Abstract

Focal acute bacterial nephritis is a localized bacterial infection of the kidney presenting as an inflammatory mass not containing drainable pus. The further distinction between acute focal bacterial nephritis and other renal masses is aided by the appropriate use of renal sonography and computed tomography. We report 4 cases with this entity.

Abstract

Actinomycosis is a suppurative and granulomatous chronic infectious disease caused by Actinomyces sp. and most commonly affecting the cervicofacial area.To study the clinical characteristics of patients with actinomycosis, with regard to clinical history, presentation, method of diagnosis, treatment and follow up.A retrospective review was performed on all cases of microbiologically or histologically proven oral or cervicofacial actinomycosis.Five patients were studied, 2 men and 3 women, 25-69 years old. Four patients had a history of surgical procedures and/or dental manipulations. Three patients showed the classic presentation of a lump and fistulization, and two patients presented intra-oral lesions. Four patients were diagnosed by cultures positive to A. israelii on microbiologic study and the remaining patient by cytologic detection of a sulfur granule. The first patient received the classic initial regimen of iv penicillin and 3 were treated with third-generation cephalosporins, continuing with oral amoxicillin during 12 months. Patient no.2 required a second surgical procedure. Patient no. 5, who had an exclusively oral process, received a short course of amoxicillin. There were no relapses during follow-up.Actinomycosis is an uncommon disease. Establishment of the definite diagnosis requires a high index of suspicion and good clinical-microbiological collaboration. The classic course of iv penicillin and oral amoxicillin during 6-12 months is effective. For the acute phase treatment, iv penicillin can be replaced by third-generation cephalosporins.

Abstract

Although advances have been made in the prevention, diagnosis and treatment of cytomegalovirus (CMV), this infection continues to be a major complication in transplant recipients, causing considerable morbidity and occasionally mortality. There is wide variability in definitions of CMV infection, which can lead to conceptual errors. The present article attempts to standardize the definitions used, which have recently been revised and recommended by the American Society of Transplantation for the purpose of developing consistent reporting of CMV in clinical trials. The concepts of infection and viral replication, persistence and recurrence of infection, viremia and disease are discussed, together with the diagnostic criteria for febrile syndrome and visceral involvement. Methods for preventing CMV infection, universal prophylaxis and preemptive therapy are also mentioned.

Actinomycosis is caused by bacilli of the Actinomyces sp. They are Gram-positive, anaerobic or microaerophilic non-spore-forming bacilli. Actinomyces israelii is the most common aetiological agent. A case is reported of primary actinomycosis in the urinary bladder successfully treated with ceftriaxone.

La biopsia del condilo femoral evidencio una inflamacion granulomatosa cronica no caseificante. La tincion de Ziehl-Neelsen y el cultivo en medio de Lowestein eran positivos para M. tuberculosis. A pesar de la mejoria progresiva del estado general, de la ganancia ponderal y de las alteraciones hematologicas, persistia una fiebre elevada, bien tolerada por la paciente. Se asocio prednisona (30-40 mg/dia) al tratamiento anteriormente descrito, y se obtuvo la remision de la fiebre. 168. Enfermedades fúngicas invasoras en receptores de trasplante de órgano sólido

Abstract

La candidiasis sigue siendo una enfermedad fungica invasora (EFI) frecuente en pacientes que reciben el trasplante de un organo solido (RTOS). Ademas su diagnostico sigue siendo problematico en estos pacientes, a pesar de los avances conseguidos, lo que complica el tratamiento y conlleva una morbimortalidad alta y costes humanos elevados. Sin embargo, otras EFI estan cobrando especial importancia. Asi, las aspergilosis invasoras han desplazado a las candidiasis como micosis mas frecuentes en determinados grupos de pacientes de riesgo alto, como los receptores de trasplante pulmonar. Las infecciones por otros hongos, como Cryptococcus, Fusarium, Scedosporium y los mucorales continuan siendo poco frecuentes, pero con una evolucion de la enfermedad mas torpida y con un peor pronostico. A pesar del aumento de las EFI, la disposicion de farmacos antifungicos continua siendo limitada, aunque las nuevas equinocandinas y los azoles de espectro extendido han ampliado los recursos terapeuticos. Sin embargo, la lucha contra la infeccion fungica necesitara nuevas moleculas activas y seguras contra los hongos emergentes5. El objetivo de este monografico es revisar algunos de los retos clinicos mas importantes que plantean las EFI en RTOS mediante la presentacion de cuatro casos perfectamente documentados y revisados por autores espanoles con amplia experiencia en este campo de las enfermedades infecciosas. Pozo-Laderas y Pontes-Moreno4 nos presentan el caso de un paciente con trasplante hepatico en una situacion de riesgo alto para desarrollar una EFI. Lopez GarciaGallo et al2 exponen el caso de una traqueobronquitis por Aspergillus fumigatus en un paciente con trasplante bilateral de pulmon. Eworo et al1 plantean el caso clinico de una receptora de trasplante cardiaco que desarrollo una endocarditis por Aspergillus tratada con voriconazol. Sole6 recoge el caso de un receptor de trasplante pulmonar con una tuberculosis pulmonar e infeccion asintomatica por citomegalovirus que desarrolla una infeccion localizada al inicio por Scedosporium apiospermum. Montejo3 realiza una revision de la epidemiologia de las EFI en RTOS y hace un comentario critico desde la perspectiva de su dilatada experiencia personal de cada uno de estos casos. Abstract

The aim of this study was to describe the characteristics of HAIE and to establish a comparison between health care and community-acquired episodes. HAIE was defined as either IE manifesting >48 hours after admission to the hospital or IE acquired in association with a significant invasive procedure performed during a stay and/or manipulation in a hospital setting within 6 months before diagnosis. Results: HAIE accounted for 16% of 793 cases. Compared with community-acquired infection, patients with HAIE tended to be older (60.1 vs. 53.6; P= 0.0001) and had more co-morbidities (Charlson index, 3.3 vs. 1.8; P= 0.0001) and staphylococcal infections (58.3% vs. 24.8%). Intra-hospital mortality (44.9 vs. 24.2%) was higher in the HAIE group. Vascular manipulation constituted the main cause of bacteremia responsible for HAIE (63%). Septic shock (OR, 10.13; 95% CI, 3.18 32.12; P= 0.0001) and severe heart failure (OR, 2.79; 95% CI, 1.09 7.13; P= 0.03) were independent predictors of intrahospital mortality in the HAIE cohort. Conclusions: The present study demonstrates that HAIE attacks a fragile population and it is principally caused by microorganisms strongly related to vascular manipulations. Extremely careful management of vascular accesses is needed in order to minimize the risk of secondary bacteraemias.

Abstract

High-level aminoglycoside resistance (HLAR) that precludes bactericidal synergism with penicillins or glycopeptides and nephrotoxicity related to aminoglycoside treatment are major problems in treating Enterococcus faecalis endocarditis.To evaluate the efficacy and safety of ampicillin plus ceftriaxone for treating endocarditis due to E. faecalis with and without HLAR.Observational, open-label, nonrandomized, multicenter clinical trial.13 centers in Spain.21 patients with HLAR E. faecalis endocarditis and 22 patients with non-HLAR E. faecalis endocarditis. All were at risk for nephrotoxicity related to aminoglycoside use.6-week course of intravenous ampicillin, 2 g every 4 hours, plus intravenous ceftriaxone, 2 g every 12 hours.Clinical and microbiological outcomes.The clinical cure rate at 3 months was 67.4% (29 of 43 patients) among all episodes. During treatment, 28.6% of patients with HLAR E. faecalis endocarditis and 18.2% of patients with non-HLAR E. faecalis endocarditis died of infection-related causes. The rate of clinical and microbiological cure in patients who completed the protocol was 100% in the HLAR E. faecalis endocarditis group. No episodes of breakthrough bacteremia occurred, although there were 2 relapses in the non-HLAR E. faecalis endocarditis group. Treatment was withdrawn in 1 case because of fever and skin rash.The study had a small sample and was observational.The combination of ampicillin and ceftriaxone is effective and safe for treating HLAR E. faecalis endocarditis and could be a reasonable alternative for patients with non-HLAR E. faecalis endocarditis who are at increased risk for nephrotoxicity.

We think it important to make the following observations.Disseminated gonococcal infection (DGI), although a more frequent disease among people less than 40 years of age, can appear at any age, including in elderly patients. This is not a new fact, as assured by the authors, since Jacobs2 published an article in 1962 describing a 62-year-old widow with arthritis in her left knee, in whom Neisseriagonorrhoeae was cultivated. In this article a reference to the literature is made where the relationship between DGI and age is discussed. Jacobs concludes that the physician needs to suspect DGI in patients of any age. Abstract

Antifungal treatment in the hematological patient has reached a high complexity with the advent of new antifungals and diagnostic tests, which have resulted in different therapeutic strategies. The use of the most appropriate treatment in each case is essential in infections with such a high mortality. The availability of recommendations as those here reported based on the best evidence and developed by a large panel of 48 specialists aimed to answer when is indicated to treat and which agents should be used, considering different aspects of the patient (risk of fungal infection, clinical manifestations, galactomanann test, chest CT scan and previous prophylaxis) may help clinicians to improve the results.
