This paper explores clinical work concerning a group of gender-questioning natal males who experienced physical difficulties in childhood. The author argues that these individuals develop a transgender (trans) identity as a defence mechanism against the psychological challenges associated with development, particularly the struggles of forming a distinct mind and identity. Anxieties related to physical health may have contributed to the formation of an anxious attachment to their primary caregiver, perceived as intrusive and demanding. Moreover, these individuals struggle to identify with their fathers, often experiencing them as distant, absent, or unsupportive of their relationship with the mother or other primary caregiver, thereby hindering separation from the primary attachment figure. These early experiences may lead to difficulties in acknowledging and owning their evolving sexuality and aggression, as well as in bearing the necessary guilt associated with separating from the primary caregiver. Consequently, the individual may fantasise about relinquishing identification with an idealised version of themselves and replacing it with a liberated version capable of living autonomously. Through a composite case, the paper illustrates how these conflicts manifest in the therapeutic setting through transference dynamics. Inpatient psychiatric units are heavily regulated physical environments designed around the twin aims of treatment and containment. Less formally regulated but no less important are emotional norms and tones that also contribute significantly to psychiatric care environments. Inpatient psychiatric units are co-created by patients and clinicians, but clinicians have authority that patients do not. This means that clinicians' management of their own transference and reactions is clinically and ethically important. This article defines transference reactions and draws on case examples to canvass how positive and negative transference reactions can influence inpatient care of patients who are suicidal. Clinical social work practice in organisational settings is complex and challenging. Effective practice is contingent on knowledge and skills that are particular for systems-based work involving multiple interprofessional mental health practitioners and clients interacting in a relational matrix. Yet, not all social work theories and constructs that were developed for application in individual, group, and family modalities are directly transferable to the practice environment of a treatment system. This paper offers a conceptual framework that synthesises contemporary psychodynamic theory with systems theory – referred to as a psychodynamic systems approach – to inform and advance knowledge of systems-based social work practice. This approach considers the interplay of dynamic processes among the four levels of the socially co-constructed system which constitute the system as a whole. It examines how five key clinical phenomena – transference, countertransference, splitting, projective identification and enactment – are actualised in systems. Definitions of these phenomena formulated from a dyadic perspective and a contemporary psychodynamic systems approach are offered, and their similarities and differences are discussed. Palliative care clinicians enhance the illness experiences of patients and their families through building therapeutic relationships. Many psychological concepts underlie a clinician's approach to a specific patient. Through high-yield tips, this article highlights ten selected psychological elements that palliative care clinicians often use to support patients. As we all (both clinicians and patients) bring our own histories and unique biographies to the work of palliative care, a more explicit focus on the psychological aspects of this work can enhance our own experience and efficacy as providers. This article explores the notion of inhibition at a theoretical and clinical level in psychoanalysis. The first part follows the development of the notion in Freud's work, from the "Project" (1950a [1895]) to Inhibitions, Symptoms and Anxiety (1926d). It identifies the two approaches to inhibition, the first from an energetic point of view, the second from the angle of its relations to anxiety. The second part of the article is devoted to the links between inhibition and Freud's thoughts about death, in particular in Inhibitions, Symptoms and Anxiety and the links to the death drive. It draws on some of Jones' notes and presents a brief clinical illustration. The third part focuses more particularly on general inhibition, especially in depression and melancholia. Based on the treatment of one patient, the author shows how the slow process of overcoming general inhibition is achieved through the gradual use of negation. From an economic point of view, it is suggested that psychoanalytic treatment, through the transference and associative speech, has an effect on the depletion of energy by diverting the "suction" of stimuli, paving the way for the formation of drive representatives, or inhibitions as symptoms, which will need to be dissected. In the introduction to The Psychology of the Transference (1946), Carl Jung sketched out a theory of "erotic phenomenology" which condenses his teaching about sexuality and romantic love into a very concise summary. But the meaning of this passage is obscured in the English translation given in the Collected Works of C. G. Jung. I propose here a new translation which makes Jung's meaning clearer, along with a commentary which explains it in the context of Freudian drive theory and German 19th century philosophy. Invoking the concepts of instinct, mind, and Eros (both the passion and the divine figure), Jung's theory says that male sexual desire can be cultivated or repressed in four distinct ways, each associated with a female symbolic figure or anima image: Eve, Helen, Mary and Sophia. Institutional transference is a phenomenon describing patients' attitudes toward the institutions where they receive mental health or medical care. While transference toward individual clinicians in palliative care has been described, attitudes of patients with serious illness toward the institutions where they receive specialized care have not been described. Here, we present three cases which demonstrate the phenomenon of institutional transference in patients with serious illness and the resulting clinical implications, which include countertransferential responses of clinicians caring for them. We consider three conditions: (1) the idealized reputation of the academic cancer center, often a tertiary referral center; (2) loss of an institutional connection during care transitions; and (3) countertransferential reactions to institutional transference. We highlight characteristics and personality styles of individuals with cancer that may complicate and intensify institutional transference and identify potential interventions to address common challenges associated with institutional transference. What can psychoanalysis learn from literary fiction? In this paper, I suggest that Kazuo Ishiguro's novel The Remains of the Day sheds light on what Freud (1900), in The Interpretation of Dreams, calls the 'day‐residues'; those unnoticed memories and fragments of experience that in the dream come to be imbued with psychic significance. Ishiguro's unreliable narrator is the butler Stevens, whose desire for 'dignity' and 'greatness' ensures he has carefully erased any trace of feeling from the account he offers of his life and relationship with Miss Kenton, the housekeeper with whom he once worked. But what readers eventually come to know of Stevens, and what Stevens comes to know about himself, is gleaned less from what he says than from what he does not say. Drawing on Freud, Laplanche, Bollas and Barthes as well as a brief clinical example, I explore parallels between the inarticulate nature of the knowledge embodied in Ishiguro's novel and the tacit kind of knowing exemplified within the psychoanalytic transference. I conclude that literary fiction has the capacity to illuminate how psychoanalysis can accommodate and expand the borders of knowledge that is unspoken or inaccessible. [ABSTRACT FROM AUTHOR]

Individuals with borderline personality disorder (BPD) struggle to identify whom they can safely trust, and this struggle contributes to profound emotional turmoil in their close relationships. Transference-focused psychotherapy (TFP) is an application of object relations theory (ORT) that posits that polarized mental representations of self and other define the personality organization of BPD. TFP aims to utilize a clear treatment frame coupled with an analysis of the therapeutic relationship (i.e., the transference) to help individuals with BPD integrate their polarized mental representations. Improvement in the capacity to trust others is inherent in the mechanisms of change in TFP. In this article, a social cognitive model of trust processing provides a new lens through which we formulate how TFP may enhance trust processing in BPD. Recent evidence from randomized clinical trials supports the argument that TFP may intervene with BPD in a way that is concordant with uniquely improved trust processing. [ABSTRACT FROM AUTHOR]

Trust is central to successful therapeutic work with adolescents, but establishing trust can be challenging, especially with adolescents with personality disorders. We present our understanding of trust in working with adolescents with and without personality disorders. We draw on complementary and overlapping perspectives, namely the attachment model, Kernberg's object relations model, and Fonagy's mentalization model. In Kernberg's object relations conceptualization, difficulties in trust experienced by patients with borderline personality disorders result from paranoia associated with splitting and identity diffusion. In Fonagy's mentalization model, epistemic trust is rooted in early experiences of being responded to and understood. We outline how techniques used in transference-focused psychotherapy for adolescents promote the development of the adolescent's capacity to trust by facilitating identity integration, thus reducing paranoia. Transference-focused psychotherapy (TFP) is an empirically supported treatment for borderline personality disorder (BPD) that improves functioning via targeting representations of self affectively relating to others, particularly as evoked in the therapeutic relationship. If change in TFP operates as theorized, then shifts in patterns of "self affectively relating to others" should be observed in the transference prior to shifts in daily relationships. Using ecological momentary assessment (EMA), a patient with BPD rated daily interpersonal events for 2-week periods during 18 months of TFP; at 9 and 18 months these ratings included interactions with the therapist. Results suggest that positive perceptions of her therapist that ran counter to her negatively biased perception in other relationships preceded changes in her perceptions of others. EMA shifts corresponded to improvements in self-reported symptoms, interview-based personality functioning, and therapist assessments. This paper discusses why similar institutional rules (Municipal Plan, Fund, and Council) and intergovernmental transferences) may influence municipal decision-making to participate in inter-municipal consortia differently in two Brazilian federal public policies—health and education. The paper argues that the more institutionalized the national policy design, the higher the probability of the municipalities getting into IMC. Based on quantitative analysis, findings show why the financial dimension is relevant for health policy, the most institutionalized in comparison with education, regarding municipal options for IMC. Strategic concepts are more than simply academic concepts but must also relate to strategic practice, which affects how we should gauge concept quality. Rather than purely academic measures, strategic concepts must also be judged according to their usefulness for practice. Yet the relationship between general theory and specific practice has hardly been explored, a task this article undertakes. Academic theory generates ad hoc concepts, which are used to build specific theories of victory, which are used to generate orders. From a self-psychology perspective, the profound disruption of the will to exist physically and psychically in patients suffering from anorexia can be seen as a primary impairment of the selfobject's capacity to make space for them within itself. Kohut viewed the primary phase of the baby's existence in the mother's mind as its "virtual conception." On this foundation, the author uses the notion of "virtual selfobject" to understand the impaired will to exist in patients with anorexia and describes how the therapist may embrace a standpoint that creates a space-for-being for the patient, facilitating the reactivation of self needs in the transference. The notion of virtuality thus entails a future perspective, which ostensibly foretells or creates the potential future emergence of the patient's self. A broad range of psychotherapies have been proposed and evaluated in the treatment of borderline personality disorder (BPD), but the question which specific type of psychotherapy is most effective remains unanswered. In this study, two network meta-analyses (NMAs) were conducted investigating the comparative effectiveness of psychotherapies on (1) BPD severity and (2) suicidal behaviour (combined rate). Study drop-out was included as a secondary outcome. Six databases were searched until 21 January 2022, including RCTs on the efficacy of any psychotherapy in adults (⩾18 years) with a diagnosis of (sub)clinical BPD. Data were extracted using a predefined table format. PROSPERO ID:CRD42020175411. In our study, a total of 43 studies (N = 3273) were included. We found significant differences between several active comparisons in the treatment of (sub)clinical BPD, however, these findings were based on very few trials and should therefore be interpreted with caution. Some therapies were more efficacious compared to GT or TAU. Furthermore, some treatments more than halved the risk of attempted suicide and committed suicide (combined rate), reporting RRs around 0.5 or lower, however, these RRs were not statistically significantly better compared to other therapies or to TAU. Study drop-out significantly differed between some treatments. In conclusion, no single treatment seems to be the best choice to treat people with BPD compared to other treatments. Nevertheless, psychotherapies for BPD are perceived as first-line treatments, and should therefore be investigated further on their long-term effectiveness, preferably in head-to-head trials. Throughout this essay the author analyzes the concept of transcendence in Jung's theoretical corpus with a focus on its philosophical parameters and therapeutic efficacy in promoting a category of value. Although Jung did not precisely define the terms transcendence, transcendent, and transcendental, it is necessary to tease out these features in order to illuminate the varieties of transcendence in Jung's thought. Distinctions are made between the ontological, epistemological, and phenomenological dimensions of transcendence, including the structure, method, form, and process of mediating, transitioning, and transforming inner experience. Jung's notion of the dialectic operative within the transcendent function is specifically critiqued. Here the author explores the possibility of a synthesis of internal opposition that leads to a greater principle of unity through the sublation of psyche. The function of the enigmatic Middle Bronze Age Courtyard Complexes at Shechem has remained at the forefront of debate concerning the courtyard palace form. Shechem's excavators have been unable to agree on the function of the Courtyard Complexes, with some viewing them as palaces and others as temples. Since the only detailed analysis of the Shechem Courtyard Complexes occurred almost four decades ago (Toombs 1985), a reassessment of the material is overdue. An analysis of the Phase 902 Courtyard Complex, the best preserved of the five sequential complexes, supports its architectural association with the courtyard palace form (Oren 1992), while also highlighting a commemorative function, centered around housing and honoring the deceased elite and administering the ancestral funerary cult. Moreover, a functional link with the later Migdol temple and standing stones suggests at least a partial transference of function to this new, and possibly introduced, temple form in the later Middle Bronze Age. Therefore, the Shechem Courtyard Complexes are to be associated with the courtyard palace form, along with a primarily funerary/commemorative function and are perhaps better viewed as elite funerary complexes. The increase in the number of people living beyond age 90 pose challenges to mental health professionals, many of whom still harbor ageist notions concerning the efficacy of psychotherapy for the oldest old. This paper demonstrates how integrated psychotherapy with an oldest old woman can provide opportunities for integration and personal growth. Integrated psychotherapy based on self-psychology psychodynamic approach, was provided to a woman from age 90 to 100 concerning age-related changes and losses and unresolved traumatic experiences in life. which troubled her notwithstanding previous efforts to resolve them. Over the course of almost ten years, the client came to terms with her aging losses, experiences, worked through the therapeutic process, and learned how to express and accept her deepest feelings, thereby achieving psychological well-being and internal peace. Psychotherapy for the oldest old may contribute to their well-being by offering significant opportunities to process and understand major events and unresolved issues along the life cycle. This study demonstrates that the older self is flexible and capable of growth, even as physical health wanes. Looking at Paul B. Preciado's relationship to psychoanalysis across texts, but especially the recent book Can the Monster Speak?: A Report to an Academy of Psychoanalysts, I seek to disentangle a possible vision for a new psychoanalysis from Preciado's concerns, ambivalence and disgust with the professional field. I call this a somato-militant psychoanalysis that leans on Freud's notion of conversion as the creation of a parasitic traumatic kernel that insists on the side of the body and shows a potential for mutuation, transference, amplified potentia gaudendi, surgical intervention and a radical exteriorization of the subject through access to desire. This somatic archival work runs against an idea of psychoanalysis as merely a privatization and interiorization of the subject, a site for upholding the colonial-patriarchal regime of gender norms and an attempt at therapeutic re-territorialization. B From the Clinical Commentaries Editors b When early attachment disruption in the analysand becomes comingled with unconscious possession in the analyst, the work in the intersubjective space may become fraught with implicit demands upon the analyst's psyche for a mutual analysis to take place. The analyst reflects upon and speculates on mother's capacity to help the infant Leah self-regulate somatically and emotionally, and how the skin-to-skin and psyche-to-psyche intersubjective states were experienced when Leah was an infant. The analyst wonders whether Leah's mother used her body to help Leah regulate her feelings. I found myself wanting to know more about the analyst's internal process, perhaps not unlike Leah, who felt a strong need to know what her analyst was feeling and thinking, and how she was managing to survive the experiences Leah was powerfully subjecting her to. Introduction: Transference (meant in this context, as the patient relational patterns expressed towards the clinician) and therapeutic alliance play a crucial role in the treatment of personality pathology. To date, no empirical study examined the association between these two dimensions of the clinical relationship and patients' personality maladaptive traits in psychotherapy. Methods: A national sample of therapists (N = 100) of different theoretical orientations assessed dysfunctional personality features of a patient in their care using a comprehensive and empirically grounded dimensional diagnostic approach from the Shedler–Westen Assessment Procedure‐200 (SWAP‐200). Moreover, they filled in the Psychotherapy Relationship Questionnaire (PRQ) to identify interpersonal patterns expressed early in treatment by the patients and the Working Alliance Inventory (WAI‐T) to evaluate quality of therapeutic alliance. Results: Overall, the most severe and maladaptive dimensions of patients' personality were associated with more negative clinician–patient dynamics and poorer levels of therapeutic alliance in statistically significant and clinically relevant ways. Notably, the hostile transference was predicted by both SWAP Hostility and Psychopathy, whereas the SWAP Narcissism was the strongest predictor of the special/entitled transference. The latter was also predicted by SWAP Emotional Dysregulation; conversely, the SWAP Dysphoria was the most robust predictor of anxious/preoccupied pattern. The SWAP Schizoid Orientation and Psychopathy predicted avoidant/dismissing attachment pattern; moreover, they were strongly and negatively related to the SWAP Psychological Health that was the best predictor of positive transference and alliance. Conclusions: Findings support that therapists' careful understanding of patients' interpersonal ways during early treatment stages may meaningfully inform diagnostic and therapeutic processes. One of the crucial differences between Ettinger and Butler is that for Ettinger the individual subject, male or female, of any gender, is informed by the feminine-maternal via its matrixial subjectivity. A redefinition of insight-type events is presented, an initial attempt to view insight in terms of behavior analysis in the clinical context and relevant variables are suggested for their study. This definition assumes the insight as a novel behavior that involves the search for solutions to psychological problems. Solving a problem is discovering an effective behavior pattern, based on the creation of verbal rules that help the client to reach the solution. In this problem-solving process, a new behavior emerges, explained through the solution of a transfer task. The theoretical proposal exposed allows a better understanding of this phenomenon, overcoming the problems of mentalist conceptions about the term of insight, and contributes to a better understanding of some relevant elements of change process. Drawing on the classical myth of the goddess Cura, I show that care is concerned with the temporality of Being, a feeling for time in tension between the desire for a world that endures despite our mortality whilst also constantly open to our innate capacity for originality. I elaborate this proposition through its intimate and ambivalent relationship with its counterpart, security, and especially through the topos of the curator. The latter's deployment and simplifications through time, implicitly tasked with removing worries and cares, disavow the paradoxes that comprise its morphology. From its emergence with the Roman Empire, I attend to its resonances within the curacy of the Catholic Church, its transformation with the Reformation and the natural theology of early modern science, its transference to the museum as one responsible for preserving and generating historical understanding, and its more recent re-fashioning as a technique for producing and performing self-identity through selection. The current ubiquity of curation beyond the museum and gallery can be understood, I suggest, not as an aberration but perhaps as a symptom of our temporal malaise, the pathology and felt exhaustion of a stalled project of modernity for which we have been repeatedly told 'there is no alternative'. I conclude by briefly considering some of the implications of accepting the complexities of care and curation for affecting temporal experience without reproducing the logic that sustains our current impasse. The Interpersonal Dynamics (ID) consultation is a structured method of group reflective practice which helps staff mentalize transference and countertransference dynamics with patients. We present a detailed ID consultation of a female patient in an inpatient therapeutic community for people with severe personality disorders and demonstrate how this method sheds light on this patient's internal world as externalised interpersonally with staff in the treatment setting. The resulting enactments are discussed in relation to several psychoanalytic theories and concepts, including Bion's theory of containment and Ogden's interpersonal definition of projective identification. We conceptualise our population of patients based on their use of primitive psychological defences such as splitting and projective identification and on the idea that they live at the border between what Klein described as the paranoid-schizoid and the depressive positions. We argue that the ID consultation functions as a container for staff and patients by bringing together and integrating the parts of the patient which are split off and projected into different staff members. Furthermore, the ID consultation is an invaluable triangular space that facilitates the move from difficult dyadic subjective experiences with patients to triadic objective perspectives and from passive reactions to responsive thinking and understanding. The authors propose a theoretical and technical reflection on the main characteristics of an intersubjective approach to psychoanalytical work. After considering some of the main theoretical orientations that develop the intersubjective relational approach and identify its fundamental elements, the authors consider some of the consequences in terms of therapeutic action and thus of technique in clinical work. The theoretical metaphors of 'field', 'intersubjective analytic third' and 'intersubjective co-construction' are considered as fundamental referents of intersubjective developments in psychoanalytical work. In the context of the intersubjective orientation in psychoanalytic work, the relational frame theory and related notions, such as 'emotional belief', as well as its value for diagnostic reflection consistent with an intersubjective relational approach, are also referred to. Building on scholarship that has sought to trace how moral economies interact and change over time, and their value for understanding Scotland's experience of deindustrialisation in particular, this article investigates the formative experiences of Scotland's community business pioneers and how they shaped a moral economy response to deindustrialisation and Thatcherism. The working-class moral economy terms of 'economic security' and 'control of resources' remained central to this episode of moral economy action. However, a 'new' crowd, drawing on the values of co-operation brought community and workplace activist traditions together to protect expectations of justice and fairness. Oral histories recorded with community business pioneers, provide insight into how the skills and built environment of the industrial past were used by the community business movement as community assets to build new futures. Building an 'ownership consciousness' around local assets was crucial to community repair, not only materially, but also in terms of their narrative representations of these areas. Just as the transference of the working-class moral economy had political consequences, the history of Scotland's community business pioneers reveals how the moral economy was finding expression in civil society and driving the shape of Scotland's social economy in the process. This study examines three therapy cases of Korean families to explore the factors that contributed to the male clients' experience of sex as pressure which led to sexual dysfunction. We used intra- and inter-case analysis to identify similarities and differences between the contributing factors in the cases. The contributing factors were psychological, in-law factors, marital communication factors, family of origin factors, and transference factors. This paper examines the modes of contact and the developmental steps that came out of the high-frequency analysis of an isolated young patient and the corresponding counter-transference feeling of »closest distance«. With this concept, the paper examines the seemingly contradictory experience of both great closeness and vast distance in such a way that they can be viewed as complementary. Artificially intelligent communication technologies (AICTs) that operate autonomously with high degrees of conversational fluency can make communication decisions on behalf of their principal users and communicate with those principals' audiences on their behalf. In this study, we explore how the involvement of AICTs in communication activities shapes how principals engage in impression management and how their communication partners form impressions of them. Through an inductive, comparative field study of users of two AI scheduling technologies, we uncover three communicative practices through which principals engaged in impression management when AICTs communicate on their behalf: interpretation, diplomacy, and staging politeness. We also uncover three processes through which communication partners formed impressions of principals when communicating with them via AICTs: confirmation, transference, and compartmentalization. We show that communication partners can transfer impressions of AICTs to principals themselves and outline the conditions under which such transference is and is not likely. We discuss the implications of these findings for the study of technological mediation of impression management and formation in the age of artificial intelligence and present propositions to guide future empirical research. Embodiment represents a new theoretical development and innovative research perspective within cognitive science and the philosophy of mind, emphasizing the role that the body has in shaping the mind. However, the idea of mind and body being interconnected is an old one and has long roots within psychoanalysis, e.g., in character analytic tradition emphasizing the "how" of relating and talking as expressed in nonverbal behavior and body language. The article explores how the patient's embodied, structuralized bearing and bodily appearance reveal relational messages. Through a case presentation, I illustrate how embodied affects and ways-of-being are actualized in the transference and can be captured through the embodied emotional response of the analyst. Problematic supervision has been found to be a common occurrence among psychotherapists worldwide. Although there are some research studies that explore therapists' experiences of problematic supervision, the literature on such experiences in the context of psychoanalytic supervision is limited. This study, as part of a larger study, explored psychoanalytic psychotherapists' experiences of problematic supervision. Using semi‐structured interviews, 14 professional psychoanalytic practitioners were interviewed. Interpretative phenomenological analysis was employed for data analysis. Five themes were identified: feeling frustrated, hurting deprivation, holding back, losing my voice and challenging boundaries. This is the first study capturing professional psychoanalytic therapists' lived experiences of problematic supervision. The findings emphasise a need to take into account the supervisees' conceptualisations about their difficult experiences in order to conduct supervision in a way that is meaningful and accessible for them. The study findings can be extended by future studies that explore the supervisors and the clients' experiences of supervision. This article presents the core ideas of bibliotherapy as an integrative psychotherapeutic channel, exploring the ways in which it integrates new meanings into psychotherapeutic modalities, concepts, and principles. The unique aspect of the bibliotherapeutic relationship is that it is based on a triangular mode of therapist-text-patient. It supplements this with the creative processes of reading, writing, and the development of the capacity to narrate. Bibliotherapy uses many of the same concepts as other psychotherapeutic treatments, such as transference, countertransference, potential space, and the capacity to play. The integration of various kinds of texts within the clinical context, such as children's literature or adolescent poems and songs, makes bibliotherapy different. The text will be integrated according to the developmental stage of the patients. This article introduces the theoretical background of bibliotherapy and places bibliotherapy within the Winnicottian context and parent–child therapy. Palliative care supports referring colleagues in multiples ways. This support to referring colleagues is not often explored in the literature, yet the psychological concept that best describes it is the holding environment. The holding environment is the relational space palliative care offers referring clinicians for processing emotions and information. Using the case of Gloria, a patient living with cancer, this article discusses ways palliative care creates a holding environment for her referring oncologist, Dr. Ko. As palliative care clinicians, we create this relational space for referring clinicians when we change the dynamic, accompany the clinician, recognize challenges, establish expectations, and share a clinical second look. We all have narcissism, but in some cases, the perception of narcissism becomes extreme and pathological. Systematic research has shown that there are three subtypes typical of narcissistic personality disorder: the grandiose/oblivious, the vulnerable/hypervigilant, and the high-functioning subtype. Both biological and psychological factors are at work, but the true cause of pathological narcissism has not been established. The psychotherapy of narcissistic personality disorder (NPD) is complicated and often frustrating because of the difficulty in engaging a person with narcissistic personality disorder in a psychotherapeutic process. Suicide risk is not rare in patients with narcissism, particularly in the context of severe narcissistic injury, where the patient feels shamed and/or vilified. In conclusion, narcissistic patients are difficult to treat, but the risk of suicide makes it imperative for clinicians to stay involved in the treatment and assist the patient in understanding their vulnerabilities.
