There is a well-documented interest in how insights from the study of complexity can be applied to policy evaluation. However, important questions remain as to how complexity is understood and used by policy evaluators. We present findings from semi-structured interviews with 30 UK policy evaluators working in food, energy, water and environment policy domains. We explore how they understand, use and approach complexity, and consider the implications for evaluation research and practice. Findings reveal understandings of complexity arising from contextual factors, scale-related issues and perceptions of unpredictability. The evidence indicates terminological and analogical use of complexity and its concepts by policy evaluators, but limited evidence of its literal use. Priorities for the future include framing complexity more pragmatically and as an opportunity not a cost. Communicating this up the policy hierarchy is the key to progressing complexity-appropriate evaluation – this can be enabled by strengthening links between policy evaluation and academic communities. The literature on policy design provides insightful resources to improve policy evaluation. We compare how this literature addresses causation, evaluation, instrumentation, and intervention, based on different methodologies. Then, we explain how the realist approach can contribute to theory-based evaluation with multi-methods. We go through the process leading from the adoption of policy aims to the production of a policy outcome by a causal mechanism linking agenda setting to policy formulation, cross-sectorial coordination, political interplays, and this policy's outcome. This hypothesis is then tested against a typology of policy instruments classified by the State resources. These instruments are hereby treated as expected empirical observations that provide an empirical basis to assess the theoretical process following a Bayesian logic. We eventually present a five-step protocol aimed at making realist policy evaluation easier, which includes process theorization and operationalization, empirical tests design, case selection and justification, congruence analysis, and deep within-case study. What explains the changing policymaking process for abortion law in Mexico over the past century? From the 1930s until the 1970s, abortion laws across the thirty-two federal entities in Mexico were nearly identical. Between 1979 and 1987, almost half of the states liberalized abortion laws, but very little attention was paid to the reforms. Abortion suddenly emerged as a central polarizing conflict in the aftermath of the 2000 presidential elections that ushered in multiparty democracy. This article provides a comparative analysis of criminal codes and abortion law through time and across the Mexican states. It builds on the theoretical insights of U.S. public policy research on federalism and Mexican research on policymaking to present a comparative framework for understanding policymaking in federal systems. This article argues that Simeon's insistence on the value of explicit comparison within individual studies of public policy needs to remain central even in historical institutionalist approaches which “take time seriously” and focus on causal mechanisms—a methodological injunction sometimes seen to augur in favour of single-case and single-outcome studies. However, if Simeon's suggested approach is to reflect the major advances that have occurred since he wrote, it will require more fully and more explicitly combining the power of the comparative method with the powerful insights generated by a logic of intertemporal causal mechanisms unfolding over time. The adoption ofImihigoas a new tool for public policy implementation emerged from the persistent observation that the existing bureaucratic system continued to react ‘in slow motion’ while the improvements promised by the government required more responsiveness. Although originally, its initiators conceived this new system as a home-grown solution, based on empirical findings, this article shows that it evolved in the form of a hybrid model relying on two opposite mechanisms to increase public service delivery: the spirit of emulation of local leaders rooted in tradition and the external control of performance rooted in the new public management philosophy. Information is very essential in any organization especially in a government based environment where information are needed for policy making. However, some of the instruments used for getting information are not well managed by the civil servants who are the custodian of this research information. This study investigated the attitude of policymakers in terms of the types, availability and accessibility of research information in making policy. Focusing particularly on the types of research information needed by senior civil servants in making policies, frequency of use of research information source, accessibility of information for decision making and the challenges encountered in the course of using them. The descriptive research method was adopted, using a pre-tested self-administered questionnaire to gather information from 166 senior civil servants randomly selected from selected ministries in Lagos state, as data collection instrument. Results indicated that information provided by ones ministry, information on project implemented by ones ministry, information on infrastructural development and information on human resources and skills available at ones ministry are the most used information by policymakers in making policy. The study also showed that internal files, colleagues or superior, newspaper, government publication and internet are the most frequently used and accessible information sources for policymaking. Impracticability of research recommendation politically and financially, too much information to absorb and contradictions in information from different researchers are the major constraints to research output. We examine how an executive's consultations with interest groups during the formative stage of the policy process affect its bargaining success during the decision-making stage after it has proposed new policies to legislative actors. Our theory sets out how consultations with interest groups strengthen the executive by bolstering its formal and informal agenda-setting power. The empirical testing ground for our theory is the European Union ( EU), and in particular the consultations held by the European Commission. The analysis assesses the effects of these consultations on the congruence between the Commission's legislative proposals on controversial issues and EU laws. Our analysis incorporates detailed information on the type and scope of each consultation. In line with our theory, we find that the Commission had more success during the decision-making stage after conducting open consultations with large numbers of interest groups during the policy formation stage. The concept of wicked problems has been increasingly recognized in policy studies over the last decade. However, 40 years after the concept was introduced, the bulk of the available research still seems to follow the same approach: Issues are identified as being wicked problems, and rather similar models are theorized to address them. We argue that the research on wicked problems would benefit from a stronger empirical slant; the current research adopts just such an empirical approach in focusing on the role of citizens in tackling wicked policy issues. More specifically, the mechanisms of deliberative democracy are analyzed. This is important because wicked policy issues are commonly associated with fragmentation and incoherence. Deliberative mechanisms are then thought to lead toward public judgment, a form of shared understanding where citizens strive to understand the complexity of the issue and, working together in deliberation, seek the best ways to address it. Drawing on the outcomes of four deliberative forums on euthanasia conducted in Finland in November 2013, the current research analyzes whether the deliberation process helped the participants to progress on the public's learning curve and whether it was ultimately likely to foster authentic public judgment on a particular wicked policy issue. Policy forums are issue-based intermediary organizations where diverse types of political and societal actors repeatedly interact. Policy forums are important elements of modern governance systems as they allow actors to learn, negotiate, or build trust. They can vary in composition, size, membership logic, and other distinct features. This article lays the foundation of a theory of policy forums based on three interrelated elements: First, it discusses conditions for the formation of a forum and describes the logic of these organizations as one of an asymmetric multipartite exchange. Second, it enumerates the potential set of goals and motivations of participating actors that are fed into this exchange. Third, it proposes eight different dimensions on which policy forums differ and which affect the exchange mechanisms among actors. This paper is interested in the interdisciplinary characteristics of European integration studies. It explores how the institutional and intellectual, internal and external boundaries of this interdisciplinary field are shaped. For this purpose, it discusses two interlocking dynamics that are most important: on the one hand, the European Union actively attempts to mobilise European integration studies to contribute to building a united Europe by providing specific spaces, resources, and infrastructures for academic research and the public dissemination of results. On the other hand, the external and internal boundaries of this interdisciplinary field are shaped by the movements and interactions of scholars from different academic backgrounds. Together, the different academic strands and policies coproduce the interdisciplinary field of European integration studies. Suggesting avenues for future research, the conclusion recommends analysing key debates in the context of European integration in terms of boundary objects around which academics, policy-makers, and publics can engage with each other and points to specific sites for further observations of these processes of coproduction. The claim that randomized controlled trials (RCTs) can improve policy-making in developing countries seemed to be severely challenged by the widespread belief that RCTs lack external validity. This belief led to the articulation of strategies to alleviate this problem which developed under the assumption that policy-making in developing countries can be best understood as a problem of prediction. The causal structures that characterize developing contexts, however, render this framing inadequate. I propose to understand policy-making in developing contexts as a planning process. In this new framework, the relevance of several kinds of evidence is highlighted and the role of causal effects refocused. In this article, it is shown that citizen process preferences are complex and include several dimensions. The argument relies on data from a representative sample of Spanish citizens (N = 2,450) to assess these dimensions. Using confirmatory factor analysis as well as Mokken analysis, it is shown that citizen process preferences capture support for three different models: participatory, representative and expert-based. The relationships between these dimensions (where the opposition between representation and participation stands as the clearest result) and the substantive and methodological implications of these findings are discussed. In response to challenges brought by population ageing, Hong Kong has adopted an active ageing policy framework and built age-friendly neighbourhoods to encourage the participation of older people. This article distinguishes between the civic and political participation and emphasizes active involvement in community and neighbourhood activities as a way to encourage the civic engagement of older adults. The promotion of age-friendliness is set at two levels: concerning policy and practical/operational issues, both of which are interrelated with the purpose of offering civic engagement opportunities for older people. The experiences of Hong Kong reflect both top-down and bottom-up approaches, where civic participation is promoted by encouraging life-long learning and developing caring communities, as well as carrying out a variety of neighourhood programmes. It is suggested that different stakeholders, such as older adults, NGOs and other social organizations, shall all be included and play important roles in promoting civic participation of the elderly through creating a conducive environment. The initiatives, whether these are in policy or at operational levels, should be integrated into actions that are aimed to enhance quality of life for all residents and create a more age-friendly neighbourhood. Public health is usually enacted through public policies, necessitating that the public engage in debates that, ideally, are grounded in solid scientific findings. Mistrust in science, however, has compromised the possibility of deriving sound policy from such debates, partially owing to justified concerns regarding undue interference and even outright manipulation by commercial interests. This situation has generated problematic impasses, one of which is the emergence of an anti-vaccination movement that is already affecting public health, with a resurgence in the United States of preventable diseases thought to have been eradicated. Drawing on British sociologist Harry Collins’ work on expertise, we propose a theoretical framework in which the paralyzing, undue public distrust of science can be analyzed and, it is hoped, overcome. The Education and Training Guidelines: A Taxonomy for Education and Training in Professional Psychology Health Service Specialties was endorsed as a policy of the American Psychological Association in 2012. These Guidelines have the potential for broad impact on the field by providing both a structure and recommendations for the consistent usage o f language— definitions and terminology—to reduce current descriptive inconsistencies across education and training programs in professional psychology. The Guidelines are not designed to define specifics of the training or practice of individual psychologists; they are to be used only to describe programmatic structure in a consistent manner. This article details the developmental history o f these Guidelines and highlights the strong alliance between the leaders of the various recognized specialties in professional psychology and the education and training community in health service psychology. This paper focuses on problem frame differences among actors (members of an advisory body, senior administrators and clinical unit managers) who are concerned with the introduction of new health technology at the regional level in Sweden. It explores issues related to problem framing, puzzling, powering, participation and the various rationales articulated in the ambiguous search for an evidence-based strategy to handle the influx of new technologies. The Health Technology Advisory Committee (HTAC) was established in one Swedish county council in 2004 with the intention of controlling both the introduction of health technology and supporting policy decision and clinical practice by promoting the use of best evidence. The HTAC followed a scientific rationality dominated by one problem frame, although the problematic situation, as it was framed by all the actor groups, was highly complex and not solely a matter of evidence. This paper illustrates how problem frame differences shape the puzzling of a policy problem and how the different distinguishable policy styles are dependent on who is participating and who is not participating in the puzzling. In recent years, there has been remarkable progress in our understanding of policy persistence, on the one hand, and of the psychological phenomenon of underreaction, on the other. Surprisingly, there has been no attempt to use robust findings, derived from these efforts, in order to understand the nuances of policy underreaction. Policy underreaction refers to systematically slow and/or insufficient response by policymakers to increased risk or opportunity, or no response at all. This article tries to give the concept of policy underreaction a robust analytical identity by integrating cognitive, social, psychological and emotional variables in the explanation of policy underreaction and by introducing a variation across different types of contextual sources of policy persistence as explanatory variables of this phenomenon. It develops an analytical framework that revolves around two key elements of decision making in situations of risk unfolding over time: (1) policymakers' underestimation and accurate estimation of increased risks and (2) intra- and extra-organizational sources of policy persistence. Since 2012, a new movement of government departments, think tanks and high-profile individuals within the UK has sought to promote the increased usage of randomised controlled trials (RCTs) in public policy. They promote RCTs as an evidence-based corrective for the inappropriate certainties of experts. Recent government reports and public debate around this initiative are reviewed and analysed within a framework for epistemic governance: normative insights into how knowledge for policymaking should be understood and governed drawn from science and technology studies and the policy sciences. The legitimacy of RCT evidence within policymaking is found to rest on the recognition of three key features: (1) how multiple meanings of evidence limit generalisability, (2) ensuring a plurality of evidence inputs, including those from other forms of research and expertise, and (3) building institutions for governing the use of RCTs in the public interest. Producing evidence for policymaking is a hybrid activity that necessarily spans both science and politics. Presenting RCTs as naively neutral evidence of what policy interventions work is misleading. The paper concludes by calling for more work on how the new RCT movement might engage with its own history in social and policy research on the value of experiments for policymaking. This study investigates the factors that underlie large variation in Medicaid and Children's Health Insurance Program (CHIP) policies among states. Both eligibility and provider payment policies are examined for low-income children and parents. I find that state variation in the cost of providing health care, due to variation in the intensity of health care use, is a key determinant of eligibility policies, and I also find tentative evidence of an effect for payment policies. Because rising health care spending increases the cost of providing health insurance coverage, state policy makers in high-spending states enact less generous Medicaid and CHIP policies. Results also indicate that the political environments of states are very important in determining their eligibility policies, but fewer political variables influence payment policies. In addition to including variables not yet examined in the context of Medicaid policy, this study uses an innovative measure of state-level health care spending and carefully constructed dependent variables that lend credibility to causal interpretations of relationships. Grand challenges stress the importance of multi-disciplinary research, a multi-actor approach in examining the current state of affairs and exploring possible solutions, multi-level governance and policy coordination across geographical boundaries and policy areas, and a policy environment for enabling change both in science and technology and in society. The special nature of grand challenges poses certain needs in evaluation practice: (a) the need for learning at the operational, policy and, especially, system level; and (b) the importance of a wider set of impacts and behavioural change. The examination of the usefulness of evaluations as learning tools thus becomes relevant as does the way current evaluation practices address broader impacts and issues such as behavioural additionality. The suitability of existing evaluation contexts in meeting the specific issues posed by the 'grand challenges' orientation is also worth examining. The objective of this article is to study agenda setting during the formulation of the Itajaí River Basin Plan by the Comitê Itajaí (Itajaí Committee). In methodological terms, it consists of a qualitative, documentary and field case study, collecting data through documentary research and interview. The studied case was that of the decision to charge for the use of water resources, to which we applied Kingdon's multiple streams model (2003) in order to understand the dynamics of the involved agents' actions in which the combination of the problem, political and policy streams led to the opening of a policy window. Hence, water resource users, acting as policy entrepreneurs, made use of the policy window to modify the formula for charging originally proposed by the Committee's board of directors, thus ensuring a more appropriate instrument for their interests. The article presents a response to the article "Bridging the Relevance Gap in Political Science" by Matthew Wood, focusing on the impact of political science on policy and governance. Topics include the impact of social and disciplinary relevance on the selection of a research topic; the need for quantitative versus qualitative analysis; and the difficulty of applying conventional methodologies to large scale policy issues.

This article argues that scholars overestimate the ability of a methodologically 'pluralistic' political science to gain impact in policymaking, which in Britain is increasingly 'positivist', privileging quantitative evidence. This 'relevance gap' between pluralistic political science and positivistic policymaking means that political scientists are disadvantaged in achieving 'impact' compared to disciplines like behavioural economics. The article proposes two solutions to bridge this 'gap': improving the 'accessibility' of research and stimulating 'interaction' between researchers and policymakers through methodological workshops. The extensive estimates of party policy preferences produced by the CMP (Comparative Manifesto Project, now MARPOR) have proved robust and valid over a wide variety of research applications. But all estimates carry some error. We demonstrate that one of the two existing assessments of non-systematic error in the CMP data strongly overstates their reliability while the other understates it, leaving much potential for mis-estimation. We develop a new method which extends classical test theory and directly estimates overall data reliability; reliabilities and standard errors of measurement for each party system; and standard errors of measurement for each data-point. The energy of patients and members of the public worldwide who care about improving health is a huge, but still largely unrecognized and untapped, resource. The aim of patient engagement is to shift the clinical paradigm from determining “what is the matter?” to discovering “what matters to you?” This article presents four case studies from around the world that highlight the proven and potential abilities of increased patient engagement to improve health outcomes and reduce costs, while extending the reach of treatment and diagnostic programs into the community. The cases are an online mental health community in the United Kingdom, a genetic screening program in the United Arab Emirates, a World Health Organization checklist for new mothers, and a hospital-based patient engagement initiative in the United States. Evidence from these and similar endeavors suggests that closer collaboration on the part of patients, families, health care providers, health care systems, and policy makers at multiple levels could help diverse nations provide more effective and population-appropriate health care with fewer resources. Recent federal initiatives have used estimates of population reach as a proxymetric for intervention impact, in part to inform resource allocation and programmatic decisions about competing priorities in the community. However, in spite of its utility, population reach as a singular metric of intervention impact may be insufficient for guiding multifaceted program decisions. State Medicaid programs and other state health agencies need to monitor and evaluate changes in health insurance coverage, access to care, financing, and the quality of health care delivery. The availability of new financial resources through the Patient Protection and Affordable Care Act is accompanied by raised expectations for such accountability. While state agencies often contract with universities on an ad hoc basis for specific policy projects, fourteen states have established formal state-university partnerships so that their analytic and technical needs can be addressed more readily. After a brief overview of these partnerships, this article provides examples of their projects, which most often affect Medicaid policy, including work on program eligibility, provider payments, and optional benefits. State-university partnerships are working on policy-relevant projects that influence decision making. Like the variation in Medicaid programs across the country, no two partnerships are alike. They thrive in a mix of structures, using different means of contracting, and with varied degrees of data access. All partnerships are interested in building a national network to share innovative practices and projects, spawn comparative policy studies across states, and support the development of new state-university partnerships. The role of the US federal government in developing tobacco control through a cooperative, interactive program with state and local private and public organizations has been underestimated. This article investigates how the government initiated and sustained a program of "capacity building" through the scientific authority of the National Cancer Institute, beginning in the 1980s. There are several major questions to be answered: ( 1 ) How did this program manage to be adopted and sustained despite the well-documented hindrances to effective tobacco control policy at the federal level ? (2) How did a tobacco control policy program become incorporated into the scientific research agenda of the National Cancer Institute? (3) How have science, social factors, and government at various levels interacted in this capacity-building program? The study emphasizes how the US federal government, blocked by a tobacco-friendly Congress from enacting effective tobacco control legislation, utilized its scientific research role and, with the cooperation of other levels of government and large, private antitobacco organizations, established an ongoing policy effort. In our study, definition of gender equality, which is a significant topic for international organizations such as UN and EU, in legal and terminal frame has made. After given the general information about topic, reflection of social gender equality in Turkey has been analyzed in case of metropolitan municipality. Objectives. To operationalise the concept of 'advanced practice roles' in pharmacy within the new integrated regionalised palliative care service model outlined in the Palliative Care Services Plan 2009-2016, SA Health. Methods. A working group was established under the auspices of the Palliative Care Clinical Network to progress the development of advanced practice pharmacist roles for regionalised palliative care services. A pharmacy stakeholder forum was conducted in December 2010 to provide further guidance on the advanced practice pharmacist roles in the following domains: education; network links and partnerships; quality and safety; and research. Results. Advanced practice pharmacist positions were created for each of the three regionalised palliative care services in South Australia (SA). Funding was obtained for a Statewide Palliative Care Pharmacy Network project, to build a sustainable community-based palliative care pharmacy network. Advanced practice pharmacists commenced in the regionalised palliative care services of SA on 4 October 2011. Conclusions. The Statewide Palliative Care Clinical Network and the SA Palliative Care Plan provided a policy framework that supported involvement and advocacy in the planning of the advanced practice pharmacist roles. Collaboration between leaders in workforce reform, service planners, specialist palliative care providers and the pharmacy sector was a key enabler for developing the advanced practice pharmacist positions for regionalised palliative care services. Starting in 2014, the Affordable Care Act (ACA) will require private insurance plans sold in the individual and small-group markets to cover a roster of ''essential health benefits.'' Precisely which benefits should count as essential, however, was left to the discretion of the Department of Health and Human Services (HHS). The matter was both important and controversial. Nonetheless, HHS announced its policy by posting on the Internet a thirteen-page bulletin stating that it would allow each state to define essential benefits for itself. On both substance and procedure, the move was surprising. The state-by-state approach departed from the uniform, federal standard that the ACA appears to anticipate and that informed observers expected HHS to adopt. And announcing the policy through an Internet bulletin appeared to allow HHS to sidestep traditional administrative procedures, including notice and comment, immediate review in the courts, and White House oversight. This article explores two questions. First, is the state-by-state approach a lawful exercise of HHS's authority? The object of policy research is the understanding of the interaction among the machinery of the state, political actors, and the public. To facilitate this understanding, a number of complementary theories have developed in the course of more than two decades. This article reviews recent scholarship on the established theories of the policy process, mostly published in 2011 and 2012. Additionally, scholarship extending these theories is identified and new theories of policy process are discussed. This review finds that the established theories have generated substantive scholarship during the period under review and have also been the springboard for much of the recent thinking in policy research. This paper shows that change in party systems within parliaments can lead to major change in policy outcomes. Specifically, we show that policy mobility of parties and fluidity in their parliamentary membership can generate or upset the existence of the policy core as well as determine its location. Our analysis applies to the general case of a multiparty parliament. We then follow up with empirical illustrations that conform to the major types of theoretical dynamics in our analysis, where party change that occurred in parliaments was consistent with attempts to manipulate the policy core. Who determines what ideas matter in reform debates? This contribution examines reform proposals from the expert groups that emerged during the recent international financial crisis. To do so it follows fractal distinctions – distinctions that replicate themselves in subsequent iterations – among the reports. Fractal distinctions, such as between ‘behaviour’ or ‘system’ as a reform focus, allow us to locate the object of regulation within expert groups, the experts' professional context and the politics behind the commissioning of work. Analysing fractal distinctions provides a useful way to understand the different stresses in reports with and without clear mandates, and the role of important members of the policy community in promoting particular reform ideas. The contribution finds that differences in ideas emerging from the financial reform expert groups reflect nested power relationships in the commissioning of work, constituent audiences and reform priorities among governing institutions, rather than distinct ‘European’ and ‘American’ ideas. A review of the article "Reassessing the concept of policy paradigm: aligning ontology and methodology in policy studies" by Pierre-Marc Daigneault, which is in this issue, is presented.

The concept of policy paradigm, which was developed by Peter Hall in 1993, has given rise to an abundance of literature on ideas and policy dynamics. Despite its theoretical value, however, the original concept was insufficiently specified and has been used in problematic ways by many policy scholars, thereby undermining the validity of their descriptive and causal inferences. The main argument defended in this article is that scholars studying policy paradigms should devote as much attention to the ideas of policy makers as to the policies they adopt, while distinguishing between the two constructs as much as possible. This article reviews these issues and puts forward seven guidelines that aim to better circumscribe the concept of policy paradigm and improve research on ideas and policy dynamics. Public health agencies face difficult decisions when allocating scarce resources to control the spread of HIV/AIDS. Decisions are often made with few local empirical data. We demonstrated the use of the robust decision making approach in Los Angeles County, an approach that is data driven and allows decision makers to compare the performance of various intervention strategies across thousands of simulated future scenarios. We found that the prevailing strategy of emphasizing behavioral risk reduction interventions was unlikely to achieve the policy goals of the national HIV/AIDS strategy. Of the alternative strategies we examined, those that invested most heavily in interventions to initiate antiretroviral treatment and support treatment adherence were the most likely to achieve policy objectives. On the occasion of the Review of International Political Economy's 20th anniversary, this paper systematically assesses RIPE's claim to represent an alternative to the ‘mainstream’ study of international political economy (IPE) with several new sources of evidence. The first is the IPE component of a 20-country survey of international relations (IR) faculty, the second a database of books in the field. The third, and most important, is derived from coding 326 RIPE articles published 2000–10 to discover key cleavages and trends. These results are compared with those from prior studies of the 12 IR journals identified as the ‘leading’ journals by the Teaching, Research and International Politics (TRIP) project. The article concentrates on five key issues: paradigmatic orientation, epistemology, methodology, policy orientation, and demography. The results provide ground for scepticism that the ‘American School’ of IPE does or will define the mainstream. Context Reducing HIV incidence in the United States and improving health outcomes for people living with HIV hinge on improving access to highly effective treatment and overcoming barriers to continuous treatment. Using laboratory tests routinely reported for HIV surveillance to monitor individuals' receipt of HIV care and contacting them to facilitate optimal care could help achieve these objectives. Historically, surveillance-based public health intervention with individuals for HIV control has been controversial because of concerns that risks to privacy and autonomy could outweigh benefits. But with the availability of lifesaving, transmission-interrupting treatment for HIV infection, some health departments have begun surveillance-based outreach to facilitate HIV medical care. Methods Guided by ethics frameworks, we explored the ethical arguments for changing the uses of HIV surveillance data. To identify ethical, procedural, and strategic considerations, we reviewed the activities of health departments that are using HIV surveillance data to contact persons identified as needing assistance with initiating or returning to care. Findings Although privacy concerns surrounding the uses of HIV surveillance data still exist, there are ethical concerns associated with not using HIV surveillance to maximize the benefits from HIV medical care and treatment. Early efforts to use surveillance data to facilitate optimal HIV medical care illustrate how the ethical burdens may vary depending on the local context and the specifics of implementation. Health departments laid the foundation for these activities by engaging stakeholders to gain their trust in sharing sensitive information; establishing or strengthening legal, policy and governance infrastructure; and developing communication and follow-up protocols that protect privacy. Conclusions We describe a shift toward using HIV surveillance to facilitate optimal HIV care. Health departments should review the considerations outlined before implementing new uses of HIV surveillance data, and they should commit to an ongoing review of activities with the objective of balancing beneficence, respect for persons, and justice. Context Evidence briefs have emerged as a promising approach to synthesizing the best available research evidence for health system policymakers and stakeholders. An evidence brief may draw on systematic reviews and many other types of policy-relevant information, including local data and studies, to describe a problem, options for addressing it, and key implementation considerations. We conducted a systematic review to examine the ways in which context- and issue-related factors influence the perceived usefulness of evidence briefs among their intended users. Methods We used a critical interpretive synthesis approach to review both empirical and nonempirical literature and to develop a model that explains how context and issues influence policymakers' and stakeholders' views of the utility of evidence briefs prepared for priority policy issues. We used a 'compass' question to create a detailed search strategy and conducted electronic searches in CINAHL, EMBASE, HealthSTAR, IPSA, MEDLINE, OAIster (gray literature), ProQuest A&I Theses, ProQuest (Sociological Abstracts, Applied Social Sciences Index and Abstracts, Worldwide Political Science Abstracts, International Bibliography of Social Sciences, PAIS, Political Science), PsychInfo, Web of Science, and WilsonWeb (Social Science Abstracts). Finally, we used a grounded and interpretive analytic approach to synthesize the results. Findings Of the 4,461 papers retrieved, 3,908 were excluded and 553 were assessed for 'relevance,' with 137 included in the initial sample of papers to be analyzed and an additional 23 purposively sampled to fill conceptual gaps. Several themes emerged: (1) many established types of 'evidence' are viewed as useful content in an evidence brief, along with several promising formatting features; (2) contextual factors, particularly the institutions, interests, and values of a given context, can influence views of evidence briefs; (3) whether an issue is polarizing and whether it is salient (or not) and familiar (or not) to actors in the policy arena can influence views of evidence briefs prepared for that issue; (4) influential factors can emerge in several ways (as context driven, issue driven, or a result of issue-context resonance); (5) these factors work through two primary pathways, affecting either the users or the producers of briefs; and (6) these factors influence views of evidence briefs through a variety of mechanisms. Conclusions Those persons funding and preparing evidence briefs need to consider a variety of context- and issue-related factors when deciding how to make them most useful in policymaking. Based on the research papers within this special issue, this overview discusses similarities and differences in stakeholding in drug user opioid substitution treatment policy in Britain, Denmark, Italy, Austria, Poland, and Finland. It explores factors that have influenced stakeholder activity, including the importance of crisis, the impact of evidence, the availability of resources, the wider political context, the influence of moral frameworks and ideologies, and the pressure of external influences. The paper highlights the important differences in the emergence and evolution of stakeholder groups and in the political, cultural, and economic circumstances, which both constrain and enable their activities. Based on documentary analyses and interviews with twenty key informants in 2012, this paper analyses the shift in British drugs policy towards 'recovery' from the perspectives of major stakeholders. The processes involved in reopening the debate surrounding the role of substitution treatment and its re-emergence on to the policy agenda are examined. Drawing on Kingdon's work on agenda-setting, the ways in which methadone maintenance was challenged and defended by key stakeholders in the initial phase of policy development and the negotiation of a 'recovery' focus as the organizing concept for British drugs policy are explored. This article discusses how opioid substitution treatment policy has developed from 2000 to 2011 in Denmark. Empirically, it takes its point of departure in a stakeholder analysis including 17 qualitative interviews with stakeholders who have played important roles in this field. Analytically, it is inspired by Kingdon's concepts of agenda and policy window. Three major shifts are identified: a shift from psychosocial to medical thinking and practice, from an abstinence driven ideology to health care, and from perceptions of passive clients to user involvement. The article suggests that we can develop a better understanding of the dynamic of governing in social policies through the lens of ‘intimacy’. Intimacy – a deep personal knowledge about body and mind which changes during the dying process – encompasses validation of one’s emotional experience. In that way, intimacy links the emotional content of dying to discursive dimensions of dignity usually associated with end-of-life care. The analysis describes how the Czech organization Homecoming has constructed its concept of end-of-life care on intimacy, and in so doing, steers specific meanings in the policy field. Hence, intimacy suggests paying more attention to modes of ‘sharing a meaning’, through which policies come to be. The article mediates between psychosocial theorizing of emotions, poststructuralist policy analysis, and current debates on governing in health care and social policies in order to uncover the role of intimacy in the dynamics of governing. Systematic reviews have the potential to promote knowledge exchange between researchers and decision-makers. Review planning requires engagement with evidence users to ensure preparation of relevant reviews, and well-conducted reviews should provide accessible and reliable synthesis to support decision-making. Yet, systematic reviews are not routinely referred to by decision-makers, and innovative approaches to improve the utility of reviews is needed. Evidence synthesis for healthy public policy is typically complex and methodologically challenging. Although not lessening the value of reviews, these challenges can be overwhelming and threaten their utility. Using the interrelated principles of relevance, rigor, and readability, and in light of available resources, this article considers how utility of evidence synthesis for healthy public policy might be improved.
