Introduction La réaction du greffon contre l’hôte transfusionnelle (TA GVHD) est causée par les lymphocytes T contaminants présents dans les produits sanguins. L’irradiation gamma (IRRG) avec 2500 cGy est utilisée pour prévenir la TA GVHD. Pour les concentrés de plaquettes (CP) et le plasma, une méthode alternative à IRRG est le traitement photochimique (PCT) avec amotosalen/UVA (Intercept™ Blood System [IBS]) utilisé en Europe depuis plus de 10 ans. Une autre méthode en développement pour les concentrés de globules (CGR) est le procédé IBS avec amustaline et GSH. Dans cette étude, nous avons évalué la capacité de l’amustaline/GSH d’inactiver les lymphocytes T dans les CGR pour prévenir la TA GVHD. Méthodes L’inactivation de lymphocytes T isolés de CGR traités par amustaline/GSH (0,2 mM/20 mM) avec incubation à température ambiante pendant 18–24 h suivie d’une centrifugation et de l’échange du surnageant avec la solution additive AS-5 a été testée. Une méthode de dilution limite (LDA) comportant l’incorporation d’un analogue éthinyl désoxyuridine (EDU) fluorescent de thymidine en présence de cellules allostimulatrices (Click-iT ® ) et identification des cellules viables en cytométrie de flux a été utilisée. Résultats L’analyse a montré un signal fort (1–8 × 10 4 cellules) associé à l’analyse des cellules viables non traitées qui ne pouvait être observé que lorsque les cellules allostimulatrices, l’EDU et le marquage fluorescent étaient utilisés ; tandis que le signal obtenu était négligeable lorsque l’un des réactifs manquait. Pas de signal (1–7 cellules) n’a été observé avec les échantillons tests (traités amustaline/GSH), ce qui indique que les cellules ont été incapables de proliférer. Il est prévu que si l’incubation des cellules est prolongée pendant 14 jours, une plage dynamique plus grande sera atteinte. Background: The potential for Zika virus (ZIKV) transfusion-transmission (TT) has been demonstrated in French Polynesia and Brazil. Pathogen inactivation (PI) of blood products is a proactive strategy to inactivate TT pathogens including arboviruses. Inactivation of West Nile, dengue, Zika, and chikungunya viruses was previously demonstrated by photochemical treatment with amotosalen and ultraviolet A (UVA) illumination. In this study, we evaluated ZIKV inactivation in red blood cell (RBC) components by a chemical approach that uses amustaline (S-303) and glutathione (GSH).Study Design and Methods: RBC components were spiked with a high titer of ZIKV. Viral titers (infectivity) and ZIKV RNA loads (reverse transcription-polymerase chain reaction) were measured in spiked RBCs before and after S-303 and GSH treatment and confirmed using repetitive passages in cell culture. A mock-treated arm validated the approach by demonstrating stability of the virus (infectivity and RNA load) during the process.Results: The mean ZIKV infectivity titer and RNA load in RBCs were 5.99 ± 0.2 log 50% tissue culture infectious dose (TCID50 )/mL and 7.75 ± 0.16 log genomic equivalents/mL before inactivation. No infectivity was detected immediately after S-303 and GSH treatment and after five serial passages in cell culture.Conclusion: Complete ZIKV inactivation of more than 5.99 log TCID50 /mL in RBCs was achieved using S-303 and GSH at levels higher than those found in asymptomatic ZIKV-infected blood donors. Background and Objectives Blood Centre logistics, staffing and donor scheduling may be optimized if pathogen inactivation ( PI) of platelets can be delayed until Day 1, but bacteria may rapidly grow during this time. This study evaluates bacterial PI performed 24 and 30 h after collection. Materials and Methods PAS-3 platelet units were collected on the Amicus and subsequently inoculated (3-53 CFU/unit) with 1of 5 transfusion relevant bacterial species ( n = 3/organism). Units were then stored for either 24 ± 0·3 or 30 ± 0·3 h at 20-24°C with agitation, subsequently treated with amotosalen and UVA, and stored for 7 days. Samples were taken before and after inactivation, on Days 2, 5 and 7 for BacT/ ALERT testing, and on Days 5 and 7 for plate counts. Results All samples from units taken prior to inactivation either demonstrated positive plate culture counts, or, in untreated positive controls, were culture-positive during storage. All contaminated units treated with amotosalen and UVA 24 after inoculation were culture-negative on all days tested. With inactivation performed 30 h following inoculation, one of 15 units (1-of-3 replicates) was culture-positive with Klebsiella pneumonia (1 × 109 CFU/ml) by Day 5. Conclusion Photochemical treatment did not inactivate 1 of 15 units to sterility in apheresis platelets stored in PAS with a 30-h delay between contamination and treatment, but did inactivate 15 of 15 units with a 24-h delay. Background: Pathogen inactivation methods are increasingly used to reduce the risk of infections after transfusion of blood products. Photochemical treatment (PCT) of platelets (PLTs) and plasma with amotosalen and ultraviolet A (UVA) light inactivates pathogens and white blood cells through formation of adducts between amotosalen and nucleic acid that block replication, transcription, and translation. The same adducts block the amplification of nucleic acids using polymerase chain reaction (PCR) in a manner that correlates with the number of adducts formed, providing a direct quality control (QC). Current QC measures for PCT rely on indirect methods that measure the delivered UVA dose or percent residual amotosalen after illumination, rather than directly measuring nucleic acid modification.Study Design and Methods: Endogenous mitochondrial DNA (mtDNA), which is detectable in PLT and plasma units, was chosen as a target for the quantification of photochemically induced modifications. DNA was extracted from untreated or amotosalen and UVA-treated PLTs or plasma, and mtDNA fragments of variable lengths were quantified using a real-time PCR inhibition assay.Results: PCT induced increasing real-time PCR inhibition of mtDNA amplification for larger amplicon sizes. Amplification was unaffected by treatment with amotosalen or UVA alone, whereas up to 3 log inhibition was observed after PCT. Blinded PCR testing of a panel of 110 samples each, from PLT or plasma components prepared for routine use within a blood center, allowed 100% discrimination between untreated and treated units.Conclusion: Our initial findings indicate that an adequately sensitive, quantitative real-time PCR inhibition assay targeting mtDNA could provide a valuable tool to confirm and monitor PCT. Background: Pathogen inactivation (PI) is a new approach to blood safety that may introduce additional costs. This study identifies costs that could be eliminated, thereby mitigating the financial impact.Study Design and Methods: Cost information was obtained from five institutions on tests and procedures (e.g., irradiation) currently performed, that could be eliminated. The impact of increased platelet (PLT) availability due to fewer testing losses, earlier entry into inventory, and fewer outdates with a 7-day shelf life were also estimated. Additional estimates include costs associated with managing (1) special requests and (2) test results, (3) quality control and proficiency testing, (4) equipment acquisition and maintenance, (5) replacement of units lost to positive tests, (6) seasonal or geographic testing, and (7) health department interactions.Results: All costs are mean values per apheresis PLT unit in USD ($/unit). The estimated test costs that could be eliminated are $71.76/unit and a decrease in transfusion reactions corresponds to $2.70/unit. Avoiding new tests (e.g., Babesia and dengue) amounts to $41.80/unit. Elimination of irradiation saves $8.50/unit, while decreased outdating with 7-day storage can be amortized to $16.89/unit. Total potential costs saved with PI is $141.65/unit. Costs are influenced by a variety of factors specific to institutions such as testing practices and the location in which such costs are incurred and careful analysis should be performed. Additional benefits, not quantified, include retention of some currently deferred donors and scheduling flexibility due to 7-day storage.Conclusions: While PI implementation will result in additional costs, there are also potential offsetting cost reductions, especially after 7-day storage licensing. This paper proves that voice over Internet protocol (IP) technology can be combined with satellite transmission in air traffic control (ATC) applications provided that the systems used to implement this approach have been meticulously designed to meet the end-to-end delay requirements and to certify the quality of service they offer under strenuous conditions.We propose a modified E-model and prove it can be used as a radio network planning tool, because once the requirements that were drawn from it were fulfilled, the users of the resulting service accepted it as sufficient in terms of call quality. The approach has been applied on the SATWAYS system, cofunded by the European Space Agency, and designed and implemented to extend the coverage of radio frequencies within airspaces when the commonly used landline communication infrastructure is either too costly or impossible to install and maintain. The impulse behavior of composite short rod-plane air gaps with a dielectric-covered rod under positive lightning and switching impulse voltages is investigated, by monitoring the electric field strength at the earthed plane, by still photographs of corona discharges and through electric field computations. Experiments were performed in normal laboratory air at pressure around 0.1 MPa, temperatures in the range 19 to 28°C and absolute and relative humidity varied naturally between 11-21 g/m3 and 60-90%, respectively. Breakdown probability distributions were obtained for composite gaps with a dielectric cover made of either PTFE or epoxy resin and for simple air gaps with a bare rod. Also, basic characteristic parameters of coronas preceding breakdown in composite gaps, namely initial corona in the vicinity of the covering tip, surface coronas emerging along the dielectric cover and developing mainly in air, and secondary corona in the vicinity of the cover upper end, were measured. Surface corona development is greatly affected by initial corona growth and facilitates secondary corona inception. Breakdown, occurring at higher voltages for composite gaps, is closely related with the secondary corona inception; a similar dependence upon impulse waveshape and cover material was seen to exist. At breakdown, the spark channel bridges the gap that includes the dielectric cover length. Abstract: Background: The presence of nucleated red blood cells (NRBCs) has been identified as a poor prognostic indicator. We investigated the relationship of NRBC trends in patients with and without trauma. Methods: We retrospectively reviewed surgical intensive care unit admissions over 4 years, categorizing trauma and nontrauma patients and subdividing them into 3 groups: group A, all-zero NRBC; group B, positive NRBC value returning to zero; and group C, positive NRBC value that did not return to zero. We analyzed all groups for outcomes of length of stay and mortality. Results: Group A was the largest and had the shortest length of stay and least mortality. Group C had the highest mortality rate. No statistical difference was observed with mortality. Conclusions: Any positive NRBC was associated with poor outcome, and increasing NRBC was associated with increasing mortality. Abstract: The Cretan School of iconography is the last great flowering of the traditional Byzantine painting manner, which is however influenced by different schools and western art. Despite their historical significance, icons of the Cretan School have been rarely studied through modern chemical techniques and therefore very few information is available on the materials used by the Cretan painters. In the present investigation, the organic materials contained in several icons of the Cretan School of Iconography from the collection of the Benaki Museum, Athens, Greece, are analyzed and identified by High Performance Liquid Chromatography coupled to a Diode Array Detector (HPLC–DAD) and Gas Chromatography with Mass Spectrometry (GC–MS). The HPLC–DAD results provide support to a recently published report which showed that kermes used in “Cretan” icons of 15th century and early 16th century was replaced by cochineal which was used in icons dated after early 16th century. This trend is known for western art and it is now proved for Cretan icons, showing thus the influence of the Cretan painters by other European painting schools. Other dyes detected in the historical samples, extracted from Cretan icons are madder (possibly Rubia tinctorum L., according to HPLC profiles) and indigoid dyes. Oils, such as linseed and walnut oil, are identified in samples extracted from the seven icons tested by GC–MS. Egg yolk is identified in the extract of only one icon and animal glue, originated most likely from the gesso ground, is found in six samples. Abstract: A series of binuclear iron compounds has been synthesized using diamide, bis-phenolate ligands in which the carbon-linker between the amide nitrogen atoms has been varied. Two diferrous compounds in the series, along with their two-electron oxidized, di-μ-methoxy-bridged counterparts, have been crystallographically characterized, as have the di-μ-methoxy compounds (H2Hbab=1,2-bis(2-hydroxybenzamido) benzene, H2Hbach= trans-1,2-bis(2-hydroxybenzamido) cyclohexane, H2Hbame=1,2-bis(2-hydroxybenzamido) ethane, H2Hbap=1,3-bis(2-hydroxybenzamido) propane, H2Hbabn=1,4-bis(2-hydroxybenzamido) butane, H2Hbapen=1,5-bis(2-hydroxybenzamido) pentane, N-MeIM= N-methylimidazole and OMe=methoxide). are structurally very similar to previously reported diferrous compounds of this family of ligands that have been shown to be active as oxygen atom transfer catalysts. Flexibility in the carbon-linker allows some variability in the orientation of the phenolate arms of the ligands in the diferric di-μ-methoxy compounds, but the Fe2O2 core remains largely unchanged across the series. Two-electron oxidation of the ferrous compounds in methanol shows a substantial ligand rearrangement that is consistent with other spectroscopic, electrochemical and kinetic investigations. The loss of both phenolate bridges upon oxidation is reminiscent of the “carboxylate shift” observed in binuclear non-heme enzymes and could provide insight into the driving force behind this family of compounds’ function as a catalyst. Abstract: Thermal carbonization of bis(2-chloroethyl)amine hydrochloride at 260°C in air leads to a new, functional layered carbonaceous material that, although different in structure and composition, shares similar characteristics as the well-known graphite oxide. Specifically, the molecularly derived carbonaceous solid is layered with relatively small lateral dimensions, highly dispersible in water providing clear colloidal sols and possesses ion-exchange properties. The carbonaceous solid strongly fluoresces in the visible, when stimulated with a wide range of excitation wavelengths. Overall, the method presents an alternative synthesis towards molecularly derived layered carbonaceous materials with novel properties. [Copyright &y& Elsevier]

Abstract: Graphite fluoride has been generally considered chemically inert against strong alkalis under ambient conditions. In the present study we demonstrate that treatment of graphite fluoride with eutectic NaOH–KOH mixture at 250°C induces dramatic structural and textural changes in the solid as evidenced by XRD, FT-IR, Raman, UV–vis absorption and fluorescence and microscopy techniques (TEM, AFM). The reaction proceeds in the molten state leading to water-soluble, graphitized carbon particles which unlike graphite fluoride, adopt a variety of morphologies, like platy, tetragonal, triangular, discoid and spherical. Giving the operator the ability to manage in-home devices greatly increases the potential of the cost-efficient provision of new profitable services. In this spirit, standardization undertaken by the DSL forum not only addresses remote management of the residential gateway but recently, also extends to in-house devices. We propose a solution that bridges remote management specified by CWMP with UPnP, already established inside the home environment. This approach will enable providers to remotely configure and manage in-house UPnP-enabled equipment, thus accelerating the deployment of new services. We investigate the nature of the two protocols and specify the bridging functionality, taking into consideration scalability and transparency. Experimental results support the applicability of the proposed approach, highlight the importance of message filtering, and provide general insight into the interplay intricacies of the particular protocol functions. Intense cosmic ray fluxes during Forbush decreases can be responsible for a number of radiation effects in electronics and sensor systems of spacecrafts and aircrafts. Monitoring, modeling and possible prediction, from the real-time database of the Athens Neutron Monitor Data Processing (ANMODAP) Center are being considered. A different kind of cosmic ray events that evolves during a Forbush decrease, as an additional intermediate enhancement and its impact on electronic systems, is also identified. BACKGROUND: The human erythrovirus B19 (B19) is a small (18- to 26-nm) nonenveloped virus with a single-stranded DNA genome of 5.6 kb. B19 is clinically significant and is also generally resistant to pathogen inactivation methods. Photochemical treatment (PCT) with amotosalen and ultraviolet A (UVA) inactivates viruses, bacteria, and protozoa in platelets (PLTs) and plasma prepared for transfusion. In this study, the capacity of PCT to inactivate B19 in human PLT concentrates was evaluated. STUDY DESIGN AND METHODS: B19 inactivation was measured by a novel enzyme-linked immunosorbent spot (ELISPOT) erythroid progenitor cell infectivity assay and by inhibition of long-range (up to 4.3 kb) polymerase chain reaction (PCR), under conditions where the whole coding region of the viral genome was amplified. B19-infected plasma was used to test whether incubation of amotosalen with virus before PCT enhanced inactivation compared to immediate PCT. RESULTS: Inactivation of up to 5.8 log of B19 as measured by the infectivity assay, or up to 6 logs as measured by PCR inhibition can be achieved under non-limiting conditions. Inactivation efficacy was found to increase with incubation prior to UVA illumination. Without incubation prior to illumination 2.1 +0.4 log was inactivated as determined by infectivity assay. When measured by PCR inhibition, inactivation varied inversely with amplicon size. When primers that spanned the entire coding region of the B19 genome were used, maximum inhibtion of PCR amplification was demonstrated. CONCLUSION: Under defined conditions, PCT with amotosalen combined with UVA light can be used to inactivate B19, a clinically significant virus that can be transmitted through blood transfusion, and heretofore has been demonstrated to be refractory to inactivation. Background. In evaluating a photochemical treatment process for inactivating parvovirus B19, there lacked simple culture methods to measure infectivity. The recently developed enzyme-linked immunospot (ELISpot) infectivity assay uses late-stage erythropoietic progenitor cells and is labor intensive and time consuming. We evaluated a novel, efficient polymerase chain reaction (PCR) inhibition assay and examined correlations with reductions in infectivity. Methods. Contaminated plasma was treated with 150 μmol/L amotosalen and 3 J/cm² ultraviolet A light and then tested for DNA modification using conventional PCR inhibition and a novel preamplification approach. The novel assay subjected the samples to preamplification cycles using long-template PCR, followed by quantitative PCR (QPCR) inhibition detection. Both approaches were tested for correlations with reductions in viral infectivity by comparing ELISpot assay results of identical samples. Results. The B19 preamplification inhibition assay showed detection ranges of 2–2.5 log and demonstrated quantitative correlation with up to a 5.8-log reduction in viral infectivity in ELISpot results. Conventional PCR detected a >5 log reduction in amplification, correlated with a 4.4-log reduction in viral infectivity. A ange of 4-log inhibition of hepatitis B virus DNA amplification was also achieved. Conclusions. The results demonstrated that a novel preamplification QPCR assay is a useful tool for predicting reductions in infectivity after photochemical treatment. We developed a new class of vaccines, based on killed but metabolically active (KBMA) bacteria, that simultaneously takes advantage of the potency of live vaccines and the safety of killed vaccines. We removed genes required for nucleotide excision repair (uvrAB), rendering microbial-based vaccines exquisitely sensitive to photochemical inactivation with psoralen and long-wavelength ultraviolet light. Colony formation of the nucleotide excision repair mutants was blocked by infrequent, randomly distributed psoralen crosslinks, but the bacterial population was able to express its genes, synthesize and secrete proteins. Using the intracellular pathogen Listeria monocytogenes as a model platform, recombinant psoralen-inactivated Lm ΔuvrAB vaccines induced potent CD4+ and CD8+ T-cell responses and protected mice against virus challenge in an infectious disease model and provided therapeutic benefit in a mouse cancer model. Microbial KBMA vaccines used either as a recombinant vaccine platform or as a modified form of the pathogen itself may have broad use for the treatment of infectious disease and cancer. A statistical model for cumulative solar proton event fluences during space missions is presented that covers, both the solar minimum and solar maximum phases of the solar cycle. It is based on data from the Interplanetary Monitoring Platform and Geostationary Operational Environmental Satellites series of satellites, which are integrated together to allow the best features of each data set to be used to the best advantage. The aglZ gene of Myxococcus xanthus was identified from a yeast two-hybrid assay in which MglA was used as bait. MglA is a 22-kDa cytoplasmic GTPase required for both adventurous and social gliding motility and sporulation. Genetic studies showed that aglZ is part of the A motility system, because disruption or deletion of ag1Z abolished movement of isolated cells and aglZ sglK double mutants were nonmotile. The aglZ gene encodes a 153-kDa protein that interacts with purified MglA in vitro. The N terminus of AglZ shows similarity to the receiver domain of two-component response regulator proteins, while the C terminus contains heptad repeats characteristic of coiled-coil proteins, such as myosin. Consistent with this motif, expression of AglZ in Escherichia coli resulted in production of striated lattice structures. Contexte Les procédés d’inactivation des agents pathogènes (IP) ont pour but l’inactivation des agents infectieux (bactériens, viraux et parasitaires) dans les produits sanguins labiles (PSL). En ce qui concerne les arbovirus, l’IP par procédé photochimique avec amotosalen (S-59) et ultraviolets A (UVA) a été démontrée, entre autres, pour les virus de la dengue (DENV) et du chikungunya (CHIKV) dans le plasma et les plaquettes et pour le virus Zika (ZIKV) dans le plasma ; l’IP par traitement chimique utilisant l’amustaline et le glutathion (S-303/GSH) a été démontrée pour le ZIKV dans les concentrés de globules rouges (CGR). Objectifs Pour compléter ces travaux, nous avons étudié l’inactivation du ZIKV dans les plaquettes par S-59/UVA ; et celles du DENV et du CHIKV dans les CGR par S-303/GSH. Méthodes Les unités plaquettaires ont été inoculées avec des titres élevés de ZIKV et les CGR avec des titres élevés de DENV et CHIKV. Les niveaux d’infectiosité (log 10 TCID 50 /mL) et les quantités d’ARN (log 10 Geq/mL) ont été mesurés avant et après IP. Résultats S-59/UVA inactive > 4,4 log 10 ZIKV TCID 50 /mL (7,5 log 10 Geq/mL) à la limite de détection dans les plaquettes ; S-303/GSH inactive > 6,61 log 10 DENV TCID 50 /mL (8,42 log 10 Geq/mL) et > 5,81 log 10 CHIKV TCID 50 /mL (10,49 log 10 Geq/mL) à la limite de détection dans les CGR. Conclusion Ces résultats combinés à ceux d’études antérieures montrent que les procédés d’inactivation utilisant S-59/UVA et S-303/GSH inactivent ZIKV, DENV et CHIKV dans le plasma, les plaquettes et les CGR. Introduction Intercept™ Blood System pour les globules rouges (non approuvé commercialement) utilise une petite molécule d’amustaline pour former des adduits covalents au sein des acides nucléiques des leucocytes et des agents pathogènes contaminants pour empêcher leur réplication. Avant incubation de 18–24 h à température ambiante, 0,2 mM d’amustaline et 20 mM de glutathion (GSH) sont ajoutés. L’inactivation est complète au bout de 3 h. Le temps supplémentaire assure une décomposition complète de l’amustaline. L’objectif de cette étude était d’évaluer l’inactivation du Calicivirus félin (CVF) pour des CGR en Optisol (AS-5). CVF est un virus à ARN simple brin sens positif, genre Vesivirus , famille Caliciviridae et sert de modèle pour les petits virus non enveloppés tels que le virus de l’hépatite E. Méthodes Pour chaque expérience, un CGR a été contaminé avec du CVF à une concentration finale de ∼10 8,0 pfu/mL. Les CGR contaminés ont été mélangés avec le GSH et des échantillons de contrôle (T = 0) pris pour déterminer les titres pré-amustaline. L’amustaline a été mélangée aux CGR et des échantillons tests (T = 3) ont été prélevés pour déterminer les niveaux d’inactivation. Les échantillons contrôles et tests ont été dilués en série et inoculés sur des cellules CrFK. Les plaques ont été incubées 3 jours à 37 °C, colorées avec du cristal violet avant numération. La réduction Log a été calculée sur base de la différence entre le titre moyen dans des échantillons pré et post-amustaline à 3 h. Résultats Une inactivation robuste de CVF a été obtenue comme résumé dans le Tableau 1 . Conclusion Le Calicivirus félin non enveloppé a été inactivé à la limite de détection dans les CGR après traitement avec le GSH et l’amustaline. Introduction Depuis 2015, l’épidémie de virus ZIKA (ZIKV) s’est étendue. Les infections asymptomatiques fréquentes ainsi que le risque de transmission sexuelle soulèvent la possibilité de transmission par transfusion. Ce risque peut être limité par la réduction des agents pathogènes (RP) avec le procédé Intercept™ Blood System . La technologie a été démontrée efficace sur d’autres arbovirus ainsi que capable d’inactiver > 6,57 log ZIKV dans le plasma humain (Aubry M et al.). Le mode d’action (MDA) est basé sur la formation d’adduits covalents irréversibles avec les acides nucléiques. Une technologie avec le même MDA utilisant l’Amustaline et le Glutathion (GSH) est en développement pour les concentrés de globules rouges (CGR). Nous rapportons ici l’inactivation de ZIKV dans les CGR et les concentrés de plaquettes (CP) quel que soit le milieu de suspension. Méthodes Des CP soit dans 65 % de solution additive plaquettaire (PAS) ou 100 % de plasma, ou des CGR préparés en AS-5 ( n = 3), ont été inoculés avec ZIKV (souche CDC PRVABC59 ; ∼ 10 7 pfu/mL). Les CP ont été traités avec Amotosalen et UVA (3j/cm 2 ). Les CGR ont été traités avec Amustaline/GSH (0,2 mM/20 mM) à température ambiante pendant 18–24 h. L’inactivation a été déterminée en comparant les titres infectieux (log 10 ) avant et après le traitement. Résultats Les titres initiaux dans les CP en 65 % PAS ou 100 % de plasma étaient de 4,0 et 4,3 log pfu/mL respectivement. Après inactivation, aucune infectivité n’a été observée, soit une inactivation moyenne > 4,6 log pfu, ou > 4,1 log pfu/mL. Les titres moyens initiaux dans les hématies AS-5 étaient de 4,3 log pfu/mL, sans infectivité après, soit une inactivation moyenne de > 5,0 log pfu, ou > 4,3 log pfu/mL. Dans tous les cas, l’étendue de l’inactivation a été limitée par l’inoculum initial. Le traitement photochimique avec amotosalen et UVA (INTERCEPT Blood System) permet d’inactiver des agents pathogènes dans les plaquettes et le plasma. La compatibilité du procédé avec divers types de plasma a été testée. Méthodes Sept publications étudiant la fonction coagulante de 6 types de plasma ont été identifiées : plasma congelé dans les 8 ou dans les 24 heures, collecté par aphérèse ou à partir de sang total, fabriqué à partir de plasma frais ou préalablement congelé, liquide, lyophilisé ou appauvri en cryoprécipité. Les mesures d’une sélection de paramètres représentant la qualité du plasma ont été rassemblées : facteur VIII, fibrinogène, protéine C, protéine S, alpha 2-antiplasmine et génération de thrombine (ETP) à 5 pM de facteur tissulaire (FT). Résultats Le Tableau 1 montre les valeurs rapportées pour les 6 paramètres de qualité du plasma choisis (quand testés). Le facteur VIII et le fibrinogène sont maintenus dans les valeurs de référence pour tous les types de plasma sauf le plasma appauvri en cryo et le plasma stocké liquide (FVIII) comme attendu. Les valeurs de protéine C, protéine S et d’alpha 2-antiplasmine sont dans les références. La capacité de génération de thrombine des différents produits est conservée ou peu modifiée après traitement. Conclusion Le procédé d’inactivation des agents pathogènes INTERCEPT peut être appliqué avec succès aux produits plasmatiques de différentes sources ou formes, provenant de sang total ou d’aphérèse, congelé, liquide, lyophilisé ou appauvri en cryoprécipité. Contexte Le traitement photochimique (PCT) de concentrés de plaquettes avec amotosalen/UVA (INTERCEPT™ Blood System) remplace l’irradiation gamma (IRRG) dans des pays Européens depuis > 10 ans pour réduire le risque de maladie du greffon contre l’hôte liée à la transfusion. Objectif L’inactivation des cellules T par PCT et IRRG a été comparée à l’aide d’un dosage par dilution limite sensible (DDL). Méthode Des cellules mononuclées de sang périphérique (CMN) ont été inoculées (10 6 /mL) dans des unités de plasma et inactivées en utilisant soit PCT ou IRRG, soit laissées non traitées. Les cellules traitées par PCT ou IRRG ont été incubées (10 7 /mL) pendant 14 jours avec des CMN allo-stimulatrices mélangées de 10 donneurs (5 × 10 6 traitées avec 7500 cGy) et des facteurs de croissance (PHA, IL-2). L’inactivation des cellules T par PCT (10 7 CMN/puits) a été comparée à celle par IRRG (10 5 ou 10 6 CMN/puits) et à un contrôle non traité. La prolifération a été évaluée par incorporation de 3H-thymidine (3H-Thy ; 6,7 Ci/mmol) dans les CMN (score sous microscope). Douze puits ont été utilisés pour chaque dilution et le protocole a été effectué avec 6 réplicats. Résultats Aucune croissance de cellules T pour les CMN inactivées par PCT n’a été détectée avec 10 7 cellules cultivées/puits. Cependant, l’incorporation de 3H-Thy au-dessus du seuil de détection a été observée avec 10 6 CMN IRRG/puits. Aucune incorporation n’a été observée avec10 5 CMN IRRG/puits alors que 4/6 réplicats ont montré une prolifération avec 10 6 CMN IRRG/puits. Conclusion L’inactivation des cellules T avec PCT (traitement amotosalen/UVA) est plus robuste (> 6,2 log 10 ) qu’avec 2500 cGy IRRG (> 4,2 log 10 – ≤ 5,2 log 10 ). [ABSTRACT FROM AUTHOR]

Introduction Le procédé Intercept™ Blood System (IBS) plaquettes est utilisé pour la préparation ex vivo de concentrés plaquettaires d’aphérèse (CP) inactivés pour un large spectre de virus enveloppés/non enveloppés, bactéries, spirochètes, parasites et leucocytes. Le mécanisme d’action se fait par génération d’adduits aux acides nucléiques qui inhibent la réplication, la traduction et la transcription. Basé sur ce mécanisme, nous émettons l’hypothèse que l’inactivation des bactéries ne dépend pas du profil de sensibilité aux antibiotiques, déterminé par des variations génomiques. Objectif Comparer l’inactivation de 2 souches de Klebsiella pneumoniae , génétiquement différentes, isolées chez des donneurs, avec des profils de sensibilité différents pour au moins 10 antibiotiques, dans des CP en solution PAS-3. Méthodes Les dispositifs Intercept™ petit volume ont été utilisés, 150 μM d’amotosalen et illumination UVA (3 J/cm 2 ). Deux souches de K. pneumoniae ont été sélectionnées basées sur des profils de sensibilité différents aux antibiotiques. La PCR ADNr 16s et l’analyse de la membrane de Klebsiella pneumoniae confirment le genre et l’espèce. Les tests de sensibilité aux antibiotiques montrent une différence de phénotype entre les souches. Les souches ont été cultivées jusqu’à des titres très élevés (9–10 log 10 /mL) et diluées au 1/100 dans les CP. Un échantillon de contrôle a été prélevé avant l’illumination et la viabilité bactérienne a été déterminée sur plaque de culture en milieu agar en parallèle avec différentes dilutions des échantillons tests obtenus après traitement. L’inactivation a été calculée en comparant les titres des contrôles et des tests. Résultats Les profils de sensibilité aux antibiotiques de K. pneumoniae sont présentés dans le Tableau 1 , les résultats d’inactivation dans le Tableau 2 . Introduction Le procédé par traitement photochimique utilisant l’amotosalen et les rayons ultraviolets (UVA), développé pour inactiver les pathogènes et les leucocytes résiduels dans les plasmas et les concentrés plaquettaires (CP), est le premier système de réduction des pathogènes approuvé par la FDA aux États-Unis. Initialement, ce procédé était approuvé pour des CP préparés dans 65 % de solution additive (PAS). L’objectif de cette étude était d’évaluer l’inactivation du Plasmodium falciparum (Pf) dans les CP suspendus dans 100 % de plasma pour étendre l’approbation. Pf est une des espèces du parasite intracellulaire Plasmodium qui affecte spécifiquement les globules rouges (GR) et provoque le paludisme. Méthodes Pour chaque expérience, un CP d’aphérèse dans 100 % plasma (∼ 365 mL) a été inoculé avec des GR humains infectés par du Pf (GRi), stade en anneau, à une concentration finale de ∼10 6 GRi/mL. Un échantillon contrôle de 80 mL a été prélevé, puis l’amotosalen ajouté, l’unité illuminée et un échantillon post-traitement a été prélevé. Pour les échantillons de contrôle et de test, les GRi ont été isolés par séparation Ficoll, resuspendus et dilués en série, à 10 −8 pour le contrôle et à 10 −3 pour le test, dans des flacons contenant un milieu avec 5 % de GR frais. Les échantillons dilués étaient utilisés pour inoculer les flacons en quatre exemplaires. La parasitémie était contrôlée par comptage des GRi sur frottis sanguins et cytométrie de flux. Les cultures étaient terminées quand parasitémie > 1 %. La réduction en log correspond à la différence entre le titre moyen des échantillons avant et après UVA. Résultats Le niveau d’inactivation du P. falciparum obtenu dans les plaquettes prélevées dans 100 % de plasma ( n = 4) est robuste ( Tableau 1 ). Conclusion Le traitement photochimique de CP dans 100 % plasma avec amotosalen et UVA inactive > 7,9 log de P. falciparum . Introduction Le virus du chikungunya (CHIKV) est un arbovirus à ARN, genre Alphavirus , famille Togaviridae . La transmission d’une infection à CHIKV par transfusion est un problème de santé dans les régions endémiques bien que non encore démontrée. L’objectif de l’étude était d’évaluer l’inactivation du CHIKV par le procédé Intercept TM Blood System (IBS) pour les concentrés de globules rouges (CGR) préparés en Optisol (AS-5). IBS pour CGR (en développement) utilise une molécule d’amustaline pour former des liaisons covalentes au sein des acides nucléiques des leucocytes et agents pathogènes contaminants pour empêcher leur réplication. Avant incubation 18–24 h à température ambiante, 0,2 mM amustaline/20 mM glutathion (GSH) sont ajoutés, centrifugation et échange du surnageant avec une solution additive (SAG-M). L’inactivation est complète au bout de 3 h. Le temps additionnel assure une décomposition complète de l’amustaline. Méthodes Pour chaque expérience, un CGR a été contaminé par le CHIKV à une concentration finale de ∼10 7,0 pfu/mL et traité avec l’amustaline. L’échantillon contrôle était prélevé avant addition de l’amustaline et dilué en série jusqu’à 10 −7 . Les dilutions ont été inoculées sur des cellules Vero76 pour déterminer le titre initial. L’amustaline a été ensuite ajoutée et un échantillon prélevé après 3 heures pour déterminer le niveau d’inactivation. Les échantillons tests ont été dilués de 10 −1 à 10 −3 et inoculés sur cellules Vero76. Les plaques ont été incubées 5 jours à 37 °C, colorées avec du cristal violet avant numération. La réduction Log a été calculée soit la différence entre le titre moyen dans les échantillons pré- et post-amustaline à 3 heures. Le procédé Intercept™ Blood System pour le plasma, utilisant l’amotosalen et les UVA, inactive un large spectre d’agents pathogènes et les leucocytes. En Suède, le plasma réfrigéré est utilisé jusqu’à 7 jours en service d’urgence et est considéré comme cliniquement équivalent au PFC. Cette étude a examiné l’impact du procédé Intercept sur le plasma liquide stocké ensuite à 4 °C pendant 2 semaines. Méthodes Quatre unités de plasma de sang total de même groupe ABO ont été regroupées dans les 18 h suivant la collecte ( n = 10). Les mélanges ont été divisés en un plasma contrôle non traité (C ; 300 mL) et un plasma liquide traité par Intercept (PL-IA ; 600 mL). PL-IA et C ont été stockés à 4 °C et échantillonnés jusqu’à j14. Les tests étaient : taux de prothrombine (INR), temps de céphaline activée (TCA), fibrinogène (Fg), FV, FVII, FVIII et génération de thrombine. Résultats Une légère prolongation du TQ et du TCA (11–12 %) est observée après le traitement. Le Fg, FV, FVII et le FVIII sont maintenus à 82 %, 93 %, 78 % et 73 % des valeurs prétraitement. Les facteurs dans PL-IA ont une stabilité similaire à C lors du stockage. Au jour 14, l’activité dans PL-IA de ces facteurs est encore à 81 % (Fg), 92 % (FV), 80 % (FVII) et 69 % (FVIII) des valeurs de C. Les tests de génération de thrombine (Tableau 1, facteur tissulaire 5pM) montrent des résultats similaires pour les paramètres du thrombogramme dans PL-IA et C. Conclusion Le plasma traité PL-IA conserve des propriétés coagulantes et une stabilité lors du stockage similaire à celles dans le plasma liquide de contrôle. Background: Currently, donor screening for selected pathogens is the only protection against infection associated with RBC transfusions. A Pathogen Inactivation (P.I.) process for RBCs based on Helinx technology has been developed using S-303, a novel compound. S-303 treatment of RBCs inactivates a broad spectrum and high titers of viruses and bacteria, while maintaining red cell in vitro and in vivo function. Early clinical trials have demonstrated viability and lack of neoantigenicity for S-303 RBCs. We are currently conducting a two-part trial in healthy volunteers utilizing the final system configuration. Part A compares the recovery and survival of S-303 RBCs to ADSOLÒ RBCs using a crossover regimen. Part B examined the safety and tolerability of S-303 RBC full unit infusions in 11 subjects. Study Design: Forty healthy subjects donated 500 mL of whole blood that was leukocyte reduced then processed into either ADSOL RBCs or S-303 RBCs. Under careful observation, 11 subjects received full unit infusions of S-303 RBCs. The other 29 subjects were randomized to receive 10-30 mL of either 51Cr labeled S-303 RBCs or ADSOL RBCs in a crossover manner. All subjects received 35 day-stored autologous RBCs. Results: No serious adverse events or laboratory abnormalities were observed in either part of the study. Posttransfusion antiglobulin tests were negative. Unpaired RBC recovery and survival pre-crossover from Part A of the study were within an acceptable range. See table below (means and standard deviations). Conclusion: Full unit transfusions of autologous S-303 RBCs were well tolerated in healthy subjects. Unpaired pre-crossover analysis of data examining RBC recovery and survival study suggest no clinically significant difference between ADSOL RBCs and S-303 RBCs. S-303 RBCs had an average recovery greater than 75% 24 h after transfusion and normal lifespan and could therefore enhance RBC transfusion safety without compromising therapeutic efficacy.
