目的比较常规体外受精(IVF)和卵胞质内单精子注射(ICSI)两种不同授精方式对解冻移植第六天(D6)囊胚患者妊娠结局的影响。方法回顾性队列研究分析中山大学附属第三医院生殖中心2018年1月至2020年4月期间解冻移植第六天囊胚移植患者的临床资料,按受精方式分为常规IVF组(446例)和ICSI受精组(200例)。按是否有第五天(D5)囊胚移植史,分为仅有D6囊胚冷冻及移植的患者,以及有D5囊胚移植史的患者。比较两组患者基本资料、囊胚质量及妊娠结局。结果IVF组和ICSI组患者BMI,不孕年限,基础FSH差异均无统计学意义(P > 0.05)。ICSI组患者年龄小于IVF组患者,原发性不孕的比例高于常规IVF组,获卵数和正常受精卵子数高于常规IVF受精组(P < 0.001)。常规IVF组五期和六期囊胚比例高于ICSI组(21.6 % vs. 3.14 %, P < 0.001)。ICSI组ICM评分为A级的囊胚高于IVF组(23.8 % vs. 14.3 %, P = 0.01)。ICSI组HCG阳性率和临床妊娠率均高于IVF组(65.5 % vs. 48.4 %, P < 0.001; 56 % vs. 41.3 %, P = 0.001),ICSI组胚胎种植率和活产率也高于常规IVF组(43.8 % vs. 30.9 %, P < 0.001; 43.0 % vs. 31.8 %, P = 0.006)。校正两组间年龄、获卵数等影响因素后,ICSI组临床妊娠率仍高于常规IVF组(OR:1.590, 95 % CI: 1.030, 2.455, P = 0.036)。ICSI组出生婴儿体质量低于IVF组(P = 0.016),早产率、性别比与分娩方式在两组间差异无统计学意义。结论ICSI受精的D6囊胚解冻移植后的临床妊娠率高于常规IVF受精的D6囊胚,活产率有升高的趋势,可能与IVF与ICSI病人导致囊胚发育慢的因素不同有关。ICSI受精的D6囊胚解冻移植后相对好的妊娠结局,可在一定程度上弥补ICSI周期囊胚发育相对较慢导致的妊娠结局的差异。

近年来,高脂血症患病率在全球范围内增长,多不饱和脂肪酸降脂作用的研究引起国内外学者高度关注并取得重要进展。为对深入研究多不饱和脂肪酸对血脂调节的相关机制及进一步开发多不饱和脂肪酸来源的药物或功能性食品提供参考,综述了近年来国内外对多不饱和脂肪酸降脂作用机制的研究进展,主要集中于激活脂质代谢相关转录因子及信号通路、调节炎症因子、改善氧化应激反应、提高脂联素水平、减少瘦素含量及表观遗传修饰等方面,并指出近年来多不饱和脂肪酸降脂研究中存在的共性问题。多不饱和脂肪酸作为一种独特的生物活性物质,已在降血脂、抗氧化、抗炎、免疫调节等多种方面展示出显著的药理活性,具有很大的发展潜力。 In recent years, the prevalence of hyperlipidemia has increased worldwide, the research on the lipid-lowering effect of polyunsaturated fatty acids has attracted great attention and made important progress. In order to provide reference for further research on the mechanism of polyunsaturated fatty acids regulating blood lipids and further development of drugs or functional foods derived from polyunsaturated fatty acids, the recent progress on the lipid-lowering mechanism of polyunsaturated fatty acids at home and abroad were reviewed, mainly focusing on activation of lipid metabolis-related transcription factors and pathways, regulation of inflammatory factors, reduction of oxidative stress, improvement of adiponectin level, reduction of leptin content and epigenetic modification. Some common problems in lipid-lowering studies of polyunsaturated fatty acids in recent years were pointed out. As a unique bioactive substance, polyunsaturated fatty acids have shown significant pharmacological activities in lowering blood lipid, antioxidation, anti-inflammatory and immune regulation,and have great potential for development.

4. 不同反刍思维类型大学生对情绪面孔的注意偏向及时间进程

目的探讨消极型、低型、积极型反刍思维大学生对情绪面孔的注意偏向及其时间进程。方法采用积极和消极反刍思维量表区分三种反刍思维类型的大学生,并从各类选取28名参与结合眼动技术的点探测范式实验,情绪刺激材料为正性-中性、负性-中性、中性-中性的情绪面孔图片对。结果反应时指标显示,在负性-中性面孔对条件下,被试类型对注意偏向分数(BI)和注意脱离困难指数(DI)的影响均有统计学意义(F = 20.11,P < 0.001, η2 = 0.33)、(F = 11.46,P < 0.001, η2 = 0.22)。在正性-中性面孔对条件下,被试类型对BI和DI的影响均无统计学意义。消极型反刍被试对负性面孔的BI (t = 7.06, P < 0.001, d = 1.34)和DI (t = 5.92, P < 0.001, d = 1.12),积极型反刍被试对正性面孔的BI (t = 2.78, P < 0.01, d = 0.53),与0比较均有统计学差异。三组被试在所有条件下的注意定向指数(OI)与0比较均无统计学差异(P > 0.05)。眼动指标显示,各组被试在所有条件下的首视点定向概率与0.5均有统计学差异,低型反刍被试对正、负性面孔的首视点潜伏期偏向分数与0均有统计学差异;所有被试对正、负性面孔的首视点注视时间偏向分数与0和总注视时间百分比与0.5均有统计学差异,同时存在面孔图片类型的主效应 (F = 8.24,P = 0.005, η2 = 0.033)、(F = 9.31,P = 0.003, η2 = 0.036)。结论反应时指标表明消极型反刍思维者对负性刺激存在注意偏向,表现为注意脱离困难,积极型反刍思维者对正性刺激存在注意偏向。眼动指标表明各类型被试对情绪刺激均存在首次注视定向偏向、最初的和总体的注意维持且对负性刺激有更大的注意维持效应。

5. 高龄女性新鲜与冷冻胚胎移植后妊娠及围产结局

目的本研究旨在分析高龄女性自体新鲜胚胎移植与冷冻胚胎移植后的妊娠和围产结局,探讨行辅助生殖技术助孕的高龄女性鲜胚移植与冻胚移植方案有效性和安全性。方法回顾性队列研究分析2015年9月至2020年5月期间在中山大学附属第三医院生殖医学中心首次接受体外受精/卵胞浆内单精子注射治疗且进行首次胚胎移植≥35岁的女性患者,共纳入1 622例患者的1 622个胚胎移植周期,其中新鲜胚胎移植903例患者 (新鲜胚胎移植组),冷冻胚胎移植719例患者 (冷冻胚胎移植组),比较两组患者的基线特征、妊娠结局、妊娠期并发症发生率和围产结局。通过logistic回归分析调整混杂因素后,分析两组胚胎移植方案的妊娠结局及并发症的发生率。结果高龄女性患者新鲜胚胎移植临床妊娠率为40.2%,冷冻胚胎移植的临床妊娠率为36.6%,两组相比差异有统计学意义[OR 95%CI为1.66 (1.323,2.078),P<0.001];两组患者的流产率[25.1% vs. 33.5%,OR 95%CI为0.53 (0.356,0.794),P=0.002]、活产率[29.7% vs. 23.8%,OR 95%CI为1.93 (1.505,2.484),P<0.001]、剖宫产率[70.5% vs. 76.6%,OR 95%CI为0.60 (0.362,0.987),P=0.044]、多胎妊娠率[25.1% vs. 12.2%,OR 95%CI为1.89 (1.114,3.190),P=0.018]、妊娠期高血压[2.2% vs. 4.6%,OR 95%CI为0.35 (0.128,0.947),P=0.039] 差异有统计学意义。两组患者胚胎移植后异位妊娠率、早产率、妊娠期糖尿病和胎膜早破差异无统计学意义(P>0.05)。冷冻胚胎移植新生儿平均出生体质量为3 133 (S=612) g,高于新鲜胚胎移植新生儿平均出生体质量2 977 (S=609) g,差异有统计学意义(P=0.006)。进一步年龄分层分析,35-37岁患者新鲜移植临床妊娠率显著高于冷冻胚胎移植[47.8% vs. 45.4%,OR 95%CI为1.68 (1.176,2.391),P=0.004]差异有统计学意义。35-37岁及38-39岁患者的新鲜胚胎移植活产率高于冷冻胚胎移植[39.3% vs. 32.9%,OR 95%CI为1.79 (1.239,2.581),P=0.002]、[35.5% vs. 30.4%,OR 95%CI为1.70 (1.023,2.809),P=0.040]差异具有统计学意义。结论高龄女性患者新鲜胚胎移植可以获得较好的妊娠及新生儿结局,建议高龄患者在自身条件许可的情况下优先行新鲜胚胎移植,鲜胚移植也可减少胚胎移植的等待时间及改善高龄女性随着时间推移导致的不良结局。

6. 囊胚培养为主时解冻卵裂期胚胎移植的妊娠结局分析

目的探讨以囊胚培养及解冻囊胚移植为主流时,移植解冻卵裂期胚胎的妊娠结局,评价冻存卵裂期胚胎的价值,以及影响解冻卵裂期胚胎移植妊娠率的因素。方法回顾性分析本中心2015年9月至2020年6月共1 607例解冻卵裂期胚胎移植的临床结局,根据是否行囊胚培养及是否有可用囊胚,分为3组,即无养囊组、养囊失败组、养囊成功组。分析3组人群的年龄、不孕年限、基础FSH、BMI、获卵数、D3优胚数等基本资料,以及使用logistic回归分析影响解冻卵裂期胚胎移植临床妊娠率的因素。结果解冻D3卵裂胚移植的总体临床妊娠率较低(35.2%,565/1 607),未养囊组随着女方年龄的增加妊娠率显著下降。养囊失败组例数较少(98例),妊娠组与未妊娠组间临床资料及胚胎发育情况差异无统计学意义。养囊成功但有移植囊胚失败史的病人,解冻卵裂胚的总体临床妊娠率高于其他两组,为46.9%,年龄、获卵数、D3优胚数、可用囊胚数、移植胚胎评分不影响此组妊娠率。与囊胚已解冻完相比,剩余冻存囊胚数≥1的妊娠率显著升高,OR 95%CI为3.789(2.088,6.877),P<0.001。ICSI病人临床妊娠率略高于常规IVF病人,OR 95%CI为1.783(0.987,3.224),P=0.055。结论对于卵巢反应差无养囊的病人,临床妊娠率与年龄和胚胎质量有关;于养囊病人,即使养囊失败,或者移植囊胚失败(未妊娠),解冻卵裂胚胚胎移植仍有相当的临床妊娠机会。因此在养囊为主流的情况下,建议D3冻存部分卵裂期胚胎,可根据患者自身条件及中心的囊胚培养效率,选择一定数量及质量的卵裂期冻存。

【目的】运用高通量测序技术检测Ι型登革病毒(DENV-1)感染人脐静脉血管内皮细胞(HUVEC)后 长链非编码 RNA(lncRNA)的差异表达,分析以探讨导致血管内皮细胞功能改变或受损的可能分子机制。【方法】 DENV-1 感 染 人 脐 静 脉 内 皮 细 胞 24 h 后 ,分 别 提 取 对 照 组 和 病 毒 感 染 组 的 RNA 样 本 进 行 测 序 ,筛 选 差 异 表 达 的 lncRNA,进行 GO 和 KEGG 富集分析,构建共表达网络图。【结果】共筛查到 2 623 个显著差异表达的 lncRNA, 其中 1 441 个为表达上调,1 182 个为表达下调。发现表达差异 lncRNA 及其预测靶基因主要富集于影响抗原提 呈、干扰素合成、细胞凋亡、细胞粘附等生物过程。【结论】DENV-1 感染 HUVEC 后,lncRNA 表达谱发生明显变 化,与登革出血热/登革休克综合征的发生发展密切相关。

为降低可见光通信系统的误码率,提高系统的传输特性,研究了可见光通信系统的信道均衡技术。建立了室内可见光通信系统的信道模型,阐述了信道的直射冲激响应和反射冲激响应。针对多径效应导致的码间串扰问题,采用一种改进的LMS(最小均方)算法对信道进行补偿。实验结果显示:采用改进的LMS算法后,收敛速度更快,在系统趋于稳定时,误码率明显降低。均衡器对时变信道跟踪效果更好,系统稳态误差相对减小。

在室内可见光通信MIMO(多输入多输出)系统中,针对用户无法公平享用通信服务的问题,提出了一种寻找光接收机中最优聚光器增益来优化多用户情况下功率分配不均匀的方法。利用改进的NGA(小生境遗传算法)建立与接收光功率相对应的节点模型,通过运算寻找一组最优的聚光器增益组合运用到光接收系统中。仿真结果表明,改进的NGA能高效地找出一组最优聚光器增益组合,从而改善用户接收光功率分配不均匀的现象。

均匀波带交换波带粒度固定,灵活性和可扩展性不足。文章提出一种新型多粒度光交叉连接结构,并在该结构的基础上提出一种FENBS(灵活可扩展的非均匀波带交换)算法。当路径重叠的业务较少时,分组成较小粒度波带;当路径重叠的业务较多时,分组成较大粒度波带,比均匀波带交换波带分组更灵活。使更多的业务容纳在波带内,节省了端口。仿真结果表明:FENBS算法比均匀粒度的BPHT(大业务优先的平衡路径路由波带分配)算法占用的端口更少。

【目的】探讨姜酚肟对谷氨酸(glutamate, Glu)诱导PC12细胞损伤的保护作用。【方法】体外培养PC12细胞,建立谷氨酸诱导PC12细胞损伤的模型;采用MTT法检测细胞活力,流式细胞术和Hoechst 33258 DNA 染色法检测细胞凋亡。【结果】经5 mmol/L的谷氨酸处理24 h后,PC12细胞活力比对照组降低,仅为对照组的58.3﹪。在一定浓度范围内,姜酚肟能保护PC12 细胞免受谷氨酸(5 mmol/L)的影响,但较高浓度时对细胞有一定的毒性。经不同浓度姜酚肟(3.125、6.25、12.5、25 μg/mL)预处理后,细胞活力明显提高,其保护作用随浓度降低而升高,当浓度为6.25 μg/mL时效果最好,使PC12细胞的存活率达到75.2﹪(P

【目的】比较不同时期慢性阻塞性肺病(COPD)患者诱导痰中细胞组成的变化,及其诱导痰中细胞成分尤其中性粒细胞百分比与气流受限的关系。【方法】对110例COPD患者[急性加重期COPD(AECOPD)患者(A1组)62例、稳定期COPD患者(B组)48例]及30例健康对照者(C组)进行肺功能测定及痰诱导,痰液处理后沉渣作细胞学分析。其中AECOPD组在治疗1周后(A2组)再次进行上述指标的测定。【结果】140例受试者中有21例因不能耐受痰诱导过程或痰标本不合格而退出研究,最终有119例受试者完成本研究(A。2组48例,B组43例,C组28例)。A,组、A2组、B组、C组诱导痰中细胞总数(106/mL)分别为(8.8±3.7)、(6.5±2.6)、(5.0±2.8)、(2.8±1.3),细胞分类中A。组、A2组、B组以中性粒细胞为主,分别为66%’15%、60%±10%、55%±11%,而在C组中则以巨噬细胞、中性粒细胞为主。分别为37%±10%、32%±8%;AECOPD患者(A1组、A2组)诱导痰中性粒细胞百分比与肺功能指标(FEV1、FEV1%pre、FVC、FEV1/FVC)呈负相关(r分别为-0.860、-0.682、-0.662、-0.300,P〈0.001);稳定期组(B组)中性粒细胞与FEV1、FEV1%pre负相关(r分别为-0.346、-0.421,P〈0.05),对照组中性粒细胞与肺功能无相关性(P〉0.05)。【结论】AECOPD、缓解期、稳定期患者诱导痰细胞以中性粒细胞为主,健康者诱导痰细胞以巨噬细胞为主;气道局部中性粒细胞增多与各期COPD患者气流受限存在密切的关系。

- Abstract
Steel fiber has become a reinforcing material with increasing application demand in civil engineering, so the damage and fracture of steel fiber reinforced concrete (SFRC) is also a key problem that needs to be solved urgently in engineering. Based on the unified phase field theory and cohesion model proposed recently, a meso-finite element model of SFRC composed of steel fiber, aggregate, mortar and interface transition zone was established and verified by experiments. Under the conditions of uniaxial tension and three-point bending, the effects of factors such as steel fiber content and length, aggregate shape, and initial notch defects on the tensile and flexural properties of SFRC were investigated, and the mechanical properties and damage mechanism of SFRC were studied. The results show that the SFRC meso-finite element model based on the phase field damage model and cohesion model has good accuracy and reliability. The tensile strength of SFRC is greatly affected by the content of steel fiber, and the cost performance is higher at 2% content, which is 34. 2% higher than that at 0. 5% content. The influence of aggregate shape and steel fiber length is small. The bending strength of SFRC is greatly affected by the content of steel fiber and the cut depth. The addition of 0. 2%, 0. 35% and 0. 5% steel fiber increase the bearing capacity of SFRC cut beam by 20. 7%, 38. 9% and 63. 7%, respectively, compared with SFRC cut beam with a cut depth of 25 mm. The bearing capacity of SFRC beam with 40 mm and 55 mm incision depth decrease by 19. 9% and 39. 8%, respectively. - Abstract
Objective To investigate the predictive value of soluble growth-stimulated expressed gene 2 protein (sST2) in predicting sepsis-associated acute kidney injury (SA-AKI). Methods From January 2020 to December 2023, a total of 395 sepsis patients with sepsis from the Emergency Department, Affiliated Hospital of Nantong University were enrolled in this retrospective observational study. These patients were further categorized into two groups: AKI group (114 cases) and non-AKI group (281 cases). Clinical data and laboratory indicators were collected within 24 hours after admission. ROC curve analysis was used to evaluate the diagnostic ability of sST2 for SA-AKI. Logistic regression analysis was performed to assess independent risk factors for the development of SA-AKI. Concurrently, two predictive models for SA-AKI were constructed, and the predictive ability of sST2 for SA-AKI was assessed using the Area Under the Curve (AUC), the Net Reclassification Index (NRI) and the Integrated Discrimination Improvement (IDI). Results T The levels of sST2 were significantly higher in the AKI group compared to the non-AKI group (P < 0.001). The AUC predicted by sST2 (0.839, 95% CI: 0.799 ~ 0.874) in sepsis patients with AKI was significantly superior to other laboratory indicators (P < 0.000 1). An sST2 concen-tration greater than 85 ng/mL independently increased the risk for SA-AKI (OR: 14.315, 95% CI: 5.867 ~ 34.926, P < 0.001). Incorporating sST2 into the prediction model substantially improved the AUC values (0.954 vs. 0.909, P < 0.000 1), NRI( 21.48%, 95% CI: 11.48% ~ 31.49%, P < 0.000 1) and IDI( 8.62%, 95% CI: 5.75% ~ 11.49%, P < 0.000 1). Objective Analyzing the applicability of pipeline simulation technology in energy measurement, and examine the deviation between simulation calculation results and actual pipeline measurement values. Methods Using a closed natural gas pipeline network with multiple gas sources will be as an example, a simulation model with functions for natural gas heat generation, pressure, and flow calculation was established. This model is based on the theoretical foundation of pipeline network simulation for energy measurement. Results The deviation between the simulation results of heat generation and the calculated values of the on-site chromatographic analyzer is within 0.3%, which meets the requirements of the maximum allowable error of assigned heat generation in GB/T 18603—2023 Technical requirement of measuring systems for natural gas. The deviation between the pressure simulation calculation result and the recorded value of the on-site pressure transmitter is within 1.0%, and the deviation between the flow simulation calculation result and the measured value of the on-site flow meter is within 2.0%. Conclusions The energy measurement technology based on pipeline simulation is a feasible method for assigning heat generation values to complex multi-gas source pipelines. With the ongoing depletion of surface and shallow mineral resources, future mining efforts must inevitably venture deeper. As mining depth increases, operations face a "three high and one disturbance" environment, severely affecting normal production activities. Therefore, pressure management has become an essential prerequisite for mine production. The study consolidates current research achievements in 3 areas, including basic theory, monitoring methods, and ground pressure management, to provide foundational guidance for practical mining operations. Basic theory explores the internal mechanical mechanisms of ground pressure activities through dynamic-static load combinations and temperature seepage stress multi-field coupling effects. Monitoring methods discuss the deployment of stress-strain monitoring systems and microseismic monitoring systems, where static stress-strain monitoring and dynamic real-time microseismic monitoring work in tandem to effectively monitor pressure activities during production.
