Introduction: Pancreatic pseudocysts (PPCs) have been a topic of debate among surgeons, gastroenterologists, and radiologists regarding their etiology, evolution, and treatment. Often associated with acute or chronic pancreatitis, these accumulations of fluid located in the tissues surrounding the pancreas are sometimes partially or completely encapsulated within the pancreatic tissue itself, thus presenting challenging management. Case Report: A 55-year-old female patient presented in October 2023 with epigastric pain and nausea. The patient, known to have type 2 diabetes mellitus and class 2 obesity, underwent a cholecystectomy one year prior and was monitored for a PPC, a complication of a severe acute biliary necrotizing pancreatitis episode. On CT examination, the cyst measured 19/13cm and the images showed significant atrophy of the pancreatic parenchyma and splenic vein thrombosis with collateral blood flow. Multiple ultrasound (US) and endoscopic ultrasound (EUS) examinations were conducted, indicating the enlargement of the PPC and raising the suspicion of a Wirsung duct rupture. Magnetic resonance cholangiopancreatography (MRCP) ruled out any communication with the PPC, but the Wirsung duct was still stented during an endoscopic retrograde cholangiopancreatography (ERCP) procedure in December 2023. In January 2024, EUS-guided transmural drainage was performed using two plastic cysto-gastric pigtail stents, improving the patient's condition. In March 2024, the patient returned for clinical assessment, reporting a weight loss of 10kg without any accompanying symptoms. The pancreatic duct plastic stent was removed. A follow-up CT revealed a significant decrease in the PPC's dimensions (4 cm). The patient was discharged with pigtail stents still in place. Discussions : This complex case exemplifies the challenges of treating PPCs due to their volatile and sometimes unpredictable nature. Therefore, many questions arise: When is the optimal time to use EUS and to remove the pigtail stents? Should one opt for a more conventional, surgical approach? Surgical drainage used to be the preferred method for draining PPCs. Thanks to advancements in endoscopic techniques, surgical drainage is now reserved for specific scenarios, such as recurrent PPCs, malignant cyst resection, or symptomatic PPCs that are challenging to access endoscopically. Conclusions: PPCs are intricate and difficult conditions, yet this case exemplifies the importance of thorough surveillance of PPCs due to their unforeseeable implications. A total of six patients with pancreatic ductal disruption (PDD), treated with image-guided endoscopy percutaneous drainage were enrolled. Initially, patients had infected pancreatic necrosis, treated with transgastric percutaneous drainage, and after the infection was controlled, they developed PDD. In the imaging study process, four patients were diagnosed with partial duct disruption and two patients with complete duct disruption. In both cases the placement of a percutaneous transgastric prosthesis to drain the pancreatic fluids to the stomach was indicated. The prosthesis remained 183 days on average and there were no mortality cases. This therapeutic minimally invasive alternative has low rates of morbimortality and thus, the endoscopy percutaneous transgastric approach is considered as a viable treatment in PPD. Background: Endoscopic ultrasound (EUS)-guided cystogastrostomy with a single 7-French (Fr) double-pigtail stent (DPS) is less popular due to the concern of stent patency. We aimed to assess the effectiveness, complications, and long-term outcomes of a single 7-Fr DPS in the endoscopic drainage of uncomplicated pseudocysts, containing no or minimal (<10%) debris. Methods: A retrospective review of patients with pancreatic pseudocysts, who underwent EUS-guided cystogastrostomy during 2010-2018, and a systematic review of the literature were conducted. Results: Of 45 patients, 14 patients underwent endoscopic drainage of uncomplicated pseudocysts using a single 7-Fr × 5 cm DPS. The mean cyst size was 10.2 ± 3.5 cm. Stent placement had a 100% technical and clinical success, defined as complete resolution of symptoms and regression of the cyst size by more than 50% at 8 weeks after drainage. The median follow-up was 42.4 months (range, 10-103). The pseudocysts resolved without recurrence in 92.8%. Spontaneous stent dislodgment was noted in 70% at a mean follow-up of 18 months. Additional interventions were required in 14% of cases due to stent occlusion and migration. A systematic review of literature related to EUS-guided cystogastrostomy using single and multiple plastic stents included 9 of 333 studies (222 patients). The analysis showed the pooled clinical success of 89% (95% confidence interval [CI], 82.0-94.2) and complication rate of 13% (95% CI, 5.7-21.8). Conclusion: Selected uncomplicated pseudocysts can be treated effectively with a single 7-Fr DPS as it provides comparable clinical success and long-term outcomes as using larger or multiple stents. Spontaneous rupture of a hemorrhagic pancreatic pseudocyst may be life-threatening. Endoscopic ultrasound (EUS)-guided drainage has been reported to be a valuable treatment option for pancreatic pseudocysts. However, its usefulness in the management of a ruptured pancreatic pseudocyst is limited. We report a rare case of a spontaneous rupture of a hemorrhagic pancreatic pseudocyst in a patient with chronic pancreatitis, which was successfully treated with EUS-guided gastrocystostomy with a fully covered self-expandable metallic stent. Pseudocysts being the most frequent local complications of acute pancreatitis (AP) have substantial effect on the disease course, hospitalization and quality of life of the patient. Our study aimed to understand the effects of pre-existing (OLD-P) and newly developed (NEW-P) pseudocysts on AP. Data were extracted from the Acute Pancreatitis Registry organized by the Hungarian Pancreatic Study Group (HPSG). 2275 of 2461 patients had uploaded information concerning pancreatic morphology assessed by imaging technique. Patients were divided into "no pseudocyst" (NO-P) group, "old pseudocyst" (OLD-P) group, or "newly developed pseudocyst" (NEW-P) groups. The median time of new pseudocyst development was nine days from hospital admission and eleven days from the beginning of the abdominal pain. More NEW-P cases were severe (15.9% vs 4.7% in the NO-P group p < 0.001), with longer length of hospitalization (LoH) (median: 14 days versus 8 days, p < 0.001), and were associated with several changed laboratory parameters. OLD-P was associated with male gender (72.2% vs. 56.1%, p = 0.0014), alcoholic etiology (35.2% vs. 19.8% in the NO-P group), longer hospitalization (median: 10 days, p < 0.001), a previous episode of AP (p < 0.001), pre-existing diagnosis of chronic pancreatitis (CP) (p < 0.001), current smoking (p < 0.001), and increased alcohol consumption (unit/week) (p = 0.014). Most of the new pseudocysts develop within two weeks. Newly developing pseudocysts are associated with a more severe disease course and increased length of hospitalization. Pre-existing pseudocysts are associated with higher alcohol consumption and smoking. OBJECTIVES: Pancreatic pseudocysts (PPC) are fluid collections with a well-defined wall that persist for more than 4 weeks inside or around the pancreas as a result of pancreatic inflammation and/or a ductal lesion. PPC have been successfully treated with endoscopic ultrasound (EUS)-guided drainage using different stents. This study aimed to evaluate the safety and efficacy of EUS-guided drainage of PPC using double-pigtail plastic stents in a tertiary hospital. METHODS: Patients with PPC referred for EUS-guided drainage between May 2015 and December 2019 were included in this case series. The primary endpoint was to evaluate the efficacy (clinical success) and safety (adverse events and mortality) of EUS-guided drainage of PPC. Secondary endpoints included technical success and pseudocyst recurrence. RESULTS: Eleven patients (mean age, 44.5±18.98 years) were included in this study. The etiologies for PPC were acute biliary pancreatitis, chronic alcoholic pancreatitis, and blunt abdominal trauma. The mean pseudocyst size was 9.4±2.69 cm. The clinical success rate was 91% (10/11). Adverse events occurred in three of 11 patients (27%). There were no cases of mortality. The technical success rate was 100%. Pseudocyst recurrence was identified in one of 11 patients (9%) at 12 weeks after successful clinical drainage and complete pseudocyst resolution. CONCLUSION: EUS-guided transmural drainage of PPC using double-pigtail plastic stents is safe and effective with high technical and clinical success rates.

Background: Many pancreatic pseudocysts spontaneously resolve, but larger or symptomatic pseudocysts may require procedural management. Though endoscopic ultrasound guided approaches are standard of care and have high success rates, complications can include bleeding, infection, and splenic perforation. This patient case report details an unusual series of complications of endoscopic cystogastrostomy that should encourage clinicians to evaluate for anatomic disruptions caused by mass effects of pancreatic pseudocysts prior to endoscopic pseudocyst drainage.Case Presentation: A 53-year-old African American male with a past medical history notable for alcohol use disorder, chronic pancreatitis, and insulin dependent diabetes presented with a 4-day history of left upper quadrant abdominal pain. Computed tomography imaging with contrast revealed enlargement of a known pancreatic pseudocyst to 15.9 × 10.4 cm. Due to pseudocyst size and the patient's symptoms, endoscopic cystogastrostomy stent placement was performed. However, postprocedurally, he developed leukocytosis to 19,800 cells/m3 (from 14,100 cells/m3 preoperatively) as well as acute hypoxemic respiratory failure with a large left pleural effusion. Postprocedural computed tomography with contrast demonstrated a new large subcapsular splenic hematoma in communication with a new subdiaphragmatic fluid collection. Due to suspicion of endoscopic procedural complication, he underwent open laparotomy which revealed grade 4 splenic laceration, septic splenic hematoma, and a subdiaphragmatic abscess.Conclusions: While endoscopic drainage of pancreatic pseudocyst was technically successful, this case demonstrates complications from mass effect of a large pancreatic pseudocyst which putatively tore the splenorenal ligament, leading to excessive separation of the left kidney and spleen. If anatomic disruptions caused by mass effect from a pancreatic pseudocyst are recognized through preprocedural abdominal imaging, such cases may be considered for early open repair versus cystogastrostomy. Introduction. The pancreas is the fourth most frequently involved solid organ in pediatric abdominal trauma. We present the case of a giant pancreatic pseudocyst secondary to trauma and how it was radiologically and surgically managed. Clinical case. This is the case of a 13-year-old male patient admitted as a result of a grade IV pancreatic lesion, which turned into a 170x86x180 mm pancreatic pseudocyst. Intracystic bleeding required radiological embolization of the proximal gastroduodenal artery. Subsequent abdominal compartment syndrome, biliary leak, and chemical peritonitis required laparotomy and collection drainage. Pancreatitis and duct fistula had a slow but favorable progression. Discussion. The presence of duct damage is a failure predictor in the conservative treatment of pancreatic trauma. Surgical management could be indicated in recurrent, multiple, or giant (> 200 mm) pseudocysts. Intracystic bleeding is rare but potentially fatal. Summary of the Event: A 57-year-old male with chronic pancreatitis presented with an enlarging pseudocyst which was treated with endoscopic ultrasound (EUS)-guided cystogastrostomy. Three days later, he presented with massive hematemesis, and abdominal computed tomography (CT) revealed hemorrhage within both the pseudocyst and the stomach. Endoscopic evaluation was impossible due to the massive amount of hematemesis. Angiography delineated a small pseudoaneurysm with contrast extravasation involving the branch of the splenic artery. Embolization using n-butyl-2 cyanoacrylate was performed leading to the immediate cessation of bleeding. Teaching Point: The risk of delayed hemorrhage after EUS-guided cystogastrostomy for a pancreatic pseudocyst should always be kept in mind. Angiography should be considered for the diagnostic and therapeutic purpose when hemostasis is not possible using an endoscope.

Over the last one-decade, endoscopic ultrasound (EUS)-guided drainage has evolved as a preferred modality for treating pseudocyst over conventional surgical or radiological procedures among adults using plastic stents or lumen opposing stents; however, studies on EUS-guided pancreatic of pseudocyst among children are mainly in the form of case reports or small case series. Therefore, we aimed to describe four pediatric cases of the pseudo-pancreatic cyst treated successfully with EUS-guided cysto-gastrostomy using plastic stents. In all four cases, EUS-guided drainage was successful using plastic stent with no major complications, and none of them required any follow-up endoscopic or surgical intervention. Pancreaticopleural fistula is one of the rarest complication of acute pancreatitis, chronic pancreatitis and pancreatic pseudocyst. Diagnosis requires a high index of clinical suspicion in alcoholic patients presenting with pancreatitis and associated recurrent pleural effusion. Diagnosis is delayed as patients usually present with predominant pulmonary symptoms of dyspnea and chest pain. Hereby authors presented a case, where a 50-year-old chronic alcoholic male presented with chief complains of shortness of breath and chest pain. Based on the chest radiograph, Computed Tomography (CT) scan and pleural fluid analysis diagnosis of pancreaticopleural fistula was made. The patient was managed with intercoastal tube drainage and symptomatic treatment for other complains. Background/Aims Endoscopic ultrasound (EUS)-guided transmural drainage for peripancreatic fluid collections (PFCs) has gained wide acceptance as a nonsurgical intervention. Although a lumen-apposing metal stent (LAMS) was recently introduced, there are few data comparing the clinical outcomes between LAMS and plastic stent (PS) drainage. Methods Endoscopy databases of all patients who had undergone EUS-guided drainage for PFCs were searched and the clinical outcomes of EUS-guided drainage according to stent-type used were compared. Results A total of 27 patients (median age, 56 years) with PFCs underwent EUS-guided transmural drainage between January 2011 and December 2017. Of these, 17 underwent PS placement and 10 underwent LAMS placement. There was no significant difference in the technical success rate between the 2 groups (94.1% vs. 100%, p=1.0). Procedure time was shorter in the LAMS group compared to that in the PS group (10.6±2.5 min vs. 21.4±9.5 min, p=0.002). Among subjects with clinical success, recurrence of PFC after stent removal occurred in 5 of 12 patients with PS and 4 of 10 with LAMS, without statistical difference (41.7% vs. 40.0%, p=1.0). Conclusions Although our study showed similar clinical outcomes for LAMS and PS, further prospective trials are required to validate the superiority of LAMS.

Congenital pancreatic cysts are rare in newborn babies, and this makes prenatal diagnosis difficult. Diagnosis can be delayed for a few months after birth until the infant presents with an abdominal mass, abdominal distension, or vomiting due to gastric outlet obstruction. Excision of the cyst is the treatment of choice. A congenital pancreatic cyst should be considered if the fetus has an abdominal cyst without a definite origin. A prompt diagnosis is crucial to prevent fatal complications such as cholangitis, pancreatitis, cyst rupture, and peritonitis. We report a case of neonate with multiple congenital pancreatic cysts suspected prenatally to be stomach diverticulum or duplication cysts of the intestine.

Abstract Background Duodenal duplication cysts in adults are rare, and a preoperative diagnosis remains difficult because clinical manifestations are nonspecific and variable. We describe a case of a duodenal duplication cyst mimicking a pancreatic pseudocyst with repeated intracystic hemorrhage. Case presentation A 47-year-old male who complained of upper abdominal pain and vomiting was referred to our hospital. He was a heavy drinker and had a past history of hospitalization for alcoholic chronic pancreatitis. Plain abdominal computed tomography (CT) showed a cystic lesion of 7 cm in size in the lumen near the second part of the duodenum. The cystic lesion showed high density inside. Gastrointestinal endoscopy revealed that the lumen of the duodenum was deformed by a submucosal tumor-like mass and the endoscope could not pass through it, but active bleeding was not seen in the lumen of the duodenum. On the fourth day of hospitalization, anemia progressed and contrast-enhanced CT demonstrated a high-density spot on the wall of the cystic lesion. A pancreatic pseudocyst complicated with intracystic hemorrhage was preliminarily considered. Angiography was immediately performed, and a pseudoaneurysm was identified in the branch of the anterior superior pancreaticoduodenal artery (ASPDA). Transcatheter arterial embolization (TAE) was performed. Anemia did not progress after that, and follow-up CT showed reduction in the size of the cystic lesion. Afterward, the same symptoms recurred twice and surgical treatment was performed for the pancreatic pseudocyst with repeated intracystic hemorrhage. Macroscopically, a cystic mass of 5 cm in size was adjacent to the second part of the duodenum on the pancreas side. A pinhole-sized communication was identified between the cyst and the duodenum lumen. Microscopically, the cyst wall was composed of gastric mucosa and shared a common proper muscle layer with the duodenum. Chronic ulcers and erosions were seen in the cyst. Based on these findings, a diagnosis of duodenal duplication cyst was made. Conclusions Duodenal duplication cysts in adults have seldom been reported and should be considered as a differential diagnosis for a patient with a cystic lesion adjoining the duodenum.

Pancreatic cystic lesions comprise diverse entities with different histopathological characteristics. Differential diagnosis is often challenging. Autoimmune pancreatitis (AIP) is usually not considered an underlying pathology in the differential diagnosis of peri-/pancreatic pseudo-/cystic lesions. We report the case of a 73-year-old male with diffuse pancreatic enlargement and an adjacent cystic lesion (60 × 80 mm) on computed tomography scan. Based on these imaging findings and an elevated serum IgG4 concentration, AIP complicated by a pancreatic pseudocyst was diagnosed, and treatment with glucocorticoids was started. Regular follow-ups showed a good response to treatment with regression of the pancreatic pseudocyst and remittent pancreatic swelling.

Objective To investigate the feasibility, efficacy and safety of endoscopic ultrasound (EUS)-guided drainage of peripancreatic fluid collection (PFC) using lumen-apposing fully covered, self-expandable metal stent (LAMS) for treatment of infected pancreatic necrosis (IPN) and pancreatic pseudocyst (PPC). Methods We retrospectively analyzed the data of 13 patients (including 11 male and 2 female patients, mean age 42.6±12.1 years) with IPN (12 cases) or PPC (1 case) treated with EUS-guided drainage of PFC using LAMS in our department from January, 2012 to December, 2017. We also searched China National Knowledge Infrastructure (CNKI), Wanfang Database, VIP Database Network, Cochrane Library, Pubmed, and Web of Science for relevant studies to analyze the efficacy and safety of LAMS in the treatment of IPN and PPC. Results Stent placement was successful in all the 13 patients treated in our department. Twelve (92.3%) of the patients were cured, and 1 (7.7%) died for severe infection and intra-abscess hemorrhage. None of the patients experienced stent displacement, abscess or cyst cavity rupture (or perforation), or drainage tube occlusion. Six (46.2%) of the patients had pulmonary infections after the operation. The mean length of hospital stay of the patients was 44.8 d (range 9-153 d). We retrieved 12 relevant studies from the databases involving a total of 331 patients (193 with IPN and 138 with PPC), among whom 326 (98.5%) were successfully treated and 5 (1.5%) died. In the total of 344 cases after combining our cases with the reported cases (including 206 patients with IPN and 138 with PPC), the success rate of LAMS surgery was 100%, the cure rate was 98.3% (338 cases), and the mortality rate was 1.7% (6 cases); stent displacement occurred in 13 cases (3.8%), postoperative bleeding in 11 cases (3.2%), stent occlusion in 3 cases (0.9%), postoperative abscess perforation in 1 case (0.3%), and postoperative with lung and urinary tract infections in 19 cases (5.5%). Conclusion EUS-guided drainage of PFC using LAMS has a high success rate for treatment of IPN and PPC and achieves good clinical outcomes with a low incidence of complications and a low mortality rate of the patients.

Abstract In recent years, technological advancements including endoscopic ultrasound (EUS) guidance and availability of specifically designed stents further expanded the indications and possibilities of interventional endoscopy. Although technically demanding and associated with non-negligible morbidity, advanced pancreatic endoscopic techniques now provide an effective minimally invasive treatment for complications of acute and chronic pancreatitis. Aiming to provide radiologists with an adequate familiarity, this pictorial essay reviews the indications, techniques, results and pre- and post-procedural cross-sectional imaging appearances of advanced endoscopic interventions on the pancreas and pancreatic ductal system. Most of the emphasis is placed on multidetector CT and MRI findings before and after internal drainage of pseudocysts and walled-off necrosis via EUS-guided endoscopic cystostomy, and on stent placement to relieve strictures or disruption of the main pancreatic duct, respectively in patients with chronic pancreatitis and disconnected pancreatic duct syndrome.

Pancreatic fluid collections (PFCs) develop as a result of damage to the major or peripheral pancreatic ducts, complication due to acute or chronic pancreatitis, trauma or iatrogenic causes. PFCs include pancreatic pseudocysts (PPs) and walled-off necrosis (WON). PFCs usually resolve spontaneously and are asymptomatic, but if they persist, increase in dimension or became symptomatics, therapeutic intervention is required. Available therapeutic interventions include surgical, percutaneous, and endoscopic drainage. The endoscopic approach is nowadays considered the first line-treatment of PFCs due to various advantages when compared with surgical or percutaneous drainage: decreased morbidity, length of hospital stay, and reduced costs. In the last few years, the endoscopic ultrasound (EUS)-guided transmural drainage, initially with plastic stents, gained popularity. More recently, fully covered self-expanding lumen-apposing metal stents (LAMS) have been demonstrated to be both, safe and effective with high clinical and technical success, reducing the risk of perforation, peritoneal leakage, migration and facilitating the drainage of necrotic contents. In the last few years, several studies evaluating the safety and efficacy of LAMS and their differences with plastic stents have been performed, but literature on the removal timing of this device and associated complications is still limited. The aim of this review is to analyze studies reporting information about the retrieval timing of LAMS and the related adverse events.

A prospective study of 182 patients with pan­creatic pseudocyst was conducted. By studying changes in ion-acid-alkaline status of cysts cavity, the correlations between these indicators, indicators of proteinogram of blood, findings of ultrasound examination and computed tomography were established. The algorithm for surgical treatment of pancreatic pseudocysts for each of the clinical groups of patients has been developed. Patients of the first group underwent interventions mainly in the cyst, as the structural changes in the pancreas were minor and did not require correction. In the second group 63.16% of patients underwent draining operations, in 57.89% of patients it was the final treatment stage, and 15.80% required the correction of manifestations of chronic pancreatitis. In the third group of patients pancreas was almost completely damaged by chronic inflammation of fibrous-degenerative character, calculosis, ductal hypertension, there with mostly direct interventions were performed on the pancreas. New approaches pathogenetically justify differential surgical treatment strategy for pancreatic pseudocyst using both minimally invasive and open surgery.

A pancreatic pseudocyst (PP) is a localized collection of pancreatic juice in pancreatic tissue or peripancreatic space reach, in amylase and other enzymes, surrounded by a cyst wall composed of fibrous wall or granulation tissue, and a cyst wall lacking epithelium. A PP develops after acute pancreatitis, chronic pancreatitis, pancreatic trauma, or obstruction of the main pancreatic duct. PP commonly presents with symptoms of abdominal pain, early satiety, nausea and vomiting in the setting of an abdominal mass. Diagnosis is based on clinical and laboratory tests (persistent high levels of amylase) and on imaging by abdominal ultrasound or computer tomography (CT) scan of a persistent pancreatic fluid collection present for at least 4 weeks. Pseudocysts cannot be removed until 6 weeks after an episode of pancreatitis, to allow the walls to thicken to at least 6mm and the pseudocyst to grow to at least 6cm in size; however, pseudocyst formation is difficult to date. We present a patient who had a ruptured PP and therefore underwent emergency laparotomy. The cyst wall was immature and therefore a cystenterostomy was not possible. PP excision was chosen because the PP was already ruptured and had dense immature cyst-bowel adhesions, a thin wall and absence of abnormality of pancreatic main duct; in addition, expertise was available. A 42-year-old Caucasian female presented with lower limb panniculitis and bilateral ankle arthritis in the absence of abdominal or other localizing symptoms. Abdominal imaging revealed subacute pancreatitis with pseudocyst formation. The clinical manifestations were compatible with pancreatitis, panniculitis, and polyarthritis syndrome (PPP syndrome), a very rare complication of pancreatic disease. The patient improved with conservative treatment for the pancreatic disease and systemic corticosteroids for the cutaneous and articular manifestations. We identified 59 patients with the PPP syndrome from the literature, the majority of patients being male (74.6%) with a median age of 49 (IQR 41–63.5) years. Acute pancreatitis is the most frequent underlying disorder (54.2%), but gastrointestinal symptoms are absent in 45.8% of patients. Pancreatic panniculitis has a predilection for the lower limbs, which are affected in 98.3% of cases. However, the cutaneous lesions may also involve the upper limbs and trunk. Arthritis is typically symmetric and polyarticular in nature, affecting both large and small joints. Of all patients who received treatment, 78.6% had a poor response. Surgical internal drainage of pancreatic pseudocyst can be done into the stomach, duodenum or jejunum depending on the anatomic relation of pseudocyst with hollow viscera. For cystojejunostomy, a Roux‑en‑Y loop is preferred over loop cystojejunostomy as former is thought to avoid the reflux of jejunal contents into the cyst cavity. This study presents our experience with laparoscopic loop cystojejunostomy showing loop cystojejunostomy for the pseudocyst of the pancreas can be safely performed laparoscopically with simpler technique with no complications including reflux. Introduction: Pancreatic pseudocysts (PQP) correspond to 80% of cystic lesions of the pancreas. A cystic tumor must be ruled out, which represents 10% to 15% of pancreatic cysts and 1% of malignant tumors, so the study and differential diagnosis is essential. The term giant pancreatic pseudocyst is traditionally used when the size is greater than 10 cm. There are few cases of giant PQP in the national literature. Objective: To report a clinical case with giant PQP, to analyze the differential diagnosis and therapeutic options. Materials and Method: Patient with progressive abdominal distension, weight loss and anorexia post episode of acute pancreatitis. Abdominal computed tomography and magnetic resonance imaging confirm a giant intra-abdominal cystic lesion. The patient was treated with an open Roux-en-Y pancreatic cystojejunostomy. Analysis of the content aspirated during surgery suggests PQP. The most relevant literature is reviewed for discussion. Results: Excellent postoperative clinical results, the histopathological study of the cyst wall, confirmed the diagnosis of pancreatic pseudocyst. At one year of follow-up, the patient remains asymptomatic. Discussion: The preoperative study is crucial to determine the differential diagnosis and rule out neoplastic or cystic parasitic lesions. Pancreatic pseudocyst (PC) and walled-off necrosis (WON) are dreaded complications of acute pancreatitis. Standard therapy consists of endoscopic ultrasound-guided transmural placement of stents to expedite resolution through internal drainage of fluids or necrotic material. Either double pigtail plastic stents (DPPS) or lumen-apposing metal stents (LAMS), or a combination of both, are available for this purpose. The objective of this study was to examine the impact of different stent types on infection rates in addition to clinical outcome measures such as periprocedural adverse events. We conducted a retrospective study comprising 77 patients who had undergone endoscopic drainage for PC or WON in a pancreatitis tertiary referral center. Analysis revealed that both bacterial and fungal infections occurred more frequently in patients treated with LAMS with or without DPPS compared to DPPS only. The use of antibiotics and antimycotics followed the same pattern. Furthermore, a prolonged length of hospital stay and a higher likelihood of transfer to an intermediate care unit were observed in patients with LAMS with or without DPPS. These differences were eliminated if only WON patients were analyzed. Our data imply that the clinical course is primarily influenced by the complexity of the pancreatic fluid collection (PFC) itself rather than the stent type. Prospective large-scale cohort studies are mandatory to underpin these findings.

Pancreatobiliary disorder is a challenging clinical condition, especially when this condition is causing severe infection or biliary sepsis, and sometimes it requires intensive care unit (ICU) treatment. Biliary drainage is the mainstay of therapy; however, the choice of the drainage method is dependent on the patient's clinical condition and the disease itself. A 79-year-old female was transferred on a ventilator to our ICU from another hospital due to biliary sepsis, a large common bile duct stone, and an infected pancreatic pseudocyst. The patient also has other comorbidities such as heart problems, hypothyroidism, and diabetes mellitus. Bedside percutaneous transhepatic biliary drainage without fluoroscopy and percutaneous cyst aspiration was successfully performed, which improved the patient's condition; this was followed by an endoscopic approach, i.e., endoscopic retrograde cholangiopancreatography (ERCP) and endoscopic ultrasound-guided pancreatic pseudocyst drainage. The clinical improvement showed itself in the change of the patient's respiratory status and ventilator mode. In conclusion, the percutaneous approach has a big role in managing critically ill patients in the ICU setting. However, expertise, training experience, and a multidisciplinary team approach are very important for successful management and patient outcome. [ABSTRACT FROM AUTHOR]
