206. Design and analysis of broadband high-efficiency pulse compression gratings

Abstract

We report on two effective methods, multiparameter optimization and local optimization combined with the diffraction bandwidth merit function, to design a broadband pulse compression grating (PCG), and we present broadband, high-efficiency PCGs based on both the multilayer dielectric grating (MDG) and metal-multilayer dielectric grating (MMDG) models. For MDG, the average diffraction efficiency is higher than 97.5% for TE polarization light over the 100 nm bandwidth centered at 800 nm. Moreover, a novel multilayer structure, which comprises higher index material in the high-reflectivity mirror and relatively lower index material on top, is first proposed to yield higher average efficiency, broader bandwidth, and excellent fabrication tolerance. For MMDG, it exhibits an ultra-broadband top-hat diffraction spectrum with average efficiency exceeding 97% over the 200 nm wavelength wide centered at 1053 nm. In addition, the MMDG structure, which has the best tolerance for grating fabrication, is determined by investigating characteristics of MMDGs with different thin-film structures. Abstract

Treatment of oral squamous cell carcinoma remains a challenge due to a high incidence of treatment resistance, which is followed by tumor recrudescence and metastasis to the lymph nodes. Thus, it is important to explore novel inhibitors of OSCC. Here, we aimed to identify drugs that may cooperate with histone deacetylase inhibitors to reverse the EMT, inhibit EMT and cell migration and invasion, and contribute to therapeutic efficacy. We found that treatment with 4sc‐202 potently reversed the EMT and thereby inhibited cell migration and invasion in vitro, in part by inducing expression of the FoxO1 tumor‐suppressor gene. Furthermore, 4sc‐202 also synergized with Ink‐128 to inhibit tumor migration and invasion in vitro. Mechanistically, 4sc‐202 induced FoxO1 expression, whereas Ink‐128 promoted nuclear translocation of FoxO1. Our findings indicated that FoxO1 might reverse the EMT by interacting with Twist1 in OSCC. In conclusion, we identified an effective combination therapy involving class I histone deacetylase and mammalian target of rapamycin complex 1/2 inhibition that effectively blocked the EMT of tumor cells by upregulating FoxO1 expression to inhibit Twist1 transcription. 213. Evaluation of Medications for Opioid Use Disorder on Overdose and Healthcare Utilization in the Us

Abstract

The United States is facing an unprecedented crisis of drug overdose and poisoning deaths. In this dissertation, we studied three different aspects of opioid use disorder and health-related outcomes using routinely collected data. Manuscript 1: We designed a cross-sectional study to evaluate the association between early buprenorphine-naloxone initiation (within 90 days after the diagnosis of opioid use disorder) and health-related outcomes among insured adults with opioid use disorder in the United States from 2010 to 2018. Generalized linear models with multiple imputation to account for missing data were applied to estimate the association and adjust for self-reported socioeconomic status, comorbidities, and concomitant medications. We found that timely buprenorphine-naloxone initiation was associated with a reduced occurrence of opioid overdose and a statistically significant reduction in opioid overdose-related emergency department visits and costs. Manuscript 2: We designed a retrospective study to evaluate the association between time-varying buprenorphine-naloxone prescribing and subsequent opioid overdose or death among insured adults with opioid use disorder in the United States from 2010 to 2017. We fit a marginal structural model using inverse probability weights to account for measured baseline and time-varying confounders, as well as selection bias due to possibly informative lost-to-follow-up. We found that continuously receiving buprenorphine-naloxone was associated with reduced risk of opioid overdose and all-cause mortality. 219. Selective Multi-Grade Charging Scheduling and Rebalancing for One-Way Car-Sharing Systems

Abstract

This paper investigates the rebalancing and charging scheduling problem of one-way car-sharing systems. Vehicles can be charged either at the stations within the system or at the public charging stations. A framework for selective multi-grade charging scheduling and fleet rebalancing is proposed. To this end, vehicles and customers are classified into different grades for scheduling and matching, according to their state of charge and travel distance, respectively. The number of grades is involved as a decision variable in the combinatorial optimization problem, with the objective of minimizing the maximum response time and the operating cost for both rebalancing and charging. A lower bound optimal solution of the problem is given analytically based on the Lagrangian analysis and the Karush-Kuhn-Tucker (KKT) condition, yielding a joint optimization algorithm that can ensure the quality of service and balanced demand-supply in the system. Simulation results show that the method is effective in reducing system response time, improving vehicle utilization and reducing reliance on vehicle charging.

Abstract

Two issues related to the application of Chapman-Jouguet (C-J) theory have been investigated in this study. Firstly, the solution of the two-γ analytical model cannot agree with the exact solution calculated from the Chemical Equilibrium Compositions and Applications (CEA) code even all the required parameters are substituted with the exact ones. To root the source, the original derivation has been reviewed. It is believed that the perfect gas approximation induces the discrepancy, which will result in a huge discrepancy in the heat release at C-J state. Hence, a four-γ model for ideal gases has been derived, whose results are consistent with those from the CEA code though it requires more input parameters. Sensitive analysis implies that the accuracy of the four-γ model is greatly impacted by γ 2 ‾ and q C J . Besides, a parabola function that can describe the heat release and the lower heating value correlation has been proposed. Using this function, about ±5% relative error of estimated M S can be achieved when γ 2 ‾ is equal to 9/7 for most selected stoichiometric gas mixtures. Secondly, the energy distribution in a C-J detonation process is found quite different when attaching the reference frame to the ground or the detonation wave front. Hence, regarding the C-J state as the equivalent burned state at the exit of a detonation combustor is questionable. Accordingly, a four-step thermodynamic process for a closed system has been developed, which can quantitatively analyze the energy balance process of a C-J detonation. Discussion is made by comparing the process with a traditional steady thermodynamic process as well as constant-volume combustion.

Abstract

In order to reduce the energy consumption and membrane fouling of the conventional membrane bioreactor (MBR), a kind of low energy consumption vortex wave flow MBR was exploited based on the combination of biofilm process and membrane filtration process, as well as the vortex wave flow technique. The experimental results showed that the vortex wave flow state in the membrane module could be formed when the Reynolds number (Re) of liquid was adjusted between 450 and 1,050, and the membrane flux declined more slowly in the vortex wave flow state than those in the laminar flow state and turbulent flow state. The MBR system was used to treat domestic wastewater under the condition of vortex wave flow state for 30 days. The results showed that the removal efficiency for CODcr and NH3-N was 82% and 98% respectively, and the permeate quality met the requirement of ‘Water quality standard for urban miscellaneous water consumption (GB/T 18920-2002)’. Analysis of the energy consumption of the MBR showed that the average energy consumption was 1.90 ± 0.55 kWh/m3 (permeate), which was only two thirds of conventional MBR energy consumption.

249. Frailty in patients undergoing percutaneous left atrial appendage closure.

Abstract

Background: Frailty is associated with significant morbidity and mortality in older adults. Whether frailty predicts adverse outcomes after percutaneous left atrial appendage closure (LAAC) remains uncertain.Objective: The purpose of this study was to examine the association between frailty and clinical outcomes after percutaneous LAAC.Methods: We identified patients 65 years and older in Medicare fee-for-service claims who underwent LAAC between October 1, 2016, and December 31, 2019. Patients were identified as frail on the basis of the Hospital Frailty Risk Score (HFRS), a validated frailty measure centered on health resource utilization, with the cohort stratified into low (<5), intermediate (5-15), and high (>15) risk groups.Results: Of the 21,787 patients who underwent LAAC, 10,740 (49.3%) were considered frail (HFRS >5), including 3441 (15.8%) in the high-risk group. The mortality rate (up to 1095 days) were 16.1% in the low-risk group, 26.7% in the intermediate-risk group, and 41.1% in the high-risk group (P < .001). After adjusting for age, sex, and comorbidities, HFRS >15 (compared with HFRS <5) was associated with a higher risk of long hospital stay (odds ratio [OR] 8.29; 95% confidence interval [CI] 5.94-11.57), 30-day readmission (OR 1.80, 95% CI 1.58-2.05), 30-day mortality (OR 5.68, 95% CI 3.40-9.40), and 1-year mortality (OR 2.83, 95% CI 2.39-3.35). In restricted cubic spline models, the adjusted OR for all outcomes monotonically increased with increasing HFRS.Conclusion: Frailty is common in patients undergoing LAAC and is associated with increased risks of long hospital stay, readmissions, and short-term mortality. Abstract

Glioblastoma (GBM) is the most common aggressive malignant tumor in brain neuroepithelial tumors and remains incurable. A variety of treatment options are currently being explored to improve patient survival, including small molecule inhibitors, viral therapies, cancer vaccines, and monoclonal antibodies. Among them, the unique advantages of small molecule inhibitors have made them a focus of attention in the drug discovery of glioblastoma. Currently, the most used chemotherapeutic agents are small molecule inhibitors that target key dysregulated signaling pathways in glioblastoma, including receptor tyrosine kinase, PI3K/AKT/mTOR pathway, DNA damage response, TP53 and cell cycle inhibitors. This review analyzes the therapeutic benefit and clinical development of novel small molecule inhibitors discovered as promising anti-glioblastoma agents by the related targets of these major pathways. Meanwhile, the recent advances in temozolomide resistance and drug combination are also reviewed. In the last part, due to the constant clinical failure of targeted therapies, this paper reviewed the research progress of other therapeutic methods for glioblastoma, to provide patients and readers with a more comprehensive understanding of the treatment landscape of glioblastoma.
