1. Adaptation and psychometric assessment of a sexual and reproductive empowerment scale in Arabic among refugee and non-refugee adolescent girls

Abstract

Abstract Background Sexual and reproductive empowerment (SRE) is an important determinant of women’s and girls’ health yet measuring it is complex due to cultural and domain-specific variations. This study describes the process of adapting an SRE scale consisting of four domains (self-efficacy; future orientation; social support; and safety) and testing its psychometric properties among Arabic speaking adolescent girls in Lebanon. Methods An SRE scale developed in a Western context was adapted in four steps: (1) reviewing the scale and selecting culturally appropriate domains for translation to standard Arabic; (2) conducting cognitive interviews with 30 11-17-year-old adolescent girls in Lebanon; (3) administering the scale to 339 refugee adolescent girls who participated in an early marriage intervention; and (4) conducting confirmatory factor analysis (CFA) on the data to assess the scale’s psychometric properties. Results The original model for the 13-item, four-domain adapted scale demonstrated poor fit in CFA. After iteratively removing two items, scale properties were improved, albeit were not optimal. The validity and reliability results for the self-efficacy domain were acceptable. Cognitive interview data revealed that Arab adolescent girls understood self-efficacy in relational terms, recognizing that autonomous decision-making is not necessarily favored but is influenced by parents and family. Conclusions This study presents an effort to customize an SRE scale for use in studies on the health of adolescent girls in an Arab cultural context. Findings from cognitive interviews highlight the importance of taking into consideration relationality in adolescent sexual and reproductive decision-making. The self-efficacy domain in the adapted scale demonstrates acceptable psychometric properties and is recommended for use in health studies to capture SRE.

Abstract

Abstract Complex humanitarian emergencies are a main driver of food and nutritional insecurity. Agricultural interventions are key to improving nutrition and food security, and their positive impacts are well-documented in stable developing countries. However, it is unclear if their positive effects on food security hold in complex emergency settings, too. In this paper, we systematically review empirical articles that apply rigorous designs to assess the causal impacts of agricultural interventions on food security, nutrition, or health outcomes in complex humanitarian emergencies. We only find six articles matching these criteria, which have mixed results on dietary diversity and food security, and little evidence on child nutrition. Our review underscores the need for more rigorous research on the impacts of agricultural interventions in complex humanitarian emergency settings.

3. Predicting poor mental health among older Syrian refugees in Lebanon during the COVID-19 pandemic: a nested cross-sectional study

Abstract

Introduction The COVID-19 pandemic has worsened pre-existing vulnerabilities among older Syrian refugees in Lebanon, potentially impacting their mental health. The study aims to describe the evolution of poor mental health over time and to develop and internally validate a prediction model for poor mental health among older Syrian refugees in Lebanon.Methods This prognostic study used cross-sectional data from a multiwave telephone survey in Lebanon. It was conducted among all Syrian refugees aged 50 years or older from households that received assistance from a humanitarian organisation. Data were collected between 22 September 2020 and 20 January 2021. Poor mental health was defined as a Mental Health Inventory-5 score of 60 or less. The predictors were identified using backwards stepwise logistic regression. The model was internally validated using bootstrapping. The calibration of the model was presented using the calibration slope (C-slope), and the discrimination was presented using the optimised adjusted C-statistic.Results There were 3229 participants (median age=56 years (IQR=53–62)) and 47.5% were female. The prevalence of poor mental health was 76.7%. Predictors for poor mental health were younger age, food insecurity, water insecurity, lack of legal residency documentation, irregular employment, higher intensity of bodily pain, having debt and having chronic illnesses. The final model demonstrated good discriminative ability (C-statistic: 0.69 (95% CI 0.67 to 0.72)) and calibration (C-slope 0.93 (95%CI 0.82 to 1.07)).Conclusion Mental health predictors were related to basic needs, rights and financial barriers. These allow humanitarian organisations to identify high-risk individuals, organise interventions and address root causes to boost resilience and well-being among older Syrian refugees in Lebanon.

Abstract

Abstract Background Gabon is a malaria-threatened country with a stable and hyperendemic transmission of Plasmodium falciparum monoinfection. Malaria drug resistance is widely spread in many endemic countries around the world, including Gabon. The molecular surveillance of drug resistance to antifolates and artemisinin-based combination therapy (ACT) is one of the strategies for combating malaria. As Plasmodium parasites continue to develop resistance to currently available anti-malarial drugs, this study evaluated the frequency of the polymorphisms and genetic diversity associated with this phenomenon among the parasites isolates in Gabon. Methods To assess the spread of resistant haplotypes among the malaria-infected population of Libreville, single nucleotide polymorphisms linked to sulfadoxine–pyrimethamine (SP) and artemisinin drugs resistance were screened for P. falciparum dihydrofolate reductase (Pfdhfr), P. falciparum dihydropteroate synthase (Pfdhps), and P. falciparum kelch 13-propeller domain (Pfk13) point mutations. Results The analysis of 70 malaria-positive patient samples screened for polymorphism showed 92.65% (n = 63) mutants vs. 7.35% (n = 5) wild parasite population in Pfdhfr, with high prevalence mutations at S108N(88.24%, n = 60), N51I(85.29%, n = 58), C59R(79.41%, n = 54); however, I164L(2.94%, n = 2) showed low frequency mutation. No wild haplotype existed for Pfdhps, and there were no mutations at the K540E, A581G, and A613T/S positions. However, the mutation rate at A437G(93.38%, n = 62) was the highest, followed by S436A/F(15.38%, n = 10). A higher frequency of quadruple IRNI–SGKAA (69.84%) than quintuple IRNI–(A/F)GKAA (7.94%) mutations was observed in the Pfdhfr–Pfdhps combination. Furthermore, none of the mutations associated with ACT resistance, especially those commonly found in Africa, were observed in Pfk13. Conclusions High polymorphism frequencies of Pfdhfr and Pfdhps genes were observed, with alternative alanine/phenylalanine mutation at S436A/F (7.69%, n = 5) for the first time. Similar to that of other areas of the country, the patterns of multiple polymorphisms were consistent with selection owing to drug pressure. Although there was no evidence of a medication failure haplotype in the studied population, ACT drug efficacy should be regularly monitored in Libreville, Gabon.

Abstract

Quality and timely antenatal care is a vital component of pregnancy care for the well-being of mothers and babies. The aim of this study of to assess self-reported obstetricians' adherence to national and international antenatal care guidelines in Lebanon. This cross-sectional study approached all obstetricians registered with the Lebanese Society of Obstetrics and Gynecology and the Lebanese Order of Physicians to participate in an online and telephone-based survey. The study tool included all of the items from the World Health Organization and national guidelines for antenatal care. Obstetricians' self-reported adherence was assessed across five modules including dietary interventions, dietary supplements, antenatal care assessment, fetal growth assessment, and antenatal care preventive measures. A total of 134 obstetricians completed the survey. Overall, adherence was high for most antenatal care guidelines except for providing nutrition services to undernourished pregnant women, screening for intimate partner violence, and providing tetanus vaccines. Abstract

# Background Many governments in low- and middle-income countries (LMICs) have difficulties paying healthcare costs upfront leading to high out-of-pocket payments for patients. A Development Impact Bond (DIB) is an innovative financing mechanism in which pr ivate investors provide pre-payment of development program expenses. At the same time, public agencies or donors repay the investor's investment with a reasonable interest rate if the program succeeds in delivering independently measurable results that are contractually agreed upon. This study assessed quantitatively and qualitatively the feasibility of a DIB for hepatitis C Virus (HCV) diagnosis and treatment in Cameroon. # Methods A revolving fund of up to €230,000 was made available by the investor. The outcome payor reimbursed the investor only in case of good performance, defined as cured patients (HCV-RNA negative). HCV carriers who were identified were referred for treatment and tested for cure 12 weeks after completion of treatment, the outcome being validated by an independent assessor. The evaluation was guided by the six-agents model, involving interviews with relevant stakeholders (N= 22). # Results In total, 253 (98%) patients completed treatment, of which 244 (96%) are cured at week 24. We estimated that the average per-patient *outcome payment* for HCV diagnosis and treatment is €1,542, and the *average costs per treated patient* is €1,858. The investor was fully repaid, including the agreed interest and bonus. Themes or findings from the interviews confirmed the feasibility of a DIB in a low-resource setting. # Conclusions This study demonstrates that a DIB can be a suitable financing mechanism for HCV services, supporting the path towards elimination. When governments in LMICs do not have sufficient resources to fund such elimination programs upfront, such public-private partnerships can offer a solution.

Abstract Jaundice caused by hyperbilirubinaemia is a common phenomenon during the neonatal period. Population-based studies evaluating assessment, management, and incidence of jaundice and need for phototherapy among otherwise healthy neonates are scarce. We prospectively explored these aspects in a primary care setting via assessing care as usual during the control phase of a stepped wedge cluster randomised controlled trial. We conducted a prospective cohort study embedded in the Screening and TreAtment to Reduce Severe Hyperbilirubinaemia in Infants in Primary care (STARSHIP) Trial. Healthy neonates were included in seven primary care birth centres (PCBCs) in the Netherlands between July 2018 and March 2020. Neonates were eligible for inclusion if their gestational age was ≥ 35 weeks, they were admitted in a PCBC for at least 2 days during the first week of life, and if they did not previously receive phototherapy. Outcomes were the findings of visual assessment to detect jaundice, jaundice incidence and management, and the need for phototherapy treatment in the primary care setting. 860 neonates were included of whom 608 (71.9%) were visibly jaundiced at some point during admission in the PCBC, with 20 being ‘very yellow’. Of the latter, four (20%) did not receive total serum bilirubin (TSB) quantification. TSB levels were not associated with the degree of visible jaundice (p = 0.416). Thirty-one neonates (3.6%) received phototherapy and none received an exchange transfusion. Five neonates did not receive phototherapy despite having a TSB level above phototherapy threshold. Jaundice is common in otherwise healthy neonates cared for in primary care. TSB quantification was not always performed in very jaundiced neonates, and not all neonates received phototherapy when indicated. Quality improvement initiatives are required, including alternative approaches to identifying potentially severe hyperbilirubinaemia. 8. The determinants of sustained adherence to COVID-19 preventive measures among older Syrian refugees in Lebanon

Abstract

Introduction Lebanon has battled the COVID-19 pandemic in the midst of an economic crisis. The evolution of the pandemic and a fragile health system have meant that public health policy has had to rely heavily on non-pharmaceutical interventions for disease control. However, changes in disease dynamics, an unraveling economy, and pandemic fatigue have meant that disease control policies need to be updated. Methods Using recent and timely data on older (50 years and above) Syrian refugees in Lebanon, this paper uses multivariate linear probability models to explore the determinants of adherence to two non-pharmaceutical COVID-19 prevention measures (wearing a mask and avoiding social gatherings) among this high-risk subgroup in a vulnerable population. Among respondents who report adhering to these measures, the paper also investigates the determinants of sustained adherence over a period of 6 months. Results The findings suggest that no individual-level characteristics were robustly associated with mask wearing. For avoiding social gatherings, education was inversely associated with adherence to this preventive measure. Avoiding social gatherings was also significantly lower for residents of informal tented settlements (ITSs). Among initial adherents, and for both preventive practices, ITS dwellers were also significantly less likely to maintain adherence. Conclusion Identifying variables associated with adherence to non-pharmaceutical preventive practices, particularly for vulnerable groups, can help inform and refine interventions in the face of changing conditions. The material, physical, administrative and socio-economic constraints of life in an ITS suggest that avoiding social gatherings is hardly feasible. Yet despite the challenging conditions of ITSs, the indication to wear a mask is initially complied with, suggesting that tailoring policies to the limits and constrains of context can lead to successful outcomes even in very adverse settings.

In this paper, we investigate the existence of parameterized algorithms running in subexponential time for two fundamental cycle-hitting problems: Feedback Vertex Set (FVS) and Triangle Hitting (TH). We focus on the class of pseudo-disk graphs, which forms a common generalization of several graph classes where such results exist, like disk graphs and square graphs. Abstract

Introduction Severe neonatal hyperbilirubinaemia can place a neonate at risk for acute bilirubin encephalopathy and kernicterus spectrum disorder. Early diagnosis is essential to prevent these deleterious sequelae. Currently, screening by visual inspection followed by laboratory-based bilirubin (LBB) quantification is used to identify hyperbilirubinaemia in neonates cared for at home in the Netherlands. However, the reliability of visual inspection is limited. We aim to evaluate the effectiveness of universal transcutaneous bilirubin (TcB) screening as compared with visual inspection to: (1) increase the detection of hyperbilirubinaemia necessitating treatment, and (2) reduce the need for heel pricks to quantify bilirubin levels. In parallel, we will evaluate a smartphone app (Picterus), and a point-of-care device for quantifying total bilirubin (Bilistick) as compared with LBB.Methods and analysis We will undertake a multicentre prospective cohort study in nine midwifery practices across the Netherlands. Neonates born at a gestational age of 35 weeks or more are eligible if they: (1) are at home at any time between days 2 and 8 of life; (2) have their first midwife visit prior to postnatal day 6 and (3) did not previously receive phototherapy. TcB and the Picterus app will be used after visual inspection. When LBB is deemed necessary based on visual inspection and/or TcB reading, Bilistick will be used in parallel. The coprimary endpoints of the study are: (1) hyperbilirubinaemia necessitating treatment; (2) the number of heel pricks performed to quantify LBB. We aim to include 2310 neonates in a 2-year period. Using a decision tree model, a cost-effectiveness analysis will be performed.Ethics and dissemination This study has been approved by the Medical Research Ethical Committee of the Erasmus MC Rotterdam, Netherlands (MEC-2020-0618). Abstract

Chronic liver diseases still represent a worrying public health issue in Sub-Saharan Africa. In this region, emphasis is generally made on hepatocellular carcinoma (HCC) albeit liver cirrhosis (LC) is also responsible for an important death toll. Very few studies have compared the presentation and etiologies of cancer and cirrhosis of the liver in Middle Africa. We conducted a comparative retrospective analysis of 74 and 134 cases of patients with HCC and LC treated in Libreville, Gabon. Viral or lifestyle risk factors, clinical symptoms, and biological features were compared. We observed that ages of diagnosis were 53.2 ± 15.7 years and 48.6 ± 18.6 years for HCC and LC with remarkably low M:F sex ratios (1.3–1.8). Ethanol consumption was highly prevalent in both disease types (65.0%–70.0%). Chronic viral infections with hepatitis B (HBV) or C (HCV) virus were also widespread with slight domination of the former in both diseases (43.4% vs. 34.3%, and 35.9% vs. 28.5%). Patients with HCC were presenting very late with a mean diameter of the main nodule of 84 ± 50 mm and a multifocal pattern in 72.7% of cases. HCC developed on a cirrhotic liver in 91.7% of cases. Serum AFP was frankly elevated (>400 ng/ml) in only 35.8% of HCC cases. The most striking feature of the HCC series was the contrasted contribution of distinct pathogenic etiologies involving sex, viral, metabolic, and toxic factors. A frequently dysmetabolic condition synergizing with hepatitis C (anti-HCV, 73.8% vs 22.7%, p < 0.0001) in females and a male cancer promoted by recreational toxicants and chronic hepatitis B (HBsAg, 83.5% vs 35.9%, p < 0.0001) were observed. Men with HCC were considerably younger than women (46.8 ± 14.5 years vs. 62.2 ± 12.2 years, p < 0.0001). Further studies are now warranted to identify routes of HCV transmission and if they are still fueling reservoirs of future patients. Public policies to prevent alcohol-related harm have also to be urgently implemented in Gabon.

Abstract Background Neonatal hyperbilirubinaemia is a physiologic phenomenon, but, when severe, may cause lifelong disability. Maternity care assistants (MCAs) play an important role in timely recognition of severe neonatal jaundice. We assessed knowledge and skills of MCAs regarding neonatal hyperbilirubinaemia. Methods All Dutch MCAs (n = 9065) were invited to fill out a questionnaire assessing knowledge, expertise, and handling of neonatal jaundice. Additionally, we developed an e-learning and provided training sessions to a subgroup of MCAs (n = 99), and assessed their knowledge on neonatal hyperbilirubinaemia before and after the training. Results One thousand four hundred sixty-five unique online questionnaires were completed (response 16.2%). The median number of correctly answered knowledge questions was 5 (out of six; IQR 1). Knowledge was significantly better when respondents had had in-service training on neonatal hyperbilirubinaemia in the previous year (p = 0.024). Although 82% of respondents felt highly skilled or skilled to assess jaundice, accuracy of estimation of total serum bilirubin levels by assessing skin colour was generally poor and prone to underestimation. Abstract

Paris is experimenting and implementing strategies to increase the capital's resilience and promote climate change adaptation. Major heatwaves have been hitting the French Capital lately (Bador et al., 2017) and have thus focused attention on UHI countermeasures, such as pavement-watering solutions and urban greening. The Oasis Project is one such strategy aiming to transform schoolyards into urban cool islands that would benefit surrounding neighborhoods and their inhabitants during heatwaves.The work presented here focuses on identifying high-priority schoolyards among the 670 city-owned schools. This is conducted using a GIS tool used to identify areas with high cooling potential, which would benefit most from UHI countermeasures. After extracting bare schoolyard from the facilities, we built a cooling indicator based on the ratio of high solar irradiance surface to the whole schoolyard area. We were thus able to identify 38 schoolyards with high cooling potential, 157 with medium cooling potential and 286 facilities with moderate cooling potential, out of the 670 facilities.The methodology can be applied to other cities, and therefore helps set up GIS tools that can provide municipalities with meaningful insight into their urban cooling strategy.

A potential candidate is HgTe but its sensitivity to nanofabrication processes restrain its development. A way to circumvent this obstacle is the selective area growth technique. Here, in-plane HgTe nanostructures are grown thanks to selective area molecular beam epitaxy on a semi-insulating CdTe substrate covered with a patterned SiO$_{\mathrm{2}}$ mask. The shape of these nanostructures is defined by the in-plane orientation of the mask aperture along the <$110$>, <$1\bar{\mathrm{1}}0$>, or <$100$> direction, the deposited thickness, and the growth temperature. Several micron long in-plane nanowires can be achieved as well as more complex nanostructures such as networks, diamond structures or rings. A good selectivity is achieved with very little parasitic growth on the mask even for a growth temperature as low as $140${\deg}C and growth rate up to $0.5$ ML/s. For <$110$> oriented nanowires, the center of the nanostructure exhibits a trapezoidal shape with {$111$}B facets and two grains on the sides, while <$1\bar{\mathrm{1}}0$> oriented nanowires show {$111$}A facets with adatoms accumulation on the sides of the top surface. Transmission electron microscopy observations reveal a continuous epitaxial relation between the CdTe substrate and the HgTe nanowire. Measurements of the resistance with fourpoint scanning tunneling microscopy indicates a good electrical homogeneity along the main NW axis and a thermally activated transport. Abstract

In the $K_r$-Cover problem, given a graph $G$ and an integer $k$ one has to decide if there exists a set of at most $k$ vertices whose removal destroys all $r$-cliques of $G$. In this paper we give an algorithm for $K_r$-Cover that runs in subexponential FPT time on graph classes satisfying two simple conditions related to cliques and treewidth. As an application we show that our algorithm solves $K_r$-Cover in time * $2^{O_r\left (k^{(r+1)/(r+2)}\log k \right)} \cdot n^{O_r(1)}$ in pseudo-disk graphs and map-graphs; * $2^{O_{t,r}(k^{2/3}\log k)} \cdot n^{O_r(1)}$ in $K_{t,t}$-subgraph-free string graphs; and * $2^{O_{H,r}(k^{2/3}\log k)} \cdot n^{O_r(1)}$ in $H$-minor-free graphs.

Abstract

The Near-Infrared Spectrometer and Photometer (NISP) on board the Euclid satellite provides multiband photometry and R>=450 slitless grism spectroscopy in the 950-2020nm wavelength range. In this reference article we illuminate the background of NISP's functional and calibration requirements, describe the instrument's integral components, and provide all its key properties. We also sketch the processes needed to understand how NISP operates and is calibrated, and its technical potentials and limitations. Links to articles providing more details and technical background are included. NISP's 16 HAWAII-2RG (H2RG) detectors with a plate scale of 0.3" pix^-1 deliver a field-of-view of 0.57deg^2. In photo mode, NISP reaches a limiting magnitude of ~24.5AB mag in three photometric exposures of about 100s exposure time, for point sources and with a signal-to-noise ratio (SNR) of 5. For spectroscopy, NISP's point-source sensitivity is a SNR = 3.5 detection of an emission line with flux ~2x10^-16erg/s/cm^2 integrated over two resolution elements of 13.4A, in 3x560s grism exposures at 1.6 mu (redshifted Ha). Our calibration includes on-ground and in-flight characterisation and monitoring of detector baseline, dark current, non-linearity, and sensitivity, to guarantee a relative photometric accuracy of better than 1.5%, and relative spectrophotometry to better than 0.7%. The wavelength calibration must be better than 5A. NISP is the state-of-the-art instrument in the NIR for all science beyond small areas available from HST and JWST - and an enormous advance due to its combination of field size and high throughput of telescope and instrument. During Euclid's 6-year survey covering 14000 deg^2 of extragalactic sky, NISP will be the backbone for determining distances of more than a billion galaxies. Abstract

The current standard model of cosmology successfully describes a variety of measurements, but the nature of its main ingredients, dark matter and dark energy, remains unknown. Euclid is a medium-class mission in the Cosmic Vision 2015-2025 programme of the European Space Agency (ESA) that will provide high-resolution optical imaging, as well as near-infrared imaging and spectroscopy, over about 14,000 deg^2 of extragalactic sky. In addition to accurate weak lensing and clustering measurements that probe structure formation over half of the age of the Universe, its primary probes for cosmology, these exquisite data will enable a wide range of science. This paper provides a high-level overview of the mission, summarising the survey characteristics, the various data-processing steps, and data products. Abstract

This paper presents the specification, design, and development of the Visible Camera (VIS) on the ESA Euclid mission. VIS is a large optical-band imager with a field of view of 0.54 deg^2 sampled at 0.1" with an array of 609 Megapixels and spatial resolution of 0.18". It will be used to survey approximately 14,000 deg^2 of extragalactic sky to measure the distortion of galaxies in the redshift range z=0.1-1.5 resulting from weak gravitational lensing, one of the two principal cosmology probes of Euclid. With photometric redshifts, the distribution of dark matter can be mapped in three dimensions, and, from how this has changed with look-back time, the nature of dark energy and theories of gravity can be constrained. The entire VIS focal plane will be transmitted to provide the largest images of the Universe from space to date, reaching m_AB>24.5 with S/N >10 in a single broad I_E~(r+i+z) band over a six year survey. The particularly challenging aspects of the instrument are the control and calibration of observational biases, which lead to stringent performance requirements and calibration regimes. With its combination of spatial resolution, calibration knowledge, depth, and area covering most of the extra-Galactic sky, VIS will also provide a legacy data set for many other fields. This paper discusses the rationale behind the VIS concept and describes the instrument design and development before reporting the pre-launch performance derived from ground calibrations and brief results from the in-orbit commissioning. VIS should reach fainter than m_AB=25 with S/N>10 for galaxies of full-width half-maximum of 0.3" in a 1.3" diameter aperture over the Wide Survey, and m_AB>26.4 for a Deep Survey that will cover more than 50 deg^2. 19. A dynamical view of Tijdeman's solution of the chairman assignment problem

Abstract

In 1980, R. Tijdeman provided an on-line algorithm that generates sequences over a finite alphabet with minimal discrepancy, that is, such that the occurrence of each letter optimally tracks its frequency. In this article, we define discrete dynamical systems generating these sequences. The dynamical systems are defined as exchanges of polytopal pieces, yielding cut and project schemes, and they code tilings of the line whose sets of vertices form model sets. We prove that these sequences of low discrepancy are natural codings of toral translations with respect to polytopal atoms, and that they generate a minimal and uniquely ergodic subshift with purely discrete spectrum. Finally, we show that the factor complexity of these sequences is of polynomial growth order $n^{d-1}$, where $d$ is the cardinality of the alphabet.

Abstract

We investigate the decidability of the monadic second-order (MSO) theory of the structure $\langle \mathbb{N};<,P_1, \ldots,P_k \rangle$, for various unary predicates $P_1,\ldots,P_k \subseteq \mathbb{N}$. We focus in particular on "arithmetic" predicates arising in the study of linear recurrence sequences, such as fixed-base powers $\mathsf{Pow}_k = \{k^n : n \in \mathbb{N}\}$, $k$-th powers $\mathsf{N}_k = \{n^k : n \in \mathbb{N}\}$, and the set of terms of the Fibonacci sequence $\mathsf{Fib} = \{0,1,2,3,5,8,13,\ldots\}$ (and similarly for other linear recurrence sequences having a single, non-repeated, dominant characteristic root). Abstract

Aperiodic order refers to the mathematical formalisation of quasicrystals. Substitutions and cut and project sets are among their main actors; they also play a key role in the study of dynamical systems, whether they are symbolic, generated by tilings, or point sets. We focus here on the relations between quasicrystals and self-similarity from an arithmetical and dynamical viewpoint, illustrating how efficiently aperiodic order irrigates various domains of mathematics and theoretical computer science, on a journey from Diophantine approximation to computability theory. In particular, we see how Pisot numbers allow the definition of simple model sets, and how they also intervene for scaling factors for invariance by multiplication of Meyer sets. Abstract

The density of a rational language can be understood as the frequency of some "pattern" in the shift space, for example a pattern like "words with an even number of a given letter." We study the density of group languages, i.e. rational languages recognized by morphisms onto finite groups, inside shift spaces. We show that the density with respect to any given ergodic measure on a shift space exists for every group language, because it can be computed by using any ergodic lift of the given measure to some skew product between the shift space and the recognizing group. We then further study densities in shifts of finite type (with a suitable notion of irreducibility), and then in minimal shifts. In the latter case, we obtain a closed formula for the density under the condition that the skew product has minimal closed invariant subsets which are ergodic under the product of the original measure and the uniform probability measure on the group. The formula is derived in part from a characterization of minimal closed invariant subsets for skew products relying on notions of cocycles and coboundaries. In the case where the whole skew product is ergodic under the product measure, then the density is just the cardinality of the subset of the group which defines the language divided by the cardinality of the group. Moreover, we provide sufficient conditions for the skew product to have minimal closed invariant subsets that are ergodic under the product measure. Abstract

Objective To test the effect of proactive home visits by trained community health workers (CHWs) on child survival. Methods We conducted a two arm, parallel, unmasked cluster-randomized trial in 137 village-clusters in rural Mali. From February 2017 to January 2020, 31 761 children enrolled at the trial start or at birth. Village- clusters received either primary care services by CHWs providing regular home visits (intervention) or by CHWs providing care at a fixed site (control). In both arms, user fees were removed and primary health centres received staffing and infrastructure improvements before trial start. Using lifetime birth histories from women aged 15-49 years surveyed annually, we estimated incidence rate ratios (IRR) for intention- to-treat and per-protocol effects on under-five mortality using Poisson regression models. Findings Over three years, we observed 52 970 person-years (27 332 in intervention arm; 25 638 in control arm). During the trial, 909 children in the intervention arm and 827 children in the control arm died. The under-five mortality rate declined from 142.8 (95% CI: 133.3-152.9) to 56.7 (95% CI: 48.5-66.4) deaths per 1000 live births in the intervention arm; and from 154.3 (95% CI: 144.3-164.9) to 54.9 (95% CI: 45.2-64.5) deaths per 1000 live births in the control arm. Intention-to-treat (IRR: 1.02; 95% CI: 0.88-1.19) and per-protocol estimates (IRR: 1.01; 95% CI: 0.87-1.18) showed no difference between study arms. Conclusion Though proactive home visits did not reduce under-five mortality, system-strengthening measures may have contributed to the decline in under-five mortality in both arms. Abstract

Human African Trypanosomiasis (HAT) is an infectious disease due to a protozoa parasite of the Trypanosoma genus. In West and Central Africa, this disease is caused by the subspecies Trypanosoma brucei gambiense . Several foci of this disease are currently active and causing the death of hundreds of people in endemic areas. In this article, we report two cases of gambiense HAT in one Indonesian and one Gabonese men in two historical foci of Gabon in 2019. Both patients had fever with temperatures above 38°C, an altered state of consciousness, cachexia, and multiple dermabrasions on the abdomen related to scratching lesions. The diagnostic revealed second-stage infection of both patients with T. b. gambiense ; this result was confirmed by a polymerase chain reaction assay. Despite treatment with a combination of eflornithine and nifurtimox, as recommended by the World Health Organization for late-stage T. b. gambiense HAT, one of the two patients died. Thus, these cases highlight the importance of early HAT diagnosis and prompt patient care to fight effectively against this disease.

Abstract

Cysticercosis is one of the main causes of secondary epilepsy in sub-Saharan Africa. To estimate the seroprevalence of cysticercosis among epileptic patients, we conducted a cross-sectional study of patients attending neurology consultation in Abidjan, Côte d’Ivoire. Methods: Patients’ socio-demographic and lifestyle data were collected as well as blood samples for serological testing using ELISA and Western blot based on IgG antibodies detection. For qualitative variables comparison, Chi2 or Fisher tests were used; a Student’s t-test was used to compare quantitative variables. A multivariate logistic regression model was fit to identify risks factors. Results: Among 403 epileptic patients included in the study, 55.3% were male; the median age was 16.9 years; 77% lived in Abidjan; 26.5% were workers. Most patients included in the study had tonic-clonic seizures (80%), and 11.2% had focal deficit signs. The seroprevalence of cysticercosis was 6.0%. The risk was higher in patients over 30 years old (aOR = 5.1 (1.3–20.0)) than in patients under 16. The risk was also considerably high in patients who reported epileptics in the family (aOR = 5 (1.7–14.6)). The risk was three-fold less in females than in males. Conclusions: This study highlighted the exposure of epileptic patients to Taenia solium larvae in an urban area. The risk of positive serology was increased with age, male gender, and family history of epilepsy.

Abstract

We demonstrate a high electron conductivity (> 10^2 S/cm and up to 10^3 S/cm) of tungsten suboxide W18O(52.4-52.9)(or equivalently WO(2.91-2.94)) nanotubes (2 to 3 nm in diameter, ca. micrometer long). The conductivity is measured in the temperature range 120 to 300K by a four probe scanning tunneling microscope in ultra high vacuum. The nanotubes are synthesized by a low temperature and low cost solvothermal method. They selfassemble in bundles of hundreds of nanotubes forming nanowires (ca. micrometer long, few tens nm wide). We observe a large anisotropy of the conductivity with a ratio (longitudinal conductivity/perpendicular conductivity) of ca. 10^5. A large fraction of them (ca. 65 to 95%) shows a metallic like, thermal activation less, electron transport behavior. Few of them, with a lower conductivity from 10 to 10^2 S/cm, display a variable range hopping behavior. In this latter case, a hopping barrier energy of ca. 0.24 eV is inferred in agreement with the calculated energy level of the oxygen vacancy below the conduction band. This result is in agreement with a relative average concentration of oxygen vacancies of ca. 3%, for which a semiconductor to metal transition was theoretically predicted. Abstract

In this paper we investigate the existence of subexponential parameterized algorithms of three fundamental cycle-hitting problems in geometric graph classes. The considered problems, \textsc{Triangle Hitting} (TH), \textsc{Feedback Vertex Set} (FVS), and \textsc{Odd Cycle Transversal} (OCT) ask for the existence in a graph $G$ of a set $X$ of at most $k$ vertices such that $G-X$ is, respectively, triangle-free, acyclic, or bipartite. Such subexponential parameterized algorithms are known to exist in planar and even $H$-minor free graphs from bidimensionality theory [Demaine et al., JACM 2005], and there is a recent line of work lifting these results to geometric graph classes consisting of intersection of "fat" objects ([Grigoriev et al., FOCS 2022] and [Lokshtanov et al., SODA 2022]). In this paper we focus on "thin" objects by considering intersection graphs of segments in the plane with $d$ possible slopes ($d$-DIR graphs) and contact graphs of segments in the plane. Assuming the ETH, we rule out the existence of algorithms: - solving TH in time $2^{o(n)}$ in 2-DIR graphs; and - solving TH, FVS, and OCT in time $2^{o(\sqrt{n})}$ in $K_{2,2}$-free contact 2-DIR graphs. These results indicate that additional restrictions are necessary in order to obtain subexponential parameterized algorithms for %these problems. In this direction we provide: - a $2^{O(k^{3/4}\cdot \log k)}n^{O(1)}$-time algorithm for FVS in contact segment graphs; - a $2^{O(\sqrt d\cdot t^2 \log t\cdot k^{2/3}\log k)} n^{O(1)}$-time algorithm for TH in $K_{t,t}$-free $d$-DIR graphs; and - a $2^{O(k^{7/9}\log^{3/2}k)} n^{O(1)}$-time algorithm for TH in contact segment graphs.

Abstract

Purpose: To implement a framework generating synthetic spectral panoramic images from single energy CT volumes. Using the framework output to compare the synthetic images against experimental spectral panoramic images for cross-verification. Methods: A simulation framework for generating synthetic spectral panoramic images from CT volumes is described. A cone beam CT scan of an anthropomorphic head phantom is used as input. An experimental spectral panoramic image of the same phantom is acquired. Results: The output of the framework of an anthropomorphic head phantom is compared against an experimental spectral panoramic image of the same phantom. The synthetic and experimental spectral panoramic images resemble each other considerably, especially the bone features. In the soft tissue images, there are some deviations, which are a result of the differences between the experimental and synthetic processing pipelines. Conclusions: It is demonstrated that generating synthetic spectral panoramic images from single energy CT volumes is possible. The synthetic images have many similarities with the experimental results, increasing the confidence in the correctness of the information contained within experimental spectral panoramic images and indicating that the synthetic images could be useful in further research.

We first survey the current state of the art concerning the dynamical properties of multidimensional continued fraction algorithms defined dynamically as piecewise fractional maps and compare them with algorithms based on lattice reduction. We discuss their convergence properties and the quality of the rational approximation, and stress the interest for these algorithms to be obtained by iterating dynamical systems. We then focus on an algorithm based on the classical Jacobi--Perron algorithm involving the nearest integer part. The Kleine-Levin syndrome is a rare pathology characterized by recurrent episodes of hypersomnia associated with behavioral and cognitive disorders with, among others, hyperphagia and hypersexuality. The disease mainly affects young males. A few studies mention cases that occurred in Africa, especially in Côte d’Ivoire. In this paper, we report the very first two cases observed in the Neurology Department of the University Hospital of Cocody. The diagnosis was clinical, based on the recurrence of hypersomnia, cognitive and behavioral disorders during the periods of hypersomnia, and the return of patients to normal state between episodes. This diagnosis was delayed due to failure to understand the pathology, thereby leading patients to wandering. In fact, the two patients were consulted, respectively, 3 years and 6 years after the hypersomnia began. The objective was to report the very first cases observed in the Neurology Department of the University Hospital of Cocody, Côte d’Ivoire.
