritoneal es una terapia igual o superior a la hemodiálisis, es costo-efectiva y ha permitido el soporte dialítico vital a poblaciones víctimas de desastres naturales. En las actuales circunstancias de pandemia y aislamiento social por el COVID 19, la diálisis peritoneal, al ser una terapia domiciliaria, reduce los riesgo de exposición para pacientes, cuidadores y equipos de atención, sin embargo, es una terapia subutilizada a nivel global. Al mostrar los beneficios de la diálisis peritoneal sobre la hemodiálisis y los desenlaces de esta en tiempos de desastre, se presenta como la mejor alternativa para estos tiempos de pandemia. Se llevó a cabo una revisión no sistemática de la literatura, en busca de los beneficios de la diálisis peritoneal vs hemodiálisis y sus desenlaces en situaciones de calamidad. En esta se incluyeron 75 artículos publicados entre1993 y marzo del 2020. Estos estudios muestran los múltiples beneficios de la diálisis peritoneal sobre la hemodiálisis y ser la mejor alternativa en situaciones de desastre. Por lo anterior concluimos que la diálisis peritoneal tiene significativos beneficios sobre la hemodiálisis, entre estos una mayor supervivencia, especialmente en los dos primeros años, una menor velocidad en la tasa de perdida de la función renal residual, permite preservar los accesos vasculares, hay mayor supervivencia post trasplante, además de ser más económica y en situaciones de desastres naturales, como lo es esta pandemia, ha facilitado la continuidad de la atención y el soporte dialítico agudo o crónico para las víctimas.

This report describes multiple congenital malformations found in three dog litters delivered by emergency caesarean section. In all of the litters, some puppies were born alive but were euthanized because of the seriousness of their malformations and low probability of survival. In two litters, gastroschisis was associated with amelia of the right anterior limb. Other malformations such as anencephaly were also found in three puppies among the different litters. This report describes the morphological findings of the affected puppies, discusses the most appropriate terminologies for each case and highlights the importance of an epidemiological survey to identify potential factors associated with the cases.

The Good Outcome Following Attempted Resuscitation (GO-FAR) Score uses pre-arrest factors to predict survival after In-Hospital Cardiac Arrest (IHCA) with minimal neurological dysfunction, (cerebral performance category (CPC) ≤1). Moderate neurological dysfunction (CPC ≤2) may be a more acceptable outcome.To predict survival after IHCA with mild or moderate neurological dysfunction based on pre-arrest factors.52,468 patients with IHCA from 2012-2017. Data was divided into training (44%), testing (22%), and validation (34%) sets. Univariate analysis was used to identify variables with3% difference in survival with CPC ≤2. These variables carried forward to the multivariate logistic regression model. The most parsimonious model that best classified patients as having a very poor (≤5%), below average (≤10%), average (11%-30%), or above average (30%) likelihood of survival with CPC ≤2 was chosen.Age85, admission CPC2, and non-surgical admission were strongly association with poor survival (-12.1%, -14.4%, and -18%, respectively). Nine variables were included in the logistic regression analysis. The final updated model, GO FAR 2, categorized 6.2% of patients with a very poor predicted survival, 24.8% of patients with a below average predicted survival, and 11.3% with above average predicted survival. The observed survival among those with very poor predicted survival was 4.5%.The GO FAR 2 score provides clinicians with a prognostic estimate of the likelihood of a good outcome after IHCA based on pre-arrest patient factors. Future research is required to validate the GO-FAR 2 score.

In Japan, falls from height result in the second highest trauma mortality rate after traffic motor vehicle collisions and the highest trauma-related mortality rate amongst young people. We aimed to identify factors that worsen injury severity and lower survival probability of patients who fell from height and to contribute to the improvement of their prehospital and in-hospital care. This retrospective analysis retrieved hospital records of 179 patients aged ≥ 15 years who were transported to our hospital after a fall from height during April 2014–March 2020. On multiple regression analysis, fall height ≥ 5 m more significantly resulted in higher the injury severity score. Logistic regression analysis revealed that fall height ≥ 5 m with the reference of 2–3 m significantly resulted in lower the survival probability with odds ratio (95% confidence interval) of 0.10 (0.02–0.55). Using ‘feet-first’ as the reference body position, the odds ratios (95% confidence interval) of survival for those who impacted the surface on the lateral or dorsal regions were 0.11 (0.02–0.64) and 0.17 (0.03–0.99), respectively. Collecting information on the abovementioned factors at pre-hospitalisation may facilitate prompt diagnosis and treatment. These results may help improve prehospital and in-hospital care, avoiding preventable trauma deaths.

Introduction Trauma is one of the most common causes of morbidity and mortality worldwide. Since the definition of preventable death has been described many studies like current one were conducted to evaluate this issue. Methods This cohort retrospective study investigated archived medical files of trauma victims from 2017 to 2020 in a referral single-center trauma hospital. Registered demographic data, vital signs, Glasgow coma scale (GCS), timing of trauma and death, executed interventions, type and mechanism of trauma in addition to time errors, clinical mismanagements, and missed injuries were extracted. Injury severity score, revised trauma score, and probability of survival based on TRISS method for each case were calculated. Eventually preventable and non-preventable death were defined and compared. Results Finally from the all 413 trauma deaths 246(54.9 %) files were enrolled. Dead persons were from 18 to 95 years. Of all 189(76.8 %) were males. Analysis manifested 135(54.9 %) of all deaths were potentially preventable and the rest 49.1 % was non-preventable for expiration(p = 0.001). Data showed that from all variables systolic blood pressure ≥80 mmHg, respiratory rate >19 per minute, GCS>8, higher RTS, road traffic accidents and control of external bleeding were contributed to prediction of preventable trauma related mortality. Conclusion This study implied on that frequency of trauma related preventable death was regionally high and associating factors that could influence the number of these mortalities included systolic blood pressure, respiratory rate, GCS, revised trauma score, mechanism of trauma, and external bleeding of trauma patients., Highlights • Preventable trauma related mortality is achieved to 55 % in this study while World Health Organization considered 20 % averagely. • Time errors, missing injuries, and clinical management errors were not generally attributed to preventable trauma death. • SBP>80, RR>19, GCS>8, road-traffic accident, and adequate control of external bleeding could rescue patients from death.

Monitoring the quality of trauma care is important but particularly challenging. Preventable death assessment aims to identify those cases where the patient's death would have not occurred if the patient had been treated differently. Determination of preventable death in trauma care is often based on calculated probability of survival, commonly by using the Trauma and Injury Severity Score (TRISS). TRISS is not suited for identifying all cases with opportunities for improvement. Combined with other methods such as morbidity and mortality conferences, however, it might be a valid approach if a complete review of all trauma deaths is not feasible at an institution.

Resumen: El desarrollo de hipertensión arterial pulmonar asociada al virus de inmunodeficiencia humana reduce la probabilidad de sobrevivencia en el paciente afectado en comparación con el que no presenta esta alteración cardiopulmonar. La fisiopatogenia aún es incierta. Existen varias líneas de investigación para asociar las diferentes proteínas del virus en la lesión endotelial. Desde el punto de vista terapéutico, existen modalidades de tratamiento que permiten una expectativa de vida aceptable. Abstract: The development of pulmonary arterial hypertension associated with human immunodeficiency virus reduces the probability of survival in the patient affected compared to those without cardiopulmonary disease. The pathophysiology is uncertain. There are several lines of research to associate the different proteins of the virus in the endothelial lesion. From a therapeutic point of view there are treatment modalities that allow an acceptable life expectancy. As reported by the World Health Organization (WHO), the world is currently facing a devastating pandemic of a novel coronavirus (COVID-19), which started as an outbreak of pneumonia of unknown cause in the Wuhan city of China in December 2019. Since then, the respiratory disease has exponentially spread to over 210 countries. By the end of April, COVID-19 had caused over three million confirmed cases of infections and over 200,000 fatalities globally. The trend poses a huge threat to global public health. Understanding the early transmission dynamics of the infection and evaluating the effectiveness of control measures is crucial for assessing the potential for sustained transmission to occur in new areas. We employed a SEIHQRD delay differential mathematical transmission model with reported Kenyan data on cases of COVID-19 to estimate how transmission varies over time and which population to target for mass testing. The model is concise in structure, and successfully captures the course of the COVID-19 outbreak, and thus sheds light on understanding the trends of the outbreak and the vulnerable populations. The results show that, the government should target population in the informal settlement for mass testing and provide affordable sanitizers and clean water to this population. The model results also indicate that people with pre-existing non-communicable diseases (NCDs) should be identified and given special medical care. Given the absence of vaccine at the moment, non-pharmaceutical intervention is needed to effectively reduce the final epidemic size.

Clinical forensic medical examinations constitute an increasing proportion of our institution’s tasks, and, concomitantly, the authorities are now requesting forensic life-threatening danger assessments based on our examinations. The aim of this retrospective study was to assess if a probability of survival (PS) trauma score could be useful for these forensic life-threatening danger assessments and to identify a cut-off PS score as a supporting tool for the forensic practice of assessing life-threatening danger. We compared a forensic database and a trauma database and identified 161 individuals (aged 15 years or older) who had both a forensic life-threatening danger assessment and a PS score. The life-threatening danger assessments comprised the following statements: was not in life-threatening danger (NLD); could have been in life-threatening danger (CLD); or was in life-threatening danger (LD). The inclusion period was 2012–2016. A statistically significant difference was found in the PS scores between NLD, CLD and LD (chi-square test: p < 0.0001). The usefulness of the PS score for categorizing life-threatening danger assessments was determined by a receiver-operator characteristic (ROC) curve. The area under the curve was 0.76 (95% CI, 0.69 to 0.84) and the ROC curve revealed that a cut-off PS score of 95.8 would appropriately identify LD. Therefore, a PS score below 95.8 would indicate life-threatening danger. We propose a further exploration of how the evidence-based PS score, including a cut-off value, might be implemented in clinical forensic medical statements to add to the scientific strength of these statements.

Trauma brain injury (TBI) is the most common cause of death and disability in young adults. A method to determine the probability of survival (Ps) in trauma called iterative random comparison classification (IRCC) was developed and its performance was evaluated in TBI. IRCC operates by iteratively comparing the test case with randomly chosen subgroups of cases from a database of known outcomes (survivors and not survivors) and determines the overall percentage match. The performance of IRCC to determine Ps in TBI was compared with two existing methods. One was Ps14 that uses regression and the other was predictive statistical diagnosis (PSD) that is based on Bayesian statistic. The TBI database contained 4124 adult cases (mean age 67.9 years, standard deviation 21.6) of which 3553 (86.2%) were survivors and 571 (13.8%) were not survivors. IRCC determined Ps for the survivors and not survivors with an accuracy of 79.0 and 71.4%, respectively, while the corresponding values for Ps14 were 97.4% (survivors) and 40.2% (not survivors) and for PSD were 90.8% (survivors) and 50% (not survivors). IRCC could be valuable for determining Ps in TBI and with a suitable database in other traumas.

Background: An estimated 350,000 individuals experience out-of-hospital cardiac arrest (OHCA) in the United States (US) annually; only approximately 10% survive. The probability of survival after OHCA doubles when a bystander uses an automated external defibrillator (AED) before emergency management services arrives; however, bystander AED use is less than 2% in the US. We conducted a series of randomized trials in a community setting comparing the delivery of an AED by aerial drone to that of a bystander searching for and retrieving an AED from fixed locations in the area. Methods: We conducted 35 simulation trials of an OHCA using a life-sized manikin accompanied by two participants, paired by gender and age (18-34, 35-49, 50-65 years), in a community setting, with seven trials per zone in five distinct zones. Zones represented different environmental challenges to drone navigation and pedestrian acquisition of an AED and varied in number of fixed AEDs in the area (range 1-8 AEDs in a 600 ft radius from the OHCA site). We used a DJI Matrice 600 Pro drone, modified to carry a standard AED, flying autonomously from a preprogrammed flight path to the OHCA site. Drone launch sites varied by zone and ranged from 800 to 1300 ft from the OHCA site. We randomized participants either to call a mock 9-1-1 telecommunicator who would initiate the drone’s flight sequence, or to conduct a ground search to locate and retrieve an AED from a fixed location. We compared the delivery time of an AED by drone to that of an AED acquired by ground search. We conducted pre- and post-trial interviews with each participant to query perceptions. Results: We conducted 18 trials with women and 17 with men. These included 15, 11, and 9 trials across the three age strata, respectively. The average time (minutes: seconds) from onset of the simulated OHCA to AED delivery was 1:21 faster by drone (4:45, standard deviation (SD) = 0.34) compared to ground search (6:06, SD = 3:21). In 71% of trials (n=25 of 35), the drone delivered the AED to the event site within 5 minutes, compared to 51% (n=18 of 35) of ground searches (p=0.09). Median AED delivery time was faster by drone than by ground search in four of the five zones (range 0:17 to 2:56). Ground search was faster than the drone in one zone with 7 AEDs within 600 ft of the simulated OHCA event site and the shortest average distance to an AED (254 ft) of all the five zones. Among participants randomized to call for the drone, 89% reported that they felt comfortable as the drone approached, and 72% reported having no safety concerns. Nearly half of participants randomized to conduct a ground search reported difficulty finding an AED. Conclusion: Our study suggests that drone delivery of an AED to the site of an OHCA is feasible and acceptable in a community setting. Drones may provide more timely access to early defibrillation compared to bystander search and acquisition of an AED from the surrounding area.

Introduction: Despite recent advances in critical care, out-of-hospital cardiac arrest (OHCA) patients still have a low probability of survival. For the purpose of an objective assessment of a patient’s risk of death or neurological deficits, several clinical tools have recently been developed. Hypothesis: The first hypothesis is that OHCA and C-GRApH score could predict neurologic outcome after OHCA patients treated with targeted temperature management (TTM). The second hypothesis is that adding neurologic examination to those scores could further improve outcome prediction after OHCA. Methods: This retrospective study included OHCA patients treated with TTM from 2009 to 2017 in a tertiary teaching hospital in Seoul, Korea. We calculated two cardiac arrest specific risk scores (OHCA and C-GRApH) at the time of admission. Initial neurologic examination (Full Outline of UnResponsiveness Brainstem reflexes (FOUR_B) score and Glasgow Coma Scale motor score (GCS_M)) was also evaluated. The primary outcome was neurological outcome at 6 months after CA. Results: Of 311 subjects, 100 (32.2%) had a good neurologic outcome at 6 months after CA. OHCA score had AUC of 0.843 (95% CI 0.797 - 0.888) and C-GRApH score had AUC of 0.774 (95% CI 0.717 - 0.830). Addition of FOUR_B or GCS_M to OHCA score improved prediction of poor neurologic outcome (AUC 0.891, p = 0.001 for FOUR_B and AUC 0.878, p=0.004 for GCS_M). Results were similar for C-GRApH score to predict poor neurologic outcome. Conclusions: This study confirms the good prognostic performance of cardiac arrest specific scores to predict neurological outcomes in OHCA patients treated with TTM. Addition of neurologic examination variables further improved prognostication for neurologic outcome.

The wide disparity in the methodology of preventable death analysis has created a lack of comparability among previous studies. The guidelines for the peer review (PR) procedure suggest the inclusion of risk-adjustment methods to identify patients to review, that is, exclude non-preventable deaths (probability of survival [Ps] 25%) or focus on preventable deaths (Ps 50%). We aimed to, through PR process, (1) identify preventable death and errors committed in a level-I trauma centre, and (2) explore the use of different risk-adjustment methods as a complement.A multidisciplinary committee reviewed all trauma patients, which died a trauma-related death, within 30 days of admission to Karolinska University Hospital, Stockholm, in the period of 2012-2016. Ps was calculated according to TRISS and NORMIT and their accuracy where compared.Two hundred and ninety-eight deaths were identified and 252 were reviewed. The majority of deaths occurred between 1 and 7 days. Ten deaths (4.0%) were classified as preventable. Sixty-seven errors were identified in 53 (21.0%) deaths. The most common error was inappropriate treatment in all deaths (21 of 67) and in preventable deaths (5 of 13). Median Ps in non-preventable deaths was higher than the cut-off (25%) and Ps-TRISS was almost twice as high as Ps-NORMIT (65% vs 33%, P .001). Two clinically judged preventable deaths with Ps25% would have been missed with both models. Median Ps in preventable deaths was above the cut-off (50%) and higher with Ps-TRISS vs Ps-NORMIT (75% vs 58%, P .001). Three and 4 clinically judged preventable deaths would have been missed, respectively, for TRISS and NORMIT, if using this cut-off.Preventable deaths were commonly caused by clinical judgment errors in the early phases but death occurred late. Ps calculated with NORMIT was more accurate than TRISS in predicting mortality, but both perform poorly in identifying preventable and non-preventable deaths when applying the cut-offs. PR of all trauma death is still the golden standard in preventability analysis.

Background: People who experience an Out-of-Hospital Cardiac Arrest (OHCA) are very unlikely to survive because the probability of survival decreases by 7-10%. Therefore, immediate treatment is needed by people around the victim (bystanders). Early cardiopulmonary resuscitation (CPR) provided by bystanders reduces death rate associated with OHCA. However many bystanders can not do CPR and the majority were young people. Objectives: This research aimed to investigate the effect of cardiopulmonary resuscitation (CPR) simulations on (knowledge, attitudes, and skills) of Youth Organization’s members in Candibinangun Village, Pakem District, Sleman. Methods: The design of this research was one group pre-test post-test.In this study, the respondents were taught CPR simulation intervention. The sampling technique used was accidental sampling with a total sample of 15 respondents. The inclusion criteria were age>13 years, had never taken CPR simulation training, unskilled bystander, and willing to become respondents. The exclusion criteria were members of the Youth Organization having experience as a trained bystander. This research used bivariate dataanalysisandWilcoxonRankTestasthedatawasnotnormallydistributed. Results: CPR simulations had an effect on the knowledge (p = 0.001), attitudes (p = 0.009), and skills of rescuing victims of cardiac arrest (p = 0.001). The average score of pre-test and post-test on the knowledge variable were 32 and 81.33. The average score of pre-test and post-test on the attitude variable were 57.33 and 82.67. The average scoreofpre-testandpost-testonskillvariableis8and75. Conclusion: CPRsimulation increases the ability of young people as bystanders. Suggestion: CPR simulation is included in Karang Taruna (Youth) Organization routine activity.

Background and Objectives: Oral anticoagulants are the hallmark of cardioembolic stroke prevention, but they are frequently underused, especially in elderly patients and patients with paroxysmal atrial fibrillation. In our paper, we analyzed the long-term outcome of severely disabled cardioembolic stroke survivors depending on the prescribed antithrombotic secondary prevention medication. Materials and Methods: In our study, we retrospectively collected data for ischemic stroke (IS) patients treated in P. Stradins Clinical University hospital, Riga, Latvia, from 2014 until 2017. Patients&rsquo, clinical data were collected using local stroke registry, including patients&rsquo, demographic data, vascular risk factors, clinical findings, and laboratory results. Severely disabled stroke survivors were followed up by phone at 30/90/180/365 days after discharge. Patients&rsquo, functional outcomes were assessed using the adapted version of The Rankin Focused Assessment&ndash, Ambulation. The collected data were compared in 4 groups according to prescribed secondary prevention medication. Results: A total of 682 (91.42%) patients were followed up and included in data analysis. The median age of patients was 80 (IQR = 75&ndash, 85) years. Of these patients, 231 (31%) were males and 515 (69%) were females. One-year probability of survival of patients not taking any preventive medication was 53% (IQR = 29&ndash, 76), while in patients taking antiplatelet agents it was 57% (IQR = 37&ndash, 78), 78% (IQR = 68&ndash, 88) of patients on Vitamin K antagonists (VKA) and 81% (IQR = 72&ndash, 90) in patients on direct oral anticoagulants (DOACs). One year after discharge 73 (31%) had mRS 0&ndash, 2, 50 (20.9%), 29 (12.1%) were still severely disabled, and 87 (36.4%) had died. Conclusions: Anticoagulant use in secondary prevention predicts better functional outcome and higher survival rate in patients with severe cardioembolic stroke due to non-valvular atrial fibrillation (NVAF), therefore severe neurological deficit must not be a reason of restriction of anticoagulation.

Prognosis after severe brain injury is highly uncertain, and decisions to withhold or withdraw life-sustaining treatment are often made prematurely. These decisions are often driven by a desire to avoid a situation where the patient becomes 'trapped' in a condition they would find unacceptable. However, this means that a proportion of patients who would have gone on to make a good recovery, are allowed to die. I propose a shift in practice towards the routine provision of aggressive care, even in cases where the probability of survival and acceptable recovery is thought to be low. In conjunction with this shift, I argue in favour of a presumption towards withdrawing life-sustaining treatment, including artificial nutrition and hydration, when it becomes clear that a patient will not recover to a level that would be acceptable to them. I then respond to three potential objections to this proposal.

International audience; PURPOSE : Catastrophic antiphospholipid syndrome (CAPS), the most severe manifestation of antiphospholipid syndrome (APS), is characterised by simultaneous thromboses in multiple organs. Diagnosing CAPS can be challenging but its early recognition and management is crucial for a favourable outcome. This study was undertaken to evaluate the frequencies, distributions and ability to predict mortality of "definite/probable" or "no-CAPS" categories of thrombotic APS patients requiring admission to the intensive care unit (ICU).METHOD : This French national multicentre retrospective study, conducted from January 2000 to September 2018, included all APS patients with any new thrombotic manifestation(s) admitted to 24 ICUs. RESULTS : One hundred and thirty-four patients (male/female ratio: 0.4; mean age at admission: 45.4+/-15.0 years), who experienced 152 CAPS episodes, required ICU admission. The numbers of definite, probable or no-CAPS episodes, respectively, were: 11 (7.2%), 60 (39.5%) and 81 (53.3%). No histopathological proof of microvascular thrombosis was the most frequent reason for not being classified as definite CAPS. Overall, 35/152 (23.0%) episodes were fatal, with comparable rates for definite/probable CAPS and no CAPS (23% vs. 28.8% respectively, p=0.4). The Kaplan-Meier curve of estimated probability of survival showed no between-group survival difference (log-rank test p=0.5). CONCLUSIONS : In this study, CAPS criteria were not associated with mortality of thrombotic APS patients requiring ICU admission. Further studies are need evaluate the adequacy of CAPS criteria for critically-ill APS patients.

Summary Mesenchymal stromal cells (MSCs) have been successfully used for the treatment of steroid‐resistant graft‐versus‐host‐disease (GvHD). However, the lack of early predictors of clinical responses impacts on the time at which to add further treatment and consequently the design of informative clinical trials. Here, we present the UK experience of one of the largest cohorts of GvHD patients undergoing MSC infusions so far reported. We show that clinical responses assessed as early as 1 week after MSC infusion predict patients’ overall survival. In our cohort, cell dose, patients’ age and type of organ involvement are crucial factors associated with clinical responses.

Purpose The match run is the automated mechanism by which UNOS prioritizes donor hearts to waitlist candidates for heart transplant (HT) based on medical urgency. We sought to determine how well match runs perform in sequencing patients according to their medical urgency after implementation of the 2016 pediatric heart allocation revisions. Methods OPTN data were used to identify all match runs between March 21, 2016 and June 9, 2020 for each pediatric donor heart offered through UNOS to US waitlist candidates. The rank-order of candidates on the match run was compared to the rank-order based on medical urgency alone defined as the probability of survival on the day of the match run using a risk prediction model developed and validated during the prior 6 years. Results Overall, 634 donors generated 2,524 unique match runs during the study period when a total of 2,608 candidates were listed for pediatric HT. The median age of donors was 5 years (Q1, Q3, 1, 13), median weight 21 (11, 54) kg with the most common cause of death natural causes (26%), non-motor vehicle accidents (MVA) (22%) and MVA (16%). The risk-prediction model for death or deterioration included 8 variables (age, weight, cardiac diagnosis and cardiac support). When the survival model was applied to the match run candidates within zone, 26% of candidates were ranked out of survival order (i.e. had a rank-reversal) (Table). Of these, 69% of rank-reversals involved candidates within the same UNOS status levels whereas 41% involved candidates spread across UNOS status levels. Conclusion There is evidence to suggest that the 2016 revision to the pediatric donor heart allocation system fails to sequence waitlisted candidates according to medical urgency, which affects a substantial fraction of match runs. Misclassification of patient risk by UNOS status level and within UNOS status levels each contribute rank-reversals. Rank-ordering patients based on a continuous survival probability model could address both problems.

Background: Characteristics and outcome in out-of-hospital cardiac arrest (OHCA) occurring at workplaces is sparsely studied. Aim: To describe (1) the characteristics and 30-day survival of OHCAs occurring at workplaces in comparison to OHCAs at other places and (2) factors associated with survival after OHCAs at workplaces. Methods: Data on OHCAs were obtained from the Swedish Registry of Cardiopulmonary Resuscitation from 1 January 2008 to 31 December 2018. Characteristics and factors associated with survival were analysed with emphasis on the location of OHCAs. Results: Among 47,685 OHCAs, 529 cases (1%) occurred at workplaces. Overall, in the fully adjusted model, all locations of OHCA, with the exception of crowded public places, displayed significantly lower probability of survival than workplaces. Exhibiting a shockable rhythm was the strongest predictor of survival among patients with OHCAs at workplaces; odds ratio (95% CI) 5.80 (2.92-12.31). Odds ratio for survival for women was 2.08 (95% CI 1.07-4.03), compared with men. At workplaces other than private offices, odds ratio for survival was 0.41 (95% CI 0.16-0.95) for cases who did not receive bystander CPR, as compared to those who did receive CPR. Among patients who were found in a shockable rhythm were 23% defibrillated before arrival of ambulance, which was more frequent than in any other location. Conclusion: Out-of-hospital cardiac arrest occurring at workplaces and crowded public places display the highest probability of survival, as compared with other places outside hospital. An initial shockable cardiac rhythm was the strongest predictor of survival for OHCA at workplaces. At 3 years after hematopoietic stem cell transplant (SCT), the probability of survival is 30%–60%, with disease recurrence accounting for 50%–60% of deaths.[1][1] Cardiovascular disease (CVD), secondary malignancy and infections are leading causes of death unrelated to the primary disease.[1][1

Aim: Inflammatory biomarkers are associated with aging and disease-specific outcomes. We propose neutrophil–lymphocyte ratio (NLR) informs survival in a noncancer-bearing population. Patients & methods: Retrospective cohort study of patients with a noncancer diagnosis. Calculation of NLR, ascertainment of age, gender, race, cardiovascular disease and diabetes status, and association with survival was determined. Results: Elevated NLR was associated with worse overall survival, independent of age, gender and comorbid status. Overall survival was significantly worse for patients with high versus low NLR. Conclusion: Elevated NLR is associated with worse overall survival in noncancer patients. It remains unclear whether NLR reflects an acute inflammatory state, depressed host immune competence or both. NLR may simply be another predictor of survival, or potentially a modifiable risk factor.

Background: Surgical Site Infections (SSIs) are the most commonly healthcare-associated infections among patients undergoing surgical procedures. Reaching zero % SSI rate seems to be impossible target, although most of them are easily preventable through effective preventive strategies. Objective: we aimed to evaluate application of a “surgical care bundle” on patients undergoing cardiac surgeries, with the goal of reducing overall SSI rates. Methodology: This interventional study was conducted on two groups of patient: The baseline group consisted of patients having cardiac surgery during a 16-month period of time from January 2012 till April 2013, while the intervention group included patients having cardiac surgery during a 16-month period of time fsrom January 2014 till April 2015, a preparatory period was from (May 2013 till December 2013) in between the 2 periods of time to educate, raise the awareness and build capacities to implement the surgical care bundle and finally reach at least 90% compliance with the bundle. SSI rates were calculated among two patient groups. SSI microbiology was noted; microorganisms were isolated and culture of wound swab or aspirate from patients with SSI, and were identified based on standard microbiological techniques. Every Patient with SSI was followed up for six months to detect mortality from infection. Results: A 4 years study showed a high statistically significant decline in the SSI rate average from 5.9% in baseline group 1.9 % in the intervention group (P value ˂ 0.001).The rate of SSI caused by MRSA revealed a highly statistical significant decline in the patients having SSI in the intervention group 8.01; compared to the baseline group 29.6 (P value ˂ 0.001). Also there was a statistically significant decline in the rate of SSI caused by MDROs from 41.8% in baseline group and 11.6% in the intervention group (P value ˂ 0.05). Probability of survival increased in the intervention group; as the mortality from infection decreased. Conclusion : Strict adherence to surgical care bundle reduced the overall SSI rate, and decreased its rate when caused by MRSA &MDROs in post cardiac surgery patients

Aim There is little data to inform the appropriate duration of resuscitation attempts for out-of-hospital cardiac arrest (OHCA). We assessed the relationship of elapsed duration since commencement of resuscitation and outcomes, highlighting differences between initial shockable and non-shockable rhythms. Methods We examined consecutive adult non-traumatic EMS-treated OHCA in a single health region. We plotted the time-dependent accrual of patients with ROSC, as well as dynamic estimates of outcomes as a function of duration from commencement of professional resuscitation, and compared subgroups dichotomized by initial rhythm. Logistic regression tested the association between time-to-ROSC and outcomes. Results Of 1627 adult EMS-treated cases of OHCA, 1617 patients were included; 14% survivors and 10% with favorable neurological outcomes. Time-to-ROSC (per minute increase) was independently associated with survival in those with initial shockable (aOR 0.95, 95% CI 0.92–0.97) and non-shockable (aOR 0.83; 95% CI 0.78–0.88) rhythms. Similar associations were seen with favorable neurologic outcome. The elapsed duration at which the probability of survival fell below 1% was 48 and 15 min in the shockable and non-shockable groups, respectively. Median time-to-termination of resuscitation was 36 and 26 min in the shockable and non-shockable groups, respectively. Conclusion The subgroup of initial shockable rhythms showed a less pronounced association of time-to-ROSC with outcomes, and demonstrated higher resilience for neurologically intact survival after prolonged periods of resuscitation. This data can guide minimum durations of resuscitation, however should not be considered as evidence for termination of resuscitation as survival in this cohort may have been improved with longer resuscitation attempts.

Determining the probability of survival after injury is important as it can inform triage, clinical research and audit. A number of methods have been reported for determining the probability of survival after injury. However, these have shortcomings and thus further developments are needed to improve their reliability and accuracy. In this study, a Bayesian method called Predictive Statistical Diagnosis (PSD) was developed to determine probability of survival in 4124 adults (age: mean = 67.9 years, standard deviation = 21.6 years) with traumatic brain injuries (TBI). In total, 86.2% of cases had survived and 13.8% of cases had not survived their injuries. The parameters considered as inputs to PSD were age, abbreviated injury score (AIS), Glasgow coma score (GCS), pulse rate (PR), systolic blood pressure (SBP) and respiration rate (RR). PSD statistically modeled the TBI cases and their associated injury outcomes, i.e., survived or not survived. The model was calibrated on randomly selected, roughly 2/3 (number 2676), of the cases and its performance was validated on the remaining cases (number 1448, i.e., validation dataset). The effectiveness of PSD in determining the probability of survival was compared with a method called Ps14 that uses regression modeling. With all parameters (i.e., age, AIS, GCS, SBP, RR and PR) included as inputs to PSD, it correctly identified 90.8% of survivors and 50.0% of non-survivors in the validation dataset while Ps14 identified 97.4% of survivors and 40.2% of non-survivors in the validation dataset. When age, AIS and GCS were used on their own as inputs to PSD, it correctly identified 82.4% of the survivors and 65.0% of non-survivors in the validation dataset. Age affected the performance of PSD in determining the survival outcomes. The number of non-surviving cases included in this study may have not been sufficiently high to indicate the full potential of PSD and a further study with a larger number of cases would be beneficial.

A preliminary computational analysis of a number of factors affecting the probability of survival in trauma injuries\ud was carried out. The study examined the manner the types and\ud extent of body injuries, specific body regions affected by the injuries, pre-exiting medical conditions, physiological parameters (e.g. heart rate, blood pressure and respiration rate), age, gender and Glasgow Comma Score contribute to the probability of survival. A more in depth analysis of these factors are currently ongoing to develop a model for the probability of survival.

OBJECTIVES To assess pre-/post-transplantation changes in autonomic tone, as measured by heart rate variability (HRV), among patients undergoing hematopoietic cell transplantation (HCT) and to look at those changes as they relate to post-transplantation survival rates. . SAMPLE A SETTING Data were derived from a sample of 27 English-speaking patients undergoing allogeneic or autologous HCT at Stanford University. . METHODS A VARIABLES A survival analysis using the Kaplan-Meier estimator was employed to explore whether increased HRV would enhance survival probabilities over time among patients undergoing HCT. . RESULTS An increased probability of survival was significantly related to increases in two HRV indexes. IMPLICATIONS FOR NURSING HRV may be a useful predictor of mortality among patients undergoing HCT. Interventions deliverable by nurses could be used to enhance HRV for patients identified as being at risk for early mortality.

With restructuring within the NHS, there is increased public and media interest in surgical outcomes. The Nottingham Hip Fracture Score (NHFS) is a well-validated tool in predicting 30-day mortality in hip fractures. VLAD provides a visual plot in real time of the difference between the cumulative expected mortality and the actual death occurring. Survivors are incorporated as a positive value equal to 1 minus the probability of survival and deaths as a negative value equal to the probability of survival. Downward deflections indicate mortality and potentially suboptimal care. We prospectively included every hip fracture admitted to UHW that underwent surgery from January–August 2014. NHFS was then calculated and predicted survival identified. A VLAD plot was then produced comparing the predicted with the actual 30-day mortality. Two hundred and seventy-seven patients have completed the 30-day follow-up, and initial results showed that the actual 30-day mortality (7.2 %) was much lower than that predicted by the NHFS (8.0 %). This was reflected by a positive trend on the VLAD plot. Variable life-adjusted display provides an easy-to-use graphical representation of risk-adjusted survival over time and can act as an “early warning” system to identify trends in mortality for hip fractures.

Advancing health screening and treatment of cancer techniques, they contribute to grow the probability of survival of cancer patients for a long time. So health behavior and quality of life of the patients are getting important. This study analysed correlation between healthy person and cancer patient's EQ-5D index, social demographic characteristics, health behavior and so on by the multiple regression analysis. The result was that EQ-5D index of cancer patients is lower than healthy persons. Patients of cervical cancer and lung cancer had very low the index especially. In conclusion, cancer have a bad influence on the quality of life. For cancer patients, smoking and drinking are a major factors of correlation. The number of non-smokers among the patients is lower than the number of smokers among healthy persons. This conclusion means that the importance of health behaviors and quality of life for cancer patients is established so that this will be used for basic reference of PHR models and service enhancing quality of life.

Background Emergency 9‐1‐1 callers use a wide range of terms to describe abnormal breathing in persons with out‐of‐hospital cardiac arrest ( OHCA ). These breathing descriptors can obstruct the telephone cardiopulmonary resuscitation ( CPR ) process. Methods and Results We conducted an observational study of emergency call audio recordings linked to confirmed OHCA s in a statewide Utstein‐style database. Breathing descriptors fell into 1 of 8 groups (eg, gasping, snoring). We divided the study population into groups with and without descriptors for abnormal breathing to investigate the impact of these descriptors on patient outcomes and telephone CPR process. Callers used descriptors in 459 of 2411 cases (19.0%) between October 1, 2010, and December 31, 2014. Survival outcome was better when the caller used a breathing descriptor (19.6% versus 8.8%, P OHCA recognition, CPR instruction, and telephone CPR were lower than when callers did not use a breathing descriptor (79.7% versus 93.0%, P P =0.0078; and 60.2% versus 66.9%, P =0.0123, respectively). The time interval between call receipt and OHCA recognition was longer when the caller used a breathing descriptor (118.5 versus 73.5 seconds, P Conclusions Descriptors of abnormal breathing are associated with improved outcomes but also with delays in the identification of OHCA . Familiarizing telecommunicators with these descriptors may improve the telephone CPR process including OHCA recognition for patients with increased probability of survival.

Background We previously identified factors associated with a greater risk of death post-transplant. The purpose of this study was to develop a clinical tool to estimate the risk of death after transplant based on pre-transplant variables. Methods We utilized the Canadian CF registry to develop a nomogram that incorporates pre-transplant clinical measures to assess post-lung transplant survival. The 1-, 3-, and 5-year survival estimates were calculated using Cox proportional hazards models. Results Between 1988 and 2012, 539 adult Canadians with CF received a lung transplant with 208 deaths in the study period. Four pre-transplant factors most predictive of poor post-transplant survival were older age at transplantation, infection with B. cepacia complex, low FEV1 percent predicted and pancreatic sufficiency. A non-linear relationship was found between risk of death and FEV1 percent predicted, age at transplant, and BMI. We constructed a risk calculator based on our model to estimate the 1-, 3-, and 5- probability of survival after transplant which is available on-line at www.cf-post-tx-survival-nomogram.appspot.com. Conclusions Our risk calculator quantifies the risk of death associated with lung transplant using pre-transplant factors. This tool could aid clinicians and patients in the decision making process and provide information regarding the timing of lung transplantation. This article is protected by copyright. Femoral cement-in-cement revisions are attractive if the cement bone mantle is well fixed. However, most available cemented stems are too long to fit in the existing cement mantle. We evaluated the medium-term outcomes of the 125 mm short tapered polished stem (Exeter Short Revision Stem (SRS)) with a 44 mm offset specifically designed to facilitate cement-in-cement revisions of hip arthroplasties. The Exeter SRS was clinically and radiographically evaluated in 24 consecutive femoral cement-in-cement revisions (11 men, 13 women) between July 2005 and February 2008 after a mean follow-up of six years (5-7). The mean age at operation was 67 years (54-83). No hip was lost to follow-up, but two patients (two hips) died. None of the deaths were related to the surgery. Kaplan Meier survival analysis was performed. Four femoral components (17%) were removed for septic loosening after a mean of 2.4 years (0.8-4.9). Three of these hips were revised again in a two-stage revision, and one was converted to a permanent excision arthroplasty. The probability of survival with re-revision for any reason was 82% (95% CI: 58-93) and survivorship with aseptic loosening as the endpoint was 100% at six years. There were no additional radiological failures. The Exeter Short Revision Stem is a valuable option for simplifying cement-in-cement revisions. Despite the short stem length, at mid-term there were no signs of instability or aseptic loosening.

We evaluated the effect of increased plasma cortisol levels on fish antipredator behavior induced by conspecific chemical alarm cues. The experimental model for the study was the Frillfin goby Bathygobius soporator. We first confirmed that the alarm substance induces typical defensive antipredator responses in Frillfin gobies and described their alarm substance cells (epidermal 'club' cells). Second, we confirmed that intraperitoneal cortisol implants increase plasma cortisol levels in this species. We then demonstrated that exogenous cortisol administration and subsequent exposure to an alarm substance decreased swimming activity to a greater extent than the activity prompted by either stimulus alone. In addition, cortisol did not abolish the sheltering response to the alarm chemical cue even though it decreased activity. As predators use prey movements to guide their first contact with the prey, a factor that decreases swimming activity clearly increases the probability of survival. Consequently, this observation indicates that cortisol helps improve the antipredator response in fish.

This study aims to determine pre-morbid parameters as possible predictors of outcome of hip fracture in octogenarians with unstable intertrochanteric fracture treated operatively. Presence of co-morbidities, pre-injury level of ambulation, type of surgery, and period of delay in surgery were considered, and their effect on the post-operative outcome was evaluated using the Harris Hip Score. The computed probability of survival of octogenarians who had surgery was approximately 11 months. In patients with two or more co-morbidities, there is a significant effect on Harris Hip Score in terms of pain and deformity. Delay in surgery of more than two weeks significantly decreased the distance travelled at one year. The overall recovery is correlated to preinjury level of ambulation and delay in surgery. Patients with intertochanteric fracture in this age group, who have less co-morbidities and with more independent ambulation, are good candidates for timely operative treatment.

6556 Background: Survival prediction models for lung cancer patients could help guide their care and therapy decisions. The objectives of this study were to predict probability of survival beyond 90, 180 and 360 days from any point in a lung cancer patient’s journey. Methods: We developed a Gradient Boosting model (XGBoost) using data from 55k lung cancer patients in the ASCO CancerLinQ database that used 3958 unique variables including Dx and Rx codes, biomarkers, surgeries and lab tests from ≤1 year prior to the prediction point, which was chosen at random for each patient. We used 40% data for training, 25% for hyper-parameter tuning, 20% for testing and 15% for holdout validation. Death date available in the Electronic Health Record was cross checked by linkage to death registries. Results: The model was validated on the holdout set of 8,468 patients. The Area Under the Curve (AUC) for the model was 0.79. The precision and recall for predicting survival beyond the three time points were between 0.7-0.8 and 0.8-0.9 respectively (see table). This compares favourably to other lung cancer survival models created using different machine learning techniques (Jochems 2017, Dekker 2009). A Cox-PH model created using the top 20 variables also had a significantly lower performance (see table). Analysis of input variables yielded distinctive patterns for patient subgroups and time points. Tumor status, medications, lab values and functional status were found to be significant in patient sub cohorts. Conclusions: An AI model to predict survival of lung cancer patients built using a large real world dataset yielded high accuracy. This general model can further be used to predict survival of sub cohorts stratified by variables such as stage or various treatment effects. Such a model could be useful for assessing patient risk and treatment options, evaluating cost and quality of care or determining clinical trial eligibility. [Table: see text]

Objective: The objective of this study was to investigate the prognostic value of a hernia sac in isolated congenital diaphragmatic hernia (CDH) with intrathoracic liver herniation (“liver-up”). Study Design: A retrospective study from the single tertiary center. Isolated “liver-up” CDH neonates referred to our institution between 2000 and 2015 were reviewed for the presence or absence of a hernia sac. Association between the presence of a hernia sac and survival was assessed. Results: Over the study period, there were 29 isolated CDH patients with “liver-up” who were treated, 7 (24%) had a sac, and 22 (76%) did not. Demographics were similar between groups. However, disease acuity, assessed from lower Apgar scores (p = 0.044), lower probability of survival (p = 0.037), and lower admission oxygenation (p = 0.027), was higher in neonates without a sac. Hospital survival was significantly higher for those with sac compared with those without (7/7, 100 vs. 7/22, 32%, p = 0.002). Conclusion: The presence of a hernia sac may be associated with better survival for isolated “liver-up” CDH. As the presence of sac can be prenatally detected, it may be a useful marker to aid perinatal decision making.

Rats are a commonly used model for aging studies, and a frailty assessment tool for rats would be of considerable value. There has been a recent focus on the development of preclinical models of frailty in mice. A mouse clinical frailty index (FI) was developed based on clinical frailty assessment tools. This FI measures the accumulation of clinically evident health-related deficits in mice. This paper aimed to develop a rat clinical FI. Male Fischer 344 rats were aged from 6 to 9 months (n = 12), and from 13 to 21 months (n = 41). A FI comprised of 27 health-related deficits was developed from a review of the literature and consultation with a veterinarian. Deficits were scored 0 if absent, 0.5 if mild, or 1 if severe. A FI score was determined for each rat every 3–4 months, and for the older group mortality was assessed up to 21 months. Mean FI scores significantly increased at each time point for the older rats. A high FI score measured at both 17 months of age and terminally was also associated with decreased probability of survival as assessed with Kaplan–Meier curves. The rat clinical FI has significant value for use in aging and interventional studies.

In 2002 and 2006, we reported the long-term results of 208 total hip replacements performed with the Zweymüller stem and a threaded cup in 200 patients. The present study gives an update on this patient cohort. At a minimum of twenty years postoperatively, seventy-three patients (seventy-five hips) were available for follow-up; twelve patients were lost to follow-up. The key findings of our previous reports were the absence of aseptic femoral stem loosening and a poor rate of survival of the threaded cup. Since then, two revisions have been performed because of aseptic stem loosening. We observed osteolytic lesions around the proximal part of the femoral component on twenty-four (47%) of fifty-one radiographs, but no stem was deemed at risk for loosening. The probability of survival of the stem at twenty years was 0.96 (95% confidence interval, 0.91 to 0.99), and the probability of survival of the cup at twenty years was 0.67 (95% confidence interval, 0.57 to 0.75). The Zweymüller femoral stem, a tapered, rectangular implant, continues to give excellent long-term results.

Introduction. Whole body CT is being used increasingly in the primary survey of major trauma patients. We evaluated whether omitting plain films of the chest and pelvis in the primary survey was safe. We compared the probability of survival of patients and time to CT who had plain X-rays to those who did not. Method. We performed a database study on major trauma patients admitted between 2008 and 2010 using data from Trauma, Audit and Research Network (TARN) and our PACS system. We included adult major trauma patients who has an ISS of greater than 15 and underwent whole body CT. Results. 245 patients were included in the study. 44 (17.9%) did not undergo plain films. The median time to whole body CT from the time of admission was longer (47 minutes) in patients having plain films, than those who did not have plain films performed (30 minutes), . Mortality was increased in the group who received plain films, 9.5% compared to 4.5%, but this was not statistically significant ( ). Conclusion. We conclude that plain films may be safely omitted during the primary survey of selected major trauma patients.

Background There is a lack of long-term data on cement-in-cement technique in revision of failed hip femoral stem. Methods We present the outcome of 69 consecutive recemented femoral prostheses, performed by one surgeon (GH) 22-40 years ago. Four patients (4 hips) were lost to follow-up. Sixty-three patients (65 hips) were followed for their lifetime or until the time of the preparation of the study. The study population consisted of 18 failed hemiarthroplasties and 47 failed total hip arthroplasties. Results The 23-year probability of survival for the recemented femoral components, with re-revision for any reason and resection arthroplasty as the end point, was 73.6% (61.8%-85.4%) and, with re-revision for aseptic loosening as the end point, was 82.2% (71.4%-93%). Conclusion Our follow-up study at 22-40 years, after recemented hip femoral prostheses, shows that recementing works well in selected cases.

Conversations regarding the chances of survival, the probability of survival without disability, or the risk of moderate to severe disability are common in such situations.

Outcome from in-hospital cardiopulmonary resuscitation (CPR) is still unsatisfactory. CPR assisted with percutaneous cardiopulmonary support (PCPS) is expected to improve the outcome of in-hospital CPR.We retrospectively analyzed 83 consecutive cases of adult in-hospital CPR assisted by a portable pre-assembled auto-priming PCPS system (EBS, Terumo, Japan) from January 2004 to December 2007.PCPS was successfully performed in 97.6% of the patients and could be weaned in 57.8% of the patients. The survival-to-discharge rate was 41.0% with an acceptable neurological status in 85.3% of the patients. The 6-month survival was 38.6%. Survival-to-discharge decreased about 1% for each 1 min increase in the duration of CPR. The probability of survival was about 65%, 45%, and 19% when the duration of CPR was 10, 30, or 60 min, respectively. Multivariate analysis adjusted with clinical factors including organ dysfunction severity scores revealed that defibrillation and CPR duration less than 35 min were independent predictors for both survival-to-discharge (odds ratio=8.0, 95% CI=2.8-23.0, p0.001) and 6-month survival (hazard ratio=3.3, 95% CI=1.9-5.9, p0.001).Our results showed that CPR assisted with PCPS results in an acceptable survival-to-discharge rate and mid-term prognosis.

OBJECTIVE: To determine the survival rate according to the main findings of emergency electroencephalography (EEGs) of patients treated in a tertiary hospital. METHOD: In this prospective study, the findings of consecutive emergency EEGs performed on inpatients in Hospital de Base in São José do Rio Preto, Brazil were correlated with survival utilizing Kaplan-Meyer survival curves. RESULTS: A total of 681 patients with an average age of 42 years old (1 day to 96 years) were evaluated, of which 406 were male. The main reasons for EEGs were epileptic seizures (221 cases), hepatic encephalopathy [116 cases of which 85 (73.3%) were men, p-value=0.001], status epilepticus (104 cases) and impaired consciousness (78 cases). The underlying disease was confirmed in 578 (84.3%) cases with 119 (17.5%) having liver disease [91 (76.0%) were men, p-value=0.001], 105 (15.4%) suffering strokes, 67 (9.9%) having metabolic disorders, 51 (7.5%) central nervous system infections and 49 (7.2%) epilepsy. In the three months following EEG, a survival rate of 75% was found in patients with normal, discreet slow activity or intermittent rhythmic delta activity EEGs, of 50% for those with continuous delta activity and generalized epileptiform discharges, and of 25% for those with burst-suppression, diffuse depression, and in alpha/theta-pattern coma. Death was pronounced immediately in patients with isoelectric EEGs. CONCLUSION: The main findings of EEGs, differentiated different survival rates and are thus a good prognostic tool for patients examined in emergencies. MÉTODO: Estudo prospectivo, por ordem de chegada, da correlação entre os achados de E-EEG, feitos nos pacientes à beira do leito, com TS, utilizando-se as curvas de sobrevidas de Kaplan Meyer no Hospital de Base de São José do Rio Preto, São Paulo/Brasil. RESULTADOS: Foram estudados 681 pacientes, dos quais 406 (59,6%) masculinos, com idade média de 42 anos (1 dia a 96 anos). As principais motivações para o E-EEG foram crises epilépticas (221 casos), encefalopatia hepática [(116 casos, dos quais 85 masculinos (73,3%), p= 0,001]; estado de mal epiléptico 104 e rebaixamento de consciência 78. O diagnóstico da doença de base foi confirmado em 578 (84,3%), sendo 119 (17,5%) hepatopatia, dos quais 91 (76,%) masculinos, p= 0,001; 105 (15,4%) acidente vascular encefálico; 67 (9,9%) distúrbio metabólico; 51 (7,5%) infecção do sistema nervoso central e 49 (7,2%) epilepsia. TS de 75% nos três primeiros meses foi encontrada nos pacientes com E-EEG com alentecimento discreto ou com atividade delta rítmica intermitente. TS por volta de 50% nos três meses foi encontrado nos pacientes com E-EEG com delta contínuo, crítico e com descargas periódicas. A TS foi menor que 25% nos dois primeiros meses após E-EEG, nos pacientes com E-EEG com surto/supressão, com depressão difusa e com comas alfa/teta e 0% nos E-EEG iselétricos. CONCLUSÃO: O E-EEG, com seus principais achados, foi capaz de diferenciar as diversas taxas de sobrevivências na amostra estudada, constituindo-se, portanto, bom instrumento de prognóstico para pacientes atendidos nas unidades de emergência hospitalar.

Using life tables, this paper analyses mortality transition in India from 1970 through 2005. The analysis reveals that the rise in the life expectancy in India has at best been slow by global standards despite the fact that mortality levels in India are still high. Improvements in the life expectancy has been particularly slow in the urban areas. The analysis also reveals that the rate of improvement in the probability of survival in the first five years of life has decelerated over time. For females, this probability has decreased in recent years. There has also been a considerable slowdown in the decrease in life table entropy, especially the female life table, indicating that the shift in the concentration of deaths towards older ages has slowed down. Hastening the pace of mortality transition in India requires a pragmatic, multi-dimensional approach, which is missing at present. There is a need to emphasize substantive health care rather than formal, institution-based care.

The systematic lymph dissection, a drastic treatment procedure in the surgical treatment of lung carcinoma, is declared to be an operative procedure helping to assess the true extent of a neoplastic [tumor] process. From the 300 operative interventions the 150 (50,0%) came to expanded operations which were followed by a systematic lymph dissection from principal considerations. The other 150 (50,0%) patients were operated typically without any systematic mediastinal lymph dissection. The general five-year probability of survival (equal to 30%), while the postoperative lethality is equal to 6,7%, speaks for a surgical assistance of a good quality in the clinical hospital. To present day 254 (85,0% from the 300) patients have been watched over.
