Los objetivos de este estudio son, por un lado, determinar las características antropométricas y de fuerza (medida mediante test de hand-grip y salto vertical con brazos libres) comprándolo con otros deportes de raqueta, y por otro, analizar las características propias del juego durante el Campeonato de España de Pádel sub-23. Para ello 14 hombres (20,38 ± 1,92 años) y 12 mujeres (19,73 ± 2,37 años) participaron en este estudio. A todos ellos se les realizó unas mediciones antropométricas, y unos tests de fuerza de miembro superior (hand-grip) y de miembro inferior (salto vertical con brazos libres) antes y después de los partidos. Por último, se registraron los datos de analisis de juego durante dicho evento. Los resultados muestran que las características antropométricas de los jugadores de pádel difieren ligeramente de las de los jugadores de otros deportes de raqueta. Los valores de fuerza, no disminuyeron después del partido, no observándose ningún indicio de fatiga. Respecto al análisis de las características del juego, el pádel se caracteriza por tener menos tiempo activo que el tenis de mesa y el badminton, pero más tiempo activo que el tenis.

Objectives: The shape is commonly used to describe the objects. State-of-the-art algorithms in medical imaging are predominantly diverging from computer vision, where voxel grids, meshes, point clouds, and implicit surface models are used. This is seen from the growing popularity of ShapeNet (51,300 models) and Princeton ModelNet (127,915 models). However, a large collection of anatomical shapes (e.g., bones, organs, vessels) and 3D models of surgical instruments is missing., Methods: We present MedShapeNet to translate data-driven vision algorithms to medical applications and to adapt state-of-the-art vision algorithms to medical problems. As a unique feature, we directly model the majority of shapes on the imaging data of real patients. We present use cases in classifying brain tumors, skull reconstructions, multi-class anatomy completion, education, and 3D printing., Results: By now, MedShapeNet includes 23 datasets with more than 100,000 shapes that are paired with annotations (ground truth). Our data is freely accessible via a web interface and a Python application programming interface and can be used for discriminative, reconstructive, and variational benchmarks as well as various applications in virtual, augmented, or mixed reality, and 3D printing., Conclusions: MedShapeNet contains medical shapes from anatomy and surgical instruments and will continue to collect data for benchmarks and applications. Background and Objectives: Digital pathology and artificial intelligence offer new opportunities for automatic histologic scoring. We applied a deep learning approach to IgA nephropathy biopsy images to develop an automatic histologic prognostic score, assessed against ground truth (kidney failure) among patients with IgA nephropathy who were treated over 39 years. We assessed noninferiority in comparison with the histologic component of currently validated predictive tools. We correlated additional histologic features with our deep learning predictive score to identify potential additional predictive features., Design, Setting, Participants, & Measurements: Training for deep learning was performed with randomly selected, digitalized, cortical Periodic acid-Schiff-stained sections images (363 kidney biopsy specimens) to develop our deep learning predictive score. We estimated noninferiority using the area under the receiver operating characteristic curve (AUC) in a randomly selected group (95 biopsy specimens) against the gold standard Oxford classification (MEST-C) scores used by the International IgA Nephropathy Prediction Tool and the clinical decision supporting system for estimating the risk of kidney failure in IgA nephropathy. We assessed additional potential predictive histologic features against a subset (20 kidney biopsy specimens) with the strongest and weakest deep learning predictive scores., Results: We enrolled 442 patients; the 10-year kidney survival was 78%, and the study median follow-up was 6.7 years. Manual MEST-C showed no prognostic relationship for the endocapillary parameter only. The deep learning predictive score was not inferior to MEST-C applied using the International IgA Nephropathy Prediction Tool and the clinical decision supporting system (AUC of 0.84 versus 0.77 and 0.74, respectively) and confirmed a good correlation with the tubolointerstitial score (r=0.41, P <0.01). We observed no correlations between the deep learning prognostic score and the mesangial, endocapillary, segmental sclerosis, and crescent parameters. In this paper, we present novel strategies for optimizing the performance of many binary image processing algorithms. These strategies are collected in an open-source framework, GRAPHGEN, that is able to automatically generate optimized C++ source code implementing the desired optimizations. Simply starting from a set of rules, the algorithms introduced with the GRAPHGEN framework can generate decision trees with minimum average path-length, possibly considering image pattern frequencies, apply state prediction and code compression by the use of Directed Rooted Acyclic Graphs (DRAGs). Moreover, the proposed algorithmic solutions allow to combine different optimization techniques and significantly improve performance. Our proposal is showcased on three classical and widely employed algorithms (namely Connected Components Labeling, Thinning, and Contour Tracing). When compared to existing approaches -in 2D and 3D-, implementations using the generated optimal DRAGs perform significantly better than previous state-of-the-art algorithms, both on CPU and GPU.

Background and Objectives: Immunohistopathology is an essential technique in the diagnostic workflow of a kidney biopsy. Deep learning is an effective tool in the elaboration of medical imaging. We wanted to evaluate the role of a convolutional neural network as a support tool for kidney immunofluorescence reporting., Design, Setting, Participants, & Measurements: High-magnification (×400) immunofluorescence images of kidney biopsies performed from the year 2001 to 2018 were collected. The report, adopted at the Division of Nephrology of the AOU Policlinico di Modena, describes the specimen in terms of "appearance," "distribution," "location," and "intensity" of the glomerular deposits identified with fluorescent antibodies against IgG, IgA, IgM, C1q and C3 complement fractions, fibrinogen, and κ - and λ -light chains. The report was used as ground truth for the training of the convolutional neural networks., Results: In total, 12,259 immunofluorescence images of 2542 subjects undergoing kidney biopsy were collected. The test set analysis showed accuracy values between 0.79 ("irregular capillary wall" feature) and 0.94 ("fine granular" feature). The agreement test of the results obtained by the convolutional neural networks with respect to the ground truth showed similar values to three pathologists of our center. Convolutional neural networks were 117 times faster than human evaluators in analyzing 180 test images. A web platform, where it is possible to upload digitized images of immunofluorescence specimens, is available to evaluate the potential of our approach., Conclusions: The data showed that the accuracy of convolutional neural networks is comparable with that of pathologists experienced in the field., (Copyright © 2020 by the American Society of Nephrology.)

Connected Components Labeling is an essential step of many Image Processing and Computer Vision tasks. Since the first proposal of a labeling algorithm, which dates back to the sixties, many approaches have optimized the computational load needed to label an image. In particular, the use of decision forests and state prediction have recently appeared as valuable strategies to improve performance. However, due to the overhead of the manual construction of prediction states and the size of the resulting machine code, the application of these strategies has been restricted to small masks, thus ignoring the benefit of using a block-based approach. In this paper, we combine a block-based mask with state prediction and code compression: the resulting algorithm is modeled as a Directed Rooted Acyclic Graph with multiple entry points, which is automatically generated without manual intervention. When tested on synthetic and real datasets, in comparison with optimized implementations of state-of-the-art algorithms, the proposed approach shows superior performance, surpassing the results obtained by all compared approaches in all settings.

Background: Detection of small pulmonary lesions has increased, and often they are difficult to localize and resect. We present our mature experience with preoperative computer tomography-guided radiotracer localization, followed by resection of these lesions., Methods: Patients with pulmonary nodule smaller than 1 cm or deep below the visceral pleura underwent computer tomography-guided injection of radiotracer technetium macroaggregates in or close to the lesion. A gamma probe was used to localize the marked area that was resected, and in case of primary lung cancer a lobectomy with nodal dissection was performed., Results: Between November 2007 and December 2017, 262 patients (196 men; median age 63 years) underwent preoperative radiotracer injection with a successful marking in all patients. Complications included 35 asymptomatic pneumothoraxes (13.4%), 36 parenchymal hemorrhage suffusions (13.7%), and 2 mild allergic reactions to contrast medium (0.7%). In all cases, except for 3, the gamma probe revealed the pulmonary lesion. Mean distance from the pleura was 10 mm (range, 0 to 40 mm). Pulmonary resection was performed by thoracoscopy in 212 cases (80.9%), intentional thoracotomy in 42 (16.0%), and converted thoracoscopy in 8 (3.1%). Mean pathologic nodule size was 9.3 mm (range, 2.5 to 39 mm). One hundred sixty-six nodules (63.4%) were nonsolid, 64 (24.4%) were partially solid, and 32 (12.2%) had a solid morphologic characteristic. Histologic examination showed 16 benign (6.1%) and 246 malignant (93.9%) lesions (218 primary lung cancers)., Conclusions: Preoperative radiotracer localization of small or indistinct pulmonary lesions is simple and feasible with a high rate of success. Misidentification of sentinel lymph node via lymphoscintigraphy for breast cancer is an infrequent event. Lung neuroendocrine tumors (NETs) comprise typical (TC) and atypical carcinoids (AC). They represent the well differentiated (WD) or low/intermediate grade forms of lung neuroendocrine neoplasms (NENs). Unlike the lung poorly differentiated NENs, that are usually treated with chemotherapy, lung NETs can be managed with several different therapies, making a multidisciplinary interaction a key point. We critically discussed the multimodal clinical management of patients with advanced lung NETs. Provided that no therapeutic algorithm has been validate so far, each clinical case should be discussed within a NEN-dedicated multidisciplinary team. Among the systemic therapies available for metastatic lung NETs everolimus is the only approved drug, on the basis of the results of the phase III RADIANT-4 trial. Another phase III trial, the SPINET, is ongoing comparing lanreotide with placebo. Peptide receptor radionuclide therapy and chemotherapy were not studied within phase III trials for lung NETs, and they have been reported to be active within retrospective or phase II prospective studies. Temozolomide and oxaliplatin are two interesting chemotherapeutic agents in lung NETs. While some European Institutions were certificated as Centers of Excellence for gastroenteropancreatic NENs by the European Neuroendocrine Tumor Society (ENETS), an equivalent ENETS certification for lung NENs does not exist yet. Ideally a lung NEN-dedicated multidisciplinary tumor board should include NEN-dedicated medical oncologists, thoracic medical oncologist, thoracic surgeons, pathologists, interventional radiologists, endocrinologists, radiotherapists, interventional pneumologists, nuclear physician., Competing Interests: Conflicts of Interest: The authors have no conflicts of interest to declare.

Neuroendocrine tumors (NETs) represent a large and heterogeneous group of malignancies with various biological and clinical characteristics, depending on the site of origin and the grade of tumor proliferation. In NETs, as in other cancer types, molecularly targeted therapies have radically changed the therapeutic landscape. Recently two targeted agents, the mammalian target of rapamycin inhibitor everolimus and the tyrosine kinase inhibitor sunitinib, have both demonstrated significantly prolonged progression free survival in patients with advanced pancreatic NETs. Despite these important therapeutic developments, there are still significant limitations to the use of these agents due to the lack of accurate biomarkers for predicting tumor response and efficacy of therapy. In this review, we provide an overview of the current clinical data for the evaluation of predictive factors of response to/efficacy of everolimus and sunitinib in advanced pancreatic NETs. Surrogate indicators discussed include circulating and tissue markers, as well as non-invasive imaging techniques.

Metronomic therapy is characterized by the administration of regular low doses of certain drugs with very low toxicity. There have been numerous debates over the empirical approach of this regimen, but fewest side effects are always something to consider in order to improve patients' quality of life. Neuroendocrine tumors (NETs) are rare malignancies relatively slow-growing; therefore their treatment is often chronic, involving several different therapies for tumor growth control. Knowing that these tumors are highly vascularized, the anti-angiogenic aspect is highly regarded as something to be targeted in all patients harboring NETs. Additionally the metronomic schedule has proved to be effective on an immunological level, rendering this approach as a multi-targeted therapy. Rationalizing that advanced NETs are in many cases a chronic disease, with which patients can live for as long as possible, a systemic therapy with regular low doses and a very low toxicity is in many cases a judicious manner of pursuing stabilization. Metronomic schedule is usually correlated with chemotherapy in oncology, but other therapies, such as radiotherapy and biotherapy can be delivered in a metronomic like manner. This review describes clinical trials and case series involving metronomic therapies alone or in combination in patients with advanced NETs. Nowadays level of evidence about metronomic therapy in NETs is quite low, therefore future prospective clinical studies are needed to validate the metronomic approach in specific clinical settings., (Copyright © 2017 Elsevier Ltd. Background: A low burden of disease represents an independent favorable prognostic factor of response to peptide receptor radionuclide therapy (PRRT) in patients affected by gastro-entero-pancreatic neuroendocrine tumors. However, it is not clear whether this is due to a lower diffusion of the disease or thanks to debulking surgery., Methods: From 1996 to 2013 those patients diagnosed with G1-G2 pancreatic neuroendocrine tumor (PNET) and synchronous liver metastases who were not deemed eligible for liver radical surgery but were eligible to receive upfront PRRT were prospectively included in the study. Two groups of comparison were identified: those submitted for primary tumor resection before PRRT and those who were not. The outcome was evaluated as: objective response to PRRT (OR), progression-free survival (PFS), and overall survival (OS)., Results: Of the 94 subjects, 31 were previously submitted for primary tumor resection. After propensity score adjustments, patients who underwent surgery before PRRT showed higher stabilization or objective responses after PRRT (p = .006), and this translated into a better median PFS (70 vs. 30 months; p = .002) and OS (112 vs. 65 months; p = .011), for operated versus nonoperated patients, respectively. At multivariate analysis, operated patients showed a statistically significantly improved PFS: HR, 5.11 (95 % CI 1.43-18.3); p = .012, whereas Ki-67 in continuous fashion was correlated significantly with OS: 1.13 (95 % CI 1-1.27); p = .048., Conclusions: Primary tumor resection prior to PRRT can be safely proposed in G1-G2 PNETs with diffuse liver metastases because it seems to enhance response to PRRT and to improve significantly PFS.

Objectives: To assess the independent prognostic value of standardized uptake value (SUV) and apparent diffusion coefficient (ADC), separately and combined, in order to evaluate if the combination of these two variables allows further prognostic stratification of patients with head and neck squamous cell carcinomas (HNSCC)., Methods: Pretreatment SUV and ADC were calculated in 57 patients with HNSCC. Mean follow-up was 21.3 months. Semiquantitative analysis of primary tumours was performed using SUV maxT/B , ADC mean , ADC min and ADC max . The prognostic value of SUV maxT/B , ADC mean , ADC min and ADC max in predicting disease-free survival (DFS) was evaluated with log-rank test and Cox regression models., Results: Patients with SUV maxT/B ≥5.75 had an overall worse prognosis (p = 0.003). After adjusting for lymph node status and diameter, SUV maxT/B and ADC min were both significant predictors of DFS with hazard ratio (HR) = 10.37 (95 % CI 1.22-87.95) and 3.26 (95 % CI 1.20-8.85) for SUV maxT/B ≥5.75 and ADC min ≥0.58 × 10 -3 mm 2 /s, respectively. When the analysis was restricted to subjects with SUV maxT/B ≥5.75, high ADC min significantly predicted a worse prognosis, with adjusted HR = 3.11 (95 % CI 1.13-8.55)., Conclusions: The combination of SUV maxT/B and ADC min improves the prognostic role of the two separate parameters; patients with high SUV maxT/B and high ADC min are associated with a poor prognosis., Key Points: • High SUV maxT/B is a poor prognostic factor in HNSCC • High ADC min is a poor prognostic factor in HNSCC • In patients with high SUV maxT/B , high ADC min identified those with worse prognosis.

Optical coherence tomography (OCT) represents a non-invasive imaging technology, which may be applied to the diagnosis of non-melanoma skin cancer and which has recently been shown to improve the diagnostic accuracy of basal cell carcinoma. Technical developments of OCT continue to expand the applicability of OCT for different neoplastic and inflammatory skin diseases. Of these, dynamic OCT (D-OCT) based on speckle variance OCT is of special interest as it allows the in vivo evaluation of blood vessels and their distribution within specific lesions, providing additional functional information and consequently greater density of data. In an effort to assess the potential of D-OCT for future scientific and clinical studies, we have therefore reviewed the literature and preliminary unpublished data on the visualization of the microvasculature using D-OCT. Information on D-OCT in skin cancers including melanoma, as well as in a variety of other skin diseases, is presented in an atlas. Prostate cancer (PCa) is the fourth most common cancer worldwide in terms of incidence and third among male, but is becoming the most common cancer in developed countries. In many patients the disease will progress despite of castration levels of testosterone, to become castration-resistant PCa (CRPC). Nearly all patients with CRPC show bone metastases. The treatment of patients with bony metastases has dramatically changed during the past three years because of new therapeutic approaches addressed to obtain pain control, reduced skeletal morbidity, and most importantly, increased survival rate. A possible therapy can be based also on the use of radiopharmaceuticals systemically administered to slow or reverse the bone metastatic progression. In facts bone-homing radiopharmaceuticals are taken up in areas of high bone turnover, including areas with high osteoblastic activity. Recently, a bone targeting radiopharmaceutical, Radium-223 dichloride was added to this group of drugs clearly representing a new generation of radiopharmaceutical in bone therapy. Clinical trials had shown that the treatment with Ra-223 allowed the reduction of the risk of death respect to placebo. No other radiometabolic treatment achieved such result, evidentiating the disease-modifying properties of this bone-homing radiopharmaceutical. In an effort to treat patients with disseminated PCa, who became resistant to hormonal therapy, molecular targets have been recently identified. Prostate specific membrane antigen (PSMA) is one attractive target for diagnosis and therapy of metastasized PCa since its expression levels are directly correlated to androgen independence, metastasis, and progression. Gastrin-releasing peptide receptors (GRPr) are also highly overexpressed in PCa. Numerous studies suggest the possibility of a high PCa-specific signal with radiolabeled bombesin analogs targeting GRPr. Low molecular weight peptides directed against these molecular targets have been radiolabeled with positron emitting radionuclides such as 68Ga in order to improve sensitivity and specificity for detecting primary, metastatic, and recurrent PCa by PET/CT over conventional imaging techniques. Although peptide radionuclide ligand therapy studies have just initiated, the diagnostic relevance of 68Ga labeled specific tracers has already been established its clinical utility and represents a valid tool against this common and deadly cancer.

Background: Screening programs have increased the identification of small or indistinct pulmonary lesions that are difficult to localize. We report our experience in their preoperative localization by radiotracer and resection., Methods: Patients with pulmonary nodules of subsolid morphology or smaller than 1 cm, deeper 1 cm below the visceral pleura, or both underwent computed tomography (CT)-guided injection of radiotracer technetium 99m macroaggregates in the vicinity of the lesion. During the operation, a handheld gamma probe was used to detect the hot spot where the radioactive tracer was localized, and this area was resected., Results: From November 2007 to May 2013, 112 patients (58 men; median age 62 years) underwent preoperative radiotracer injection with a successful marking in all patients. Adverse events included 33 (29.4%) pneumothoraces (one requiring chest tube placement), 23 (20.5%) parenchymal hemorrhage suffusions, and 1 (0.9%) allergic reaction to contrast medium. In all cases except two, the gamma probe revealed pulmonary lesions. Overall, 123 pulmonary nodules were localized and resected. The mean distance from the pleura was 12 mm (range, 0 to 39 mm). Pulmonary resection was performed by thoracoscopy in 70 (62.5%) cases, thoracotomy in 36 (32.1%), and conversion of thoracoscopy to thoracotomy in 6 (5.4%). The mean nodule size was 9 mm (range, 3 to 24 mm). Histology showed 14 (11.4%) benign lesions and 109 (88.6%) malignant lesions (85 primary lung cancers and 24 metastases)., Conclusions: Radiotracer localization of pulmonary lesions is a simple and feasible procedure with a high rate of success. Objective: Given the heavy burden of hepatitis C virus (HCV) and human immunodeficiency virus (HIV) infections in correctional facilities, we examined knowledge about these infections among case workers and correctional officers in penal institutions in Puerto Rico., Methods: We used data from a cross-sectional study of state prisons, commissioned by the Puerto Rico Department of Correction and Rehabilitation, to assess knowledge about HCV and HIV (10 items each) among 256 case workers and correctional officers from 18 penal institutions selected in the prison system. Total scores for each scale ranged from 0 to 10 points, with higher scores reflecting more knowledge., Results: Of 256 participants, 64.8% were males, 39.6% were aged 30-39 years, and 70.3% were case workers. The percentage of correct responses for knowledge items ranged from 8.5% to 97.0% for HCV infection and from 38.7% to 99.6% for HIV infection. The vast majority (>96%) of participants knew that injection drug users should be tested for HCV infection and that sharing of needle injection equipment and multiple sex partners increase the risk of HIV infection. However, misconceptions about routes of transmission for these viral infections were found, with larger gaps in knowledge for HCV infection. Mean knowledge scores for HCV and HIV infections were 4.20±0.17 and 6.95±0.22, respectively, being significantly (p<0.05) higher for case workers., Conclusion: The findings about HCV and HIV knowledge in an important segment of the correctional system staff support the urgent need for increasing educational opportunities for correctional staff.

Introduction: Patients with small intestine neuroendocrine tumors present with liver metastases in 50-75% of cases at diagnosis. The aim of the present study was to assess prognostic factors in patients with liver metastases from intestinal neuroendocrine tumor after primary tumor surgical removal with or without liver surgery or radiofrequency ablation. The primary endpoint was disease-specific survival., Methods: Data regarding seventy-eight consecutive patients with liver metastases who undergone primary tumor surgical removal between 1996 and 2011 were extracted from the institutional tumor registry and retrospectively analyzed., Results: Liver tumor burden was <25% in 43 (55.1%) 25-50% in 30 (38.5%) and >50% in 5 (6.4%) patients. For the whole cohort of patients disease-specific survival at 3, 5 and 8 years was 93.2%, 83.6% and 77.3%, respectively. Fifteen patients who underwent radical liver surgery were all alive with a median survival of 106 months (range 18-152 months). In multivariate analysis the Ki-67 index in a continuous fashion significantly correlate with prognosis (p = 0.021). Liver tumor burden (p = 0.036) and extrahepatic involvement (p = 0.03), were the most powerful prognosticators for patients who underwent only debulking surgery., Conclusion: The Ki-67 index, the liver tumor burden and the presence of extrahepatic metastases should be carefully considered in the selection criteria for liver debulking in asymptomatic patients., (Copyright © 2015 IJS Publishing Group Limited. Background: Pancreatic neuroendocrine tumors (PNETs) present in more than 50% of cases with liver metastases as the only systemic localization. Liver metastases are unresectable in 80% of cases at diagnosis. In the context of a metastatic disease, the benefit of primary tumor removal in terms of survival is controversial., Methods: A single-center series of patients with PNETs presenting with synchronous unresectable hepatic metastases and treated within a framework of a multidisciplinary team was analyzed retrospectively to assess the prognostic factors and the potential benefit of primary tumor resection on long-term survival., Results: At the time of diagnosis, 12 of 43 patients (28%) underwent primary tumor resection. After a median follow-up of 5 years (range, 0.6-14 years), 22 disease-related deaths were observed. The corresponding 5-year survival and median disease-specific duration of survival were 58% and 77 months, respectively. In the operated and nonoperated patients the 5-year disease-specific survival was 82% and 50%, respectively (P = .027). At multivariate analysis, patients with primary tumor removed had an improved survival compared with patients who did not (hazard ratio 0.18; 95% CI 0.05-0.66; P = .010). Other important factors associated with improved survival at multivariate analysis were lesser age, lesser Ki-67 index, and 25% less liver tumor burden., Conclusion: In the present series of patients with PNETs and unresectable liver metastases, resection of the primary tumor was associated with an improved survival. This observation suggests that resection of the primary tumor should be part of a global therapeutic strategy and its indication and timing should be discussed within a multidisciplinary team., (Copyright © 2014 Mosby, Inc. We evaluated clinical activity of ⁹⁰Yttrium-ibritumomab (⁹⁰Y-ibritumomab) tiuxetan in extranodal marginal-zone lymphoma. From May 2004 to April 2011, 30 patients affected by relapsed/refractory marginal-zone lymphoma--arisen at any extranodal site--received ⁹⁰Y-ibritumomab tiuxetan at the activity of 0.4 mCi/kg. Median age was 57 years. At time of treatment, 13 out of 30 patients had disseminated disease (stage III/IV). All patients had received a previous treatment with a maximum of 7. Overall response rate was 90%: 23 patients achieved a complete response (77%); partial response occurred in 4 patients (13%), stable disease in 2 patients (7%) and 1 progression (3%). With a median follow-up of 5.3 years, median time to relapse was not reached; 2 patients relapsed after complete response; 18 out of 23 complete responses are still responders after >3 years, 12 of them after >5 years. ⁹⁰Y-ibritumomab tiuxetan seems to be active in patients with extranodal marginal-zone lymphoma relapsed/refractory to conventional treatment including radiotherapy. Unlabelled: We present here a systematic analysis of lymphoma and MM patients recruited into 2 clinical trials or treated with radretumab according to compassionate use, describing the biodistribution, dosimetry, safety, and clinical activity of radretumab., Methods: Uptake in lymphoma lesions, safety, and clinical activity of radretumab radioimmunotherapy (R-RIT) were evaluated in 18 relapsed lymphoma or multiple myeloma patients., Results: In 14 of 18 patients, selective tumor uptake was found; 11 of 15 lymphoma patients, including 9 of 11 with Hodgkin lymphoma (HL), were eligible for R-RIT (a priori criteria-based target-to-bone marrow ratio > 10:1 for EudraCT no. 2005-000545 or > 4:1 for EudraCT no. 2007-007241-12 at dosimetric imaging). Two HL and 1 diffuse large B cell lymphoma patient achieved complete response; 1 HL patient had partial response. Both multiple myeloma patients receiving R-RIT experienced stabilization of disease. Therefore, the overall objective response rate was 40%. Uncomplicated grade 3-4 thrombocytopenia or leukocytopenia was observed in 5 R-RIT patients, lasting 4-129 d., Conclusion: R-RIT showed a favorable benefit and risk profile in advanced relapsed lymphoma patients and induced complete response in 2 heavily pretreated, relapsed HL patients and in 1 diffuse large B cell lymphoma patient. These results warrant further exploration of R-RIT in larger phase II clinical trials.

Neuroendocrine tumours (NET) are a heterogeneous group of neoplasms commonly occurring in the gastrointestinal tract or lungs but can occur in other regions. Primary ovarian NET account for 5% of all NET and 0.1% of all ovarian malignancies. In metastatic disease, the therapeutic goal is to extend survival and to improve quality of life. As these tumours express somatostatin receptors, somatostatin analogues are frequently used to control symptoms. Here we present a case of a pregnant woman with an ovarian NET with liver metastases and carcinoid syndrome who was treated with the somatostatin analogue, Octreotide LAR. We also summarize reported data of the use of somatostatin analogues during pregnancy.

In this paper, we define a new paradigm for eight-connection labeling, which employs a general approach to improve neighborhood exploration and minimizes the number of memory accesses. First, we exploit and extend the decision table formalism introducing OR-decision tables, in which multiple alternative actions are managed. An automatic procedure to synthesize the optimal decision tree from the decision table is used, providing the most effective conditions evaluation order. Second, we propose a new scanning technique that moves on a 2 x 2 pixel grid over the image, which is optimized by the automatically generated decision tree. An extensive comparison with the state of art approaches is proposed, both on synthetic and real datasets. The synthetic dataset is composed of different sizes and densities random images, while the real datasets are an artistic image analysis dataset, a document analysis dataset for text detection and recognition, and finally a standard resolution dataset for picture segmentation tasks. The algorithm provides an impressive speedup over the state of the art algorithms.

Current treatment of hematologic malignancies involves rather unspecific chemotherapy, frequently resulting in severe adverse events. Thus, modern clinical research focuses on compounds able to discriminate malignant from normal tissues. Being expressed in newly formed blood vessels of solid cancers but not in normal mature tissues, the extradomain B of fibronectin (ED-B FN) is a promising target for selective cancer therapies. Using immunohistology with a new epitope retrieval technique for paraffin-embedded tissues, ED-B FN expression was found in biopsies from more than 200 Hodgkin and non-Hodgkin lymphoma patients of nearly all entities, and in patients with myeloproliferative diseases. ED-B FN expression was nearly absent in normal lymph nodes (n = 10) and bone marrow biopsies (n = 9). The extent of vascular ED-B FN expression in lymphoma tissues was positively correlated with grade of malignancy. ED-B FN expression was enhanced in lymph nodes with severe lymphadenopathy and in some hyperplastic tonsils. The in vivo accessibility of ED-B FN was confirmed in 3 lymphoma patients, in whom the lymphoma lesions were visualized on scintigraphy with (131)I-labeled L19 small immunoprotein ((131)I-L19SIP). In 2 relapsed Hodgkin lymphoma patients(131)I-L19SIP radioimmunotherapy induced a sustained partial response, qualifying ED-B FN as a promising target for antibody-based lymphoma therapies.

Treatment modalities for resistant/relapsing gastric mucosa associated lymphoid tissue (MALT) non-Hodgkin's lymphoma (NHL) are not yet well standardized. In the past, most patients were treated surgically with a gastrectomy, while, more recently, radiotherapy and systemic approaches (chemotherapy and immunotherapy) have been used with improving results.Here, we report the case of a patient affected by MALT NHL resistant to antibiotics, chemotherapy and immunotherapy, who achieved a durable complete remission after radio-immunotherapy treatment with Zevalin ((90)Y ibritumomab-tiuxetan), administered in a single-standard dose. This observation must be confirmed on a larger series but suggests that radio-immunotherapy may be a valid approach in treating relapsing MALT NHL patients, or those resistant to conventional therapies, so avoiding more aggressive and toxic approaches.

Background: To overcome subjectivity and variability in the interpretation of dermoscopic images, image analysis programs, enabling the numerical description of melanocytic lesion images, have been developed., Objectives: Our aim was to assess a method for the description of colours in melanocytic lesion images, based on the subdivision of image colours into red, green and blue clusters., Methods: Melanomas and naevi of the test set were described by means of 23 colour clusters previously selected by a training set comprising 369 melanocytic lesion images. The diagnostic performance obtained by this automated method was compared to sensitivity and specificity of diagnosis of 4 dermatologists., Results: Colour cluster values significantly differed between melanomas and naevi. Moreover, sensitivity and specificity values of computer diagnosis were similar to those achieved by the dermatologists., Conclusion: Our image analysis program based on the assessment of one single parameter has the diagnostic accuracy of dermatologists employing dermoscopy on a regular basis., (Copyright 2007 S. Background: Semiquantitative algorithms were applied to dermoscopic images to improve the clinical diagnosis for melanoma., Objective: The aim of the study was to develop a computerized method for automated quantification of the 'A' (asymmetry) and 'B' (border cut-off) parameters, according to the ABCD rule for dermoscopy, thus reproducing human evaluation., Methods: Three hundred and thirty-one melanocytic lesion images, referring to 113 melanomas and 218 melanocytic nevi, acquired by means of a digital videodermatoscope, were considered. Images were evaluated by two experienced observers and by using computer algorithms developed by us. Clinical evaluation of asymmetry was performed by attributing scores to shape asymmetry and asymmetry of pigment distribution and structures, whereas computer evaluation of shape and pigment distribution asymmetries were based on the assessment of differences in area and lightness in the two halves of the image, respectively. Borders were evaluated both by clinicians and by the computer, by attributing a score to each border segment ending abruptly. Differences between nevus and melanoma values were evaluated using the chi-square test, while Cohen's Kappa index for agreement was employed for the evaluation of the concordance between human and computer., Results: Pigment distribution asymmetry appears the most striking parameter for melanoma diagnosis both for human and for automated diagnosis. A good concordance between clinicians and computer evaluation was achieved for all asymmetry parameters, and was excellent for border cut-off evaluation., Conclusions: These algorithms enable a good reproduction of the 'A' and 'B' parameters of the ABCD rule for dermoscopy, and appear useful for diagnostic and learning purposes.

Despite years of intensive research, the prognosis of high-grade gliomas (HGG) remains poor, as these tumors are highly resistant to currently available therapies. Therefore, there is a need for the development of new therapeutic strategies, such as the use of monoclonal antibodies (MoAbs) in association with radioisotopes, in order to achieve better responses and prognosis. This article describes our experience in radioimmunotherapy (RIT) with MoAbs and tumor pre-targeting with the avidin-biotin system, either in systemic or locoregional administrations. This therapy offers the exciting prospect of increasing the specificity of tumor cell irradiation with radioisotopes. We suggest that RIT, both systemic and locoregional, should be used as part of a combined modality approach: in combination with surgery, radiotherapy and chemotherapy.

Digital dermoscopy improves the accuracy of melanoma diagnosis. The aim of this study was to develop and validate software for assessment of asymmetry in melanocytic lesion images, based on evaluation of colour symmetry, and to compare it with assessment by human observers. An image analysis program enabling numerical assessment of asymmetry in melanocytic lesions, based on the evaluation and comparison of CIE L*a*b* colour components (CIE L*a*b* is the name of a colour space defined by the Commission Internationale de l'Eclairage) inside image colour blocks, was employed on the recorded lesion images. Clinical evaluation of asymmetry in dermoscopic images was performed on the same image set employing a 0-1 scoring system. Asymmetry judgement was expressed by the clinicians for 12.8% of benign naevi, 44.7% of atypical naevi and 64.2% of malignant melanomas, whereas the computer identified as asymmetric 6.3%, 33.3% and 82.2%, respectively. Numerical parameters referring to malignant melanomas were significantly higher, both with respect to benign naevi and atypical naevi. The numerical parameters produced could be effectively employed for computer-aided melanoma diagnosis.

Background/purpose: The assessment of colors is essential for melanoma (MM) diagnosis, both for pattern analysis on dermoscopic images, and when using semiquantitative methods. Our aim was to provide a simple, precise characterization and reproducible calibration of the color response for dermoscopic instruments., Methods: Three processes were used to correct the non-uniform illumination pattern of the instrument, to easily estimate the camera gamma settings and to describe the color space conversion matrices required to produce standard images, in any color space. A specific color space was also developed to optimize the representation of dermatoscopic colors. The calibration technique was tested both on synthetic reference surfaces and on real images by comparing the difference between the images colors obtained with two different equipments., Results: The differences between the images acquired by means of the two instruments, calculated on the reference patterns after calibration, were up to 10 times lower then before, while comparison of histograms referring to real images provided an improvement of about seven times on average., Conclusions: A complete workflow for dermatologic image calibration, which allows the user to continue using his own software and algorithms, but with a much higher informative content, is presented. The technique is simple and may improve cooperation between different research centers, in teleconsulting contexts or for result comparisons.

Background/purpose: Since in early melanoma (MM) and especially in in situ MM differential structures, which are diagnostic for MM may be lacking, pigment distribution asymmetry represents an important diagnostic feature. Our aim was to automatically assess pigment distribution in images referring to MMs, atypical nevi (AN) and clearly benign nevi (BN), and to evaluate the diagnostic capability of numerical parameters describing a non homogeneous distribution of pigmentation., Methods: An image analysis program enabling the numerical assessment of pigment distribution in melanocytic lesions (ML), based on evaluation and comparison of red, green, blue (RGB) colour components inside image colour blocks, was employed on 459 videomicroscopic digital images, referring to 95 MMs, 76 AN and 288 BN., Results: Significant differences in pigment distribution parameters (mean RGB distance, variance and maximum distance) between the three ML populations were observed, permitting a good discrimination of MMs. On the test set comprising 230 lesion images, the area under the curve value of the receiver operating characteristic curve was 0.933. For a D score equal to 0, corresponding to the best diagnostic accuracy (86.6%), a sensitivity of 87.5% and a specificity of 85.7% were obtained., Conclusion: This original evaluation method for digital pigment distribution, based on mathematical description and comparison of colours in different image blocks, provides numerical parameters to be implemented in image analysis programs for computer-aided MM diagnosis.
