Resumo Objetivo Avaliar a correlação do conceito de glenoid track e da perda óssea da cavidade glenoidal com a taxa de recidiva de luxação e pontuação na escala de Rowe. Métodos Estudo retrospectivo que aferiu o glenoid track e a perda óssea da cavidade glenoidal por ressonância magnética pré-operatória. Foram incluídos pacientes submetidos a reparo artroscópico primário de Bankart anterior. Não foram incluídos pacientes com perda óssea da cavidade glenoidal maior que 21%, rotura do manguito rotador, fratura de cintura escapular, instabilidade posterior ou multidirecional. A pontuação pela escala de Rowe foi o desfecho primário, e a taxa de recidiva foi o desfecho secundário. Resultados Cento e dois pacientes foram incluídos. A recidiva foi relatada por 8 pacientes (7,8%). Quatro pacientes (50%) do grupo com recidiva apresentaram lesão da cavidade glenoidal maior que 13,5% contra 24 (25,5%) do grupo sem recidiva (p = 0,210), com valor preditivo negativo de 94,6%. Três pacientes (37,5%) do grupo com recidiva foram considerados off-track, contra 13 (13,8%) do grupo sem recidiva (p = 0,109), com valor preditivo negativo de 94,2%. Pacientes com valor absoluto do glenoid track menor ou igual a 1,5 mm tiveram piores resultados em relação ao grupo com recidiva, sendo que 6 pacientes (75%) apresentaram recidiva (p = 0,003). Conclusão Lesão off-track e perda óssea da cavidade glenoidal maior que a subcrítica não apresentam relação significativa com a taxa de recidiva e a pontuação de Rowe, apesar do alto valor preditivo negativo. O corte do valor absoluto do glenoid track em 1,5 mm apresentou relação significativa com a taxa de recidiva.

Resumo Objetivo O objetivo do presente estudo foi avaliar a eficácia e a segurança da reconstrução capsular superior (RCS) com a utilização do aloenxerto de fáscia lata. Métodos Uma série de casos prospectivos de 15 pacientes com ruptura irreparável do supraespinhal foi submetida a RCS com aloenxerto de fáscia lata, sendo adotada como desfecho primário a escala American Shoulder and Elbow Surgeons (ASES, na sigla em inglês) aos 12 meses do pós-operatório. Como desfechos secundários, foram adotadas as escalas da University of California Los Angeles (UCLA, na sigla em inglês), Constant-Murley, e Single Assessment Numeric Evaluation (SANE, na sigla em inglês), além da amplitude de movimento. Os parâmetros radiológicos também foram avaliados por radiografias simples e ressonância magnética (RM). Resultados Quinze pacientes completaram 12 meses de acompanhamento pós-operatório. O escore ASES aumentou de 34,0 para 73,0 (p = 0,005). As escalas UCLA, Constant-Murley e SANE também apresentaram diferenças estatisticamente significativas (p = 0,001; p = 0,005; e p = 0,046). Na avaliação da amplitude de movimento, houve melhora na elevação e rotação externa (95 a 140°, p = 0,003; 30 a 60°, p = 0,007). Seis pacientes (40%) tiveram cicatrização completa do enxerto. Os desfechos clínicos foram significativamente maiores nos pacientes que apresentaram cicatrização do enxerto. Conclusões A RCS com aloenxerto de fáscia lata é um procedimento seguro e eficaz com um curto acompanhamento de tempo. Nível de evidência IV; Estudo Terapêutico; Série de casos.

The subscapularis is biomechanically important for the shoulder. However, few studies have clinically assessed its importance using a comparative design. Our objective was to compare the functional outcomes in patients who underwent isolated repair of posterosuperior rotator cuff tears and those with repair of combined tears involving the subscapularis.We performed a retrospective cohort study evaluating patients who underwent arthroscopic full-thickness rotator cuff repair between January 2013 and May 2017. The patients were divided into 2 groups: isolated repair of posterosuperior tears and repair of combined tears involving the subscapularis. The primary outcome was to evaluate the American Shoulder and Elbow Surgeons (ASES) and University of California, Los Angeles (UCLA) scales at 24 months' follow-up.A total of 326 patients were evaluated: 194 with isolated posterosuperior repairs and 132 with combined subscapularis repairs. Both groups showed significant improvement with the procedure (P.001). The ASES score at 24 months showed no significant difference (P = .426) between the group without subscapularis repair (median, 90.0; interquartile range [IQR], 24.8) and the group with subscapularis repair (median, 86.3; IQR, 33.2). Similarly, the UCLA score showed no difference between the groups (median, 33.0 [IQR, 6.0] and 32.5 [IQR, 8.8], respectively; P = .190). The preoperative functional evaluation also showed no significant differences between the groups.The functional results did not differ between patients who underwent isolated repair of posterosuperior tears and those with repair of combined tears involving the subscapularis, according to the ASES and UCLA scales at 24 months.

Resumo Objetivos Descrever o resultado funcional do tratamento não operatório de fraturas desviadas da extremidade proximal do úmero (FEPU) pela escala da American Shoulder and Elbow Surgeons (ASES, na sigla em inglês) após 12 meses e avaliar se as diferentes classificações e medidas radiográficas iniciais têm correlação com os resultados clínicos. Métodos Foram avaliados em tempos padronizados (3, 6 e 12 meses), 40 pacientes > 60 anos com FEPU submetidos ao tratamento não operatório. Foram utilizadas as escalas da ASES, Constant-Murley e Single Assessment Numeric Evaluation (SANE, na sigla em inglês). As variáveis radiográficas incluíram as classificações de Neer e Resch, a presença de fratura e desvio dos tubérculos, cominuição metafisária, lesão periosteal medial, desvios angulares e translacionais da cabeça no plano coronal e sagital e desvio dos tubérculos. Resultados Observamos resultados pela escala de ASES de 77,7 ± 23,2 para toda a amostra, pela de Constant-Murley de 68,7 ± 16 e de 82,6% para a escala em relação ao lado contralateral. A escala de SANE aos 12 meses foi de 84,8 ± 19. Os critérios radiográficos que apresentaram influência negativa no resultado clínico pela escala de ASES aos 12 meses foram a gravidade pela classificação de Neer e pelo desvio angular no plano coronal (mensurado pelo ângulo cabeça-diáfise) e a presença de fratura dos tubérculos. Conclusão O tratamento não operatório de fraturas desviadas da extremidade proximal do úmero em pacientes idosos resulta em bons resultados clínicos. Os resultados clínicos são influenciados negativamente pelo desvio angular da cabeça do úmero e pela presença de fratura dos tubérculos maior e menor, assim como pela classificação de Neer.

Resumo Objetivo Avaliar o resultado funcional de pacientes submetidos ao reparo parcial por via artroscópica de roturas extensas do manguito rotador. Métodos Série de casos retrospectiva, avaliando pacientes com roturas extensas do manguito rotador submetidos ao reparo parcial por via artroscópica. O desfecho primário foi a escala American Shoulder and Elbow Surgeons Standardized Shoulder Assessment Form (ASES, na sigla em inglês) aos 24 meses. Foram desfechos secundários a escala Modified-University of California at Los Angeles Shoulder Rating Scale (UCLA, na sigla em inglês), e seus subdomínios satisfação, flexão anterior ativa e força de flexão anterior ativa. Resultados Avaliamos 33 pacientes. A escala da ASES evoluiu de 39,7 ± 19,6 para 77,6 ± 17,4 (p 150° passou de 12 (36,4%) para 25 (75,8%) (p = 0,002). O número de pacientes com força de flexão anterior ativa normal ou boa passou de 9 (27,3%) para 22 (66,7%) (p = 0,015). Conclusão O reparo parcial nas roturas irreparáveis do manguito rotador leva a melhora significativa de acordo com as escalas da ASES e UCLA.

Background/Hypothesis The Latarjet procedure changes the anatomy of the shoulder, moving the coracoid process distally through an incision in the subscapularis muscle. Some authors have studied the effect of this surgery on shoulder rotator strength. Our hypothesis is that the Latarjet procedure decreases elbow supination and flexion strength. Methods A retrospective case series, evaluating patients submitted to the Latarjet procedure between May 2013 and June 2017. Movements of the elbow (supination, pronation, flexion and extension) and shoulder (internal and external rotation) were evaluated bilaterally using a Biodex System 3 isokinetic dynamometer (Biodex Medical Systems, New York) in concentric/concentric mode. Results We evaluated 20 patients with an average follow-up of 36 months. In the elbow, we observed a 9.1% decrease in supination strength at a speed of 60°/s (p = 0.044), without statistical difference at 120°/s (p = 0.570). In the shoulder, there was a 13.5% decrease in external rotation strength at 60°/s and 4.5% at 180°/s (p = 0.009 and p = 0.040, respectively). The other movements did not demonstrate any statistically significant differences. Conclusion After the Latarjet procedure, the supination strength at 60°/s was decreased, as was the external rotation strength of the shoulder at 60°/s and 180°/s. We did not observe any reduction in strength for shoulder flexion or internal shoulder rotation. Level of evidence IV, case series.

ResumoObjetivo Avaliar as prevalências de antecedente familiar de rotura do manguito e de tendinopatia em outras articulações em pacientes com rotura do manguito rotador e compará-las com controles pareados. Estimar a razão de chances de uma rotura do manguito rotador para estes dois fatores de risco.Métodos Realizamos um estudo de caso-controle comparando pacientes submetidos ao tratamento para rotura do manguito rotador com controles assintomáticos. Todos os casos e controles foram avaliados por exames de imagem e pareados por idade (±2 anos) e sexo. Realizamos uma entrevista utilizando um questionário padronizado, e coletamos dados referentes a vários fatores de risco.Resultados Avaliamos 144 pacientes, 72 por grupo. Os pacientes com rotura do manguito rotador relataram, em maior número, a presença de familiares consanguíneos que realizaram tratamento para a mesma doença e de lesões tendíneas em outras articulações em relação aos indivíduos controles (p = 0,005 e p = 0,045, respectivamente). Indivíduos com antecedente familiar de tratamento para rotura do manguito rotador ou com tendinopatias em outras articulações tiveram maior probabilidade de apresentar rotura do manguito rotador, com razões de chances de 3,3 (intervalo de confiança de 95% [IC95%] = 1,4–7,7) e 2,7 (IC95% = 1,1–6,9), respectivamente.Conclusões Os pacientes com rotura do manguito rotador têm maior prevalência de familiares com a mesma doença e de tendinopatias ou lesões tendíneas em outras articulações. A presença de familiares consanguíneos com tratamento para rotura do manguito rotador e tendinopatias em outras articulações são fatores de risco para presença de roturas do manguito rotador.

Resumo Objetivo Demonstrar os resultados clínicos e a taxa de complicações da liberação cirúrgica por via única posterior no tratamento da rigidez pós-traumática de cotovelo. Métodos Estudo prospectivo, com pacientes submetidos a cirurgia entre maio de 2013 e junho de 2018 em um único centro. Foi realizado acesso ao cotovelo por via posterior. O seguimento dos pacientes foi feito por uma equipe de terapia ocupacional, e eles foram submetidos a um protocolo de reabilitação padronizado, com órteses estáticas progressivas e dinâmicas. O desfecho primário foi a amplitude de flexoextensão do cotovelo após 6 meses. Resultados Um total de 26 pacientes completaram o seguimento mínimo de 6 meses. A média de flexoextensão do cotovelo, ao final de 6 meses, foi de 98,3° ± 22,0°, com um ganho de amplitude de 40,0° ± 14,0° em relação ao pré-operatório (p Conclusões A liberação cirúrgica do cotovelo associada a protocolo de reabilitação é técnica segura, com resultados satisfatórios e baixa taxa de complicações.

Resumo Objetivo Avaliar a influência do padrão da rotura do supraespinal nas avaliações funcionais pré e pós-operatória. Métodos Estudo de coorte retrospectivo, comparando pacientes com rotura do supraespinal em crescente versus em L ou U. Incluímos pacientes submetidos ao reparo artroscópico completo do supraespinal. Não incluímos pacientes com reparo dos tendões do subescapular ou infraespinal, aqueles submetidos a cirurgia aberta, ou aqueles nos quais foi obtido apenas o reparo parcial. As escalas clínicas utilizadas foram The American Shoulder and Elbow Surgeons Standardized Shoulder Assessment (ASES) e Modified-University of California at Los Angeles Shoulder Rating Scale (UCLA), aplicadas uma semana antes e 24 meses após o procedimento. Resultados Analisamos 167 ombros (de 163 pacientes). No pré-operatório, a escala da ASES demonstrou ser significativamente superior no padrão em crescente (43,5 ± 17,6 versus 37,7 ± 13,8; p = 0,034). A escala da UCLA teve o mesmo padrão (15,2 ± 4,6 versus 13,5 ± 3,6; p = 0,028). No pós-operatório, entretanto, não ocorreu diferença significativa. De acordo com a escala da ASES, roturas em crescente tiveram 83,7 ± 18,7 pontos, e as roturas em L ou U, 82,9 ± 20,1 (p = 0,887). Respectivamente, os valores foram de 30,9 ± 4,9 e 30,5 ± 5,6 (p = 0,773) pela escala da UCLA. Conclusão As roturas em crescente e em L ou U do supraespinal apresentam resultados funcionais pós-operatórios semelhantes. No pré-operatório, os resultados funcionais são superiores nas roturas em crescente.

ResumoUma revisão que envolveu as seis principais revistas ortopédicas internacionais foi publicada recentemente. A publicação descreveu as ferramentas usadas para a avaliação dos desfechos no tratamento cirúrgico da luxação anterior recidivante do ombro. Não existe um levantamento que mostre as principais formas de avaliação para essa doença no Brasil. Os autores avaliaram os desfechos usados nos estudos clínicos que envolveram a instabilidade glenoumeral anterior e que foram publicados na última década nos dois principais periódicos ortopédicos brasileiros, Revista Brasileira de Ortopedia e Acta Ortopédica Brasileira. Foi feita uma revisão da literatura na qual foram incluídos todos os artigos clínicos publicados entre 2007 e 2016 que descreveram ao menos uma medida de desfecho antes e após intervenção cirúrgica. Os desfechos avaliados foram amplitude de movimento, força muscular, testes de exame físico, satisfação, retorno ao esporte, exames de imagem, complicações e escalas clínicas. Foram publicados 12 estudos que avaliaram os resultados clínicos do tratamento cirúrgico da instabilidade anterior do ombro. Dez estudos (83%) eram séries de caso (nível de evidência IV), 1 (8%) caso-controle (III), e 1 coorte retrospectiva (III). A média de desfechos avaliados por estudo foi de 3,7 ± 1,7. A escala Rowe foi usada em 9 estudos (75%), e 7 publicações (58%) usaram a escala da University of California Los Angeles (UCLA). Dez estudos (83%) relataram as complicações relacionadas ao tratamento cirúrgico. A complicação mais frequentemente relatada foi a recidiva da luxação glenoumeral, encontrada em 9 estudos (75%). Os estudos nacionais usaram preferencialmente escalas consideradas de baixa conficabilidade, responsividade, e consistência interna.

ResumoA reconstrução da cápsula superior é um procedimento descrito recentemente para o tratamento das roturas irreparáveis do tendão do músculo supraespinal. Como opções de enxerto podemos citar o uso de fáscia lata autógena e enxerto dermal acelularizado. Nenhum estudo foi publicado com o uso de aloenxerto de fáscia lata. O objetivo desta nota é descrever a técnica cirúrgica da reconstrução da cápsula superior com aloenxerto de fáscia lata. O procedimento é feito por visão artroscópica, com o paciente posicionado em decúbito lateral. Os autores descrevem uma técnica baseada no uso do nó em dupla polia na glenoide e no tubérculo maior, que facilita o procedimento e permite que o enxerto seja levado para o espaço subacromial na posição definitiva e com a tensão adequada. Os aloenxertos usados são provenientes de banco de tecidos, onde são criopreservados e submetidos à avaliação microbiológica e histopatológica. A reconstrução da cápsula superior é uma cirurgia promissora. A técnica descrita mostra uma opção artroscópica viável, com uso de menor número de âncoras quando comparada com as demais descrições.

Background/Hypothesis Minimal clinically important difference (MCID) is a vital tool in the analysis of clinical results. It allows the determination of clinical relevance of statistical data. Our hypothesis was that specific differences between preoperative and postoperative scores would be able to accurately predict patient perception of improvement and satisfaction as reflected by anchor and distribution-based questions. Methods Retrospective cohort with patients that underwent rotator cuff repair. We evaluated the University of California at Los Angeles Shoulder Rating Scale (UCLA) and the American Shoulder and Elbow Surgeons Assessment Form (ASES) before and 12-months after surgery. Anchor-based, distribution-based and minimum detectable change (MDC) approaches were utilized. Results We evaluated 289 shoulders. The MCID for the UCLA scale was 4.5 points using the anchor method, 2.5 by the distribution method and 3.6 by MDC. Patients with a baseline score > 20 presented a lower MCID (1.5, 1.1 and 1.7, respectively). For the ASES score, the MCID was 6.1 by the anchor method, 10.5 based on the distribution method and 26.3 by MDC. In the group of patients above the 60 point cutoff, the obtained values were 2.4, 4.9 and 13.6, respectively. Conclusion The mean MCID value for the UCLA shoulder score is 3.5 points, ranging from 2.5 points (distribution method) to 4.5 points (anchor method). The mean MCID value for the ASES score was 15.2 points, ranging from 6.1 (anchor method) to 26.3 (MDC). Patients groups presenting with higher preoperative scores showed lower MCID values. This fact needs to be considered in postoperative comparisons between treatment groups. Level of evidence Basic Science Study, Validation of Outcomes Instruments/Classification Systems.

Resumo Objetivo Descrever as caracteristicas clinicas e epidemiologicas de pacientes com artrite septica do ombro ou cotovelo e buscar fatores prognosticos para complicacoes durante o tratamento. Metodos Foi feita uma serie de casos retrospectiva com pacientes atendidos entre 2004 e 2014. As caracteristicas clinicas e epidemiologicas dos pacientes foram coletadas. As complicacoes clinicas e ortopedicas foram identificadas e possiveis fatores prognosticos foram avaliados. Resultados O estudo avaliou 27 pacientes, 17 com pioartrite no ombro e dez no cotovelo. A mediana da idade foi de 46 anos (IIQ 24,5; 61). Doenca articular previa foi observada em nove pacientes (33%). Uma ou mais comorbidades clinicas foram identificadas em 23 pacientes (85%). Staphylococcus aureus foi isolado em 14 casos (52%). Quatorze pacientes (52%) tiveram pelo menos uma complicacao clinica e cinco pacientes foram a obito (19%). Nove pacientes (33%) tiveram alguma complicacao ortopedica. O tempo entre o inicio dos sintomas e o tratamento cirurgico foi maior nos pacientes com complicacoes ortopedicas (p = 0,020). Em relacao ao desenvolvimento de complicacoes clinicas, leucocitose na admissao hospitalar (p = 0,021) e presenca de comorbidades clinicas (p = 0,041) foram fatores preditivos. Conclusoes A pioartrite do ombro e cotovelo acomete preferencialmente individuos com comorbidades clinicas e/ou imunocomprometidos. O Staphylococcus aureus e o patogeno mais frequente no Brasil. Leucocitose na admissao hospitalar e a presenca de comorbidades clinicas sao fatores associados a presenca de complicacoes clinicas. Maior tempo entre o inicio dos sintomas e o tratamento cirurgico foi correlacionado a complicacoes ortopedicas.

Resumo Objetivo Avaliar se a etnia asiatica e um fator de risco no desenvolvimento da capsulite adesiva. Os objetivos secundarios foram descrever a prevalencia da capsulite adesiva no ambulatorio especializado em ombro e a dispersao dos casos de capsulite por faixa etaria. Metodos Estudo transversal que comparou a taxa de capsulite adesiva na etnia asiatica (casos) com outras etnias (controles). Excluimos pacientes com fraturas e sintomatologia que nao envolviam o ombro. O risco relativo foi exposto em razao de chance, ajustado para fatores confundidores por uma regressao logistica binaria. Resultados Foram avaliados os prontuarios de 1.331 pacientes. Apos aplicacao dos criterios de selecao, restaram 814. Observamos 134 casos de capsulite adesiva (15,6%). O pico de incidencia foi aos 60‐64 anos na etnia asiatica e 55‐59 anos nas demais. A razao de chance nao ajustada foi de 4,2 (IC 95%, 2,4 a 7,4), enquanto a ajustada para sexo e diabetes mellitus foi de 3,6 (IC 95%, 2,0 a 6,5). Conclusao A etnia asiatica se mostrou um fator de risco independente para o desenvolvimento da capsulite adesiva, com uma razao de chance ajustada de 3,6. O diagnostico de capsulite adesiva esteve presente em 15,6% da amostra, com pico entre 55 e 64 anos.

To evaluate the correlation between the UCLA and Constant-Murley scores in the surgical treatment of rotator cuff tears and proximal humeral fractures (PHF).Retrospective study evaluating patients submitted to arthroscopic rotator cuff repair and surgical treatment of PHF with 2-year follow-up. Patients were evaluated by the UCLA and Constant-Murley scores in the preoperative period for the rotator cuff repairs, and 3, 6, 12 and 24 months after surgery for both diagnoses. Pearson's correlation coefficient (We evaluated 109 patients: 54 with rotator cuff tear and 55 with PHF. Twenty-four months after surgical treatment, the scores according to the UCLA and Constant-Murley scores were 32.6 ± 4.0 and 85.0 ± 12.0 for the rotator cuff tears and 30.3 ± 5.3 and 73.8 ± 13.9 for the PHF, demonstrating significant improvements in both, in relation to the initial evaluation (The UCLA and Constant-Murley scores presented high or very high correlation in the evaluation of surgical treatment of rotator cuff tears and PHF. The correlation in the preoperative evaluation was high.Avaliar a correlação entre as escalas da UCLA e de Constant-Murley no tratamento cirúrgico de roturas do manguito rotador e de fraturas da extremidade proximal do úmero (FEPU).Estudo retrospectivo, que avaliou pacientes submetidos ao reparo do manguito rotador por via artroscópica e tratamento cirúrgico de FEPU com dois anos de seguimento. Os pacientes foram avaliados pelas escalas da UCLA e de Constant-Murley no período pré-operatório nas roturas do manguito rotador e após seis, 12 e 24 meses da cirurgia em ambos os diagnósticos. O coeficiente de correlação de Pearson (Avaliamos 109 pacientes, 54 com rotura do manguito rotador e 55 com FEPU. Após 24 meses do tratamento cirúrgico, as pontuações pelas escalas da UCLA e da Constant-Murley foram de 32,6 ± 4,0 e 85,0 ± 12,0 nas roturas do manguito rotador e 30,3 ± 5,3 e 73,8 ± 13,9 nas FEPU, com melhoria significativa em ambas em relação à avaliação inicial (p0,001). As escalas demostraram alta correlação (r = 0,88, p0,001). Em todas as avaliações clínicas pós-operatórias as pontuações obtidas nas duas escalas se correlacionaram de modo alto ou muito alto (r = 0,79 a 0,91, p0,001). No pré-operatório a correlação foi alta (r = 0,73, p0,001).As escalas da UCLA e de Constant-Murley apresentam uma correlação alta ou muito alta na avaliação do tratamento cirúrgico das roturas do manguito rotador e das FEPU. No pré-operatório a correlação é alta.

To compare quality of life (according to the SF-12) in patients with rotator cuff arthropathy with controls paired by sex and age. Secondary objectives are to compare the groups according to the ASES and VAS scales.This cross-sectional study with controls paired by sex and age compared patients with rotator cuff arthropathy with surgical indication for reverse shoulder arthroplasty. The groups were compared according to the SF-12, ASES, and VAS scales.The groups consisted of 38 individuals, 28 women. The SF-12 demonstrated a significant difference in the physical component, with the cases scoring 31.61 ± 6.15 and the controls 49.39 ± 6.37 (p0.001). For the mental component, the difference was not significant, with the cases scoring 44.82 ± 13.18 and the controls 48.96 ± 8.65 (p=0.109). The cases scored 7.34 ± 2.11 on the VAS and 31.26 ± 15.12 on the ASES, while the controls scored 0.55 ± 1.31 and 97.53 ± 6.22, respectively (p0.001).Patients with rotator cuff arthropathy had poorer results for the physical component of the SF-12 than the controls. They also had poorer functional results according to the ASES scale, and more pain according to the VAS.Comparar a qualidade de vida, de acordo com o SF-12, entre pacientes com artropatia do manguito rotador e controles pareados por sexo e idade. É objetivo secundário a comparação dos grupos de acordo com as escalas ASES e EVA.Estudo transversal com controles pareados por sexo e idade, que comparou pacientes com artropatia do manguito rotador e indicação de artroplastia reversa do ombro com indivíduos sadios. Os grupos foram comparados quanto às escalas SF-12, ASES e EVA.Os grupos foram formados por 38 indivíduos, sendo 28 do sexo feminino. O SF-12 apresentou diferença significativa no componente físico, tendo os casos registrado 31,61 ± 6,15 e os controles 49,39 ± 6,37 (p0,001). Para o componente mental, a diferença não foi significativa, tendo os casos apresentado 44,82 ± 13,18 e os controles 48,96 ± 8,65 (p=0.109). Os casos apresentaram EVA de 7,34 ± 2,11 e ASES de 31,26 ± 15,12, enquanto os controles apresentaram 0,55 ± 1,31 e 97,53 ± 6,22, respectivamente (p0,001).Os pacientes com artropatia do manguito rotador apresentam piores resultados no componente físico do SF-12 quando comparados aos controles. Têm, ainda, piores resultados funcionais pela escala da ASES e mais dor pela EVA.

Background Two recent experimental studies evaluated the influence of shoulder rotation on neck-shaft angle, with conflicting results. However, there have been no clinical studies of whether this angle varies in different shoulder positions. The present study aimed to determine whether shoulder rotation affects neck-shaft angle on standard radiographs in patients with complaints of shoulder pain. Hypothesis Shoulder rotation does not affect neck-shaft angle. Materials and methods A prospective study was conducted in selected patients with shoulder pain. Three true anterior-posterior radiographic views were obtained: in neutral rotation, 30° external rotation, and internal rotation with patient's arm in a sling. The X-rays were evaluated by three shoulder and elbow surgeons. Inter- and intra-observer reliability was evaluated by intraclass correlation coefficient (ICC). Results Neck-shaft angle on true AP view did not differ between neutral rotation and 30° external rotation: 132 ± 6° and 130 ± 9°, respectively (P > 0.999). In internal rotation with the hand resting on the abdomen, neck-shaft angle was 145 ± 6°: i.e., significantly different (P Conclusions Radiographic neck-shaft angle was significantly different in internal rotation with the patient's arm in a sling, compared with views in neutral or 30° external rotation. Resumo Objetivo Avaliar a integridade do tendao do supraespinal por meio da ultrassonografia (US) em pacientes submetidos a fixacao de fraturas da extremidade proximal do umero (FEPU) com haste intramedular bloqueada. Metodos Foram avaliados por exame de ultrassonografia aos seis meses de pos‐operatorio e clinicamente aos seis e 12 meses de pos‐operatorio 31 pacientes com FEPU entre 50 e 85 anos, tratados com haste intramedular bloqueada inclinada. O objetivo primario foi avaliar a integridade do tendao supraespinal e os secundarios incluiam descrever os demais achados da ultrassonografia, as escalas de Constant‐Murley, EVA e Dash e a taxa de complicacoes e comparar os resultados clinicos dos pacientes com e sem rotura do manguito rotador. Resultados Roturas transfixantes do manguito rotador foram observadas em quatro pacientes (13%), com rotura do supraespinal em tres casos (10%) e do subescapular em um caso (3%). Roturas parciais foram diagnosticadas em dez casos (32%). Os resultados pela escala de Constant‐Murley aos 12 meses foram de 71,3 ± 15,2 pontos para toda a amostra, de 73,2 ± 16,1 pontos para os pacientes sem rotura do manguito rotador e de 68,7 ± 14,1 pontos para aqueles com rotura parcial ou completa (p = 0,336). Complicacoes, exclusive a rotura do manguito rotador, foram observadas em nove pacientes (29%). Conclusao Observou‐se uma alta taxa de alteracoes nos tendoes do manguito rotador, com roturas parciais em 32% dos casos e transfixantes em 13%. No entanto, os resultados clinicos sao satisfatorios, nao influenciados pela presenca de rotura do manguito rotador.

Objective: To evaluate supraspinatus tendon integrity with ultrasound (US) in patients submitted to proximal humeral fracture (PHF) fixation with a locking intramedullary nail., Methods: Thirty-one patients with PHF treated with curvilinear locking intramedullary nail, aged between 50 and 85 years, were assessed by US at six months postoperatively and clinically at six and 12 months postoperatively. The primary aim was supraspinatus tendon integrity, evaluated by US at six months postoperatively. Secondary aims included the Constant-Murley, DASH score, and visual analog pain scores, as well as complications and reoperation rates. Results: Full-thickness rotator cuff ruptures were observed in four patients (13%), supraspinatus ruptures in three cases (10%), and subscapularis ruptures in one case (3%). Partial ruptures were diagnosed in 10 cases (32%). The results using the Constant-Murley score at 12 months were 71.3 ± 15.2 points for the entire series, with 73.2 ± 16.1 points for patients without rotator cuff ruptures and 68.7 ± 14.1 points for those with partial or complete ruptures, without a statistically significant difference (p= 0.336). Complications, exclusively for rotator cuff ruptures, were observed in nine patients (29%). Conclusion: A high rate of rotator cuff ruptures was demonstrated, with partial ruptures in 32% of cases and full-thickness ruptures in 13%. However, clinical results are satisfactory, and are not influenced by the presence of rotator cuff ruptures. Studies suggest that the collagen degeneration and disordered arrangement of collagen fibers in rotator cuff tears are associated with an increase in activity of matrix metalloproteases 1 and 3 (MMP-1 and MMP-3), and that MMP activity may be in part genetically mediated. The degree to which this might be clinically relevant in patients with rotator cuff tears has not been well characterized. (1) Is genetic polymorphism of MMP-1 and MMP-3 associated with rotator cuff tears? (2) Are there haplotypes of MMP-1 and MMP-3 correlated with rotator cuff tears? (3) Compared with control subjects, do patients with rotator cuff tears have a higher proportion of relatives with the same disease? We evaluated 64 patients with full-thickness rotator cuff tears and 64 asymptomatic control subjects. Patients younger 65 years, with nontraumatic tears, were included. There are more than 40 outcome scores for evaluating shoulder pain and function. Some studies have correlated the results obtained using different scales, but none has compared the results obtained by the University of California, Los Angeles (UCLA) and American Shoulder and Elbow Surgeons (ASES) scores.We performed a retrospective study to evaluate patients who underwent arthroscopic rotator cuff repair with 2 years' follow-up. The patients were evaluated by the UCLA and ASES scores preoperatively and at 6, 12, and 24 months after surgery. The Pearson correlation coefficient (r) was calculated to measure the degree of correlation between the 2 outcome scores.We evaluated 143 patients. At 24 months postoperatively, the UCLA and ASES scores were 30.4 ± 5.8 and 81.2 ± 20.8, respectively (P .001). The UCLA and ASES scores showed a very high correlation (r = 0.91, P .001). In all the postoperative clinical evaluations, the scores obtained from the 2 scales were highly or very highly correlated (r = 0.87-0.92, P .001). For the preoperative scores, the correlation was moderate (r = 0.67, P .001).The UCLA and ASES scores presented a very high correlation in the evaluation of surgical treatment of rotator cuff tear. In the preoperative period, the correlation was moderate.

Background There are no previous randomized trials comparing surgical to conservative treatment for post-traumatic elbow stiffness. The aim of our study was to compare elbow range of motion (ROM) and clinical outcomes among patients undergoing surgical treatment or a standardized rehabilitation for post-traumatic elbow stiffness. Methods Randomized clinical trial of patients with post-traumatic elbow stiffness for more than 6 months who failed conventional physical therapy for 4 months. Patients were randomized into 2 treatment groups. The conservative group underwent the rehabilitation protocol associated with the use of orthoses (static progressive for extension and dynamic for flexion) and continuous passive motion. The surgical group underwent surgical release by a posterior approach without triceps detachment, followed by a rehabilitation protocol similar to the conservative group. The primary outcome of the study was flexion-extension ROM at 6 months of follow-up. Secondary outcomes included the visual analog scale for pain, the Mayo Elbow Performance Score, the Disabilities of the Arm, Shoulder, and Hand score, absolute and relative increase in flexion-extension ROM, and complication rates. Results Thirty patients were analyzed in the study, 15 in each group. The mean elbow flexion-extension ROM at the end of 6 months of follow-up was 108° in the surgical group and 88° in the conservative group (P = .002). The mean absolute and the relative increase of elbow flexion-extension at 6 months were, respectively, 17° and 27% in the conservative group and 41° and 59% in the surgical group (P Conclusion Surgical elbow release associated with the rehabilitation protocol resulted in a greater flexion-extension ROM, as well as a greater absolute and relative increase compared with rehabilitation alone at 6 months of follow-up. The groups did not differ regarding clinical scores and complication rates.

Background The limits of the glenoid track have been defined through methods that do not take properly into account the physiological articular forces involved in the articular contact, which may interfere with its size. Finite elements numerical models can simulate joint forces more realistically. Objective To evaluate the glenoid track in a finite element numerical model of the shoulder. Methods We developed a finite element numerical model of the shoulder, based on imaging exams of a volunteer, including the proximal humerus, scapula, their respective articular cartilages, and the rotator cuff muscles. An algorithm to balance the weight of the arm calculated muscle, wrapping, and articular reaction forces. The model has freedom of translation in three axes. The articular contact characteristics and glenoid track's dimensions according to the literature references were evaluated in 60°, 90° and 120° of abduction, all at the 90° external rotation. Results The model's anatomy and physiology were validated. The value of the glenoid track (according to Yamamoto's parameters) was 86% of glenoid length at 90° abduction before loading of forces, and 79% afterwards. The glenoid track at 60°, 90° and 120° of abduction (Omori's parameters) corresponded, respectively, to 71%, 88% and 104% of glenoid length before loading of forces, and 76%, 84% and 103% afterward. Conclusion The numerical model is suitable for the shoulder articular contact evaluation. The articular contact analysis ratifies the glenoid track concept and contributes to its evolution. Objective: To evaluate the influence of partial- and full-thickness upper third subscapularis tendon tears on the functional scores of patients undergoing arthroscopic rotator cuff repair. Methods: Patients who underwent arthroscopic rotator cuff repair were divided into three groups according to the subscapularis tendon condition: intact, partial-thickness tear, or full-thickness upper third tear. Functional scores were compared among groups. Second, the influence of biceps and infraspinatus tears on the scores was tested using multivariate regression analysis. Results: We evaluated 307 shoulders in 297 patients. Full-thickness upper third subscapularis tears presented significantly worse scores than intact tendons. Partial-thickness tears had scores that did not differ significantly from those of the other groups. Patients with full-thickness upper third tears presented a greater rate of injured and unstable biceps tendons. The multivariate analysis showed that biceps and infraspinatus tendon tears did not influence the scores or the intergroup comparison. Conclusion: Full-thickness upper third subscapularis tendon tears presented worse functional scores than intact subscapularis tendons among patients undergoing posterosuperior rotator cuff repair. Patients with full-thickness subscapularis tears were more likely to suffer biceps tears, but this fact did not influence functional scores. Métodos: Divisão em três grupos, conforme condição do tendão subescapular: intacto; ruptura de espessura parcial ou ruptura do terço superior de espessura total. Comparamos escores funcionais. Em seguida, a influência das lesões do bíceps e do infraespinhal nos escores foi testada através de análise de regressão multivariada. Resultados: Avaliamos 307 ombros em 297 pacientes. Rupturas subescapulares no terço superior de espessura total apresentaram escores significativamente piores aos pacientes com tendão intacto. Rupturas de espessura parcial apresentaram escore sem diferença significativa em relação aos demais. Pacientes com rupturas do terço superior de espessura total apresentaram maior taxa de lesão e instabilidade tendínea do bíceps. Análise multivariada mostrou que as rupturas tendíneas do bíceps e infraespinal não influenciaram os escores. Conclusão: Pacientes com lesão do terço superior de espessura total do tendão do subescapular apresentaram escores funcionais piores do que pacientes com tendão intacto submetidos ao reparo artroscópico posterossuperior do manguito. The subscapularis has received little attention in the orthopedic literature, although such lesions are evident in up to 40 % of arthroscopies. An accurate diagnosis is important in clinical practice, as it affects both patient prognosis and surgical planning. The aim of this study was to evaluate the accuracy of preoperative MRI for the identification of subscapularis tears. Our secondary aim involved the identification of factors that may be predictive for the presence of subscapularis tears. This retrospective case series included patients undergoing shoulder arthroscopy due to rotator cuff injury. MRI was performed using a 1.5 T unit, and the results were assessed by a musculoskeletal radiologist. The findings were compared to those of arthroscopic inspection. A total of 93 shoulders were analyzed. The overall accuracy was 82 %, with values of 79 % for partial tears and 89 % for full-thickness tears. Tears requiring repair demonstrated an accuracy of 88 %. Infraspinatus tears, fatty degeneration of the subscapularis, biceps instability and age were identified as predictive factors for the presence of injury. MRI of the shoulder demonstrated an accuracy of 82 % for the diagnosis of subscapularis tears. The accuracy values for partial and full-thickness tears were 79 and 89 %, respectively. Background Previous studies have shown good clinical results in patients with proximal humeral fractures (PHFs) treated with locking intramedullary nails or locking plates. Our study compared the clinical and radiographic outcomes in patients with 2- and 3-part surgical neck fractures. Methods In this prospective, randomized controlled trial, 72 patients with 2- or 3-part surgical neck PHFs were randomly assigned to receive fixation with locking intramedullary nails (nail group) or locking plates (plate group). The primary outcome was the 12-month Constant-Murley score. The secondary outcomes included the Disabilities of the Arm, Shoulder and Hand score, the visual analog scale pain score, the shoulder passive range of motion, the neck-shaft angle, and complication rates. Results There was no significant mean treatment group difference in the Constant-Murley score at 12 months (70.3 points for the nail group vs. 71.5 points for the plate group; P = .750) or at individual follow-up assessments. There were no differences in the 3-, 6- and 12-month Disabilities of the Arm, Shoulder and Hand scores, visual analog scale scores, and range of motion, except for the medial rotation at 6 months. The neck-shaft angle was equivalent between the groups at 12 months. There were significant differences over 12 months in total complication rates ( P = .002) and reoperation rates ( P = .041). There were no significant differences for the rotator cuff tear rate ( P = .672). Conclusion Fixation of PHFs with locking plates or locking intramedullary nails produces similar clinical and radiologic results. Nevertheless, the complication and reoperation rates were higher in the nail group.

Sugaya’s classification is the most commonly used for postoperative evaluation of rotator cuff repairs. However, the correlation between this classification and clinical outcomes after supraspinatus tendon repair were not performed with serial MRI examinations in standardized time intervals. This prospective case series involved 54 patients undergoing repair of the supraspinatus tendon tear. Magnetic resonance imaging (MRI, 1.5 T) was used to determine the Sugaya’s classifications at 3, 6, and 12 months, and these data were correlated with the visual analog scale for pain (VAS), Constant and University of California at Los Angeles (UCLA) assessments. Patients with types I, II, and ≥III of Sugaya’s classification experienced pain of 1.27 ± 1.95, 1.00 ± 1.40, and 3.43 ± 3.36, respectively (p = 0.010), according to the VAS. The Constant and UCLA scales did not differ significantly. Type II predominated, though their percentage decreased over time (from 77.8 to 66.7 %), whereas type I became more frequent (from 1.9 to 20.4 %). The pain was more intense in patients classified as types III, IV, or V of Sugaya’s classification. The postoperative appearance of the supraspinatus tendon was not correlated with the Constant and UCLA scales. The occurrence of type II, the most prevalent, decreased over time, whereas the occurrence of type I increased; these differences were not significant. Background/Hypothesis The Latarjet procedure changes the anatomy of the shoulder, moving the coracoid process distally through an incision in the subscapularis muscle. Some authors have studied the effect of this surgery on shoulder rotator strength. Our hypothesis is that the Latarjet procedure decreases elbow supination and flexion strength. Methods A retrospective case series, evaluating patients submitted to the Latarjet procedure between May 2013 and June 2017. Movements of the elbow (supination, pronation, flexion and extension) and shoulder (internal and external rotation) were evaluated bilaterally using a Biodex System 3 isokinetic dynamometer (Biodex Medical Systems, New York) in concentric/concentric mode. Results We evaluated 20 patients with an average follow-up of 36 months. In the elbow, we observed a 9.1% decrease in supination strength at a speed of 60°/s (p = 0.044), without statistical difference at 120°/s (p = 0.570). In the shoulder, there was a 13.5% decrease in external rotation strength at 60°/s and 4.5% at 180°/s (p = 0.009 and p = 0.040, respectively). The other movements did not demonstrate any statistically significant differences. Conclusion After the Latarjet procedure, the supination strength at 60°/s was decreased, as was the external rotation strength of the shoulder at 60°/s and 180°/s. We did not observe any reduction in strength for shoulder flexion or internal shoulder rotation. Level of evidence IV, Case Series.

Background: Platelet-rich plasma (PRP) has been studied with the objective of reducing the retear rate and improving functional outcomes after rotator cuff repair. Only one study to date has reported its midterm effect. Hypothesis: PRP promotes better functional and structural results in arthroscopic rotator cuff repair. Study Design: Randomized controlled trial; Level of evidence, 2. Methods: All patients underwent arthroscopic single-row repair of small to medium supraspinatus tears. At the end of the surgical procedure, liquid PRP prepared by apheresis with autologous thrombin was applied in the tendon-to-bone interface in the PRP group. The outcomes were assessed by the University of California, Los Angeles (UCLA) and Constant scales and the visual analog scale (VAS) for pain at 6, 12, 24, and 60 months after surgery and magnetic resonance imaging at 12 and 60 months. Results: Of 54 patients initially randomized, we analyzed the clinical outcomes in 51 (25 control, 26 PRP) and the structural outcomes in 44 (22 each group). At 60-month follow-up, the mean UCLA scores were 32.5 ± 3.8 and 32.1 ± 4.6 in the control and PRP groups, respectively ( P = .992). The mean Constant scores were 82.0 ± 9.5 in the control group and 82.1 ± 11.0 in the PRP group ( P = .699). The mean VAS scores were 1.4 ± 1.8 and 1.5 ± 2.1 in the control and PRP groups, respectively ( P = .910). None of the clinical assessments at 6, 12, and 24 months in either group produced statistically significant differences, and both groups showed significant improvements throughout the follow-up time in the 3 evaluations ( P < .001). The control group exhibited 1 full-thickness retear (Sugaya type IV) and 11 partial-thickness retears (Sugaya type III), while the PRP group had 7 partial-thickness retears (Sugaya type III). The overall number of retears did not differ between groups ( P = .203). Conclusion: PRP obtained by apheresis and applied in liquid form with the addition of thrombin at the end of single-row repair of supraspinatus tears did not promote better clinical or structural results at 60-month follow-up. Objective: To evaluate the correlation between the UCLA and Constant-Murley scores in the surgical treatment of rotator cuff tears and proximal humeral fractures (PHF). Methods: Retrospective study evaluating patients submitted to arthroscopic rotator cuff repair and surgical treatment of PHF with 2-year follow-up. Patients were evaluated by the UCLA and Constant-Murley scores in the preoperative period for the rotator cuff repairs, and 3, 6, 12 and 24 months after surgery for both diagnoses. Pearson's correlation coefficient (r) was calculated to measure the degree of correlation between the two clinical scales. Results: We evaluated 109 patients: 54 with rotator cuff tear and 55 with PHF. Twenty-four months after surgical treatment, the scores according to the UCLA and Constant-Murley scores were 32.6 ± 4.0 and 85.0 ± 12.0 for the rotator cuff tears and 30.3 ± 5.3 and 73.8 ± 13.9 for the PHF, demonstrating significant improvements in both, in relation to the initial evaluation (p < 0.001). The scales demonstrated high correlation (r = 0.88, p < 0.001). The scores obtained in the two scales showed high or very high correlation in all the postoperative clinical evaluations (r = 0.79-0.91, p < 0.001). The correlation was high in the preoperative evaluation (r = 0.73, p < 0.001). Conclusion: The UCLA and Constant-Murley scores presented high or very high correlation in the evaluation of surgical treatment of rotator cuff tears and PHF. The correlation in the preoperative evaluation was high. RESUMO Objetivo: Avaliar a correlação entre as escalas da UCLA e de Constant-Murley no tratamento cirúrgico de roturas do manguito rotador e de fraturas da extremidade proximal do úmero (FEPU). Métodos: Estudo retrospectivo, que avaliou pacientes submetidos ao reparo do manguito rotador por via artroscópica e tratamento cirúrgico de FEPU com dois anos de seguimento. Os pacientes foram avaliados pelas escalas da UCLA e de Constant-Murley no período pré-operatório nas roturas do manguito rotador e após seis, 12 e 24 meses da cirurgia em ambos os diagnósticos. O coeficiente de correlação de Pearson (r) foi calculado para medir o grau de correlação entre as duas escalas clínicas. Resultados: Avaliamos 109 pacientes, 54 com rotura do manguito rotador e 55 com FEPU. Após 24 meses do tratamento cirúrgico, as pontuações pelas escalas da UCLA e da Constant-Murley foram de 32,6 ± 4,0 e 85,0 ± 12,0 nas roturas do manguito rotador e 30,3 ± 5,3 e 73,8 ± 13,9 nas FEPU, com melhoria significativa em ambas em relação à avaliação inicial (p < 0,001). As escalas demostraram alta correlação (r = 0,88, p < 0,001). Em todas as avaliações clínicas pós-operatórias as pontuações obtidas nas duas escalas se correlacionaram de modo alto ou muito alto (r = 0,79 a 0,91, p < 0,001). No pré-operatório a correlação foi alta (r = 0,73, p < 0,001). Conclusão: As escalas da UCLA e de Constant-Murley apresentam uma correlação alta ou muito alta na avaliação do tratamento cirúrgico das roturas do manguito rotador e das FEPU. No pré-operatório a correlação é alta.

The accuracy of MRI for subscapularis tear is lower than that of overall rotator cuff tears. Until now, no systematic reviews and meta-analysis have been conducted to compile these data. The purpose of this study was to determine, through a systematic review and meta-analysis, the diagnostic accuracy of MRI in the detection of subscapularis tendon tears. A systematic review of PubMed, EMBASE, and MEDLINE databases up to April 2017 was performed. All studies assessing the sensitivity and specificity of the MRI (index test) compared to arthroscopic surgical findings (reference test) for subscapularis tendon tear were included. A meta-analysis was performed to calculate pooled sensitivity, specificity, sROC curve, and diagnostic odds ratio values. A total of 497 citations were identified. After applying the eligibility criteria, 14 articles were included, including 1858 shoulders with 613 subscapularis tears. For overall subscapularis tears, sensitivity was 0.68 (95% CI 0.64–0.72) and specificity was 0.90 (95% CI 0.89–0.92). Sensitivity was 0.93 (95% CI 0.83–0.98) for full-thickness tears and 0.74 (95% CI 0.66–0.82) for partial tears. Specificity was 0.97 (95% CI 0.94–0.98) for full-thickness tears and 0.88 (95% CI 0.85–0.91) for partial tears. Analyzing only studies with field of strength ≥ 1.5 T, sensitivity was 0.80 (95% CI 0.76–0.84) and specificity 0.84 (95% CI 0.81–0.87). MRI is an accurate method for diagnosing subscapularis tendon tears; however, its accuracy is lower than that of overall rotator cuff tears, due to its lower sensitivity. Background In the event of a traumatic rotator cuff tear, patients are routinely advised that early surgical intervention produces an optimal repair, despite a lack of direct evidence to support this recommendation. To address this knowledge gap, massive rotator cuff tears in rats were assessed by biomechanical and bone morphometric analyses after early or late repair. Methods Combined supraspinatus and infraspinatus tendon tears of the left shoulder were created in 21 adult Wistar rats, which were divided into 2 groups. The tendons of the injured shoulder in the animals in group I were surgically repaired 8 weeks after the injury. Under the same anesthesia, the same injury was created on the right shoulder, which was immediately repaired. The rats from group I were euthanized 8 weeks after the repairs. No repair was performed in the rats from group II, which were euthanized 8 weeks after the injury. Tissues from both groups were harvested and biomechanically tested for supraspinatus tendon and bone morphometry analysis of the humeral head. Results All biomechanical properties were significantly increased in the early repair group compared with the late repair group. No significant differences were observed in bone morphometry of the humeral head when early and late repair groups were compared. Conclusion Early surgical repair of a massive rotator cuff tear leads to improved biomechanical properties of the tissue after healing. Proximal humerus bone morphometry was unaffected by surgical repair timing.

Humeral shaft fractures (HSFs) represent 3% of the fractures of the locomotor apparatus, and the middle third of the shaft is the section most affected. In the majority of cases, it is treated using nonsurgical methods, but surgical indications in HSF cases are increasingly being adopted. The diversity of opinions makes it difficult to reach a consensus regarding the types of osteosynthesis, surgical technique and quantity and quality of synthesis materials that should be used. It would appear that specialists are far from reaching a consensus regarding the best method for surgical treatment of HSFs. We believe that less invasive methods, which favor relative stability, are the most appropriate methods, since the most feared complications are less frequent.

Proximal humerus fractures (PHF) are common fractures and are the third most common type of fractures among older adults. The most commonly used implants include the locking plate and the locking intramedullary nail. Areas covered: The aim of this study is to perform a literature review of biomechanical and clinical studies that compare the locking plate and intramedullary nail for PHF osteosynthesis. Expert commentary: Twelve clinical studies and seven biomechanical studies were identified that met this criterion. The findings of this review showed that intramedullary nailing and locking plate fixation yielded similar functional results, but with contrasting complication rates. The biomechanical studies showed controversial results, with most of the studies demonstrating better biomechanical properties for the intramedullary nail. Different types of intramedullary nail for PHF have different characteristics, with curvilinear nails presenting a higher risk of complications.

Background Minimally invasive plate osteosynthesis for humeral shaft fractures has been described recently, but there are no randomized studies comparing the clinical results for shoulder function between this technique and locking intramedullary nailing. Methods A prospective randomized study was performed. Forty-one humeral shaft fractures (40 patients) were randomized to be treated with a minimally invasive plate (n = 21) or a locking intramedullary nail (n = 19). Clinical and radiographic outcome assessments were conducted at 1 year postoperatively. Shoulder function was the primary outcome, as measured by the University of California, Los Angeles Shoulder Scale. Elbow function was measured by the Broberg-Morrey score, and fracture consolidation and complications were the main secondary outcomes. Results At 1 year postoperatively, no significant difference was found with regard to shoulder function according to the University of California, Los Angeles scale between the minimally invasive plate and locking intramedullary nail (31.4 points vs 31.2 points, P = .98). There was also no difference in elbow function (94.8 points vs 94.1 points, P = .96). Complications were similar between the groups, without significant differences regarding infection ( P > .99), symptomatic shoulder stiffness ( P = .488), and neurapraxia of the lateral cutaneous nerve of the forearm ( P = .475). Fracture union was achieved in all but 1 patient (2.4%) in the intramedullary nail group within 1 year after the surgical procedure. Conclusion There is no significant difference in shoulder function between antegrade intramedullary nailing and minimally invasive plate osteosynthesis for the treatment of displaced humeral shaft fractures, despite the limited power of our study.

ResumoObjetivoAvaliar os resultados clínicos e radiográficos e as complicações das fraturas do terço proximal do úmero tratadas com a placa Philos® e correlacionar esses resultados com critérios prognósticos.MétodosForam estudados 40 pacientes submetidos a osteossíntese de fraturas do terço proximal do úmero com a placa Philos®. As cirurgias foram feitas entre 2004 e 2011 e os pacientes foram submetidos a avaliação funcional (escalas de Constant-Murley e Dash [Disability of Arm-Shoulder-Hand]) e radiográfica. Os resultados funcionais foram correlacionados com variáveis clínicas e radiográficas por meio de regressão múltipla.ResultadosOs pacientes apresentavam em média 61,8±16,28 anos e a maioria era do sexo feminino (70%). Observamos pontuação de 72,03±14,01 pela escala de Constant-Murley e 24,96±19,99 pela de Dash. A radiografia pós-operatória evidenciou um ângulo cabeça-diáfise de 135,43°±11,82. A análise por regressão demonstrou que a idade do paciente e a classificação de Hertel exercem influência direta na escala de Constant-Murley (p=0,0049 e 0,012, respectivamente). Outros critérios prognósticos, como a classificação de Neer e AO, o ângulo cabeça-diáfise, a presença de cominuição metafisária e a extensão do fragmento metafisário não demonstraram influência no prognóstico em nossa amostra. Complicações ocorreram em quatro pacientes (10%).ConclusãoA osteossíntese com a placa Philos® proporcionou, em nossa amostra, bons resultados clínicos e radiográficos, com baixo índice de complicações. A idade do paciente e a classificação de Hertel foram demonstradas como fatores preditores do resultado funcional.AbstractObjectiveEvaluate functional outcomes, radiographic findings and complications of proximal humeral fractures treated with locking plates and to determine prognostic factors for successful clinical outcomes.MethodsForty patients undergoing internal fixation of fractures of the proximal humerus with the Philos® plate were included in the study. The surgeries were performed between 2004 and 2011 and the patients underwent radiographic and clinical evaluation, by Constant-Murley and Dash score. Outcomes were analyzed by use of multivariate regression with several different variables.ResultsPatients were on average of 61.8±16.28 years, and most were female (70%). The Constant-Murley score was 72.03±14.01 and Dash was 24.96±19.99. The postoperative radiographs showed a head-shaft angle of 135.43°±11.82. Regression analysis showed that the patient's age and the Hertel classification influenced the Constant-Murley scale (p=0.0049 and 0.012 respectively). Others prognostic criteria such as Neer and AO classification, head-shaft angle, the presence of metaphyseal comminution and extension of the humeral metaphyseal fragment showed no effect on prognosis. Complications occurred in four patients (10%).ConclusionThe fixation with the Philos® plate provided good clinical and radiographic results in fractures of the proximal humerus, with a low complication rate. The patient's age and Hertel classification were defined as prognostic factors that led to worse functional outcomes.

OBJECTIVE: To evaluate the functional and radiographic results in patients undergoing shoulder anterior soft tissue stretching in association with open reduction and internal rotation osteotomy to centralize the humeral head as a treatment for Erb-Duchenne obstetric palsy sequelae. METHOD: A total of 35 patients underwent this surgical treatment, and the mean follow-up was 4.6 years. The Mallet scale was applied before and after the surgical procedure. A total of 20 patients underwent computed tomography to assess the glenoid version and humeral head subluxation. OBJETIVO: Avaliar a segurança do procedimento de Latarjet artroscópico em cadáveres. MÉTODOS: Doze ombros foram submetidos ao procedimento de Latarjet artroscópico em nosso laboratório de artroscopia, por quatro cirurgiões diferentes. Após a cirurgia os espécimes foram submetidos a exames radiográficos e avaliados por um examinador independente. Dezenove parâmetros foram avaliados, incluindo a fixação do enxerto do coracoide, o posicionamento e a angulação dos parafusos, lesões neurológicas e a integridade dos tendões. RESULTADOS: Quatro dos procedimentos foram considerados satisfatórios, sem diferença entre os cirurgiões. A angulação média dos parafusos foi de 27,2°. A incisão no tendão do subescapular foi, em média, a 17,8mm da sua borda superior. O enxerto coracoide foi corretamente posicionado em relação ao equador da glenoide em 11 casos. Não houve lesão nos nervos axilar ou musculocutâneo. As principais complicações encontradas foram: interposição de tecidos moles, lesão do nervo supraescapular, desvio articular do enxerto, diastase e lesão do tendão conjunto. CONCLUSÃO: O procedimento de Latarjet artroscópico é uma técnica complexa, em que cada etapa deve ser precisa para diminuir o risco de complicações. Nosso estudo demonstrou um alto risco de insucesso do procedimento. Nível de evidência iv, série de casos. OBJECTIVE: To evaluate the safety of arthroscopic Latarjet procedure in cadavers. METHODS: Twelve cadaveric shoulders underwent arthroscopic Latarjet procedure in our laboratory for arthroscopy, by four different surgeons. Following surgery, the specimens were subjected to radiographic examination and evaluated by an independent examiner. Nineteen parameters were evaluated, including the coracoid graft fixation, positioning and angulation of the screws, neurological damage and integrity of tendons. RESULTS: Four procedures were considered to be satisfactory, with no difference among the surgeons. The mean angulation of the screws was 27.2º. The subscapularis splitting was, on average, 17,8mm from the upper edge. The coracoid graft was properly positioned relative to equator of the glenoid in 11 cases. There were no injury to the axillary or musculocutaneous nerves. The main complications were: interposition of soft tissue, suprascapular nerve injury, articular deviation of the graft, diastasis and conjoined tendon injury. CONCLUSION: The arthroscopic Latarjet procedure is a complex technique in which each step must be precise to reduce the risk of complications. Our study showed a high risk of failure of the procedure. Level of evidence iv, case series.

ResumoObjetivoFazer uma análise das publicações da Revista Brasileira de Ortopedia (RBO), visando a avaliar os tipos e o nível de evidência científica dos estudos.MétodosTodos os artigos científicos publicados na RBO de janeiro de 2006 a dezembro de 2010 foram classificados de acordo com o tipo de estudo. Os trabalhos clínicos foram ainda estratificados conforme o nível de evidência clínica, de acordo com a norma editorial da revista. Os estudos classificados como ensaios clínicos controlados e randomizados (ECCR) tiveram sua qualidade avaliada pelo sistema proposto por Jadad.ResultadosNesse período, foram publicados na RBO 376 artigos. Os estudos clínicos corresponderam à maioria das publicações, com 60,64% do total, sendo que desses 61,4% eram séries de casos. Foram publicados 13 ECCR, correspondendo a 3,46% do total dos artigos publicados e a 5,7% dos estudos clínicos. A análise da qualidade dos ECCR mostrou que cinco (38,46%) foram considerados de alta qualidade, enquanto oito (61,54%) foram de baixa qualidade. Dentre os estudos nos quais o nível de evidência clínica não se aplica (não clínicos) prevaleceram os artigos de revisão não sistemática (46) e os de pesquisa básica (40), que representaram, respectivamente, 12,23% e 10,64% do total de publicações.ConclusõesAs publicações do tipo série de casos foram as mais prevalentes (37,23%), enquanto os ECCR corresponderam a 3,46% do total. A maioria dos ECCR (61,54%) foi considerada de baixa qualidade. Apenas 1,32% dos estudos clínicos foi classificado como nível I de evidência.AbstractObjectiveProfile analysis of articles from scientific journals is rare in our country. The aim of this study was to perform an analysis of publications of the Revista Brasileira de Ortopedia (RBO), to specify the designs of the studies and their level of evidence.MethodsAll articles published in RBO from January 2006 to December 2010 were classified according to the design of the study. The clinical studies were further stratified according to the level of evidence, in agreement with the norm of the journal. The studies classified as randomized and controlled clinical trials (RCTs) had their quality assessed by the system proposed by Jadad.ResultsIn this period, there were 376 articles published in RBO. Clinical studies represented most of the papers, with 60.64% of the total. Case series represented 61.4% of the clinical studies. Thirteen RCTs were published, accounting for 3.46% of the total, and 5.7% of the clinical studies. The analysis of the quality of the RCTs showed that 5 (38.46%) were considered high quality, while 8 (61.54%) were of low quality. Among the studies in which the level of evidence does not apply (non-clinical), non-systematic reviews (46 articles) and basic research (40 articles) have prevailed, representing 12.23% and 10.64% respectively of the total.ConclusionsCase series were the most prevalent (37.23%) studies published in RBO between 2006 and 2010, while RCTs accounted for 3.46% of the articles. The majority of RCTs (61.54%) were considered low quality, and only 1.32% of the clinical studies were classified as level I evidence.

Background The head-shaft angle is used to plan osteotomies and arthroplasties and to assess the radiographic outcomes of surgical treatment for proximal humerus fractures. There are no published data showing whether different degrees of arm rotation interfere with the evaluation of this angle. Methods Eighteen humeri from adult cadavers were used. Radiographs were taken with the specimens initially placed in a true anteroposterior position and then subsequently positioned with internal and external rotations of 10°, 20°, and 30°. All radiographs were evaluated by 3 shoulder and elbow surgeons at 2 different times 3 months apart. The head-shaft angle was measured using a picture archiving and communication system. Results For the humerus in the neutral position, the head-shaft angle was 137° ± 4°. With the anatomic specimen positioned with increasing external and internal rotations, there was a maximum difference of 2° compared with the value observed in the neutral position, which was not a significant difference ( P = .911). Measurements of the head-shaft angle showed a good interobserver correlation coefficient, with a value of 0.788 (0.728-0.839) for all measurements. The intraobserver correlation coefficient ranged from moderate to excellent (0.536-0.938). Conclusion The head-shaft angle did not change significantly with varying degrees of humeral rotation. The interobserver correlation coefficient showed good reliability, and the intraobserver correlation was moderate to excellent.

Objective: To evaluate shoulder functional results and the retear rate of arthroscopic repair of the rotator cuff augmented with platelet-rich plasma (PRP).Methods: Prospective case series with single-row arthroscopic repair of the rotator cuff augmented with PRP. Only cases of isolated supraspinatus tears with retraction of less than 3 cm were included in this series. The PRP used was obtained by apheresis. It was applied on liquid consistency in its activated form, with the addition of autologous thrombin. Patients were evaluated after 12 months of the surgical procedure. The Constant-Murley, UCLA and VAS scales were used, and the retear rate was assessed using magnetic resonance imaging (MRI). Results: Fourteen patients were evaluated (14 shoulders). The mean Constant-Murley score was 45.64 ± 12.29 before the operation and evolved to 80.78 ± 13.22 after the operation (p < 0.001). The UCLA score increased from 13.78 ± 5.66 to 31.43 ± 3.9 (p < 0.001). The patients’ pain level decreased from a median of 7.5 (p25% = 6, p75% = 8) to 0.5 (p25% = 0, p75% = 3) (p = 0.0013) according to the VAS score. None of the patients presented complete retear. Three patients (21.4%) showed partial retear, without transfixation. Only one patient developed complications (adhesive capsulitis). Conclusion: Patients submitted to arthroscopic rotator cuff repair augmented with PRP showed significant functional improvement and none of them had complete retearing.

Objective: To evaluate the clinical result from the filling (“remplissage”) technique in association with Bankart lesion repair for treating recurrent anterior shoulder dislocation. Methods: Nine patients (10 shoulders), with a mean follow-up of 13.7 months, presented traumatic recurrent anterior shoulder dislocation. All of them had a Bankart lesion, associated with a Hill-Sachs lesion showing the “engaging” sign. The Hill-Sachs lesion defect was measured and showed an average bone loss of 17.3% (7.7% to 26.7%) in relation to the diameter of the humeral head. All the cases underwent arthroscopic repair of the Bankart lesion, together with filling of the Hill-Sachs lesion by means of tenodesis of the infraspinatus. Results: The Rowe score ranged from 22.5 (10 to 45) before the operation to 80.5 (5 to 100) after the operation (p > 0.001). The UCLA score ranged from 18.0 (8 to 29) to 31.1 (21 to 31) (p > 0.001). The measurements of external and internal rotation at abduction of 90° after the operation were 63.5° (45° to 90°) and 73° (50° to 92°) respectively. Two patients presented recurrence (one with dislocation and the other with subluxation). None of the patients presented pain in the region of the infraspinatus tendon after the operation. Conclusion: Over the short term, the filling (“remplissage”) arthroscopic technique produced improvements in functional scores and a low complication rate when used for treating glenohumeral instability associated with Hill-Sachs lesions.

Objective: To describe the initial experience of four orthopedic clinics from using Bi-Contact® total elbow arthroplasty (TEA), reporting the results and complications of the procedure. Methods: This was a retrospective study, through analysis on the medical records of patients who underwent primary TEA using a prosthesis model developed in conjunction with IOT-HCFMUSP. Forty-six elbows (45 patients) that were operated at four orthopedic clinics between 2000 and 2009 were evaluated. Results: The majority of the patients were female (74%), and the median age was 62.5 years. The diagnoses encountered were trauma sequelae (47.83%), rheumatoid arthritis (32.61%), primary osteoarthrosis (8.7%), acute fractures (6.52%) and heterotopic ossification (2.17%). The median length of follow-up was 2.08 years (0.25-9). The procedure significantly alleviated pain and improved range of motion. It was observed that at least one complication was present in 69.57% of the cases, and the main ones were infection (28.26%), need for revision (28.26%), intraoperative fracture (15.22%) and aseptic loosening (15.22%). Conclusion: Bi-Contact® TEA provided significant alleviation of pain and improvement of range of motion in the present series. The complication rate was high, and the most frequently observed complications were infection, aseptic loosening and intraoperative fracture.

To justify and describe the humeral head centering osteotomy---an original surgical technique---associated with anterior soft tissue elongation (Sever procedure) for the prevention and treatment of shoulder osteoarticular deformities in children with sequelae of obstetric palsy.

OBJETIVO: Apresentar os resultados clínicos e radiográficos de uma série de casos com diagnóstico de Luxação Acromioclavicular (LAC) Aguda, tratados através da fixação coracoclavicular com âncoras por via artroscópica. MÉTODO: Vinte pacientes apresentando LAC com menos de 30 dias de evolução foram operados pela técnica da estabilização coracoclavicular com âncoras por via artroscópica. Duas âncoras metálicas com dois fios cada, foram inseridas no coracóide. Os fios foram amarrados sobre a clavícula passando por túneis transósseos claviculares. Para a avaliação radiográfica, foi utilizada a medida comparativa da distância coracoclavicular com o lado contralateral e a avaliação funcional através dos escores de Constant e UCLA o seguimento foi de seis meses. RESULTADO: Dos vinte casos inicialmente selecionados, seis necessitaram de novo procedimento cirúrgico e foram excluídos do estudo. Dos quatorze pacientes restantes, apenas dois mantiveram redução da articulação acromioclavicular, enquanto os demais apresentaram algum grau de desvio no decorrer da evolução. Desconsiderando os pacientes excluídos, os escores de Constant e UCLA tiveram média 94,79 (82-100) e, 32,64 (26-35), respectivamente. CONCLUSÃO: A técnica apresentou um alto índice de perda da redução ao longo da evolução de seis meses. A avaliação funcional apresentou resultado satisfatório com escore médio elevado de Evidência: Nível de Evidência: Nível III, estudo retrospectivo.

To evaluate the clinical and radiographic results from arthroscopic surgical treatment of the rotator cuff in patients with calcifying tendinitis. Method: A retrospective study was conducted on twenty patients who underwent arthroscopic treatment for calcifying tendinitis of the shoulder between March 1999 and November 2005. Six patients were excluded due to loss of follow-up. The average follow-up period was 41.4 months. Eight patients (57%) were female and six (43%) were male. The right side was affected in 10 cases (71%) and the left in four cases (29%). Nine cases (64%) had calcification in the supraspinatus tendon, two (14%) in the infraspinatus tendon, and three (21%) in both tendons. Results: In all cases, resection of the calcium deposits was performed by means of a needle (Jelco® No. 14) in combination with curettage (mini-curette). Two shoulders (14%) underwent subacromial decompression, and one (7%) underwent excision of the distal clavicle. A tendon-tendon suture was performed in three shoulders (21%). None of the patients underwent tendon-bone reinsertion. The mean score obtained on the UCLA scale was 33 points (26-35), thus indicating that a majority of patients had good results. In the final radiographic evaluation, none of the patients showed signs of calcification. Conclusion: Arthroscopic treatment of calcifying tendinitis of the shoulder safely allows excision of the calcification, leading to good results in relation to shoulder pain and function.
