1. Salivary melatonin levels in correlation with sleep quality, quality of life and clinical features in patients with chronic spontaneous urticaria

Uvod: U većine bolesnika s kroničnom spontanom urtikarijom (KSU), veliki problem predstavlja svrbež koji se učestalije javlja u večernjim i noćnim satima što uzrokuje poteškoće sa spavanjem. Za postizanje dobre kvalitete sna (KS) je ključan melatonin, hormon koji sudjeluje u započinjanju i održavanju ciklusa spavanja te je izuzetno bitna njegova adekvatna proizvodnja. Cilj ovog istraživanja je bio ispitati vrijednosti salivarnoga melatonina te povezanost težine bolesti i poremećaja kvalitete života (KŽ) i KS u tih bolesnika. Metodologija: U 38 bolesnika s KSU-om i 38 ispitanika kontrolne skupine određivan je salivarni melatonin (ELISA), KŽ (“Dermatology Quality of Life”) i KS (“Pittsburgh Sleep Quality Index”), a u bolesnika s KSU-om je dodatno određivana težina (“Urticaria Activity Score”) i kontrola bolesti (“Urticaria Control Test”). Rezultati: Sniženi salivarni melatonin je uočen u 34/38 bolesnika s KSU. Teški oblik urtikarije imalo je 12/38 ispitanika, a značajno narušenu KŽ 8/38 bolesnika. Težina urtikarije je linearno proporcionalno povezana s KŽ i KS u tih bolesnika, uz jaku korelaciju s KŽ i slabu korelaciju s KS, dok koncentracija salivarnoga melatonina nije povezana ni s KS ni s KŽ. Zaključak: Ovo su prvi literaturni podaci u svijetu o sniženim vrijednostima melatonina, salivarnog biomarkera poremećaja spavanja, u bolesnika s KSU-om. Introduction: For the majority of patients with chronic spontaneous urticaria (CSU), a big problem is the nocturnal itch, which leads to sleep disturbance. Melatonin regulates promoting and maintaining the sleep cycle and its adequate production is crucial for good sleep quality (SQ). The aim of this study was to examine salivary melatonin and the association between disease severity, quality of life (QoL) and SQ in these patients. Methodology: Salivary melatonin (ELISA), QoL (“Dermatology Quality of Life”) and SQ (“Pittsburgh Sleep Quality Index “) were determined in 38 patients with CSU and 38 healthy subjects. In patients with CSU, the severity of the urticaria (“Urticaria Activity Score”) and the disease control (“Urticaria Control Test”) were additionally determined. Results: Decreased salivary melatonin was observed in 34/38 patients with CSU. Severe form of urticaria was detected in 12/38 patients and QoL was significantly impaired in 8/38 patients. The severity of urticaria was linearly proportional to QoL and SQ in these patients, with a strong correlation with QoL and a weak correlation with SQ, while the salivary melatonin was not related to QoL nor SQ. Conclusion: These are the first literature data in the world indicating decreased levels of the salivary melatonin in patients with CSU.
