151. Oxidation of 14C-β-hydroxybutyrate administered by ventriculo-cisternal perfusion in the dog

Abstract

A ventriculo-cisternal perfusion system (from the 3rd ventricle to cisterna magna) was employed to study the oxidation of Na-DL-3-hydroxybuty-rate-3-14C when added to the perfusion fluid. 14CO2 appeared in the perfusate and in the blood taken from both the confluens sinuum and a peripheral artery, in the course of the perfusion. Specific activity of CO2 in the perfusate usually exceeded that of the venous blood, which in turn was always higher than that of arterial blood. The calculated rate of oxidation of ßOHB to CO2 was about 20 mµmole/min, or 5–6% of the perfused ßOHB. It is postulated that ßOHB supplied locally in CSF may serve some supplemental physiological role as an oxidizable substrate.

152. Hepatic and Splanchnic Uptake and Oxidation of Free Fatty Acids1

Abstract

The uptake and oxidation of free fatty acids by the liver and by the non-hepatic splanchnic area were investigated in anesthetized; control and diabetic dogs. The animals received a constant infusion of albumin-bound 1-14C-palmitate. The liver of the control dogsremoved 67 μmoles of FFA/min, the NHSA took up 32 μmoles/-min. Hepatic extraction of the labeled FFA was 35%; splanchnic extraction 22% of the arterial level. About 13% of the removed FFA was oxidized both by the liver and by the NHSA. The liver released 13% of the removed labeled carbon as TG, and 7% of the fatty acid carbon as βOHB. Hepatic FFA uptake accounted for 37%, NHSA uptake for 22% of the total body FFA flux. NHSA removed glucose in the control, but not in the diabetic animals. Uptake and oxidation of FFA, both by the liver and by the NHSA, were considerably elevated in the diabetic dogs.However; the ratios of FFA uptake to total FFA flux were not significantly different. The fractional release of TG from the liver was not significantly changed. A direct correlation was shown between the arterial FFA level and either the uptake or oxidation of FFA by the NHSA. Since neither the pancreas nor the spleen exhibited a more active FFA metabolism than did the intestine, it seemed that most of the metabolic changes noted in the NHSA were due to the gastrointestinal tract.Data analysis was aided by the Hahnemann Computer Center which is supported by Grant FR 00266 from the National Institutes of Health.

Abstract

MAJOR ACCOMPLISHMENTS IN THE LAST 5 YEARS HAVE BEEN TO CLARIFY THE METABOLIC ALTERATIONS THAT ACCOMPANY ENDOTOXIN SHOCK, PRIMARILY WITH REGARD TO GLUCOSE TURNOVER AND GLUCONEOGENESIS FROM LACTATE AND ALANINE. Also, the alterations in myocardial substrate utilization have been identified, and the significance of substrate preference characterized following endotoxin administration. Furthermore, a useful and physiologically sound isolated myocardial cell preparation has been developed which enables us to further investigate the sub-cellular effects of both cardiac depressant factors and also cardiotonic agents. Although much has been accomplished, it is quite clear that many features of the exact sub-cellular defects caused by endotoxin administration, or any other form of shock, still remain to be learned.

Abstract

The major metabolic alterations during shock have been studied in detail. The importance of the elevated lactate concentration in influencing oxidative metabolism has been emphasized. This change is probably responsible, at least in part, for the alteration in myocardial metabolism found previously during shock. Phospholipid changes have also been studied with special reference to phospholipids and especially to phosphatidylglycerol. The metabolism of phosphatidylglycerol is quite unique during shock and the importance of red blood cells as well as plasma in producing the alterations have been pointed out. More recent investigations point to important hormonal alterations during shock. Some of these have been investigated in detail and especially the role of insulin release by hyperglycemia was found to be altered during shock.

Abstract

Several of the metabolic alterations characterizing various shock models have been uncovered and described. Special attention was given to the metabolic changes of the myocardium which are quite marked and consistent from one shock model to another. It appears that in this tissue the importance of fatty acid oxidation is underemphasized during shock while the significance of lactate utilization becomes greater. These studies take us a step closer to the understanding of metabolic inadequacies accompanying shock with the eventual understanding of subcellular damage leading to the irreversibility of this condition.
