Abstract Background To maintain adequate oxygenation is of utmost importance in intraoperative care. However, clinical evidence supporting specific oxygen levels in distinct surgical settings is lacking. This study aimed to compare the effects of 30% and 80% oxygen in off-pump coronary artery bypass grafting (OPCAB). Methods This multicenter trial was conducted in three tertiary hospitals from August 2019 to August 2021. Patients undergoing OPCAB were cluster-randomized to receive either 30% or 80% oxygen intraoperatively, based on the month when the surgery was performed. The primary endpoint was the length of hospital stay. Intraoperative hemodynamic data were also compared. Results A total of 414 patients were cluster-randomized. Length of hospital stay was not different in the 30% oxygen group compared to the 80% oxygen group (median, 7.0 days vs 7.0 days; the sub-distribution hazard ratio, 0.98; 95% confidence interval [CI] 0.83–1.16; P = 0.808). The incidence of postoperative acute kidney injury was significantly higher in the 30% oxygen group than in the 80% oxygen group (30.7% vs 19.4%; odds ratio, 1.94; 95% CI 1.18–3.17; P = 0.036). - Abstract
Paroxysmal nocturnal haemoglobinuria (PNH) is a clonal haematopoietic stem cell disease that presents with haemolytic anaemia, thrombosis and bone marrow failure. We report a case of a 51-year-old male with a history of PNH in treatment with Eculizumab admitted to our Hospital for acute chest pain and dyspnoea. The diagnosis was a triple vessel disease and patient was scheduled for coronary artery bypass grafting surgery. To balance the risk between thrombosis and bleeding in this particular clinical setting, we decided to use thromboelastography (TEG) as point of care solution and we used the R parameter as the target of our anticoagulant therapy. The R parameter between 11 and 14 sec can be used as a target value to balance the risk; in addition, there was no evidence of acute hemolysis during the surgery and supplemental dose of Eculizumab was administered in order to minimize any potential exacerbation of intravascular hemolysis., Пароксизмальная ночная гемоглобинурия (ПНГ) — клональное заболевание гемопоэтических стволовых клеток, которое проявляется гемолитической анемией, тромбозами и недостаточностью функции костного мозга. Пациент мужского пола 51 года с ПНГ в анамнезе, по поводу которой он получал лечение экулизумабом, поступил в больницу с жалобами на острую боль в грудной клетке и одышку. Поставили диагноз трехсосудистого поражения коронарного русла, по поводу чего запланировали проведение аортокоронарного шунтирования. Для того, чтобы избежать развития как тромбоза, так и кровотечения, в данном сложном клиническом случае, решили провести пациенту тромбоэластографическое исследование и использовать параметр R в качестве целевого при проведении антикоагулянтной терапии. При поддержании значений данного параметра в рамках 11-14 сек можно достичь оптимального баланса между риском тромбоза и кровотечения. Кроме того, во время оперативного вмешательства не наблюдали острого гемолиза, а для уменьшения риска развития внутрисосудистого гемолиза дополнительно назначили экулизумаб.

Perioperative anemia is an important risk factor for cardiac surgery-associated acute kidney injury (CSA-AKI). Nonetheless, the severity of the anemia and the time in the perioperative period in which the hemoglobin level should be considered as a risk factor is conflicting. The present study introduces the concept of perioperative hemoglobin area under the curve (pHb-AUC) as a surrogate marker of the evolution of perioperative hemoglobin concentration. Through a retrospective analysis of prospectively collected data, we assessed this new variable as a risk factor for the development of acute kidney injury after cardiac surgery in 966 adult patients who underwent cardiac surgery with cardiopulmonary bypass, at twenty-three academic hospitals in Spain. Exclusion criteria were patients on renal replacement therapy, who needed a reoperation because of bleeding and/or with missing perioperative hemoglobin or creatinine values. Background: Coronary artery bypass grafting (CABG) and surgical aortic valve replacement (AVR) are the 2 most common cardiac surgery procedures in North America. We derived and externally validated clinical models to estimate the likelihood of death within 30 days of CABG, AVR or combined CABG + AVR.Methods: We obtained data from the CorHealth Ontario Cardiac Registry and several linked population health administrative databases from Ontario, Canada. We derived multiple logistic regression models from all adult patients who underwent CABG, AVR or combined CABG + AVR from April 2017 to March 2019, and validated them in 2 temporally distinct cohorts (April 2015 to March 2017 and April 2019 to March 2020).Results: The derivation cohorts included 13 435 patients who underwent CABG (30-d mortality 1.73%), 1970 patients who underwent AVR (30-d mortality 1.68%) and 1510 patients who underwent combined CABG + AVR (30-d mortality 3.05%). The final models for predicting 30-day mortality included 15 variables for patients undergoing CABG, 5 variables for patients undergoing AVR and 5 variables for patients undergoing combined CABG + AVR. Model discrimination was excellent for the CABG (c-statistic 0.888, optimism-corrected 0.866) AVR (c-statistic 0.850, optimism-corrected 0.762) and CABG + AVR (c-statistic 0.844, optimism-corrected 0.776) models, with similar results in the validation cohorts.Interpretation: Our models, leveraging readily available, multidimensional data sources, computed accurate risk-adjusted 30-day mortality rates for CABG, AVR and combined CABG + AVR, with discrimination comparable to more complex American and European models. The ability to accurately predict perioperative mortality rates for these procedures will be valuable for quality improvement initiatives across institutions. [ABSTRACT FROM AUTHOR]- Published

Background: Atrial fibrillation (AF) is the most common postoperative complication following cardiac surgery, with incidence of (20 – 50 %) was reported. its occurrence has been linked to the inflammatory process post cardiopulmonary bypass. Objectives: To test whether nonsteroidal anti-inflammatory drugs could reduce the incidence of atrial fibrillation post cardiac surgery. Double blind, randomized study conducted at Queen Alia Heart Institute from June 2014 to June 2015 on 200 patients who underwent elective first time coronary artery bypass grafting. Patients were divided into two equal groups; one group of patients was given 400mg of Ibuprofen three times daily while the other group received conventional treatment. The primary end point was the incidence of atrial fibrillation. Results: Atrial fibrillation occurred in 12% of the treatment group in contrast to 27% in the control group P value There was 51.8% decrease in the incidence of AF. The length of hospital stay was found to be significant statistically between the two groups. 4.6 ± 1.3 for the ibuprofen group VS 6.2 ± 2.8 for the control group P Conclusion: Use of nonsteroidal anti-inflammatory drugs such as Ibuprofen was found to be relatively safe and effective in reducing the incidence of post-operative atrial fibrillation in patients undergoing elective coronary artery bypass grafting

Administration of prophylactic glucocorticoids has been suggested as a strategy to reduce postoperative AKI and other adverse events after cardiac surgery requiring cardiopulmonary bypass. In this post hoc analysis of a large placebo-controlled randomized trial of dexamethasone in 4465 adult patients undergoing cardiac surgery, we examined severe AKI, defined as use of RRT, as a primary outcome. Secondary outcomes were doubling of serum creatinine level or AKI-RRT, as well as AKI-RRT or in-hospital mortality (RRT/death). The primary outcome occurred in ten patients (0.4%) in the dexamethasone group and in 23 patients (1.0%) in the placebo group (relative risk, 0.44; 95% confidence interval, 0.19 to 0.96). Prophylactic intra-operative administration of dexamethasone may improve short-term clinical outcomes in cardiac surgical patients. The purpose of this study was to evaluate long-term clinical outcomes and cost effectiveness of dexamethasone versus placebo. Patients included in the multicentre, randomised, double-blind, placebo-controlled DExamethasone for Cardiac Surgery (DECS) trial were followed up for 12 months after their cardiac surgical procedure. In the DECS trial, patients received a single intra-operative dose of dexamethasone 1 mg.kg(-1) (n = 2239) or placebo (n = 2255). The effects on the incidence of major postoperative events were evaluated. Also, overall costs for the 12-month postoperative period, and cost effectiveness, were compared between groups. Of 4494 randomised patients, 4457 patients (99%) were followed up until 12 months after surgery. There was no difference in the incidence of major postoperative events, the relative risk (95%CI) being 0.86 (0.72-1.03); p = 0.1. Treatment with dexamethasone reduced costs per patient by £921 [€1084] (95%CI £-1672 to -137; p = 0.02), mainly through reduction of postoperative respiratory failure and duration of postoperative hospital stay. The probability of dexamethasone being cost effective compared with placebo was 97% at a threshold value of £17,000 [€20,000] per quality-adjusted life year. We conclude that intra-operative high-dose dexamethasone did not have an effect on major adverse events at 12 months after cardiac surgery, but was associated with a reduction in costs. Routine dexamethasone administration is expected to be cost effective at commonly accepted threshold levels for cost effectiveness.

Новонароджені з критичною вродженою вадою серця (ВВС), прооперовані в умовах штучного кровообігу, під час якого використовували аутологічну пуповинну кров, після операції мали статистично вірогідне зниження вмісту білка S100β у сироватці крові порівняно з доопераційним рівнем. Новонароджені з ВВС, яким під час операції застосовували компоненти донорської крові, на першу добу після операції мали статистично вірогідне підвищення рівня досліджуваного білка порівняно з доопераційним, що може свідчити про більш виражені порушення мозкового кровообігу під час операції в пацієнтів, яким переливали донорську кров., Новорожденные с критическим врожденным пороком сердца (ВПС), прооперированные в условиях искусственного кровообращения, во время которого использовали аутологичную пуповинную кровь, после операции имели статистически достоверное снижение содержания белка S100β в сыворотке крови по сравнению с дооперационным уровнем. Новорожденные с ВПС, которым во время операции переливали компоненты донорской крови, в первые сутки после операции имели статистически достоверное повышение уровня исследуемого белка по сравнению с дооперационным, что может указывать на более выраженные нарушения мозгового крово-обращения во время операции у пациентов, которым переливали донорскую кровь., Neonates with critical congenital heart disease (CHD), who underwent surgery with cardiopulmonary bypass, during which autologous cord blood was used, after surgery had a statistically significant reduction of S100β protein content in the blood serum compared with the preoperative level. Neonates with CHD, in whom during surgery donated blood components were used, in the first day after surgery had a statistically significant increase in the level of the test protein compared with the preoperative level, which may indicate a more severe disorders of cerebral circulation during surgery in patients with donor-specific transfusion.

В статье обобщены литературные данные о врожденных аномалиях развития печени у детей. Проведен анализ 3 клинических наблюдений транспозиции добавочных долей печени в грудную полость у детей. Во всех случаях имели место истинные диафрагмальные грыжи. Клинические проявления данной патологии зависели от степени выраженности компрессии органов средостения. Показания к оперативной коррекции определялись индивидуально. У одного пациента выполнены перемещение добавочной доли печени в брюшную полость и аллопластика купола диафрагмы. На основании сопоставления клинических данных с этапами эмбриогенеза печени, диафрагмы и сердца обсуждены возможные механизмы формирования этих сложных врожденных пороков развития., У статті узагальнено літературні дані про вроджені аномалії розвитку печінки в дітей. Проведено аналіз 3 клінічних спостережень транспозиції додаткових часток печінки в грудну порожнину в дітей. У всіх випадках мали місце справжні діафрагмальні грижі. Клінічні прояви цієї патології залежали від ступеня вираженості компресії органів середостіння. Показання до оперативної корекції визначалися індивідуально. В одного пацієнта виконані переміщення додаткової частки печінки в черевну порожнину й алопластика купола діафрагми. На підставі зіставлення клінічних даних з етапами ембріогенезу печінки, діафрагми та серця обговорені можливі механізми формування цих складних вроджених вад розвитку., The literature data on hepatic congenital anomalies in children are summarized in this article. Three clinical observations of transposition of extra lobe of the liver (ELL) into the thoracic cavity in children were analyzed. The true diaphragmatic hernia was found in all cases. Clinical manifestations of this disease depend on the severity of the compression of mediastinum organs. Indications for surgical correction were determined individually. ELL move into the abdominal cavity, excision of the hernia sac and alloplasty of the diaphragmatic defect were performed for one patient. Possible mechanisms for the formation of these complex congenital malformations were discussed based on the comparison of clinical data with the stages of embryogenesis liver, diaphragm and heart.

We undertook a study to describe changes in heart rate variability (HRV) postoperatively in children undergoing cardiac bypass surgery for congenital heart disease (CHD). HRV was recorded for a 1-h period preoperatively and a 24-h period postoperatively in 20 children with CHD. We found a highly significant reduction in HRV in both time and frequency domain indices compared to preoperative values, which was sustained throughout the 24-h study period. There was a negative correlation between both time and frequency domain HRV measurements and length of cardiac bypass. HRV is reduced postoperatively and correlates with cardiac bypass time. Length of cardiac bypass time may be one mechanism whereby HRV is reduced following surgery.

30. Intraoperative High-Dose Dexamethasone in Cardiac Surgery and the Risk of Rethoracotomy

BACKGROUND: Cardiac surgery with the use of cardiopulmonary bypass is associated with a systemic inflammatory response. Intraoperative corticosteroids are administered to attenuate this inflammatory response. The recent Dexamethasone for Cardiac Surgery (DECS) trial could not demonstrate a beneficial effect of dexamethasone on major adverse events in cardiac surgical patients. Previous studies suggest that corticosteroids may affect postoperative coagulation and blood loss, and therefore could influence the risk of surgical reinterventions. We investigated the effects of prophylactic intraoperative dexamethasone treatment on the rate of rethoracotomy after cardiac surgery. METHODS: We performed a post-hoc additional data collection and analysis in the DECS trial. A total of 4,494 adult patients undergoing cardiac surgery with cardiopulmonary bypass were randomly assigned to intravenous dexamethasone (1.0 mg/kg) or placebo. The primary endpoint for the present study was the incidence of any rethoracotomy within the first 30 postoperative days. Secondary endpoints included the reason for rethoracotomy and the incidence of perioperative transfusion of blood products. RESULTS: In the dexamethasone group, 217 patients (9.7%) underwent a rethoracotomy, and in the placebo group, 165 patients did (7.3%; relative risk 1.32, 95% confidence interval: 1.09 to 1.61, p = 0.005). The most common reason for rethoracotomy was tamponade in both groups: 3.9% versus 2.1%, respectively (relative risk 1.84, 95% confidence interval: 1.30 to 2.61, p < 0.001). CONCLUSIONS: Intraoperative high-dose dexamethasone administration in cardiac surgery was associated with an increased rethoracotomy risk.

It is unclear how best to predict peri-operative cardiovascular risk in patients with atrial fibrillation undergoing non-cardiac surgery. This study examined the accuracy of the revised cardiac risk index and three atrial fibrillation thrombo-embolic risk models for predicting 30-day cardiovascular events after non-cardiac surgery in patients with a pre-operative history of atrial fibrillation. We conducted a prospective cohort study in 28 centres from 2007 to 2013 of 40,004 patients ≥ 45 years of age undergoing inpatient non-cardiac surgery who were followed until 30 days after surgery for cardiovascular events (defined as myocardial injury, heart failure, stroke, resuscitated cardiac arrest or cardiovascular death). The 2088 patients with a pre-operative history of atrial fibrillation were at higher risk of peri-operative cardiovascular events compared with the 34,830 patients without a history of atrial fibrillation (29% vs. 13%, respectively, adjusted odds ratio 1.30 (95%CI 1.17-1.45). Compared with the revised cardiac risk index (c-index 0.60), all atrial fibrillation thrombo-embolic risk scores were significantly better at predicting peri-operative cardiovascular events: CHADS2 (c-index 0.62); CHA2 DS2 -VASc (c-index 0.63); and R2 CHADS2 (c-index 0.65), respectively. Although the three thrombo-embolic risk prediction models were significantly better than the revised cardiac risk index for prediction of peri-operative cardiovascular events, none of the four models exhibited strong discrimination metrics. BACKGROUND: Postoperative atrial fibrillation (AF) is a common complication after cardiac surgery. Inflammation is believed to play a pivotal role in the etiology of postoperative AF. There is a suggestion from small studies that perioperative treatment with corticosteroids may reduce postoperative AF. The DExamethasone for Cardiac Surgery (DECS) study was a large randomized trial showing no protective effect of dexamethasone on major adverse events. The aim of this study was to investigate the effect of dexamethasone treatment on the occurrence of AF after cardiac surgery. METHODS: The DECS study compared intra-operative dexamethasone (1mg/kg) or placebo treatment in 4494 adult patients undergoing cardiac surgery. AF was defined by the occurrence of any reported AF within 30days after surgery. We also performed an in-depth analysis of a subset of 1565 patients on new-onset AF. Relative risks (RRs) with 95% confidence intervals (CIs) were calculated. RESULTS: The incidence of any AF in the main study of 4494 patients was 33.1% in the dexamethasone and 35.2% in the placebo group (RR 0.94, 95% CI: 0.87-1.02, p=0.14). In the substudy of 1565 patients, the incidence of new-onset AF was 33.0% vs. 35.5% (RR 0.93, 95% CI: 0.81-1.07, p=0.31), respectively. There was no protective effect of dexamethasone across clinically important patient subgroups. CONCLUSION: Intraoperative administration of dexamethasone had no protective effect on the occurrence of any or new-onset atrial fibrillation after cardiac surgery. Therefore, the use of dexamethasone for the reduction of postoperative AF should not be recommended.

Pediatric heart surgery aims to correct or palliate cardiac malformations, and to maximize the quality of life of the patients. This thesis investigates the late outcome after surgery for congenital cardiac defects to assess the quality and progress of treatment during the last decades. It also studies outcomes after the arterial switch operation (ASO) for treatment of transposition of the great arteries (TGA) and pediatric heart transplantation. Data was obtained from a national database containing all pediatric heart operations performed since 1953 in one of five university- and one regional hospital in Finland. Causes of death were obtained from Statistics Finland and categorized into congenital heart defect (CHD)- related and non-CHD-related causes of death. CHD-related deaths were further divided into heart failure-, post-reoperative early-, cardiovascular-, and sudden deaths. Between 1953 and 2009, 13,786 cardiac operations were performed on 10,964 pediatric patients in Finland. Follow-up coverage was 98%. Early operative mortality was 5.6% for all operations. The 60-year survival for the entire study was 70% versus 86% for the general population. The number and proportion of severe cardiac defects increased in the 2000s, whereas surgery for simple defects decreased. Operative age and early operative mortality decreased significantly among all defect groups. Long-term survival of all patients improved significantly during recent decades. The most common cause of death was heart failure, which decreased among the majority of defect groups. Sudden death was reduced to zero among patients with simple defects and TGA, but remained a challenge among patients with univentricular heart configurations. ASO resulted in markedly improved early operative mortality and late outcome after correction of TGA when compared to the Mustard and Senning operations, with zero sudden deaths. Tutkimuksessa tarkastellaan erikseen myös valtasuonten transposition (TGA) hoidossa käytetyn arterial switch menetelmän (ASO) ja lasten sydänsiirtojen pitkäaikaistuloksia. Tutkimuksessa käytetään lasten sydänleikkausten tietokantaa, ProCardio:ta. Tietokanta perustettiin vuonna 1995, ja se sisältää tiedot kaikista Suomessa tehdyistä lasten sydänleikkauksista vuodesta 1953 lähtien. Kuolinsyyt on jaettu kardiogeenisiin ja ei- kardiogeenisiin, ja kardiogeeniset kuolinsyyt edelleen sydämen vajaatoimintaan, kardiovaskulaariseen, perioperatiiviseen ja äkilliseen kuolemaan. Vuosien 1953 ja 2009 välillä Suomessa tehtiin 13 786 sydänleikkausta 10 694:lle lapsipotilaalle. Seurannan kattavuus oli 98%. Leikkauskuolleisuus oli yhteensä 5.6%. 60 vuoden pitkäaikaisselviytyminen oli 70% kaikilla potilailla, ja 86% sukupuolen-, iän-, synnyinajan-, ja sairaanhoitopiirin suhteen samankaltaistetulla kontrolliväestöllä. Vaikeiden sydänvikojen osuus kaikista leikatuista sydänvioista nousi tasaisesti 2000-luvulle asti, samalla kun yksinkertaisten sydänvikojen leikkaukset vähenivät. Leikkausikä pieneni ja leikkauskuolemat vähenivät merkittävästi kaikissa sydänvikaryhmissä, ja pitkäaikaisselviytyminen parani viime vuosikymmenten aikana leikattujen keskuudessa. Tavallisin kuolinsyy oli sydämen vajaatoiminta, jonka esiintyvyys väheni merkittävästi suurimmassa osassa sydänvikaryhmiä. - Abstract
Atrial fibrillation (AF) usually develops within the first 72 h following cardiac surgery, and is often self-limiting. Within 48 h of acute onset of symptoms, approximately 50% of patients spontaneously convert to normal sinus rhythm. Thus, the relative risks and benefits of therapy must be carefully considered. The etiology of AF following cardiac surgery is similar to that in non-surgical patients except that pericardial inflammation and increased adrenergic tone play an increasingly important role. Further, AF after surgery may be associated with transient risk factors that resolve as the patient moves out from surgery, and the condition is less likely to recur compared to AF arising in other circumstances. Immediate heart rate control is important in preventing ischemia, tachycardia-induced cardiomyopathy, and left ventricular dilatation. At our institution, amiodarone is frequently used as a first-line drug for treating AF after cardiac surgery. Inconsistent prescribing practices, variable dosage regimens, and a lack of consensus regarding the appropriate use of amiodarone prompted the need for developing practice guidelines. Multidisciplinary collaboration between the departments of cardiac surgery, pharmacy, and anesthesiology led to the development of a protocol for postoperative AF. We review the clinical evidence from published trials and discuss our guidelines, defining amiodarone use for AF in the cardiac surgery setting. Copyright © 2007 Wiley Periodicals, Inc.

48. External validation of predictive models for acute kidney injury following cardiac surgery: A prospective multicentre cohort study.

- Abstract
Background: Four predictive models for acute kidney injury associated with cardiac surgery were developed by Demirjian in the United States in 2012. However, the usefulness of these models in clinical practice needs to be established in different populations independent of that used to develop the models.Objectives: Our aim was to evaluate the predictive performance of these models in a Spanish population.Design: A multicentre, prospective observational study.Data Sources: Twenty-three Spanish hospitals in 2012 and 2013.Eligibility Criteria: Of 1067 consecutive cardiac patients recruited for the study, 1014 patients remained suitable for the final analysis.Main Outcome Measures: Dialysis therapy, and a composite outcome of either a doubling of the serum creatinine level or dialysis therapy, in the 2 weeks (or until discharge, if sooner) after cardiac surgery.Results: Of the 1014 patients analysed, 34 (3.4%) required dialysis and 95 (9.4%) had either dialysis or doubled their serum creatinine level. The areas under the receiver operating characteristic curves of the two predictive models for dialysis therapy, which include either presurgical variables only, or combined presurgical and intrasurgical variables, were 0.79 and 0.80, respectively. The model for the composite endpoint that combined presurgical and intrasurgical variables showed better discriminatory ability than the model that included only presurgical variables: the areas under the receiver operating characteristic curves were 0.76 and 0.70, respectively. All four models lacked calibration for their respective outcomes in our Spanish population.Conclusion: Overall, the lack of calibration of these models and the difficulty in using the models clinically because of the large number of variables limit their applicability. Context: beta-Blockade therapy has recently been shown to convey a survival benefit in preoperative noncardiac vascular surgical settings. The effect of preoperative beta-blocker therapy on coronary artery bypass graft surgery (CABG) outcomes has not been assessed.Objectives: To examine patterns of use of preoperative beta-blockers in patients undergoing isolated CABG and to determine whether use of beta-blockers is associated with lower operative mortality and morbidity.Design, Setting, and Patients: Observational study using the Society of Thoracic Surgeons National Adult Cardiac Surgery Database (NCD) to assess beta-blocker use and outcomes among 629 877 patients undergoing isolated CABG between 1996 and 1999 at 497 US and Canadian sites.Main Outcome Measure: Influence of beta-blockers on operative mortality, examined using both direct risk adjustment and a matched-pairs analysis based on propensity for preoperative beta-blocker therapy.Results: From 1996 to 1999, overall use of preoperative beta-blockers increased from 50% to 60% in the NCD (P<.001 for time trend). Major predictors of use included recent myocardial infarction; hypertension; worse angina; younger age; better left ventricular systolic function; and absence of congestive heart failure, chronic lung disease, and diabetes. Patients who received beta-blockers had lower mortality than those who did not (unadjusted 30-day mortality, 2.8% vs 3.4%; odds ratio [OR], 0.80; 95% confidence interval [CI], 0.78-0.82). Preoperative beta-blocker use remained associated with slightly lower mortality after adjusting for patient risk and center effects using both risk adjustment (OR, 0.94; 95% CI, 0.91-0.97) and treatment propensity matching (OR, 0.97; 95% CI, 0.93-1.00). Procedural complications also tended to be lower among treated patients. This treatment advantage was seen among the majority of patient subgroups, including women; elderly persons; and those with chronic lung disease, diabetes, or moderately depressed ventricular function. Among patients with a left ventricular ejection fraction of less than 30%, however, preoperative beta-blocker therapy was associated with a trend toward a higher mortality rate (OR, 1.13; 95% CI, 0.96-1.33; P =.23).Conclusions: In this large North American observational analysis, preoperative beta-blocker therapy was associated with a small but consistent survival benefit for patients undergoing CABG, except among patients with a left ventricular ejection fraction of less than 30%. This analysis further suggests that preoperative beta-blocker therapy may be a useful process measure for CABG quality improvement assessment. [ABSTRACT FROM AUTHOR]- Published
