Purpose: The aim of this study was to assess the test–retest reliability of a smartphone-based hearing test, performed without supervision of a hearing professional in an uncontrolled environment. Method: The hearing application is based on an automated hearing test (DuoTone) and relies on verification procedures (ambient noise monitoring algorithm, graphical user interface) to ensure appropriate measurement conditions. Thresholds obtained with DuoTone were compared to those obtained with standard clinical audiometry for 0.5, 1, 2, and 4 kHz in 13 subjects. Subsequently, test–retest reliability was analyzed using anonymized cloud-stored data from a large group of app users (1,641 subjects) who performed multiple hearing tests. Thresholds at minimum or maximum presentation level of the hearing test (10 dB HL, 85 dB HL) were excluded to avoid floor/ceiling effects. A subset (500 subjects) was created to exclude potentially unreliable data. Test–retest thresholds were compared at 12 test frequencies, from 125 Hz to 12 kHz. Results: Thresholds determined by DuoTone and clinical audiometry did not differ significantly for each test frequency. Regarding test–retest analysis, the percentage of test–retest results within 5 dB ranged from 60% to 77% per test frequency. Results from the subset were not substantially different. Test–retest reliability for app users was comparable to results published in the literature regarding test–retest reliability of audiometry, performed in the clinic. Conclusions: Initial validation results suggest that thresholds obtained with DuoTone are comparable to clinical audiometry (four frequencies tested). The hearing app provides reliable hearing thresholds between 15 and 80 dB HL (12 frequencies tested) with a test–retest reliability comparable to clinical audiometry.

Purpose Heavily T2-weighted 3D FLAIR (hT2w-3D-FLAIR) sequence with constant flip angle (CFA) has been reported as being more sensitive to low concentrations of gadolinium (Gd) enabling endolymphatic hydrops (EH) visualization. The purpose of this study was to compare signal-to-noise (SNR) ratio, detection rate of EH, and increased perilymphatic enhancement (PE) as well as diagnostic accuracy in diagnosing definite Menière’s disease (MD), using 3D-SPACE FLAIR versus conventional 3D-TSE FLAIR. Methods This retrospective study included 29 definite MD patients who underwent a 4-h delayed intravenous (IV) Gd-enhanced 3D-TSE FLAIR and 3D-SPACE FLAIR MRI between February 2019 and February 2020. MR images were qualitatively and quantitatively analyzed twice by 2 experienced head and neck radiologists. Qualitative assessment included grading of cochlear and vestibular EH and visual comparison of PE. Quantitative assessment of PE was performed by placing a region of interest (ROI) and ratio calculation in the basal turn of the cochlea and the brainstem. Results The intra- and inter-reader reliability for grading of EH and PE was excellent (0.7 Conclusion Four-hour delayed IV Gd-enhanced 3D-SPACE FLAIR sequence has a higher sensitivity and reproducibility than 3D-TSE FLAIR for the visualization of EH and increased PE in definite MD patients.

With the introduction of more flexible and thinner electrodes, such as Cochlear's Slim Modiolar Electrode, there is a higher risk of electrode insertion problems, in particular the tip foldover. Timely intraoperative detection of the problem would allow for direct intraoperative correction. This paper describes a non-radiological method for intraoperative tip foldover detection that is applicable in all surgical centers and can quickly deliver accurate results.Postoperative radiographs of 118 CI-recipients implanted with Nucleus devices were retrospectively analyzed on the presence of a tip foldover. Electrode Voltage Telemetry (EVT), also called Electric Field Imaging, was performed by means of Cochlear's EVT software tool, which is now integrated into Custom Sound-EP as the Trans-Impedance-Matrix measurement option. Tip foldover detection was automated by using the linear Hough transform for extracting straight-line patterns in the Trans-Impedance Matrix's heatmap.The six cases of electrode tip foldover were accurately identified by the EVT measurements, including two cases with folding location very close to the electrode tip (contact 20).Electrode Voltage Telemetry measures the Trans-Impedance Matrix, which can accurately detect tip foldovers of the cochlear implant electrodes within 1 min. This method can be reliably applied in all patients with normal cochlear anatomy and is able to intraoperatively detect foldovers localized even very close to the electrode tip. Application of the linear Hough transform allows for automatic detection of electrode tip foldovers that shows excellent agreement with visual evaluation of the radiological images and the transimpedance matrix's heatmap.

Objective: To evaluate early results on hygiene, safety and functional outcome in a population undergoing a canal wall up technique with bony obliteration of the mastoid and epitympanic space (CWU-BOT) for extensive cholesteatoma, performed by a single surgeon. This study compares different techniques of tympanic membrane reconstruction, viz. allografts and autografts. Patients: A consecutive series of 61 ears with acquired cholesteatoma treated with primary or revision CWU-BOT surgery from 2009 to 2014. Intervention: Obliteration was performed by the use of cortical bone-chips and bone pâté. Patients were followed up with micro-otoscopy and MRI with diffusion-weighted imaging. Ossicular reconstruction was performed using a remodelled autologous or allogenic incus or malleus. Main outcome measures: Residual and recurrence rate and short- and mid-term hearing outcome prior to any revision tympanoplasty were analysed, the effect of type of tympanic membrane reconstruction was considered. Results: 44 Ears were primary cholesteatoma cases, 17 cases were referred for revision surgery. Mean postoperative follow up was 45 months (SD 18.08) and mean follow-up until the last non-EP DW MRI 42 months (SD 17.72). Recurrent disease was present in 3%, no residual disease was present. An AC gain was seen in 75% of all ears undergoing ossicular reconstruction. Conclusion: Reproducible safety, hygiene and hearing results with limited recurrence and residual disease can be obtained by younger otologic surgeons performing the BOT-CWU for extensive cholesteatoma while using a variety of grafts for tympano-ossicular reconstruction. The tympano-ossicular allograft nevertheless shows superior hearing results when a mobile intact stapes is present. Level of evidence: Level 4

Purpose In this retrospective study the relationship between cochleovestibular function and a magnetic resonance imaging (MRI-) based classification system of endolymphatic hydrops was investigated. Methods Seventy-eight patients with unilateral definite Meniere's disease who underwent MRI were included. The parameters of Pure Tone Audiometry (PTA), caloric irrigation test, cervical vestibular evoked myogenic potentials, and video Head Impulse Test were compared between the grades of endolymphatic hydrops (EH) and perilymphatic enhancement (PE) on MRI. Results The low-frequency PTA was significantly different between cochlear EH grades I and II (p = 0.036; Grade I: mean (Standard Deviation, SD) = 51 decibel Hearing Level (dB HL) (18 dB HL); Grade II: mean (SD) = 60 dB HL (16 dB HL)), and vestibular EH grades 0 and III (p = 0.018; Grade 0: mean (SD) = 43 dB HL (21 dB HL); Grade III: mean = 60 dB HL (10 dB HL)). The ipsilateral caloric sum of ears with vestibular EH grade I (n = 6) was increased with regards to vestibular EH grades 0 (p = 0.001), II (p < 0.001), and III (p < 0.001) (Grade 0: mean (SD) = 24 degrees/s (15 degrees/s); Grade I: mean (SD) = 47 degrees/s (11 degrees/s); Grade II: mean (SD) = 21 degrees/s (13 degrees/s); Grade III: mean (SD) = 16 degrees/s (8 degrees/s)). Conclusion According to these results we can conclude that only the highest grades of cochlear and vestibular EH seem to be associated with decreased cochleovestibular functioning

Recently, synchrotron radiation computed microtomography (SRµCT) has emerged as a promising tool for non-destructive, in situ visualization of cochlear implant electrode arrays inserted into a human cochlea. Histological techniques have been the `gold standard' technique for accurate localization of cochlear implant electrodes but are suboptimal for precise three-dimensional measurements. Here, an SRµCT experimental setup is proposed that offers the benefit of a high spatial and contrast resolution (isotropic voxel size = 4.95 µm and propagation-based phase-contrast imaging), while visualizing the soft-tissue structures and electrode array of the cochlear implant simultaneously. In this work, perimodiolar electrode arrays have been tested, which incorporate thick and closely spaced platinum–iridium contacts and wiring. These data can assist cochlear implant and hearing research, can be used to verify electrode segmentation techniques for clinical computed tomography or could be utilized to evaluate cochlear implant electrode array designs.

Summary Background Worldwide use of nose-and-mouth-covering respiratory protective mask (RPM) has become ubiquitous during COVID19 pandemic. Consequences of wearing RPMs, especially regarding perception and production of spoken communication, are gradually emerging. The present study explored how three prevalent RPMs affect various speech and voice sound properties. Methods Pre-recorded sustained [a] vowels and read sentences from 47 subjects were played by a speech production model (‘Voice Emitted by Spare Parts’, or ‘VESPA’) in four conditions: without RPM (C1), with disposable surgical mask (C2), with FFP2 mask (C3), and with transparent plastic mask (C4). Differences between C1 and masked conditions were assessed with Dunnett's t test in 26 speech sound properties related to voice production (fundamental frequency, sound intensity level), voice quality (jitter percent, shimmer percent, harmonics-to-noise ratio, smoothed cepstral peak prominence, Acoustic Voice Quality Index), articulation and resonance (first and second formant frequencies, first and second formant bandwidths, spectral center of gravity, spectral standard deviation, spectral skewness, spectral kurtosis, spectral slope, and spectral energy in ten 1-kHz bands from 0 to 10 kHz). Results C2, C3, and C4 significantly affected 10, 15, and 19 of the acoustic speech markers, respectively. Furthermore, absolute differences between unmasked and masked conditions were largest for C4 and smallest for C2. Conclusions All RPMs influenced more or less speech sound properties. However, this influence was least for surgical RPMs and most for plastic RPMs. Surgical RPMs are therefore preferred when spoken communication is priority next to respiratory protection.

Positioning of the cochlear implant (CI) electrode in relation to the anatomical structures is a key factor for the hearing outcome and the preservation of residual hearing after cochlear implantation. Determin-ing the exact electrode's location is therefore expected to play an important role in optimisation of the electrode design, the surgical techniques and the post-operative device fitting. The aim of this study is the development and validation of a robust and efficient computerised algorithm for three-dimensional (3D) localisation of the CI-electrode contacts with respect to the relevant cochlear structures, such as the basilar membrane and the modiolus, from modern clinical in vivo cone-beam computed tomography (CBCT). In the presented algorithm, the pre-and post-implantation CBCT are spatially aligned. To localise the anatomical structures, a cochlear microanatomical template derived from lab-based X-ray computed microtomography (mu CT) measurements is warped to match the patient-specific cochlear shape acquired from pre-implantation CBCT. The electrode-contact locations, determined from the post-operative CBCT, are superimposed onto the cochlear fine-structure of the microanatomical template to localise the array. The accuracy of this method was validated in a temporal bone study by comparing the distance of the electrode contacts from the modiolar wall, as derived by the algorithm from CBCTs, with the distance determined from synchrotron-radiation (SR) mu CT on the same specimens. Due to the achievable spatial resolution, good tissue contrast and limited presence of metallic artifacts, the SR mu CT technique is consid-ered to be a golden standard in the proposed approach. In contrast to other approaches, this validation method allowed for the evaluation of the final electrode-to-modiolus distance (EMD) error, and covers the error in co-alignment of the images, in the determination of the electrode contact location and in the localisation of the cochlear structures. The absolute mean error on the EMD parameter was determined at 0.11 mm (max = 0.29 mm, SD = 0.07 mm) across five samples, slightly lower than the voxel size of the CBCT-scans. In a retrospective study, the algorithm was applied to identify scalar translocations of the electrode from clinical in vivo CBCT datasets of 23 CI-recipients, which showed perfect (100%) agreement with the blinded opinion of two experienced neuroradiologists.(c) 2022 Elsevier B.V. Measurement of the complex electrical impedance of the electrode contacts can provide new insights into the factors playing a role in the preservation of residual hearing with cochlear implants (CIs). However, unraveling the contributions related to the different phenomena from impedance data necessitates more advanced measurement and analysis techniques. The present study explores a new impedance measurement option recently included into the cochlear-implant programming software and aims to contribute to a more solid basis for the clinical use of impedance measures as a biomarker for fibrous tissue formation. Twenty adult CI-recipients were followed from surgery until 1 year after implantation by means of Electrode Voltage Telemetry (EVT), also called Electric Field Imaging or TransImpedance-Matrix measurement, and a 4-point technique for probing the voltage between adjacent electrode contacts. The data were compared to the electrode location derived from computed tomography, and to the device usage log. Using our impedance model for electrical stimulation of the cochlea, the polarization impedance related the electrode-tissue interface was determined, and the bulk impedance (access resistance) was split into a near-field and a far-field component. On average, the polarization impedance increased abruptly after surgery, indicating a strong passivation of the electrode contacts before cochlear-implant initiation. Its initial rise resolved almost completely soon after device switchon (2-4 weeks). The gradual increase of the access resistance mainly happened during the first 40 days on a time scale very similar to that observed in a guinea-pig study correlating impedance changes to fibrous tissue growth. The higher increase towards the round window is consistent with the higher amount of tissue observed in histological animal studies close to the electrode entry point. While the initial changes were due to the near-field resistance, the far-field resistance began to rise only after one month for half of the study group, once the near-field component had reached its critical value. This suggests indeed fibrosis initiating near the electrode contacts and spreading thereafter farther away. The near-field resistance positively correlated to device usage. EVT data allow for a further decomposition of the impedance at a cochlear-implant electrode, yielding a more detailed description of the postoperative intracochlear phenomena, such as fibrosis.

Purpose To better understand hypernasality (HN), we explored the relations between velopharyngeal orifice, auditory perception of HN, and acoustic-spectral measures in an in vivo within-subject design: (a) with a normally functioning velum as the control condition and (b) with a temporarily paralyzed velum as the experimental condition. Method The velum of eight volunteers was injected with ropivacaine hydrochloride (Naropin) in the area of the levator veli palatini and tensor veli palatini muscles to induce temporary velopharyngeal inadequacy (VPI) and HN. Sustained [a] and [i] and oronasal text readings were recorded, and 3D cone-beam computed tomography images of the vocal tract were built before and during velar anesthesia. Differences between conditions and correlations in normal-to-numb differences between velopharyngeal cross-sectional area (VP area ), mean ratings of HN severity, and nine acoustic-spectral measures were determined. Results Three subjects already had some incomplete velopharyngeal closure in the control condition. Temporary motor nerve blockage of the velum (increased VP area ) was accomplished in seven subjects, leading to increased HN and changes in three acoustic-spectral measures. Furthermore, significant correlations only emerged between VP area , HN, and Model Kataoka . Conclusions In most of the participants, it was possible to temporarily increase the velopharyngeal orifice to investigate HN while controlling other speech variables and cephalic morphology. Although this study was exploratory and its are findings preliminary, it provided additional evidence for the possible clinical value of Model Kataoka , A 3 – P 0 , and B F 1 for the objective measurement of VPI or HN.

OBJECTIVES: This study aimed to investigate the effect of canal wall up with bony obliteration tympanoplasty (CWU-BOT) on the health-related quality of life (HRQOL) in patients with chronic otitis media with cholesteatoma by using the chronic otitis media questionnaire 12 (COMQ-12). MATERIALS AND METHODS: This study is a retrospective analysis of the COMQ-12 of 26 patients who completed the COMQ-12 before and after a CWU-BOT with eradication of cholesteatoma followed by obliteration of the mastoid and paratympanic space with bone chips and bone pâté and reconstruction of the tympanic membrane and ossicular chain. RESULTS: All patients were operated upon in our institute between 2014 and 2017. The median score of the 12 questions was preoperatively and postoperatively calculated, and then compared. A large effect was observed in the total score and the questions about running ear, discharge, and visits to the general practitioner. A medium positive size effect was observed in the questions about hearing in noisy surroundings, discomfort, dizziness, tinnitus, medication use, and the mental aspect of the patient. In the questions about the hearing at home and quality of life and impact on work, we noted a small positive size effect. In 50% of patients, the HRQOL became normal; the remaining 50% improved to a level very close to normal. CONCLUSION: Canal wall up with bony obliteration tympanoplasty (CWU-BOT) showed a clear decrease in the severity of the symptoms, life and work impact, and health care after surgery.

Electrical stimulation with cochlear implants is able to significantly suppress the tinnitus sensations in 25–72% of implanted patients. Up to this point, no clear predictors for the effectiveness of tinnitus suppression with cochlear implants have been found and this substantially limits the possibility of the application of cochlear implants for this purpose. The objective of the study was to investigate if a trial electrical round window stimulation (RWS) could be used as a diagnostic tool for identifying candidates in whom electrical stimulation would be successful as treatment for tinnitus. Thirty-four patients with unilateral severe tinnitus and ipsilateral moderate to severe sensorineural hearing loss underwent a trial RWS under local anesthesia. Thirteen patients received a cochlear implant. All patients qualified for cochlear implantation on the basis of the trial RWS showed tinnitus suppression with the implant switched on. Complete or almost complete tinnitus suppression was obtained in 77% and partial in 23%. The mean tinnitus loudness reduction was 68% (VAS score reduction from 7.7 to 2.5). False negative results are estimated not to exceed 10–15%. We conclude that significant tinnitus suppression achieved during trial RWS under local anesthesia is a simple procedure allowing the efficient identification of candidates in whom electrical stimulation with a cochlear implant would be successful as treatment for intractable tinnitus.

Several studies report that sound localization performance of acute and chronic monauralized normal-hearing listeners can improve through training. Typically, training sessions are administered daily for several days or weeks. While this intensive training is effective, it may also be that monaural localization abilities improve instantly after providing explicit top-down information about the direction dependent change in timbre and level. The aim of the present study was to investigate whether cognitive feedback (i.e., top-down information) could instantly improve sound localization in naive acutely monauralized listeners. Forty-three normal-hearing listeners (experimental group), divided over five different centers, were tested. Two control groups, consisting of, respectively, nine and eleven normal-hearing listeners, were tested in one center. Broadband sounds (0.5-20 kHz) were presented from visible loudspeakers, positioned in azimuth (- 90° to 90°). Participants in the experimental group received explicit information about the noticeable difference in timbre and the poor localization in the monauralized listening condition, resulting in an instant improvement in sound localization abilities. With subsequent roving of stimulus level (20 dB), sound localization performance deteriorated immediately. The reported improvement is related to the context of the localization test. The MO-meatocanalplasty is the oblique modification of the M-meatoplasty. The MO-meatocanalplasty was designed to address the superior quadrants of the meatus and the bony canal without the need for a retro-auricular incision. This retrospective analysis was performed to evaluate the long-term results of the MO-meatocanalplasty in patients with a narrow external auditory canal (EAC) with recurrent otitis externa or in patients unable to wear a hearing aid. Twenty-two ears in twenty consecutive patients who received a MO-meatocanalplasty for a narrow EAC with recurrent otitis externa or the inability to wear a hearing aid were analysed retrospectively. There were no patients included with any type of previous or planned second stage tympanoplasty procedures. A follow-up period of 3 years was analysed for postoperative recurrent narrowing, the self-cleaning capacity of the EAC, the recurrence of otitis externa, the inability to wear a hearing aid, change in hearing level and for all types of aesthetical complaints. The MO-meatocanalplasty procedure was effective in 82% (n = 18). Postoperative recurrent narrowing was detected in 9% (n = 2). Insufficient self-cleaning capacity of the EAC was 9.1% (n = 2). The ability to wear a hearing aid was restored in all patients with the need for a hearing aid. No aesthetical complaints were reported. The MO-meatocanalplasty is an effective, safe and aesthetical accepted procedure to address the narrow meatus and external auditory canal. With this procedure, there is no need for a retro-auricular incision in order to create a well aerated, dry and self-cleaning EAC in patients with a narrow EAC with recurrent otitis externa or in patient with the inability to wear a hearing aid.

Objective To report the long-term follow-up with diffusion-weighted magnetic resonance imaging (DW MRI) after subtotal petrosectomy (SP) with blind sac closure of the external auditory canal for extensive cholesteatoma and chronic suppurative otitis media. Study design Retrospective clinical record study. Setting Tertiary referral center. Patients Thirty-one patients (31 ears) with extensive cholesteatoma and 17 patients (19 ears) with chronic suppurative otitis media without cholesteatoma who underwent SP between July 1995 and December 2015. Interventions All 48 patients were followed clinically and with DW MRI to rule out residual cholesteatoma. Main outcome measure Residual cholesteatoma, indicated by a marked hyperintensity on non-echoplanar (non-EP) DW MRI. Results In the cholesteatoma group the mean interval between surgery and the latest DW MRI was 3.9 years. Seven patients presented with a residual cholesteatoma pearl. The mean interval between surgery and detection of residual disease was 3.7 years. In the chronic suppurative otitis media group the mean interval between surgery and the latest DW MRI was 5.3 years. Residual cholesteatoma was found in three patients. Mean interval between surgery and the detection of disease was 4.5 years. Conclusions This study shows the importance of DW MRI in the follow-up after SP for cholesteatoma and chronic suppurative otitis media. A little higher residual cholesteatoma rate was found compared with earlier studies, where patients were followed only clinically. One may wonder whether reintervention is always needed or whether in selected cases with small pearls, one may still observe these by a watchful waiting policy with DW MRI.

In this case report, the air-conducted cervical vestibular evoked myogenic potentials (AC cVEMP) test was only sensitive for the left superior semicircular canal dehiscence (SCD), even though the contralateral SCD was of equal length (2.5 mm). Furthermore, a lysis of the processus lenticularis incudis caused a real conductive hearing loss in the left ear. A diminished left AC cVEMP was thus expected, but the opposite was shown (increased corrected amplitude, lowered detection threshold). The patient only experienced hearing loss, so middle ear surgery was performed to repair the lysis. The postoperative AC cVEMP showed a further "uncovering" of the SCD with increased corrected amplitude on the left but no vestibular symptoms. The significance of an SCD should be interpreted with caution, even when the AC cVEMP and the imaging are significant. Furthermore, AC cVEMPs should not be considered as evidence for the absence or presence of conductive hearing loss.

Background: The peripheral vestibular end organ is considered to consist of semi-circular canals (SCC) for detection of angular accelerations and the otoliths for detection of linear accelerations. However, otoliths being phylogenetically the oldest part of the vestibular sensory organs are involved in detection of all motions. Summary: This study elaborates on this property of the otolith organ, as this concept can be of importance for the currently designed vestibular implant devices. Key Message: The analysis of the evolution of the inner ear and examination of clinical examples shows the robustness of the otolith system and inhibition capacity of the SCC. The otolith system must be considered superior to the SCC system as illustrated by evolution, clinical evidence, and physical principles.

(1) To compare the in vivo impedances of electrical contacts of the Nucleus Contour Advance (CA) perimodiolar electrodes and the Nucleus Slim Straight (SS) lateral wall electrodes. (2) To compare the relation between the electrode contact impedances and the behavioural T-/C-levels for both types of electrodes. Retrospective case review in two quaternary otologic referral centres was performed. Data on the impedance of the electrode contacts and the T-/C-levels have been collected from 70 consecutive paediatric CI patients. (1) SS electrodes show significantly higher impedance values of the contacts compared to the CA electrodes. This can be explained by differences in the active surface of the electrode contacts and is true for the whole electrode arrays. (2) There are significant negative correlations observed between the impedances of the electrode contacts and the behaviourally measured T-/C-levels. (3) The strength correlation between the electrode impedances and the behavioural T-/C-levels is significantly higher and the T-/C-level variability is lower for the perimodiolar CA electrodes than for the lateral wall SS electrodes. There exist significant differences in the contact impedance between different cochlear implant electrode arrays. These differences could influence the implant programming parameters and should be considered in any protocols related to automatic implant programming based on objective measures. The predictive value of the electrode impedances for the behavioural T-/C-levels is higher for the perimodiolar CA electrodes than for the lateral wall SS electrodes.

Purpose This study compared auditory-perceptual measures of presence/absence and severity of vocal tremor to acoustic markers of vocal tremor. The validity (both concurrent and diagnostic) of various acoustic markers of vocal tremor was also assessed. Method Fifty-six midvowel sustained [a:] recordings were selected to yield a representative convenience sample of vocal tremor. After training with 10 synthesized samples, 4 female audiologists rated these samples on “voice tremor severity” on a continuous 10-cm scale. Afterward, 15 randomly selected recordings were presented a 2nd time for intrarater reliability assessment. Customized audio signal processing in Praat yielded 12 acoustic measures of rate, extent and perturbation of fundamental frequency ( f 0 ), and intensity level (IL) modulation. Enter-type multiple linear regression analysis was applied to weight and combine these acoustic variables into an acoustic model of vocal tremor severity. Results After removing the vocal tremor severity ratings of 1 of the audiologists because of insufficient intra- and interrater reliability, mean single-measures consistency-type intraclass correlation coefficients equaled .83 within raters and .72 between raters. Correlation between mean ratings and the 12 acoustic markers ranged from .76 for median extent of f 0 modulation to .11 for rate of IL modulation. Correlation between mean ratings and the acoustic model was .89. Analysis of this model's receiver operating characteristics yielded an area under receiver operating characteristic curve of .93, denoting sensitivity of .87 and specificity of .91. Conclusions This study demonstrated that auditory-perceptual ratings of vocal tremor severity are guided primarily by f 0 and IL modulation extent, less by modulation perturbation, and least by modulation rate. The acoustic model covering all these modulation properties yielded acceptable results in terms of both concurrent and diagnostic validity. However, external cross-validation of this model is warranted before applying it in clinical voice/speech assessment.

Facial nerve schwannomas are rare benign neoplasms. We report a case of a 60-year-old woman who initially presented with vestibular complaints. Magnetic resonance imaging (MRI) revealed a facial nerve schwannoma centered on the right geniculate ganglion extending in the labyrinthine segment. The patient consulted again after 2 months because she developed a sudden and severe right-sided sensorineural hearing loss. MRI showed no progression or pathological enhancement in the membranous labyrinth. A cone beam computed tomography (CT) of the temporal bone was performed and revealed a large erosion at the region of the geniculate ganglion in open communication with the middle turn of the cochlea. This case report demonstrates the importance of CT in facial nerve schwannomas for evaluating the impact on the surrounding structures.

Background The DHI is a widely used questionnaire for the evaluation of the self-reported disability in patients with dizziness and balance problems. Objective To investigate the relationship between the DHI scores and demographic, symptomatic and diagnostic parameters. Methods Retrospective study in 568 patients with balance problems. Results We observed a total of 61.3% of patients with moderate (DHI total score between 30 and 59) to severe (DHI total score between 60 and 100) disability.Patients with long-standing complaints (lasting longer than 3 months) experience their self-reported disability to a greater extent than patients with new onset pathology (illness duration of one month and less). Moreover, patients suffering from continuous complaints have a larger DHI score than patients with shorter symptom duration. The first effect (new onset vs. long-standing pathology) is primarily caused by emotional factors, the latter effect (symptom duration) is attributable to functional and physical factors, not to emotional aspects. Patients with daily and weekly complaints have larger DHI scores than patients who reported only one episode. Female patients reported larger DHI scores than males. We found no effect of age, diagnostic group (no diagnosis, episodic, acute or chronic vestibular syndrome) or reported symptoms on the DHI scores. Conclusions The information retrieved from the DHI questionnaire is complementary to the information obtained from clinical investigation and diagnostic tests and therefore is an essential tool in a vestibular clinic.

Objective The purpose of this study was to investigate etiological, clinical, and pathological characteristics of traumatic injuries of the middle ear ossicular chain and to evaluate hearing outcome after surgery. Material and methods Thirty consecutive patients (31 ears) with traumatic ossicular injuries operated on between 2004 and 2015 in two tertiary referral otologic centers were retrospectively analyzed. Traumatic events, clinical features, ossicular lesions, treatment procedures, and audiometric results were evaluated. Air conduction (AC), bone conduction (BC), and air-bone gap (ABG) were analyzed preoperatively and postoperatively. Amsterdam Hearing Evaluation Plots (AHEPs) were used to visualize the individual hearing results. Results The mean age at the moment of trauma was 27.9±17.1 years (range, 2-75 years) and the mean age at surgery was 33.2±16.3 years (range, 5-75 years). In 10 cases (32.3%), the injury occurred by a fall on the head and in 9 (29.0%) by a traffic accident. Isolated luxation of the incus was observed in 8 cases (25.8%). Dislocation of the stapes footplate was seen in 4 cases (12.9%). The postoperative ABG closure to within 10 and 20 dB was 30% and 76.7%, respectively. Conclusion Ossicular chain injury by direct or indirect trauma can provoke hearing loss, tinnitus, and vertigo. As injuries are heterogeneous, they require a tailored surgical approach. In this study, the overall hearing outcome after surgical repair was favorable.

7th IEEE Asia-Pacific Conference on Applied Electromagnetics, APACE 2016 -- 11 December 2016 through 13 December 2016 -- 127632 ‘Second Consensus Meeting on Management of Complex Inner Ear Malformations: Long Term Results of ABI in Children and Decision Making Between CI and ABI’ took place on 5–6 April 2013 in Kyrenia, Northern Cyprus with the participation of 20 centers from 11 countries. These centers presented their auditory brainstem implantation (ABI) experience in children and infants and also provided the selection criteria and cochlear implant (CI) results in patients with an abnormal cochlea and nerve. In addition, Cochlear and Med El companies provided presentations regarding solutions to problems during revision surgery and future projections of device technology. According to the results from different centers, it was evident that an ABI is capable of providing hearing sensation in prelingually deafened children with complex inner ear malformations and diseases. It is possible to obtain a pure tone average with an ABI between 30 and 60 dB HL in most of these patients. It has been observed that the majority of children obtain Categories of Auditory Performance (CAP) scores around 5, but occasionally certain ABI users obtained scores of up to CAP 8-II, NEAP-Nottingham Early Assessment Package. The Ear Foundation 2009). CAP scores tend to increase with earlier implantation, i.e. under 2 years of age. However, after 3 years of use, it usually reaches a plateau. Children with additional disorders cannot reach CAP scores of children without disorders. However, they obtain an improvement of cognitive functions but duration of ABI use and increasing chronological age also play a role in this improvement.

The meatoplasty of the external auditory canal is a frequently performed otologic procedure in recurrent otitis externa, eczema or frequent accumulation of cerumen due to a narrow meatus of the external ear canal. Numerous surgical techniques have been described. The M-meatoplasty described by Mirck for addressing the external meatus is widely used. However, this technique does not sufficiently enlarge the external ear canal in all cases. Specifically in patients where the ear canal narrowing is most prominent in the postero- and/or anterosuperior quadrants of the lateral meatus the technique needs some modifications. In these cases, an oblique conversion of the M-meatoplasty, the MO-meatocanalplasty, is useful. In cases where the bony canal is also narrow this modification allows for a bony canalplasty while avoiding a retro-auricular approach. The MO-meatocanalplasty can be used in combination with myringoplasty and tympanoplasty.

Die moderne operative Therapie des Mittelohrcholesteatoms hat sich von der fruheren, fast dogmatischen Anwendung entweder der 2-Wege Technik oder der Radikalhohlenoperation zu einer individualisierten Technik gewandelt, in Kenntnis der Pathomechanik (Entwicklung des Cholesteatoms aus einer Retraktionstasche) und den vielfaltigen zuverlassigen Einsatzmoglichkeiten einer Knorpelrekonstruktion der Gehorgangswande. In der modernen Ohrchirurgie kommt daher in der Mehrzahl die sogenannte Verfolgungstechnik (die transmeatale Exposition des Cholesteatomsackes ohne zwingende Mastoidektomie mit anschliesender Rekonstruktion der Gehorgangswand durch Knorpelplatten) zum Einsatz. Nur in seltenen Fallen ist noch die sogenannte 2-Wege Technik (mit Mastoidektomie) oder die Anlage einer Radikalhohle erforderlich. Auch hier hat ein Umdenken stattgefunden in Anbetracht der unertraglich hohen Rezidivraten bei der 2-Wege Technik: Sowohl die hohe Zahl der Residualcholesteatome (typischerweise im Schatten der stehengelassenen hinteren Gehorgangswand) als auch die rekurrenten Rezidive (Einsenken einer neuen Retraktion in die vormals ausgebohrte Mastoidhohle) lassen dieses Verfahren nur noch fur sehr gut pneumatisierte und mit reizloser, gasproduzierender Schleimhaut ausgekleidete Mittelohren als erfolgsversprechend erscheinen. Bei der klassischen Radikalhohlentechnik besteht trotz ihrer geringeren Rezidivrate als groser Nachteil das chronische Ohrenlaufen aufgrund der standig feuchten Hohle. Ursachlich und schon lange bekannt ist das ungunstige Beluftungsverhaltnis der hautausgekleideten grosen Hohle (zu kleine Offnung des Gehorgangs im Verhaltnis zum grosen Hohenvolumen). Eine stabile Verkleinerung der Hohle mit deutlicher Verringerung der Rezidivrate ist mit korpereigenem Knochenmehl und luckenloser Knorpelplattenabdeckung (aus dem Cavum conchae) moglich. In dem Video wird das Prinzip der Knochenmehl- und Knorpelplattenobliteration mit unserer in Dresden entwickelten Technik dargestellt und es werden einige Tipps zur Erzielung einer stabilen Hohle gegeben.

The objective is to evaluate the influence of the presentation rate on intraoperative ECAP thresholds in cochlear implant users.The design was data on the ECAP thresholds (t-NRT) as well as the behavioural T- and C-levels have been collected in CI patients of a quaternary otologic referral centre. Measurements of the tNRT thresholds were performed intraoperatively for 250 Hz and 80 Hz presentation rates and correlated to the stabilised T- and C-levels measured at the 5th fitting session, 4-6 months after surgery.There was a study sample of 35 consecutive CI patients. All patients were users of the Nucleus 24RECA (Freedom) or Nucleus CI512 cochlear implants with the Contour Advance-of-Stylet electrode.The result showed that the t-NRT thresholds were higher for the 250 Hz pulse rate typically used during the intraoperative stimulation under general anaesthesia than for the 80 Hz rate used typically during the postoperative fitting sessions. This difference was more pronounced for the basal electrodes where it exceeded 10 current levels (CL). Pearson's correlation coefficients between the t-NRT-measurements and the stabilised T- and C-levels r ranged between 0.34 and 0.47.In conclusion, the magnitude of the ECAP thresholds (t-NRT) recorded intraoperatively depends significantly on the stimulus presentation rate.

Background: In patients with bilateral vestibulopathy, the regular treatment options, such as medication, surgery, and/or vestibular rehabilitation, do not always suffice. Therefore, the focus in this field of vestibular research shifted to electrical vestibular stimulation (EVS) and the development of a system capable of artificially restoring the vestibular function. Key Message: Currently, three approaches are being investigated: vestibular co-stimulation with a cochlear implant (CI), EVS with a vestibular implant (VI), and galvanic vestibular stimulation (GVS). All three applications show promising results but due to conceptual differences and the experimental state, a consensus on which application is the most ideal for which type of patient is still missing. Summary: Vestibular co-stimulation with a CI is based on “spread of excitation,” which is a phenomenon that occurs when the currents from the CI spread to the surrounding structures and stimulate them. It has been shown that CI activation can indeed result in stimulation of the vestibular structures. Therefore, the question was raised whether vestibular co-stimulation can be functionally used in patients with bilateral vestibulopathy. A more direct vestibular stimulation method can be accomplished by implantation and activation of a VI. The concept of the VI is based on the technology and principles of the CI. Different VI prototypes are currently being evaluated regarding feasibility and functionality. So far, all of them were capable of activating different types of vestibular reflexes. A third stimulation method is GVS, which requires the use of surface electrodes instead of an implanted electrode array. However, as the currents are sent through the skull from one mastoid to the other, GVS is rather unspecific. It should be mentioned though, that the reported spread of excitation in both CI and VI use also seems to induce a more unspecific stimulation. Although all three applications of EVS were shown to be effective, it has yet to be defined which option is more desirable based on applicability and efficiency. It is possible and even likely that there is a place for all three approaches, given the diversity of the patient population who serves to gain from such technologies.

Introduction Previous auditory experience modifies the sensitivity of the auditory cortex to the afferent activity of the auditory pathways and may influence the threshold (T) and comfort (C) levels in patients receiving a cochlear implant (CI). Literature data on this particular topic is very scarce. Objective This study aimed to evaluate the differences in T/C-levels between pre- and postlingually implanted cochlear implant patients. Methods Retrospective case review in a quaternary otologic referral centre was performed. Data on the T/C-levels have been collected in 90 consecutive CI patients divided into 2 groups. Group 1 comprised 16 prelingually deaf children implanted between 8 months and 10 years of age. Group 2 comprised 74 postlingually deaf adults (average age of 62 years). All patients were users of the Nucleus 24RECA (Freedom, Contour Advance-of-Stylet electrode) cochlear implant. All measurements were performed at the fifth implant programming session at 4–6 months after surgery, when stable T/C thresholds have already been obtained. Results The behavioural C-levels present important and statistically significant differences between the pre- and postlingually implanted patients for all electrode contacts that could reach 30 CL. For the T-levels the observed differences were smaller and statistically insignificant for most electrode contacts. Conclusions The previous auditory experience (pre- or postlingual deafness) seems to be an independent parameter influencing the T/C-levels in patients receiving a CI. Together with the electrode contact impedance and the contact position in the electrode array it can explain up to 37% of the variability in the definition of the C-levels. The fact that the stabilised C-levels measured 4–6 months postoperatively can be up to 30 CL higher in the prelingually deaf patients than in the postlingual ones results also in a much higher dynamic range observed in prelingual subjects. Therefore implant programming of the prelingual patients should be very cautious in order to avoid the risk of overstimulation.

Purpose There is still a clinical-radiologic discrepancy in patients with Meniere's disease (MD). Therefore, the purpose of this study was to investigate the reliability of current MRI endolymphatic hydrops (EH) criteria according to Barath in a larger study population and the clinical utility of new imaging signs such as a supplementary fourth low-grade vestibular EH and the degree of perilymphatic enhancement (PE) in patients with Meniere's disease (MD). Methods This retrospective study included 148 patients with probable or definite MD according to the 2015 American Academy of Otolaryngology, Head and Neck Surgery criteria who underwent a 4-h delayed intravenous Gd-enhanced 3D-FLAIR MRI between January 2015 and December 2016. Vestibular EH, vestibular PE, cochlear EH, and cochlear PE were reviewed twice by three experienced readers. Cohen's Kappa and multivariate logistic regression were used for analysis. Results The intra- and inter-reader reliability for the grading of vestibular-cochlear EH and PE was excellent (0.7 < kappa < 0.9). The two most distinctive characteristics to identify MD are cochlear PE and vestibular EH which combined gave a sensitivity and specificity of 79.5 and 93.6%. By addition of a lower grade vestibular EH, the sensitivity improved to 84.6% without losing specificity (92.3%). Cochlear EH nor vestibular PE showed added-value. Conclusions MRI using vestibular-cochlear EH and PE grading system is a reliable technique. A four-stage vestibular EH grading system in combination with cochlear PE assessment gives the best diagnostic accuracy to detect MD.

The human nose is a complex organ that shows large morphological variations and has many important functions. However, the relation between shape and function is not yet fully understood. In this work, we present a high quality statistical shape model of the human nose based on clinical CT data of 46 patients. A technique based on cylindrical parametrization was used to create a correspondence between the nasal shapes of the population. Applying principal component analysis on these corresponded nasal cavities resulted in an average nasal geometry and geometrical variations, known as principal components, present in the population with a high precision. The analysis led to 46 principal components, which account for 95% of the total geometrical variation captured. These variations are first discussed qualitatively, and the effect on the average nasal shape of the first five principal components is visualized. Hereafter, by using this statistical shape model, two application examples that lead to quantitative data are shown: nasal shape in function of age and gender, and a morphometric analysis of different anatomical regions. Shape models, as the one presented here, can help to get a better understanding of nasal shape and variation, and their relationship with demographic data.
