A multicomponent reaction (MCR) strategy, alternative to the known cycloaddition reaction, towards variously substituted 1-amino-1H-imidazole-2(3H)-thione derivatives has been successfully developed. The novel approach involves α-halohydrazones whose azidation process followed by tandem Staudinger/aza-Wittig reaction with CS2 in a sequential MCR regioselectively leads to the target compounds avoiding the formation of the regioisomer iminothiazoline heterocycle. The approach can be applied to a range of differently substituted α-halohydrazones bearing also electron-withdrawing groups confirming the wide scope and the substituent tolerance of the process for the synthesis of the target compounds. Interestingly, the concurrent presence of reactive functionalities in the scaffolds so obtained ensures post-modifications in view of N-bridgeheaded heterobicyclic structures.

OBJECTIVES: As a starting point, a vast variety of 200 technical papers and documents published during the ten years 1999-2008, from Brazilian and international organizations dedicated to the control of leprosy, was taken. A study was then undertaken to investigate its future evolutive possibilities by employing resources obtained from scenario analyses. DESIGN: The methodological reconstruction in use was of a qualitative nature, based on a bibliographic review and content analysis techniques. The latter were employed in a documental, categorical, contingent, frequency-based format, in compliance with appropriate and pertinent conditions. RESULTS: Nowadays, important elements on epidemiological and operational aspects have been regained, as well as respective perspectives. CONCLUSIONS: A projection is made towards the fact that the maintenance of the disease's present incidence levels constitute economic and sanitary challenges that confront issues ranging from the neoliberal model of global societal organization to specific competences of actions taken by health teams in the field.

In this paper a modular version of the classical Korovkin theorem in multivariate modular function spaces is obtained and applications to some multivariate discrete and integral operators, acting in Orlicz spaces, are given.

Here we state a quantitative approximation theorem by means of nets of certain modified Hadamard integrals, using iterates of moment type operators, for functions f defined over the positive real semi-axis ]0, +∞[, having Mellin derivatives. The main tool is a suitable K-functional which is compatible with the structure of the multiplicative group ]0, +∞[. Some numerical examples and graphical representations are illustrated.

We deal with a new type of statistical convergence for double sequences, called Ψ-A-statistical convergence, and we prove a Korovkin-type approximation theorem with respect to this type of convergence in modular spaces. Finally, we give some application to moment-type operators in Orlicz spaces.

INTRODUÇÃO: Uma das realidades atuais da hanseníase a nível do coletivo consiste na ação simultânea de órgãos oficiais e organizações não governamentais seguindo eventualmente estratégias diferenciadas. A presente comunicação visa contribuir para com tal questão, revendo alguns aspectos marcantes de respectiva evolução contemporânea. MÉTODOS Aplicação das técnicas de revisão bibliográfica e análise textual, enquanto metodologias conteudísticas, a dados primários de acervo circunstancial de publicações específicas em circulação aberta. RESULTADOS Os desencontros sucessivos demarcados entre ambos os setores explicitam-se no referente ao objetivo a ser atingido pela luta contra a doença, se seu controle ou sua eliminação. CONCLUSÕES Novas perspectivas de ações conjuntas são buscadas agora quando, mais realisticamente, se retoma a prioridade do controle da endemia e se contemplam, de fato, seus determinantes sociais.

Apesar da escassez relativa de estudos prospectivos a respeito, com a atual expansão da incidência da hanseníase, suas incapacidades físicas assumem destaque cada vez maior, fazendo com que o significado das mesmas se intensifique ainda mais, agravado por manifestações até de autoridades religiosas máximas: A terapêutica semântica não resolve, se a ideologia da mensagem se mantém. Em decorrência, a execução de medidas de prevenção, diagnóstico e tratamento na unidade local, já razoavelmente sistematizadas em respectivos aspectos técnicos, se torna mandatória e indispensável. Segundo a Organização Mundial da Saúde, trata-se de implementar medidas, sobretudo de: fortalecimento dos programas, políticas e estratégias nacionais para implementação de regras básicas (para atenção médica, reabilitação e serviços de assistência); aumento de maior consciência do público sobre a importância do tema de incapacidades, bem como de coordenação de todos os setores da sociedade na participação de atividades de prevenção de incapacidades; cooperação dos estados-membros na coleta de dados mais confiáveis e relevantes. Tais questões constituem os principais elementos tratados na presente comunicação, a partir de informações técnicas disponíveis na respectiva literatura internacional especializada, bem como da experiência pessoal e profissional dos autores.

Resumo O Grupo de Saúde Coletiva/Epidemiologia e Atividade Física da Unicamp apresenta recortes atuais de sua atuação no desenvolvimento de projetos técnicos a partir de três núcleos temáticos de interesse: i) caracterizações de metodologias de pesquisa e ensino empregadas em seu interior; ii) exploração de relações entre níveis de Qualidade de Vida e diferentes programas de Atividade Física e iii) aspectos do binômio corpo-movimento em ações de controle da hanseníase. Especificamente, apresentam-se como resultados expressivos os significados que podem ser explicitados pela análise conjunta de diferentes projetos conduzidos de forma integrada por mesma equipe acadêmica.

Abstract Objectives Available evidence on axillary surgery has accumulated dramatically in the last two decades in favor of less invasive care. The aim of this paper is to study 16-years trends in the surgical management of the axilla in a large population-based data set of screen-detected breast cancers in Italy and to document at what extent recommendations have been adopted in actual clinical care. Material and methods This is a retrospective cohort study documenting the surgical management of the axilla in primary breast cancer patients over time. We retrieved from the Italian database of screen-detected cancers 41213 cases diagnosed in women aged 50–69 between years 2000 and 2015 in twelve Italian Regions. Results In pN0 cases, an increasing trend (p < 0.001) in the number of patients who received sentinel lymph node biopsy (SLNB) as the only axillary staging procedure was observed. In pN + cases SLNB was the only staging procedure in an increasing number of patients (p < 0.001) especially since the publication of the ACOSOG-Z0011 paper. In ductal carcinoma in situ (DCIS) SLNB was more frequent in mastectomies and in high grade and large lesions. However, 45% of low grade, small DCIS over the whole time period had some form of axillary surgery. Conclusion This large series of screen-detected cases documents a strong time trend in the direction of reducing axillary surgery and hence potential harms from treatment. The continuing practice of SLNB in low risk DCIS is of concern in an era of increasing awareness towards overdiagnosis and overtreatment. Highlights • Invasiveness of axillary surgery decreased dramatically in the last two decades. • Completion ALND in N+ patients decreased since publication of the Z0011 paper. • Smaller size, low grade and ER + are inversely associated to completion ALND. • Sentinel lymph node biopsy in low risk DCIS shows large variation in use.
