Abstract

International audience; Colistin has become a last-resort antibiotic for the treatment of infections caused by highly drug-resistant Gram-negative bacteria. Moreover, it has been widely used in the livestock sector. As a consequence, colistin resistance is emerging worldwide. Among the colistin resistance mechanisms, the spread of the plasmid-encoded colistin resistance gene mcr-1 (mostly in Escherichia coli) is of particular concern due to its increased transferability compared to that of chromosome-encoded resistance. The early detection of MCR-1-producing bacteria is essential to prevent further spread and provide appropriate antimicrobial therapy. Lateral flow immunoassays (LFIAs) were manufactured with selected monoclonal antibodies. A collection of 177 human and 121 animal enterobacterial isolates was tested in a multicentric study. One bacterial colony grown on agar plates was suspended in extraction buffer and dispensed on the cassette. Migration was allowed for 15 min, and the results were monitored by the appearance of a specific band. The positive results showed a pink line resulting in an unambiguous interpretation. All MCR-1-producing isolates were found to be positive by the LFIA, and no false-negative results were observed. Three out of four MCR-2-producing isolates were also found to be positive. Our test does not detect MCR-3-, MCR-4-, or MCR-5-producing isolates. LFIA allows the detection of MCR-1 with 100% sensitivity and 98% specificity. This test is fast, sensitive, specific, easy to use, and cost-effective and can therefore be implemented in any microbiology laboratory worldwide. LFIA is a major tool for the rapid detection and monitoring of MCR-1 producers in humans and animals.

Abstract

Ocular herpes simplex keratitis (HSK) is a consequence of viral reactivations from trigeminal ganglia (TG) and occurs almost exclusively in the same eye in humans. In our murine oro-ocular (OO) model, HSV-1 inoculation in one side of the lip propagates virus to infect the ipsilateral TG. Replication here allows infection of the brainstem, and infection of the contralateral TG. Interestingly, HSK is observed in our OO model only from the eye ipsilateral to site of lip infection. Thus, unilateral restriction of HSV-1 may be due to differential kinetics of virus arrival in the ipsilateral versus contralateral TG. We inoculated mice with HSV-1 reporter viruses, and then superinfected them to monitor changes in acute and latent phase gene expression in TG after superinfection, compared to control (single inoculation). Delaying superinfection by 4 days after initial right lip inoculation elicited failed superinfecting-virus gene expression, and eliminated clinical signs of disease. Initial inoculation with a thymidine kinase-deficient HSV-1 (TKdel) completely abolished reactivation of WT superinfecting virus from TG during latent stage. In light of these seemingly failed infections, viral genome was detected in both TGs. Our data demonstrate that inoculation of HSV-1 in the lip propagates virus to both TG, but with delay in reaching the TG contralateral to the side of lip infection. This delay is responsible for restricting viral replication to the ipsilateral TG, which abrogates ocular disease and viral reactivations from the contralateral side. These observations may help to understand why HSK is observed unilaterally in humans, and provides insight into vaccine strategies to protect against HSK.IMPORTANCEHerpetic keratitis (HK) is the leading cause of blindness by an infectious agent in the developed world. This disease can occur after reactivation of Herpes simplex virus 1 in the trigeminal ganglia leading to dissemination of virus to, and infection of, the cornea. A clinical paradox is evidenced by the bilateral presence of latent viral genomes in both trigeminal ganglia, while for any given patient the disease is unilateral with recurrences in a single eye. Our study links the kinetics of early infection to unilateral disease phenomenon, and demonstrates protection against viral reactivation when kinetics are exploited. Our results have direct implications in the understanding of human disease pathogenesis, and immunotherapeutic strategies for the treatment of HK and viral reactivations.

Abstract

The high frequency autonomous retrotransposition of the human long interspersed nuclear elements (L1) L1.2 and LRE2in a cultured human cell line is demonstrated. The retrotransposed products resemble endogenous L1 insertions. Point mutations in conserved domains of L1.2-encoded proteins decreased retrotransposition by 100 to 1,000 times. L1.2 also retrotransposed in a mouse cell line, suggesting that L1-based vectors play a significant part in random insertional mutagenesis.

Abstract

Hybridization studies of carbapenemase separated from the chromosome of a clinical isolate of Enterobacter cloacae NOR-1 reveal that the mature protein (NmcA) belongs to serine beta-lactamase class A. Amino acid sequence of NmcA exhibits similarity with the extended-spectrum class A beta-lactamase MEN-1 from Escherichia coli. Amino acid sequence of NmcR exhibits homology with the LysR-type transcriptional regulatory protein. Removal of NmcR reduces carbapenem resistance and loss of beta-lactamase inducibility, proving the vital function of NmcR in NmcA expression.

Abstract

Ventilator-associated pneumonia (VAP) is associated with increased hospital stay and high morbidity and mortality in critically ill patients. The aims of this study were to (i) determine the incidence of multidrug-resistant (MDR) pathogens in the first episodes of VAP and to assess potential differences in bacterial profiles of subjects with early- versus late-onset VAP. This was a retrospective cohort study over a period of 18 months including all patients who had a first episode of VAP confirmed by positive bacterial culture. Subjects were distributed into two groups according to the number of intubation days: early-onset VAP (<5 days) or late-onset VAP (≥5 days). The primary endpoint was the nature of causative pathogens and their resistance profiles. Sixty patients were included, 29 men and 31 women, with an average age of 38 ± 16 years. The IGS 2 at admission was 40.5 [32–44] and APACHE was 19 [15–22]. Monomicrobial infections were diagnosed in 77% of patients (n = 46). The most frequently isolated bacteria were A. baumannii, 53% (n = 32); P. aeruginosa in 37% (n = 22); Enterobacterales in 28% (n = 17) and S. aureus in 5% (n = 3). Ninety-seven percent of the bacteria were MDR. The VAP group comprised 36 (60%) episodes of early-onset VAP and 24 (40%) episodes of late-onset VAP. There was no significant difference in the distribution of the bacterial isolates, nor in terms of antibacterial resistances between early- and late-onset VAPs. Our data support recent observations that there is no microbiological difference in the prevalence of potential MDR pathogens or in their resistance profiles associated with early- versus late-onset VAPs, especially in countries with high rates of MDR bacteria. Abstract

Background Polymyxins are currently considered a last-resort treatment for infections caused by MDR Gram-negative bacteria. Recently, the emergence of carbapenemase-producing Enterobacteriaceae has accelerated the use of polymyxins in the clinic, resulting in an increase in polymyxin-resistant bacteria. Polymyxin resistance arises through modification of lipid A, such as the addition of phosphoethanolamine (pETN). The underlying mechanisms involve numerous chromosome-encoded genes or, more worryingly, a plasmid-encoded pETN transferase named MCR. Currently, detection of polymyxin resistance is difficult and time consuming. Objectives To develop a rapid diagnostic test that can identify polymyxin resistance and at the same time differentiate between chromosome- and plasmid-encoded resistances. Methods We developed a MALDI-TOF MS-based method, named the MALDIxin test, which allows the detection of polymyxin resistance-related modifications to lipid A (i.e. pETN addition), on intact bacteria, in Results Using a characterized collection of polymyxin-susceptible and -resistant Escherichia coli, we demonstrated that our method is able to identify polymyxin-resistant isolates in 15 min whilst simultaneously discriminating between chromosome- and plasmid-encoded resistance. We validated the MALDIxin test on different media, using fresh and aged colonies and show that it successfully detects all MCR-1 producers in a blindly analysed set of carbapenemase-producing E. coli strains. Conclusions The MALDIxin test is an accurate, rapid, cost-effective and scalable method that represents a major advance in the diagnosis of polymyxin resistance by directly assessing lipid A modifications in intact bacteria.

Abstract

Two AMR isolates were found in the soapy water used for cleaning medical devices: one I Acinetobacter indicus i expressing I bla i SB NDM-1 sb and I bla i SB OXA-235 sb genes and one I Enterobacter chengduensis i expressing a I bla i SB CTX-M-15 sb gene. MDR Gram-negatives constitute a global public health concern, as they may cause severe, difficult-to-treat and often deadly infections, such as bloodstream infections and pneumonia. Carbapenem-resistant (CR) I Acinetobacter i spp. I Pseudomonas i spp. and Enterobacterales are among the most worrisome.[1] Even though these bacteria are increasingly described worldwide, epidemiological studies are very scarce from the African continent, except in Northern countries.
