Musculoskeletal (MSK) models offer great potential for predicting the muscle forces required to inform more detailed simulations of vertebral endplate loading in adolescent idiopathic scoliosis (AIS). In this work, simulations based on static optimization were compared with in vivo measurements in two AIS patients to determine whether computational approaches alone are sufficient for accurate prediction of paraspinal muscle activity during functional activities. We used biplanar radiographs and marker-based motion capture, ground reaction force, and electromyography (EMG) data from two patients with mild and moderate thoracolumbar AIS (Cobb angles: 21° and 45°, respectively) during standing while holding two weights in front (reference position), walking, running, and object lifting. Using a fully automated approach, 3D spinal shape was extracted from the radiographs. Geometrically personalized OpenSim-based MSK models were created by deforming the spine of pre-scaled full-body models of children/adolescents. Simulations were performed using an experimentally controlled backward approach. Differences between model predictions and EMG measurements of paraspinal muscle activity (both expressed as a percentage of the reference position values) at three different locations around the scoliotic main curve were quantified by root mean square error (RMSE) and cross-correlation (XCorr). Predicted and measured muscle activity correlated best for mild AIS during object lifting (XCorr's ≥ 0.97), with relatively low RMSE values. For moderate AIS as well as the walking and running activities, agreement was lower, with XCorr reaching values of 0.51 and comparably high RMSE values. This study demonstrates that static optimization alone seems not appropriate for predicting muscle activity in AIS patients, particularly in those with more than mild deformations as well as when performing upright activities such as walking and running. Objectives: Non-operative management (NOM) is increasingly utilised in blunt abdominal trauma. The 1994 American Association of Surgery of Trauma grading (1994-AAST) is applied for clinical decision-making in many institutions. Recently, classifications incorporating contrast extravasation such as the CT severity index (CTSI) and 2018 update of the liver and spleen AAST were proposed to predict outcome and guide treatment, but validation is pending. Methods: CT images of patients admitted 2000–2016 with blunt splenic and hepatic injury were systematically re-evaluated for 1994/2018-AAST and CTSI grading. Diagnostic accuracy, diagnostic odds ratio (DOR), and positive and negative predictive values were calculated for prediction of in-hospital mortality. Correlation with treatment strategy was assessed by Cramer V statistics. Results: Seven hundred and three patients were analysed, 271 with splenic, 352 with hepatic and 80 with hepatosplenic injury. Primary NOM was applied in 83% of patients; mortality was 4.8%. Comparing prediction of mortality in mild and severe splenic injuries, the CTSI (3.1% vs. 10.3%; diagnostic accuracy = 75.4%; DOR = 3.66; p = 0.006) and 1994-AAST (3.3% vs. 10.5%; diagnostic accuracy = 77.9%; DOR = 3.45; p = 0.010) were more accurate compared with the 2018-AAST (3.4% vs. 8%; diagnostic accuracy = 68.2%; DOR = 2.50; p = 0.059). In hepatic injuries, the CTSI was superior to both AAST classifications in terms of diagnostic accuracy (88.7% vs. 77.1% and 77.3%, respectively). CTSI and 2018-AAST correlated better with the need for surgery in severe vs. mild hepatic (Cramer V = 0.464 and 0.498) and splenic injuries (Cramer V = 0.273 and 0.293) compared with 1994-AAST (Cramer V = 0.389 and 0.255; all p < 0.001). Conclusions: The 2018-AAST and CTSI are superior to the 1994-AAST in correlation with operative treatment in splenic and hepatic trauma. The CTSI outperforms the 2018-AAST in mortality prediction. Many modern Internet services and applications operate on top of geographically distributed datacenters, which replicate large volumes of business data and other content, to improve performance and reliability. This leads to high volumes of continuous network traffic between datacenters, constituting the bulk of traffic exchanged over the Wide-Area Networks (WANs) that connect datacenters. Operators heavily rely on the efficient and timely delivery of such traffic, as it is key for the performance of distributed datacenter applications, and bulk inter-datacenter traffic also plays an important role for network capacity planning. In this article, we discuss the unique and salient features of bulk across-datacenter transfers and extensively review recent technologies and existing research solutions proposed in the literature for optimization of such traffic. This paper initiates the study of the classic balanced graph partitioning problem from an online perspective: Given an arbitrary sequence of pairwise communication requests between n nodes, with patterns that may change over time, the objective is to service these requests efficiently by partitioning the nodes into L clusters, each of size k, such that frequently communicating nodes are located in the same cluster. The partitioning can be updated dynamically by migrating nodes between clusters. The goal is to devise online algorithms which jointly minimize the amount of intercluster communication and migration cost. The problem features interesting connections to other well-known online problems. For example, scenarios with L = 2 generalize online paging, and scenarios with k = 2 constitute a novel online variant of maximum matching. We present several lower bounds and algorithms for settings both with and without cluster-size augmentation. In particular, we prove that any deterministic online algorithm has a competitive ratio of at least k, even with significant augmentation. Our main algorithmic contributions are an O(k log k)-competitive deterministic algorithm for the general setting with constant augmentation and a constant competitive algorithm for the maximum matching variant. Randomness beacons are services that periodically emit a random number, allowing users to base decisions on the same random value without trusting anyone: ideally, the randomness beacon does not only produce unpredictable values, but is also of low computational complexity for the users, bias-resistant and publicly verifiable. Such randomness beacons can serve as an important primitive for smart contracts in a variety of contexts. This paper first presents a structured security analysis, based on which we then design, implement, and evaluate a trustworthy and efficient randomness beacon. Our approach does not require users to register or run any computationally intensive operations. This paper studies a fundamental dynamic clustering problem. The input is an online sequence of pairwise communication requests between n nodes (e.g., tasks or virtual machines). Our goal is to minimize the communication cost by partitioning the communicating nodes into ℓ clusters (e.g., physical servers) of size k (e.g., number of virtual machine slots). We assume that if the communicating nodes are located in the same cluster, the communication request costs 0; if the nodes are located in different clusters, the request is served remotely using inter-cluster communication, at cost 1. Additionally, we can migrate: a node from one cluster to another at cost α ≥ 1 . We initiate the study of a stochastic problem variant where the communication pattern follows a fixed distribution, set by an adversary. Thus, the online algorithm needs to find a good tradeoff between benefitting from quickly moving to a seemingly good configuration (of low inter-cluster communication costs), and the risk of prematurely ending up in a configuration which later turns out to be bad, entailing high migration costs. Our main technical contribution is a deterministic online algorithm which is O (log n) -competitive with high probability (w.h.p.), for a specific but fundamental class of problems: namely on ring graphs. Purpose: To assess the impact of serum androgen levels and androgen receptor CAG polymorphism on sexual function in 45 healthy midlife women living in a heterosexual relationship.Methods: Cross-sectional study [Cantonal Ethics Committee Bern (Ref.-Nr.Kek-be: 087/13)].Main Outcome Measures: Association between androgen serum levels, androgen receptor CAG polymorphism and sexual function was assessed by the FSFI-d questionnaire.Results: In our cohort of healthy, midlife, well-educated, middle-class, mostly postmenopausal women living in a heterosexual satisfying partnership, sexual function was perceived to remain stable or to decline during menopausal transition with sexual desire scoring lowest (FSFI-d 3.3 ± 0.9). Androgen serum levels did not correlate with sexual function. Mean CAG repeat number was 21.6 ± 1.9. There was a highly inverse though non-significant correlation between female sexual function and AR CAG repeat polymorphism with specifically higher numbers of CAG repeats being significantly positively correlated to more frequent or more severe pain during or after sexual intercourse.Conclusion: The AR polymorphism is a non-negligible factor in female sexual function. An ophiolitic mélange of Avala Mountain located south of Belgrade was studied to obtain age data from blocks of radiolarian cherts and to enable better understanding of age and nature of this mélange. The studied area is located south of Avala Mt. and its geotectonic position is rather obscure. The obtained data allow us to determine radiolarians from five samples; one is of Late Anisian to Early Ladinian, and four of them are of Middle Jurassic (Late Bathonian-Early Callovian) age. The finding of Triassic as well as Jurassic age radiolarians in one and the same mélange suggests that this mélange formed during obduction of the Western Vardar ophiolites. [ABSTRACT FROM AUTHOR]

Background: Structural leg length discrepancy (LLD) is a common phenomenon. However, its effect on spinal gait kinematics remains unclear.Research Question: How does LLD affect spinal gait kinematics in patients with structural LLD and what is the immediate effect of a shoe lift?.Methods: 10 adolescents with structural LLD (20-60 mm) and 14 healthy controls were included. All of whom were fitted with a trunk marker set and requested to walk barefoot as well as with an orthotic shoe lift (only patients). Data were collected using a 12-camera motion capture system. Group comparisons were conducted using one-dimensional Statistical Parametric Mapping (SPM).Results: Patients with LLD showed statistically significant increased frontal plane lumbar bending angles to the longer side (p = 0.007), increased pelvic drop on the shorter side (p < 0.001) and increased hip adduction angles on the longer leg (p < 0.001) compared to the healthy controls. In the sagittal plane, patients demonstrated changed knee (shorter leg) and ankle joint (longer leg) motion. All gait deviations observed in patients with LLD could immediately be altered by correcting the LLD using a shoe lift.Significance: Due to the LLD, patients showed a lateral pelvic drop on the shorter side, which appeared to be compensated for by a contralateral bending in the lumbar spine and a lateral shift of the pelvis towards the longer side. Diminished hip abductor strength decreases postural control which is a parameter that is associated with an increased risk of ankle sprains. In our study we evaluated the influence of hip abductor fatigue on sagittal plane ankle kinematics and shank muscle activity during a single-leg forward jump. Sagittal ankle plane kinematics are important in ankle sprains but less studied than frontal plane kinematics. Therefore, we evaluated differences in sagittal ankle kinematics (12-camera motion capture system) and lower limb surface electromyographic muscle activity in 20 healthy, recreationally active adults (9 females, 11 males, mean age 30.3 SD 4.0 years, mean BMI 23.6 SD 2.8 kg/m2) before and after a hip abductor fatigue protocol (200-ms prior, at and in the 250-ms following initial contact (IC)). After fatigue, the maximal ankle plantar-flexion angle decreased prior to IC (median 3.8° [interquartile range 0.1, 7.2], p = 0.014), at IC (4.1° [-0.3, 5.0], p = 0.027) and post IC (4.1° [-1.3, 5.0] p = 0.036). Gastrocnemius activity onset was delayed (-28.0 ms [-44.0, 0.0], p < 0.01). Average activity of the tibialis anterior increased prior to IC (pre-fatigue 19.32% [14.89, 33.45], post-fatigue 28.95% [18.49, 34.81], p < 0.05). Background: Unilateral knee flexion contractures (KFC) are frequently seen in orthopedic rehabilitation and often interfere with the normal gait pattern, resulting in passive and/or active secondary deviations. In order to prevent KFC-related complications such as patellofemoral pain or the knee spine syndrome, a comprehensive understanding of such deviations is necessary.Research Question: How does an artificially induced unilateral KFC affect whole body biomechanics in young healthy adults during gait?Methods: Twenty-four healthy young adults (females/males: 13/11; mean age: 27.3 ± 3.8 years) were included in this cross-sectional study. Using an 8-camera optical motion capture system and two embedded force plates, three-dimensional lower extremity, pelvis and trunk kinematics as well as lower extremity joint moments were derived during normal walking and walking with unilateral KFCs by means of a lightweight knee brace locked at 30° and 60° of flexion. Data were analyzed using one-dimensional statistical parametric mapping, allowing explorative group comparisons of continuous data rather than pre-defined discrete parameters.Results: On the braced side, increased hip flexion (p < 0.001, 1-100 % of gait cycle [%GC]), knee flexion (p < 0.001, 1-72 and 82-100 %GC) and ankle dorsiflexion angles (p < 0.001, 1-100 %GC) as well as external knee flexion moments (p < 0.001, 1-98 % of stance phase [%StPh]) and decreased ankle dorsiflexion (p < 0.001, 74-94 %StPh), hip flexion and hip adduction moments (p < 0.001, 1-32 %StPh and p < 0.001, 71-92 %StPh, respectively) were observed. The unbraced side showed similar but less pronounced deviations. Pelvis and upper body kinematics were not altered, suggesting that the lower limbs fully compensate for KFCs of less than 30°.Significance: Asymmetric limb loading and considerable increases in external knee flexion moments might increase the risk for adverse effects of lower extremity joints in the long-term. Emerging networked systems become increasingly flexible, reconfigurable, and "self-⁎". This introduces an opportunity to adjust networked systems in a demand-aware manner, leveraging spatial and temporal locality in the workload for online optimizations. However, it also introduces a tradeoff: while more frequent adjustments can improve performance, they also entail higher reconfiguration costs. This paper studies self-adjusting grid networks in which frequently communicating nodes (e.g., virtual machines) are moved topologically closer in an online and demand-aware manner, striking a balance between the benefits and costs of reconfigurations. The paper presents a general Ω (log ⁡ n) lower bound for this problem, even in scenarios where the demand graph is constant once learned. This paper considers the problem of how to efficiently share a wireless medium which is subject to harsh external interference or even jamming. So far, this problem is understood only in simplistic single-hop or unit disk graph models. We in this paper initiate the study of MAC protocols for the SINR interference model (a.k.a. physical model). This paper makes two contributions. First, we introduce a new adversarial SINR model which captures a wide range of interference phenomena. Concretely, we consider a powerful, adaptive adversary which can jam nodes at arbitrary times and which is only limited by some energy budget. Our second contribution is a distributed MAC protocol called Sade which provably achieves a constant competitive throughput in this environment: we show that, with high probability, the protocol ensures that a constant fraction of the non-blocked time periods is used for successful transmissions. Central Anatolia exposes previously buried and metamorphosed, continent-derived rocks – the Kırşehir and Afyon zones – now covering an area of ∼300 × 400 km. So far, the exhumation history of these rocks has been poorly constrained. We show for the first time that the major, >120 km long, top-NE ‘Ivriz’ Detachment controlled the exhumation of the HP/LT metamorphic Afyon Zone in southern Central Anatolia. We date its activity at between the latest Cretaceous and early Eocene times. Combined with previously documented isolated extensional detachments found in the Kırşehir Block, our results suggest that a major province governed by extensional exhumation was active throughout Central Anatolia between ∼80 and ∼48 Ma. Although similar in dimension to the Aegean extensional province to the east, the Central Anatolian extensional province is considerably older and was controlled by a different extension direction. From this, we infer that the African slab(s) that subducted below Anatolia must have rolled back relative to the Aegean slab since at least the latest Cretaceous, suggesting that these regions were underlain by a segmented slab. Whether or not these early segments already corresponded to the modern Aegean, Antalya, and Cyprus slab segments remains open for debate, but slab segmentation must have occurred much earlier than previously thought. Most studies investigating trunk kinematics have not provided adequate quantification of spinal motion, resulting in a limited understanding of the healthy spine's biomechanical behavior during gait. This study aimed at assessing spinal motion during gait in adolescents, adults and older individuals. Fourteen adolescents (10-18years), 13 adults (19-35years) and 15 older individuals (≥65years) were included. Using a previously validated enhanced optical motion capture approach, sagittal and frontal plane spinal curvature angles and general trunk kinematics were measured during shod walking at a self-selected normal speed. Postural differences indicated that lumbar lordosis and thoracic kyphosis increase throughout adolescence and reach their peak in adulthood. The absence of excessive thoracic kyphosis in older individuals could be explained by a previously reported subdivision in those who develop excessive kyphosis and those who maintain their curve. Furthermore, adults displayed increased lumbar spine range of motion as compared to the adolescents, whereas the increased values in older individuals were found to be related to higher gait speeds. This dataset on the age-related kinematics of the healthy spine can serve as a basis for understanding pathological deviations and monitoring rehabilitation progression. [ABSTRACT FROM AUTHOR]

The waning stage(s) of the Tethyan ocean(s) in the Balkans are not well understood. Controversy centres on the origin and life-span of the Cretaceous Sava Zone, which is allegedly a remnant of the last oceanic domain in the Balkan Peninsula, defining the youngest suture between Eurasia- and Adria-derived plates. In order to investigate to what extent Late-Cretaceous volcanism within the Sava Zone is consistent with this model we present new age data together with trace-element and Sr-Nd-Pb isotope data for the Klepa basaltic lavas from the central Balkan Peninsula. Our new geochemical data show marked differences between the Cretaceous Klepa basalts (Sava Zone) and the rocks of other volcanic sequences from the Jurassic ophiolites of the Balkans. The Klepa basalts mostly have Sr-Nd-Pb isotopic and trace-element signatures that resemble enriched within-plate basalts substantially different from Jurassic ophiolite basalts with MORB, BAB and IAV affinities. Trace-element modelling of the Klepa rocks indicates 2%-20% polybaric melting of a relatively homogeneously metasomatised mantle source that ranges in composition from garnet lherzolite to ilmenite+apatite bearing spinel-amphibole lherzolite. Thus, the residual mineralogy is characteristic of a continental rather than oceanic lithospheric mantle source, suggesting an intracontinental within-plate origin for the Klepa basalts. Two alternative geodynamic models are internally consistent with our new findings: (1) if the Sava Zone represents remnants of the youngest Neotethyan Ocean, magmatism along this zone would be situated within the forearc region and triggered by ridge subduction; (2) if the Sava Zone delimits a diffuse tectonic boundary between Adria and Europe which had already collided in the Late Jurassic, the Klepa basalts together with a number of other magmatic centres represent volcanism related to transtensional tectonics. Background and Purpose: Depending on the extent of a structural leg length discrepancy (LLD), several compensatory mechanisms take place in order to maintain function and to optimize energy consumption during gait. However, studies describing the influence of a structural LLD on upper limb motion are lacking. The current study therefore aimed at the evaluation of upper limb motion during gait in LLD patients compared to healthy controls.Methods: Motion capture data from 14 patients with structural LLD and 15 healthy controls that were collected during barefoot walking at a self-selected speed were retrospectively analyzed. Specifically, kinematic parameters of the shoulder and elbow joints as well as the trunk segment were investigated and considered in relation to a minimal clinically important difference of 5°.Results: The shoulders in LLD patients were kept constantly in a more extended and at initial contact in a more adducted position as compared to healthy controls. In addition, the patients' elbow joints showed increased flexion motion and the trunk segment indicated a constant trunk lateral-flexion and axial rotation tendency towards the affected side.Conclusions: Patients with structural LLD indicated clinically relevant secondary deviations in shoulder and elbow motion. While some of these deviations were most likely passive physical effects, others might have occurred as active strategies to maintain balance or to regulate total body angular momentum. These findings contribute to the understanding of secondary gait deviations induced by a structural LLD and might serve as a basis for further investigations using complex musculoskeletal models. Fear-avoidance beliefs, particularly the fear of lifting with a flexed spine, are associated with reduced spinal motion during object lifting. Low back pain patients thereby also showed potentially clinically relevant changes in the spatial distribution of back muscle activity, but it remains unknown whether such associations are also present in pain-free individuals. This cross-sectional observational study investigated the relationship between fear-avoidance beliefs and the spatial distribution of lumbar paraspinal muscle activity in pain-free individuals during a repetitive lifting task. Thirty participants completed two pain-related fear questionnaires and performed 25 repetitions of lifting a 5 kg-box from a lower to an upper shelf and back, while multi-channel electromyographic signals were recorded bilaterally from the lumbar erector spinae muscles. Changes in spatial distribution were defined as the differences in vertical position of the weighted centroids of muscle activity (centroid shift) between the first and last few repetitions. Linear regression analyses were performed to examine the relationships between centroid shift and fear-avoidance belief scores. Fear of lifting an object with a flexed spine was negatively associated with erector spinae activity centroid shift (R2 adj. = 0.1832; p = 0.045), which might be an expression of behavioral alterations to prevent the back from possible harm. Thermochronometric data collected across the Alps over the last three decades allows for investigation of the evolution of this orogen, which is subject to changes in climate and geodynamics. Exhumation rates are inferred from the thermochronometric ages using a statistical inversion method based on the fact that the distance a sample traveled since closure is equal to the integral of the exhumation rate from the present day to the age of the sample. Exhumation rates are assumed to be spatially correlated but are free to vary through time. This results in the quantification of exhumation rates across the Alps, since 32 Ma, along with assessments of the quality of these inferences. We find that exhumation rates are initially fast in the internal arc of the Western Alps at rates up to 0.8 km/Myr at 30 Ma, decreasing at 20 Ma to 0.3 km/Myr to remain slow to the present. At the same time, around 20 Ma, rates across the External Crystalline Massifs of Western Alps increase to 0.6 km/Myr. We also find that the onset of high exhumation rates in the Tauem Window and the Lepontine Dome occurs at around 20 Ma, a time characterized by major reorganizations in the Alpine chain. A general increase in exhumation rates at around 5 Ma over the entire Alps is not confirmed. Instead we find that the Western Alps exhibit a 2 to 3 fold increase in exhumation rate over the last 2 Ma, during a recent event not seen further east, in spite of very similar topographic characteristics. Introduction: It is important to assess and quantify gait in order to determine the severity of impairments during gait and to evaluate therapeutic interventions. However, laboratory gait assessment is expensive and time consuming and there is a lack of an easily applicable tool for the quantification of gait in clinical practice. The aim of this study was to validate a smartphone-based measurement tool for the quantification of level walking.Methods: Vertical center of mass displacement and step duration of 22 healthy young adults were assessed by a smartphone application and a motion capture system. Intra-session reliability was evaluated by repeated-measures ANOVA, intraclass correlation coefficient (ICC), and standard error of measurement. In order to evaluate the concurrent validity of the smartphone application, smartphone- and motion capture-derived values were compared by Pearson correlation coefficient and Bland-Altman limits of agreement.Results: Six out of eight variables derived by the smartphone application showed an excellent reliability (ICC≥0.75) and all variables correlated significantly with measurements of the motion capture system with moderate to strong correlations ranging from 0.61 to 0.92.Conclusion: The results showed a great potential of the smartphone application to be a user-friendly and valid tool for the assessment of gait in clinical practice. Further research needs to investigate whether the smartphone application is able to detect differences in gait patterns following therapeutic or orthopedic interventions and whether it is valid for the quantification of gait in people with movement disorders. Introduction Recently, it has been demonstrated that biomechanical parameters measured with accelerometers during everyday activities might provide early information about mobility in seniors. The use of smartphones has been suggested for this purpose. The aim of this study was to develop and validate a smartphone-based application for the quantification of the sit-to-stand movement (STS) in healthy seniors. Methods Sixteen healthy seniors performed the STS with a smartphone first attached to the lower back and then to the sternum. The subjects’ vertical ground reaction forces (VGRF) and vertical acceleration (VAcc) were recorded simultaneously using two force plates (reference standard) and the smartphone, respectively. VAcc data were converted to force; power was then calculated based on both force-time curves. Subsequently, the total movement duration ( T total ), peak force ( F max ), rate of force development (RFD) and peak power ( P max ) were determined. Relative (intraclass correlation coefficient [ICC]) and absolute (standard error of measurement [SEM]) reliability were also calculated. For the variables showing strong reliability (ICC ≥ 0.75), Pearson correlation coefficients ( r ) between VGRF- and VAcc-derived data and one-sample T -tests for the mean differences were carried out. Results In the sternal configuration, all variables showed strong reliability (range: 0.86–0.93), except the smartphone-derived parameter RFD (ICC = 0.43). F max and T total were found to be highly valid ( r = 0.86 and 0.98, respectively). Conclusion The results showed that the smartphone-based application can be used as a reliable and valid tool for the quantification of the STS in healthy seniors if the proposed protocol is followed. Introduction Admission blood glucose is known to be a predictor for outcome in several disease patterns, especially in critically ill trauma patients. The underlying mechanisms for the association of hyperglycaemia and poor outcome are still not proven. It was hypothesised that hyperglycaemia upon hospital admission is associated with haemorrhagic shock and in-hospital mortality. Methods Data was extracted from an observational trauma database of the level 1 trauma centre at Innsbruck Medical University hospital. Trauma patients (≥18 years) with multiple injuries and an Injury Severity Score ≥17 were included and analysed. Results In total, 279 patients were analysed, of which 42 patients (15.1%) died. With increasing blood glucose upon hospital admission, the rate of patients with haemorrhagic shock rose significantly [from 4.4% (glucose 4.1–5.5 mmol/L) to 87.5% (glucose >15 mmol/L), p < 0.0001]. Mortality was also associated with initial blood glucose [≤5.50 mmol/L 8.3%; 5.51–7.50 mmol/L 10.9%, 7.51–10 mmol/L 12.4%; 10.01–15 mmol/L 32.0%; ≥15.01 mmol/L 12.5%, p = 0.008]. Admission blood glucose was a better indicator for haemorrhagic shock (cut-off 9.4 mmol/L, sensitivity 67.1%, specificity 83.9%) than haemoglobin, base excess, bicarbonate, pH, lactate, or vital parameters. Regarding haemorrhagic shock, admission blood glucose is more valuable during initial patient assessment than the second best predictive parameter, which was admission haemoglobin (cut-off value 6.5 mmol/L (10.4 g/dL): sensitivity 61.3%, specificity 83.9%). Conclusions In multiple trauma, non-diabetic patients, admission blood glucose predicted the incidence of haemorrhagic shock. Admission blood glucose is an inexpensive, rapidly and easily available laboratory value that might help to identify patients at risk for haemorrhagic shock during initial evaluation upon hospital admission. An estimated 78% of women regularly walk in high heels. However, up to 58% complain about low back pain, which is commonly thought to be caused by increased lumbar lordosis. However, the extent to which a subject's posture is modified by high-heeled shoes during dynamic activities remains unknown. Therefore, we sought to evaluate whether low- or high-heeled shoes influence the kinematics of the pelvis and the spine during walking. Twenty-three inexperienced women, and seventeen women experienced in wearing high-heeled shoes, all aged 20-55 years, were measured barefoot and while wearing low- (4cm) and high-heeled (10cm) shoes during gait at a self-selected speed. A 22-camera motion capture system was used to assess the gait patterns for each condition. No significant inter-experience-group kinematic differences were found. In contrast to the results of some studies, our results show that the heels' height does indeed influence the motion of the pelvis and the spine during walking, whereby low-heeled shoes influenced the subjects' trunk kinematics during gait less than high-heeled shoes compared to barefooted walking. However, inexperienced high-heel wearers showed less thoracic curvature angle while wearing high-heels than while wearing low-heels. Importantly, both groups exhibited significantly lower maximum and minimal lumbar and thoracic curvature angles when wearing high-heeled shoes compared to the barefoot condition. Topological self-stabilization is an important concept to build robust open distributed systems (such as peer-to-peer systems) where nodes can organize themselves into meaningful network topologies. The goal is to devise distributed algorithms where nodes forward, insert, and delete links to neighboring nodes, and that converge quickly to such a desirable topology, independently of the initial network configuration. This article proposes a new model to study the parallel convergence time. Our model sheds light on the achievable parallelism by avoiding bottlenecks of existing models that can yield a distorted picture. As a case study, we consider local graph linearization-i.e., how to build a sorted list of the nodes of a connected graph in a distributed and self-stabilizing manner. In order to study the main structure and properties of our model, we propose two variants of a most simple local linearization algorithm. For each of these variants, we present analyses of the worst-case and best-case parallel time complexities, as well as the performance under a greedy selection of the actions to be executed. It turns out that the analysis is non-trivial despite the simple setting, and to complement our formal insights we report on our experiments which indicate that the runtimes may be better in the average case. Background Since 2002, the professional education for Swiss physiotherapists has been upgraded to a tertiary educational level. With this change, the need for research related to professional practice has become more salient. The elaboration of research priorities is seen as a possible way to determine the profession's needs, to help coordinate research collaborations and to address expectations regarding physiotherapy. There is still limited evidence about stakeholders' views with regard to physiotherapy research. The objective of this study was to investigate key stakeholders' opinions about research in physiotherapy in Switzerland. Methods Focus groups with patients, health professionals, researchers and representatives of public health organizations were conducted, and semi-structured interviews were conducted with politicians, health insurers and medical doctors from three linguistic regions in Switzerland. An interview guide was elaborated. Data were transcribed and analysed using inductive content analysis (Atlas-ti 6®). Results Eighteen focus groups and 23 interviews/written commentaries included 134 participants with various research experiences and from different settings. Fourteen categories were defined reflecting three themes: identity, interdisciplinarity and visibility. Stakeholders had positive views about the profession and perceived physiotherapists' important role now and in the future. Yet, they also felt that physiotherapy was not sufficiently recognized in society and not visible enough. A stronger professional identity would be key to enhancing interdisciplinary work. Conclusions Results of this qualitative study provide insights into key aspects for moving the physiotherapy profession forward. Identity is at the heart of physiotherapy, not necessarily in terms of research priorities but in the definition of domains of competence and future positioning. Identity is also tightly connected to Interdisciplinarity as this might threaten the existence of the profession. Stakeholders outside the profession insist on the importance of visibility. Multichronometric analyses were performed on samples from a transect in the French–Italian Western Alps crossing nappes derived from the Briançonnais terrane and the Piemonte–Liguria Ocean, in an endeavour to date both high-pressure (HP) metamorphism and retrogression history. Twelve samples of white mica were analysed by 39Ar–40Ar stepwise heating, complemented by two samples from the Monte Rosa nappe 100 km to the NE and also attributed to the Briançonnais terrane. One Sm–Nd and three Lu–Hf garnet ages from eclogites were also obtained. White mica ages decrease from c. 300 Ma in the westernmost samples (Zone Houillère), reaching c. 300°C during Alpine metamorphism, to <48 Ma in the internal units to the east, which reached c. 500°C during the Alpine orogeny. The spatial pattern of Eocene K–Ar ages demonstrates that Si-rich HP white mica records the age of crystallization at 47–48 Ma and retains Ar at temperatures of around 500°C. Paleocene–early Eocene Lu–Hf and Sm–Nd ages, recording prograde garnet growth before the HP peak, confirm eclogitization in Eocene times. Petrological and microstructural features reveal important mineralogical differences along the transect. All samples contain mixtures of detrital, syn-D1 and syn-D2 mica, and retrogression phases (D3) in greatly varying proportions according to local variations in the evolution of pressure–temperature–fluid activity–deformation (P–T–a–D) conditions. Samples from the Zone Houillère mostly contain detrital mica. The abundance of white mica with Si > 6·45 atoms per formula unit increases eastward. Across the whole traverse, phengitic mica grown during HP metamorphism defines the D1 foliation. Syn-D2 mica is more Si-poor and associated with nappe stacking, exhumation, and hydrous retrogression under greenschist-facies conditions. Syn-D1 phengite is very often corroded, overgrown by, or intergrown with, syn-D2 muscovite. Most importantly, syn-D2 recrystallization is not limited to S2 schistosity domains; micrometre-scale chemical fingerprinting reveals muscovite pseudomorphs after phengite crystals, which could be mistaken for syn-D1 mica based on microstructural arguments alone. The Cl/K ratio in white mica is a useful discriminator, as D2 retrogression was associated with a less saline fluid than eclogitization. As petrology exerts the main control on the isotope record, constraining the petrological and microstructural framework is necessary to correctly interpret the geochronological data, described in both the present study and the literature. Our approach, which ties geochronology to detailed geochemical, petrological and microstructural investigations, identifies 47–48 Ma as the age of HP formation of syn-D1 mica along the studied transect and in the Monte Rosa area. Cretaceous apparent mica ages, which were proposed to date eclogitization by earlier studies based on conventional ‘thermochronology’, are due to Ar inheritance in incompletely recrystallized detrital mica grains. The inferred age of the probably locally diachronous, greenschist-facies, low-Si, syn-D2 mica ranges from 39 to 43 Ma. Coexistence of D1 and D2 ages, and the constancy of non-reset D1 ages along the entire transect, provides strong evidence that the D1 white mica ages closely approximate formation ages. Volume diffusion of Ar in white mica (activation energy E = 250 kJ mol−1; pressure-adjusted diffusion coefficient D'0 < 0·03 cm2 s−1) has a subordinate effect on mineral ages compared with both prograde and retrograde recrystallization in most samples. This study presents zircon fission track data from the Bucovinian nappe stack (northern part of the Inner Eastern Carpathians, Rodna Mountains) and a neighbouring part of the Biharia nappe system (Preluca massif) in order to unravel the thermal history of the area and its structural evolution by integrating the fission track data with published data on the tectonic and sedimentary evolution of the area. The increase of metamorphic temperatures towards the SW detected by the zircon fission track data suggests SW-wards increasing tectonic overburden (up to at least 15 km) and hence top NE thrusting. Sub-greenschist facies conditions during the Alpine metamorphic overprint only caused partial annealing of fission tracks in zircon in the external main chain of the Central Eastern Carpathians. Full annealing of zircon points to at least 300 °C in the more internal elements (Rodna Mountains and Preluca massif). The zircon fission track central and single grain ages largely reflect Late Cretaceous cooling and exhumation. A combination of fission track data and stratigraphic constraints points to predominantly tectonic differential exhumation by some 7-11 km, connected to massive Late Cretaceous extension not yet detected in the area. Later events such as the latest Cretaceous ('Laramian') juxtaposition of the nappe pile with the internal Moldavides, causing exhumation by erosion, re-burial by sedimentation and tectonic loading during the Cenozoic had no impact on the zircon fission track data; unfortunately it prevented a study of the low temperature part of the Late Cretaceous exhumation history. Abstract: Network virtualization is an important concept to overcome the ossification of today’s Internet as it facilitates innovation also in the network core and as it promises a more efficient use of the given resources and infrastructure. Virtual networks (VNets) provide an abstraction of the physical network: multiple VNets may cohabit the same physical network, but can be based on completely different protocol stacks (also beyond IP). One of the main challenges in network virtualization is the efficient admission control and embedding of VNets. The demand for virtual networks (e.g., for a video conference) can be hard to predict, and once the request is accepted, the specification/QoS guarantees must be ensured throughout the VNet’s lifetime. This requires an admission control algorithm which only selects high-benefit VNets in times of scarce resources, and an embedding algorithm which realizes the VNet in such a way that the likelihood that future requests can be embedded as well is maximized. This article describes a generic algorithm for the online VNet embedding problem which does not rely on any knowledge of the future VNet requests but whose performance is competitive to an optimal offline algorithm that has complete knowledge of the request sequence in advance: the so-called competitive ratio is, loosely speaking, logarithmic in the sum of the resources. Our algorithm is generic in the sense that it supports multiple traffic models, multiple routing models, and even allows for nonuniform benefits and durations of VNet requests. Most pesticides, herbicides and other plant treatment agents are applied to the crop surface. Direct mass spectrometric methods, such as desorption electrospray ionization (DESI), offer new ways to analyze plant samples directly and rapidly. A strategy for the development and optimization of a DESI method for the direct determination of chemicals on complex surfaces is described. Chlorpropham (CP) was applied to potato surfaces as an example for a crop protection agent and analyzed using a self-made DESI source. Aspects such as instrument selectivity, sensitivity and reproducibility were investigated. The MS4 fragmentation pattern of CP was analyzed to achieve the necessary detection selectivity, and is discussed in detail. Similar fragmentation was found in the ESI and DESI mass spectra, indicating that the mechanisms of ESI and DESI are closely related. A DESI method for semi-quantification of CP on potatoes was developed. Detection limits of 6.5 µg/kg were found using MS/MS. Mobile network operators are experiencing a tremendous increase in data traffic due to the growing popularity of bandwidth-intensive video services. This challenge can be faced either by boosting the capacity of the network infrastructure, or by means of offloading traffic from the backhaul and core network and serving contents from distributed cache servers close to the users. Network operators can extend the coverage of traditional CDNs by making usage of caching locations much closer to the users than traditional CDNs. Additionally, network operators can optimize the caching and delivery of contents by exploiting the complete knowledge of their network for designing a cost-effective infrastructure able to achieve both improved user satisfaction and cost savings. This article provides thoroughly justified design principles for a highly distributed operator-owned CDN while focusing on four key aspects: the optimal location of cache servers, mechanisms for request routing, content replica placement, and content outsourcing and retrieval. With the popularity of YouTube and other video services (e.g., VoD), video content is becoming a dominant traffic type in mobile networks and will soon form the major bulk of the traffic. This poses a serious challenge to mobile network operators when it comes to delivering video content to multiple users in a wireless environment, where the quality of experience of each user must be met as per user expectation. To this end, mobile content distribution networks (mCDNs) are gaining attraction as one possibility to enable controllable and resource-efficient delivery of video content. In this article we motivate the deployment of mCDN serving point (mCSP) nodes in an mCDN infrastructure and describe its benefits for a mobile network operator who wishes to employ an mCDN system in their existing mobile network architecture. In this respect we highlight the key features and requirements of such an mCSP node and the advantages it offers. One particularly beneficial feature of our proposed mCSP node is to ensure fairness during the delivery of progressive video streaming services employed by the likes of YouTube, Daily Motion, and others. In this regard we first highlight the unfairness issue when delivering progressive video streaming services over TCP to multiple users over a wireless network infrastructure and its effect on the user perceived QoE. We then demonstrate, as a proof of concept, the effectiveness of employing application-level scheduling in an mCSP node to ensure fairness among multiple simultaneous progressive video sessions in scenarios where the backhaul link in a mobile network infrastructure may become congested. Abstract: This article studies the construction of self-stabilizing topologies for distributed systems. While recent research has focused on chain topologies where nodes need to be linearized with respect to their identifiers, we explore a natural and relevant 2-dimensional generalization. In particular, we present a local self-stabilizing algorithm which is based on the concept of “local Delaunay graphs” and which forwards temporary edges in greedy fashion reminiscent of compass routing. constructs a Delaunay graph from any initial connected topology and in a distributed manner in time in the worst-case; if the initial network contains the Delaunay graph, the convergence time is only rounds. also ensures that individual node joins and leaves affect a small part of the network only. Such self-stabilizing Delaunay networks have interesting applications and our construction gives insights into the necessary geometric reasoning that is required for higher-dimensional linearization problems. Purpose: The aim of this retrospective cohort study was to evaluate the rate of complications in relation to the extent of surgery and some of its consequences. Methods: Between 1972 and 2010, a total of 5,277 consecutive thyroid gland surgeries with 7,383 nerves at risk were performed at our teaching institution. Data of all patients undergoing thyroidectomy were collected prospectively. A total of 2,867 subtotal resections (first study period from 1972 to 1990) were compared with 2,410 extended thyroid resections involving at least a hemithyroidectomy (second period from 1991 to 2010). Results: The incidence of permanent recurrent laryngeal nerve palsy in primary operations was significantly higher in the first period compared to the second period (3.6 vs. 0.9 %; p < 0.001). Permanent hypoparathyroidism decreased from 3.2 % in the first period to 0.8 % in the second period ( p < 0.001). The incidence of recurrent goiter surgery decreased from 11.1 % in the first period to 8.1 % in the second period ( p < 0.001). No significant difference was found in permanent recurrent laryngeal nerve palsy in recurrent disease between the two periods. The socioeconomic benefits of an extended thyroid resection in our patient population are 360 preventable operations, 90 preventable permanent recurrent laryngeal nerve palsies, and 58 preventable cancers. Furthermore, 37 preventable radioiodine ablations and 15 preventable deaths were associated with more radical thyroid resection. Conclusion: Improvements in surgical technique and change in surgical strategy significantly decreased the prevalence of recurrent laryngeal nerve palsy, hypoparathyroidism, and recurrent disease as well as reduced public health costs associated with recurrent goiter. We report on three-body recombination of a single trapped Rb+ ion and two neutral Rb atoms in an ultracold atom cloud. We observe that the corresponding rate coefficient K3 depends on collision energy and is about a factor of 1000 larger than for three colliding neutral Rb atoms. In the three-body recombination process large energies up to several 0.1 eV are released leading to an ejection of the ion from the atom cloud. It is sympathetically recooled back into the cloud via elastic binary collisions with cold atoms. Further, we find that the final ionic product of the three-body processes is again an atomic Rb+ ion suggesting that the ion merely acts as a catalyzer, possibly in the formation of deeply bound Rb2 molecules. Two age groups were determined for the Cenozoic granitoids in the Dinarides of southern Serbia by high-precision single grain U-Pb dating of thermally annealed and chemically abraded zircons: (1) Oligocene ages (Kopaonik, Drenje, Željin) ranging from 31.7 to 30.6 Ma (2) Miocene ages (Golija and Polumir) at 20.58-20.17 and 18.06-17.74 Ma, respectively. Apatite fission-track central ages, modelling combined with zircon central ages and additionally, local structural observations constrain the subsequent exhumation history of the magmatic rocks. They indicate rapid cooling from above 300°C to ca. 80°C between 16 and 10 Ma for both age groups, induced by extensional exhumation of the plutons located in the footwall of core complexes. Hence, Miocene magmatism and core-complex formation not only affected the Pannonian basin but also a part of the mountainous areas of the internal Dinarides. Based on an extensive set of existing age data combined with our own analyses, we propose a geodynamical model for the Balkan Peninsula: The Late Eocene to Oligocene magmatism, which affects the Adria-derived lower plate units of the internal Dinarides, was caused by delamination of the Adriatic mantle from the overlying crust, associated with post-collisional convergence that propagated outward into the external Dinarides. Miocene magmatism, on the other hand, is associated with core-complex formation along the southern margin of the Pannonian basin, probably associated with the W-directed subduction of the European lithosphere beneath the Carpathians and interfering with ongoing Dinaridic-Hellenic back-arc extension. The Valais units in Savoy (Zone des Brèches de Tarentaise) have been re-mapped in great detail and are subject of combined stratigraphic, structural and petrological investigations summarized in this contribution. The sediments and rare relics of basement, together with Cretaceous age mafic and ultramafic rocks of the Valais palaeogeographical domain, represent the heavily deformed relics of the former distal European margin (External Valais units) and an ocean-continent transition (Internal Valais unit or Versoyen unit) that formed during rifting. This rifting led to the opening of the Valais ocean, a northern branch of the Alpine Tethys. Post-rift sediments referred to as 'Valais trilogy' stratigraphically overlie both External and Internal Valais successions above an angular unconformity formed in Barremian to Aptian times, providing robust evidence for the timing of the opening of the Valais ocean. The Valais units in Savoy are part of a second and more external mid-Eocene high-pressure belt in the Alps that sutured the Briançonnais microcontinent to Europe. Top-N D1-deformation led to the formation of a nappe stack that emplaced the largely eclogite-facies Internal Valais unit (Versoyen) onto blueschist-facies External Valais units. The latter originally consisted of, from internal to external, the Petit St. Bernard unit, the Roc de l'Enfer unit, the Moûtiers unit and the Quermoz unit. Ongoing top-N D2-thrusting and folding substantially modified this nappe stack. Post 35 Ma D3 folding led to relatively minor modifications of the nappe stack within the Valais units but was associated with substantial top-WNW thrusting of the Valais units over the Dauphinois units along the Roselend thrust during W-directed indentation of the Adria block contributing to the formation of the arc of the Western Alps. This study monitors regional changes in the crystallinity of carbonaceous matter (CM) by applying Micro-Raman spectroscopy to a total of 214 metasediment samples (largely so-called Bündnerschiefer) dominantly metamorphosed under blueschist- to amphibolite-facies conditions. They were collected within the northeastern margin of the Lepontine dome and easterly adjacent areas of the Swiss Central Alps. Three-dimensional mapping of isotemperature contours in map and profile views shows that the isotemperature contours associated with the Miocene Barrow-type Lepontine metamorphic event cut across refolded nappe contacts, both along and across strike within the northeastern margin of the Lepontine dome and adjacent areas. Further to the northeast, the isotemperature contours reflect temperatures reached during the Late Eocene subduction-related blueschist-facies event and/or during subsequent near-isothermal decompression; these contours appear folded by younger, large-scale post-nappe-stacking folds. A substantial jump in the recorded maximum temperatures across the tectonic contact between the frontal Adula nappe complex and surrounding metasediments indicates that this contact accommodated differential tectonic movement of the Adula nappe with respect to the enveloping Bündnerschiefer after maximum temperatures were reached within the northern Adula nappe, i.e. after Late Eocene time. regional tectono-geomorphic analysis indicates a Pliocene to recent rock uplift of the outermost segment of the Jura fold-and-thrust belt, which spatially coincides with the intra-continental Rhine-Bresse Transfer Zone. Elevated remnants of the partly eroded Middle Pliocene Sundgau-Forêt de Chaux Gravels identified by heavy mineral analyses allow for a paleo-topographic reconstruction that yields minimum regional Latest Pliocene to recent rock uplift rates of 0.05 ± 0.02 mm/year. This uplift also affected the Pleistocene evolution of the Ognon and Doubs drainage basins and is interpreted as being tectonically controlled. While the Ognon River was deflected from the uplifted region the Doubs deeply incised into it. Focused incision of the Doubs possibly sustained ongoing deformation along anticlines which were initiated during the Neogene evolution of the thin-skinned Jura fold-and-thrust belt. At present, this erosion-related active deformation is taking place synchronously with thick-skinned tectonics, controlling the inversion of the Rhine-Bresse Transfer Zone. Terra Nova, 22, 155–165, 2010 Sequence stratigraphy in the hinterland, kinematic analysis of thin-skinned thrusting in the foreland and thermochronological tracking of exhumation in the orogenic core are combined to quantify the mechanics of an orogen with low topographic build-up. The Carpathian system demonstrates that collisional deformation can couple and thicken the lower orogenic plate along reverse faults that dip more steeply than the subduction zone, defining a ‘foreland-coupling’ type of collision. Near the surface, this is expressed by wide antiforms in the upper plate and the thin-skinned orogenic wedge. A sequence stratigraphic analysis of the back-arc Transylvanian Basin demonstrates that the sedimentary architecture records orogenic uplift pulses with both short and long wavelengths. Terra Nova, 00, 1–8, 2010 The present tectonic activity of the Jura fold-and-thrust belt is a matter of scientific debate. At its north-western front, differentially uplifted palaeo-meanders of the Doubs River record Late Quaternary growth of the Citadelle Anticline, associated with a minimum rock uplift of 13 m. Local rock uplift rates estimated from optical stimulated luminescence dating of oxbow lake deposits measure 0.17 ± 0.05 mm per annum. Reconstruction of the deformation history reveals that buckling took place simultaneously with focused Pleistocene river incision. The structural, geomorphic and temporal setting suggests that active deformation could possibly be sustained by incision-related erosion. These observations shed new light on the modes of recent deformation in the Jura Mountains and on the present-day dynamics of the Alpine orogenic front. [ABSTRACT FROM AUTHOR]

Purpose: To compare the effects of two arginine vasopressin (AVP) dose regimens on the hemodynamic response, catecholamine requirements, AVP plasma concentrations, organ function and adverse events in advanced vasodilatory shock. Methods: In this prospective, controlled, open-label trial, patients with vasodilatory shock due to sepsis, systemic inflammatory response syndrome or after cardiac surgery requiring norepinephrine >0.6 μg/kg/min were randomized to receive a supplementary AVP infusion either at 0.033 IU/min ( n = 25) or 0.067 IU/min ( n = 25). The hemodynamic response, catecholamine doses, laboratory and organ function variables as well as adverse events (decrease in cardiac index or platelet count, increase in liver enzymes or bilirubin) were recorded before, 1, 12, 24 and 48 h after randomization. A linear mixed effects model was used for statistical analysis in order to account for drop-outs during the observation period. Results: Heart rate and norepinephrine requirements decreased while MAP increased in both groups. Patients receiving AVP at 0.067 IU/min required less norepinephrine ( P = 0.006) than those infused with AVP at 0.033 IU/min. Arterial lactate and base deficit decreased while arterial pH increased in both groups. During the observation period, AVP plasma levels increased in both groups (both P < 0.001), but were higher in the 0.067 IU/min group ( P < 0.001) and in patients on concomitant hydrocortisone. The rate of adverse events and intensive care unit mortality was comparable between groups (0.033 IU/min, 52%; 0.067 IU/min, 52%; P = 1). Conclusions: A supplementary AVP infusion of 0.067 IU/min restores cardiovascular function in patients with advanced vasodilatory shock more effectively than AVP at 0.033 IU/min. Abstract: This paper presents a methodology developed for a study to evaluate the state of the art of automated map generalization in commercial software without applying any customization. The objectives of this study are to learn more about generic and specific requirements for automated map generalization, to show possibilities and limitations of commercial generalization software, and to identify areas for further research. The methodology had to consider all types of heterogeneity to guarantee independent testing and evaluation of available generalization solutions. The paper presents the two main steps of the methodology. The first step is the analysis of map requirements for automated generalization, which consisted of sourcing representative test cases, defining map specifications in generalization constraints, harmonizing constraints across the test cases, and analyzing the types of constraints that were defined. The second step of the methodology is the evaluation of generalized outputs. In this step, three evaluation methods were integrated to balance between human and machine evaluation and to expose possible inconsistencies. A key aspect of the 3GPP system architecture evolution is the specification of an evolved packet core that supports multiple access networks. The EPC enables operators to deploy and operate one common packet core network for 3GPP radio accesses (E-UTRAN, UTRAN, and GERAN), as well as other wireless and wireline access networks (e.g., eHRPD, WLAN, WIMAX, and DSL/Cable), providing the operator with a common set of services and capabilities across the networks. A key requirement of the EPC is to provide seamless mobility at the IP layer as the user moves within and between accesses. This article provides an overview of the EPC specifications that use a network-based mobility mechanism based on Proxy Mobile IPv6 to enable mobility between access networks. An important facet of providing seamless mobility for a user's sessions across technologies is to ensure that quality of service is maintained as the user moves between accesses. Background: Stress hyperglycemia is common in trauma patients. Increasing injury severity and hemorrhage trigger hepatic gluconeogenesis, glycogenolysis, peripheral and hepatic insulin resistance. Consequently, we expect glucose levels to rise with injury severity in liver, kidney and spleen injuries. In contrast, we hypothesized that in the most severe form of blunt liver injury, stress hyperglycemia may be absent despite critical injury and hemorrhage. Methods: All patients with documented liver, kidney or spleen injuries, treated at a university hospital between 2000 and 2020 were charted. Demographic, laboratory, radiological, surgical and other data were analyzed. Results: A total of 772 patients were included. In liver (n = 456), spleen (n = 375) and kidney (n = 152) trauma, an increase in injury severity past moderate to severe (according to the American Association for the Surgery of Trauma, AAST III-IV) was associated with a concomitant rise in blood glucose levels independent of the affected organ. While stress-induced hyperglycemia was even more pronounced in the most severe forms (AAST V) of spleen (median 10.7 mmol/L, p < 0.0001) and kidney injuries (median 10.6 mmol/L, p = 0.004), it was absent in AAST V liver injuries, where median blood glucose level even fell (5.6 mmol/L, p < 0.0001). Conclusions: Absence of stress hyperglycemia on hospital admission could be a sign of most severe liver injury (AAST V). Hyperphosphataemia can originate from tissue ischaemia and damage and may be associated with injury severity in polytrauma patients. In this retrospective, single-centre study, 166 polytrauma patients (injury severity score (ISS) ≥ 16) primarily requiring intensive care unit (ICU) treatment were analysed within a five-year timeframe. ICU-admission phosphate levels defined a hyperphosphataemic (>1.45 mmol/L; n = 56) opposed to a non-hyperphosphataemic group (n = 110). In the hyperphosphataemic group, injury severity was increased (ISS median and IQR: 38 (30–44) vs. 26 (22–34); p < 0.001), as were signs of shock (lactate, resuscitation requirements), tissue damage (ASAT, ALAT, creatinine) and lastly in-hospital mortality (35.7% vs. 5.5%; p < 0.001). Hyperphosphataemia at ICU admission was shown to be a risk factor for mortality (1.46–2.10 mmol/L: odds ratio (OR) 3.96 (95% confidence interval (CI) 1.03–15.16); p = 0.045; >2.10 mmol/L: OR 12.81 (CI 3.45–47.48); p < 0.001) and admission phosphate levels alone performed as good as injury severity score (ISS) in predicting in-hospital mortality (area under the ROC curve: 0.811 vs. 0.770; p = 0.389). Hyperphosphataemia at ICU admission is related to tissue damage and shock and indicates injury severity and subsequent mortality in polytrauma patients.
