Objectif : Cette étude examine les systèmes de soutien et les besoins des frères et sœurs des jeunes touchés par un cancer, mettant en lumière les défis émotionnels et informationnels auxquels les frères et sœurs sont confrontés. Ce domaine a reçu relativement peu d'attention. Devis et méthodologie : Des entretiens qualitatifs ont été menés et une analyse thématique a été utilisée pour obtenir des perspectives approfondies sur les expériences et les points de vue des frères et sœurs. Bien que la taille relativement petite de l'échantillon de l'étude et l'homogénéité des participants soient reconnues comme des limites, l'approche présente plusieurs forces, notamment la pertinence et la diversité des participants dans les cohortes d'âge. Résultats : Les résultats soulignent le rôle essentiel des professionnels de la santé, en particulier des infirmières pour fournir du soutien émotionnel et informationnel aux frères et sœurs. Une approche centré sur la famille, un soutien psycho- social, et des interventions personnalisées sont recommandés pour répondre de manière efficace aux besoins spécifiques de la fratrie. Conclusion : Dans l'ensemble, cette étude met en lumière l'importance de reconnaître et de répondre aux besoins de soutien des frères et sœurs dans les soins en oncologie pédiatrique, en mettant en avant leur rôle en tant que composante essentielle du système familial. Cette étude souligne l'importance de fournir un soutien à la fratrie pendant et après l'expérience du cancer.

6. Evaluation d'une formation en sécurité transfusionnelle auprès des étudiants en médecine au Maroc.

Introduction: students worldwide receive little training on blood transfusion therapies during their time at medical schools and hospitals, then reformulation of academic programs is mandatory. Indications for blood transfusions are more frequent than recommended, which helps to increase the risks associated with this procedure. To overcome this, structured objective training on nursing care and procedures was organised with the topic of transfusion safety. The purpose of our study was to assess medical students' appreciation of this training and the degree to which this population had mastered it. Methods: we conducted a cross-sectional, descriptive, singlecentre study. We conducted a survey among the 3rd-year medical students. A self-administered assessment questionnaire, as well as an evaluation grid for acquired skills, filled in by the training doctors during the session, were used. Data analysis was based on descriptive statistics using Excel software. Results: three hundred and eightyfour (n=384) students were invited to attend this training course, 275 (71.6%) of whom were enrolled in the study. The overall satisfaction rate was 93.8%. The objectives and organisation were 95.6% successful. The quality of the training was 90.3% satisfactory. The choice of topic for the station was satisfactory for 80% of participants, the flow of the session for 86.3%, the quality of the organisation and debriefing for 89%, the interaction between trainers and learners for 90.2%, the motivation of trainers and the reflection stimulated in the learners for 92%. We noted that 93% of students had never participated in training in transfusion safety. Mastery of global skills was total for 67%, partial for 26% and absent for 7%. The students mastered the procedures forchecking the identity and grouping of the product to be transfused at 97%, the principle of interpreting the ultimate pre-transfusion test (96%), the purpose of ultimate test at the patient's bedside 93% with mastery of its technical implementation 83%. Conclusion: training in transfusion safety was well received, with a satisfactory level of proficiency. This experience can easily be extended to other topics. [ABSTRACT FROM AUTHOR]

Le gouvernement du Québec a investi dans une offensive de transformation numérique. Elle vise à accélérer le virage technologique de plusieurs secteurs d’activités incluant celui de l’enseignement. Pour les universités, cette décision se traduit par un rehaussement de l’intégration pédagogique des technologies de l’information et des communications (TIC). D’ailleurs, dans plusieurs cas, ces TIC rejoignent les personnes dans leur manière d’apprendre. Parmi ces nouvelles méthodes pédagogiques, nous retrouvons la simulation clinique par réalité virtuelle (SimRV), constituant un exemple d’actualisation des approches technopédagogiques au service de l’apprentissage. En reprenant des scénarios de simulation clinique traditionnels, la SimRV amène la personne, par le biais d’expériences immersives, à réaliser des simulations qui, autrefois, nécessitaient l’accès à une structure formelle. Bien que cette approche soit récente, plusieurs avenues viennent lui conférer un grand potentiel d’application. Une équipe de la Faculté des sciences infirmières de l’Université Laval a testé ce potentiel dans un projet exploratoire de simulations interactives par vidéo 360 degrés interactive, dans le programme de deuxième cycle de formation des infirmiers praticiens spécialisés. Cet article présentera les avantages, les limites et les enjeux identifiés pour une intégration pédagogique réussie de cette nouvelle technologie.

Background: Informed consent (IC) is an ethical and legal obligation protected by constitutional rights to bodily integrity, well-being, and privacy in South Africa. The National Health Act 2003 codified IC regulations, requiring that all healthcare professionals inform patients about diagnosis, risks, benefits, options, and refusal rights while factoring in patients’ language and literacy levels. Objectives: This study’s primary aim was to determine the extent of South African professional/staff nurses’ compliance with current IC regulations and ascertain sociocultural impediments impacting proper IC practice. Methods: A cross-sectional survey using semi-structured questionnaires was used to evaluate knowledge and practice of IC among nurses in KwaZulu-Natal province. Data were analyzed using SPSS, v.21. Descriptive statistics, chi-squared tests, and content analysis were used to compare nursing domains. Results: Three hundred fifty-five (355) nurses, 92% females, with 1 to 41 years of professional experience, completed this study. Information disclosed by nurses to patients included diagnosis (77%), treatment benefits (71%), risks (69%), recommendations (65%), risks of refusal (80%), and right of refusal (67%). Nurses (80%) felt information disclosure was adequate, while 85% reported that patients understood disclosed information. Conclusions: Nurses practicing in local public hospitals had moderate knowledge of IC regulations. Practical implementation appeared deficient. Barriers to IC included language, workload, time constraints, lack of interpreters, and skewed gender norms in the nursing profession. Nurses require continuing professional education in healthcare law and ethics, a “corps of trained interpreters”, and gender transformation in the nursing profession to improve IC practice and overall quality of healthcare service delivery in South Africa. La situation actuelle en milieux de psychiatrie légale ne permet pas aux patients d’exprimer pleinement leur sexualité durant leur hospitalisation. Dans certains cas, les relations sexuelles sont tout simplement interdites. Non seulement, les politiques institutionnelles en place briment, en partie, les personnes dans l’exercice de leurs droits fondamentaux elles manquent aussi de clarté au regard de la façon dont le personnel infirmier devrait gérer les besoins des patients. Une étude ethnographique critique a été conduite dans un centre de psychiatrie légale canadien. La collecte de données a inclus des entrevues semi-structurées, la collecte de documents institutionnels et l’observation du milieu. Nos résultats s’articulent autour de trois thèmes: situer la sexualité en contexte médico-légal, faire l’expérience de la sexualité en tant que patient et gouverner la sexualité des patients. Les données récoltées permettent de constater que ces discours façonnent les croyances et les actions des patients, des infirmières et des autres professionnels tels que les psychiatres, les éducateurs et les psychologues.

Les dispositifs d’enseignement hybride dans la formation en soins infirmiers sont peu représentés dans la littérature scientifique (Li et al., 2019). Pourtant le public de cette formation est particulier puisque nombre d’étudiants ont une expérience professionnelle dans le milieu des soins avant même de débuter leur cursus. Cette étude se propose d’identifier les variables qui prédisent les performances des étudiants, notamment en prenant en compte leur expérience professionnelle. Dans un dispositif hybride alternant un enseignement sous la forme de capsules vidéo, accompagnées d’évaluations formatives en ligne et des travaux dirigés en présentiel, des variables liées à la motivation et à la participation sont mesurées auprès de 143 étudiants. Les résultats montrent que les comportements observables et la motivation des étudiants influencent différemment leurs performances selon qu’ils ont ou non une expérience professionnelle préalable. Ces résultats sont décrits et discutés et pourraient constituer un point d’ancrage à une littérature scientifique combinant l’enseignement hybride et la formation en soins infirmiers. Blended learning in nursing education is currently hardly represented in the scientific literature (McCutcheon et al., 2015). However, the public of this education is unusual, especially since many of them already have a professional background in the healthcare environment. Using multiple regression models, this study aims to identify predictive variables of student performance according to their professional background. Variables related to motivation and participation were measured among 143 students in a blended course alternating videos, online formative assessments, and face-to-face supervised exercises. Results showed that participation and motivation influence their performance differently depending on whether they have prior professional experience. These results are described and discussed and could be an anchor for a scientific literature combining blended and nursing education.

The aim of the study was to analyze how occupational commitment and perceived job opportunities (organizational factors), career adaptability and Coping (individual factors) contribute to the prediction of burnout. This quantitative study had a sample of 246 nurses from three hospitals in the south region of Brazil. Data were submitted to stepwise backward multiple regressions using the software SPSS 22. Predictive analysis of all the independent variables with respect to the characterization burnout revealed an Raj (predictive factor) of 49.5%. The results confirm that the organizational factors trigger burnout. However, it is possible to also see the importance of individual factors that trigger burnout syndrome. There is a need to build an environment that minimizes the predictive variables of burnout. [ABSTRACT FROM AUTHOR]

Bien que les taux de survie des patients atteints de cancer aient augmenté, cette maladie peut affecter les couples de manière significative. La religion et la culture semblent faire partie du processus thérapeutique des personnes atteintes de cancer. Malgré l’importance de la communauté arabo-musulmane dans les pays occidentaux, peu de données sont disponibles quant à l’expérience de ces individus aux prises avec un cancer. Cette étude a pour but de comprendre l’expérience de couples arabo-musulmans aux prises avec un cancer. Des entrevues semi-structurées ont été menées auprès de couples mariés (n=6), dont un des membres est atteint ou a été traité d’un cancer. Une analyse des données itérative a été effectuée. Les résultats s’articulent sous les thèmes suivants: i) vivre la maladie dans un contexte d’immigration; ii) faire preuve d’acceptation face à la maladie grâce aux croyances et aux pratiques religieuses musulmanes; iii) vivre des enjeux relatifs à l’expression des besoins et des sentiments au sein du couple; iv) vivre un rapprochement au sein de la famille ; v) vivre la maladie dans le contexte hospitalier en tant que musulmans. Les résultats de cette étude pourraient sensibiliser les infirmières quant à l’importance d’explorer les croyances religieuses et spirituelles dans l’accompagnement des couples touchés par le cancer, en plus de demeurer à l’affût des besoins des deux conjoints. Par conséquent, les soins culturellement adaptés favoriseraient une expérience de santé empreinte d’humanisme., Although survival rates for cancer patients have increased, this disease continues to affect couples significantly. That said, religion and culture seem to be part of the therapeutic process for people with cancer. Despite the abundance of the Arab-Muslim community in Western countries, there is a lack of documented data on their experience with cancer. This study aims to understand the experience of Arab-Muslim couples facing cancer. Semi-structured interviews were conducted with married couples (n=6), identifying with the Arab-Muslim culture, and being affected by cancer. An iterative data analysis was performed. Results were reported under the following themes: i) experiencing illness in an immigration context; ii) accepting illness through Muslim religious beliefs and practices; iii) experiencing issues related to the expression of needs and feelings within the couple; iv) experiences of increased family bonding; v) experiencing illness in the hospital setting as Muslims. The results of this study could raise awareness within the nursing community about the importance of exploring religious and spiritual beliefs when supporting couples affected by cancer. Consequently, culturally appropriate care would promote a humanistic experience.

44. L’accompagnement infirmier personnalisé à l’expérience de jeunes hommes atteints d’un cancer

Au Canada, plus de 7 500 adolescents et jeunes adultes sont diagnostiqués d’un cancer chaque année. Les types de cancers les plus diagnostiqués chez les jeunes hommes sont la glande thyroïde, les testicules, le lymphome de Hodgkin, le mélanome et le lymphome non-Hodgkinien. Selon les études disponibles, cette clientèle semble moins encline à demander de l’aide des infirmières. De plus, plusieurs jeunes adultes, dont les hommes, peuvent être réticents à l’idée de communiquer leurs préoccupations aux infirmières. Ainsi, certains jeunes hommes pourraient ressentir que l’accompagnement infirmier n’est pas personnalisé à leur parcours de vie et à leurs préoccupations. Cette étude avait pour but de décrire et comprendre un accompagnement infirmier personnalisé à l’expérience de jeunes hommes atteints d’un cancer. Une étude qualitative descriptive interprétative inspirée par l’approche méthodologique de Thorne (2016) a été réalisée. Huit jeunes hommes atteints d’un cancer, âgés de 20 à 33 ans, ont participé à une entrevue individuelle. Une analyse qualitative itérative des données a été réalisée. Les résultats ont été divisés en deux thèmes. Le premier thème porte sur l’importance de l’établissement d’une relation de confiance entre l’infirmière et le jeune homme atteint d’un cancer et se décline en ces sous-thèmes : 1) accorder une présence humaine aux jeunes hommes atteints d’un cancer; 2) établir un partenariat de soins entre l’infirmière et le jeune homme atteint d’un cancer; 3) être accessible pour le jeune homme et 4) s'intéresser au ressenti du jeune homme atteint d'un cancer. Le deuxième thème discute des besoins qui demeurent à être comblés par l’accompagnement infirmier et comporte les sous-thèmes suivants : 1) besoin de recevoir des soins adaptés à leur réalité; 2) besoin de s’identifier à d’autres jeunes hommes atteints d’un cancer; 3) besoin d’informations ciblées et 4) besoin de se sentir dans l’action; 5) besoin de soutien psychologique adapté aux jeunes hommes. Les résultats proposent que les jeunes hommes atteints d’un cancer veuillent recevoir une présence humaine des infirmières et que celles-ci s’intéressent davantage à leur ressenti. Cette clientèle semble également avoir plusieurs besoins non comblés, dont le besoin de soutien psychologique. Finalement, cette étude a permis de décrire qu’un accompagnement infirmier personnalisé aux jeunes hommes atteints d’un cancer devrait inclure une présence humaine, l’aide à l’expression des sentiments, l’implication du jeune lors des soins et un soutien psychologique. Différentes recommandations sont émises pour optimiser l’accompagnement infirmier chez les jeunes hommes atteints d’un cancer, telles que l’élaboration de formations sur l’exploration des sentiments., In Canada, more than 7,500 teenagers and young adults are diagnosed with cancer each year. The most diagnosed types of cancers in young men are thyroid gland, testicular, Hodgkin's lymphoma, melanoma and non-Hodgkin's lymphoma. According to available studies, these patients seemed less inclined to seek help from nurses. Also, many young adults, including men, may be reluctant to share their concerns with nurses. Thus, some young men may feel that nursing accompanying is not personalized to their lifestyles and their concerns. The aim of this study was to describe and understand the nursing accompanying personalized to the experience of young men with cancer. An interpretative descriptive qualitative study inspired by Thorne’s (2016) methodological approach of was carried out. Eight young men with cancer, between the ages of 20 and 33, participated in an individual interview. An iterative qualitative analysis of the data was carried out. The results were divided into two themes. The first theme focuses on the importance of establishing a relationship of trust between the nurse and the young man with cancer and is divided into these sub-themes: 1) giving a human presence to young men with cancer; 2) establishing a care partnership between the nurse and the young man with cancer; 3) be accessible for the young man and 4) be interested in the feelings of the young man with cancer. The second theme discusses the needs that remain to be met by nursing support and includes the following sub-themes: 1) need to receive care adapted to their reality; 2) need to identify with other young men with cancer; 3) need for targeted information and 4) need to feel in the action; 5) need for adapted psychological support. The results suggest that young men with cancer want to feel a human presence from nurses, whom would be more interested in their feelings. These patients also seemed to have several unmet needs, including the need for psychological support. Finally, this study made it possible to describe that personalized nursing accompanying for young men with cancer should include a human presence, some help in the expression of feelings, the involvement of the young person during care, along with psychological support. Various recommendations are issued to optimize nursing accompanying for young men with cancer, such as the development of training to explore patients’ feelings.

45. Perceptions des infirmières en oncologie et des patients accompagnateurs quant à l’intégration de patients accompagnateurs dans les équipes cliniques et lors des soins

Au Canada, deux personnes sur cinq auront un diagnostic de cancer au cours de leur vie et cela aura un impact aux niveaux physiques, et psychosociales. Malgré l’accompagnement par des infirmières en oncologie (IO) ou des infirmières pivots en oncologie (IPO), les écrits scientifiques disponibles montrent que les patients ont toujours des besoins informationnels et psychosociaux insatisfaits. Pour répondre à ces besoins, le rôle de patient accompagnateur (PA) a été créé dans le cadre du projet de recherche québécois PAROLE-Onco. Des PA ont été introduits dans des équipes cliniques oncologiques et collaborent avec des IO dans le but d’améliorer l’expérience des patients. Bien que ce rôle soit novateur, peu d’études documentent cette intégration. Cette étude a pour but d’explorer les perceptions des PA et d’IO quant à l’intégration des PA dans les équipes cliniques et lors des soins de patients aux prises avec des problèmes de cancer. Une étude qualitative exploratoire a été réalisée à l’aide d’entrevues auprès de 12 participantes (six IO et six PA). Une analyse qualitative de contenu a permis d’analyser les données obtenues. Les résultats suggèrent que l’intégration des PA dans les équipes cliniques et lors des soins de patients atteints de cancer est variable et évolutive dans le temps. Les participantes ont constaté des effets bénéfiques sur les professionnels de la santé, les patients et les PA eux-mêmes. Les participantes ont également identifié plusieurs facteurs influençant l’intégration des PA dans les équipes cliniques tels que des facteurs liés à l’équipe clinique, liés à l’implantation du rôle, intrinsèques au PA et liés aux patients. Enfin, des participantes ont émis des recommandations en lien avec la dynamique de l’équipe clinique et la reconnaissance du rôle unique de PA afin d’améliorer leur intégration dans les équipes cliniques. Finalement, l’intégration des PA dans les équipes cliniques se fait de manière progressive et nécessite l’implication active des équipes cliniques. Plus particulièrement, les IO jouent un rôle essentiel dans cette intégration par leurs compétences de collaboration et de travail d’équipe., In Canada, two out of five people will be diagnosed with cancer in their lifetime, and this will impact them at the physical and psychosocial levels. Despite being supported by oncology nurses (ONs) or oncology nurse navigators (ONNs), the available scientific literature shows that patients still have unmet informational and psychosocial needs. To address these needs, the role of accompanying peers (AP) was created as part of the Quebec research project PAROLE-Onco. APs have been introduced into oncology clinical teams and work with ONs to improve the patient experience. Although this role is innovative, few studies document this integration. The purpose of this study was to explore the perceptions of APs and ONs regarding the integration of APs into clinical teams and in the care of patients with cancer. An exploratory qualitative study was conducted using interviews with 12 participants (six ONs and six APs). A qualitative content analysis was used to analyze the data obtained. Results suggest that the integration of APs into clinical teams and cancer care is variable and evolving over time. Participants found beneficial effects on health care professionals, patients, and APs themselves. Participants also identified several factors influencing the integration of APs into clinical teams, such as factors related to the clinical team, related to the implementation of the role, intrinsic to AP, and related to patients. Lastly, participants made recommendations related to clinical team dynamics and recognition of the unique role of APs to improve their integration into clinical teams. Finally, the integration of APs into clinical teams is a gradual process that requires the active involvement of clinical teams. Specifically, ONNs play a key role in this integration through their collaborative and teamwork skills.

International audience; Les instituts de formation en soins infirmiers ont à composer avec une injonction à l'hybridation de leur dispositif, impliquant une transformation des pratiques pédagogiques pour les formateurs. L'étude explore la manière dont les formateurs référents d'une même unité d'enseignement dans six instituts se sont saisis de ce nouvel environnement technopédagogique et interroge les conditions de leur développement professionnel. A partir d'entretiens semi-directifs, l'étude met en lumière un faible investissement de la plateforme au profit d'une survalorisation du présentiel, et des formateurs construisant un rapport distant à la distance. Ces résultats interrogent le soutien institutionnel à l'introduction de l'hybridation et révèle possiblement un enjeu identitaire pour ces formateurs dans le contexte de l'universitarisation de la formation infirmière.

48. The Second “Great Transformation”: Renegotiating Nursing Practice in Ontario, 1945–70.

In the period following the Second World War, hospitals in Ontario faced a shortage of nurses that prompted the renegotiation of nursing practice. This article examines the introduction of “certified” nursing assistants, a new category of worker who became the fastest-growing segment of the nursing labour force. Nursing assistants occupied an important position in the reorganization of nursing labour in these years, intentionally positioned between registered nurses and a pool of other support workers, such as practical nurses and ward aides. Nursing assistant training programs were established throughout Ontario and helped to create a credentialed community of practice. From 1946 to 1959, 4,840 nursing assistants were registered in Ontario. Between 1960 and 1966, more than 12,000 nursing assistants were registered, increasing to 17,723 in the final three years of the 1960s. The rapid growth of nursing assistants reveals the extent by which nursing labour in Ontario was reorganized in these years. Importantly, registered nurses were not mere bystanders in this transformation but, rather, effectively managed the critical issue of encroachment, at least for the short term. The system of registration created in Ontario effectively legitimated the participation of registered nurses in the regulation of nursing assistants. Registered nurses also participated in important ways in the governance of nursing assistants, in overseeing education programs, and in supervising the clinical work done by nursing assistants. Finally, the shift toward recognizing “registered nursing assistants” acknowledged that nursing labour was undergoing segmentation, with tasks being divided among different kinds of workers. Résumé Objectif La polyarthrite rhumatoïde (PR) est un facteur de risque cardiovasculaire aussi important que le diabète et sa prise charge est insuffisante. Le Heart Disease Fact Questionnaire-Rheumatoid Arthritis (HDFQ-RA 1&2), est un auto-questionnaire permettant l’évaluation des connaissances des patients sur le risque cardiovasculaire (RCV) en général et sur le risque cardiovasculaire de la PR et des traitements de celle-ci (AINS, corticothérapie).L’objectif de ce travail est de traduire et adapter le HDFQ-RA en français et d’évaluer ses propriétés psychométriques pour qu’il soit utilisé comme un outil d’évaluation par les infirmières dans l’éducation thérapeutique. Méthodes Le questionnaire a été traduit en français puis retraduit en anglais conformément aux « Guidelines for the process of cross-cultural adaptation of self-report measures ». Les propriétés psychométriques ont été évaluées dans un échantillon de 60 patients atteints de PR (procédure de test-retest ) entre juin et décembre 2013. L’acceptabilité, la cohérence interne (coefficient alpha de Cronbach), la reproductibilité test-retest ont été analysées. Résultats Une version culturellement acceptable pour les patients français a été obtenue. Le coefficient alpha de Cronbach est supérieur à la valeur recommandée de 0,6. La reproductibilité est correcte (coefficient Kappa > 0,56 [valeur recommandée = 0,4]). Les résultats ont montré que la connaissance du RCV était satisfaisante (taux de réponses correctes ≥ 60 %), mais les connaissances spécifiques du RCV associées à la PR sont restés faibles. Conclusion La version française du HDFQ-RA est valide pour évaluer les connaissances des patients sur le RCV en général et le RCV associée à la PR et de ses traitements. [ABSTRACT FROM AUTHOR]
