Abstract

Background and Aims Whereas a beneficial effect of intravenous ferric carboxymaltose (FCM) on symptoms and exercise capacity among patients with iron deficiency and heart failure (HF) has been consistently demonstrated, the effects of treatment on clinical events remain the subject of research. This meta-analysis aimed to characterize the effects of FCM therapy on hospitalizations and mortality. Methods Patient-level data from randomized, placebo-controlled FCM trials including adults with HF and iron deficiency with ≥52 weeks follow-up were analysed. The co-primary efficacy endpoints were (i) composite of total/recurrent cardiovascular hospitalizations and cardiovascular death and (ii) composite of total HF hospitalizations and cardiovascular death, through 52 weeks. Key secondary endpoints included individual composite endpoint components. Event rates were analysed using a negative binomial model. Treatment-emergent adverse events were also examined. Results Three FCM trials with a total of 4501 patients were included. Ferric carboxymaltose was associated with a significantly reduced risk of co-primary endpoint 1 (rate ratio 0.86; 95% confidence interval 0.75–0.98; P =.029; Cochran Q : 0.008), with a trend towards a reduction of co-primary endpoint 2 (rate ratio 0.87; 95% confidence interval 0.75–1.01; P =.076; Cochran Q : 0.024). Treatment effects appeared to result from reduced hospitalization rates, not improved survival. Treatment appeared to have a good safety profile and was well tolerated. Conclusions In iron-deficient patients with HF with reduced left ventricular ejection fraction, intravenous FCM was associated with significantly reduced risk of hospital admissions for HF and cardiovascular causes, with no apparent effect on mortality. Abstract

Importance Perioperative anemia is a common comorbid condition associated with increased risk of morbidity and mortality in patients undergoing elective surgical procedures. Objective We conducted a systematic literature review (SLR) to determine the efficacy and safety of the use of intravenous ferric carboxymaltose (FCM) for the treatment of perioperative anemia in preoperative, intraoperative, and postoperative elective surgical care. Evidence review Studies meeting inclusion criteria for the SLR reported on treatment efficacy in an adult study population randomly allocated to FCM for the treatment of perioperative anemia during the perioperative period. After screening, 10 of 181 identified studies from searches in MEDLINE and EMBASE databases were identified for inclusion in this review. Findings Preoperative treatment was reported in six studies, intraoperative treatment in one study, postoperative treatment in two studies, and both pre- and postoperative treatment in one study. Together, 1975 patients were studied, of whom 943 were randomized to FCM, of whom 914 received FCM treatment. The 10 studies reported elective surgical populations for colorectal, gastric, orthopedic, abdominal, urologic, plastic, neck, gynecologic, and otolaryngologic procedures. Given the clinical and methodological heterogeneity of the studies, the analyses were limited to qualitative assessments without meta-analyses. All 10 studies reported statistically greater changes in hemoglobin concentration, serum ferritin, and/or transferrin saturation with FCM treatment compared with comparators (placebo, oral iron, standard care, or a combination of these). Two studies reported statistically significant differences in transfusion rate and 2 studies reported significant differences in length of hospital stay between FCM and its comparator(s). Conclusions and relevance This SLR adds to existing data that administration of FCM in preoperative and postoperative settings improves hematologic parameters. Several studies in the review supported the beneficial effects of FCM in reducing transfusion rate and length of stay. Larger, well-designed, longer-term studies may be needed to further establish the efficacy and safety of FCM in elective surgery patients with perioperative anemia.

The trial test results of eltrombopag, a new orally active thrombopoietin-receptor agonist that elevates platelets counts in normal and in patients with immune thormobocytopenia is presented. The drug was found to be effective therapy to increase the platelet count, permitting for antiviral treatment of patients with thrombocytopenia and cirrhosis associated with the hepatitis C virus.

Abstract

This report presents the outcomes of a sampling strategy and industrial hygiene study specifically designed to characterize the elemental composition and size distribution of dust found in portland cement manufacturing plants. Two sites with differing processing methods were selected to further determine if there were any differences based on dry and wet pyroprocessing. This study found that, during the sampling period, area dust measurements were well below existing Federal levels for respirable dust and that not all dust at a portland cement plant is portland cement. Additional study is warranted to increase the number of data points, perform personal sampling, potentially develop a method for portland cement dust analysis, and/or to derive an adjustment factor when using surrogate exposures for industrial hygiene monitoring. 30. Characterization of serum phosphate levels over time with intravenous ferric carboxymaltose versus placebo as treatment for heart failure with reduced ejection fraction and iron deficiency: An exploratory prospective substudy from HEART‐FID.

Abstract

Aims Methods and results Conclusions Ferric carboxymaltose (FCM) is guideline‐recommended for iron deficiency (ID) in heart failure with reduced ejection fraction (HFrEF). Despite a well‐established safety profile, the magnitude and clinical significance of FCM‐induced hypophosphataemia in HFrEF remains unclear. This pre‐specified substudy of HEART‐FID evaluated serum phosphate, 1,25‐dihydroxyvitamin D, and plasma parathyroid hormone (PTH) subsequent to FCM.HEART‐FID was a randomized, double‐blind, placebo‐controlled trial of ambulatory patients with HFrEF and ID randomized to FCM versus placebo. This substudy assessed mean change from baseline across eight visits over 6 months for the following endpoints: serum phosphate, 25‐hydroxyvitamin D, 1,25‐dihydroxyvitamin D, and PTH, in addition to the clinical severity of potential hypophosphataemia. Overall, 133 patients (n = 62 FCM, n = 71 placebo) were prospectively enrolled. Mean age was 68 ± 11 years, 55 (41.4%) were women, and 29 (21.8%) had chronic kidney disease. Phosphate levels decreased in 34 (57.6%) patients in the FCM group compared with 7 (10.3%) in the placebo group. Mean change in phosphate levels reached a nadir at day 21 (−0.36 ± 0.27 mmol/L) subsequent to FCM infusion with 28 (51%) having moderate‐to‐severe hypophosphataemia. Reductions in 1,25‐dihydroxyvitamin D were also observed, whilst PTH increased. These biochemical changes returned to baseline levels by day 91. Serum levels of 25‐hydroxyvitamin D remained stable throughout the study. No serious adverse events associated with hypophosphataemia were reported.Transient moderate‐to‐severe hypophosphataemia was frequent subsequent to FCM infusion, accompanied by 1,25‐dihydroxyvitamin D decrease and PTH increase. Serum levels of 25‐hydroxyvitamin D remained stable. No evidence of symptomatic hypophosphataemia was reported, collectively indicating FCM‐related hypophosphataemia to be clinically benign and transient in HFrEF. [ABSTRACT FROM AUTHOR]
