OBJETIVO: avaliar a proliferação de células T e a produção de citocinas em gestantes infectadas pelo HIV-1 e seu impacto na replicação viral in vitro. MÉTODOS: sangue periférico de 12 gestantes infectadas pelo HIV-1 e de seus neonatos, bem como de 10 gestantes HIV-1 negativas, foi colhido e a quantidade de linfócitos TCD4+ e TCD8+ periféricos foi avaliada por citometria de fluxo. Para obter plasma ou células mononucleares periféricas (PBMC), as amostras foram centrifugadas na ausência ou presença de um gradiente de Ficoll-Hypaque, respectivamente. As PBMC foram mantidas em cultura por sete dias na presença de fito-hemaglutinina mais IL-2 recombinante e a resposta linfoproliferativa de células T foi analisada pelo método de exclusão em azul de Trypan. Em alguns experimentos, as culturas foram mantidas na presença adicional de anticorpo anti-IL-10. Os plasmas e sobrenadantes das culturas de PBMC ativadas foram submetidos à análise da produção de citocinas, pelo método ELISA indireto, e a carga viral, detectada pelo RT-PCR. Despite being considered a disease mediated by autoantibodies, different studies suggest the participation of CD4+ T cell subtypes and its severity has been associated with the influence of genetic and environmental factors, such as infections. A recent study published by our group demonstrated a high expansion of Th17 cells positives for TLR2, TLR4 and TLR9 in the peripheral blood of NMOSD patients. Therefore, the objective of this study was to evaluate the ability of different PAMPs in modulating the in vitro functional status of CD4+ T cells in those patients and its correlation with clinical parameters. For this, peripheral blood samples were collected from 15 NMOSD patients, in clinical remission, and CD4+ T cells were purified by negative selection using magnetic columns. The plasmatic analysis of cytokines and sCD14 were done by the ELISA technique. CD4+ T cells were maintained in the presence of medium alone or TLR2 (Pam3C), TLR4 (LPS), TLR5 (FLA) and TLR9 (ODN) agonists. Their ability to modulate the activation of these lymphocytes via TCR was assessed following addition of anti-CD3/anti-CD28. After 3 days, the proliferative response and cytokine production were determined by [3H] TdR uptake and ELISA, respectively. Our results demonstrated that both cell proliferation and IL-6, IL-17 and IL-21 production were higher in the supernatants of NMOSD-derived CD4+ T cell cultures in response to LPS, Pam3C and FLA, as compared with the control group. In addition, all these PAMPs amplified the TCR-dependent lymphocytes activation. A positive correlation was observed between the degree of neurological disability of the patients and the secretion of cytokines related to the Th17 profile in cell cultures maintained in the presence of Pam3C and LPS. Plasma sCD14 levels were higher in the patients, in purchased with the control group, and their levels were directly correlated to IL-6 and IL-1β cytokines, measured in vivo, and IL-6, IL-17 and IL-21 levels produced by CD4+ T cells in response to LPS. The same correlation was observed between sCD14 and IL-6 and IL-17 released byCD4+ T cells from patients in response to Pam3C. Although sCD14 levels were not associated with clinical parameters, the production of IL-6, IL-17 and IL-21 by CD4+ T cells in response to Pam3C and LPS, as well as IL-17 and IL-21 following addition of FLA, were positively correlated to the degree of neurological disability. The risk of new relapses was mainly detected in patients whose CD4+ T cell cultures responded to LPS releasing high levels of IL-6 and IL-17. In summary, our results suggest that TLR ligands and, especially, TLR2 and TLR4 agonits may exert adverse effects in the course of NMOSD. Although preliminary, these findings help to explain how infections and dysbiosis affect NMOSD severity. As doenças do espectro da neuromielite óptica (NMOSD) são condições autoimunes do sistema nervoso central raras e sem cura, com desfecho mais desfavorável que a esclerose múltipla. Apesar de ser considerada uma doença mediada por autoanticorpos, diferentes estudos sugerem a participação de subtipos de células T CD4+ e sua gravidade tem sido associada à influência de fatores genéticos e ambientais, tal como as infecções. Como um estudo recente publicado pelo nosso grupo demonstrou elevada expansão de células do tipo Th17 positivas para TLR2, TLR4 e TLR9 no sangue periférico dos pacientes com NMOSD, o objetivo do atual estudo foi avaliar a capacidade de diferentes PAMPs em modular in vitro o status funcional das células T CD4+ desses pacientes e sua correlação com parâmetros clínicos. Para tal, amostras de sangue periférico foram colhidos de 15 pacientes com NMOSD, em remissão clínica, e as células T CD4+ purificadas através da seleção negativa usando colunas magnéticas. A análise das citocinas e do sCD14 no plasma foi feita através da técnica ELISA. As células T CD4+ foram mantidas na presença apenas de meio ou dos agonistas de TLR 2 (Pam3C), TLR4 (LPS), TLR5 (FLA) e TLR9 (ODN). A sua capacidade em modular a ativação desses linfócitos via TCR foi avaliada através da adição de anti-CD3/anti-CD28. Após 3 dias, a resposta proliferativa e a produção de citocinas foram determinadas através da captura de [3H]TdR e ELISA, respectivamente. Nossos resultados demonstraram que a proliferação assim como a produção de IL-6, IL17 e IL-21 foram superiores nos sobrenadantes das culturas de células T CD4+ dos pacientes em resposta ao estímulo com LPS, Pam3C e FLA quando comparado ao grupo controle. Ademais, todos esses PAMPs amplificaram a ativação desses linfócitos via TCR. Uma correlação positiva foi observada entre o grau de incapacidade neurológica dos pacientes e a secreção de citocinas relacionadas ao perfil Th17 nas culturas mantidas na presença de Pam3C e LPS. Em comparação ao grupo controle, as concentrações plasmáticas de sCD14 foram maiores nos pacientes, e seus níveis foram diretamente correlacionados às citocinas IL-6 e IL-1β, dosados in vivo, e aos níveis de IL-6, IL-17 e IL-21 produzidos pelas células T CD4+ em resposta ao LPS. A mesma correlação foi observada entre sCD14 e a IL-6 e IL-17 liberados pelas células T CD4+ dos pacientes em resposta ao Pam3C. Apesar dos níveis de sCD14 não terem sido associados aos parâmetros clínicos, a produção de IL-6, IL-17 e IL-21 pelas células T CD4+ em resposta ao Pam3C e LPS, assim como, IL-17 e IL-21 seguindo à adição de FLA, foram positivamente correlacionados ao grau de incapacidade neurológica. O risco de novas recaídas foi principalmente detectado nos pacientes cujas culturas de células T CD4+ responderam ao LPS liberando elevados níveis de IL-6 e IL-17. Em resumo, os nossos resultados sugerem que a presença de ligantes de TLR, principalmente, de TLR2 e TLR4 possa exercer efeitos adversos no curso da NMOSD. Apesar de preliminares, esses achados ajudam a explicar como infecções e disbiose são consideradas fatores de risco para gravidade da NMOSD.

Many studies have suggested that immunological activity in response to HCV antigens is strongly related to liver fibrosis. The role of different IL-17 secreting CD4+ (Th17) and CD8+ (Tc17) T cells in CHCis not fully understood. In our study, elevated plasma levels of IL-1β, IL-6 and IL-17 were positively correlated with fibrosis and alanine transaminase (ALT) rates in patients with CHC. In addition, these cytokines, secreted by peripheral blood mononuclear cell (PBMC) cultures in response to core antigen from virus, were also correlated to advanced liver injury. While the percentage of IL-17 producing Th17/Tc17 cells in association with IFN-γ or IL-21 was higher in patients with lower liver damage, accumulation of single IL-17-producing T lymphocytes was correlated with liver fibrosis. Interestingly, core protein favored the expansion of Th17/Tc17 cell subtypes related to disease progression, identified by production of IL-17 alone or IL-17 in association with IL-6. Further, although the percentage of Tregs FoxP3+CD25+CD4+) was higher in CHC patients with more advanced disease, the proportions of both TregsCD39+or Treg17 (IL-17+ IL-10 +) subsets were significantly lower in these individuals. Addition of HCV core protein to the PBMC cultures elevated the percentage of CD39-Tregs cells. In addition, the percentage of exhausted (PD-1+ CD28+) and senescent (CD28-CD57+) T cells were not only higher in patients with advanced hepatic fibrosis, but also their proportion were elevated by core antigen. Finally, we believe that the presence of a persistent and pro-fibrotic inflammatory state is facilitated by an elevated bacterial translocation, since, in the present study, plasma levels of sCD14 were not only higher in patients with worse prognosis, but their concentrations were directly correlated with the systemic levels of IL-1β, IL-6 and IL-17, as well as the percentage of Th17 / Tc17 cells negatives for IL-21 and IFN-γ. In summary, although preliminary, our results suggest that the expansion of different Th17 / Tc17 and Tregs cell subsets may influence the evolution of liver disease in patients with CHC, and reveals that the core antigen may exert adverse effects by amplifying the imbalance among these different subtypes of these T cells. In addition, inflammatory events favoring the expansion of different Th17/Tc17 cells implicated in hepatic fibrosis may be favored by greater intestinal microbial translocation. A hepatite C crônica (HCC) é frequentemente associada à fibrose hepática, o que pode levar a cirrose e ao carcinoma hepatocelular. Muitos estudos têm sugerido que a atividade imunológica em resposta aos antígenos do HCV está fortemente relacionada à fibrose hepática. O papel de diferentes células T CD4+ (Th17) e CD8+ (Tc17) secretoras da IL-17 na HCC não está completamente esclarecido. Em nosso estudo, níveis plasmáticos elevados de IL-1β, IL-6 e IL-17 foram positivamente correlacionados com a fibrose e também com as taxas de alanina transaminase (ALT) nos pacientes com HCC. Além disso, essas citocinas, secretadas por culturas de células mononucleares do sangue periférico (PBMC) em resposta ao antígeno core viral, também foram correlacionadas à lesão hepática avançada. Enquanto a percentagem de células Th17/Tc17 produtoras de IL-17 em associação com o IFN-γ ou com IL-21 foi maior nos pacientes com menor dano hepático, acúmulo desses linfócitos capazes de produzir apenas IL-17 foi correlacionada à fibrose do fígado. De forma interessante, a proteína core favoreceu a expansão de subtipos de células Th17/Tc17 relacionadas ao avanço da doença, identificadas pela produção apenas de IL-17 ou IL-17 em associação com IL-6. Ademais, apesar do percentual das células Tregs (CD4+FoxP3+CD25+) ter sido maior nos pacientes HCC mais avançada, as proporções tanto dos subtipos TregsCD39+ou Treg17 (IL-17+IL-10+) foram significativamente inferiores nesses indivíduos. A adição da proteína core do HCV às culturas de PBMC elevou a percentagem das células Tregs CD39-. Ademais, uma maior percentagem de células T exauridas (PD-1+CD28+) e senescentes (CD28- CD57+) não apenas foram maiores nos pacientes com fibrose hepática acentuada, como também a proporção desses linfócitos foram elevados após adição do antígeno core. Finalmente, acreditamos que a presença de um estado inflamatório persistente e pró-fibrótico é facilitado pela maior translocação bacteriana, desde que, nos presentes estudos, os níveis plasmáticos de sCD14 não apenas foram maiores nos pacientes com pior prognóstico, como também suas concentrações foram diretamente correlacionadas aos níveis sistêmicos de IL-1β, IL-6 e IL-17, assim como ao percentual de células Th17/Tc17 negativas para IL-21 e IFN-γ. Em resumo, apesar de preliminares, nossos resultados sugerem que a expansão de diferentes subtipos de células Th17/Tc17 e de Tregs pode influenciar a evolução da doença hepática nos pacientes com HCC, e revela que o antígeno core pode exercer efeitos adversos por amplificar o desequilíbrio entre esses diferentes subtipos de células T. Ademais, eventos inflamatórios que favorecem a expansão de subtipos de células Th17/Tc17 implicados na fibrose hepática podem ser favorecidos pela maior translocação microbiana intestinal.

In light of that, in our first paper, we observed that the in vitro blockage of IL-6 receptor, using anti-IL-6R IgG, reduced the Th17-related cytokines production. In addition, the ability of supernatants from lipolyssacharide (LPS)-activated monocytes cultures, in increasing IL-17 production and reducing IL-10 levels by autologous CD4+ T cells was abolished after neutralization of IL-6R signaling in these phagocytes' cultures. Interestingly, the addition of this monoclonal antibody increased the ability of hydrocortisone in inhibiting proliferation and IL-17 production by CD4+ and CD8+ T cells from MS patients. In our second paper, we observed that the stress-related dopamine levels, by IL-6-depending mechanisms, elevated the cytokine production by CD4+ T cells with an encephalitogenic profile and hydrocortisone-resistance. In our third and last paper, an elevated percentage of circulating CD4+ and CD8+ T cells capable of expressing toll-like receptors (TLR)2, TLR4 and TLR9 was detected in MS patients samples as compared to the control group. Among these cells, the subtypes of CD4+ and CD8+ T lymphocytes able to produce IL-17 with along IL-6 or IFN-gamma were positively related with the number of active brain lesions and the neurological disability. When CD4+ and CD8+ T cells were maintained in the presence of TLR ligands alone, greater levels of Th17-related cytokines were dosed at the supernatants of cell cultures stimulated with the TLR2L, Pam3CSK4. In these cell cultures, the levels of IL-6 and IL-17 in response to Pam3CSK4 were positively related to clinical parameters of the disease. Finally, CD4+ T cells cultures from patients with severe MS symptoms released higher levels of GM-CSF and IFN-gamma. Collectively, our findings suggest that IL-6 has an important role in MS immunopathogenesis and the production of this cytokine, as well as others Th17-related ones, are increased by dopamine and different PAMPs. The findings obtained here help not only to explain how psychological stress and infectious diseases are considered risk factors for MS, but they offer information that could aid in the development of new therapeutic strategies. A esclerose múltipla (EM) é uma doença desmielinizante, de caráter autoimune, do sistema nervoso central, cujo o desenvolvimento e gravidade dependem da influência de fatores ambientais, incluindo estresse psicológico e infecções, no sistema imune de indivíduos com predisposição genética para a doença. Os mecanismos pelos quais esses fatores impactam negativamente na EM podem estar relacionados ao favorecimento de vias de ativação e expansão de clones de linfócitos T encefalitogênicos. Diante do exposto, em nosso primeiro artigo, observamos que o bloqueio in vitro do receptor de IL-6 reduziu a produção de citocinas relacionadas ao perfil Th17. Ademais, a capacidade dos sobrenadantes de monócitos, ativados com lipopolissacarídeo (LPS), em potencializar a produção da IL-17 e reduzir os níveis de IL-10 pelas células T CD4+ de pacientes com EM foi anulada após a neutralização da sinalização via IL-6R nas culturas desses fagócitos. De forma interessante, a adição desse anticorpo monoclonal potencializou a capacidade da hidrocortisona (HC) em inibir a proliferação e produção de IL-17 pelas células T CD4+ e T CD8+ dos pacientes com EM. No segundo artigo, nós observamos que níveis de dopamina relacionados ao estresse, por mecanismos dependentes da IL-6, elevaram a produção de citocinas por células T CD4+ com perfil encefalitogênico e resistentes à HC. Em nosso último artigo, elevada percentagem de células T CD4+ e T CD8+ circulantes capazes de expressar receptores do tipo Toll (TLR)2, TLR4 e TLR9 foram detectadas no sangue periférico dos pacientes com EM quando comparado ao grupo controle. Dentre essas células, os subtipos de células T CD4+ e T CD8+ capazes de produzir simultaneamente IL-17 com IL-6 ou com IFN-γ foram diretamente associados ao número de lesões cerebrais ativas e grau de incapacidade neurológica. Quando as células T CD4+ e T CD8+ foram mantidas na presença apenas de ligantes de TLR (TLR-L), maiores níveis de citocinas relacionados ao fenótipo Th17 foram dosados nos sobrenadantes recolhidos nas culturas estimuladas com TLR2-L, o Pam3CSK4. Nessas culturas, os níveis de IL-6 e IL-17 em resposta ao Pam3CSK4 foram diretamente correlacionados aos parâmetros clínicos da doença. Finalmente, culturas de células T CD4+ dos pacientes com EM mais grave responderam ao agonista de TLR2 liberando elevados níveis de GM-CSF e IFN-γ. Coletivamente, nossos achados sugerem que a IL-6 exerce um papel importante na imunopatogênese da EM, sendo a produção dessa citocina, assim como de outras relacionadas a diferentes subtipos de células Th17, elevada pela dopamina e diferentes padrões moleculares associados a patógenos. Os achados obtidos aqui ajudam não apenas a explicar como o estresse psicológico e doenças infecciosas são considerados fatores de risco para a EM, como também podem ajudar na criação de novas estratégias terapêuticas.

Despite genetic susceptibility, development and severity of NMOSD are influenced by environmental factors, like microbial molecules, that can impact the functional behavior of the T cells phenotype mainly implicated in NMOSD, the Th17 phenotype. In this context, the objective of the present thesis is to analyze functional aspects of CD4+ T cells from NMOSD patients, in remission, following in vitro activation with polyclonal and microbial stimulation and determine the Toll-like receptors (TLRs) expression pattern on these cells. In our first paper, the results showed a high immune reactivity of Th17 cells in response to E. coli, characterized by enhanced production of IL-17 and IL-6,. Interestingly, levels of these cytokines were directly associated to the disease severity, determined by the Expanded Disability Status Scale (EDSS) scores, and to IL-6 and IL-23 production in monocytes cultures when activated with lipopolysaccharide (LPS) from E. coli. The role of IL-6 as a cytokine involved in the pathogenesis of NMOSD was suggested in our second paper, in which the inhibition of the IL-6 receptor (IL-6R) signaling reduced the Th17-related cytokine production, enhanced IL-10 production and the ability of the corticoid in controlling the activation of these cells in vitro. As higher levels of LPS were detected in plasma from NMOSD patients, and they were positively correlated with EDSS scores, our last objective was to evaluate TLR expression on CD4+ T cells derived from patients. Our results showed an enhanced percentage of CD4+ T cells expressing TLR2, 4 and 9 when compared to healthy subjects. A more detailed phenotypical characterization demonstrated that a percentage of cells capable of producing, simultaneously, IL-17 and IL-6, was consistently associated to plasmatic levels of sCD14 and to NMOSD severity. On the other hand, IL-10-secreting Th17 TLR+ cells correlated inversely to the sCD14 levels and to EDSS scores. Taken together, our results suggest microbial products can effectively influence NMOSD prognosis by favoring the proportion of different TLR-expressing Th17 subtypes. This type of investigation needs to be conducted in a higher number of patients because, if confirmed, these data can help in the development of new therapies for NMOSD. As Doenças do Espectro da Neuromielite Óptica (NMOSD, do inglês neuromyelitis optica spectrum disorders ) são um conjunto de doenças autoimunes, inflamatórias, idiopáticas, do sistema nervoso central que, diferente da Esclerose Múltipla, cursam principalmente com episódios de neurite óptica e/ou mielite aguda. Apesar da suscetibilidade genética, o desenvolvimento e gravidade da NMOSD são influenciados por fatores ambientais, como produtos microbianos, que podem impactar o comportamento funcional das células T mais implicadas na NMOSD, o fenótipo Th17. Nesse sentido, o objetivo da presente tese foi analisar aspectos funcionais de células T CD4+ de pacientes com NMOSD em remissão, através da ativação in vitro com estímulos policlonais e microbianos, assim como determinar o padrão de expressão de receptores do tipo Toll (TLR) nessas células. Em nosso primeiro artigo, os resultados demonstraram uma alta resposta das células Th17 à E. coli, caracterizada pela produção de altos níveis de IL-17 e IL-6 nos pacientes. De forma interessante, os níveis dessas citocinas foram diretamente associados à gravidade da doença, determinada pela escala expandida do estado de incapacidade (EDSS), do inglês Expanded Disability Status Scale ), e à produção de IL-6 e IL-23 por monócitos ativados pelo lipopolissacarídeo (LPS) de E. coli. O papel da IL-6 como citocina envolvida na patogênese da NMOSD foi sugerido em nosso segundo artigo que mostrou que a inibição da sinalização via receptor da IL-6 reduziu a produção das citocinas relacionadas ao fenótipo Th17, aumentou a produção de IL-10 e a capacidade do corticoide em controlar a ativação dessas células in vitro. Como elevados níveis de LPS foram detectados nos plasmas dos pacientes com NMOSD, e estes foram positivamente correlacionados com a escala do EDSS, nosso último objetivo, foi avaliar a expressão de TLR nas células T CD4+ desses pacientes. Nossos resultados, demonstraram elevada percentagem de células T CD4+ expressando TLR2, 4 e 9 nos pacientes, quando comparado a indivíduos saudáveis. Ademais, a percentagem de células capazes de produzir simultaneamente IL-17 e IL-6 foi associada aos níveis plasmáticos de CD14 solúvel (sCD14) e à gravidade da NMOSD. Por outro lado, células Th17 TLR+ secretoras de IL-10 correlacionaram inversamente com os níveis de sCD14 e com o EDSS. Em conjunto, nossos resultados sugerem que produtos microbianos podem influenciar o prognóstico da NMOSD por favorecer diferentes subtipos de células Th17 TLR-positivas. Esse tipo de investigação precisa ser conduzido em um número maior de pacientes, pois, se confirmado, esses dados podem ajudar no desenho de novas terapias para a NMOSD.

Few studies have described the molecular epidemiology of MR Mycobacterium tuberculosis isolates in Brazil. In this study, we investigated the genetic diversity and mutations associated with drug resistance of 99 MR and 7 non-MR isolates collected in a 8-year period from 12 Brazilian states. This investigation was performed by restriction fragment length polymorphism analysis of the IS6110 insertion element (IS6110-RFLP), spoligotyping and sequencing of the rpoB and katG genes that, when mutated, confer resistance to isoniazid and rifampin, respectively. Mutations in rpoB and katG were found in 90.9% and 93% of the MR isolates analyzed, respectively. For the rpoB gene, 91.9% of the mutations were found in the 81-bp RRDR region Fifty-one (51.5%), 23 (23.3%) and 11 (11.1%) MR isolates had mutations in codons 531, 526 and 516, respectively. In the katG gene, mutations were found in 93% of the MR isolates analyzed. Seven isolates had mutations only in the first region analyzed (katG1). Codon 315 of the second katG gene region analyzed (katG2) had mutations in 82.8% of the MR isolates, mostly being a Ser315Thr substitution. Mutations were found in 30.3% of the MR isolates in the katG1 region and the majority was a codon 4 deletion. Spoligotyping detected 5 different M. tuberculosis families (with their subfamilies) in the MR isolates in this study. The most common were: LAM (46%), T (17%) and H (12%). We observed that one of the families, the EAI5, carries mutations in codon 463 of the katG gene, which does not occur for the other isolates. In addition, among our isolates we identified an isolate belonging to the Beijing family (highly virulent), but this fact is not alarming since this was only one case. We described new alleles for the rpoB and katG genes. With the exception of the X2 family, we identified an initial region of the katG gene with high frequency of mutations in the MR isolates. IS6110-RFLP analysis demonstrated that 25 isolates formed 11 genotypic groups while 74 showed a unique pattern of bands. This high rate of polymorphism indicates independent acquisition of resistance among our isolates. Concerning the H family, an inversion in the frequency of mutations occurred in the rpoB gene with codon 516 being the most mutated one followed by codons 526 and 531. The results of this study provide useful data for better understanding the spectrum of mutations of the MR isolates from patients in Brazil. Our results are also useful for the development of diagnostic tests for MR tuberculosis and to aid in tracking global transmission of this disease. A tuberculose multirresistente (MR) a drogas é uma séria ameaça à saúde pública devido à maiores complexidade, custo e efeitos colaterais do tratamento. Poucos estudos descreveram a epidemiologia molecular de isolados de Mycobacterium tuberculosis MR no Brasil. Neste trabalho foi investigada a diversidade genética e mutações associadas à resistência a drogas de 99 isolados MR e 7 não MR coletados em um período de 8 anos e provenientes de 12 estados brasileiros. Esta investigação foi feita através da análise do polimorfismo de fragmentos de restrição do elemento de inserção IS6110 (IS6110-RFLP), spoligotyping e sequenciamento de regiões dos genes rpoB e katG que conferem resistência aos antibióticos rifampicina e isoniazida, respectivamente. Mutações nos genes katG e rpoB foram encontradas em 90,9% e 93% dos isolados MR analisados, respectivamente. Para o gene rpoB, 91,9% das mutações estavam contidas na região RRDR de 81-pb. Um total de 51 (51.5%), 23 (23.3%) e 11 (11.1%) isolados MR apresentaram mutações nos códons 531, 526 e 516, respectivamente. Com relação ao gene katG, foram encontradas mutações em 93% dos isolados MR analisados, sendo que 7 apresentaram mutações apenas na primeira região analisada (katG1). O codon 315 da segunda região analisada do gene katG (katG2) apresentou mutações em 82.8% dos isolados MR, sendo a maioria Ser315Thr. A região katG1 apresentou mutações em 30.3% dos isolados MR sendo a maioria deleção do códon 4. Pelo spoligotyping foi possível determinar que os isolados MR deste estudo pertencem a 5 diferentes famílias (com suas subfamílias) de M. tuberculosis circulantes no Brasil, onde as mais frequentemente encontradas foram: LAM (46%), T (17%) e H (12%). Nós observamos que uma das famílias, a EAI5, carrega mutações no códon 463 do gene katG, o que não ocorre para as demais. Além disso, entre nossos isolados foi identificada um isolado pertencente à cepa Beijing (extremamente virulenta), mas este fato não é alarmante já que se tratou de apenas um caso. Através de nossos dados foram descritos novos alelos mutados para os genes rpoB e katG. Com exceção da família X2, foi identificada uma região inicial do gene katG com alta frequência de mutações nos isolados MR. A análise por IS6110-RFLP revelou que 25 isolados formaram 11 grupos genotípicos enquanto 74 mostraram um padrão único de bandas. Esta alta taxa de polimorfismo indica aquisição independente de resistência entre nossos isolados. Para a família H, foi identificada uma inversão na freqüência de ocorrência de mutações no gene rpoB, sendo o códon 516 o mais mutado, seguido pelo 526 e 531. Os resultados deste estudo fornecem informações úteis para um melhor entendimento do espectro de mutações dos isolados MR de pacientes no Brasil. Nossos resultados também se tornam úteis no desenvolvimento de testes diagnósticos de tuberculose MR e para auxiliar no rastreamento da transmissão global desta doença.

The immunological nuances resulting from the immunosenescence phenomenon may promote significant alterations in the clinical course of HIV infection, as well as in treatment response. The major purpose of this Thesis was to evaluate the impact of age on the functional immune recovery in aids patients aged more than 55 years, when adequately treated with anti-retroviral therapy, characterizing the immunological result of advanced age and HIV infection. Thus, four experimental groups were enrolled: healthy or HIV-infected young adults, and healthy or HIV-infected adults over 55 years old. All the HIV-infected patients had diagnosis of aids and were under anti-retroviral treatment with therapeutic success. In the first presented article, we evaluated the lymphoproliferative response and cytokine production in vitro and humoral response in vivo, after antigenic challenge with tetanus toxoid (TT) in previously immunized individuals against tetanus. The results revealed significant age-related immunological impairments concerning anti-TT IgG production, lymphoproliferative response and production of IFN-γ. On the other hand, the production of IL-10 significantly higher in individuals aged more the 55 years, HIV-infected or not. In the second article, T cell subsets were characterized after polyclonal activation or specific stimulus with antigens derived from the HIV envelope (Env). In fresh unstimulated PBMC cultures obtained from the aged aids patients, there was a reduced frequency of naïve and central memory T cells, associated with increased frequency of effector T cells. When polyclonally stimulated, these cultures showed deficient production of IFN-γ and hyperproduction of IL-10, like in response to TT. In Env-stimulated cultures, flow cytometry revealed high frequency of T CD4+FoxP3-CD152+ T cells with strong intracellular staining for IL-10, indicating a dominant Tr-1 phenotype, and not the classical Treg cells. Interestingly, in both articles, the viral replication in vitro was significantly lower aids patients over 55 years old, which is in consonance with their excellent virological response to anti-retroviral treatment. IL-10 neutralization with anti-IL-10 antibody in Env-activated cultures enhanced the viral replication in culture supernantants. Both in TT and in Env-peptides-stimulated cultures, the IL-10 blockade enhanced the levels of pro-inflammatory cytokines, but it did not improve IFN-γ production from aged aids patients. Altogether, the results reported in this Thesis reveal disturbances in several segments of the immune response, particularly in the Th1 compartment, of anti-retroviral-treated aids patients older than 55 years, suggesting that, for these patients, immune reconstitution after treatment does not occur with the same efficacy as in young patients. And although the enhanced IL-10 production probably contributes, at least in part, to the virological control, it can compromise the immune response both to HIV and to other antigenic challenges, such as tetanus toxoid. It is suggested, therefore, the need for specific recommendations regarding the clinical management of these patients A proporção de idosos portadores da síndrome da imunodeficiência adquirida (aids) tem aumentado de maneira importante nos últimos anos e, até a presente data, existem poucos estudos que abordam a infecção nessa população especial. As particularidades imunológicas decorrentes do fenômeno da imunossenescência podem acarretar mudanças significativas na evolução da infecção pelo HIV, bem como na resposta ao tratamento. O objetivo maior desta Tese foi avaliar o impacto da idade na recuperação funcional do sistema imune de pacientes com aids acima de 55 anos, quando tratados adequadamente com terapia anti-retroviral, caracterizando a resultante imunológica da idade avançada e da infecção pelo HIV. Para tanto, foram estudados quatro grupos experimentais: indivíduos jovens saudáveis ou com aids, e indivíduos acima de 55 anos saudáveis ou com aids. Todos os pacientes com aids estavam recebendo terapia anti-retroviral, em sucesso terapêutico. No primeiro artigo apresentado, avaliamos resposta linfoproliferativa e produção de citocinas in vitro e resposta humoral in vivo mediante desafio antigênico com toxóide tetânico (TT) em indivíduos previamente vacinados contra o tétano. Os resultados mostraram deficiências imunológicas significativas relacionadas à idade avançada no que diz respeito a produção de IgG anti-TT, resposta linfoproliferativa e produção de IFN-γ. Em contrapartida, a produção de IL-10 foi significativamente maior nos indivíduos acima de 55 anos, infectados ou não pelo HIV. No segundo artigo, foram caracterizadas as subpopulações de células T mediante estímulo policlonal ou específico com antígenos do envelope do HIV (Env). Em culturas não-estimuladas de PBMC do grupo com aids e idade avançada, observamos frequência reduzida de células T naive e de memória central, associada a aumento de células T efetoras. Quando estimuladas policlonalmente, essas culturas apresentaram deficiência na produção de IFN-γ e hiperprodução de IL-10, como na resposta ao TT. Mediante estímulo específico com Env, a citometria de fluxo revelou frequência elevada de células T CD4+FoxP3-CD152+ com forte marcação intracelular para IL-10, indicando predomínio do fenótipo Tr-1, e não das células Treg clássicas. Interessantemente, em ambos os artigos, a replicação viral in vitro foi significativamente menor nos pacientes com aids acima de 55 anos, condizendo com a excelente resposta virológica desses pacientes ao tratamento antirretroviral. A neutralização da IL-10 com anticorpo anti-IL-10 nas culturas ativadas pelos peptídeos Env aumentou de forma significativa a replicação viral no sobrenadante. Tanto na resposta ao TT quanto aos peptídeos Env, o bloqueio da IL-10 aumentou os níveis de citocinas pró-inflamatórias, mas não melhorou a produção de IFN-γ dos pacientes acima de 55 anos com aids. Coletivamente, os achados dessa Tese revelam distúrbios em vários segmentos da resposta imune, particularmente no compartimento Th1, de pacientes acima 55 anos com aids e adequadamente tratados, sugerindo que, para esses pacientes, a reconstituição imune pós-tratamento não ocorre com a mesma eficácia que no jovem. Apesar do aumento da produção de IL-10 provavelmente contribuir, ao menos em parte, para o controle virológico, pode comprometer a resposta tanto ao próprio HIV, quanto a outros desafios antigênicos, a exemplo do toxóide tetânico. Sugere-se, portanto, a necessidade de recomendações específicas de manejo clínico para esse grupo de pacientes

The development of strategies for vertical transmission avoidance has enhanced the number of uninfected neonates born from infected mothers. The objective of the present study was to evaluate, in vivo and in vitro, the cytokine profile of uninfected neonates born from pregnant women infected with HIV-1.The results demonstrated high levels of proinflammatory cytokines related to Th17 phenotype associated with low IL-10 production, in plasma as well as in culture supernatants of activated T cells obtained from umbilical cord blood of neonates born from mothers infected with HIV -1 with plasma viral load (PVL) detectable.Interestingly, a similar pro-inflammatory cytokine profile was observed in peripheral blood samples from their respective mothers.On the other hand, high levels of IL-10 associated to reduced production of inflammatory cytokines were measured in blood and in the supernatants of activated T cells culturesfrom women that controlled the PVL, as well as their neonates.Regarding the phenotypic profile of pregnant women infected of HIV-1, the main T lymphocyte subset involved in producing IL-10 was CD4+Foxp3-. Interestingly, the in vitro HIV-1 replication was lower in cell cultures from pregnant patients, and it was related to the inhibitory effects of IL-10 on the production of TNF-α. Finally, ART-in utero-exposed neonates showed to be born with lower weight, and it was inversely correlated with maternal peripheral TNF-α level. In conclusion, the results suggest that pregnant women that are able to control the PVL, and the increment in the production of IL-10 may favor maternal Tr-1 cell expansion, which could help to reduce the risk of vertical transmission of HIV-1 by reducing the rate of viral replication.However, other results also presented here, although preliminary, have revealed adverse effects of viral replication, as well as the use of ART by pregnant women infected with HIV-1 in the development of functional T cells from uninfected newborns.These data may help to explain why some of these children have elevated risk of clinical morbidity and mortality due to a baseline state of pathological hypersensitivity. O número de mulheres jovens infectadas pelo HIV-1 vem crescendo desde o início da epidemia da aids, principalmente nos países em desenvolvimento, onde a frequência de gravidez é elevada. O desenvolvimento de estratégias para evitar a transmissão vertical tem aumentado o número de recém-nascidos não infectados nascidos de gestantes portadora do vírus. O objetivo da presente tese foi avaliar, in vivo e in vitro, o perfil de citocinas de neonatos não infectados, porém nascidos de mulheres infectadas pelo HIV-1. Os resultados demonstraram elevados níveis de citocinas pró-inflamatórias relacionadas ao fenótipo Th17, associadas à baixa produção de IL-10, tanto nos plasmas quanto nos sobrenadantes das culturas de células T ativadas obtidas do sangue do cordão umbilical de neonatos nascidos de gestantes infectadas pelo HIV-1 com cargas virais plasmáticas(PVL) detectáveis. De modo interessante, um perfil similar pró-inflamatório foi observado em amostras de sangue periférico de suas respectivas mães. Por outro lado, níveis elevados de IL-10, associados à reduzida produção de citocinas inflamatórias, foram dosados no sangue e nos sobrenadantes das culturas de células T ativadas de gestantes que controlavam a PVL, assim como de seus neonatos. Com relação à caracterização fenotípica das células T maternas produtoras de IL-10, a análise por citometria de fluxo demonstrou que essa citocina é majoritariamente produzida por células T CD4+Foxp3-. Curiosamente, a replicação viral in vitro do HIV-1 foi inferior nas culturas das gestantes infectadas pelo HIV-1 e foi relacionada aos efeitos inibitórios da IL-10 sobre a produção de TNF-α. Por fim, os neonatos não infectados expostosin utero à terapia antirretroviral (ART) apresentaram um menor peso ao nascer, e este achado foi inversamente correlacionado com os níveis periféricos maternos de TNF-α. Em conclusão, nossos achados sugerem que gestantes que conseguem controlar a PVL, e o incremento na produção de IL-10 materna possam favorecer a expansão das células Tr-1, as quais podem auxiliar em reduzir o risco de transmissão vertical do HIV-1 por reduzir a taxa de replicação viral. Por outro lado, outros resultados também apresentados aqui, apesar de preliminares, revelaram efeitos adversos da replicação viral e da ART em gestantes infectadas pelo HIV-1 no desenvolvimento funcional das células T de neonatos não infectados. Esses dados podem ajudar a explicar por que algumas dessas crianças apresentam elevado risco à morbidade e mortalidade devido a um estado basal de hipersensibilidade patológica.

The pathogenesis of neuromyelitis optica (NMO) is influenced by a combination of genetic and environmental factors, including infectious agents. Different infectious diseases can both trigger or exacerbate autoimmunity. Therefore, the objective of the present work was to evaluate the in vitro immune responsiveness to Escherichia coli, Staphylococcus aureus and Candida albicans in remittent-recurrent NMO patients, and correlate it to the level of neurological disability. In this context, the extent of lymphoproliferation and cytokine profile in response to S. aureus- and C. albicans-stimulated peripheral blood mononuclear cells (PBMC) cultures was similar between NMO patients and healthy individuals. Nevertheless, a higher in vitro T cell proliferation associated with elevated IL-1β, IL-6 and IL-17 release was observed in NMO-derived E. coli-stimulated cell cultures. Additionally, in these cultures, the IL-10 production was significantly lower as compared with control group. Experiments performed with PBMC cultures depleted with different lymphocytes subsets demonstrated that, while both CD4+ and CD8+ T cells produced IL-6 in response to E. coli, the IL-17 production was mainly depended with CD4+ T cells.The in vitro E.coli-induced IL-6 and IL-17 levels were positively related with neurological disabilities. This higher tendency in producing Th-17-related cytokines was directly associated with IL-23 levels produced by LPS-activated monocytes. Interestingly, elevated LPS levels were quantified in the plasma of NMO patients and they were related to plasmatic levels of IL-6. In conclusion, our results suggest that a higher Th17-responsiveness to E. coli could be involved in the NMO pathogenesis. This kind of investigation is very important because inhibitors of TLR binding or signaling could be considered as great potential therapeutics as adjuvant in the treatment of NMO patients A Neuromielite Óptica (NMO), anteriormente considerada como um subtipo de Esclerose Múltipla, é uma doença autoimune, inflamatória do sistema nervoso central, na qual o sistema imune ataca a mielina dos neurônios localizados nos nervos ópticos e medula espinhal, produzindo, então, mielite e neurite óptica simultânea ou sequenciais. A patogênese da neuromielite óptica é influenciada pela combinação de fatores genéticos e ambientais, incluindo agentes infecciosos. Diferentes doenças infecciosas podem tanto desencadear como exacerbar a autoimunidade. Portanto, o objetivo do presente estudo foi de analisar a responsividade imune in vitro a Escherichia coli, Staphylococcus aureus e Candida albicans em pacientes com NMO recorrente-remitente, e a correlacionar ao nível de incapacidade neurológica. Nesse contexto, a extensão da linfoproliferação e perfil de citocinas em resposta a S. aureus e C. albicans, em culturas de células mononucleares do sangue periférico (CMSP) foram similares entre pacientes com NMO e indivíduos saudáveis. Entretanto, maior proliferação de células T associada à elevada liberação de IL-1β, IL-6 e IL-17 foi observada em culturas de células derivadas de pacientes com NMO quando estimuladas com E. coli. Ademais, nessas culturas, a produção de IL-10 foi significativamente menor quando comparada ao grupo controle. Ensaios conduzidos em culturas de CMSP depletadas de diferentes subtipos de linfócitos demonstraram que, enquanto células T CD4+ e T CD8+ produzem IL-6 em resposta a E. coli, a produção de IL-17 foi praticamente restrita às células T CD4+. Os níveis de IL-6 e IL-17 in vitro induzidos por E. coli foram correlacionados positivamente às incapacidades neurológicas. Essa maior tendência a produzir citocinas relacionadas ao perfil Th17 foi diretamente associada aos níveis de IL-23 produzidos por monócitos ativados com LPS. De modo interessante, níveis elevados de LPS foram quantificados no plasma de pacientes com NMO e estes foram correlacionados aos níveis plasmáticos de IL-6. Em conclusão, nossos resultados sugerem que uma maior responsividade a E. coli poderia estar envolvida na patogênese da NMO. Esse tipo de investigação é muito importante pois inibidores da ligação ou sinalização do TLR poderiam ser considerados terapias com grande potencial como adjuvantes no tratamento de pacientes com NMO

Although studies about the immune events in MS patients during clinical relapses have demonstrated the involvement of Th17 cells in the immunopathogenesis, few data are available concerning the immune profile from these patients during remission phase. In this context, the objective of this study was evaluate functional aspects of T cells from remittent-recurrent MS (RR-MS) patients during the clinical remission and their relationship with neurological disabilities and the in vitro hidrocortisone (HC) response. Our results demonstrated that T cell proliferation induced by phytohemagglutinin (PHA) was lower in cell cultures from RR-MS patients, as compared with healthy individuals. The HC, however, was less potent in reducing T cell proliferation in cell cultures from RR-MS patients, as compared with control group. Among Th1 (IFN- γ and IL-2) and Th2 (IL-4 e IL-5) cytokines, only the production of IL-2 was significantly lower in cultures containing policlonally-activated T cells from patients. On the other hand, the production of Th17-related cytokines TNF-α, IL-6 and IL-17 was significantly higher in cell cultures from RR-MS patients, as compared with control group. Although higher IL-17 production in cell cultures from patients, no difference was observed with regard to anti-inflammatory cytokines production, IL-10 and TGF-β, between the two groups studied. Nevertheless, the addition of HC was less efficient in reducing IL-17 production by cell cultures from RR-MS patients. Finally, higher tendency in mounting Th17 response in patients was directly related to both higher LPS-induced in vitro IL-23 and IL-6 production by monocytes and higher in vivo LPS dosage. In summary, our results demonstrated that, even during clinical remission, Th17-related cytokines are dominant in T cells cultures of RR-MS patients, and this strong tendency should be related, at least in part, to higher sensibilization of innate immune cells by peripheral LPS and lower sensibility to inhibition to glucocorticoids. A esclerose múltipla (EM) é uma doença progressiva autoimune que afeta o sistema nervoso central tendo como alvos peptídeos das proteínas dos oligodendrócitos, célula formadora da bainha de mielina. Apesar de estudos sobre eventos imunes conduzidos em pacientes com EM na fase ativa da doença terem demonstrado o envolvimento das células Th17 na imunopatogênese, poucos dados existem sobre o perfil imune desses indivíduos durante a fase de remissão. Nesse sentido, o objetivo do presente estudo foi avaliar aspectos funcionais das células T de pacientes com EM recorrente-remitente (EM-RR) na fase de remissão clínica e sua relação com o grau de incapacidade neurológica e resposta in vitro à hidrocortisona (HC). Nossos resultados demonstraram que a proliferação de células T induzida por fitohemaglutinina foi inferior nas culturas de pacientes com EM-RR, quando comparada a indivíduos saudáveis. A HC, no entanto, foi menos eficaz em reduzir a proliferação das células T nas culturas obtidas dos pacientes com EM-RR. Dentre as citocinas do tipo Th1 (IFN-γ e IL-2) e Th2 (IL-4 e IL-5), apenas a produção de IL-2 foi significativamente inferior nas culturas contendo células T policlonalmente ativadas dos pacientes. Por outro lado, a produção de citocinas relacionadas ao fenótipo Th17, TNF-α, IL-6 e IL-17, foi significativamente superior nas culturas dos pacientes com EM-RR, quando comparado ao grupo controle. Apesar dessa maior produção de IL-17 observada nas culturas dos pacientes, nenhuma diferença foi observada, entre os dois grupos, quanto à produção de citocinas anti-inflamatórias, a IL-10 e o TGF-β. Entretanto, a adição de HC foi menos eficaz em reduzir a produção de IL-17 pelas culturas de células dos pacientes com EM-RR. Finalmente, a maior tendência em montar uma resposta típica Th17 nos pacientes foi diretamente relacionada tanto à maior produção in vitro de IL-23 e IL-6 por monócitos estimulados com LPS, quanto à maior dosagem in vivo de LPS. Em resumo, nossos resultados demonstram que, mesmo na remissão clínica, citocinas relacionadas ao fenótipo Th17 são dominantes em culturas de células T de pacientes com EM-RR e que essa forte tendência deve estar relacionada, ao menos em parte, a maior sensibilização das células da imunidade inata pelo LPS circulante e a menor sensibilidade à inibição pelos glicocorticóides.

Although the great majority of the pediatric AIDS cases all over the world results from vertical transmission, approximately, two thirds of the children exposed to HIV during fetal life are not contaminated. In this context, following brazilian consensus recommendations (Health Ministry), every child whom vertical transmission had been laboratorialy discarded does not need a specific ambulatorial follow up. However, our previous results demonstrated that HIV-1-infected pregnant women who did not control their plasma viral load presented elevated levels of inflammatory cytokines, and in the present study our results revealed that non-infected neonates, born from these pregnant women display immune functional abnormalities in umbilical cord T cells compartment when exposed in vitro to policlonal activators, but not to HIV-1 antigens. Furthermore, when compared to non-exposed neonates, T cell in vitro activation with anti-CD3/anti-CD28 from neonates exposed to HIV-1 induced production of high IL-17 levels and decreased of IL-10. Interestingly, this T cell bias in secreting IL-17 seem to be related to liberation of high IL-23 levels by dendritic cells derived from umbilical cord blood monocytes following stimulation with bacterial lipopolysacharide (LPS). The lack of uterine sensitization to HIV-1 antigens suggests that, these alterations, may translate an adverse effect of a high level maternal inflammatory cytokines production on neonate s immune system, which may unbalance events related to neonatal and fetal immune maturation and homeostasis, favoring Th anomalous phenotypes predominance, such as Th17. This Th17 hyper-responsiveness may then compromise not only the child s capacity to respond in an adequate way to different antigenic stimuli through life, as well as becoming them more susceptible to immune-mediated disorders A síndrome da imunodeficiência adquirida (AIDS), causada pelo vírus da imunodeficiência humana (HIV), é uma das epidemias mais impactantes do milênio e, desde o início, o número de mulheres jovens infectadas vem aumentando vertiginosamente, principalmente nos países em desenvolvimento onde muitas destas engravidam precocemente. Apesar da grande maioria dos casos de AIDS pediátrico no mundo resultar da transmissão vertical, aproximadamente dois terços dos neonatos expostos ao HIV durante a vida fetal não são contaminados. Neste sentido, seguindo as recomendações do consenso brasileiro (Ministério da Saúde), toda criança cuja transmissão vertical tenha sido descartada laboratorialmente não necessita de acompanhamento ambulatorial particularizado. Entretanto, resultados anteriores obtidos pelo nosso grupo demonstraram que, gestantes infectadas pelo HIV-1 que não controlam a carga viral plasmática (CVP), apresentam níveis elevados de citocinas inflamatórias e, no presente estudo, resultados revelam que neonatos não-infectados nascidos dessas gestantes apresentam anormalidades imunofuncionais no compartimento das células T do cordão umbilical quando expostos in vitro a ativadores policlonais, mas não aos antígenos do HIV-1. Ademais, quando comparada a neonatos não expostos, a ativação in vitro das células T de neonatos expostos ao HIV-1 com anti-CD3/anti-CD28 induziu a produção de níveis elevados de IL-17 e reduzidos de IL-10. Interessantemente, essa tendência das células T em secretar IL-17 parece estar relacionada à liberação de níveis elevados de IL-23 pelas células dendríticas derivadas de monócitos do sangue do cordão umbilical estimuladas com lipopolissacarídeo bacteriano. Uma ausência de sensibilização uterina aos antígenos do HIV-1 sugere que essas alterações possam traduzir um efeito adverso da produção elevada de citocinas inflamatórias maternas sobre o sistema imune do neonato, o que pode desequilibrar os eventos envolvidos na maturação e homeostasia imune fetal e neonatal, favorecendo o predomínio de fenótipos Th anômalos, tal como Th17. This fact has an important impact on vertical virus transmission. Although the great majority of the pediatric AIDS cases all over the world results from vertical transmission, approximately two thirds of the children exposed to the HIV during the fetal life are not infected by the virus. Many works suggest that during pregnancy, maternal infectious diseases could have complex consequences to the fetus development, and few works have explored the impact of HIV exposition on immunological responsiveness of uninfected children to different stimuli, particularly in the era of the anti-retroviral drugs. Therefore, this work aimed to evaluate immune events in HIV-1-exposed uninfected neonates born from pregnant women who controlled (G1) or not (G2) the plasma viral load, using unexposed neonates as control. Cord blood from each neonate was collected, plasma and mononuclear cells were separated and the lymphoproliferation and cytokine pattern were evaluated. The results demonstrated that the in vitro lymphoproliferation induced by polyclonal activators was higher in the G2 neonates. Nevertheless, no cell culture responded to poll synthetic HIV-1 envelope peptides. The cytokine dosage in the plasma and supernatants of polyclonally-activated cultures demonstrated that, while IL-4 and IL-10 were the dominant cytokines produced in G1 and control groups, the secretion of IFN-γ, IL-1, Il-6, IL-17 and TNF-α was significantly higher in G2 neonates. Systemic levels of IL-10 observed among the G1 neonates were higher in those born from mothers treated with viral transcriptase reverse inhibitors drugs. On the other hand, higher levels of inflammatory cytokines were observed among them born from pregnant women treated with highly active anti-retroviral therapy. In summary, our results indicate an altered immune responsiveness in neonates exposed in utero to HIV and support the role of maternal anti-viral treatment with less potent drugs to attenuate it. A síndrome da imunodeficiência adquirida (AIDS), causada pelo vírus da imunodeficiência humana (HIV), é uma das mais destrutivas epidemias do mundo, e a infecção pelo HIV em mulheres jovens vem aumentando rapidamente nos dias atuais. Esse fato tem um impacto importante na transmissão vertical do vírus. Apesar da grande maioria dos casos de aids pediátrica em todo mundo resultar da transmissão vertical, aproximadamente dois terços das crianças expostas ao HIV durante a vida fetal não são infectadas pelo vírus. Muitos trabalhos sugerem que durante a gestação doenças infecciosas maternas podem ter consequências complexas para o desenvolvimento do feto, e poucos trabalhos têm explorado o impacto da exposição ao HIV sobre a responsividade imunológica de crianças não infectadas a diferentes estímulos, particularmente na era das drogas antirretrovirais. Portanto, esse trabalho teve como objetivo avaliar eventos imunes em neonatos não-infectados expostos ao HIV-1 nascidos de gestantes que controlam (G1) ou não (G2) a carga viral plasmática, usando neonatos não expostos como controle. Para tanto, sangue do cordão umbilical de cada neonato foi coletado, plasma e células mononucleares foram separados e a linfoproliferação e o perfil de citocinas foram avaliados. Os resultados demonstraram que a linfoproliferação in vitro induzido por ativadores policlonais foi maior nos neonatos do G2. Entretanto, nenhuma cultura de célula respondeu a um conjunto de peptídeos sintéticos do envelope do HIV-1. A dosagem de citocinas no plasma e nos sobrenadantes das culturas ativadas policlonalmente demonstrou que, enquanto a IL-4 e IL-10 foram as citocinas dominantes produzidas nos grupos G1 e controle, a secreção de IFN-γ, IL-1, Il-6, IL-17 e TNF-α foi significativamente superior nos neonatos G2. Níveis sistêmicos de IL-10 observados dentre os neonatos G1 foram maiores naqueles nascidos de mães tratadas com drogas inibidoras da transcriptase reversa do vírus. Por outro lado, níveis superiores de citocinas inflamatórias foram observados dentre estes nascidos de gestantes tratadas com terapia antirretroviral de alta eficácia. Em resumo, nossos resultados indicam uma responsividade imune alterada em neonatos expostos in utero ao HIV-1 e reforça o papel do tratamento materno anti-viral com drogas menos potentes em atenuar tais distúrbios.
