This study investigated another type of carbonic anhydrase inhibitor and antiepileptic drug, zonisamide, in order to evaluate its potential effectiveness for migraine prophylaxis refractory to topiramate, and to assess intolerability to adverse events, paresthesia in particular. This is an open-labeled retrospective single center study. We included headache patients who met the requirement of migraine without aura and were refractory to topiramate. Patients were treated only with zonisamide 100mg/day, directly switching from topiramate. Patients were monitored every month for three months. A positive response to treatment (responders)was defined as a 50% or greater reduction in headache days at three months after study commencement, compared with baseline. One hundred and twenty migraineurs who were refractory to topiramate were recruited. Compared with baseline, headache days with zonisamide showed a decrease, compared with baseline (P < 0.01). Two patients complained of adverse effects, such as paresthesia. These results suggest that zonisamide is effective for migraine prophylaxis refractory to topiramate, or intolerable patients due to topiramateinduced paresthesia. [ABSTRACT FROM AUTHOR]

This study investigated the effect of bonemarrowmesenchymal stem cells (BMSCs) on themotor pathway in the transient ischemic rat brain that were transplanted through the carotid artery, measuring motor-evoked potential (MEP) in the four limbs muscle and the atlantooccipital membrane, which was elicited after monopolar and bipolar transcortical stimulation. After monopolar stimulation, the latency of MEP was significantly prolonged, and the amplitude was less reduced in the BMSC group in comparison with the control group (P < .05).MEPs induced by bipolar stimulation in the left forelimb could be measured in 40% of the BMSC group and the I wave that was not detected in the control group was also detected in 40% of the BMSC group. Our preliminary results imply that BMSCs transplanted to the ischemic rat brain mediate effects on the functional recovery of the cerebral motor cortex and the motor pathway. [ABSTRACT FROM AUTHOR]

Sodium ozagrel (SO) prevents platelet aggregation and vasoconstriction in the cerebral ischemia. It plays an important role in the prevention of brain damage induced by cerebral ischemia/reperfusion. Recently, many animal studies have suggested that the Panax ginseng (PG) has neuroprotective effects in the ischemic brain. In this study, we assessed the neuroprotective effects that come from a combination therapy of SO and PG in rat models with middle cerebral artery occlusion (MCAO). Animals with MCAO were assigned randomly to one of the following four groups: (1) control (Con) group, (2) SO group (3 mg/kg, intravenously), (3) PG group (200mg/kg, oral feeding), and (4) SO + PG group. The rats were subjected to a neurobehavior test including adhesive removal test and rotarod test at 1, 3, 7, 10, and 15 days after MCAO. The cerebral ischemic volume was quantified by Metamorph imaging software after 2-3-5-triphenyltetrazolium (TTC) staining. The neuronal cell survival and astrocytes expansion were assessed by immunohistofluorescence staining. In the adhesive removal test, the rats of PG or SO + PG group showed significantly better performance than those of the control group (Con: 88.1 ± 24.8, PG: 43.6 ± 11, SO + PG: 11.8 ± 7, P < .05). Notably, the combination therapy group (SO + PG) showed better performance than the SO group alone (SO: 56 ± 12, SO + PG: 11.8 ± 7, P < .05). In TTC staining for infarct volume, cerebral ischemic areas were also significantly reduced in the PG group and SO + PG group (Con: 219 ± 32, PG: 117 ± 8, SO + PG: 99 ± 11, P < .05). Immunohistofluorescence staining results showed that the group which received SO + PG group therapy had neuron cells in the normal range. They also had a low number of astrocytes and apoptotic cells compared with the control or SO group in the peri-infarction area. During astrocytes staining, compared to the SO + PG group, the PG group showed only minor differences in the number of NeuN-positive cells and quantitative analysis of infarct volume. In conclusion, these studies showed that in MCAO rat models, the combination therapy with SO and PG may provide better neuroprotective effects such as higher neuronal cell survival and inhibition of astrocytes expansion than monotherapy with SO alone. Double compression of the ulnar nerve, including Guyon's canal syndrome associated with cubital tunnel syndrome caused by the anconeus epitrochlearis muscle, is a very rare condition. We present a case of double crush syndrome of the ulnar nerve at the wrist and elbow in a 55-year-old man, as well as a brief review of the literature. Although electrodiagnostic findings were consistent with an ulnar nerve lesion only at the elbow, ultrasonography revealed a ganglion compressing the ulnar nerve at the hypothenar area and the anconeus epitrochlearis muscle lying in the cubital tunnel. Careful physical examination and ultrasound assessment of the elbow and wrist confirmed the clinical diagnosis prior to surgery. [ABSTRACT FROM AUTHOR]

Facial cellulitis is defined as infections or inflammation of the skin or connective tissue in orbital, periorbital area and cheeks, and is known to be caused mainly by bacterial infections, for which treatment with proper antibiotics and incision and drainage are necessary. Candidal cellulitis is a rare disease and only two cases have been reported in the world to our knowledge. Candidal facial cellulitis is a non-haematogenous, deep-seated infection and we should figure out for known risk factors of candidal colonisation or overgrowth and possible routes of infection for candidiasis. The article describes the case of a 55 year old who visited the orthopedic department with acute right anterior right pain. Needle electromyography showed prominent abnormal spontaneous activities and reduced interference patterns of motor unit action potentials in the right iliopsoas and vastus medialis muscles and an ultrasonography revealed marked swelling of the right femoral nerve and large cystic mass below the femoral nerve. The patient died 4 months after symptom onset.

Ammonia is one of the precursor gases in the formation of particulate matter (PM) that reacts with nitrogen oxides and sulfur oxides in the atmosphere. Based on the Clean Air Policy Support System (CAPSS) of Korea, the annual ammonia emissions amounted to 261,207 tons in 2020 and the agricultural source (manure management sector) contributes the highest proportion of the ammonia inventory. However, the methodology for the study of ammonia emissions in Korea has some limitations regarding the representativeness of the sites selected and the reliability of the measurement method. In this study, we aimed to recalculate the ammonia emissions from the livestock industry in Korea using the UK’s estimation method, which uses the life cycle assessment of livestock manure. Three major animal types, i.e., cattle (beef cattle and dairy cows), pigs and chickens, and three major processes based on the manure flow, i.e., housing, manure storage and treatment and land application processes, were considered. The total ammonia emissions were estimated to be approximately 33% higher than the official ammonia emissions stated by the CAPSS. For the manure flow, the ammonia emissions were the highest from land application processes. The ammonia emissions from dairy cow and poultry manure were much higher than those stated by the CAPSS, while the emissions from beef cattle and pig manure showed similar levels. The methodology used in this study can offer an alternative approach to the ammonia emission estimation of the manure management sector in the agriculture industry of Korea. Korean emission factors based on the manure flow should be developed and applied in the future.

Stringent regulations have been implemented to address vehicle exhaust emissions and mitigate air pollution. However, the introduction of exhaust gas reduction devices, such as Three-Way Catalytic converters, has raised concerns about the generation and release of additional pollutants such as NH3. This study utilized a chassis dynamometer to investigate the characteristics of exhaust pollutants, including carbon monoxide (CO), hydrocarbons (HC), nitrogen oxides (NOx), particulate matter (PM), ammonia (NH3), organic carbon (OC), and elemental carbon (EC). The emissions were examined across various vehicle fuel types, namely liquefied petroleum gas, gasoline, and diesel (EURO4, EURO6), to assess their individual contributions to exhaust emissions. The results revealed significant variations in the emission levels of regulated pollutants (CO, HC, NOx, and PM) during driving, depending on factors such as engine technology, emissions control strategies, fuel type, and test cycle. Notably, NH3 emissions analysis according to driving mode indicated that gasoline vehicles exhibited the highest NH3 emissions, while diesel vehicles emitted negligible amounts. This observation can be attributed to the production of NH3 as a byproduct of catalytic reduction processes implemented by exhaust gas reduction devices targeting CO, HC, and NOx. In addition, EURO4 vehicles demonstrated higher emission levels of OC and EC compared with other fuel types. Furthermore, the presence of diesel particulate filters (DPFs) in diesel vehicles effectively reduced PM emissions. Moreover, this study investigated the emission characteristics of organic molecular markers within the organic carbon fraction, revealing distinct emission profiles for each vehicle and fuel type. These findings contribute to the identification of emission sources by discerning the primary components emitted by specific fuel types.

Background Many studies about the levator aponeurosis complex of the blepharoptosis have already been presented. However, the studies about the changes of the levator aponeurosis are relatively insufficient. So, this study was performed to observe histological changes of levator aponeurosis that arise depending on the severity of blepharoptosis and the age. Methods Twenty patients who have undergone surgical treatment for blepharoptosis from 2013 to 2014 were analyzed in this study. Patients were categorized mild or severe group according to the severity of blepharoptosis, and the age. Through the blepharoplasty incision, we harvested the specimens of the levator aponeurosis on the upper border of tarsal plate. After staining the specimens with the Verhoeff-van Gieson technique, the changes of elastin was analyzed in a histopathological manner. Results Light microscopy of the levator aponeurosis stained positively for elastic fibers using the Verhoeff-van Gieson technique. Elastic fibers appear to have direct connections with the collagen fiber of the levator aponeurosis. The amount of the elastin was decreased in the old age group. And the amount of elastin was decreased markedly in severe blepharoptosis group. Conclusions The elastin of the levator aponeurosis was decreased in old age and elastin tended to decreased markedly in severe levator function group. The levator aponeurosis plays a greater role in the eyelid ptosis. Therefore, knowledge about the histologic changes of the levator aponeurosis may give more help us to understand the high recurrence rate of the blepharoptosis in old age. Also, considering this information, will be helpful to the blepharoptosis surgery.

To evaluate the pathological effect of the duration of arm swelling on the shoulder pathology in patients with breast cancer-related lymphedema.Forty seven breast cancer patients with unilateral arm lymphedema were assessed. The duration of the arm swelling and shoulder pain were recorded. Ultrasound examination of the shoulder joint was performed in all patients to detect any lesions.Abnormalities were detected by ultrasound in 41/47 (87.2%) study participants. Subacromial subdeltoid bursal thickening was found in 26/47 (55.3%) participants, distension of the biceps brachii tendon sheath was found in 14/47 (29.8%) and a supraspinatus tendon tear was found in 13/47 (27.7%). Patients with a supraspinatus tendon tear were found to have a significantly longer duration of lymphedema (1310 days vs. 398 days, p = 0.032).The duration of arm lymphedema has a progressive pathological effect on rotator cuff. Clinicians should adopt an early management approach of shoulder pain in patients with breast cancer-related lymphedema.

In this study, we investigated whether gongjin-dan improves functional recovery and has neuroprotective effects on reducing the infarct volume after transient middle cerebral artery occlusion ( MCAo). Infarct volume was measured using TTC staining and glucose utilization by F-18 FDG PET. Functional improvement was evaluated with the Rota-rod, treadmill, Garcia score test, and adhesive removal test. At 14 days after MCAo, neuronal cell survival, astrocytes expansion, and apoptosis were assessed by immunohistofluorescence staining in the peri-infarct region. Also, the expression of neurotrophic factors and inflammatory cytokines such as VEGF, BDNF, Cox-2, TNF-α, IL-1β, and IL-1α was measured in ischemic hemisphere regions. The gongjin-dantreated group showed both reduced infarct volume and increased glucose utilization. Behavior tests demonstrated a significant improvement compared to the control. Also in the gongjin-dan treated group, NeuN-positive cells were increased and number of astrocytes, microglia, and apoptotic cells was significantly decreased compared with the control group in the ischemic peri-infarct area. Furthermore, the expression of VEGF and BDNF was increased and level of Cox-2, TNF-α, IL-1β, and IL-1α was decreased. These results suggest that gongjin-dan may improve functional outcome through the rapid restoration of metabolism and can be considered as a potential neuroprotective agent. [ABSTRACT FROM AUTHOR]
