Giant adrenal myelolipoma (AML) may cause severe symptoms. In contrast to the previous reports, laparoscopy may play a pivotal role in the management of giant AML. This report aims to discuss a case of giant AML managed successfully by laparoscopy. A 63-year-old male was found to have a giant (12 × 10 × 8 cm) left AML during a workup for left lower chest pain on imaging. laparoscopic excision of a left adrenal gland with the lesion was performed under general anesthesia. The patient was discharged from the hospital after 3 days uneventfully. AML is a benign tumor that is characterized by the presence of adipose tissue and hematopoietic elements. Myelolipomas are typically asymptomatic. AML diagnosis is based on imaging and blood workup. Small asymptomatic AML is usually managed conservatively, while symptomatic AML is managed with surgery. Even though an open approach is the standard option, laparoscopy, as a minimally invasive technique, in some centers may replace laparotomy. Laparoscopy can be a successful method for managing AML, even when they are large in size.

Although self-citation is a common practice among scholars, its impact and significance remain under scrutiny within the academic community. The current study aimed to provide a detailed analysis and ranking of the top 2 % scientists on self-citation, ranked by Stanford University researchers. Data extraction and organisation were performed between January and February 2024. Self-citation percentages were collected for authors belonging to 20 fields and 174 subfields using Excel spreadsheets. Entities were categorised into quartiles based on their self-citation percentages and rankings were assigned accordingly. Comparative analyses were conducted to assess the impact of self-citations on overall rankings. The study focused on data published by a group of researchers from Stanford University regarding the top 2 % of researchers, analysing the career-long records of 204,643 scholars and 210,198 scholars for the most recent year, 2022. Notably, in the single-year analysis, among the top 20 countries, approximately eight (40 %) exhibited self-citation percentages exceeding the average of 25.96 %. The self-citation percentages ranged from 4.47 % in Economics and Business to 20.88 % in Physics and Astronomy. Regarding career-long analysis, the percentage of self-citations ranged from 22.84 % in Poland to 41.31 % in Armenia, with significant drop in rankings among most entities when self-citations were excluded. These findings highlight the dramatic impact of self-citation exclusion on the rankings of the top 2 % of the researchers, underscoring the critical importance of accounting for self-citation in ranking assessments.

Abstract Background Carotid body tumors are uncommon neuroendocrine growths near the carotid bifurcation. While some advocate preoperative embolization to minimize bleeding, others avoid it due to complications. This study shares the experience of a single center in managing patients with carotid body tumors without practicing preoperative embolization. Methods This was a cross-sectional study of patients with carotid body tumors managed between 2020 and 2024. Data were collected from the hospital’s registry. When necessary, routine blood tests, neck ultrasonography, and computed tomography scans were conducted. The tumors were categorized according to Shamblin’s classification. The average duration of follow-up was 20 months. Results The study involved 25 patients, 22 (88%) females and 3 (12%) males. Their ages ranged from 27 to 85 years old. Twenty (80%) cases presented with neck swelling, and six (24%) had a positive medical history. Tumors were mainly on the right side (52%), with 20 (80%) showing ill-defined neck masses. Introduction: In ureterolithiasis, the prediction of spontaneous passage poses a challenge for urologists. Moreover, there is controversy surrounding the preferred management approach, whether medical or surgical, as each approach has its disadvantages. Procalcitonin and other inflammatory markers were studied for predicting stone passage spontaneously, but their significance remains controversial. This study aims to assess the association between these markers, especially procalcitonin, and spontaneous ureteral stone passage. Materials and methods: In this multicenter prospective cohort study from March 2022 to October 2023, consecutive patients with a single unilateral distal ureteric stone less than 10 mm were enrolled. Exclusion criteria were specified. Patients underwent medical expulsive therapy (MET) and were monitored for stone passage. The significance level was set at p < 0.05. Results: Out of 94 patients enrolled, 72.3% were male and 27.7% were female, with a mean age of 38.84± 10.41 years. Stone sizes varied, with the most common range being 4 mm- 5.9 mm. Participants were categorized based on spontaneous stone passage as spontaneous stone passage (SSP) and non-SSP. No significant differences were observed in most demographic and laboratory variables. However, serum procalcitonin and C-reactive protein showed significant differences between the SSP and non-SSP groups. Conclusions: Although several inflammatory markers were studied to predict the spontaneous passage of the ureteral stone, the current study concluded that only elevated procalcitonin, C-reactive protein, and large stone diameter decrease the chance of spontaneous ureteral stone passage.

IntroductionA Urethral diverticulum can be defined as sac-like dilation lined with epithelial tissue, which may be congenital or acquired. It usually develops in the penoscrotal angle region but can also be observed in the penile urethra. It usually occurs in female teenagers. This report aims to discuss a male infant with a large urethral diverticulum.Case presentationA 5-month-old male presented to the urological department at Sulaimani Teaching Hospital with a penile swelling that had been noticeable since birth. Clinical examination revealed a ventral cystic penile shaft swelling, which would fill with fluid during urination. A urethrocystoscopy was performed and showed a wide cystic ventral diverticulum. Diverticulectomy was performed as a surgical approach to remove the diverticulum.DiscussionCongenital anterior urethral diverticulum is an uncommon condition that typically begins in early life. It can manifest with various symptoms, like recurrent infections of the urinary tract, painful urination, and post-void urine dribbling. Diagnosis involves imaging, with urethrocystoscopy, to rule out other potential diagnoses. Different surgical techniques exist that show promising results in preventing recurrence. The current case involved diverticulectomy and multi-layered wound closure with a dartos flap.ConclusionLarge anterior diverticulum in early infancy is rare but possible; operation is the preferred intervention method.

IntroductionWhile different methods are employed for fixing narrowed nasal valves and preventing the notching of soft triangles, this study aims to demonstrate the effectiveness of a new technique called Süreyya-Dani Technique.MethodsThis prospective study composed 100 patients who underwent rhinoplasty using the Süreyya-Dani technique. All patients presented with either notching of the soft triangle and/or external nasal valve dysfunction. Patients with the absence of soft triangle notching and external nasal valve dysfunction were excluded from this study.
