Abstract: Abstract Background Natalizumab and fingolimod are used as high-efficacy treatments in relapsing–remitting multiple sclerosis. Several observational studies comparing these two drugs have shown variable results, using different methods to control treatment indication bias and manage censoring. The objective of this empirical study was to elucidate the impact of methods of causal inference on the results of comparative effectiveness studies. Methods Data from three observational multiple sclerosis registries (MSBase, the Danish MS Registry and French OFSEP registry) were combined. Four clinical outcomes were studied. Propensity scores were used to match or weigh the compared groups, allowing for estimating average treatment effect for treated or average treatment effect for the entire population. Analyses were conducted both in intention-to-treat and per-protocol frameworks. The impact of the positivity assumption was also assessed. Results Overall, 5,148 relapsing–remitting multiple sclerosis patients were included. In this well-powered sample, the 95% confidence intervals of the estimates overlapped widely. Propensity scores weighting and propensity scores matching procedures led to consistent results. Some differences were observed between average treatment effect for the entire population and average treatment effect for treated estimates. Intention-to-treat analyses were more conservative than per-protocol analyses. The most pronounced irregularities in outcomes and propensity scores were introduced by violation of the positivity assumption. Conclusions This applied study elucidates the influence of methodological decisions on the results of comparative effectiveness studies of treatments for multiple sclerosis. According to our results, there are no material differences between conclusions obtained with propensity scores matching or propensity scores weighting given that a study is sufficiently powered, models are correctly specified and positivity assumption is fulfilled.

Abstract: International audience; Background: Studies have shown that oral high-dose methylprednisolone (MP) is non-inferior to intravenous MP in treating multiple sclerosis relapses in terms of effectiveness and tolerance. In order to assist with resource allocation and decision-making, its cost-effectiveness must also be assessed. Our objective was to evaluate the cost-utility of per os high-dose MP as well as the cost-savings associated with implementing the strategy.Methods: A cost-utility analysis at 28 days was carried out using data from the French COPOUSEP multicenter, double-blind randomized controlled non-inferiority trial and the statutory health insurance reimbursement database. Costs were calculated using a societal perspective, including both direct and indirect costs. An incremental cost-effectiveness ratio was calculated and bootstrapping methods assessed the uncertainty surrounding the results. An alternative scenario analysis in which MP was administered at home was also carried out. A budgetary impact analysis was carried at five years.Results: In the conditions of the trial (hospitalized patients), there was no significant difference in utilities and costs at 28 days. The incremental cost-effectiveness ratio was €15,360 per quality-adjusted life-year gained. If multiple sclerosis relapses were treated at home, oral MP would be more effective, less costly and associated with annual savings up to 25 million euros for the French healthcare system.Conclusions: Oral MP is cost-effective in the treatment of multiple sclerosis relapses and associated with major savings.

Abstract: Objectives Establish recommendations for the management of UTIs in MS patients. Background Urinary tract infections (UTIs) are common during multiple sclerosis (MS) and are one of the most common comorbidities potentially responsible for deaths from urinary sepsis. Methods The recommendations attempt to answer three main questions about UTIs and MS. The French Group for Recommendations in MS (France4MS) did a systematic review of articles from PubMed and universities databases (01/1980–12/2019). The RAND/UCLA appropriateness method, which has been developed to synthesize the scientific literature and expert opinions on health care topics, was used for reaching a formal agreement. 26 MS experts worked on the full-text review and a group of 70 multidisciplinary health care specialists validated the final evaluation of summarized evidences. Results UTIs are not associated with an increased risk of relapse and permanent worsening of disability. Only febrile UTIs worsen transient disability through the Uhthoff phenomenon. Some immunosuppressive treatments increase the risk of UTIs in MS patients and require special attention especially in case of hypogammaglobulinemia. Experts recommend to treat UTIs in patients with MS, according to recommendations of the general population. Prevention of recurrent UTIs requires stabilization of the neurogenic bladder. In some cases, weekly oral cycling antibiotics can be proposed after specialist advice. Asymptomatic bacteriuria should not be screened for or treated systematically except in special cases (pregnancy and invasive urological procedures). Conclusion Physicians and patients should be aware of the updated recommendations for UTis and MS.

Abstract: International audience; Objective: We aimed to identify timing distortions in production and perception of rhythmic events in patients with idiopathic REM sleep behavior disorder (iRBD) as early markers of Parkinson's disease (PD).Methods: Rhythmic skills, clinical characteristics, dysautonomia, depression, and olfaction were compared in 97 participants, including 21 participants with iRBD, 38 patients with PD, and 38 controls, matched for age, gender, and education level. Rhythmic disturbances can be easily detected with dedicated motor tasks via a tablet application. Rhythm production was tested in two conditions: to examine the ability to generate a spontaneous endogenous rhythm, tapping rate and variability in a finger tapping task without external stimulation was measured, while the ability to synchronize to an external rhythm was tested with finger tapping to external auditory cues. Rhythm perception was measured with a task, in which the participants had to detect a deviation from a regular rhythm. Participants with iRBD had dopamine transporter imaging.Results: Participants with iRBD and PD revealed impaired spontaneous rhythm production and poor rhythm perception compared to controls. Impaired rhythm production was correlated with olfaction deficits, dysautonomia, impaired non-motor aspects of daily living, and dopamine uptake measures.Conclusions: Participants with iRBD show impaired rhythm production and perception; this impairment is correlated with other early markers for PD. Testing rhythmic skills with short and inexpensive tests may be promising for screening for potential future PD in iRBD patients.

The most common aetiology comes from virus but depend on age, location of the patient and co-morbidities. Acute Flaccid paralysis and acute transverse myelitis are considered as emergencies. Differential diagnosis is mainly relapses of autoimmune diseases, which can mimic infectious myelopathies.

Abstract: Objectives To establish recommendations on immunization for patients with multiple sclerosis (MS). Background Vaccines have been suspected in the past to trigger MS and relapses. With the extension of the immunoactive treatment arsenal, other concerns have been raised more recently about an increased risk of infection or a decreased effectiveness of immunization in immunosuppressed patients. Methods The French Group for Recommendations into Multiple Sclerosis (France4MS) performed a systematic search of papers in Medline and other university databases (January 1975–June 2018). The RAND/UCLA appropriateness method was chosen to review the scientific literature and to formalize the degree of agreement among experts on 5 clinical questions related to immunization and MS. Readers from the steering committee conducted a systematic analysis, wrote a critical synthesis and prepared a list of proposals that were evaluated by a rating group of 28 MS experts. The final version of the recommendations was finally reviewed by a reading group of 110 health care professionals and classified as appropriate, inappropriate or uncertain. Results Neurologists should verify the vaccination status as soon as MS is diagnosed and before disease-modifying treatments (DMTs) are introduced. The French vaccination schedule applies to MS patients and seasonal influenza vaccination is recommended. In the case of treatment-induced immunosuppression, MS patients should be informed about the risk of infection and the vaccination standards of the French High Council of Health should be applied. Live attenuated vaccines are contra-indicated in patients recently treated with immunosuppressive drugs, including corticosteroids; other vaccines can be proposed whatever the treatment, but their effectiveness may be partly reduced with some drugs. Conclusion Physicians and patients should be aware of the updated recommendations for immunizations of patients with MS.

Abstract: The Cluster HEritage project with XMM-Newton - Mass Assembly and Thermodynamics at the Endpoint of structure formation (CHEX-MATE) is a three mega-second Multi-Year Heritage Programme to obtain X-ray observations of a minimally-biased, signal-to-noise limited sample of 118 galaxy clusters detected by Planck through the Sunyaev-Zeldovich effect. The programme, described in detail in this paper, aims to study the ultimate products of structure formation in time and mass. It is composed of a census of the most recent objects to have formed (Tier-1: 0.05 < z < 0.2; 2 x 10e14 M_sun < M_500 < 9 x 10e14 M_sun), together with a sample of the highest-mass objects in the Universe (Tier-2: z < 0.6; M_500 > 7.25 x 10e14 M_sun). The programme will yield an accurate vision of the statistical properties of the underlying population, measure how the gas properties are shaped by collapse into the dark matter halo, uncover the provenance of non-gravitational heating, and resolve the major uncertainties in mass determination that limit the use of clusters for cosmological parameter estimation. We will acquire X-ray exposures of uniform depth, designed to obtain individual mass measurements accurate to 15-20% under the hydrostatic assumption. Abstract: Purpose New multiple sclerosis (MS) disease-modifying therapies (DMTs), which exert beneficial effects through prevention of relapse, limitation of disability progression, and improvement of patients’ quality of life, have recently emerged. Nonetheless, these DMTs are not without associated complications (severe adverse events like. progressive multifocal leukoencephalopathy). Patient follow-up requires regular clinical evaluations and close monitoring with magnetic resonance imaging (MRI). Detection of new T2 lesions and potential brain atrophy measurements contribute to the evaluation of treatment effectiveness. Current MRI protocols for MS recommend the acquisition of an annual gadolinium (Gd) enhanced MRI, resulting in administration of high volume of contrast agents over time and Gd accumulation in the brain. Methods A consensus report was established by neuroradiologists and neurologists from the French Observatory of MS, which aimed at reducing the number of Gd injections required during MS patient follow-up. Recommendations The French Observatory of MS recommends the use of macrocyclic Gd enhancement at time of diagnosis, when a new DMT is introduced, at 6-month re-baseline, and when previous scans are unavailable for comparison. Gd administration can be performed as an option in case of relapse or suspicion of intercurrent disease such as progressive multifocal leukoencephalopathy. Other follow-up MRIs do not require contrast enhancement, provided current and previous MRI acquisitions follow the same standardized protocol including 3D FLAIR sequences.

Abstract: Background: Obstetrical analgesia remains a matter of controversy because of the fear of neurotoxicity of local anesthetics on demyelinated fibers or their potential relationship with subsequent relapses. Objective: To assess the impact of neuraxial analgesia on the risk of relapse during the first 3 months post-partum, with a focus on women who experienced relapses during pregnancy. Methods: We analyzed data of women followed-up prospectively during their pregnancies and at least 3 months post-partum, collected in the Pregnancy in Multiple Sclerosis (PRIMS) and Prevention of Post-Partum Relapses with Progestin and Estradiol in Multiple Sclerosis (POPARTMUS) studies between 1992–1995 and 2005–2012, respectively. The association of neuraxial analgesia with the occurrence of a post-partum relapse was estimated by logistic regression analysis. Results: A total of 389 women were included, 215 from PRIMS and 174 from POPARTMUS. In total, 156 women (40%) had neuraxial analgesia. Overall, 24% experienced a relapse during pregnancy and 25% in the 3 months post-partum. Women with a pregnancy relapse were more likely to have a post-partum relapse (odds ratio (OR) = 1.83, p = 0.02), independently of the use of neuraxial analgesia. There was no association between neuraxial analgesia and post-partum relapse (OR = 1.08, p = 0.78). Conclusion: Neuraxial analgesia was not associated with an increased risk of post-partum relapses, whatever multiple sclerosis (MS) activity during pregnancy.

14. Impact du vaccin contre la fièvre jaune sur l’évolution clinique de la sclérose en plaques

Abstract: Introduction Le vaccin contre la fievre jaune est requis pour voyager en zone d’endemie de fievre jaune mais il est deconseille chez les patients atteints de sclerose en plaques (SEP) en raison du risque potentiel de poussees post-vaccinales mis en evidence dans une seule publication incluant 7 patients. Materiels et methodes Evaluer chez les patients SEP recurrentes-remittentes, ayant beneficie d’une vaccination fievre jaune, le risque de survenue d’une activite de la maladie dans les 12 mois suivant la vaccination. Dans cette etude retro-prospective et observationnelle exposes/non-exposes, chaque patient vaccine contre la fievre jaune apres le debut de la maladie SEP a ete apparie a 3 patients SEP sans historique de vaccination contre la fievre jaune et identifies par la base de donnees locale European Database for Multiple Sclerosis (EDMUS). Les criteres d’appariement etaient : l’âge et l’activite de la maladie avant vaccination. Le delai de survenue d’une poussee des exposes et non-exposes a ete analyse par un test log-rank. Un modele de Cox a estime les hazard ratio bruts et ajustes. Resultats Trente et un (20 F/11 H) patients vaccines ont ete inclus. L’âge moyen de debut de la maladie etait de 38 ans et la duree moyenne de la maladie avant la vaccination etait de 10,8 ans. Parmi les patients, 19,35 % avaient une activite clinique l’annee precedant la vaccination, l’Expanded Disability Status Scale (EDSS) moyen etait a 0,78 lors de la vaccination. La moitie des patients n’etaient pas traites pour le SEP au moment de la vaccination (51,6 %), 41,9 % d’entre eux avaient un traitement de 1re ligne et 6,4 % etaient traites par natalizumab. Les resultats complets de cette etude exposes/non-exposes seront disponibles fin fevrier. Conclusion La fievre jaune est une maladie grave potentiellement mortelle, pouvant etre contractee en zones d’endemie (Afrique subsaharienne et Amerique du Sud) et ne disposant pas de traitement. Le vaccin anti-amaril, tres efficace, est exige pour entrer sur le territoire de certains pays en zone d’endemie. Ce vaccin vivant attenue est contre-indique chez les patients traites par immunosuppresseurs. Les resultats de cette etude epidemiologique cas/temoin permettront de faire evoluer les pratiques et les recommandations vaccinales contre la fievre jaune et de conseiller au mieux les patients atteints de SEP se rendant en zone d’endemie.

Abstract: BACKGROUND: The main objective of this study was to explore the patterns of alternative therapy use, in parallel to the standard treatment, in glioma patients in France. MATERIAL AND METHODS: Questionnaires were distributed during visits at hospital and filled in and returned anonymously by adult patients with histologically confirmed glioma and with an ECOG performance score of 0–2. The protocol was approved by the French authorities on April 13th, 2016. Descriptive analyses of the population as well as correlation analysis according to gender, age, WHO grade IV versus other, recurrent disease as declared by the patients versus none, were conducted. RESULTS: Two hundred questionnaires were collected. Patients were male in 65% of the cases. Median age was 46 (range 18–85). Forty-six patients had glioblastoma, 83 patients declared having recurrent disease. Patients declared taking a complementary treatment in 46.5% (mainly vitamins and other food supplements), having modified their diet (ketogenic diet, Servent-Schreiber diet, addition of curcuma) in 48%, and using another alternative medicine approach (mainly magnetism, acupuncture, sophrology), in 34%. In total, 69% of the patients declared using at least one of these approaches. Abstract: Neuromyelitis optica (NMO) is a central nervous system inflammatory autoimmune disease characterized by medullary and/or optical nerve damage. It is rare but life-threatening. Concerning the treatment of NMO, many drugs have been used in background therapy. Some studies have shown efficacy of rituximab (an antiCD20 monoclonal anti-body) either on the reduction of the annual number of exacerbation or the mean score EDSS. In 2013, a Korean team reported a new protocol during which they administered rituximab only when memory B lymphocytes CD27+ were detectable in the bloodstream. In our patient, institution of this protocol led to clinical benefit with a major decrease in the EDSS score over time (7 in August 2012 vs. 1 in October 2015), a reduction of the total administered dose (4g in 2013 vs. 1.375g in 2014 vs. 0g in 2015) and side effects. Compared with the rate of theoretical administration, health expenditure savings reached 1700 Euros per month over the 11-month treatment. Monitoring therapeutic response markers with memory B lymphocyte counts appear to be an efficient cost-effective way to measure clinical efficiency, reduce total doses, and limit side effects.

Abstract: SummaryBackground High doses of intravenous methylprednisolone are recommended to treat relapses in patients with multiple sclerosis, but can be inconvenient and expensive. We aimed to assess whether oral administration of high-dose methylprednisolone was non-inferior to intravenous administration. Methods We did this multicentre, double-blind, randomised, controlled, non-inferiority trial at 13 centres for multiple sclerosis in France. We enrolled patients aged 18–55 years with relapsing-remitting multiple sclerosis who reported a relapse within the previous 15 days that caused an increase of at least one point in one or more scores on the Kurtzke Functional System Scale. With use of a computer-generated randomisation list and in blocks of four, we randomly assigned (1:1) patients to either oral or intravenous methylprednisolone, 1000 mg, once a day for 3 days. Patients, treating physicians and nurses, and data and outcome assessors were all masked to treatment allocation, which was achieved with the use of saline solution and placebo capsules. The primary endpoint was the proportion of patients who had improved by day 28 (decrease of at least one point in most affected score on Kurtzke Functional System Scale), without need for retreatment with corticosteroids, in the per-protocol population. The trial was powered to assess non-inferiority of oral compared with intravenous methylprednisolone with a predetermined non-inferiority margin of 15%. This trial is registered with ClinicalTrials.gov, number NCT00984984. Findings Between Jan 29, 2008, and June 14, 2013, we screened 200 patients and enrolled 199. We randomly assigned 100 patients to oral methylprednisolone and 99 patients to intravenous methylprednisolone with a mean time from relapse onset to treatment of 7·0 days (SD 3·6) and 7·4 days (3·9), respectively. In the per-protocol population, 66 (81%) of 82 patients in the oral group and 72 (80%) of 90 patients in the intravenous group achieved the primary endpoint (absolute treatment difference 0·5%, 90% CI −9·5 to 10·4). Rates of adverse events were similar, but insomnia was more frequently reported in the oral group (77 [77%]) than in the intravenous group (63 [64%]). Interpretation Oral administration of high-dose methylprednisolone for 3 days was not inferior to intravenous administration for improvement of disability scores 1 month after treatment and had a similar safety profile. This finding could have implications for access to treatment, patient comfort, and cost, but indication should always be properly considered by clinicians. Abstract: Rhythmic auditory cues can immediately improve gait in Parkinson’s disease. However, this effect varies considerably across patients. The factors associated with this individual variability are not known to date. Patients’ rhythmic abilities and musicality (e.g., perceptual and singing abilities, emotional response to music, and musical training) may foster a positive response to rhythmic cues. To examine this hypothesis, we measured gait at baseline and with rhythmic cues in 39 non-demented patients with Parkinson’s disease and 39 matched healthy controls. Cognition, rhythmic abilities and general musicality were assessed. A response to cueing was qualified as positive when the stimulation led to a clinically meaningful increase in gait speed. We observed that patients with positive response to cueing (n = 17) were more musically trained, aligned more often their steps to the rhythmic cues while walking, and showed better music perception as well as poorer cognitive flexibility than patients with non-positive response (n = 22). Gait performance with rhythmic cues worsened in six patients. We concluded that rhythmic and musical skills, which can be modulated by musical training, may increase beneficial effects of rhythmic auditory cueing in Parkinson’s disease. Screening patients in terms of musical/rhythmic abilities and musical training may allow teasing apart patients who are likely to benefit from cueing from those who may worsen their performance due to the stimulation., Musicality in Parkinson’s disease: Trained to follow the beat Listening to rhythmic auditory cues improves the ability to walk in patients with Parkinson’s disease (PD). Previous studies have shown that music can help patients with neurological disorders synchronize their movements to a beat. An international study led by Valérie Cochen De Cock at Clinique Beau Soleil in Montpellier (France) and Simone Dalla Bella at the International Laboratory For Brain, Music and Sound Research (BRAMS) in Montreal (Canada), measured gait speed in 39 patients with PD without dementia in response to rhythmic stimulation. Twenty-two patients increased their gait speed by spontaneously synchronizing their steps to the beat. The remaining 17 patients showed no effect or significant worsening of gait performance (e.g., smaller step length). The patients who benefited the most from rhythmic cues exhibited better rhythmic skills and were more musical than the others. Assessing musical abilities may serve to identify patients who are likely to benefit from this music-based intervention and may foster individualization of the treatment.

Abstract: Contexte L’indicage rythmique auditif peut ameliorer la marche de facon immediate au cours de la maladie de Parkinson (MP). Cependant, cet effet varie considerablement parmi les patients, pouvant meme parfois etre deletere chez certains. Les facteurs associes a la variabilite de cette reponse ne sont pas connus a ce jour. Les competences rythmiques et musicales comme le fait d’avoir « l’oreille musicale », de chanter juste, d’etre sensible a la musique ou d’avoir recu une formation musicale pourraient etre des facteurs de bonne reponse a cet indicage. Methodes La marche spontanee, puis avec indicage a ete mesuree chez 39 patients parkinsoniens non dements et 39 sujets sains apparies. Les parametres spatiotemporels de la marche, la cognition, les competences rythmiques et la musicalite de facon generale ont ete explores. La reponse a l’indicage a ete consideree comme positive quand la vitesse augmentait significativement. Resultats Les patients avec une reponse positive a l’indicage (n = 17) avaient une atteinte de la marche spontanee plus severe, une moins bonne flexibilite mentale aux tests cognitifs, une meilleure synchronisation du pas sur le rythme de l’indicage lors de la marche et de meilleures competences musicales avec en particulier une meilleure capacite a detecter le rythme de la musique et une meilleure formation musicale que les patients sans reponse positive (n = 22). Conclusions Les competences rythmiques et musicales, qui peuvent etre modulees par la formation musicale, auraient un effet benefique sur la reponse a l’indicage auditif au cours de la MP probablement via un renforcement de reseaux neuronaux epargnes. La selection de patients en fonctions de leur formation musicale et de leur capacite a percevoir le rythme pourrait permettre d’identifier ceux qui pourront beneficier de l’indicage de ceux que risqueraient d’avoir un effet deletere.

Abstract: BACKGROUND Accurate allergen quantification is needed to document the consistency of allergen extracts used for immunotherapy. Herein, we characterize the epitope specificities of two monoclonal antibodies used in an ELISA for the quantification of the major birch pollen allergen Bet v 1, established as a reference by the BSP090 European project. METHODS The ability of mAbs 5B4 and 6H4 to recognize Bet v 1 isoforms was addressed by immunochromatography. The capacity of each mAb to compete with patients' IgE for binding to Bet v 1 was measured by ELISA inhibition. Epitope mapping was performed by pepscan analysis, site-directed mutagenesis, and hydrogen/deuterium exchange-mass spectrometry. RESULTS The 5B4 epitope corresponds to a peptide sequence (I56-K68) overlapping with the binding sites of patients' serum IgEs. Mutation of residues P59, E60, and K65 abolishes 5B4 binding to Bet v 1 and reduces the level of IgE recognition. In contrast, 6H4 recognizes a conformational epitope lying opposite to the 5B4 binding site, involving residues located in segments I44-K55 and R70-F79. Substitution of E45 reduces the binding capacity of 6H4, confirming that it is critical for the interaction. Both mAbs interact with >90% of Bet v 1 content present in the birch pollen extract, while displaying a weak cross-reactivity with other allergens of the PR-10 family. CONCLUSIONS MAbs 5B4 and 6H4 recognize structurally distinct epitopes present in the vast majority of Bet v 1 isoforms. These results support the relevance as a reference method of the Bet v 1-specific quantitative ELISA adopted by the European Pharmacopoeia.

Abstract: Background Among women aged 50 to 59 years at baseline in the Women's Health Initiative (WHI) Estrogen‐Alone (E‐Alone) trial, randomization to conjugated equine estrogen‐alone versus placebo was associated with lower risk of myocardial infarction and mortality, and, in an ancillary study, the WHI‐CACS (WHI Coronary Artery Calcification Study) with lower CAC, measured by cardiac computed tomography ≈8.7 years after baseline randomization. We hypothesized that higher CAC would be related to post‐trial coronary heart disease (CHD), cardiovascular disease (CVD), and total mortality, independent of baseline randomization or risk factors. Methods and Results WHI‐CACS participants (n=1020) were followed ≈8 years from computed tomography scan in 2005 (mean age=64.4) through 2013 for incident CHD (myocardial infarction and fatal CHD, n=17), CVD (n=69), and total mortality (n=55). Incident CHD and CVD analyses excluded women with CVD before scan (n=89). Women with CAC=0 (n=54%) had very low age‐adjusted rates/1000 person‐years of CHD (0.91), CVD (5.56), and mortality (3.45). In comparison, rates were ≈2‐fold higher for women with any CAC (>0). Associations were not modified by baseline randomization to conjugated equine estrogen–alone versus placebo. Adjusted for baseline randomization and risk factors, the hazard ratio (95% confidence interval) for CAC >100 (19%) was 4.06 (2.11, 7.80) for CVD and 2.70 (1.26, 5.79) for mortality. Conclusions Among a subset of postmenopausal women aged 50 to 59 years at baseline in the WHI E‐Alone Trial, CAC at mean age of 64 years was strongly related to incident CHD, CVD, and to total mortality over ≈8 years, independent of baseline randomization to conjugated equine estrogen–alone versus placebo or CVD risk factors. Abstract: A case of episodic amnesia with impairment of time perception is described; it illustrates the link between time perception and autobiographical memory. This woman suffered from a Sheehan syndrome with anoxia at the age of 36 and since that date has had a strong and isolated difficulty to estimate the date and duration of events in a range of weeks, months or years. Conversely, short duration time spans are correctly evaluated. The patient's complaints also involve episodic memory. She reports many events from her biography very imprecisely while the semantic autobiographical data are preserved. The patient has difficulty in recalling the date of public events and the period of celebrity of well-known people. That observation confirms the specificity of time organization for long periods and the link with the episodic memory where the context of the dating task is crucial. The results are discussed in reference to autobiographical memory that involves mental wandering in time-space and the constitution of self over a time continuum.

Abstract: Resume Introduction La presence d’une leucoencephalopathie vasculaire sur l’IRM cerebrale traduit le plus souvent une microangiopathie cerebrale secondaire a la presence de facteurs de risque vasculaire essentiellement representes par l’âge et l’hypertension arterielle. Chez des sujets jeunes et sans facteur de risque vasculaire, une maladie de CADASIL doit etre recherchee. L’objectif de ce travail etait de preciser les caracteristiques cliniques et neuroradiologiques de huit patients sans mutation NOTCH3 dont la severite de la leucoencephalopathie vasculaire n’etait pas expliquee par les facteurs de risque vasculaire. Patients et methode Nous rapportons une serie multicentrique de huit patients. Les criteres d’inclusion etaient leucoencephalopathie stade 3 de la classification de Fazekas, âge inferieur a 70 ans, et absence de mutation du gene NOTCH3 . La presence de facteurs de risque vasculaire severes ou d’atherosclerose averee etait un critere d’exclusion. Les donnees cliniques, neuroradiologiques et les resultats des autres explorations vasculaires ont ete analyses. Resultats Huit patients (quatre femmes) ont ete inclus, cinq n’avaient aucun facteur de risque vasculaire. L’âge de debut moyen etait de 59,5 ans. A l’exception de deux malades presentant un accident ischemique de type lacunaire, le debut des symptomes etait progressif caracterise le plus souvent par une astasie-abasie. Six patients se sont aggraves, avec apparition progressive de troubles cognitifs et de la marche. Un d’entre eux notamment, devenu grabataire, est decede a l’âge de 66 ans de son atteinte neurologique, soit quatre ans apres le debut des symptomes. L’IRM cerebrale objectivait une atrophie precoce chez cinq malades sur huit ; l’atteinte des lobes temporaux (deux sur huit) et des capsules externes (cinq sur huit, asymetrique le plus souvent) etait peu marquee. Quatre patients n’avaient aucune localisation d’atherome. Seul un patient avait une microangiopathie retinienne. Une HTA a ete decouverte chez deux patients. Conclusion La decouverte d’une leucoencephalopathie vasculaire chez un sujet jeune sans facteur de risque vasculaire doit faire realiser un bilan complet a la recherche notamment d’une HTA meconnue afin d’instaurer un traitement correcteur. La presentation clinique est le plus souvent caracterisee par une astasie-abasie. Dans ce contexte, l’IRM cerebrale ne permettant pas de distinguer avec certitude une maladie de CADASIL d’une microangiopathie acquise, le sequencage de 23 exons du gene NOTCH3 doit etre realise.

26. Les traitements de première ligne dans la sclérose en plaques

Abstract: Resume Les traitements de fond specifiquement prescrits aux patients ayant une sclerose en plaques (SEP) en premiere intention, sont pour la plupart de la famille des immunomodulateurs (Interferon β et acetate de glatiramere) et plus rarement, des immunosuppresseurs (mitoxantrone, natalizumab et fingolimod). La plupart de ces produits sont disponibles dans le monde entier et des medicaments biosimilaires commencent a etre commercialises. Les traitements par immunomodulateurs reduisent le taux de poussees d’environ un tiers et le taux d’apparition de nouvelles lesions en IRM d’environ deux tiers. Pour certains produits, les etudes ont egalement montre une diminution de la progression du handicap mesuree par l’EDSS et un ralentissement de l’atrophie cerebrale evaluee avec la fraction parenchymateuse. L’evolution de la strategie therapeutique dans les pathologies demyelinisantes du systeme nerveux central de l’adulte va vers une prescription de plus en plus precoce apres le diagnostic de la maladie et les immunomodulateurs ont maintenant pour la plupart la possibilite d’etre prescrits des le premier evenement demyelinisant pour peu que les patients aient les criteres de haut risque de developper une SEP definis par des criteres IRM. Cette revue decrit les resultats des etudes concernant les immunomodulateurs dans la SEP et leur effet sur les principaux parametres de la maladie, leur tolerance et les voies de developpement en premiere ligne de traitement.

Abstract: Background: In multiple sclerosis (MS), the relapse rate declines during pregnancy and increases during the first three months post-partum before returning to the pre-pregnancy rate. It is unknown whether pregnancy impacts the risk of clinical conversion in those within the presymptomatic period. Objectives: We investigate the impact of pregnancy on developing a clinical event in women diagnosed with radiologically isolated syndrome (RIS). Methods: All women with RIS underwent clinical and radiological assessments as part of an observational, prospective, longitudinal study. Clinical and MRI outcomes were analyzed during and after pregnancy. Subjects who became pregnant were compared with an age-matched female RIS group who did not become pregnant during the same follow-up period. Results: A total of 60 women with RIS were followed for up to seven years. Among them, seven became pregnant and were compared with 53 age-matched control women with RIS who did not become pregnant during the observation period. A significantly shorter time of conversion to the first neurological event was observed in the pregnant group [15.3 months (10–18)] compared with the non-pregnant controls [35.7 months (8–76)], yielding an absolute difference of 20.4 months ( pConclusions: The risk for clinical conversion from RIS to a clinical event and new MRI disease activity seems to be influenced by pregnancy. Pregnancy related physiological changes could operate as early as the presymptomatic period in patients with MS.

Abstract: Resume Introduction Les therapies par anti-TNF alpha (ATα) sont prescrites dans certaines maladies inflammatoires rhumatologiques et digestives. Des evenements indesirables neurologiques ont ete rapportes. Observation Une femme âgee de 35 ans, suivie pour un rhumatisme psoriasique, a presente une nevrite optique retro-bulbaire gauche quatre mois apres instauration d’un traitement par ATα. L’IRM initiale remplissait d’emblee les criteres de Barkhof et l’analyse du liquide cephalorachidien (LCR) revelait des bandes oligoclonales surnumeraires. Apres arret des ATα, l’evolution a ete marquee par la survenue d’un deuxieme episode clinique confirmant le diagnostic de sclerose en plaques (SEP). La patiente est actuellement traitee par rituximab pour sa pathologie rhumatologique avec une evolution favorable de ses deux maladies auto-immunes. Commentaires Des evenements neurologiques sont rapportes sous ATα. Le tableau clinique est le plus souvent evocateur d’une pathologie inflammatoire du systeme nerveux central. L’IRM peut montrer des hypersignaux de la substance blanche en T2 suggestifs de pathologie demyelinisante ou d’allure aspecifique. L’etude du LCR semble etre le meilleur element predictif de l’evolution ulterieure vers une SEP.

Abstract: Inelastic neutron scattering for (12)C, (58)Fe, (89)Y and (208)Pb have been measured at 96 MeV at the The Svedberg Laboratory in Uppsala and double-differential cross sections are reported. Data cover an excitation energy range of 0-45 MeV and the angular intervals are 28 - 58 degrees for (12)C, 26 - 65 degrees for (58)Fe and 26 - 52 degrees for (89)Y and (208)Pb. In this experiment, neutron detection is based on conversion to protons in an active scintillator converter. An analysis technique in which the neutron spectra have been obtained through a folding procedure using the response of the detector system has been used. The results are compared to and are in reasonable agreement with several model predictions and with inelastic neutron scattering data at 65 MeV from University of California, Davis, USA.

Abstract: Of the 1603 civil liability cases filed between 2002 and 2012 related to upper-limb surgery, we found 20 proceedings (11 in court and 9 in front of the French CCI [commission for medical accidents]) that involved a total of 24 physicians: three proceedings involved multiple parties for a single patient and implicated two physicians in two cases and three physicians in one case. The grounds for liability being invoked varied greatly, but a good many of these cases concerned outcomes that were deemed unsatisfactory or insufficient in light of the initial injury. Information is always difficult to convey in an emergency setting, but is not generally disputed. However, the information process must be completed during the postoperative period and the patient informed of the injury assessment, expected results, potential complications and the possibility of surgical revision in the medium term. An expert found grounds for physician liability and breach of best practices in only three cases. In summary, this study shows that trauma-related flexor tendon injuries are rarely grounds for indictment of physicians before courts of civil jurisdiction (12 per 1000) and findings of liability for breach of best practices are even more rare (2 per 1000).

Abstract: The stellar occultation spectrometer GOMOS (Global Ozone Monitoring by Occultation of Stars) on ESA's Envisat satellite measures vertical profiles O3, NO2 and NO3 with a high long-term stability due to the self-calibrating nature of the technique. More than 6 years of GOMOS data from August 2002 to end 2008 have been analysed to study the inter-annual variation of O3, NO2 and NO3 in the tropics. It is shown that the QBO of the equatorial wind induces variations in the local concentration larger than 10% for O3 and larger than 25% for NO2. Quasi-Biennial Oscillation signals can be found in the evolution of the three constituents up to at least 45 km. We found that NO3 is positively correlated with temperature up to 40 km in the region where it is in chemical equilibrium with O3. Above 40 km, NO3 is no more in equilibrium during night and its concentration is correlated with both O3 and NO2. For O3 and NO2, our results confirm the existence of a transition from a dynamical control of O3 below 28 km with O3 correlated with NO2 and temperature and a chemical/temperature control between 28 and 38 km with O3 anti-correlated with NO2 and temperature. Above 38 km and up to 50 km a regime never described before is found with both O3 and NO2 anti-correlated with temperature. For the NO2/temperature anti-correlation, our proposed explanation is the modulation of the N2O ascent in the upper stratosphere by the QBO and the modulation of the Brewer-Dobson circulation. The oxidation of N2O is the main source of NOy in this altitude region. An enhancement of the ascending motion will cool adiabatically the atmosphere and will increase the amount of N2O concentration available for NOy formation.

Abstract: Mood disturbance and depression occur in more than half of multiple sclerosis patients. It can be diagnosed in established cases, although it can be present early in the disease course. Depression is not clearly related to specific MS brain lesions but is frequently associated with other symptoms such as fatigue or cognitive impairment. Depression in MS is more frequent than in an age-matched population or in other chronic diseases. There seems to be an elevated risk of suicide. Immunomodulatory treatments do not interfere with depression, but the screening for depression and monitoring of mood should be a feature of the medical management of all patients with MS, regardless of whether they are receiving interferon. MS patients respond well to treatment, either psychotherapy or antidepressants.

Abstract: The organization of episodic and semantic memory is currently debated, and especially the rule of the hippocampus in the functioning of these two systems. Since theories derived from the observation of the famous patient HM, that highlighted the involvement of this structure in these two systems, numerous studies questioned the implication of the hippocampus in learning a new semantic knowledge. Among these studies, we found Vargha-Kadem's cases of developmental amnesia. In spite of their clear hippocampal atrophy and a massive impairment of episodic memory, these children were able to acquire de novo new semantic knowledge. In the present paper, we describe a new case of developmental amnesia characteristic of this syndrome. In conclusion, the whole published data question the implication of the hippocampus in every semantic learning and suggest the existence of a neocortical network, slower and that needs more exposures to semantic stimuli than the hippocampal one, which can supply a massive hippocampal impairment.

Abstract: Semantic dementia (SD) is a syndrome of progressive loss of semantic knowledge for objects and people. International criteria propose that SD be included in the frontotemporal lobar degeneration syndromes, with progressive non-fluent aphasia and frontotemporal dementia (FTD). However, several related syndromes have been defined that clinically and conceptually share both similarities and differences with SD: fluent progressive aphasia, progressive prosopagnosia, temporal variant of FTD. In order to establish a French consensus for the diagnosis and modalities of evaluation and follow-up of SD, a working group, composed of neurologists, neuropsychologists and speech-therapists, was established by the Groupe de reflexion sur les evaluations cognitives (GRECO). New criteria were elaborated, based on clinical, neuropsychological, and imaging data. They define typical and atypical forms of SD. A diagnosis of typical SD relies on an isolated and progressive loss of semantic knowledge, attested by a deficit of word comprehension and a deficit of objects and/or people identification, with imaging showing temporal atrophy and/or hypometabolism. SD is atypical if the deficit of semantic knowledge is present only within a single modality (verbal versus visual), or if non-semantic deficits (mild and not present at onset) and/or neurological signs, are associated with the semantic loss.

Abstract: An improvement of the radial-velocity measurement accuracy is crucial for the detection of tiny stellar oscil- lations and exoplanets. Through the analysis of week-long extensive observations of solar-type stars (Procyon in 2000, 2002, and 2006/2007 andCet in 2002 and 2006/2007), we have carefully examined, revised, and finely tuned the widely used multiple Gaussian IP fitting method for the spectrograph, HIDES. By determining a necessary and sufficient number of free parameters in the model as well as introducing an iterative process in the radial-velocity analysis, we can reach a precision of below 3 m s � 1 , which is much smaller than the precision of 6 m s � 1 officially announced so far for HIDES. We also make our technique refined for the 2002 McDonald Procyon data. Even with our revised method, slow radial velocity variations with an amplitude of about 10 m s � 1 are left in the Procyon data. We emphasize that it is neither due to particular observing instruments nor radial-velocity analysis, and thus could be due to stellar origin. The analysis presented here makes the foundations of our next scientific analysis of the radial-velocity variations of Procyon, which will be presented in our forthcoming papers.

Abstract: Cholesterol in the plasma membrane of eukaryotic cells contributes to modulating the functions and signalling pathways of numerous transmembrane proteins, including G protein Coupled Receptors (GPCRs). We have previously shown that the function of the human micro-opioid receptor (hMOR) expressed in Saccharomyces cerevisiae is modulated by sterols including cholesterol. Here, we investigated the effects of cholesterol content on hMOR pharmacology and on hMOR partitioning in cholesterol-poor and -rich domains in eukaryotic mammalian cells (CHO). We show that cholesterol is required for the stabilization of a receptor conformation with high agonist affinity and for triggering G-protein activation after agonist binding to the receptor. Biochemical analysis of untreated and cholesterol-depleted membranes in cells expressing hMOR indicated that the receptor is only present in cholesterol poor domains, in the basal state. After agonist binding to untreated CHO membranes, two distinct populations of receptor were found in cholesterol-rich and -poor domains. Cholesterol depletion or treatment of CHO membranes with the G-protein-decoupling agent GppNHp prevented the redistribution, indicating that receptor activated states localized into cholesterol-rich domains. Pharmacological data and biochemical analysis indicate that distinct activated conformations of hMOR exist in CHO plasma membrane and correspond to microdomains differing by thickness and proportions of lipid components, including cholesterol.

Abstract: This paper reports a 9-night asteroseismic observation program conducted in January 2007 with the new spectrometer Sophie at the OHP 193-cm telescope, on the F5 IV-V target Procyon A. This first asteroseismic program with Sophie was intended to test the performance of the instrument with a bright but demanding asteroseismic target and was part of a multisite network. The Sophie spectra have been reduced with the data reduction software provided by OHP. The Procyon asteroseismic data were then analyzed with statistical tools. The asymptotic analysis has been conducted considering possible curvature in the echelle diagram analysis. These observations have proven the efficient performance of Sophie used as an asteroseismometer, and succeed in a clear detection of the large spacing. An \'echelle diagram based on the 54-$\mu$Hz spacing shows clear ridges. Identification of the peaks exhibits large spacings varying from about 52 $\mu$Hz to 56 $\mu$Hz., Comment: 7 pages, 7 figures

Abstract: Substituting used tyres for anthracite in the EAF in France and in Belgium is reported. First trials have been carried out in Lorraine in 1997 in the frame of a partnership between Usinor (now Arcelor Mittal), Michelin and Ademe. The substitution process has been eventually implemented by LME in cooperation with Aliapur over the years 2002-2003. Industeel Belgium got started on substitution in 2004. To day, both steel plants have achieved stable operation, with a regular load of 8 to 12 kg used tyres per ton of steel, thus allowing significant and sustainable savings on anthracite.

Abstract: Objective: The placement of automated external defibrillators (AEDs) in schools and public sporting venues is a growing national trend. The purpose of this study was to determine the prevalence and utilization of AEDs in Washington State high schools and examine existing emergency preparedness for sudden cardiac arrest. Design: Cross-sectional survey. Setting: High schools in Washington State. Participants: The principal at each high school in the Washington Interscholastic Activities Association (N=407) was invited to complete a web-based questionnaire using the National Registry for AED Use in Sports (www.AEDSPORTS.com). Main Outcome Measurements: The primary outcome measures studied included AED prevalence and location, funding for AEDs, AED training of school personnel, coordination of AED placement with local emergency response agencies, and prior AED utilization. Results: One-hundred eighteen schools completed the survey (29% response rate). Fifty-four percent (64/118) of schools have at least one AED on school grounds (mean 1.6, range 1-4). The likelihood of AED placement increased with larger school size (p = 0.044). Sixty percent of AEDs were funded by donations, 27% by the school district, and 11% by the school or athletic department itself. Coaches (78%) were the most likely to receive AED training, followed by administrators (72%), school nurses (70%), and teachers (48%). Only 25% of schools coordinated the implementation of AEDs with an outside medical agency and only 6% of schools coordinated with the local emergency medical system. One school reported a prior AED use to treat sudden cardiac arrest (SCA) in a basketball official who survived following a single shock. The estimated probability of AED use to treat SCA was 1 in 154 schools per year. Conclusions: Over half of Washington State high schools have an AED on school grounds. AED use occurred in less than 1% of schools annually and was effective in the treatment of SCA. Funding of AED programs was mostly through private donations with little coordination with local emergency response teams. Significant improvement is needed in structuring emergency response plans and training targeted rescuers for a SCA in the high school setting.

Abstract: Controversial results have been published on potential link between cancer and multiple sclerosis. Multiple sclerosis has been linked to reduced rates of cancer prior to the era of immunomodulating or immunosuppressive treatments and until today, only 9 studies can be found in the literature. New strategies and early use of IM or IS drugs in MS justify to study and follow patients to detect a potential increase of cancer's incidence in treated patients. It is important to follow and collect prospectively in MS centers, patients with history of cancer, to document histologies, and potential relations with repeated IM or IS treatments. A prospective study is in progress in French MS centers on behalf the Club Francophone de la SEP (CARIMS Project).

Abstract: Fixation of distal radius fractures is now the primary surgical procedure leading to medical liability claims for surgeons dealing with injuries to the upper limb. The main reasons for incrimination are complex regional pain syndrome and an unfavorable progression toward malunion. Infectious problems are relatively rare and, in general, well controlled. The associated capsule and ligament damage can be underestimated at the time of the initial assessment, which points to the need for complementary imaging in case of doubt. The prevention of claims inevitably requires a judicious choice of the fixation technique, which is adapted to the anatomical fracture type and to the patient. The best defense is a complete medical record, with emphasis on traceable information.

Abstract: Isolated tumefactive demyelinating lesion (TDL) is a rare disease and a challenging entity especially for the differential diagnosis, biopsy indications, and therapeutic decisions. Long-term evolution is not well known. The objective of the study is to describe clinical and MRI characteristics and long-term follow-up of patients with isolated TDL. We performed a retrospective study including patients (1) with one TDL radiologically defined by a ≥20 mm FLAIR hyperintensity involving the white matter associated with T1 hypointensity that enhanced after gadolinium injection and (2) without any other MS lesion on the first MRI. Tumor, abscess, or other inflammatory diseases (ADEM, Balo’s concentric sclerosis, systemic disease) were excluded. Sixteen patients (11 females/5 males) were included. The mean age of onset was 35.7 years (range 20–65). MRI disclosed supratentorial lesions with a mean size of 39.4 mm and usually mild edema/mass effect. Peripheral (mainly open-ring pattern) and central (mainly heterogeneous) enhancement were respectively seen in 9/16 and 11/16 patients. CSF study (n = 15) found oligoclonal bands (OCB) in seven. A cerebral biopsy was performed in 11 cases showing acute inflammatory demyelination. Thirteen patients were treated by pulse steroids with marked improvement in ten. At last clinical follow-up (mean 65.8 months, range 6–181), diagnosis was MS in 5 (31 %), isolated TDL in 10 (63 %) and one patient had a second TDL (6 %). Isolated tumefactive demyelinating lesions are a rare diagnostic entity. After a mean follow-up of 5 years, almost one-third became MS whereas most of the patients had no further event.

49. Dynamic control of fossil fuel injections in EAF through continuous fumes monitoring

Abstract: EAF process has been continuously improving for the last 30 years and has achieved spectacular performances in terms of productivity and electricity specific consumption. A general trend observed worldwide is an increase in oxygen and carbon injections. Multi-function tools for oxygen injection have become a standard for new EAFs and revamping of old EAFs. With these tools, oxygen can be injected under various parameters for: decarburisation with supersonic O2 nozzles combined with carbon injection for foamy slag forming, in-situ post-combustion with low velocity injectors, scrap melting boosting with oxy-fuel burners. Fossil fuel energies (oxygen, carbon, natural gas) represent up to 40% of total energy consumed in EAF, even more in some cases. A very critical issue for EAF improvement is a control of oxygen and fossil fuels injections, taking into account real furnace conditions at each step of the melt. Calculated benefits of a dynamic control compared to set-points control of oxygen injections for post-combustion optimization are presented. Based on near IR laser- diode spectroscopy, a system enabling dynamic and continuous CO and CO2 monitoring of dust-loaded EAF fumes during the whole melt for long-term operations was developed. Several industrial trials are presented, describing the technology and results achieved are discussed.

50. L’apport du test de mémoire de reconnaissance visuelle DMS-48 dans la maladie bipolaire vieillissante

Abstract: Resume Dans la litterature, le retentissement cognitif de la maladie bipolaire concerne surtout les fonctions executives (en particulier la planification), les difficultes d’encodage et le rappel libre d’informations. Nous avons evalue 20 patients bipolaires (BP) et 20 patients Alzheimer (MA) apparies par l’âge, le sexe et le niveau culturel. Leur âge moyen etait de 71 ans. Tous les patients ont realise deux epreuves de memoire classique. D’une part, ils ont realise l’epreuve de memoire verbale RL/RI-16 items et, d’autre part, l’epreuve de reconnaissance visuelle du DMS-48 (delayed matching-to-sample). Les patients BP ont des difficultes de memoire dans les deux epreuves que nous avons realisees. On retrouve des performances mediocres dans les epreuves de rappel libre en memoire verbale. Le rappel indice aide les patients, mais ne normalise pas le score global de bonnes reponses. Les patients BP se distinguent quantitativement des patients MA dans les rappels libres et les rappels globaux, et font moins d’intrusions que les patients MA. Les patients BP ont des difficultes proportionnellement plus importantes dans l’epreuve de reconnaissance visuelle et ne se distinguent pas des patients MA en terme de score global de bonnes reponses. Par contre, les patients BP font davantage d’erreurs abstraites, et ce, des le Set 1. Cette difficulte plaide pour une difficulte specifique a l’encodage des items abstraits et/ou pour une difficulte de type plus « fonction executive », et fait discuter une sensibilite propre aux stimuli du DMS-48 qui pourrait etre un outil sensible a la deterioration cognitive des patients BP.
