A worsening trend of critical shortages in senior health care workers across low- and middle-income countries (LMICs) in sub-Saharan Africa has been documented for decades. This is especially the case in Ethiopia that has severe shortage of mental health professionals. Consistent with the WHO recommended approach of task sharing for mental health care in LMICs, Acceptance and Commitment Therapy (ACT), which is an empirically validated psychological intervention aimed at increasing psychological flexibility, may be delivered by trained laypersons who have a grassroots presence. In this paper, we discuss the need for and potential role of ACT to be delivered by health extension workers (HEWs) to address mental health care needs across Ethiopia. To this end, we also reviewed previous studies that have examined the effectiveness of ACT-based interventions in African countries including in Nigeria, Sierra Leone, Uganda, and South Africa. All studies revealed significant improvements of various mental health-related outcome measures such as decreased psychological distress and depressive symptoms, or increased subjective wellbeing and life satisfaction in the groups that received an ACT-based intervention. However, to date, there is no study that applied ACT in Ethiopia. Thus, more research is warranted to examine the effectiveness and, if proven successful, to scale up a task sharing approach of an ACT-based intervention being delivered by trained HEWs at a grassroots level, possibly paving the way for an innovative, sustainable mental health service in Ethiopia as well as other African LMICs.

UDP-glucuronic acid is a component of the extracellular matrix. Here, the authors report biallelic variants in the gene encoding UDP-Glucose 6-Dehydrogenase (UGDH) in individuals affected by developmental epileptic encephalopathies that impair UGDH stability, oligomerization, or enzymatic activity in vitro.

Due to several physical limitations in the realization of quantum hardware, today's quantum computers are qualified as noisy intermediate-scale quantum (NISQ) hardware. NISQ hardware is characterized by a small number of qubits (50 to a few hundred) and noisy operations. Moreover, current realizations of superconducting quantum chips do not have the ideal all-to-all connectivity between qubits but rather at most a nearest-neighbor connectivity. All these hardware restrictions add supplementary low-level requirements. They need to be addressed before submitting the quantum circuit to an actual chip. Satisfying these requirements is a tedious task for the programmer. Instead, the task of adapting the quantum circuit to a given hardware is left to the compiler. In this article, we propose a hardware-aware (HA) mapping transition algorithm that takes the calibration data into account with the aim to improve the overall fidelity of the circuit. Evaluation results on IBM quantum hardware show that our HA approach can outperform the state of the art, both in terms of the number of additional gates and circuit fidelity.

Aim and Background: Psychiatric disorders are gaining top rank in the burden of disease. Undoubtedly, knowing their prevalence in children and adolescents can help prevent and control these problems and save money. Therefore, the purpose of this study was to investigate the prevalence of psychiatric disorders in children and adolescents in Isfahan. Methods and Materials: According to the nature of the research, the research method is a fundamental type that was performed on 1010 children and adolescents in Isfahan province in 2017 that were randomly selected using multistage cluster sampling. Clinical psychologists are trained to conduct research refer to the selected children's homes and using the Persian version of the semi-structured diagnostic interview for Mental Disorders and Schizophrenia for children and adolescents, Current Detection and Lifespan (K-SADS-PL). In addition, demographic data (gender, age, education, parental education, and economic status) were also collected. After collecting the data, the data were analyzed by SPSS software using frequency indices and frequency. Findings: According to the findings, the prevalence of psychiatric disorders in children and adolescents in Isfahan province is 14.8% and the prevalence of these disorders in boys is higher than in girls. Also, according to the results, the prevalence of these disorders in the age range of 10-14 years was more than other ages. Psychiatric disorders in children and adolescents in the city were more than children and adolescents in the village. Conclusions: According to the results, the prevalence of psychiatric disorders in Isfahan province is in the middle range compared to other studies carried out in other researches carried out inside and outside the country. However, the need for mental health policies in childhood and adolescence is suggested to reduce the burden of damage in the future and provide solutions to the proble m.

In essence this article explores the relationship between the labour movement and formal education and the origins and development of teaching unions. The first part presents the educational proposals and actions implemented by the libertarian anarchist and socialist movements during the first thirty years of the 20th century and then, during the Civil War. It further examines the steps taken by these movements to set up their own schooling network, and the influence exerted by the Soviet education system of the time –either as a model to follow or, on the contrary, one to reject. In the second half, the article assesses the role played by the resurgence of the labour movement under Franco’s dictatorship (1950-1977), including the rise of the teacher movement (1968-1977), and, finally, the reconfiguration of the teaching unions. It is in this context that the article considers the proposal of «an alliance between the forces of labour and those of the arts», as advocated by certain political groups, including the then current debate about the designation of teachers as educational workers, as well as the ‘Educational Alternatives’ that flourished between 1975 and 1977, the nature of which was both revolutionary and reformist.

Background: Changes in temperature and precipitation pattern seriously affect the amount of river runoff coming into Dam Lake. These changes could influence the operating conditions of reservoir systems such as Jor hydropower reservoir system (Malaysia) with the total capacity of 150 MW. So, it is necessary to analyze the effect of changes in weather parameters on the river runoff and consequently, the hydropower production. Methods: In this research, LARS-WG was used to downscale the weather parameters such as daily minimum temperature, maximum temperature, and precipitation based on one of the general circulation sub-model (HADCM3) under three emission scenarios, namely, A1B, A2, and B1 for the next 50 years. Then, the artificial neural network (ANN) was constructed, while rainfall and evapotranspiration were used as input data and river runoff as output data to discover the relationship between climate parameters and runoff at the present and in the future time. Results: It was revealed that the monthly mean temperature will increase approximately between 0.3-0.7°C, while the mean monthly precipitation will vary from -22% to +22% in the next 50 years. These changes could shift the dry and wet seasons and consequently, change the river runoff volume. In most months, the results of models integration showed reductions in river runoff. Conclusion: It can be concluded that the output of hydropower reservoir system is highly dependent on the river runoff. So, the impacts of climate changes should be considered by the reservoir operators/managers to reduce these impacts and secure water supplies.

Background and Objective: The primary goal of this study is to develop a rigorous understanding ofthe correlation between COX-2 expression and malignant melanoma prognostic factors. Material and Methods: In this cross-sectional study, we analyzed 60 cases of cutaneous malignant melanoma. The related stained slides were reviewed by two pathologists. The results were interpreted according to the COX2 staining index (SI), tumor thickness (Breslow, Clark), number of mitoses per 10 hpf, and melanoma types. Gender, lymph node involvement, metastasis, and survival were considered as evaluation factors as well. Results: The expression of the COX-2 protein was evident in 98.4% of cases. A strong Staining Index(SI) was reported in 60% of all melanomas, moderate staining was detected in 20.8% and weak staining in 10%; 1.6% of studied cases showed no staining. Benign nevus specimens showed no staining for the COX-2 enzyme. Conclusion: We have demonstrated that COX-2 is strongly expressed in the majority of malignant melanomas and that the SI score of COX-2 is related to the number of mitoses, tumor thickness (based on Clark level and Breslow), melanoma sub-type, lymph node involvement, and metastases; No association was noted between the anatomic site, gender, and survival. COX-2 can be applied as a prognostic factor in malignant melanoma and a promising candidate for future target therapies.

Computing paradigm based on von Neuman architectures cannot keep up with the ever-increasing data growth (also called “data deluge gap”). This has resulted in investigating novel computing paradigms and design approaches at all levels from materials to system-level implementations and applications. An alternative computing approach based on artificial neural networks uses oscillators to compute or Oscillatory Neural Networks (ONNs). ONNs can perform computations efficiently and can be used to build a more extensive neuromorphic system. Here, we address a fundamental problem: can we efficiently perform artificial intelligence applications with ONNs? We present a digital ONN implementation to show a proof-of-concept of the ONN approach of “computing-in-phase” for pattern recognition applications. To the best of our knowledge, this is the first attempt to implement an FPGA-based fully-digital ONN. We report ONN accuracy, training, inference, memory capacity, operating frequency, hardware resources based on simulations and implementations of 5 × 3 and 10 × 6 ONNs. We present the digital ONN implementation on FPGA for pattern recognition applications such as performing digits recognition from a camera stream. We discuss practical challenges and future directions in implementing digital ONN.

Introducción: La situación de cuarentena comunitaria ha sido una de las acciones de mayor alcance llevadas a cabo para contener la propagación de la COVID-19. El cambio brusco de las dinámicas sociales trae consigo una diversidad de efectos a nivel individual y familiar que ha sido menos abordado desde la perspectiva de la comunidad. El objetivo de este trabajo es describir los impactos psicosociales de la cuarentena en el reparto 26 de julio, en Holguín, comunidad en la que se identificó un evento de transmisión local y permaneció aislada por 33 días. Métodos: Se llevó a cabo una investigación cualitativa, exploratoria y descriptiva con el empleo de grupos focales en distintas categorías de la población. Resultados: Se concluye que el impacto psicosocial fue apreciado en todos los grupos poblacionales estudiados; se identificaron efectos negativos y positivos, relacionados con la capacidad de adaptación de la comunidad, entre ellos se reconoció la gestión de gobierno. Se exponen, además, las limitaciones del estudio.

Nano-oscillators based on phase-transition materials are being explored for the implementation of different non-conventional computing paradigms. In particular, vanadium dioxide (VO2) devices are used to design autonomous non-linear oscillators from which oscillatory neural networks (ONNs) can be developed. In this work, we propose a new architecture for ONNs in which sub-harmonic injection locking (SHIL) is exploited to ensure that the phase information encoded in each neuron can only take two values. In this sense, the implementation of ONNs from neurons that inherently encode information with two-phase values has advantages in terms of robustness and tolerance to variability present in VO2 devices. Unlike conventional interconnection schemes, in which the sign of the weights is coded in the value of the resistances, in our proposal the negative (positive) weights are coded using static inverting (non-inverting) logic at the output of the oscillator. The operation of the proposed architecture is shown for pattern recognition applications.

The eating patterns in a population can be estimated through dietary surveys in which open-ended assessment methods, such as diaries and interviews, or semi-quantitative food frequency questionnaires are administered. A harmonized dietary survey methodology, together with a standardized operational procedure, in conducting the study is crucial to ensure the comparability of the results and the accuracy of information, thus reducing uncertainty and increasing the reliability of the results. Dietary patterns (i) include several target variables (foods, energy and nutrients, other food components), (ii) require several explanatory variables (age, gender, anthropometric measurements, socio-cultural and economic characteristics, lifestyle, preferences, attitudes, beliefs, organization of food-related activities, etc.), and (iii) have impacts in several domains: imbalance diets; acute and chronic exposures affect health, specifically non-communicable diseases; and then sanitary expenditure. On the other hand, food demand has impacts on the food system: production, distribution, and food services system; food wastes and other wastes generated by food-related activities of the households (e.g., packaging disposal) have consequences on the “health of the planet” which in turn can have effects on human health. Harmonization and standardization of measurement methods and procedures in such a complex context require an ad hoc structured information system made by databases (food nomenclatures, portion sizes, food atlas, recipes) and methodological tools (quantification methods, food coding systems, assessment of nutritional status, data processing to extrapolate what we consider validated dietary data). Establishing a community of professionals specialized in dietary data management could lead to build a surveillance system for monitoring eating habits in the short term, thus reducing costs, and to arrange a training re-training system. Creating and maintaining the dietary data managers community is challenging but possible. In this context, the cooperation between the CREA Research Centre for Food and Nutrition and the Italian National Health Institute (ISS) promoted and supported by the Italian Ministry of Health may represent a model of best practice that can ensure a continuous training for the professional community carrying out a nutritional study.

Mitral regurgitation is the second-most frequent valvular heart disease in Europe after degenerative aortic stenosis. It is associated with significant morbidity and mortality, and its prevalence is expected to increase with population aging. Echocardiography is the first diagnostic approach to assess its severity, constituting a challenging process in which a multimodality evaluation, integrating quantitative, semiquantitative and qualitative methods, as well as a detailed evaluation of the morphology and function of both left ventricle and atria is the key. In this review, we would like to provide a practical diagnosis approach on the mitral valve regurgitation mechanism, severity quantification, and planning of future therapeutic options.
