Objective To explore and analyze the application value of patient demand-oriented refined nursing intervention in the perioperative period of interventional surgery for lower limb deep vein thrombosis. Methods A total of 79 patients with lower limb deep vein thrombosis who underwent interventional surgery in our hospital were selected from March 2023 to January 2024 and randomly divided into two groups by digital table method, including 39 cases in the control group (routine nursing intervention) and 40 cases in the observation group (demand-oriented refined nursing intervention). The intervention outcomes of the two groups were compared and analyzed. Results After intervention, compared with the control group, the observation group had higher compliance rate[97.50% (39/40) vs. 82.05% (32/39）],lower complication rate[2.50% (1/40) vs. 20.51% (8/39）],higher the World Health Organization Quality of Life Scale Short Form (WHOQOL-BREF) score and higher satisfaction[95.00% (38/40) vs. 76.92% (30/39）],and the differences were statistically significant (P＜0.05). Conclusion The application of demand-oriented refined nursing in the perioperative period of lower limb deep vein thrombosis intervention surgery is helpful to improve patient compliance, reduce perioperative complications, and improve their quality of life and satisfaction. OBJECTIVE: To probe into the safety and efficacy of non-intubated general anesthesia with remimazolam combined with remifentanil in AngioJet mechanical thrombectomy for lower extremity deep vein thrombosis. METHODS: Totally 120 patients who underwent AngioJet mechanical thrombectomy for lower extremity deep vein thrombosis in Linyi People’s Hospital from Oct. 2021 to Oct. 2023 were extracted to be divided into the non-intubated general anesthesia group (n =60) and awake sedation group (n =60) through the random number table method. The non-intubated general anesthesia group received remimazolam combined with remifentanil for non intubated general anesthesia, while the awake sedation group was given awake sedation anesthesia. Vital signs of two groups of patients, including systolic blood pressure (SBP), diastolic blood pressure (DBP), cardiac output (CO), systemic vascular resistance (SVR), stroke volume and heart rate were compared at at the time of admission (T1), target vessel puncture (T2), drug spraying (T3), 15 minutes after drug spraying (T4 ), thrombus aspiration (T5), and 5 minutes after surgery (T6), 1 hour after surgery (T7), 4 hours after surgery (T8), and 12 hours after surgery (T9). The use of antihypertensive drugs during and after surgery, occurrence of hematuria and adverse events, target vessel puncture time, patients’ satisfaction score, and clinicians’ satisfaction score were recorded and compared between two groups. RESULTS: (1) From T1 to T9, there were statistically significant differences within the SBP and DBP groups between the non-intubated general anesthesia group and awake sedation group (P<0. 001). Both groups showed an increase in SBP and DBP at T3 and returned to normal at T9. The non-intubated general anesthesia group showed a significant decrease in SBP and DBP from T2 to T5 compared with the awake sedation group, with statistically significant difference (P<0. 001). (2) From T1 to T6, there were statistically significant differences with SVR and CO groups between the non-intubated general anesthesia group and awake sedation group (P<0. 001). SVR increased in both groups from T3 to T6, while CO decreased in both groups from T2 to T5. At various time points from T2 to T6, SVR of the non-intubated general anesthesia group was significantly lower than that of the awake sedation group, with statistically significant difference (P<0. 05). At various time points from T3 to T6, CO in the non-intubated general anesthesia group were significantly higher than those in the awake sedation group, with statistically significant difference (P<0. 05). (3) The clinicians’ satisfaction score and patients’ satisfaction score in the non-intubation general anesthesia group were significantly higher than those in the wake sedation group, with statistically significant differences ( P < 0. 001). (4) The use rate of antihypertensive drugs during surgery, the incidence of body movements, the incidence of severe hypertension and hypertensive subacute disease during surgery in the non-intubated general anesthesia group were significantly lower than those in the wake sedation group, with statistically significant differences (P < 0. 001). Objective To investigate the incidence and influencing factors of lower limb deep vein thrombosis (DVT) in patients during the stroke recovery phase and to develop a predictive risk model for DVT in this patient popula- tion. Methods A retrospective study was conducted involving 431 stroke recovery patients. Influencing factors for DVT were identified using univariate and multivariate logistic regression analyses. A risk prediction model was constructed based on these factors and presented using a nomogram. The model's predictive performance was evaluated using the area under the receiver operating characteristics (ROC) curve (AUC), sensitivity, and specificity. Internal validation was performed using the Bootstrap method. Results Among the 431 patients, 36 developed DVT, yielding an incidence rate of 8.35%. In the modeling group (301 patients), 26 cases of DVT were observed (8.64%), while in the validation group (130 patients), 10 cases of DVT were recorded (7.69%). Logistic regression analysis identified modified Barthel index (MBI) score, Caprini DVT risk score, mean platelet volume (MPV), creatinine (Cr), thrombin time (TT), and D-dimer as independent risk factors for DVT in stroke recovery patients (OR=0.978, 1. 186, 0.662, 0.979, 1.043, and 1. 766, respectively, all P<0.1). A nomogram based on these six factors was developed. The model showed good predictive performance with an AUC of 0.842 in the modeling group, a Youden index of 0. 525, a diagnostic value of 0. 115, sensitivity of 0. 833, and specificity of 0.692. Hosmer Lemeshow goodness of fit test yielded x² = 7.458, P = 0.589 In the validation group, the AUC was 0.720, with a Youden index of 0. 425, a diagnostic value of 0.072, sensitivity of 0. 675, and specificity of 0.700 (Hosmer - Lemeshow chi x² = 11.414 P = 0.248 ) Conclusion Stroke recovery patients exhibit a high incidence of DVT. Key factors such as MBI score, Caprini DVT risk score, MPV, Cr, TT, and D-dimer significantly influence the risk of DVT. This paper summarizes the nursing practice of a patient with abortive form of herpes zoster after opening surgery for lumbar disc herniation. KEY issues of nursing included the management of pain, skin condition, mental states, potential complications and other aspects. Application of multi-system, multi-element, multi-approach integrated traditional Chinese and Western medicine nursing model, combined with multi-disciplinary nursing intervention, is effective to relieve the pain and negative emotion, and improve the postoperative rehabilitation. This paper summarized the nursing practice of a patient with constipation after surgery of thoracic vertebra compression fracture treated by midnight-noon ebb-flow acupiont application combined with Chinese herbal medicine hot compress. Based on the comprehensive nursing assessment, targeted nursing interventions on pain, postoperative constipation, anxiety and potential complications were carried out. In addition, Traditional Chines medicine nursing techniques such as midnight-noon ebb-flow acupiont application combined with Chinese herbal medicine hot compress were conducted to relieve the constipation symptoms and improve the recovery of the patient. Objective To analyze the value of thromboelastogram (TEG) parameters in predicting deep venous thrombosis of the lower limbs in patients with chronic obstructive pulmonary disease (COPD) . Methods Thirty-five COPD patients complicated with deep venous thrombosis of the lower limbs were assigned into the observation group, and 35 COPD patients without deep venous thrombosis of the lower limbs of the same period were recruited in the control group. TEG parameters (R value of coagulation reaction time,K value of blood coagulation time, α angle of coagulation and MA value of maximum clot intensity), routine blood test, blood gas analysis and baseline data were collected within 24 hours after admission. Logistic regression analysis and receiver operating characteristic (ROC) curve analysis were used to analyze the predictive value of all parameters of TEG for deep venous thrombosis of the lower limbs in patients with COPD. Results R value,K value and α angle were significantly correlated with deep venous thrombosis of the lower limbs in patients with COPD (all P < 0.05) . The area under the ROC curve (AUC) of R value was 0.787 (95%CI: 0.679-0.895), 0.758 for K value (95%CI: 0.646-0.870), 0.689 for α angle (95%CI: 0.565-0.812), and 0.660 for MA value (95%CI: 0.533-0.787), respectively. The combination of four parameters yielded higher sensitivity and specificity for predicting deep venous thrombosis of the lower limbs (AUC:0.882, 95%CI:0.796-0.969, all P < 0.001), the cut-off value was 0.436, the sensitivity was 94.3% and the specificity was 80%, respectively. Conclusions R value,K value and α angle in TEG are the independent predictors of deep venous thrombosis of the lower limbs in patients with COPD. R value,K value and α angle can properly predict deep venous thrombosis of the lower limbs in patients with COPD, and the combination of R value,K value, α angle and MA value yields higher sensitivity and specificity. Objective To investigate the effect of quality control circle (QCC)activities in improving the standardized implementation rate of peripheral venous thrombolysis for lower-limb deep venous thrombosis (DVT). Methods A QCC group was set up. Factors causing failed implementation rate of DVT were analyzed, and countermeasures were carried out for continuous quality improvement. Results The standardized implementation rate of peripheral venous thrombolysis for lower-limb DVT increased form 33. 33% before QCC activities to 91. 25% after QCC activities, with a significant difference (P＜0. 05). Conclusion The implementation of QCC activities for continuous quality improvement of standardized procedure of peripheral venous thrombolysis may help to improve the effect of thrombolytic therapy. Objective: To investigate the correlation between ultrasonic elastography parameters and deep vein thrombosis of lower limbs after orthopedic surgery. Methods: Between June 2021 and June 2022, 80 patients with postoperative deep vein thrombosis of lower limbs who were diagnosed and treated in the orthopaedic department of our hospital were selected as the observation group, and 60 healthy patients who underwent physical examination at the same time were selected as the control group. Compare the ultrasound elasticity scores and strain values between the two groups; Pearson test was used to analyze the correlation between ultrasonic elasticity score, strain value and limb deep vein thrombosis; Multivariate logistic regression analysis was used to analyze the independent risk factors of limb deep vein thrombosis; The ROC was used to analyze the value of ultrasonic elasticity score and strain value in predicting deep limb vein thrombosis. Results: The ultrasound elasticity score and strain value in the observation group were higher than those in the control group (P<0.05). Pearson correlation test showed that ultrasonic elasticity score and strain value were positively correlated with lower limb deep vein thrombosis (r=0.785, 0.826, P<0.05). Multivariate logistic regression analysis showed that ultrasonic elasticity score and strain value were independent risk factors for lower limb deep vein thrombosis (OR=4.632, 4.768, P<0.05). ROC curve analysis results show that ultrasonic elasticity score and strain value are of great value in predicting lower limb deep vein thrombosis. Conclusion: With the formation of lower limb deep vein thrombosis, patients' ultrasonic elasticity scores and strain values increase. These two indicators have high value in predicting lower limb deep vein thrombosis. Objective: To explore the clinical efficacy of neuromuscular electrical stimulation(NMES) combined with intermittent pneumatic compression in the prevention of lower extremity deep venous thrombosis(LEDVT) in patients with craniocerebral trauma, and to provide evidence for the selection of intervention measures to prevent LEDVT. Methods: A total of 72 patients with craniocerebral trauma who were treated at the Lu'an Hospital Affiliated to Anhui University of Traditional Chinese Medicine from November 2020 to August 2021 were included in the study. They were randomly divided into control group and an observation group, with 36 patients in each group. The control group was given routine measures plus intermittent pneumatic compression intervention, while the observation group was given NMES based on the control group. Patient data were collected and compared between the two groups in terms of LEDVT status, length of bed, hospital stay,coagulation function indicators (prothrombin time (PT), activated partial thromboplastin time (APTT), D-dimer (D-D), and fibrinogen (FIB)) levels, blood flow velocity of popliteal vein and common femoral vein,Visual analogue scale (VAS) score, and activities of daily living (ADL) score before and after intervention. Results: The incidence of LEDVT in the observation group was 5.56% (2/36) lower than that 25.00% (9/36) in the control group, and the, length of bed and hospital stay in the observation group was shorter than that in the control group(P<0.05). After intervention, the levels of PT, APTT, D-D, FIB in the two groups were significantly improved compared to those before intervention, and the improvement in the observation group was more significant than that in the control group(P<0.05). After intervention, the blood flow velocity of popliteal vein and common femoral vein and ADL score in the two groups increased compared to those before intervention,while the VAS score decreased compared to those before intervention. The blood flow velocity of popliteal vein and common femoral vein and ADL score in the observation group were higher than those in the control group, while the VAS score were lower than those in the control group (P<0.05). Conclusion: NMES combined with intermittent pneumatic compression can shorten the bedtime and hospitalization time of patients with craniocerebral trauma, improve the coagulation function and blood circulation of patients, reduce the occurrence of LEDVT, and help to alleviate pain and improve their ability of live. Objective To analyze the application of hierarchical management based on bleeding risk assessment in patients with catheter-directed thrombolysis for treatment of lower extremity deep venous thrombosis (DVT). Methods Totally 100 patients requiring catheter-directed thrombolysis for treatment of lower extremity DVT were selected as the research objects, and they were randomly divided into the control group and the observation group, with 50 cases in each group. The control group received routine nursing, while the observation group received hierarchical nursing management based on bleeding risk assessment. The therapeutic effectiveness outcomes (patency rate, relief of lower extremity edema and treatment effect) and therapeutic safety outcomes (incidences of mild bleeding and severe bleeding) were measured and compared between two groups. Results The proportion of patients with patency rate≥50% and with relief rate of lower extremity edema≥50% in the observation group were higher than those in the control group, respectively (P＜0. 01). The therapeutic effective rate in the observation was higher than that in the control group(P＜0. 05). The incidences of mild bleeding and severe bleeding in the observation was lower than those in the control group, respectively(P＜0. 05). Conclusion The implementation of hierarchical management based on bleeding risk assessment can effectively improve the treatment effect of catheter-directed thrombolysis and reduce the incidence of bleeding complications. Objective To investigate the effect of different knee flexion angles at lithotomy position on deep venous blood flow of lower limb in patients undergoing radical cervical cancer resection. Methods A total of 120 patients undergoing radical cervical cancer resection in this hospital from May to December 2022 were selected and divided into the observation group and the control group by random number table method, with 60 cases in each group. Lithotomy position was adopted in all cases. The knee flexion angle of the control group was 110°, and the knee flexion Angle of the observation group was 130°. The changes of blood flow velocity difference between femoral vein and popliteal vein, vascular diameter and coagulation index were observed before and after surgery in the two groups. Results The blood flow velocity difference of left and right femoral vein and popliteal vein in the observation group before and after surgery was smaller than that of the control group, and the vascular diameter was larger than that of the control group, the differences were statistically significant (P <0. 001). The activated partial thromboplastin time (APTT) and prothrombin time (PT) 24 h after operation in the observation group were higher than those in the control group, and the D-dimer (D-D) value was lower than that in the control group, with all statistical significance (P <0. 001). Objective To investigate the clinical value of monitoring lower extremity venous flow velocity by ultrasound in screening the risk of lower extremity deep vein thrombosis (DVT). Methods 353 postoperative patients admitted to the Department of Neurosurgery from April 2020 to April 2021 were selected. Color Doppler ultrasound was used to monitor the lower extremity venous blood flow 4 times a day on the 1st to 3rd day after surgery. Patients were divided into DVT group and non-DVT group according to whether DVT occurred during hospitalization. Data were analyzed to analyze and evaluate the value of monitoring lower extremity blood flow velocity for screening the risk of DVT. Results There were significant differences in the flow velocity of femoral vein, popliteal vein and great saphenous vein between DVT group and non-DVT group (P<0.01). ROC curve analysis on the first postoperative day showed that the AUC of popliteal blood flow velocity screening for postoperative thrombosis risk in neurosurgery patients was the largest, and the sensitivity was 99.7% and the specificity was 100% when the cut-off value was 13. 504 cm/s. Conclusion Monitoring the lower extremity venous flow velocity, especially the popliteal vein flow velocity, has important clinical value in screening the risk of DVT2 after neurosurgery. Objective: To investigate the relationship between neutrophil count, D-dimer and Caprini thrombus score on lower extremity deep vein thrombosis (DVT) after cerebrovascular stenosis intervention surgery and the construction of prediction model. Methods: 281 patients with cerebrovascular stenosis who received endovascular interventional therapy in the Department of Neuro- surgery of our hospital from February 2021 to February 2022 were selected. Peripheral blood neutrophils count and D-dimer were detected before surgery. The Caprini thrombus score was used to evaluate the risk of DVT, and the patients were divided into DVT group (31 cases) and non-DVT group (250 cases) according to whether or not DVT occurred after surgery. Clinical data were collected, multivariate Logistic regression was used to analyze the risk factors of DVT after cerebrovascular stenosis interventional surgery, and the prediction model was constructed. Hosmer-Lemeshow was used to test the fitting degree of the prediction model, and receiver operating characteristic (ROC) curve was used to analyze the prediction value of the prediction model for DVT after cerebrovascular stenosis interventional surgery. Results: The proportions of neutrophil count, D-dimer and Caprini thrombus score with high risk and very high risk in the DVT group were higher than those in the non-DVT group (P<0.05). Furthermore, central venous catheter, bed duration exceeding 72 h, high neutrophil count, high D-dimer, Caprini thrombus score with high risk and very high risk were risk factors for DVT after cerebrovascular stenosis interventional surgery(P<0.05). According to the prediction model constructed by the above indexes, the AUC (0.95CI) of DVT after cerebrovascular stenosis interventional surgery was 0.871 (0.779~0.938), the sensitivity was 0.871 (27/31), and the specificity was 0.832(208/250). Hosmer-Lemeshow test showed good fitting effect(P>0.05). Conclusions: Central venous catheter, bed duration exceeding 72 h, high neutrophil count, high D-dimer, Caprini thrombus score with high risk and very high risk are risk factors for DVT after cerebrovascular stenosis interventional surgery. Objective To investigate the effect of Traditional Chinese Medicine (TCM) acupoint application in relief of symptoms in patients with peripheral deep venous thrombosis of the lower extremity (LEDVT). Methods Totally 60 patients with peripheral LEDVT were divided into the control group and study group, with 30 cases in each group. The control group received the routine treatment and nursing, and the study group received TCM acupoint application on the basis of routine treatment and nursing. The changes of edema degree, leg circumference and pain degree in the two groups were observed. Results Better outcomes of pain relief and edema regression were observed in the study group than those in the control group after treatment(P＜0. 05). 目的: 讨论初期康复护理干预对老年人脑血管意外患者腿部深静脉血栓 (DVT) 预防实际效果。方法: 130 例老年人脑血管意外患者, 住院即开启 Autar 深静脉血栓风险评估表并对评定, 根据分数鉴定区划普通高中低危, 并 进行一定的康复护理干预。观查患者护理干预实际效果, 较为患者干预前后左右心血管系统指标值水准。结果: 130 例患者干预后效果显著 85 例, 合理30例, 失效 15 例, 总有效率为 88.46%。干预后, 患者的血浆粘度、全血高切黏度和 全血低切粘度水准分别是 (1.62±0.23)、(4.31±0.55)、(9.05±1.68) mPa·s, 均明显低于干预前 (1.97±0.37)、(5.96±0.79)、 (12.54±2.24) mPa·s, 差异性具备统计意义 (P < 0.05)。结论: 初期康复护理干预可以显著降低老年人脑血管意外患者 腿部深静脉血栓发生率, 改正患者的不良的生活习惯, 改进患者的生活品质, 促进其尽快地恢复。 [ABSTRACT FROM AUTHOR]

目的 探讨老年髋部骨折患者受伤至入院时间与下肢深静脉血栓（DVT）形成的关系。方法 选择老年 髋部骨折患者485例，根据入院时血管超声检查是否发生下肢 DVT分为 DVT 组 81 例和非 DVT 组404例。回顾性分 析两组临床相关资料，包括年龄、性别、BMI、吸烟史、饮酒史、合并内科疾病（心血管疾病、肺部疾病、肝脏疾病、肾脏 疾病、肿瘤）、髋部骨折类型、骨折部位、有无多处骨折等，收集患者入院实验室检查指标（白细胞计数、中性粒细胞计 数、淋巴细胞计数、血红蛋白、血小板、白蛋白、纤维蛋白原、D-二聚体、肌酐、血清钠）。根据入院记录的主诉，提取患 者受伤至入院时间。采用 Cochran-Armitage 趋势检验分析不同受伤至入院时间与DVT发生率之间的关系。比较两 组患者的临床资料，对有统计学差异的计量资料行单因素 Logistic 回归分析，受试者工作特征（ROC) 曲线确定计量 资料的最佳截断值，对单因素分析有意义的因素进一步纳入多因素 Logistic 回归分析。结果 DVT组受伤至入院时 间长于非 DVT 组，股骨转子间骨折比例、纤维蛋白原水平高于非DVT组，血红蛋白、白蛋白、血清钠水平低于非DVT 组（P均<0. 05），两组其他临床资料比较均无统计学差异。趋势性检验结果显示，随着受伤至入院时间的增长，DVT 发生率升高（P<0. 01）。单因素分析结果显示，受伤至入院时间、骨折类型及白蛋白、纤维蛋白原、血清钠水平是 DVT 发生的影响因素（P均<0. 05）；ROC 分析显示，受伤至入院时间及白蛋白、纤维蛋白原、血清钠的最佳截断值分 别为2. 0 d、38. 5 g/L、3. 5 g/L及143. 1 mmol/L。多因素分析结果显示，受伤至入院时间>2 d、纤维蛋白原>3. 5 g/L、 血清钠<143. 1 mmol/L、股骨转子间骨折均是老年髋部骨折患者发生下肢 DVT 的独立危险因素（P 均<0. 05). 结论 老年髋部骨折患者 DVT 发生率随着受伤至入院时间的增长而升高，受伤至入院时间>2. 0 d是 DVT 发生的独 立危险因素，医护人员应高度警惕延迟入院老年髋部骨折患者 DVT 发生的高风险. BACKGROUND: With the update of artificial biomaterials and surgical techniques, total knee arthroplasty has become a reliable surgical method for the treatment of severe knee osteoarthritis. It can effectively relieve joint pain and stiffness, correct joint deformity, and restore joint function in patients with knee osteoarthritis. However, total knee arthroplasty can produce a large wound in such patients, and destroy the blood circulation environment. The blood flow is then slow and coagulated, forming a hypercoagulable state in the body and further inducing deep vein thrombosis, which seriously impacts surgical efficacy and patient satisfaction. Serious patients have the risk of developing fatal pulmonary embolism, which is a clinical problem concerned by joint surgical experts all over the world. OBJECTIVE: To observe the efficacy of Tuina in preventing lower extremity deep venous thrombosis after total knee arthroplasty for knee osteoarthritis. METHODS: Using a randomized controlled design, 98 patients with knee osteoarthritis scheduled for primary unilateral total knee arthroplasty were randomly divided into Tuina group and control group (n=49 per group). The control group was treated with oral rivaroxaban tablets after operation, and the Tuina group was treated with Tuina on the basis of oral rivaroxaban tablets. The intervention time was set from the 1st day to the 14th day after operation. The femoral vein stasis index, the incidence of deep venous thrombosis, D-dimer value, pressure pain thresholds, American Knee Society Score, thigh and calf circumference difference, and adverse events were recorded. The clinical trial protocol was approved by the Ethics Committee of Guanghua Hospital Affiliated to Shanghai University of Traditional Chinese Medicine on May 16, 2020 (approval No.2020-K-43). Patients who participated in the clinical trial were fully informed of the study protocol and process. Informed consent was signed prior to the start of the clinical trial. This clinical trial was registered at Chinese Clinical Trial Registry on June 4, 2020 (http://www.chictr.org.cn/listbycreater.aspx, ChiCTR2000033537). RESULTS AND CONCLUSION: After 14 days of intervention, the total femoral vein stasis index in the Tuina group was significantly lower than that in the control group (P < 0.05). After 14 days of intervention, the incidence of deep venous thrombosis in the Tuina group was lower than that in the control group, but there was no significant difference between the two groups (P > 0.05). After 14 days of intervention, the D-dimer value of the Tuina group was significantly lower than that of the control group (P < 0.05). After 14 days of intervention, pressure pain thresholds at medial and lateral points of the knee joint in the Tuina group were significantly higher than those in the control group (P < 0.05). After 14 days of intervention, the American Knee Society Score of the Tuina group was significantly higher than that of the control group (P < 0.05). After 14 days of intervention, the circumference difference of thigh and calf in the Tuina group was significantly lower than that in the control group (P < 0.05). No adverse events occurred in both groups. To conclude, Tuina is safe and reliable, which can improve the lower limb femoral vein stasis state, reduce the D-dimer value, reduce thigh and calf circumference difference, improve joint pain, and promote joint function rehabilitation. BACKGROUND: The incidence of lower extremity deep venous thrombosis after knee arthroscopy is between 0.27% and 17.5%, so it is of great significance to actively prevent the occurrence of lower extremity deep venous thrombosis after knee arthroscopy. OBJECTIVE: To investigate the effect of using exsanguination band under routine tourniquet during knee arthroscopy on the incidence of lower extremity venous thrombosis in the early postoperative period. METHODS: Totally 200 patients between the ages of 40 and 60 years who underwent knee arthroscopic surgery in the Taihe Hospital of Hubei Medical College from June 2019 to December 2020 were included, including 100 patients in the driven blood band group and 100 patients in the leg raising driven blood group. Pressure tourniquet was used to stop bleeding in both groups after blood evacuation. Blood coagulation was detected in both groups 1 day before operation, 1, 3, 7, and 14 days after operation. Color Doppler ultrasound of the lower extremity veins was performed 2 days after the operation to evaluate the occurrence of lower extremity vein thrombus. Motion range of the knee was compared between the two groups at 1, 3, and 7 days after operation. This study was approved by the Taihe Hospital of Hubei Medical College (approval No. 2021KS096). RESULTS AND CONCLUSION: (1) Prothrombin time, activated partial thromboplastin time, and D-dimer level were increased after operation compared with those before operation in both groups. Prothrombin time, activated partial thromboplastin time, and D-dimer level were increased at 1, 3 and 7 days postoperatively in the driven blood band group compared with the leg raising driven blood group (P < 0.05). There was no significant difference between the two groups at 14 days postoperatively (P > 0.05). (2) Color Doppler ultrasound examination showed that there was no deep venous thrombosis in the lower extremities between the two groups, but there were more intermuscular venous thrombosis in the lower extremities. The incidence of intermuscular venous thrombosis was higher in the driven blood band group than that in the leg raising driven blood group (28%, 16%, P < 0.05). (3) There was no significant difference in motion range of the knee after operation in both groups (P > 0.05). (4) Results suggest that tourniquet combined with exsanguination band use can aggravate the hypercoagulable state of patients after knee arthroscopy, which increases the incidence of intermuscular venous thrombosis in the lower extremities and the risk of secondary pulmonary embolism caused by this, but will not increase the incidence of deep vein thrombosis in the lower extremities after surgery. Objective　 To explore the cluster nursing for preventing deep vein thrombosis of lower extremity after total hysterectomy for benign gynecological diseases. Methods　A total of 90 patients with benign gynecological diseases after total hysterectomy from June 2020 to February 2021 were selected as the case group, and 88 patients from September 2019 to May 2020 were selected as the control group. The control group was treated with routine gynecological nursing, and the case group was treated with cluster nursing. Implement a collection of evidence-based, interrelated care interventions and measures which can improve patient outcomes. Meanwhile to systematize the decentralized care process. Three indicators were compared between two groups，including the incidence of deep vein thrombosis of lower extremity，the satisfaction of patients，and satisfaction of medical staff with cluster nursing plan. Results　The incidence of deep vein thrombosis of lower extremity in the case group was 1.11% （1/90），which was lower than that in the control group （7.95%，7/88)，and the difference was statistically significant （ P < 0.05）. Objective: In order to improve the efficiency of treatment, this study compared catheter contact thrombolysis in patients with iliac vein compression syndrome and deep vein thrombosis at the same time as balloon dilatation + stent treatment plan and staged treatment plan. Methods: Sixty-five patients with iliac vein compression syndrome combined with deep vein thrombosis of the lower extremities were taken as the research objects. According to the treatment method, they were divided into study group and control group. Thirty-two patients in the study group received catheter-contact thrombolysis and balloon dilatation + stent therapy at the same time; 33 patients in the control group received catheter-contact thrombolysis and balloon dilatation+ stent therapy. The patient's thigh circumference, calf circumference, vascular patency score, surgical indicators (hospitalization time, hospitalization times, thrombolysis time and puncture times), complications (thrombosis recurrence, bleeding, and pulmonary embolism) and sequelae of deep vein thrombosis are used as indicators. Results: The thigh circumference, calf circumference and venous patency scores of the two groups of patients were not significantly different before treatment (P>0.05). After treatment, the thigh circumference, calf circumference and venous patency scores of the two groups were significantly improved (P<0.05). There was no significant difference in various indicators between the two (P>0.05). The length of hospitalization, the number of hospitalizations, the time of thrombolysis and the number of punctures in the study group were significantly lower than those in the control group (P<0.05). There were 0 cases of thrombosis recurrence in the study group and 4 cases of thrombosis recurrence in the control group (P＜0.05). There were 1 bleeding in the study group, 0 pulmonary embolism, 2 bleeding in the control group, and 0 pulmonary embolism. There was no significant difference in bleeding and pulmonary embolism between the two groups(P>0.05). The incidence of PTS in the study group was 6.25 %, which was significantly lower than 18.18 % in the control group (P<0.05). Conclusions: Catheter contact thrombolysis at the same time balloon dilatation + stent treatment program has a good effect on iliac vein compression syndrome with deep vein thrombosis of lower limbs. BACKGROUND: Lower extremity deep vein thrombosis is a common complication after hip and knee arthroplasty. There are many introductions about its influencing factors, but there are few introductions about the thresholds of influencing factors. OBJECTIVE: To investigate the diagnostic value of D-dimer, erythrocyte sedimentation rate and C-reactive protein in deep vein thrombosis of lower limbs after hip and knee arthroplasty.METHODS: The clinical data of 216 patients who underwent hip and knee arthroplasty in Hebei Provincial People’s Hospital from January 2018 to November 2020 were retrospectively analyzed, including 77 cases of total knee arthroplasty, 31 cases of unicondylar arthroplasty, 35 cases of total hip arthroplasty, and 73 cases of hemiarthroplasty of the hip. Color Doppler ultrasonography of deep veins in both lower extremities was performed on the first day after surgery, and the diagnosis of deep vein thrombosis in lower extremities was confirmed by more than two attending physicians. Among them, there were 86 patients with lower extremity deep vein thrombosis and 130 patients without deep vein thrombosis. The rates of deep vein thrombosis in lower limbs were compared. According to the criteria of age, gender and replacement type, 86 patients without lower extremity deep vein thrombosis after surgery were selected as the control group using PSM propensity score matching method. The levels of plasma D-dimer, albumin and hemoglobin in 172 patients (86 patients in the thrombosis group and 86 patients in the control group) were compared, and the diagnostic efficacy of each index in the diagnosis of lower extremity deep vein thrombosis after hip and knee arthroplasty was analyzed by ROC curve. RESULTS AND CONCLUSION: (1) Compared with the control group, the D-dimer level of 172 patients in thrombus group was increased (P < 0.05). (2) The sensitivity and specificity of D-dimer for the diagnosis of deep vein thrombosis of lower extremity after hip arthroplasty were 71.8% and 74.1%. The time sensitivity and specificity of prothrombin were 66.7% and 66.7% respectively. The sensitivity and specificity of erythrocyte sedimentation rate were 61.5% and 70.4%. The sensitivity and specificity of C-reactive protein level were 94.9% and 33.3% respectively. The age sensitivity was 71.8% and the specificity was 100%. (3) The sensitivity and specificity of D-dimer for the diagnosis of deep venous thrombosis of lower extremity after knee arthroplasty were 78.7% and 44.1%, respectively. (4) The above data confirmed that the sensitivity of D-dimer level indicators in the diagnosis of deep vein thrombosis of lower extremities after knee replacement was higher, while the sensitivity and specificity of prothrombin time, erythrocyte sedimentation rate, C-reactive protein level and age in the diagnosis of deep vein thrombosis of lower extremities after hip arthroplasty were higher. Objective: To explore the effects of different anticoagulation strategies on the incidence rates of deep vein thrombosis (DVT) and pulmonary embolism in patients with acute multiple trauma and lower limbs fractures. Methods: 200 patients with acute multiple trauma and lower limbs fractures who were treated in our hospital from January 2017 to December 2017 were divided into observation group and control group according to the random number table method, with 100 cases in each group. Control group was given low-molecular-weight heparin calcium, and observation group was given rivaroxaban. After 1 month of follow-up, the occurrence of DVT and pulmonary embolism, thrombelastogram indexes and hemorheology before operation and at 1 d and 5 d after operation, and intraoperative blood loss, postoperative drainage volume and occurrence of adverse drug reactions were compared between the two groups. Results: At 1 month of follow-up, the incidence rate of DVT was lower in observation group was significantly lower than that in control group (P<0.05), and there was no significant difference in the incidence rate of pulmonary embolism between the two groups (P>0.05). At 1d after operation, the R time and K time in the two groups were shorter than those before operation (P<0.05), and the α angle, MA value and middle shear whole blood viscosity, low shear whole blood viscosity and plasma viscosity were all higher than those before operation (P<0.05), but there was no significant difference in the above indexes between the two groups (P>0.05). At 5d after operation, the R time and K time in the two groups were longer than those at 1d before operation (P<0.05), and the α angle, MA value and middle shear whole blood viscosity, low shear whole blood viscosity and plasma viscosity were all lower than those at 1d before operation (P<0.05), and the changes of above indexes in observation group were greater than those in control group (P<0.05). There were no statistically significant differences in intraoperative blood loss, postoperative drainage volume and incidence rate of adverse reactions between the two groups (P>0.05). Conclusion: Rivaroxaban and low-molecular-weight heparin sodium are beneficial to improve the hemorheology and coagulation function of patients with acute multiple trauma complicated with lower limbs fractures, but the former can more effectively reduce the risk of DVT. Objective:To investigate the risk factors of deep venous thrombosis in patients with endometrial cancer during perioperative period. Methods: A retrospective analysis of 516 cases of endometrial cancer diagnosed in the First Hospital of Lanzhou University from February 2013 to July 2018, 41 patients with deep venous thrombosis of lower extremity were selected as experimental group and 41 patients in the same period with endometrial carcinoma as control group. The two groups were compared in general condition, preoperative laboratory examination, intraoperative status and postoperative pathology. Results: Univariate analysis showed hypertension (P=0.012), diabetes (P=0.018), body mass index (BMI, P=0.028), platelets >300×109 /L (P=0.018), cholesterol (P=0.000), D-dimer (P=0.023), operative time (P=0.025), intraoperative bleeding (P=0.029), lymph node metastasis (P=0.012), tumor differentiation (P=0.018), and tumor staging (P=0.002) showed statistically significant differences between two groups. Logistic regression analysis showed that patients with hypertension, patients with BMI>27 kg/m2, platelet count>300×109 /L, increased operative time, increased bleeding, increased lymph node metastasis, low tumor differentiation, tumor FIGO staging are high risk factors for deep venous thrombosis in patients with endometrial cancer. Conclusions: For endometrial cancer patients with hypertension, BMI >27 kg/m2, platelet count >300 ×109 /L, increased operative time, increased intraoperative blood loss, lymph node metastasis, low degree of tumor differentiation and late FIGO stage, it is necessary to control the above risk factors during the perioperative period and adopt other methods to prevent lower limb deep vein thrombosis. Objective To study the effect of vitamin K epoxide reductase complex 1 (VKORC1) and cytochrome P4502 C9 (CYP2C9) gene polymorphisms in Kunming Ilan population on steady-state dose of warfarin and to establish a predictive dose model. Methods Totally 162 blood samples from patients with deep venous thrombosis of lower extremity treated with warfarin anticoagulation were enrolled. The amplification block mutant system-polymerase chain reaction technology was used to detect the gene polymorphisms of VK0RC1 and CYP2C9. The patients' age. sex, height, body weight, international standard ratio (INR) and the genotypes of VK0RC1 and CYP2C9 were recorded. Pearson correlation analysis was used to screen the factors related to steady-state dose of warfarin. The multiple stepwise regression analysis of VKORC1 and CYP2C9 genotypes, age, sex, body weight, height, INR, and warfarin status in patients was used to predict the dose model. In addition, 20 patients were selected to observe the occurrence of hemorrhagic events after using warfarin dosage recommended by the predicted dose model. Results During 162 cases of patients, there were 151 cases of CYP2C9 AA and 1 1 cases of CYP2C9 AC; among them, the warfarin recommended dose (combined amiodarone and non-amiodarone) of genotype AC was lower than that of genotype AA. The AA, AG, and GG types of VKORC1 were found in 138 cases, 22 cases, and 2 cases, respectively; among them, the warfarin recommended dose (combined amiodarone and non-amiodarone) of AA type was significantly lower than that of the AG and GG types. The age, body weight, VKORC1 , and CYP2C9 gene polymorphisms were related to the steady-dose of warfarin, but the height, BMI, and INR were not. Twenty patients using the predicted dose model did not have bleeding during the follow-up period. Conclusions There are VKORC1 and CYP2C9 gene polymorphisms in the Han population of Kunming. The gene polymorphisms of VKORC1 and CYP2C9 have a certain influence on steady-state dose of warfarin. Objective: To investigate the effects of different doses of rivaroxaban on coagulation function and deep venous thrombosis (DVT) in elderly patients undergoing hip replacement. Methods: A total of 200 patients, who underwent hip replacement in Jingzhou Central Hospital of Hubei Province from June 2013 to June 2016, were selected and randomly divided into group I (n=100) and group II (n=100). After surgery, group I and group II were treated with 5 mg and 10 mg rivaroxaban orally to prevent DVT. Fibrinogen (Fbg). prothrombin time (PT). thrombin time (TT). activated partial thromboplastin time (APTT) and D-dimer (D-D) were detected before surgery and Id and 7d after surgery. At the same time, lower extremity color Doppler ultrasound was used to determine the occurrence of DVT. the clinical bleeding events were observed. Results: The lewis of Fbg in the two groups were decreased significantly Id after surgery’, and PT was significantly prolonged (all P<0.05). 7d after surgery, the levels of Fbg in the two groups were significantly higher than those of Id after surgery’, and PT was significantly shortened (all P<0.05). The levels of D-D were significantly increased in the two groups Id after surgery (P<0.05), and group I was higher than group II (P<0.05). The levels of D-D in the two groups 7d after surgery were significantly lower than those Id after operation (P<0.05), and group II was lower than that group I (P<0.05). The incidence of early DVT in group II was significantly lower than that in group I (P<0.05). There was no significant difference in the incidence of clinical bleeding events after prophylactic anticoagulation between the two groups (P>0.05). Conclusion: Oral high dose (10 mg) rivaroxaban after hip arthroplasty in elderly patients can effectively improve the hypercoagulable state and reduce the incidence of DVT. and will not significantly increase the clinical bleeding events, which is worth popularizing. Objective To analyze the clinical efficacy and safety of balloon assisted transcatheter thrombolysis (CDT) in treatment of lower extremity deep venous thrombosis (DVT) complicated by surgery， and provide a reference for the selection of clinical treatment in patients with surgery of lower extremity deep venous thrombosis. Methods The clinical data of 60 cases of deep venous thrombosis in the department of general surgery and Department of orthopedics， obstetrics and gynecology of our hospital from February 2013 to March 2014 were analyzed. The enrolled patients were divided into two groups，37 cases in A group were treated with the traditional treatment of anticoagulation and thrombolysis， 23 cases in B group were given balloon dilatation therapy assisted CDT . Then we compared two groups in the baseline data， clinical effect and complications after treatment before and after treatment， the plasma D- two dimer， Wells clinical score changes， hospitalization time， dosage of urokinase. Results There was no significant difference between the two groups in terms of gender， age， type of operation， cumulative lower limb condition， lower extremity diameter， internal medicine complication， plasma two aggregation level and Wells score before admission (P>0.05) . A group was cured in 23 cases (62.2%)， the total effective rate was 83.80%， B group was cured 17 (73.9%)， total effective rate was 91.3%， and the total effective rate of the two groups was not statistically significant (P>0.05) . The dosage of urokinase， the rate of hematuria and subcutaneous hemorrhage in group B were significantly lower than those in group A (P < 0.05) . The hospitalization time of group A was 6-14 (8.6 + 2.47) days， the B group was 5-15 days (6.1 + 1.19) days， and the hospitalization time of group B was lower than that of group B，the difference between the two groups was statistically significant (P<0.05). The plasma D- two，and Wells scores of the two groups were significantly lower than those before treatment (P<0.05)，and the B group was significantly lower than that of the A group (P<0.05) . Conclusions CDT has the advantages of short hospitalization time， low complication rate and low dosage of thrombolytic drugs in surgical treatment of patients with deep vein thrombosis， but it can not improve the overall effective rate of patients. Objective: To investigate the clinical efficacy and safety of intravenous cavity filter implantation combined with catheter directed thrombolysis for deep venous thrombosis patients. Methods: The clinical data of SO cases of patients with deep vein thrombosis in our hospital from October 2012 to October 2014 were retrospectively analyzed, according to the different treatment methods and therapy, the patients were divided into two groups, the lower limb circumference difference, venous patency, post thrombotic syndrome and complications were compared before and after the treatment between two groups. Results: After treatment, the average circumferences and swelling of lower limb as well as venous patency score of both gr oups were decreased compared with those before treatment (P<0.05), which were more obvious in the observation group compared with those of the control group after treatment(P<0.05). After treatment. PTS occurred in 9 cases (23.68%) in the control group, while the incidence of PTS was lower in the observation group (2.38%)(P<0.05). There was no significant difference irr the incidence of complications after treatment between two groups (P>0.05). Conclusion: The use of vena cava filter implantation combined with catheter directed thrombolysis could make the vein obstruction site as early as possible to restore patency, reduce the valve dysfunction and the incidence of PTS and other complications in the treatment of lower extremity deep venous thrombosis. Objective: To investigate the treatment effect of low molecular weight heparin calcium combined with Ginkgo Leaf Extract and Dipyridamole Injection on the lower extremity deep femoral intertrochanteric fracture postoperative venous thrombosis treatment. Methods: Selected SO patients with femoral intertrochanteric fractures at department of orthopedics in our hospital from June 2015 to December 2016, which were randomly divided into study group (n=40) and control group (n=40) according to the random number table method. Patients of two groups were treated with surgical treatment. The control group was treated with low molecular weight heparin calcium 6000IU after surgery, subcutaneous injection. 2 times/d: The study group were treated with Ginkgo Leaf Extract and Dipyridamole Injection 20 mL on the basis of the control group, intravenous drip. 1time/d. and the course of treatment in two groups was l0d. Observed incidence of lower extremity deep venous thrombosis, the 15 cm diameter and the swelling rate of the upper pole of the patella and the inferior pole of the patella of patients in two groups after surgery, the prothrombin time (PT), activated partial thromboplastin time (APPT). D-dimer (D-D) levels and blood rheology examination index before and after treatment. Results: The incidence of lower extremity deep venous thrombosis in the study group was 2.50%, which was lower than that 15.00% in the control group (P<0.05). After treatment, 15 cm diameter of the superior pole of the patella and the 15cm diameter of the inferior pole of the patella of patients in two groups were increased slightly, and there was no statistical difference compared with before treatment (P>0.05). There was no significant difference between the two groups in the swelling rate of 15 cm on the upper pole of the patella and 15 cm in the inferior pole of the patella (PX1.05). PT and APPT of patients in the study group were significantly higher than the control group at 2 d after treatment, and D-D was significantly lower than the control group (P<0.05). At l0d after treatment, APPT and PT were significantly higher than the control group, D-D was significantly lower than the control group (P<0.05). At 2d, l0d after treatment, the whole blood high and low shear blood plasma viscosity, hematocrit, erythrocyte aggregation index and fibrinogen in study group were significantly lower than the control group (P<0.05). Conclusion: Low molecular weight heparin calcium combined with Ginkgo Leaf Extract and Dipyridamole Injection can reduce blood coagulation, improve blood rheology, which has a good effect on prevention of deep vein thrombosis of lower extremity in patients with femoral intertrochanteric fractures. Objective: To investigate the clinical efficacy of xueshuantong injection combined with rivaroxaban on preventing deep venous thrombosis (DVT) after operation of lower limb fracture and its influence on hemorheology. Methods: One hundred and twenty-five patients with lower limb fracture admitted into our hospital from April 2012 to July 2015 were randomly divided into control group and observation group. Sixty-three patients in control group were treated with xueshuantong injection, and sixty-two patients in observation group were treated with xueshuantong injection combined with rivaroxaban. The incidence rate of DVT, early diagnosis index including D-dimer (DD), homocysteine (Hcy), and hypersensitive C-reactive protein (hs-CRP), hemorheology index including hematocrit (HTC), whole blood viscosity (WBV), and fibrinogen, and safety were analyzed and compared before and after treatment. Results: After three weeks of treatment, observation group with total incidence rate of 9.7% was significantly lower than that of control group with44.4% (P<0.05). The DD, Hcy, hs-CRP, HTC, WBV, and fibrinogen in both groups were markedly lower (P<0.05) than those before treatment, and the six indexes in observation group were significantly lower than those of control group (P<0.05). In addition, difference in adverse reaction between observation group with 9.68% and control group with 7.94% was not obvious (P>0.05). Conclusion: Xueshuantong injection combined with rivaroxaban could be a effective treatment for the prevention of DVT and the improvement of hemorheology of patients with lower limb fracture.
