目的 探讨院内发生的大血管闭塞性卒中行血管内治疗患者的临床特征、发病机制、血管内治疗及预后，为制订院内大血管闭塞性卒中预防策略提供参考信息。 方法 回顾性分析2020年7月—2023年7月因非缺血性卒中原因在徐州市中心医院住院期间发生大血管闭塞性卒中的45例患者（院内组）的临床资料。选择同期通过急诊绿色通道入院的大血管闭塞性卒中患者100例作为对照（院外组）。收集两组患者的基线资料、实验室指标以及血管内治疗相关资料，比较两组人口学信息、危险因素、发病机制及治疗结局。采用多因素logistic回归分析院内发生大血管闭塞性卒中对患者预后的影响。 结果 院内组大血管闭塞性卒中患者的首诊住院科室主要有脑外科（10/45，22.2%）、心脏外科（9/45，20.0%）、血甲疝外科（9/45，20.0%）及胸外科（5/45，11.1%）等，23例（51.1%）与外科手术有关。两组患者性别、年龄、烟酒史比例、高血压、糖尿病、高脂血症、冠心病、心脏瓣膜病、心功能不全及既往卒中病史差异无统计学意义。与院外组患者相比，院内组患者合并心房颤动（53.3% vs. 19.0%，P＜0.001）、恶性肿瘤（15.6% vs. 1.0%，P=0.002）、其他动脉栓塞（15.6% vs. 1.0%，P=0.002）以及围手术期（51.1% vs. 1.0%，P＜0.001）的比例更高，发病时NIHSS评分更高[21.0（14.5～35.0）分 vs. 18.0（13.5～24.5）分，P=0.019]。院内组的白细胞计数（U=1385.000，P＜0.001）、D-二聚体（U=654.500，P＜0.001）及IL-6（U=1376.000，P＜0.001）水平高于院外组，血红蛋白（U=1758.000，P=0.035）、收缩压（t=-2.766，P=0.006）和舒张压（U=1317.500，P＜0.001）水平低于院外组，差异均有统计学意义。院内组与院外组卒中在前后循环的分布上差异无统计学意义，院内组大动脉粥样硬化性、心源性、其他原因大血管闭塞性卒中患者分别有7例（15.6%）、22例（48.9%）、16例（35.6%），院外组分别有63例（63.0%）、 26例（26.0%）、11例（11.0%），差异有统计学意义（χ2=29.432，P＜0.001）。两组行血管内治疗前缺血区与梗死区的体积比以及术后TICI血流分级、出血转化/造影剂渗出及死亡患者比例差异无统计学意义。与院外组比较，院内组发病到血管内治疗时间及再通时间更短，术中使用补救措施更少 （P＜0.05）。在校正了发病时NIHSS评分、心房颤动、心脏瓣膜病、缺血区与梗死区的体积比后，院内卒中与大血管闭塞性卒中血管内治疗患者的良好预后结局仍呈负相关（OR 0.213，95%CI 0.063～0.711，P=0.012）。 结论 院内外发生大血管闭塞性卒中行血管内治疗的患者有着不同的临床特征、发病机制及治疗结局。院内大血管闭塞性卒中患者的神经系统损伤症状较严重，合并基础疾病较多，病因机制复杂，易预后不良。严格把握手术适应证、预防及控制感染、积极干预危险因素可能会减少院内大血管闭塞性卒中的发生。 Abstract: Objective To investigate the clinical characteristics, pathogenesis, endovascular treatment and prognosis of patients treated with endovascular treatment for large vessel occlusive stroke occurring in hospital, and to provide reference information for the prevention strategies of large vessel occlusive stroke in hospital. Methods A retrospective analysis was performed on 45 patients (in-hospital group) who were hospitalized in Xuzhou Central Hospital for non-ischemic stroke from July 2020 to July 2023 and had large vessel occlusive stroke during hospitalization. During the same study period, 100 patients with large vessel occlusive stroke who entered the emergency greenway of our hospital from outside the hospital were used as control cases (out-of-hospital group). Baseline data, laboratory-related indicators, and endovascular treatment-related data were collected to analyze the demographics, risk factors, pathogenesis, and treatment outcomes of the two groups and compared. Multivariate logistic regression was used to analyze the prognostic effects of patients with large vessel occlusive stroke occurring in hospital. Results Patients with in-hospital large vessel occlusive stroke had a wide variety of first diseases, mainly occurring in brain surgery (10/45, 22.2%), cardiac surgery (9/45, 20.0%), blood nail hernia surgery (9/45, 20.0%), and thoracic surgery (5/45, 11.1%), and 23 cases (51.1%) were associated with surgery. There was no statistically significant difference between the two groups in terms of gender, age, history of smoking and alcohol, hypertension, diabetes mellitus, hyperlipemia, coronary artery heart disease, valvular heart disease, cardiac insufficiency and history of previous stroke. Compared with the out-of-hospital group, more patients in the in-hospital group had a combination of atrial fibrillation (53.3% vs. 19.0%, P＜0.001), malignant tumor (15.6% vs. 1.0%, P=0.002), other arterial thrombosis (15.6% vs. 1.0%, P=0.002), and perioperative period (51.1% vs. 1.0%, P＜0.001), and the NIHSS score at the onset of the disease was higher [21.0 (14.5-35.0) points vs. 18.0 (13.5-24.5) points, P=0.019]. The levels of white blood cell count (U=1385.000, P＜0.001), D-dimer (U=654.500, P＜0.001) and IL-6 (U=1376.000, P＜0.001) in the in-hospital group were higher than those in the out-of-hospital group, and the levels of hemoglobin (U=1758.000, P=0.035), systolic blood pressure (t=-2.766, P=0.006) and diastolic blood pressure (U=1317.500, P＜0.001) were lower than those in the out-of-hospital group, and the differences were all statistically significant. The difference between the in-hospital group and the out-of-hospital group in the distribution of anterior and posterior circulation was not statistically significant. There were 7 cases (15.6%), 22 cases (48.9%), and 16 cases (35.6%) of large vessel occlusive stroke with atherosclerosis, cardiogenic type, and other causes in the in-hospital group, and 63 cases (63.0%), 26 cases (26.0%), and 11 cases (11.0%) in the out-of-hospital group, respectively, with a statistically significant difference (χ2=29.432, P

This paper summarized the nursing management for a patient with primary dysmenorrhea treated by auricular acupressure combined with auricular pressure. Under the guidance of TCM theory, auricular acupressure combined with auricular pressure was applied to primary dysmenorrhea based on symptom and pain assessment. The combination of auricular acupressure and auricular pressure was effective to improve the symptoms of dysmenorrhea and patient's quality of life. This paper summarized the practice of acupoint application guided by midnight-noon ebb-flow theory and nursing measures for a patient with hepatitis B and ascites due to cirrhosis of the liver. Key issues included nursing evaluation, routine nursing, application time, operation method and acupoints selection under the guidance of midnight-noon ebb-flow theory. By nursing according to syndrome differentiation and selecting the corresponding time for acupoint application, the symptoms of ascites was relieved and patient’s quality of life was improved. Objective: To study the efficacy of Alteplase in repairing T-lymphocyte mediated brain tissue immunoinflammatory damage after acute ischemic stroke（AIS）. Methods: A total of 100 patients with AIS admitted to Hengshui People's Hospital from June 2022 to June 2023 were selected， and divided into regular group and Alteplase group， with 50 cases in each group， after enroll-ment， the regular group received routine intervention treatment， while the Ateplase group received thrombolytic therapy with Ateplase. Compared changes in neurological function， cerebrovascular reserve function， cerebral hemodynamics， T lymphocytes mediated brain tissue immunoinflammatory damage indicators CD4+T cell subsets ［pro-inflammatory cells （Th1，Th17）， anti-inflammatory cells （Th2， Treg cells）］ and related cytokines levels between two groups of patients， and evaluated clinical efficacy and prognosis. Results: After treatment， NIHSS score of Alteplase group was lower than that of regular group ［（6.57±0.79） points vs （9.34±1.23） points， t=13.400， P<0.05］. CVR and BHI of cerebral vascular reserve in Alteplase group were higher than those in regular group ［（34.54±4.35）% vs （29.89±2.31）%， 1.00±0.23 vs 0.92±0.08， t=6.676， 2.323， P<0.05］， PI was lower than that of regular group （0.60±0.07 vs 0.72±0.08， t=7.982， P<0.05）. Cerebral hemodynamic CBF of Alteplase group was higher than that of regular group ［（49.89±9.76） ml（/100 g/min） vs（ 40.34±7.86） ml（/100 g/min）， t=5.389， P<0.05］. Conclu-sion: In treatment of AIS， Alteplase has effect of repairing the immune inflammation injury of brain tissue mediated by T lymphocytes， alleviating brain injury， improving cerebrovascular reserve function and nerve function， and the prognosis is good. Objective To evaluate the detailed nursing management in nosocomial infection control in the operation room. Methods Totally 70 patients undergoing surgical treatment were divided into the observation group (n=35) and control group (n=35) according to the admission sequence number. Patients in both control and observation received routing nursing management during the hospitalization period, and those in the observation group were given detailed nursing management. The rates of nosocomial infection and postoperative complications among patients were observed. The patient statifacion was evaluated by using a self-made questionnaire. The outcomes of theoretical knowledge and practice skills assessment among nurses were analyzed. Results There were three cases of nosocomial infection reported in the control group. The incidence of postoperative complications in the observation group was lower than that in the control group(P＜0. 05). The score of patient satisfaction assessment in the observation group was higher than that in the control group (P＜0. 01). The scores of theoretical knowledge and practice skills assessment among nurses in the observation group were higher than those in the control group(P＜0. 01). Conclusion The detailed nursing management is an effective tool for reducing risk of nosocomial infection and postoperative complications. The detailed nursing management represents a good opportunity for nurses to grow their knowledge and nursing skills, therefore improving the quality of specialized care. [Objective] The present paper aimed to explore the changing law of twig and leaf size relationship in the vertical structure of woody plants in the dry-hot valley of northwest Guangxi, and provide a theoretical basis for plant protection and vegetation restoration in the dry-hot valley of northwest Guangxi. [Method] The natural secondary forest community in Yachang Forest Farm in the dry-hot valley of northwest Guangxi was studied by using one-way ANOVA, standardized principal axis estimation and correlation analysis to analyze the size difference, allometric growth relationship and correlation relationship among twigs and leaves in different forest layers. [Result] There were significant differences in the traits of most twigs and leaves in different forest layers (P<0.05, the same as below). With the increase of forest layer, twig fresh weight, twig dry weight, twig diameter, total leaf area, leaf fresh weight, leaf dry weight and leaf dry matter content showed an increasing trend, while twig relative water content, branching intensity, individual leaf area, leaf relative water content and specific leaf area showed a decreasing trend. There was a significant positive allometric growth relationship between twig cross-sectional area and total leaf area, and a significant negative allometric growth relationship between twig cross-sectional area and branching density in different forest layers, with a common slope and significant differences in the intercept of the regression equation, indicating that vertical structure changes do not change the ecological tradeoff between branch size, branch number and leaf area, but significantly changed the proportion of resource allocation for twig and leaf traits. Tall forest plants supported fewer twigs per unit area and more total leaf area. Correlation analysis showed that vertical structure changes have a significant impact on plant twig and leaf traits, and the correlation between plant twig and leaf traits was closer in lower and middle forest than in upper forest. [Conclusion] Vertical structural changes do not significantly affect the ecological tradeoffs between leafy traits in woody plants, but significantly affect the biomass configuration of branches and leaves. The upper woody plants per unit area have a larger leaf area, while the lower woody plants support a larger number of branches, which is an ecological strategy adopted by woody plants in the dry-hot valley of northwest Guangxi. The 2023 China Health Report on Spine Degeneration noted a significant increase in lumbar surgery among patients under 35 years old in recent years, indicating a trend towards younger onset of cervical and lumbar diseases. Lumbar intervertebral disc herniation has become a major concern, making the study of disc degeneration pathogenesis and treatment methods clinically significant. At present, human intervertebral disc diseases are primarily diagnosed through imaging due to the challenges of obtaining tissue samples from the spine. Therefore, experimental animals have emerged as alternative research subjects because they are cost-effective, have short experimental cycles, and are easily accessible. Given the structural and physiological differences between human and other animal intervertebral discs, comparing their anatomy and histological characteristics forms the foundation of research into human disc degeneration. The purpose of this paper is to collect and review relevant studies on anatomical and histological structures of intervertebral discs in different animals and conduct a comparative analysis from four aspects, namely, intervertebral disc height, lumbar disc geometry, lumbar disc cartilaginous endplate characteristics, and extracellular matrix components. The results show that humans, kangaroos, sheep, pigs, and rats exhibit similar relative heights between the sixth and seventh cervical vertebrae. Mice possess lumbar disc geometries most akin to humans. Compared to other animals, humans have the thickest cartilaginous endplates and the lowest cell densities. The collagen within the fibrous annulus differs most notably in pigs compared to humans, while water content in the nucleus pulposus is consistent across pigs, sheep, rabbits, rats, and humans. Additionally, this paper describes the commonalities and discrepancies in disc degeneration manifestations between humans and animals, and summarizes modeling methods for disc degeneration in different experimental animals. Ultimately, the aims of this paper is to provide fundamental data for selecting suitable experimental animal models for the study of intervertebral disc degeneration. [ABSTRACT FROM AUTHOR]

The value of electromagnetic load is closely related to the current quench characteristics in the tokamak during plasma disruption. The characteristics include the current quench waveform and current quench rate, which will determine the life of the Tokamak device to a certain extent. The spontaneous disruption data of the HI-2A are statistically analyzed by using three different intervals of 100% -40%, 90% -10% and 80% - 20%, and the statistical results are compared and analyzed. The statistical results show that; under the same definition, not only the average current quenching time and current quenching rate distribution have the least difference between 100%-40% and80%-20%, but also the average current quenching time and current quenching rate statistical distribution almost are the same. In addition to the 80%-20% range, the 100% - 40% range can also be used as a suitable range to provide an approximation of the maximum linear average plasma current quenching rate. [Objective] The objectives of this study are to assess the influence of soil, topography, and crop parameters on the simulation accuracy of aboveground biomass and losses of nitrogen and phosphorus in tea gardens using the DNDC model, to identify the most sensitive parameters for each model output, and to provide guidance for calibrating the model. [Methods] Based on the data of tea garden plot in the Tianmu Lake Basin and DNDC model, qualitative Morris method and quantitative Sobol method were applied to carry out sensitivity analysis on the simulated aboveground biomass, nitrogen and phosphorus loss (including nitrogen and phosphorus leaching and nitrogenous gas emissions) and water leaching of tea garden, and to evaluate the sensitive parameters of each output. The uncertainty of model output was quantified by Monte Carlo method. [Results] The most important parameter affecting aboveground biomass is the grain fraction of total biomass at maturity, the most important parameter affecting nitrogen leaching is soil clay fraction, the most important parameter affecting phosphorus leaching is soil bulk density, the most important parameter affecting water leaching is slope gradient, the most important parameter affecting N2O emission is the initial soil organic carbon (SOC) content, and the most important parameter affecting NO emission and NH3 emission is soil pH value. Among the output results, the uncertainty of phosphorus leaching loss is the largest, and the coefficient of variation is 43.84%. The uncertainty of water leakage is the least, and the coefficient of variation is 2.36%. Compared with Sobol method, Morris method is convenient and time-saving, while Sobol method is accurate and comprehensive. The correlation comparison of parameter sensitivity ranking obtained by the two methods is 0.86, which has certain substitutability. [Conclusion] Before calibrating the model, it is advisable to employ the Morris method for preliminary identification of sensitive parameters and subsequently utilize the Sobol method for precise quantification. This approach enables efficient and accurate screening of the model′s sensitive parameters. Based on this premise, the correction of sensitive crop parameters is prioritized while keeping non-sensitive parameters fixed, followed by addressing sensitive soil parameters. The results of this study can provide scientific support for the localization of DNDC model and improve the simulation accuracy and efficiency of the tea garden ecosystem in the Tianmu Lake Basin. In recent years, rapid progress has been made in strategies for the prevention and treatment of hepatitis C virus （HCV） in organ transplant candidates and recipients, and although HCV infections no longer threaten transplantation outcomes in liver or non-hepatic solid organ transplantation, they remain a focus of research. Since hepatitis C is still a leading cause of death worldwide due to decompensated cirrhosis, liver failure, and hepatocellular carcinoma, appropriate organ transplantation is needed to improve survival rate and quality of life. With the increase in HCV-positive solid organ donors in recent years and the fact that the demand for organs still greatly exceeds organ supply, as well as the development of direct-acting antiviral agents, transplantation of HCV-viraemic organs into HCV-naïve recipients will significantly increase transplantation rates and reduce waitlist mortality. The efficacy of current HCV therapies has created an important opportunity to improve the survival rate of patients with end-organ failure by enhancing access to organ transplantation and reducing waitlist mortality. Objective The aim of this study is to explore the clinical effect of applying Traditional Chinese Medicine prepared with different solvents to Shenque(RN8) acupoint for the treatment of constipation. Methods A total of 60 patients with constipation during their hospital stay between Mary 2020 and Mary 2021 were included and randomly assigned into control group and treatment group, with 30 cases in each group. The control group and treatment group received the therapy of applying the TCM drugs prepared with warm water and rice wine respectively to Shenque(RN8) acupoint for 4 h once a day and there were two rounds of treatment, with each round lasting 7days. Results The total effective rate of treatment group and control group was 96. 67% and 73. 33% respectively and there was statistically significant difference between the two groups (P＜0. 05). Conclusion Application of the TCM drugs prepared with rice wine to Shenque(RN8) acupoint for the treatment of patients with constipation during their hospital stay is clinically effective and safe and it can be promoted to use in clinical nursing. (目的 探讨不同溶剂调制药物贴敷神阙穴治疗便秘的临床疗效。方法 选取2020年5月—2021年5月北京中医药大学东方医院心内科住院期间合并便秘患者, 共选择病例60例, 按照随机对照法分为对照组和治疗组, 各30例。对照组给予温水调和中药剂贴敷治疗, 治疗组给予黄酒调和中药剂贴敷治疗, 分别给予神阙穴贴敷4 h, 1次/d, 7 d为1个疗程, 共治疗2个疗程。 根据症状改善情况评价干预效果。结果 治疗组总有效率为96. 67%(29/30), 对照组总有效率为73. 33%(22/30), 2组差异有统计学意义(P＜0. 05)。结论 采用黄酒调和中药剂贴敷神阙穴治疗住院期间便秘患者的临床疗效较好, 安全性良好, 可以在临床护理中推广使用。)

This paper analyzed the clinical data of 5 patients with skin rash caused by pegylated interferon α-2a combined with ribavirin therapy of chronic viral hepatitis C. According to the clinical symptoms, patients were given internal administration of Chinese herbal medicine decoction combined with external wash of Chinese herbal medicine decoction. Comprehensive interventions including skin care, infection prevention, nutrition support and psychological nursing were carried out during the treatment. This paper summarized the nursing experience of a case of phlebitis caused by peripheral intravenous infusion of amiodarone hydrochloride treated with Traditional Chinese Medicine collapse stains. The patient was treated with Traditional Chinese Medicine collapse stains, and interventions on exercise and mental health were carried out to improve the rehabilitation. Traditional Chinese Medicine collapse stains, characterized by easy-to-operate, safe, wide scope of application, comfort and high rate of acceptance among patients, is effective in treating phlebitis caused by amiodarone hydrochloride injection.
