Abstract

В исследование включены пациентки с неинвазивным раком и раком молочной железы c TNMстадией, получившие 1a-b-c00 лечение с 1985 по 2009 г. в России (РОНЦ им. Н.Н. Блохина РАМН и Клиника РМАПО – 1036 больных) и в Нидерландах (LUMC – 560 пациенток, Национальный Канцрегистр – 22196 больных). Проведен сравнительный анализ типов выполненных операций по странам, а также во временных подгруппах. Частота органосохраняющего лечения при «раннем» раке молочной железы в России и Нидерландах идентична – 53,7 % и 52,5 %. Доля органосохраняющих операций в течение 20 лет в России существенно не изменилась, тогда как в Нидерландах отмечено уменьшение доли органосохраняющего лечения с 56,2 % до 41,2 %, что, вероятнее всего, объясняется увеличением числа пациенток, находящихся в постменопаузе (>60 лет) с 53,1 % до 63,8 %. Основным вариантом органосохраняющего лечения в России является радикальная резекция молочной железы, в Нидерландах – лампэктомия с БСЛУ или аксиллярной лимфодиссекцией, причем БСЛУ и в настоящее время не является «рутинной» операцией во всей стране., The study included patients with noninvasive cancer and stage T 1a-b-cN 0M 0 invasive breast cancer who were treated between 1985 to 2009 in Russia (at the N.N. Blokhin Russian Cancer Research Center and at the Clinic of the Russian Medical Academy of Postgraduate Training, 1036 patients), and in the Netherlands (LUMC, 560 patients, National Cancer Register, 22196 patients). The comparative analysis of surgery types between countries was carried out. The frequency of organ-preserving surgeries for early breast cancer in Russian and in the Netherlands was identical (53.7 % and 52.5 %). The percentage of organ-preserving surgeries over the past 20 years in Russia was not significantly changed, whereas the decrease in the rate of organ-preserving treatment from 56.2 % to 41.2 % was observed in the Netherlands.

Abstract

Цель исследования изучить прогностическое значение возраста при раннем раке молочной железы (T1N0M0-стадии) на большом клиническом материале. Материалы и методы. В мультицентровое исследование включены архивные данные о пациентках с ранним раком молочной железы (T1N0M0-стадии), которые получали лечение с 1985 по 2009 гг. в России (РОНЦ им. Н.Н. Блохина РАМН и Клиника РМАПО 1036 человек) и Нидерландах (Медицинский центр Лейденского университета, LUMC 560 человек). Для сравнения изучены данные Канцрегистра Нидерландов (22 196 больных, включенных в Национальный архив с 1989 по 2009 гг.). Результаты. Возраст при раннем раке молочной железы является важным прогностическим фактором: доля рецидивов болезни статистически значимо выше среди пациенток моложе 40 лет, а показатели безрецидивной выживаемости достоверно лучше у женщин старше 50 лет. В Нидерландах по сравнению с данными российских клиник преобладают пациентки старше 50 лет. «Прирост» возраста пациенток с ранним раком молочной железы в течение 20 лет произошел в обеих странах., The aim of the investigation is the study of prognostic significance of age in early breast cancer (T1N0M0-stage) on the basis of large clinical data. Materials and methods. In multi-centre study included archival data of patients with early breast cancer (T1N0M0-stage) who had been treated from 1985 to 2009 in Russia (N.N. Blokhin Russian Cancer Research Center of RAMS and Clinic of Russian Medical Academy for Postgraduate Education 1036 female) and in the Netherlands (Leiden University Medical Center, LUMC 560 patients). For comparison there were studied the data of Dutch Cancer Database (22 196 patients included into National Archive from 1989 to 2009). Results. Age of patients is an important prognostic factor for early breast cancer: the rate of recurrences is higher in patients 50 years. Abstract

Voor patiënten met een melanoom is het van groot belang om het stadium van de ziekte nauwkeurig vast te stellen om adequate behandeling te kunnen krijgen. Uitzaaiingen van het melanoom bij patiënten met bewezen lymfklieruitzaaiingen (stadium III) kunnen goed worden opgespoord met FDG-PET en CT scans. Onderzoeker Esther Bastiaannet leidde vanuit het UMCG een landelijke studie waarin bij 25% van de patiënten uitzaaiingen werden gevonden die met gebruikelijke diagnostiek niet zichtbaar worden. De kosten voor diagnose en behandeling per patiënt dalen met 5% als alleen een CT scan wordt gedaan, stijgen met 7% bij alleen FDG-PET, en stijgen met 15% als beide scans worden gedaan. Het aantal mensen bij wie melanoom wordt vastgesteld stijgt sterk de laatste jaren. De verwachting is dat in 2012 bij 4500 Nederlanders de diagnose melanoom, ofwel huidkanker, wordt vastgesteld. Het melanoom is verantwoordelijk voor meer dan 75% van de sterfte als gevolg van huidkanker. Een van de kenmerken van melanoom is dat het uitzaait naar onvoorspelbare locaties in het lichaam. Tijdig opsporen van uitzaaiingen is belangrijk om de juiste prognose te kunnen geven aan de patiënt en om te kunnen behandelen.

Abstract

BACKGROUND: After breast-conserving surgery, radiotherapy reduces recurrence and breast cancer death, but it may do so more for some groups of women than for others. We describe the absolute magnitude of these reductions according to various prognostic and other patient characteristics, and relate the absolute reduction in 15-year risk of breast cancer death to the absolute reduction in 10-year recurrence risk. METHODS: We undertook a meta-analysis of individual patient data for 10,801 women in 17 randomised trials of radiotherapy versus no radiotherapy after breast-conserving surgery, 8337 of whom had pathologically confirmed node-negative (pN0) or node-positive (pN+) disease. Abstract

Evidence exists for an immunomodulatory effect of endocrine therapy in hormone receptor-positive (HR+ve) breast cancer (BC). Therefore, the aim of this study was to define the prognostic and predictive value of tumor immune markers and the tumor immune profile in HR+ve BC, treated with different endocrine treatment regimens. 2,596 Dutch TEAM patients were treated with 5 years of adjuvant hormonal treatment, randomly assigned to different regimens: 5 years of exemestane or sequential treatment (2.5 years of tamoxifen–2.5 years of exemestane). Immunohistochemistry was performed for HLA class I, HLA-E, HLA-G, and FoxP3. Tumor immune subtypes (IS) (low, intermediate & high immune susceptible) were determined by the effect size of mono-immune markers on relapse rate. Patients on sequential treatment with high level of tumor-infiltrating FoxP3+ cells had significant (p = 0.019, HR 0.729, 95 % CI 0.560–0.949) better OS. Significant interaction for endocrine treatment and FoxP3+ presence was seen (OS p < 0.001). Tumor IS were only of prognostic value for the sequentially endocrine-treated patients (RFP: p = 0.035, HR intermediate IS 1.420, 95 % CI 0.878–2.297; HR low IS 1.657, 95 % CI 1.131–2.428; BCSS: p = 0.002, HR intermediate IS 2.486, 95 % CI 1.375–4.495; HR low IS 2.422, 95 % CI 1.439–4.076; and OS: p = 0.005, HR intermediate IS 1.509, 95 % CI 0.950–2.395; HR low IS 1.848, 95 % CI 1.277–2.675). Tregs and the tumor IS presented in this study harbor prognostic value for sequentially endocrine-treated HR+ve postmenopausal BC patients, but not for solely exemestane-treated patients. Therefore, these markers could be used as a clinical risk stratification tool to guide adjuvant treatment in this BC population.

Abstract

Evidence exists for an immunomodulatory effect of endocrine therapy in hormone receptor-positive (HR+ve) breast cancer (BC). Therefore, the aim of this study was to define the prognostic and predictive value of tumor immune markers and the tumor immune profile in HR+ve BC, treated with different endocrine treatment regimens. 2,596 Dutch TEAM patients were treated with 5 years of adjuvant hormonal treatment, randomly assigned to different regimens: 5 years of exemestane or sequential treatment (2.5 years of tamoxifen-2.5 years of exemestane). Immunohistochemistry was performed for HLA class I, HLA-E, HLA-G, and FoxP3. Tumor immune subtypes (IS) (low, intermediate & high immune susceptible) were determined by the effect size of mono-immune markers on relapse rate. Patients on sequential treatment with high level of tumor-infiltrating FoxP3+ cells had significant (p = 0.019, HR 0.729, 95 % CI 0.560-0.949) better OS. Abstract

Aim According to established guidelines, patients with Stage III colon cancer should receive adjuvant chemotherapy. However, a significant proportion do not. This study assessed factors associated with the administration of adjuvant chemotherapy and causes of death. Methods Patients with Stage III colon cancer who underwent surgery between 2000 and 2009 were selected from two hospitals in the Netherlands. Patient characteristics including comorbidities and treatment preferences, tumour characteristics and follow-up were extracted from the medical records. The patient and tumour characteristics of patients who did receive chemotherapy were compared with those who did not using chi-squared analysis. Differences between the groups in causes of death were recorded together with the duration of follow-up. Results A total of 348 patients were included. The median age was 73 years (range 33-93). Over half of the patients received adjuvant chemotherapy (50.6%). Patients who did not receive adjuvant chemotherapy were significantly older ( P < 0.001), had more comorbidities ( P < 0.001) and were more often living alone ( P < 0.001). Patients who received no adjuvant chemotherapy had a reduced overall survival, and the cause of death was more often attributed to other causes (60%) than colon cancer (40%). For patients who received chemotherapy, the cause of death was usually attributed to colon cancer (71%). Conclusion Patients who did not receive adjuvant chemotherapy had a worse overall survival and the majority died due to other causes than colon cancer. In our aging society it will become even more important to develop tools to estimate remaining life expectancy in order to improve the selection of older patients for adjuvant treatments. Several studies have shown remarkable differences in colorectal cancer survival across Europe. Most of these studies lacked information about stage and treatment. In this study we compared short-term survival as well as differences in tumour stage and treatment strategies between five European countries: Norway, Sweden, Denmark, Belgium, and the Netherlands. For this retrospective cohort study all patients aged 18 years or older and operated on adenocarcinoma of the rectum without distant metastases and diagnosed in 2008 and 2009 were selected in national audit registries from Norway, Sweden, Denmark, Belgium, and the Netherlands. Differences in pre-operative treatment between the countries were compared using univariable and multivariable logistic regression. One year relative survival and one year relative excess risk of death (RER) were compared between the five countries. Large variation in the use of preoperative radiotherapy and chemoradiation was found between the countries. Even though, there was little variation in relative survival between the countries, except Sweden, which had a significant better one year RER of death among the elderly patients after adjustment. The differences in survival are expected to be caused by differences in pen-operative care, selection of patients, and especially management of elderly patients. The effects of preoperative treatment are expected to be seen on long term follow-up.
