This study evaluated the prevalence of positive screening for depression in Brazil and its associated factors. We used data from National Health Survey 2019 (Pesquisa Nacional de Saúde - PNS), a population-based survey with 88,531 adults. The Patient Health Questionnaire (PHQ-9) was used with two scoring methods, the algorithm and the cutoff point≥10. The variables included sociodemographic characteristics. The prevalence ratios and 95% confidence intervals (95%CI) were estimated using Poisson regression. The positive screening for depression was 10.8% (95%CI: 10.4-11.0), at the cutoff point ≥10 and 5.7% (95%CI: 5.4-6.0) for algorithm. Significant differences were found in prevalence in some Brazilian states. Multivariable analyses showed that being female, black, under 70 years of age, having little education, being single, and living in an urban area were independently associated with a depressive symptoms. The highest association was found in the states of Sergipe, Goiás, Piauí, Espírito Santo, São Paulo, Alagoas and lowest in Pará, Mato Grosso and Maranhão. The prevalence of positive screening for depression in Brazil has increased in recent years. More investment in mental health resources is necessary and surveys such as the PNS should be continued. Resumo Este estudo avaliou a prevalência de triagem positiva para depressão no Brasil e seus fatores associados. Utilizou-se dados da Pesquisa Nacional de Saúde 2019 (PNS), um inquérito de base populacional com 88.531 adultos. Para avaliar os sintomas depressivos utilizou-se o Patient Health Questionnaire (PHQ-9) com dois métodos de pontuação: o algoritmo e o ponto de corte ≥10. As variáveis incluíram características sociodemográficas. Utilizou-se a regressão de Poisson para obter razões de prevalência, com intervalo de confiança de 95% (IC95%). A triagem positiva para depressão foi de 10,8% (IC95%: 10,4-11,0), no ponto de corte ≥10 e 5,7% (IC95%: 5,4-6,0) para o algoritmo. Houve diferenças significativas na prevalência entre alguns estados brasileiros. Análises multivariadas mostraram que ser do sexo feminino, negro, ter menos de 70 anos, ter baixa escolaridade, ser solteiro e residir em área urbana estiveram independentemente associados a sintomas depressivos. A maior associação foi encontrada nos estados de Sergipe, Goiás, Piauí, Espírito Santo, São Paulo, Alagoas e a menor no Pará, Mato Grosso e Maranhão. A prevalência de triagem positiva para depressão no Brasil tem aumentado nos últimos anos. É necessário mais investimento em saúde mental e pesquisas como a PNS devem ser feitas continuamente.

People with severe mental illness have a mortality rate higher than the general population, living an average of 10-20 years less. Most studies of mortality among people with severe mental illness have occurred in high-income countries (HICs). We aimed to estimate all-cause and cause-specific relative risk (RR) and excess mortality rate (EMR) in a nationwide cohort of inpatients with severe mental illness compared with inpatients without severe mental illness in a middle income country, Brazil.This national retrospective cohort study included all patients hospitalised through the Brazilian Public Health System (Sistema Único de Saúde [SUS]-Brazil) between Jan 1, 2000, and April 21, 2015. Probabilistic and deterministic record linkages integrated data from the Hospital Information System (Sistema de informações Hospitalares) and the National Mortality System (Sistema de Informação sobre Mortalidade). Follow-up duration was measured from the date of the patients' first hospitalisation until their death, or until April 21, 2015. Severe mental illness was defined as schizophrenia, bipolar disorder, or depressive disorder by ICD-10 codes used for the admission. RR and EMR were calculated with 95% CIs, comparing mortality among patients with severe mental illness with those with other diagnoses for patients aged 15 years and older. We redistributed deaths using the Global Burden of Diseases, Injuries, and Risk Factors Study methodology if ill-defined causes of death were stated as an underlying cause.From Jan 1, 2000, to April 21, 2015, 72 021 918 patients (31 510 035 [43·8%] recorded as male and 40 974 426 [56·9%] recorded as female; mean age 41·1 (SD 23·8) years) were admitted to hospital, with 749 720 patients (372 458 [49·7%] recorded as male and 378 670 [50·5%] as female) with severe mental illness. 5 102 055 patient deaths (2 862 383 [56·1%] recorded as male and 2 314 781 [45·4%] as female) and 67 485 deaths in patients with severe mental illness (39 099 [57·9%] recorded as male and 28 534 [42·3%] as female) were registered. The RR for all-cause mortality in patients with severe mental illness was 1·27 (95% CI 1·27-1·28) and the EMR was 2·52 (2·44-2·61) compared with non-psychiatric inpatients during the follow-up period. The all-cause RR was higher for females and for younger age groups; however, EMR was higher in those aged 30-59 years. The RR and EMR varied across the leading causes of death, sex, and age groups. We identified injuries (suicide, interpersonal violence, and road injuries) and cardiovascular disease (ischaemic heart disease) as having the highest EMR among those with severe mental illness. Data on ethnicity were not available.In contrast to studies from HICs, inpatients with severe mental illness in Brazil had high RR for idiopathic epilepsy, tuberculosis, HIV, and acute hepatitis, and no significant difference in mortality from cancer compared with inpatients without severe mental illness. To investigate overall and sex-related characteristics associated with the risk of death by suicide within 365 days of discharge in patients admitted to psychiatric hospitals in the Brazilian Unified Health System (SUS).A non-concurrent prospective cohort of adult patients admitted to psychiatric hospitals in the SUS, from 2002 to 2015. Patients were stratified according to sociodemographic and clinical variables. Adjusted hazard ratios (aHRs) of suicide within 365 days of discharge were estimated using Cox proportional hazard regression models.This sample comprised 1,228,784 adult patients admitted to psychiatric hospitals. Of these, 3201 died by suicide within 365 days of discharge. The risk of suicide was positively associated with male sex, age between 18 and 29 years, living in the South region, and living in rural or intermediate municipalities. The highest risk of suicide was among patients with depressive disorders (aHR, 3.87; 95%CI, 3.41-4.38) follow by opioid-related disorders (aHR, 2.71; 95%CI, 2.00-3.67), particularly among female patients.Patients with a psychiatric hospital admission should have access to mental health care services immediately after discharge and in the long term. Findings of this study may support suicide prevention policies and have implications for clinical decisions related to patient discharge and follow-up.

Introduction. The Patient Health Questionnaire (PHQ-9) is one of the most validated tools used to detect depressive episodes in Brazil. Objective. This study investigates the psychometric properties of the PHQ-9 using the Item Response Theory. Method. We used the gradual response model to assess depression in 764 residents of Brazilian rural communities of descended from slaves (quilombos) from the county of Vitoria da Conquista, state of Bahia, Brazil, who had responded to PHQ-9. We estimated the parameters for item discrimination and difficulty. Results. The items of the PHQ-9 showed the ability to discriminate from moderate to very high. The items evaluating thoughts of hurting oneself and death showed the greatest discrimination while feeling depressed showed the lowest discrimination. Discussion and conclusion. The Item Response Theory enables advances in the analysis of the psychometric properties of the screening tools assessing depression, and indicates that PHQ-9 can be used in rural populations in Brazil.

OBJECTIVE To characterize the profile of patients hospitalized for mental and behavioral disorders by the Unified Health System (SUS) in Brazil between 2000 and 2014, and to verify how aspects of the new mental health policy influenced the rate of hospitalized patients in that period. METHODS Non-concurrent prospective cohort study using secondary data from inpatients with a primary diagnosis of mental and behavioral disorders between 01/01/2000 and 12/31/2014. Sociodemographic, clinical, and hospital characteristics variables were selected. Overall rates of hospitalized patients were calculated according to reason for admission, type of hospital, legal nature, and number of admissions per year for each patient. The association between rates of hospitalized patients, number of psychiatric beds per year, and number of Psychosocial Care Centers per year were tested. RESULTS We selected a total of 1,549,298 patients, whose most frequent diagnoses on first admission were psychoactive substance use disorders, followed by schizophrenia and mood disorders. The median of hospitalizations per patient was 1.9 and the length of stay per patient was 29 days. The overall rate of hospitalized patients was reduced by almost half in the period. The number of beds per year was positively associated with the rates of hospitalized patients; the number of CAPS per year was negatively associated with some rates of hospitalized patients. CONCLUSION Even in the face of adversity, the National Mental Health Policy has advanced in its goal of progressively reducing hospital beds and increasing the supply of substitute services such that both strategies were associated with the reduced inpatient rates. But the changes were felt with greater intensity in the first years of the policy’s implementation, becoming less pronounced in recent years. RESUMO OBJETIVO Caracterizar o perfil dos pacientes que foram internados por transtornos mentais e comportamentais pelo Sistema Único de Saúde (SUS) no Brasil entre 2000 e 2014, bem como verificar como aspectos da nova política de saúde mental influenciaram a taxa de pacientes internados no referido período. MÉTODOS Estudo de coorte prospectiva não concorrente utilizando dados secundários de pacientes internados com diagnóstico primário de transtornos mentais e comportamentais entre 01/01/2000 e 31/12/2014. Foram selecionadas variáveis sociodemográficas, clínicas e de características do hospital, além disso, foram calculadas as taxas gerais de pacientes internados segundo motivo de internação, tipo de hospital, natureza jurídica e número de internações de cada paciente por ano. Foi testada a associação entre taxas de pacientes internados, número de leitos psiquiátricos por ano e número de Centros de Atenção Psicossocial por ano. RESULTADOS Foram selecionados 1.549.298 pacientes dos quais os diagnósticos mais frequentes na primeira internação foram os transtornos devidos ao uso de substâncias psicoativas, seguidos por esquizofrenia e transtornos de humor. A mediana de internações por paciente foi de 1,9 e a de tempo de internação por paciente foi de 29 dias. A taxa geral de pacientes internados foi reduzida à quase metade no período. O número de leitos por ano apresentou associação positiva com as taxas de pacientes internados, e o número de CAPS por ano teve associação negativa com algumas taxas de pacientes internados. CONCLUSÃO Verificou-se que, mesmo diante de um contexto de adversidades, a Política Nacional de Saúde Mental avançou em suas metas de reduzir progressivamente os leitos hospitalares e aumentar a oferta de serviços substitutivos de tal modo que ambas as estratégias foram associadas à redução das taxas de pacientes internados. Contudo, as mudanças foram percebidas com maior intensidade nos primeiros anos de implantação da política, tornando-se menos pujante nos últimos anos.

Background Depression is one of the major causes of disability worldwide. The objective of this study was to analyze the results of the Global Burden of Disease Study 2017 (GBD-2017) for depressive disorders in Brazil and its Federated Units (FUs) in 1990 and 2017. Methods We used GBD-2017 study methodology to evaluate the prevalence estimates, the disability-adjusted life-year (DALY), and the years lived with disability (YLDs) for depressive disorders, which include major depressive disorder and dysthymia. The YLD estimates and the position of these disorders in the DALY and YLD rankings were compared to those of seven other countries. The observed versus expected YLD, based on the sociodemographic index (SDI), were compared. Results In GBD-2017, the prevalence of depressive disorders in Brazil was 3.30% (95% uncertainty interval [UI]: 3.08 to 3.57), ranging from 3.79% (3.53 to 4.09) in Santa Catarina to 2.78% in Pará (2.56 to 3.03), with significant differences between the Federated Units. From 1990 to 2017, there was an increase in number of YLD (55.19%, 49.57 to 60.73), but a decrease in the age-standardized rates (− 9.01%, − 11.66 to − 6.31). The highest proportion of YLD was observed in the age range of 15–64 years and among females. These disorders rank 4th and 13th as leading causes of YLD and DALY, respectively, in Brazil. In the other countries evaluated, the ranking of these disorders in the YLD classification was close to Brazil’s, while in the DALY classification, there was higher variability. All countries had YLD rates similar to the overall rate. The observed/expected YLD ratio ranged from 0.81 in Pará to 1.16 in Santa Catarina. Morbidity of depressive disorders was not associated with SDI. Conclusions Depressive disorders have been responsible for a high disability burden since 1990, especially in adult women living in the Southern region of the country. The number of people affected by these disorders in the country tends to increase, requiring more investment in mental health aimed at advancements and quality of services. The epidemiological studies of these disorders throughout the national territory can contribute to this planning and to making the Brazilian health system more equitable.

Objetivo: Descrever as projeções do IHME para a COVID-19 no Brasil e seus estados e discutir acurácia e implicações em diferentes cenários. Métodos: Descrevemos e estimamos a acurácia das previsões para o Brasil, comparando-as com as mortes cumulativas observadas. Resultados: A projeção prevê 192.511 mortes causadas pela pandemia até 1 de dezembro de 2020. O relaxamento continuado do isolamento físico obrigatório, apesar do aumento continuado dos óbitos, pode causar >63.000 mortes adicionais; o rápido aumento no uso de máscara pode reduzir o número para ~25.000. Vários estados poderão ter que reinstituir restrições. As diferenças entre as projeções do IHME até 6 semanas e as mortes registradas variaram de -11% a 48% para o Brasil. Conclusões: As projeções de curto a médio prazo do IHME fornecem informações válidas para informar os gestores de saúde, autoridades eleitas e a sociedade em geral. Elas sugerem curso prolongado, grande mortalidade e prováveis novas restrições.

O objetivo do presente estudo foi analisar o consumo, seja moderado ou excessivo, de álcool entre residentes de comunidades quilombolas de Vitória da Conquista (BA), bem como os fatores associados. Estudo transversal envolvendo 750 indivíduos com idade ≥ 18 anos, entrevistados em 2011, utilizando uma versão adaptada do questionário da Pesquisa Nacional de Saúde. O consumo de bebidas alcoólicas foi classificado em abstenção, uso moderado ou excessivo, e analisado por meio do modelo de regressão multinomial. Observou-se o consumo entre 41,5% dos participantes (IC95%%: [37,8; 44,98]); sendo identificados na categoria consumo excessivo e, na categoria consumo moderado, 10,7% e 30,8%, respectivamente. O consumo moderado estava relacionado inversamente com a idade, sendo mais frequente entre indivíduos brancos ou pardos, os mais escolarizados, aqueles em atividade laborativa e entre fumantes. O consumo excessivo aumentou com o grau de escolaridade, sendo mais prevalente entre os que tinham pelo menos 8 anos de estudo, os fumantes e os que exerciam atividade laborativa. O estudo mostra que nessas comunidades existem diferenças segundo as características sociodemográficas dos diversos perfis de consumidores de álcool, e que estas diferenças devem ser consideradas na elaboração de propostas para promoção de hábitos saudáveis. The scope of the study was to assess moderate and excessive alcohol consumption and associated factors among residents of Quilombola Communities in Vitória da Conquista in the State of Bahia. It involved a cross-sectional study among 750 individuals aged ≥ 18 years interviewed in 2011 using an adapted version of the National Survey of Health Questionnaire. Alcohol consumption was classified into abstemious, moderate and heavy drinking categories and was analyzed using the multinomial regression model. Consumption was observed among 41.5% of participants (95% CI [37.8, 44.98]); 10.7% being identified in the excessive consumption category and 30.8% in the moderate consumption category. Moderate consumption was inversely associated with age and is more common among whites and mulattoes, the better educated, the gainfully employed and among smokers. Heavy consumption increased with level of education, being more prevalent among those who had at least 8 years of schooling, smokers and the gainfully employed. The study shows that in these communities there are differences in the sociodemographic characteristics among the profiles of alcohol drinkers and these differences should be taken into account when drawing up proposals for the promotion of healthy habits.

Background Political, economic, and epidemiological changes in Brazil have affected health and the health system. We used the Global Burden of Disease Study 2016 (GBD 2016) results to understand changing health patterns and inform policy responses. Methods We analysed GBD 2016 estimates for life expectancy at birth (LE), healthy life expectancy (HALE), all-cause and cause-specific mortality, years of life lost (YLLs), years lived with disability (YLDs), disability-adjusted life-years (DALYs), and risk factors for Brazil, its 26 states, and the Federal District from 1990 to 2016, and compared these with national estimates for ten comparator countries. Findings Nationally, LE increased from 68·4 years (95% uncertainty interval [UI] 68·0–68·9) in 1990 to 75·2 years (74·7–75·7) in 2016, and HALE increased from 59·8 years (57·1–62·1) to 65·5 years (62·5–68·0). All-cause age standardised mortality rates decreased by 34·0% (33·4–34·5), while all-cause age-standardised DALY rates decreased by 30·2% (27·7–32·8); the magnitude of declines varied among states. In 2016, ischaemic heart disease was the leading cause of age-standardised YLLs, followed by interpersonal violence. Low back and neck pain, sense organ diseases, and skin diseases were the main causes of YLDs in 1990 and 2016. Leading risk factors contributing to DALYs in 2016 were alcohol and drug use, high blood pressure, and high body-mass index. Interpretation Health improved from 1990 to 2016, but improvements and disease burden varied between states. An epidemiological transition towards non-communicable diseases and related risks occurred nationally, but later in some states, while interpersonal violence grew as a health concern. Policy makers can use these results to address health disparities Antecedentes As mudanças políticas, econômicas e epidemiológicas no Brasil têm afetado a saúde e o sistema de saúde. Nós usou os resultados do Global Burden of Disease Study 2016 (GBD 2016) para entender a mudança nos padrões de saúde e informar respostas políticas. Métodos Analisamos as estimativas do GBD 2016 para expectativa de vida ao nascer (LE), expectativa de vida saudável (HALE), todas as causas e mortalidade por causa específica, anos de vida perdidos (YLLs), anos vividos com incapacidade (YLDs), anos de vida ajustados por incapacidade (DALYs) e fatores de risco para o Brasil, seus 26 estados e o Distrito Federal de 1990 a 2016, e comparou-os com estimativas nacionais para dez países comparadores. Achados Nacionalmente, LE aumentou de 68,4 anos (intervalo de incerteza de 95% [UI] 68,0–68,9) em 1990 para 75,2 anos (74·7–75·7) em 2016, e a HALE aumentou de 59·8 anos (57·1–62·1) para 65·5 anos (62·5–68·0). As taxas de mortalidade padronizadas por idade por todas as causas diminuíram em 34,0% (33,4–34,5), enquanto as taxas de DALY padronizadas por idade por todas as causas diminuíram em 30,2% (27,7–32,8); a magnitude dos declínios variou entre os estados. Em 2016, a cardiopatia isquêmica foi o principal causa de YLLs padronizados por idade, seguido por violência interpessoal. Lombalgia e dor no pescoço, órgão dos sentidos doenças e doenças de pele foram as principais causas de YLDs em 1990 e 2016. Principais fatores de risco que contribuem para Os DALYs em 2016 foram uso de álcool e drogas, pressão alta e alto índice de massa corporal. Interpretação A saúde melhorou de 1990 a 2016, mas as melhorias e a carga de doenças variaram entre os estados. RESUMO: Introdução: O uso de álcool é um dos principais fatores de risco preveníveis para mortalidade ou incapacidade prematuras. Objetivo: Descrever as estimativas de mortalidade e anos de vida perdidos por morte prematura (YLL) por cirrose, câncer hepático e transtornos devidos ao uso de álcool no Brasil e suas unidades da federação (UFs), em 1990 e 2015. Métodos: Estudo descritivo com dados do estudo de Carga Global de Doenças (2015) e do Sistema de Informações sobre Mortalidade (SIM). Modelos estatísticos foram empregados para obter estimativas corrigidas de mortalidade pelas causas selecionadas. As taxas de mortalidade foram padronizadas por idade (TMPI). Resultados: Em 1990, foram estimados 16.226 óbitos para as 3 condições (17,0/100 mil habitantes), enquanto em 2015 foram 28.337 (15,7/100 mil habitantes). Houve redução da mortalidade (por 100 mil habitantes) por cirrose (de 11,4 para 9,5) e estabilidade por câncer hepático (1,5 e 1,9) e transtornos devidos ao uso de álcool (4,1 e 4,3). As TMPI foram 5,1 vezes maiores entre os homens, e as 5 UFs com maiores TMPI e YLL foram da Região Nordeste: Sergipe, Ceará, Pernambuco, Paraíba e Alagoas. As taxas de mortalidade e de YLL pelas três condições estudadas ascenderam no ranking das causas de óbito, em ambos os sexos, exceto a cirrose no feminino. Conclusão: As três condições estudadas são responsáveis por importante carga de mortalidade prematura no Brasil, principalmente entre homens e residentes na região nordeste. Esses resultados reforçam a necessidade de políticas públicas para o enfrentamento ao consumo nocivo do álcool no Brasil. ABSTRACT: Introduction: Alcohol use is one of the main preventable risk factors affecting mortality and premature disability. Objective: To describe the estimates of mortality and years of life lost as a result of premature death (YLL) due to cirrhosis, liver cancer, and disorders attributed to alcohol use in Brazil and its federated units in 1990 and 2015. Methods: Descriptive study using data from the Global Burden of Disease Study (2015) and the Mortality Information System (SIM). Statistical models were used to obtain corrected mortality estimates for selected causes. Rates were standardized by age. Results: In 1990, 16,226 deaths were estimated for the three conditions (17.0/100 thousand inhabitants), while in 2015 there were 28,337 deaths (15.7/100 thousand inhabitants). There was a reduction in mortality (per 100 thousand) due to cirrhosis (from 11.4 to 9.5), stability in mortality rates related to liver cancer (1.5 and 1.9), and stability in mortality rates caused by alcohol use disorders (4.1 and 4.3). Mortality rates were 5.1 times higher among men, and the five states with the highest mortality rates and YLL were from the Northeast Region: Sergipe, Ceará, Pernambuco, Paraíba, and Alagoas. Mortality and YLL rates for the three conditions studied increased in the ranking of causes of death in both sexes, with the exception of cirrhosis in the female population. Conclusion: The three conditions studied are responsible for a significant burden of premature mortality in Brazil, especially among men and residents of the northeast region. These results reinforce the urgent need for public policies that address harmful alcohol consumption in Brazil.

RESUMO: Introdução: Os transtornos mentais e decorrentes do uso de substâncias psicoativas (TM) são altamente prevalentes, gerando elevado custo social e econômico. Objetivo: Descrever a carga dos TM no Brasil e Unidades Federativas (UFs), em 1990 e 2015. Métodos: Estudo descritivo da carga de doença dos TM, por meio de estimativas padronizadas por idade do Global Burden of Disease Study 2015: anos de vida perdidos por morte prematura (YLL); anos vividos com incapacidade (YLD); e anos de vida perdidos por morte ou incapacidade (DALY=YLL+YLD). Resultados: No Brasil, apesar da baixa taxa de mortalidade, observa-se alta carga para os TM desde 1990, com elevados YLD. Em 2015, esses transtornos foram responsáveis por 9,5% do total de DALY, ocupando a 3ª e a 1ª posições na classificação de DALY e YLD, respectivamente, com destaque para os transtornos depressivos e de ansiedade. Os transtornos decorrentes do uso de drogas apresentaram a maior elevação das taxas de DALY entre 1990 e 2015 (37,1%). A maior proporção de DALY ocorreu na idade adulta e no sexo feminino. Não houve diferenças substanciais na carga dos TM entre as UFs. Conclusão: Apesar da baixa mortalidade, os TM são altamente incapacitantes, indicando necessidade de ações preventivas e protetivas, principalmente na atenção primária em saúde. A homogeneidade das estimativas em todas as UFs, obtidas a partir de estudos realizados majoritariamente nas regiões Sul e Sudeste, provavelmente não reflete a realidade do Brasil, e indica necessidade de estudos em todas as regiões do país. ABSTRACT: Introduction: Mental and substance use disorders (MD) are highly prevalent and have a high social and economic cost. Objective: To describe the burden of disease attributable to mental and substance use disorders in Brazil and Federated Units in 1990 and 2015. Methods: Descriptive study of the burden of mental and substance use disorders, using age-standardized estimates from the Global Burden of Disease Study 2015: years of life lost due to premature mortality (YLL); years lived with disability (YLD); and disability-adjusted life year (DALY=YLL+YLD). Results: In Brazil, despite low mortality rates, there has been a high burden for mental and substance use disorders since 1990, with high YLD. In 2015, these disorders accounted for 9.5% of all DALY, ranking in the third and first position in DALY and YLD, respectively, with an emphasis on depressive and anxiety disorders. Drug use disorders had their highest increase in DALY rates between 1990 and 2015 (37.1%). The highest proportion of DALY occurred in adulthood and in females. There were no substantial differences in burden of mental and substance use disorders among Federated Units. Conclusion: Despite a low mortality rate, mental and substance use disorders are highly disabling, which indicates the need for preventive and protective actions, especially in primary health care. The generalization of estimates in all the Federated Units obtained from studies conducted mostly in the south and southeast regions probably does not reflect the reality of Brazil, indicating the need for studies in all regions of the country.

Alcohol use is one of the main preventable risk factors affecting mortality and premature disability.To describe the estimates of mortality and years of life lost as a result of premature death (YLL) due to cirrhosis, liver cancer, and disorders attributed to alcohol use in Brazil and its federated units in 1990 and 2015.Descriptive study using data from the Global Burden of Disease Study (2015) and the Mortality Information System (SIM). Statistical models were used to obtain corrected mortality estimates for selected causes. Rates were standardized by age.In 1990, 16,226 deaths were estimated for the three conditions (17.0/100 thousand inhabitants), while in 2015 there were 28,337 deaths (15.7/100 thousand inhabitants). There was a reduction in mortality (per 100 thousand) due to cirrhosis (from 11.4 to 9.5), stability in mortality rates related to liver cancer (1.5 and 1.9), and stability in mortality rates caused by alcohol use disorders (4.1 and 4.3). Mortality rates were 5.1 times higher among men, and the five states with the highest mortality rates and YLL were from the Northeast Region: Sergipe, Ceará, Pernambuco, Paraíba, and Alagoas. Mortality and YLL rates for the three conditions studied increased in the ranking of causes of death in both sexes, with the exception of cirrhosis in the female population.The three conditions studied are responsible for a significant burden of premature mortality in Brazil, especially among men and residents of the northeast region. These results reinforce the urgent need for public policies that address harmful alcohol consumption in Brazil.

Objective To assess correlates of hepatitis B among adults with mental illness under care in Brazil. Method Cross-sectional national multicenter study of 2206 patients with mental illnesses randomly selected from 26 public mental health services. Sociodemographic and behavioral data were obtained from face-to-face interviews and psychiatric diagnoses from medical charts. Serology testing was conducted, and prevalence rate ratios were estimated by log-binomial regression. Results The weighted prevalence rates of current hepatitis B virus (HBV) infection (HBsAg +) and previous HBV exposure (anti-HBc +) were 2.0% [95% confidence interval (CI): 1.5%–2.7%] and 17.1% (95% CI: 16.0%–19.0%), respectively. Correlates of HBsAg + included male gender, younger age (18–29 years), unstable place of residence, intellectual disability, main psychiatric diagnosis of dementia, presence of other medical comorbitidy, use of alcohol/drugs during sex, more than one sexual partner and use of cocaine. Correlates of anti-HBc + included male gender, older age (≥ 30 years), black skin color, lower education, unstable place of residence, currently hospitalized, intellectual disability, history of any sexually transmitted disease or syphilis, poor HIV knowledge, history of imprisonment and sexual violence. Conclusions Hepatitis B is an important comorbidity among psychiatric patients in Brazil. Screening for HBV, effective prevention and intervention strategies, including universal HBV immunization, should be routine practices in these mental health services.

OBJETIVO: Estimar a prevalência e fatores associados à triagem positiva para episódio depressivo maior (EDM) em comunidades quilombolas do estado da Bahia, Brasil. MÉTODOS: Estudo populacional, transversal, com 764 participantes selecionados aleatoriamente em cinco comunidades quilombolas de Vitória da Conquista, Bahia, Brasil. A depressão é definida por 10 ou mais pontos e presença de cinco ou mais sintomas no Patient Health Questionnaire (PHQ-9). Utilizou-se a regressão de Poisson para obter razões de prevalência (RP), com intervalo de confiança de 95% (IC95%). RESULTADOS: A triagem foi positiva para EDM em 12% dos participantes (IC95%: 10 a 14), mas apenas 2,3% das pessoas relataram diagnóstico prévio. A triagem para depressão associou-se a autoavaliação da saúde ruim ou muito ruim (RP = 1,14; IC95%: 1,04 a 1,26), doenças crônicas (RP = 1,08; IC95%: 1,04 a 1,13), sedentarismo (RP = 1,06; IC95%: 1,01 a 1,11) e acesso razoável (RP = 1,07; IC95%: 1,01 a 1,13) ou ruim aos serviços de saúde (RP = 1,07; IC95%: 1,02 a 1,12). CONCLUSÕES: A prevalência de EDM na população quilombola foi semelhante à da população geral brasileira. A relação entre triagem para EDM e pior acesso aos serviços de saúde indica perda de oportunidade de diagnóstico precoce. Ações públicas de combate à desigualdade social e de saúde devem ser implantadas para garantir a equidade nessas comunidades.

Mental and substance use disorders (MD) are highly prevalent and have a high social and economic cost.To describe the burden of disease attributable to mental and substance use disorders in Brazil and Federated Units in 1990 and 2015.Descriptive study of the burden of mental and substance use disorders, using age-standardized estimates from the Global Burden of Disease Study 2015: years of life lost due to premature mortality (YLL); years lived with disability (YLD); and disability-adjusted life year (DALY=YLL+YLD).In Brazil, despite low mortality rates, there has been a high burden for mental and substance use disorders since 1990, with high YLD. In 2015, these disorders accounted for 9.5% of all DALY, ranking in the third and first position in DALY and YLD, respectively, with an emphasis on depressive and anxiety disorders. OBJECTIVE To estimate factors associated to illicit drug use among patients with mental illness in Brazil according to gender. METHODS A cross-sectional representative sample of psychiatric patients (2,475 individuals) was randomly selected from 11 hospitals and 15 public mental health outpatient clinics. Data on self-reported illicit drug use and sociodemographic, clinical and behavioral characteristics were obtained from face-to-face interviews. Logistic regression was used to estimate associations with recent illicit drug use. RESULTS The prevalence of any recent illicit drug use was 11.4%. Men had higher prevalence than women for all substances (17.5% and 5.6%, respectively). Lower education, history of physical violence, and history of homelessness were associated with drug use among men only; not professing a religion was associated with drug use in women only. For both men and women, younger age, current hospitalization, alcohol and tobacco use, history of incarceration, younger age at sexual debut, and more than one sexual partner were statistically associated with illicit drug use. CONCLUSIONS Recent illicit drug use among psychiatric patients is higher than among the general Brazilian population and it is associated with multiple factors including markers of psychiatric severity. Our data indicate the need for the development of gender-based drug-use interventions among psychiatric patients in Brazil. Integration of substance use treatment strategies with mental health treatment should be a priority.

OBJECTIVE: To assess individual and/or health service factors associated with patients returning for results of HIV or sexually transmitted infection (STI) tests in mental health centers. METHODS: Cross-sectional national multicenter study among 2,080 patients randomly selected from 26 Brazilian mental health centers in 2007. Multilevel logistic regression was used to assess the effect of individual (level 1) and mental health service characteristics (level 2) on receipt of test results. RESULTS: The rate of returning HIV/STI test results was 79.6%. Among health service characteristics examined, only condom distribution was associated with receiving HIV/STI test results, whereas several individual characteristics were independently associated including living in the same city where treatment centers are; being single; not having heard of AIDS; and not having been previously HIV tested. CONCLUSIONS: It is urgent to expand HIV/STI testing in health services which provide care for patients with potentially increased vulnerability to these conditions, and to promote better integration between mental health and health services. OBJETIVO: Analisar fatores associados ao recebimento de resultados de exames sorológicos para HIV e outras infecções sexualmente transmissíveis (IST) entre pacientes psiquiátricos. MÉTODOS: Estudo de corte transversal multicêntrico nacional de 2.080 participantes selecionados aleatoriamente dentre 26 instituições de saúde mental brasileiras em 2007. O efeito do indivíduo (nível 1) e dos serviços de saúde mental (nível 2) no recebimento dos resultados dos exames foi avaliado utilizando-se regressão logística multinível. RESULTADOS: A proporção de retorno dos resultados de exames HIV/IST foi de 79,6%. Entre as características individuais, houve associação do desfecho com: viver na mesma cidade onde se encontravam os serviços, ser solteiro, não ter ouvido falar sobre aids e não ter sido previamente testado para HIV. Entre as características dos serviços de saúde, apenas distribuição de preservativos esteve associada ao recebimento dos resultados de exames. CONCLUSÕES: É urgente promover melhor integração entre os serviços de saúde, além de expandir a oferta de exame anti-HIV e outras IST, em especial em serviços de atenção a pacientes psiquiátricos potencialmente mais vulneráveis a essas condições. OBJETIVO: Analizar factores asociados a la recepción de resultados de exámenes serológicos para VIH y otras infecciones de transmisión sexual (ITS) entre pacientes psiquiátricos. MÉTODOS: Estudio de corte transversal multicéntrico nacional de 2.080 participantes seleccionados aleatoriamente entre 26 instituciones brasileñas de salud mental en 2007. El efecto del individuo (nivel 1) y de los servicios de salud mental (nivel 2) en la recepción de los resultados de los exámenes fue evaluado utilizándose regresión logística multinivel. RESULTADOS: La proporción de retorno de los resultados de exámenes VIH/ITS fue 79,6%. Entre las características individuales, hubo asociación del desenlace con: vivir en la misma ciudad donde se encontraban los servicios, ser soltero, no haber escuchado hablar sobre el sida, y no haber sido previamente evaluado para VIH. Entre las características de los servicios de salud, sólo la distribución de preservativos estuvo asociada a la recepción de los resultados de los exámenes. CONCLUSIONES: El retorno de los resultados de exámenes indicó que es urgente promover una mejor integración entre los servicios de salud, así como expandir la oferta de examen anti-VIH y otras ITS, en especial en servicios de atención a pacientes psiquiátricos potencialmente más vulnerables a estas condiciones.

BACKGROUND There is evidence that a high proportion of people living with HIV infection are unaware of their serologic status, which indicates unmet prevention needs. The aim of the present study was to estimate the rate of lifetime HIV testing and assess the association between previous HIV testing and sociodemographic, clinical characteristics, and risk behaviors in psychiatric patients. METHODS Cross-sectional national multicenter study including 2380 chronic mentally ill patients randomly selected from 26 Brazilian mental health institutions. Multivariate logistic regression analysis was used to describe the association between patient characteristics and previous HIV testing. RESULTS The rate of previous lifetime HIV testing was 27%. Most participants were female (52%), 40 years old or younger (52%) with low schooling (59%), and had previous psychiatric hospitalizations (57%) and schizophrenia as a primary condition (48%). Being tested for HIV was independently associated (P < 0.05) in the multivariate analysis with higher schooling, being younger than 40 years old, living alone, having other self-reported medical conditions, higher HIV/AIDS knowledge, substance use as a main psychiatric condition, a history of sexually transmitted diseases, sexual abuse, and inconsistent lifetime condom use. However, those who never had sexual intercourse and did not know their HIV risk were less likely to be tested. CONCLUSIONS The rate of HIV testing among these psychiatric patients was low despite their increased risk for HIV infection. These findings support the need for increasing HIV testing and awareness of HIV status among psychiatric patients. Mental health providers should play an active role in providing HIV testing and counseling to this population.

The objective of this study is to measure HIV/AIDS knowledge among patients with mental illness in Brazil and to examine individual and treatment setting predictors of knowledge. We conducted a cross-sectional national multicenter study among 2,475 patients in 26 randomly selected mental health institutions throughout Brazil. We used Item Response Theory to standardize knowledge scores and multilevel multiple linear regression to determine the effect of individual and treatment setting characteristics on standardized knowledge score. Schizophrenia was the main diagnosis (48%) of participants. Mean knowledge score was 6.78 (range 1-10). Treatment setting characteristics were not associated with knowledge scores. Higher HIV/AIDS knowledge scores were significantly associated with a history of sexually transmitted disease (STD), previous HIV testing and consistent condom use; lower HIV/AIDS knowledge scores were significantly associated with specific sociodemographic, psychiatric, and HIV risk-perception factors. Psychiatric patients in Brazil lag behind the general population with knowledge scores comparable to those of nearly a decade ago. The mental health system in Brazil and elsewhere must consider strategies beyond dispensing information, for preventing HIV/AIDS transmission in the psychiatric population.

INTRODUCTION: The Quilombola population is subject to numerous sources of social vulnerability, but few studies investigate their physical or mental health conditions. OBJECTIVES: To investigate the factors associated with depression in men and women, separately. METHODOLOGY: Cross-sectional population-based study with 764 randomly selected participants from five quilombo communities in Vitória da Conquista, Bahia, Brazil. The cutoff point for depression was ≥ 10 points, assessed by the Patient Health Questionnaire score (PHQ-9), and the presence of five or more symptoms. We estimated the prevalence ratio with 95% confidence intervals using Poisson regression models with robust estimators stratified by gender. RESULTS: Among men, factors associated with depression were previous diagnosis of chronic illnesses, poor/very poor health self-assessment, and poor access to health services. Among women, the associated factors were previous diagnosis of psychiatric disorders, poor/very poor health self-perception, history of tobacco smoking, and self-declaration of race as not black. CONCLUSION: Factors associated with depression differ between men and women and must be considered in interventions to fight depression within this population. INTRODUÇÃO: A população quilombola está sujeita a inúmeros fatores de vulnerabilidade social, mas poucos estudos investigam suas condições de saúde física ou mental. OBJETIVOS: Investigar os fatores associados à depressão para homens e mulheres. METODOLOGIA: Estudo populacional, transversal, com 764 participantes selecionados aleatoriamente em 5 comunidades quilombolas de Vitória da Conquista, Bahia, Brasil. A depressão foi definida por ≥ 10 pontos avaliados pela escala Patient Health Questionnaire (PHQ-9) e presença de 5 ou mais sintomas. Estimou-se a razão de prevalência, com intervalo confiança de 95% por meio de regressão de Poisson com estimadores robustos, estratificada por sexo. RESULTADOS: Para os homens, os fatores associados à depressão foram diagnóstico prévio de doença crônica, autoavaliação de saúde ruim/muito ruim e acesso ruim aos serviços de saúde. Para as mulheres, os fatores associados foram diagnóstico prévio de transtorno psiquiátrico, autoavaliação de saúde ruim/muito ruim, histórico de tabagismo e se autodeclarar como não negra. CONCLUSÃO: Os fatores associados à depressão diferem entre homens e mulheres e precisam ser considerados nas intervenções para combater a depressão nessa população.

The scope of the study was to assess moderate and excessive alcohol consumption and associated factors among residents of Quilombola Communities in Vitória da Conquista in the State of Bahia. It involved a cross-sectional study among 750 individuals aged ≥ 18 years interviewed in 2011 using an adapted version of the National Survey of Health Questionnaire. Alcohol consumption was classified into abstemious, moderate and heavy drinking categories and was analyzed using the multinomial regression model. Consumption was observed among 41.5% of participants (95% CI [37.8, 44.98]); 10.7% being identified in the excessive consumption category and 30.8% in the moderate consumption category. Moderate consumption was inversely associated with age and is more common among whites and mulattoes, the better educated, the gainfully employed and among smokers. Heavy consumption increased with level of education, being more prevalent among those who had at least 8 years of schooling, smokers and the gainfully employed. The study shows that in these communities there are differences in the sociodemographic characteristics among the profiles of alcohol drinkers and these differences should be taken into account when drawing up proposals for the promotion of healthy habits.

The aim of this study was to estimate tobacco smoking prevalence among psychiatric patients attended in care facilities in Brazil and assess associated factors. A cross-sectional multicenter study was conducted of psychiatric patients (N = 2,475) selected from 26 care facilities. Current and ex-smokers were compared to those who had never smoked. Odds ratios were estimated using logistic regression. The current and past smoking prevalence rates were 52.7% and 18.9%, respectively. Being male, aged 40 years or over, drug and alcohol use, unprotected sex and a history of physical violence were factors associated with both current and past smoking, while a low education level (≤ 8 years of schooling), history of homelessness, not practicing a religion, current or previous psychiatric hospitalization, and main psychiatric diagnosis substance use disorders, were factors only associated with current smoking. Tobacco smoking prevalence among this population was high and was higher than the rate in the general population. Appropriate interventions and smoking prevention policies should be incorporated into mental health services. O objetivo do estudo foi estimar a prevalência e os fatores associados com o tabagismo entre pacientes psiquiátricos no Brasil. Estudo multicêntrico de corte transversal foi conduzido com 2.475 pacientes com transtorno mental, selecionados de 26 serviços. Fumantes e ex-fumantes foram comparados com aqueles que nunca fumaram. Odds ratios foram estimados por meio de regressão logística. As prevalências de fumantes e ex-fumantes foram 52,7% e 18,9%, respectivamente. Sexo masculino, idade 40+ anos, uso de drogas e álcool, prática de sexo desprotegido e histórico de agressão física estiveram associados tanto com fumantes quanto com ex-fumantes. Já a escolaridade ≤ 8 anos, história de ter morado na rua, não ter prática religiosa, internação psiquiátrica atual ou anterior, e transtorno por uso de substâncias como diagnóstico psiquiátrico principal foram associados somente ao tabagismo atual. A prevalência de tabagismo nessa população foi alta e maior do que para a população geral brasileira. Políticas de intervenção e prevenção do tabagismo devem ser adotadas pelos serviços de saúde mental. El objetivo del estudio fue estimar la prevalencia y los factores asociados con el tabaquismo entre los pacientes psiquiátricos en Brasil. Un estudio transversal multicéntrico fue realizado en 2.475 pacientes con trastorno mental, seleccionados de 26 servicios de salud. Los fumadores y los exfumadores fueron comparados con aquellos que nunca habían fumado. Las odds ratios se calcularon mediante regresión logística. La prevalencia de fumadores y ex fumadores fue un 52,7% y un 18,9%, respectivamente. Hombre, edad 40+ años, uso de las drogas y alcohol, sexo sin protección, e historia de agresión física se asociaron con fumadores y ex fumadores. Escolaridad ≤ 8 años, estar sin hogar, no practicar ninguna religión, hospitalización psiquiátrica actual o anterior, y trastornos por uso de sustancias, así como el diagnóstico psiquiátrico primario, se asociaron únicamente con el tabaquismo actual. La prevalencia de tabaquismo en esta población fue alta y superior a la de la población general de Brasil. Se deben adoptar políticas de intervención y prevención del tabaquismo por parte de los servicios de salud mental.

People living with mental illness are at increased risk for HIV. There are scarce data on correlates and prevalence of HIV infection, and none with a nationally representative sample. We report on correlates of HIV infection from a cross-sectional national sample of adults receiving care in 26 publicly funded mental health treatment settings throughout Brazil. Weighted prevalence rate ratios were obtained using multiple log-binomial regression modeling. History of homelessness, ever having an STD, early age of first sexual intercourse before 18 years old, having suffered sexual violence, previous HIV testing, self-perception of high risk of HIV infection and not knowing one’s risk were statistically associated with HIV infection. Our study found an elevated HIV seroprevalence and correlates of infection were not found to include psychiatric diagnoses or hospitalizations but instead reflected marginalized living circumstances and HIV testing history. These adverse life circumstances (history of homelessness, having suffered sexual violence, reporting a sexually transmitted disease, and early sexual debut) may not be unique to people living with mental illness but nonetheless the mental health care system can serve as an important point of entry for HIV prevention in this population.

To assess the prevalence and the factors associated with positive screening for major depressive episode (MDE) in former slave communities in the state of Bahia, Brazil.In this population-based, cross-sectional study, 764 participants were randomly selected from five former slave communities in Vitória da Conquista, Bahia, Brazil. Depression was defined as a cutoff score of 10 or more points and the presence of five or more symptoms according to the Patient Health Questionnaire (PHQ-9). Poisson regression was used to assess prevalence ratios (PR), with a 95% confidence interval (95%CI).Screening for MDE was positive in 12% of participants (95%CI: 10 - 14%), but only 2.3% reported a previous diagnosis. Depression was associated with self-reported poor or very poor health status (PR = 1.14; 95%CI: 1.04 - 1.26), chronic disease (PR = 1.08; 95%CI: 1.04 - 1.13), no physical activity (PR = 1.06; 95%CI: 1.01 - 1.11), and reasonable (PR = 1.07; 95%CI: 1.01 - 1.13) or poor access to health services (PR = 1.07; 95%CI: 1.02 - 1.12).The prevalence of MDE in this population was similar to that of the general Brazilian population. The association between MDE and poor access to health services indicates a missed opportunity for early diagnosis. Public measures against social and health inequalities are necessary to ensure equity in these communities.

Objectives: To describe perceptions about concept and modes of HIV/Aids transmission among psychiatric patients and to assess whether these perceptions differed according to sociodemographic characteristics and psychiatric conditions. We also investigated whether these characteristics were associated with the completely ignorance about the concept and the modes of transmission of HIV/Aids. Methods: Cross-sectional national multicenter study among 2,475 patients selected from 26 Brazilian mental health centers (11 hospitals, 15 outpatient). The perceptions emerged from the following open ended questions: What is Aids for you?; and, How do you think Aids is transmitted?. The textual material was analyzed using content analysis methods. Results were stratified according to sociodemographic variables and psychiatric conditions and analyzed using the chisquared test. Participants who did not know how to answer any of these questions were compared to the others by logistic regression. Results: Overall, perceptions were of disease, transmissible (through blood and sexual contact), incurable and were permeated by negative aspects as threat, suffering and death. These perceptions showed significant differences according to sociodemographic characteristics and psychiatric conditions. 18.6% did not know how to answer the concept or mode of transmission of HIV/Aids and these were more likely to be older (40+ years old), to have less than 8 years of education, no individual income and be on treatment in psychiatric hospitals. Conclusion: The results reinforce the need for macro-level social policy to reduce inequalities and to implement HIV prevention policies targeted at psychiatric patients in Brazil.

Objective The objective was to assess factors associated with previous exposure to hepatitis C virus (HCV) infection among Brazilian adults in mental health centers. Method A cross-sectional national multicenter study of 2087 psychiatric patients randomly selected from 26 public mental health services was conducted between 2005 and 2007 in Brazil. An interview was conducted for obtaining sociodemographic, clinical and behavioral data. Psychiatric diagnoses were obtained from medical charts. Serology testing was conducted using anti-HCV. The magnitude of the associations was estimated by the prevalence rate ratio under Poisson distribution. Multivariate analysis for previous HCV exposure was carried out by weighted Poisson regression. Results The weighted prevalence of previous HCV exposure was 2.53% (95% confidence interval: 2.22–2.83). Factors associated with previous exposure to HCV infection included male gender; age ≥ 40 years; history of sexually transmitted infections; current tobacco use; lifetime injecting drug use; exchange of money/drugs for sex; and main psychiatric diagnoses of psychoses and bipolar disorders, substance use disorders or dementia. Conclusions Our results indicate a high prevalence of previous exposure to HCV in this population of Brazilian psychiatric patients. Most factors statistically associated with HCV were demographic or behavioral related, indicating a potential high-vulnerability profile. Screening for HCV in high-risk patients should be routine practices in these mental health services in Brazil.

OBJECTIVE: The prevalence of HIV among psychiatric patients is higher than general population rates worldwide. Many risk behaviors have been identified in studies from both developing and developed countries, though sampling limitations restrict the generalizability of their results. The objective of this study was to report findings from the first national sample of psychiatric patients about lifetime practice of unsafe sex and associated factors. METHOD: A national multicenter sample of adults with mental illness was randomly selected from 26 public mental health institutions throughout Brazil. Sociodemographic, sexual behavior and clinical data were obtained from person-to-person interviews and blood was collected for serology testing. Logistic regression was used for analysis. RESULTS: The overall prevalence of lifetime unprotected sex was 80.3%. Married, older, female patients, those with multiple partners and living with children or partners only and those with less severe psychiatric diagnosis more often practised unsafe sex. DISCUSSION: Risk behavior assessment is a critical tool for clinicians to be able to determine needed HIV-related services for their clients and ensure appropriate follow-through with care and prevention. Interventions that address situational risks in psychiatric patients' lives-institutional and individual- and increase their ability to make informed decisions about their sexual health are urgently needed.

Introdução: Evidências indicam que pacientes com transtornos mentais têm elevada prevalência de infecções sexualmente transmissíveis (IST), quando comparados à população geral, assim como um aumento de comportamentos de risco. Este estudo analisa a prevenção e atenção às IST/aids, na saúde mental no Brasil, com enfoque na avaliação dos serviços de saúde mental, considerando aspectos da estrutura e processo na atenção às IST/aids, inseridos em estudo multicêntrico brasileiro - Projeto PESSOAS (Pesquisa em Soroprevalência de Aids na Saúde Mental). São avaliados o retorno de resultados dos exames sorológicos de HIV, Sífilis, Hepatite B e C, para os participantes. Enfoca ainda o grau de conhecimento sobre HIV/aids, entre os pacientes psiquiátricos, e avalia se fatores individuais ou contextuais (serviços de saúde mental) se relacionam com esse grau de conhecimento. Métodos: Estudo de corte transversal multicêntrico em amostra representativa nacional de 2475 usuários de serviços de saúde mental. Foram selecionados aleatoriamente 11 hospitais psiquiátricos e 15 CAPS (Centro de Atenção Psicossocial) em todas as regiões do país. Um questionário de avaliação de serviços de saúde mental foi aplicado pelo supervisor local da pesquisa, para avaliar descritivamente indicadores selecionados de estrutura e de processo. Os participantes foram submetidos a uma entrevista semi-estruturada, para verificar as características sociodemográficas, de comportamento e situação de risco, relacionadas à atenção à saúde. Na última fase da pesquisa, um questionário sobre a devolução dos resultados de exames de IST/aids foi aplicado nos 2300 participantes, que se submeteram a coleta de sangue. Resultados: De maneira geral, verificou-se um despreparo para a realização de prevenção e cuidado aos pacientes psiquiátricos em risco para as IST/aids. Mais detidamente, observou-se nos serviços de saúde mental uma insuficiência de recursos humanos e equipamentos, indisponibilidade de medicamentos, especialmente em atenção às urgências clínicas, dificuldades para fornecer suporte clínico aos pacientes (IST/aids, inclusive) e precária estrutura dos serviços de referência e contra-referência. Além disso, poucos serviços dispunham de programas de educação sexual ou distribuição de preservativos, apesar do conhecimento de pacientes sabidamente portadores da infecção pelo HIV. Ao se avaliar o retorno de resultados de exames, observa-se que, dentre os 2080 participantes que realizaram a coleta de sangue e tiveram um questionário de devolução de exames respondidos, 1656(80%) receberam os resultados acompanhados de aconselhamento pós-teste. Os CAPS devolveram os resultados para 89% dos participantes, enquanto os hospitais os devolveram apenas para 60%. Os serviços de saúde mental receberam somente 68% das contra-referências solicitadas aos serviços especializados, para aqueles pacientes que apresentaram algum marcador positivo nos exames sorológicos de IST/aids. É importante ressaltar a baixa testagem para anti-HIV alguma vez na vida (27%) entre os pacientes psiquiátricos, sendo que os serviços de saúde mental que relataram distribuir preservativos aos seus pacientes, devolveram mais exames de IST/aids aos participantes. O grau de conhecimento dos participantes sobre HIV/aids alcançou uma média de escore de 6.78 (escala de 1 a 10 acertos). OBJECTIVE: There is evidence that patients with mental illness have increased prevalence of sexually transmitted infections, but data in Brazil are scarce. The objective of this study was to determine the prevalence of HIV, hepatitis C and B, and syphilis among patients with mental illness in Brazil. METHOD: A multicenter representative sample of adults with mental illness was randomly selected from 26 mental health institutions throughout Brazil. Sociodemographic, sexual behavior and clinical data were obtained from person-to-person interviews and blood was collected for serology testing. Seroprevalence with 95% confidence limits were obtained correcting for sampling scheme. RESULTS: Of the 2,475 patients interviewed, 2,238 had blood collected. Most participants were sexually active ever (88.8%) or in the last 6 months (61.6%), female (51.9%), and single (66.6%). Half of the sample had less than 5 years of schooling and the mean monthly individual income was low (US$ 210.00). Condom use was very low either during lifetime (8%) or in the last 6 months (16%). Overall seroprevalence were 1.12%, 0.80%, 1.64%, 14.7% and 2.63% for, respectively, syphilis, HIV, HBsAg, anti-HBc and anti-HCV. CONCLUSIONS: Seroprevalences found were higher than other populations with representative studies in Brazil, with high rates of sexual risk behavior. This is of public health concern, and prevention and care strategies for sexually transmitted infections among psychiatric patients should urgently be implemented by health authorities. RESUMO OBJETIVO: Evidências indicam que pacientes com transtornos mentais têm elevada prevalência de infecções sexualmente transmissíveis, mas dados brasileiros são escassos. O objetivo deste estudo foi determinar a prevalência do HIV, hepatites C e B, e sífilis entre pacientes com transtornos mentais no Brasil. MÉTODO: Uma amostra representativa de pacientes adultos com transtornos mentais foi aleatoriamente selecionada de instituições públicas de saúde mental no Brasil. Dados sociodemográficos, comportamentais e clínicos foram obtidos por entrevista face-a-face e sangue foi coletado para exames sorológicos. Soroprevalências com intervalo de 95% de confiança foram obtidas com correção para o esquema amostral. RESULTADOS: Dos 2.475 pacientes entrevistados, 2.238 tiveram sangue coletado. A maioria era sexualmente ativa ao longo da vida (88,8%) ou nos últimos seis meses (61,4%), do gênero feminino (51,9%), solteira (66,6%), com metade dos participantes com menos de cinco anos de escolaridade e renda média mensal baixa individual (US$210). Uso de preservativo foi baixo em toda a vida (8%) ou nos últimos seis (16%). As soroprevalências gerais foram 1,12%, 0,80%, 1,64%, 14,7% e 2,63% para, respectivamente, sífilis, HIV, HBsAg, anti-HBc e anti-HCV. CONCLUSÕES: As soroprevalências encontradas são maiores do que outros estudos com populações representativas no Brasil, com altos índices de comportamento sexual de risco. Isto é preocupante e estratégias de prevenção e cuidado para as infecções sexualmente transmissíveis entre pacientes psiquiátricos devem ser urgentemente implementadas pelos serviços de saúde.
