作为一种决策支持系统,模糊系统不仅具有处理不确定性信息的能力,又能够明确表达不确定性知识和推理过程。但现存的一个问题是,对于包括采用模糊方法的系统在内的计算机决策支持系统,目前还未出现能够明确评估系统实际可行性的方法。提出了不可区分性的概念框架,并将其作为评估计算机决策支持系统的关键部分,给出了相关案例研究。案例证明人类专家的评判并非完美,模糊系统能够在技术层面模拟人类的决策,包括人类专家在评判时表现出的差异性。使用模糊方法进行基于知识不确定性的表达与推理是非常必要的,而差异则是学习时不可避免的表现形式,在评估人工智能系统时应接受其不完美的决策。

3. 数字PCR 和血培养检测儿童MRSA 血流感染应用价值比较

【目的】比较数字PCR 法和血培养法在检测儿童耐甲氧西林金黄色葡萄球菌血流感染中的应用效果。【方法】收集2017 年10 月到2019 年9 月在我院住院患儿的血液,同步进行数字PCR 方法检测和血培养法检测患儿的耐甲氧西林金黄色葡萄球菌感染情况。统计学分析采用SPSS18.0 软件统计分析。【结果】收集到246 例患儿血液标本,检测为耐甲氧西林金黄色葡萄球菌感染阳性的69 份,其中血培养和数字PCR 均为阳性的53 份,仅PCR 阳性的11 份,仅血培养阳性的5 份,血培养和数字PCR 均阴性的177 份。数字PCR 和血培养结果相符合的占93.50%(230/246);血培养平均阳性培养周期为48h,数字PCR 检测的平均时间为2.5 h;血培养为金标准,数字PCR 的敏感性为91.40%(53/58),特异性为94.15%(177/188),阳性预测值82.81%(53/64),阴性预测值97.25%(177/182)。结果显示数字PCR 的曲线下面积达93%,诊断价值较高,相比血培养法平均耗时需48 h,而数字PCR 法检测时间平均2.5 h,明显比培养法耗时少。【结论】在诊断儿童耐甲氧西林金黄色葡萄球菌血流感染时,数字PCR 法方法简单、快捷、灵敏度高、特异性好,有望提高耐甲氧西林金黄色葡萄球菌感染的快速诊断能力,为早期精准治疗赢取时间。

Objective To analyze the colonization characteristics and risk factors of methicillin -resistant Staphylococcus aureus (MRSA) in children with severe pneumonia. Methods From January 2018 to February 2023, 659 children with severe pneumonia diagnosed by MRSA were screened in Anhui Children s Hospital. The family background and clinical characteristics of children with positive and negative MRSA colonization were analyzed. Univariate and multivariate Logistic regression were used to analyze the risk factors of MRSA colonization. Results A total of 74 cases (11.2%) were positive for MRSA colonization. Age (02? =0.96, 95%CI: 0.94-0.98), history of hospitalization within one year (OR=2, 95%CZ: 1.06-2.96), history of skin and soft tissue infection (OR=1.82, 95%CI: 1.32-2.24), history of hospitalization of family members (02? =233, 95%CI: 2.1-2.53), and history of multiple antibiotic use (OR=1.25, 95%CI: 1.19-2.07) were independent risk factors for MRSA colonization. The lung MRSA infection rate of children with severe pneumonia with positive MRSA colonization was 16.2%, which was significantly higher than that of negative children (1.7%) (Z=42.841, P < 0,001). Conclusion MRSA colonization has an important influence on the condition and treatment of children with severe pneumonia. Based on related risk factors, high-risk children with MRSA colonization can be identified, which is of great significance for early detection and treatment of severe pneumonia.
