Purpose: The rates of caesarean section (CS) have been increasing in recent years in Türkiye and throughout the world. A previous CS is known to not be an absolute indication for repeat CS, and vaginal delivery (VD) (trial of labor after caesarean (TOLAC)) can occur after a CS. The aim of this study was to investigate the neonatal outcomes of TOLAC patients in Konya City Hospital. Materials and Methods: A retrospective analysis was made of the records of all the births in Konya City Hospital between 10 August 2020 and 10 June 2022. The study included all patients with full or nearly complete cervical dilation-effacement and who had a previous CS. These patients were compared with patients who had elective CS and VD. A total of 119 patients were included in the study in 3 groups; TOLAC n:32, CS n:42, VD n:45. Results: TOLAC patients gave birth earlier and the infant birth weight (2991.25gr), length (50.66cm) and head circumference (34.09cm) measurements were found to be significantly lower than those of the other groups. The development of any problem (icterus, respiratory distress syndrome (RDS), oxygen requirement, hypoglycemia, hypothermia) was determined at a significantly higher rate in the TOLAC (21.9%). Conclusion: Patients with a history of CS do not have to have a repeat CS. TOLAC can be performed in appropriate cases, but care should be taken against uterine rupture, the need for emergency CS, and adverse neonatal outcomes. [ABSTRACT FROM AUTHOR]

This review is a synthesis of the literature on occupational therapy interventions in intensive care. Each year, millions of people discharged from intensive care units face newly acquired cognitive impairment and/or functional disability. Occupational therapy includes studies on daily living activities such as feeding, dressing, early mobilization, ambulation, and cognitive processes such as memory, attention, problem solving, and advanced executive functions such as performing multistep tasks. Increasing arousal and awareness, optimizing alertness, and participation in early mobility activities to elicit a purposeful behavior are the goals of occupational therapy interventions in intensive care. In the literature, it is seen that occupational therapy interventions performed to achieve these goals show significant results. However, studies on this subject, which include that occupational therapists have shown intense activities in the recent past, are insufficient. Future research is needed to clarify the specific interventions and role of occupational therapists in the intensive care unit and the effectiveness of these interventions. The occupational therapy and occupational therapist occupation operate in the intensive care units, which is newly developing in Turkey, is also a new situation in the world. This study investigated whether some factors regarding especially pregnancy and post‐pregnancy and 0–36‐month‐old infants' exposure to digital screens played a role in the attention deficit hyperactivity disorder (ADHD) scores of 4–11‐year‐old children. The cross‐sectional research method was used. The sample consisted of the mothers of 2835 (48.2%) daughters and 3042 (51.8%) sons. The children of the mothers in the sample were between 4 and 11 years old. The mothers were asked to fill in the personal information form about their children and the Conners Parent Rating Scale to determine the ADHD score of the child. The main results revealed associations between ADHD and digital screen exposure at 0–36 months. In conclusion, exposure to digital screen—whether much or little—for the purposes of feeding, silencing, distraction, language acquisition, and digital gaming might pose a serious risk for ADHD. Practitioner points: The prevalence based on the Attention Deficit and Hyperactivity Disorder (ADHD) total‐scores was found to be 16.91%, and on the ADHD‐Index (including DSM criteria) was calculated as 9.72% in the research population.Significant differences were found in terms of ADHD related to some pregnancy and post‐pregnancy factors.There were relationships between ADHD and exposure to digital screen at 0–36 months. Exposure to digital screen, whether much or little (frequently or rarely), for the purposes of feeding, silencing/distraction, language acquisition (even if for such cognitive benefits) and digital gaming posed a serious risk for ADHD. Objective: We aimed to examine the demographic characteristics, etiology and postoperative mortality rates of neonates operated on in our hospital. Methods: The records of neonates who were operated in our tertiary care level university hospital between 2013 and 2019 were reviewed retrospectively. Patients with a diagnosis requiring surgical procedure, age (days), gender, anatomical region where the surgical procedure was performed, length of hospital stay, and mortality rates were investigated. Results: 329 neonates were included in the study of which 213 (64.75%) were male and 116 (35.25%) were female. Gastrointestinal system surgery was performed in 214 patients (65.04%), respiratory system surgery in 95 (28.87%) patients and urogenital system surgery in 17 (5.16%) patients. The mortality rate of 156 patients that were followed up in our neonatal unit in the postoperative period was 16.6%. The mortality rate of female babies (21.3%) was higher than that of male babies (13%). Considering the mortality rates of the patients according to the surgical areas, the highest mortality rate was found in respiratory system surgery (38.46%) which was trailed by gastrointestinal system surgery (12.28%) and urogenital system surgery (7.69%), respectively. Conclusions: Despite advances in neonatal care and surgical techniques, immaturity and congenital anomalies still have an important place among the most common causes of death in infants. In particular, the detection of organ dysfunctions due to congenital anomalies which are considered among the preventable causes of death in infants, efforts to correct them will be beneficial to a certain extent in reducing the mortality rates of neonates. Introduction: There is no information about the clinical ultrasonography (US) use, opinion and experiences in neonatal physicians in Turkey. The aim of our study is to evaluate the attitudes, preferences, frequency of use and perspectives of neonatal physicians in our country regarding clinical use of US. Materials and Methods: The study was conducted through a survey with the participation of neonatal physicians in Turkey with electronically between 1 February to 1 March 2020. The information about participants' age, gender, type of institution, experience period in neonatal intensive care and academic title were recorded. The participants were asked questions such as whether they use US in the clinic, if they are using, which part of body and for which intervention, and whether they received training for this purpose. The attitudes about Clinical US were evaluated using a 5-point Likert scale. Results: 120 neonatal physicians participated in the study. The average age of the participants was determined as 40.38 ± 5.24 years. While 90% (n=108) of the participants had US devices in their clinics, the use of frequency of US among the participants was found as 69.1% (n=83). There was no difference between groups in the answers according to academic title about US device availability in the clinic, use of, participation in clinical US training, pediatric radiology rotation in the neonatal fellow education program,use of US for cranial US, abdominal US and central vein catheterization (p>0,05). Lung US use was found at a significantly higher rate in the neonatology fellows group (86.7%) compared to other academic titles (p<0.05). Conclusions: The study is the first national study in its field. It has been determined that US, which has an important role in the diagnosis, monitoring and treatment of the critically ill group in Turkey, is not used effectively by neonatal physicians. Objective: To investigate pregnancy outcomes and to define preventative measures against to risk factors and complications in pregnancy after liver transplantation. Secondary aim is to report postpartum allograft functions in these patients.Method: This is a case series study. All pregnant women with liver transplantations performed in our hospital were enrolled. Patients' hospital medical records, electronic records Neonatal Intensive Care Unit (NICU) records were used to collect data. Obstetric characteristics and antenatal complications, risk factors, pregnancy and neonatal outcomes, all aspects of liver transplant variables and allograft functions after pregnancy were studied.Results: A total of 11 patients were included in the study. All of them were singleton. Mean maternal age was 29.3 ± 3.9 years. And mean gestational week of delivery was 37.2 ± 2.2. 78% of the women were delivered at term (>37 weeks) only two babies were preterm and discharged from NICU without any complications. Birth weight (gr median ± SD) was 2575 ± 345. Five (45%) patients were nulliparous, majority of the cases (8/11, 72%) were conceived of pregnant with natural way. Live birth rate was 81% (9/11). Only one patient for each has suffered from fetal growth restriction, maternal anemia, maternal hyperthyroidism, and ulcerative colitis. Vaginal bleeding was seen in five women during the pregnancy. There was no maternal death, stillbirth or neonatal death. The mode of delivery for all live birth pregnancies was C-section. And none of them was emergency C-section. No complication of allograft function was seen after pregnancy.Conclusion: Pregnancy in women with liver transplantation is not associated with poor pregnancy outcomes and complications for both the mother and the neonates when these patients followed up at tertiary referral center with multidisciplinary approaches. Aim: This study was carried out in order to determine patient safety related implementations of nurses working in neonatal intensive care. Materials and Methods: This descriptive study was carried out in Neonatal Intensive Care Units (NICU) in six hospitals in Konya city center between April and May 2012. Ninety-six nurses working in intensive care units participated in this survey. Newborn Patient Safety Survey Form was used in this study. The analysis of the data was done by number, percentage, Kruskall-Wallis, Man-Whitney U and Independent t tests. Results: It was seen that nurses scored most in interventions regarding falling among the fields of patient safety implementations (92.42±6.36). A meaningful difference was found between NICU nurses' state of education and patient safety intervention regarding the medication score average (p<0.05) and also nurses' working years and patient safety intervention regarding identity implementations score average (p<0.05). Conclusion: It was determined that nurses implemented interventions regarding falling most among patient safety implementations. It was seen that patient safety implementations of nurses were related to their state of education, working years, the number of patients cared for daily, and also receiving patient safety training. Background: To present the differences in prenatal, labor and neonatal outcomes for Syrian refugees and Turkish citizens. Methods: Between January 2013 and December 2016, all patients in our hospital were screened retrospectively. Totally, 17 000 pregnant women included in the study were divided into three groups: group 1: 4802 pregnant in Syrian refugees group; group 2: 6752 pregnant in the low-income Turkish citizens (LI groups); and group 3: 5446 women in high-income Turkish citizens (HI groups). The groups were compared for demographic parameters, prenatal, labor and postnatal results. Results: Age, gestational week, birth weights, antenatal follow-up, antenatal iron supplementation and prenatal hemoglobin (Hb) values were significantly lower in the Syrian refugee group (P < 0.001). Only moderate preterm delivery and moderate low birth weight were higher in the refugee group (P = 0.023 and P = 0.001). Stillbirth rates were similar in all three groups (P = 0.203), but all other neonatal complications were higher in the Turkish citizens group. Conclusion: In comparison to non-refugee control patients, adverse perinatal outcomes were not observed in pregnant refugees. The refugee health policies of the Republic of Turkey seem to be working. However, further larger multicenter studies may provide more convincing data about obstetric outcomes and health results in the Syrian refugee population. [ABSTRACT FROM AUTHOR]

Objectives: To describe the effect of extremely advanced maternal age (EAMA) on maternal/neonatal outcomes.Methods: This was a case-control study in which 127 women ≥40 years at the time of delivery out of 2853 singleton hospital deliveries in Ondokuz Mayis University between 1 January 2008 and 31 August 2010 constituted the study group. One hundred and twenty-seven else were chosen randomly out of 2412, 21-35 years old women, via a computer system as controls. Demographic features of 254 mothers and infants as well as maternal and neonatal complications were recorded.Results: Mean maternal age was 41.5 ± 1.9 (40-49) years in EAMA group and 28.9 ± 4.2 (21-35) years in controls. Primigravidity was 19.6% in the EAMA group, whereas 37.8% in controls (p = 0.003). No difference was found between groups according to route of delivery, stillbirth, preterm birth, congenital abnormalities, gender of babies, NICU admission and respiratory problems (for all p > 0.05). A 5th min Apgar score <7 was more frequent in babies born to EAMA mothers compared to controls (9.8% versus 4.9%, p = 0.004).Conclusion: The present study shows that EAMA mothers and their offsprings have similar peri and neonatal risks compared to younger mothers, except lower 5th minute Apgar scores. We conclude that with good perinatal care, EAMA women and their babies can pass through the perinatal period with similar risks of younger women. [ABSTRACT FROM AUTHOR]

In Islamic religion, daytime fasting during the month called Ramadan is an annual practice. In this study, we aimed to investigate the effect of Ramadan fasting and maternal hypoalbuminaemia on neonatal growth parameters. A prospective case–control study was conducted in Diyarbakir and Istanbul, Turkey. The sample size of fasting group was 168 and that of non-fasting group was 170. Demographic characteristics, obstetrics ultrasonographic findings and laboratory parameters of the participants were recorded. Neonatal anthropometric parameters and placental weight were noted. The mean placental weight was significantly higher in the fasting group (p =0.037). Also, in the fasting group, pregnant women with hypoalbuminaemia had significantly higher placental weight (p =0.009). In conclusion, the mean placental weight in the fasting group was significantly higher. Also a significant correlation between placental weight and maternal serum albumin level was observed in the fasting group. [ABSTRACT FROM PUBLISHER]

Background: Infant mortality rate (IMR) and neonatal mortality rate (NMR) are accepted as good indicators to measure the health status of a nation. This report describes recent declines in IMR and NMR in Turkey.Methods: Data on infants who died before 12 months of life were obtained from the Infant Mortality Monitoring System of Ministry of Health of Turkey between 2007 and 2012. A total of 94,038 infant deaths were evaluated.Results: Turkey IMR and NMR exhibited a marked decline from 2007 (16.4 and 12.2) to 2010 (10.1 and 6.6) and then plateaued in 2012 (9.7 and 6.3), despite regional differences. Prematurity, congenital anomalies and congenital heart diseases (CHD) were the three most common causes of infant deaths between 2007 and 2012. While the rates of respiratory distress syndrome (RDS), sudden infant death syndrome (SIDS), and metabolic diseases increased, the rates of congenital anomalies and birth injuries decreased. IMR and NMR significantly increased with the number of infants per paediatrician, per doctor, and per midwife, while was decreasing with the increased rate of hospital birth, caesarean delivery, antenatal care, infant follow-up, and staff trained within the Neonatal Resuscitation Programme (NRP).Conclusion: From 2007-2012, Turkey showed remarkable encouraging advances in reducing IMR and NMR. Any interventions aimed at further reductions in IMR and NMR should target the common causes of death and defined risk factors especially in socioeconomically disadvantaged regions. Purpose: Vitamin B12 deficiency in infancy can cause severe demyelinating central nervous system disorder. Most cases are exclusively breast-fed infants born to mothers with deficient vitamin B12 stores. As maternal vitamin B12 stores are the main determinant of neonatal vitamin B12 status, we aimed to determine the vitamin B12 status of pregnant women at term in our region. Material and Methods: Serum vitamin B12, folate and plasma homocysteine concentrations in addition to routine testings were measured 1-24 hour(s) before delivery in 62 pregnant women. Besides, infants born to mothers with laboratory evidence of vitamin B12 deficiency were referred to pediatric hematology unit for evaluation. Results: Thirty-six (58.1%) women had a low serum vitamin B12 level (<200 pg/ml). The mean plasma homocysteine was found as 6.82 ± 2.38 µmol/L. Among the 36 neonates whose mothers had a low serum vitamin B12 before delivery, 20 were admitted for examination. Plasma hcy level was not available in four infants. In the remaining 16, 13 (81.3%) had both a decreased vitamin B12 level (<259 pg/ml), and an elevated homocysteine concentration (>9.99 µmol/L). All of the 20 infants examined had normal neurodevelopmental findings. Vitamin B12 supplementation resulted in normalization of vitamin B12 and homocysteine levels in all the cases. Conclusion: The frequency of neonatal vitamin B12 deficiency secondary to maternal deficiency may be higher than thought in Samsun province of Turkey. Large-scale screening studies may reveal its prevalence more accurately, and may help in taking preventive measurements more effectively. [ABSTRACT FROM AUTHOR]

Multiple births and related complications have increased with the widespread use of assisted reproductive techniques (ART) in recent years. The aim of this multicenter study of the Turkish Neonatal Society was to evaluate the effects of ART on neonatal morbidity and mortality in Turkey. Between February-December 2006, data from 18 different centers were prospectively collected and analyzed. Of the 6356 newborns recruited in this study, 793 infants (12.5%) were conceived with ART. Eighty-seven percent of the study population was singleton, 10.9% was twin and 1.4% was triplet. Sixty-two percent of the multiples were conceived with ART. Prematurity, multiple births and very low birth weight were more frequent in the ART group. The frequencies of neonatal intensive care unit (NICU) admissions and surfactant and mechanical ventilation along with morbidities such as intraventricular hemorrhage, patent ductus arteriosus, sepsis, and mortality were higher in ART infants. ART-induced pregnancies carry higher risks of neonatal morbidity and mortality. Couples considering ART should be informed about the possible maternal and neonatal risks of ART and multiple gestations. [ABSTRACT FROM AUTHOR]

Abstract: Objective: In this review, we have presented the data of our National Newborn Hearing Screening Program (NNHSP) with total 764,352 newborns those screened in last five years. Methods: National Newborn Hearing Screening Program (NNHSP) has been conducted in Turkey since the year 2003. National Newborn Hearing Screening Program (NNHSP) had begun at the end of 2003 only in 1 center. After birth, in the third day, Transient Evoked Otoacoustic Emissions (TEOAEs) test criteria and if necessary, auditory brain response (ABR) testing evaluation methods were applied to newborn. The children diagnosed with hearing loss were further referred for advanced treatment and rehabilitation to advanced audiologic centers. Results: After five years of carrying out the program (between 2004 and 2008) a total number of 764,352 newborns were screened for hearing impairment. In the year 2008, National Newborn Hearing Screening Program (NNHSP) had given the chance for 2136 children with various types of hearing loss (320 with unilateral and 417 with bilateral hearing loss) to detect and refer to more experienced centers for further treatment. Conclusions: Our results indicate that the necessity of newborn hearing screening is an indispensable issue. We have been targeted to develop National Newborn Hearing Screening Program (NNHSP) till given chance to access for every newborn in Turkey in next five years. [Copyright &y& Elsevier]

Background: Prenatal diagnosis ameliorates some preoperative and postoperative outcomes in critical congenital heart disease (CHD). Despite large variability among anatomical defect types, nearly half of them are diagnosed antenatally. We aimed to investigate the effect of the antenatal diagnosis on prereferral mortality of infants with critical CHD in a center without cardiovascular surgery clinic. Methods: Medical records of the neonates who were diagnosed with critical CHD between the years 2010 and 2016 in Zekai Tahir Burak Women's health Education and Research Hospital were retrospectively reviewed for the study. Patients were divided in two groups as prenatal and postnatal regarding the time of diagnosis. Groups were compared in terms of demographical, clinical characteristics, and prereferral mortality rates. Results: Seventy seven neonates were included in the study. Of those, 39 (50%) infants had prenatal diagnosis. Most common types of CHD were tetralogy of Fallot (TOF) with pulmonary atresia, hypoplastic left heart syndrome (HLHS), and transposition of the great arteries (TGA), respectively. Demographical and clinical characteristics were similar between the groups. Minor congenital anomalies were more common in the patients with prenatal diagnosis (41 versus 18%, p  = .021). Blood gas parameters were similar except pCO 2 levels, which were slightly lower in the postnatal diagnosis group ( p  = .048). There was no difference with regard to prereferral mortality between prenatal and postnatal diagnosed infants. Conclusions: Prenatal diagnosis may not be always associated with improved survival in critical CHD in a center without immediate surgical intervention opportunity.

Aim: Newborns with inborn errors of metabolism can present with hyperammonaemic coma. In this study, we evaluated the effect of peritoneal dialysis on plasma ammonium levels and on the short-term outcome in neonatal patients with urea cycle defects and organic acidaemia., Methods: Data from infants with hyperammonaemia due to urea cycle defects or organic acidaemia treated with dialysis were collected and retrospectively analyzed. The results of patient groups (group I, survived; and group II, died) were compared., Results: Fourteen neonates were enrolled in this study. In group I, plasma ammonium levels before dialysis were median (IQR) 1652 μg/dL (1165-2098 μg/dL); in group II, they were 1289 μg/dL (1070-5550 μg/dL). There was no statistically significant difference. Urea cycle defects were diagnosed in eight, and organic acidaemia in six patients. The duration of a blood ammonia level >200 μg/dL was longer in group II (P = 0.04). A <60.8% decline in the ammonia level from the beginning of dialysis to the 12th hour of dialysis carried a 3.33-fold higher risk of mortality, when compared with a greater decline. Five patients with urea cycle defects, and one with organic acidaemia, died. The mortality risk was 8.33-fold (95% CI = 0.63-90.86) higher for patients with urea cycle defects than for those with organic acidaemia., Conclusion: In patients with hyperammonaemia treated with peritoneal dialysis, the rate of ammonia removal and the underlying aetiology appear to be important prognostic factors. Objective: Platelet mass index (PMI) is related to the platelet functionality. The aim of this study was to evaluate the correlation between PMI and the occurrence of various inflammation-related morbidities of prematurity, such as bronchopulmonary dysplasia (BPD), necrotizing enterocolitis (NEC), retinopathy of prematurity (ROP), intraventricular hemorrhage (IVH) and sepsis in very low-birth weight (VLBW) infants., Methods: This retrospective analysis of VLBW infants admitted to a level 3 neonatal intensive care unit from October 2012 and 2014, n = 330. Platelet mass was calculated and recorded on the day of birth and between 3 and 7 days (second measure) for each patient. Statistical analysis included analysis of paired samples t test and independent samples t test., Result: Among VLBW neonates, PMI values were lower in infants with ROP (p = 0.016), BPD (p = 0.002), IVH (p = 0.018) and NEC (p = 0.011) when compared with the control group in the second measurement., Conclusions: In this study, we found that premature infants with BPD, NEC, ROP, IVH and sepsis had lower PMI levels in early postnatal life than infants without these diseases. This might be associated with the inflammatory process.
