152. Achieving durable compliance with venous thromboembolism prophylaxis in bariatric surgery: 3-year data from a major academic medical center.

Abstract

Metabolic and bariatric surgery (MBS) venous thromboembolism (VTE) prescribing practices vary widely. Our institutional VTE prophylaxis protocol has historically been unstandardized. To create a standardized MBS VTE prophylaxis protocol, track protocol compliance, and identify barriers to protocol compliance and address them with Plan-Do-Study-Act (PDSA) cycles. Single Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program-accredited academic hospital. We conducted a retrospective study for all patients undergoing MBS (January 2019 to September 2022). A multidisciplinary group of bariatric clinicians reviewed literature and developed the following standardized VTE prophylaxis protocol: 5000 units preoperative subcutaneous (SC) heparin within 60 minutes of anesthesia induction and postoperative 40 mg SC low molecular weight heparin (LMWH) within 24 hours of surgery. This protocol was distributed to relevant clinical stakeholders. We assessed monthly compliance rates through chart review. Goal compliance was ≥90%. We identified sources of noncompliance and addressed them with PDSA methodology. A total of 796 patients were included. Preoperative heparin administration increased from a mean of 47% (107/228) preintervention to 96% (545/568) postintervention (P <.0001), and postoperative LMWH administration increased from 71% (47/66) to 96% (573/597, P =.0002). These compliance rates were sustained for 3 years. Barriers to protocol noncompliance included order set timing errors (n = 45), surgeon error (n = 44), surgeon discretion (n = 40), and nursing error (n = 20). No change in bleeding or VTE rates was observed. Developing a standardized VTE prophylaxis protocol, monitoring process measures, and engaging relevant stakeholders in PDSA cycles resulted in drastic and durable improvement in VTE prophylaxis compliance rates. • A VTE prophylaxis protocol was developed by a bariatric surgery division. • The protocol was distributed to relevant clinicians, and compliance was tracked. • Compliance with VTE prophylaxis protocol increased from 47% to 96%. • Applying best practice QI principles, compliance was sustained for 3 years. Abstract

Twelve hands-on workshops were assigned to one fifty-person lecture class of heat transfer, while a second class of the same size was given corresponding homework problems. Although everything else between the classes was maintained the same, the overall performance of the workshop class was previously shown to be significantly better than the nonworkshop class. This was true including both concept questions and quantitative problem solutions. The current paper looks in detail at specific workshops that were helpful on the topics of transient conduction, energy balance with multiple conduction pathways and relative resistances, external convection, and gray body radiation. Student feedback on the workshops is used to help elucidate what worked for each workshop and what needs improvement. This includes suggestions for better workshops on fins and lumped capacitance based on what concept is most difficult for the students to grasp. 162. An Exploration of Distributed Parallel Sorting in GSS

Abstract

When the members of a group work collaboratively using a group support system (GSS), they often "brainstorm" a list of ideas in response to a question or challenge that faces the group. The satisfaction levels of group members are usually high following this activity. However, satisfaction levels with the process almost always drop dramatically when the group is forced to sort, distill, or classify all of the brainstorming feedback in a synchronous, serially-conducted activity, held immediately after the brainstorming activity. Past explanations for the drop in satisfaction often point to the increased time required to complete a sort and to the mental difficulty in sorting large lists (i.e., increased "cognitive load"). The experiment conducted in this study was designed to expose the participants to conditions featuring different levels of cognitive demand, achieved by varying the number of items to be sorted. This design simulates an asynchronous method of sorting group feedback--a process that can be viewed as a "distributed parallel sort." This dissertation explores methods for measuring the cognitive load experienced by a participant during a sorting activity (using the NASA Task Load Index), evaluating the effectiveness of having group members sort partial lists of items instead of working synchronously on the same full list (objectively measured using normalized clustering error against a "gold standard" result), and proposes new methods for mitigating the drop in satisfaction levels that regularly occur in these collaborative settings without compromising the effectiveness of their sorting results. The experimental results imply that an individual's perceived difficulty of the task may rely on other factors, rather than just the length of a list. The results also imply that the NASA-TLX framework to measure cognitive load may need to be refined further (or implemented differently), if it is to be used in GSS research contexts. Finally, two methods are proposed (a facilitation-based recommendation and another technology-enabled option) that may help to mitigate the drop in satisfaction levels, improve a group's effectiveness, and reduce the time required for that group to effectively sort their feedback in collaborative GSS sessions. The increasing requirements for high-performance lithium-ion batteries from applications such as electric aviation and electric mobility require further improvement of this energy storage technology. Prelithiation is a method to improve the energy density and lifespan by integrating additional lithium during battery cell production to compensate for occurring irreversible losses of active lithium, which lead to a reduced energy density and capacity fade. Direct contact prelithation incorporates lithium metal in contact to the anode active material to generate a lithium reservoir, which can be consumed by parasitic reactions inside the cell.

165. Visual Cue Based Corrective Feedback for Motor Skill Training in Mixed Reality: A Survey

Abstract

When learning a motor skill it is helpful to get corrective feedback from an instructor. This will support the learner to execute the movement correctly. With modern technology, it is possible to provide this feedback via mixed reality. In most cases, this involves visual cues to help the user understand the corrective feedback. We analyzed recent research approaches utilizing visual cues for feedback in mixed reality. The scope of this article is visual feedback for motor skill learning, which involves physical therapy, exercise, rehabilitation etc. While some of the surveyed literature discusses therapeutic effects of the training, this article focuses on visualization techniques. We categorized the literature from a visualization standpoint, including visual cues, technology and characteristics of the feedback. This provided insights into how visual feedback in mixed reality is applied in the literature and how different aspects of the feedback are related. The insights obtained can help to better adjust future feedback systems to the target group and their needs. This article also provides a deeper understanding of the characteristics of the visual cues in general and promotes future, more detailed research on this topic.

173. ADHD in the College Student: Is Anyone Else Worried?

Abstract

The illegal, non-prescriptive use of prescription stimulants appears to be growing among college students. Recent analyses using "DSM-IV" criteria suggest that this group of misusers may actually represent cases of undiagnosed ADHD. Such analyses, however, are limited by a diagnostic system that is neither contextural nor dimensional. The ADHD symptoms of the newly diagnosed college student may be highly context and time specific and represent a normal response to temporarily increased demands on intellect and motivation. Diagnosing college students who are misusing stimulants with ADHD runs the risk of further trivializing the ADHD diagnosis. Also from a historical perspective, legitimizing the use of prescription stimulants in this age group may unintentionally only further increase the likelihood of a greater prescription stimulant abuse epidemic.

174. Commentary on Forest Grove School District v. T. A.

Abstract

Forest Grove School District v. T.A. involved a student with learning disabilities, Attention Deficit/Hyperactivity Disorder (ADHD), and depression (T.A.) whose parents had moved him to a private academy for students with special needs without first consulting with his public school. In the past, the school district had found T.A. to be ineligible for special education services and not in need of an Individualized Education Program (IEP). At issue in the case was whether parents who unilaterally enroll their child with a disability in a private school are entitled to tuition reimbursement "if the child never received special education services from the district." According to the School District, T.A.'s parents unilaterally withdrew him from the public school without prior notice and he never received special education, or related services from a public agency. The District contended that since IDEA only discusses reimbursement for children who had previously received special-education services from the public school that T.A.'s parents should not be entitled to private school tuition reimbursement. According to the parents, the District was well aware of T.A.'s learning struggles throughout his school career and made few attempts to provide assistance under the IDEA, the Rehabilitation Act, or by informal means. The parents contended that the District's repeated failure to properly evaluate T.A. and provide a free, appropriate public education (FAPE) entitled them to reimbursement of the private school tuition they paid in order to obtain an appropriate education for their child. The parent's position was supported by a "friend of the court" brief filed by the U.S. Department of Justice. The Supreme Court found that IDEA authorizes reimbursement for private special education services when a public school fails to provide a FAPE and the private school placement is appropriate, regardless of whether the child previously received special-education services through the public school. The opinion reached in Forest Grove serves to affirm the IDEA's strong commitment to ensuring that students with disabilities are identified and served by the nation's public schools. That commitment does not, however, negate the need for mechanisms to provide for speedy and effective resolution of disputes between parents and schools when disagreements arise. The latest renewal of IDEA, which occurred after the events in Forest Grove, seeks to provide expanded options for dispute resolution. Without doubt, parents and schools must strive to work together to assist struggling learners. The Forest Grove decision will only serve to advance the needs of all students, including those with disabilities.

Abstract

Librarians at Washington State University Vancouver helped the campus develop a method of assessing its General Education Program, a program based on university learning goals, one of which is information literacy. The assessment method, which relies on an electronic portfolio (ePortfolio) along with rubrics to evaluate work in the ePortfolio, enables the librarians to evaluate their information literacy program based on ACRL best practices guidelines, authentic assessment techniques, and the tenets of phenomenography. This paper will describe the library's use of this assessment method, while looking at the advantages and disadvantages of this process for assessment. (Contains 1 figure, 1 chart, and 28 notes.)

Comparisons have been made between Buddhism and the philosophy of science in general, but there have been only a few attempts to draw comparisons directly with the philosophy of radical behaviorism. The present review therefore considers heretofore unconsidered points of comparison between Buddhism and radical behaviorism in terms of their respective goals, conceptualization of human beings, and the outcomes of following either philosophy. From these comparisons it is concluded that the commonalities discerned between these two philosophies may enhance both philosophical systems.

Abstract

Venous thromboembolism (VTE) is a leading cause of 30-day mortality after metabolic and bariatric surgery (MBS). Multiple predictive tools exist for VTE risk assessment and extended VTE chemoprophylaxis determination. To review existing risk-stratification tools and compare their predictive abilities. MBSAQIP database. Retrospective analysis of the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program (MBSAQIP) database was performed (2015–2019) for primary minimally invasive MBS cases. VTE clinical factors and risk-assessment tools were evaluated: body mass index threshold of 50 kg/m2, Caprini risk-assessment model, and 3 bariatric-specific tools: the Cleveland Clinic VTE risk tool, the Michigan Bariatric Surgery Collaborative tool, and BariClot. MBS patients were deemed high risk based on criteria from each tool and further assessed for sensitivity, specificity, and positive predictive value. Overall, 709,304 patients were identified with a.37% VTE rate. Bariatric-specific tools included multiple predictors: procedure, age, race, gender, operative time, length of stay, heart failure, and dyspnea at rest; operative time was the only variable common to all. The body mass index cutoff and Caprini risk-assessment model had higher sensitivity but lower specificity when compared with the Michigan Bariatric Surgery Collaborative and BariClot tools. While the sensitivity of the tools varied widely and was overall low, the Cleveland Clinic tool had the highest sensitivity. The bariatric-specific tools would have recommended extended prophylaxis for 1.1%–15.6% of patients. Existing MBS VTE risk-assessment tools differ widely for inclusion variables, high-risk definition, and predictive performance. Further research and registry inclusion of all significant risk factors are needed to determine the optimal risk-stratified approach for predicting VTE events and determining the need for extended prophylaxis. 189. Adherence Patterns and Acceptability of a Perioperative, App-Based Mindfulness Meditation Among Surgical Patients With Chronic Pain

Abstract

Identifying barriers to compliance with mindfulness-based interventions (MBIs) during the perioperative period is paramount to the design and implementation of non-pharmacologic, opiate-sparing pain regimens. We prospectively investigated the acceptability and adherence patterns of an app-based MBI amongst surgical patients with chronic pain. Patients were provided a subscription code to a meditation app and encouraged to use the app an average of 10 minutes/day over a 6-week study period. Patients reported a high level of interest in using the app prior to enrollment. 71% used the app at least once, 30% for half the recommended time, and 8% for the recommended amount of time. 88% of respondents enjoyed using the app and agreed it improved mental health and stress levels. Surgical patients with pre-existing pain report a high level of interest in utilizing MBIs perioperatively. Identifying barriers to compliance represents an important area of future investigation.

Abstract

Background: Resistant hypertension is associated with increased cardiovascular risk. The endothelin pathway has been implicated in the pathogenesis of hypertension, but it is currently not targeted therapeutically, thereby leaving this relevant pathophysiological pathway unopposed with currently available drugs. The aim of the study was to assess the blood pressure lowering efficacy of the dual endothelin antagonist aprocitentan in patients with resistant hypertension. Methods: PRECISION was a multicentre, blinded, randomised, parallel-group, phase 3 study, which was done in hospitals or research centres in Europe, North America, Asia, and Australia. Patients were eligible for randomisation if their sitting systolic blood pressure was 140 mm Hg or higher despite taking standardised background therapy consisting of three antihypertensive drugs, including a diuretic. The study consisted of three sequential parts: part 1 was the 4-week double-blind, randomised, and placebo-controlled part, in which patients received aprocitentan 12·5 mg, aprocitentan 25 mg, or placebo in a 1:1:1 ratio; part 2 was a 32-week single (patient)-blind part, in which all patients received aprocitentan 25 mg; and part 3 was a 12-week double-blind, randomised, and placebo-controlled withdrawal part, in which patients were re-randomised to aprocitentan 25 mg or placebo in a 1:1 ratio. The primary and key secondary endpoints were changes in unattended office systolic blood pressure from baseline to week 4 and from withdrawal baseline to week 40, respectively. Secondary endpoints included 24-h ambulatory blood pressure changes. The study is registered on ClinicalTrials.gov, NCT03541174. Findings: The PRECISION study was done from June 18, 2018, to April 25, 2022. 1965 individuals were screened and 730 were randomly assigned. 194. Process and Pedagogy in Writing: Neurolinguistic Considerations. Working Draft.

Abstract

The relation between reading and writing is important because tacit and possibly unwarranted assumptions underlie the theories and pedagogies which govern these processes. These assumptions are challenged by the claims that: (1) reading and writing are related in neurolinguistically specifiable ways; (2) they do not seem to be simple inverses of each other; and (3) they do not seem clearly hierarchically dependent on one another. Some evidence for the claims is seen in the fact that reading and writing are not always equally impaired after damage to the left or dominant hemisphere. Also, people are considered good readers if they can read and understand sentences they have not seen before, even if it is impossible for them to write such sentences. The syndrome called alexia without agraphia is taken to indicate that reading and writing as neuropsychological processes are partially separated. Pedagogy, while admittedly different from pathology, can learn from the neurolinguistic evidence. This evidence seems to challenge the assumption that reading instruction must precede writing. Approaches in first and foreign language instruction are cited as evidence that writing can be taught first as a road to reading. Although closely related processes, the decoding and encoding involved are not symmetrical. Neurolinguistic evidence does not support the assumption that reading depends on the prior acquisition of writing. (AMH)
